3. A randomised controlled feasibility trial to investigate the effects of a functional standing frame programme versus usual physiotherapy to improve function and quality of life and reduce neuromuscular impairment in people with severe sub-acute stroke

Abstract

Background Task-related training can aid functional recovery post-stroke but has not been investigated in people with severe stroke. Orthostatic hypotension (OH) may limit rehabilitation, therefore, the effects of undertaking prolonged standing and sit to stand repetitions (functional standing frame programme) early after severe stroke during inpatient sub-acute rehabilitation is unknown. Methods A systematic review of non-pharmacological interventions to treat OH in people with neurological conditions was undertaken to inform a protocol for the management of OH during the functional standing frame programme. The feasibility of a blinded randomised controlled trial (RCT) investigating the effects of a functional standing frame programme compared to usual physiotherapy for people with severe stroke was conducted. Primary (Barthel Index, Edmans ADL Index for Stroke) and secondary outcomes (including lower limb joint range of movement, knee extensor strength, and quality of life) were assessed at baseline, post-intervention and 15-, 29- and 55-weeks post-randomisation. Semi-structured interviews were conducted with a subset of participants, relatives and physiotherapists to explore experiences of the intervention and trial procedures. Data were analysed using thematic analysis and descriptive analysis. Results The systematic review included randomised controlled trials (n=13), quasiexperimental (n=27), case control (n=1) and case report (n=2). A meta-analysis of seven studies concluded electrical stimulation, lower limb compression and resistance exercise training were favourable in treating or reducing OH. Forty-five participants (51-96 years; 42% male, mRS 4=80% 5=20%) were recruited; n=22 randomised to intervention, n=23 to control. Twenty-seven participants completed the trial: n=12 died (n=7 intervention), n=2 moved out of area, n=4 withdrawn. Adherence to the intervention was low: 38-51% of possible sessions being completed; average session duration 39.40 minutes (±18.8); standing duration 12.52 minutes (±8.8); and mean sit-to-stand repetitions 4.64 (±3.9 SD) per session. 91% of sessions were enjoyed. Adherence was affected by patient, physiotherapist and organisational factors. Conclusion A definitive RCT of a functional standing frame programme is feasible for people with severe stroke. However, intervention adherence need to be addressed before progressing to a definitive trial, which will investigate clinical and cost effectiveness.

Abstract

Purpose Nirmatrelvir/ritonavir is one of few options for outpatient treatment of COVID-19, but its use has been limited in transplant recipients due to significant drug interactions with immunosuppressants. Tacrolimus toxicity is possible when the drug is coadministered with nirmatrelvir/ritonavir and may require urgent reduction of tacrolimus levels. This case series describes the use of phenytoin for enzyme induction in 5 adult solid organ transplant recipients with supratherapeutic tacrolimus levels resulting from coadministration with nirmatrelvir/ritonavir. Summary Solid organ transplant recipients are at high risk for complications related to COVID-19. Outpatient treatment options are limited, and therapeutic drug monitoring is complex in patients requiring quarantine. The 5 solid organ transplant recipients described herein were initiated on nirmatrelvir/ritonavir in the outpatient setting and subsequently presented with supratherapeutic tacrolimus concentrations greater than 59 ng/mL and developed signs and symptoms of tacrolimus toxicity. In all patients, nirmatrelvir/ritonavir and tacrolimus were discontinued, and oral phenytoin (200-400 mg/day) was given for 2 to 4 days. Tacrolimus was resumed once tacrolimus levels decreased to appropriate levels. Conclusion These observations demonstrate that metabolism induction using phenytoin may be a useful strategy in the setting of supratherapeutic tacrolimus levels resulting from concomitant administration with nirmatrelvir/ritonavir. Abstract

Telerehabilitation has increasingly been used since the COVID-19 pandemic but with limited guidance available on undertaking physical assessments using remote methods. We aimed to provide such guidance by developing a Telerehab Toolkit, an online information and training resource for practitioners, patients, and carers on telerehabilitation for people with physical disabilities and movement impairment. Development and evaluation of the toolkit were informed by the Knowledge to Action framework and took place iteratively in two phases—knowledge creation and action. Information was collated from various sources including literature review, online survey, service evaluation, and focus group discussions. The toolkit has been evaluated using think-aloud interviews, e-mail and social media feedback from users, and analytics data on user engagement with the website. The Telerehab Toolkit focuses on remote physical assessments, and contains information on technology, digital skills, remote assessment tools, information governance, and safety for telerehabilitation. Resources include top tips from practitioners and patients, how-to guides, checklists, videos, and links to evidence. The Telerehab Toolkit has been well-received by practitioners, healthcare students, patients, and carers, is being disseminated widely, and is freely available (). Telerehabilitation has been increasingly used since the COVID-19 pandemic, but with limited guidance and training for practitioners on undertaking safe and effective remote physical assessments. The Telerehab Toolkit has been developed iteratively using the Knowledge to Action framework; it is a free online resource for practitioners and patients with specific guidance on telerehabilitation for physical disabilities and movement impairment. Abstract

Human mitochondrial DNA (mtDNA) shows extensive within-population sequence variability (1). Many studies suggest that mtDNA variants may be associated with ageing or diseases (2-4), although mechanistic evidence at the molecular level is lacking (5,6). Mitochondrial replacement has the potential to prevent transmission of disease-causing oocyte mtDNA. However, extension of this technology requires a comprehensive understanding of the physiological relevance of mtDNA sequence variability and its match with the nuclear-encoded mitochondrial genes. Studies in conplastic animals (7-9) allow comparison of individuals with the same nuclear genome but different mtDNA variants, and have provided both supporting and refuting evidence that mtDNA variation influences organismal physiology. However, most of these studies did not confirm the conplastic status, focused on younger animals, and did not investigate the full range of physiological and phenotypic variability likely to be influenced by mitochondria. Here we systematically characterized conplastic mice throughout their lifespan using transcriptomic, proteomic, metabolomic, biochemical, physiological and phenotyping studies. This project is a scoping review with two main aims. First to examine the literature regarding the effectiveness of different dual-task training types and task combinations. Second to examine what outcome measurements for improving bipedal upright balance in people with mild to moderate PD are in use. The objectives are to explore the primary task types involving bipedal balance-related exercises and secondary task types involving motor or cognitive tasks and their combinations in dual-task training; to explore the balance-related outcome measurements embedded within the clinical effectiveness studies published in the literature; to determine existence of evidence investigating the superiority of one dual-task combination over another dual-task combination in the training for improving balance in people with mild to moderate PD; to summarise the differences in relevant balance outcomes in trials investigating the superiority of the task combinations within dual-task training interventions; to identify and summarise the experiences and views of people with PD regarding dual-task interventions within the qualitative studies.
