Abstract

Introduction. Metal-on-metal (MoM) hip resurfacings have been associated with a variety of complications resulting from adverse reaction to metal debris. Pseudotumors have rarely been reported to cause deep venous thrombosis (DVT). Study Design. A case report and a review of the literature. Case Presentation. A 75-year-old female who had left metal-on-metal hip resurfacing 6 years ago presented with left groin pain associated with unilateral lower limb edema and swelling. By duplex and MRI studies, our patient had an extensive soft tissue necrosis associated with a large pelvic mass causing extensive DVT of the lower limb secondary to mechanical compression of the left iliac vein. Results. Our case was initially treated for DVT followed by dual surgical approach. The pseudotumor was excised through a separate iliofemoral approach and revision of the hip implant was undertaken through a posterior approach in the same setting. An inferior vena cava (IVC) filter was inserted to minimise the perioperative risks of handling the iliac veins. Conclusion. A combined approach with vascular surgeons is required. Combined resection of the pseudotumor and revision of the metal bearing surfaces is essential, in order to achieve a good surgical outcome in this rare complication.

Abstract

International audience; Object Grammars define mappings between text and object graphs. Parsing recognizes syntactic features and creates the corresponding object structure. In the reverse direction, formatting recognizes object graph features and generates an appropriate textual presentation. The key to Object Grammars is the expressive power of the mapping, which decouples the syntactic structure from the graph structure. To handle graphs, Object Grammars support declarative annotations for resolving textual names that refer to arbitrary objects in the graph structure. Predicates on the semantic structure provide additional control over the mapping. Furthermore, Object Grammars are compositional so that languages may be defined in a modular fashion. We have implemented our approach to Object Grammars as one of the foundations of the Ensō system and illustrate the utility of our approach by showing how it enables definition and composition of domain-specific languages (DSLs).

Abstract

Program differencing tools such as GNU diff identify individual differences but do not determine how those differences are related to each other. For example, an extract superclass refactoring on several subclasses will be represented by diff as a scattered collection of line additions and deletions which must be manually pieced together. In our previous work, we developed LSdiff, a novel program differencing technique that automatically identifies systematic structural differences as logic rules. This paper presents an LSdiff Eclipse plug-in that provides a summary of systematic structural differences along with textual differences within an Eclipse integrated development environment. This plugin provides several additional features to allow developers to interpret LSdiff rules easily, to select the abstraction level of program differencing analysis, and to reduce its running time through incremental program analysis. Abstract

Purpose: Formocresol and ferric sulfate were evaluated as pulpotomy medicaments using evidence-based dentistry principles. Pormocresol has been challenged as a potential carcinogen and mutagen, leading to consideration of ferric sulfate. Methods: The PICOT statement was: (P) In human carious primary molars with reversible coronal pulpitis, (I) does a pulpotomy performed with ferric sulfate, (C) compared with formocresol, (O) result in clinical/radiographic success, (T) in time periods up to exfoliation? Relevant papers (N=894) were identified from databases and inclusion criteria were applied; 94 papers remained (randomized clinical trials [RCTs]=7; clinical trials [CTs]=28; case-control studies=14; opinions, cohort, and cross-sectional studies=4; reviews=22; irretrievable papers=19). Three RCTs and 10 CTs (total teeth: formocresol=753; ferric sulfate=90) were meta-analyzed; 1 RCT and 1 CT were tested for homogeneity (odds ratios; 95% confidence intervals); 3 RCTs and 10 CTs were examined by student's nest. Results: Clinical data indicated ferric sulfate was significantly more successful than formocresol (OR=1.95; CI=1.01-3.80). Radiographic data indicated no difference between medicaments (OR=0.90; CI=0.58-1.39). Medicaments did not differ with t-tests of clinical (P>.10) and radiographic (P>.50) data. Conclusions: This evidence-based assessment concluded that, in human carious primary molars with reversible coronal pulpitis, pulpotomies performed with either formocresol or ferric sulfate are likely to have similar clinical/radiographic success.
