20. Protecting the poor? Impact of the national health equity fund on utilization of government health services in Cambodia, 2006-2013

Abstract

INTRODUCTION: Cambodia's health equity fund (HEF) is the country's most significant social security scheme, covering the poorest one-fifth of the national population. During the last two decades, the HEF system was scaled up from an initial two health districts to national coverage of public health facilities. This is the first national study to examine the impact of the HEF on the utilisation of public health facilities. METHODS: We first investigated the level of national HEF population coverage and health service use made by HEF eligible members using an administrative HEF operational dataset. Second, through multilevel interrupted time series analysis of routine monthly utilisation statistics during 2006-2013, we evaluated the impact of the HEF on hospital and health centre utilisation. RESULTS: The proportion of HEF beneficiaries using hospital services in a given year (4.6%) appeared to exceed rates in the general population (3.3%). The introduction of the HEF was associated with: a significant level change in the monthly number of consultations at HCs followed by a gradual slope increase in time trend and a significant level change in the monthly number of deliveries. Overall, this was equivalent to a 15.6% net increase in number of consultations and 5.3% in deliveries in the first year. At RHs: a significant level change in the number of RH inpatient cases, followed by a sustained slope increase; a significant slope increase in the number of outpatient consultations and in the overall number of newborn deliveries. Overall, this was equivalent to a 47.9% net increase in inpatient cases, 24.1% in outpatient cases and 31.4% in deliveries in the first year. CONCLUSION: The implementation of the HEF scheme was associated with increased utilisation of primary and secondary care services by the poor.

26. Ka-band monopulse antenna-pointing systems analysis and simulation

NASA 's Deep Space Network (DSN) has been using both 70-m and 34-m reflector antennas to communicate with spacecraft at S-band (2.3 GHz) and X-band (8.45 GHz). To improve the quality of telecommunication and to meet future mission requirements, JPL has been developing 34-m Ka-band (32-GHz) beam waveguide antennas. Presently, antenna pointing operates in either the open-loop mode with blind pointing using navigation predicts or the closed-loop mode with conical scan (conscan). Pointing accuracy under normal conscan operating conditions is in the neighborhood of 5 mdeg. This is acceptable at S- and X-bands, but not enough at Ka-band. Due to the narrow beamwidth at Ka-band, it is important to improve pointing accuracy significantly (approximately 2 mdeg). Monopulse antenna tracking is one scheme being developed to meet the stringent pointing-accuracy requirement at Ka-band. Other advantages of monopulse tracking include low sensitivity to signal amplitude fluctuations as well as single-pulse processing for acquisition and tracking. This article presents system modeling, signal processing, simulation, and implementation of Ka-band monopulse tracking feed for antennas in NASA/DSN ground stations.

Abstract

This paper investigates the physics behind the overall on-current (Ion) improvement of n-channel metal-oxide-semiconductor (NMOS) transistors by uniaxial tensile stress. The strain-induced change in subthreshold off-current (Ioff) is related to the following strain-induced effects: (i) mobility enhancement, (ii) reduction in the saturation threshold voltage (Vth,sat), and (iii) improvement in subthreshold swing (Sts). By selecting transistors whose Ioff is less sensitive to the statistical variation in gate length, we studied the effects of process-induced tensile stress on Ion and Ioff of NMOS transistors. We found that both externally applied tensile stress and process-induced tensile stress led to a bigger percentage increase in subthreshold Ioff compared to the percentage increase in Ion. Our explanation is that the increase in Ioff is mainly due to an increase in mobility and a decrease in Vth,sat by tensile stress. The improvement of subthreshold swing by tensile stress can lead to a decrease in subthreshold Ioff; for the time being, this seems to be a relatively minor effect. Since the subthreshold Ioff is more sensitive to change in Vth,sat than Ion, the increase in subthreshold Ioff can be removed by a slight adjustment in Vth,sat without too much effect on Ion such that tensile stress can bring about an overall Ion improvement in NMOS transistors despite increase in both Ion and Ioff by tensile stress. Abstract

The cellular manufacturing system (CMS) is a well-known strategy which enhances production efficiency while simultaneously cutting down the system-wide operation cost. Most of the researchers have been focused on developing different approaches in order to identify machine-cells and part-families more efficiently. In recent years, researchers have also focused their studies more scrupulously by collectively considering CMS with production volume, operation sequence, alternative routing or even more. However, very few of them have tried to investigate both the allocation sequence of machines within the cells (intra-cell layout) and the sequence of the formed cells (inter-cell layout). Solving this problem is indeed very important in reducing the total intracellular and intercellular part movements which is especially significant with large production volume. In this paper, a two-phase approach has been proposed to tackle the cell formation problem (CFP) with consideration of both intra-cell and inter-cell part movements. In the first phase, a mathematical model with multi-objective function is formed to obtain the machine cells and part families. Afterwards, in the second phase, another mathematical model with single-objective function is presented which optimizes the total intra-cell and inter-cell part movements. In other words, the scope of problem has been identified as a CFP together with the background objective of intra-cell and inter-cell layout problems (IAECLP). The primary assumption for IAECLP is that only linear layouts will be considered for both intra-cell and inter-cell. In other words, the machine within cells and the formed cells are arranged linearly. This paper studies formation of two mathematical models and used the part-machine incidence matrix with component operational sequence. The IAECLP is considered as a quadratic assignment problem (QAP). Since QAP and CFP are NP-hard, genetic algorithm (GA) has been employed as solving algorithm. GA is a widespread accepted heuristic search technique that has proven superior performances in complex optimization problems and further it is a popular and well-known methodology. Abstract

Because of unfavourable economic conditions, organizations worldwide have to constantly reconstruct their business strategies in order to survive. Recent studies have stressed the importance of supply quality management (SQM) for the general competitiveness of manufacturers. However, very little empirical research has been carried out that specifically examines how SQM practices lead to improvements in quality across an organization. To scrutinise the impact of SQM practices, a survey was conducted of 138 leading manufacturing companies in the Pearl River Delta (PRD), a manufacturing hub in southern China dubbed 'the factory floor of the world'. By using path analytic techniques, a model was constructed to portray the relationships among SQM practices and organizational quality performance. This model could act as a tool of reference for companies seeking to improve their SQM systems, and help them in their efforts to raise quality across the company. This paper demonstrates that quality-conscious management practices speed up the implementation of SQM practices. Abstract

Mutation, irregular expression and sustained activation of the Transient Receptor Potential Channel, type Melastatin 4 (TRPM4), have been linked to various cardiovascular diseases. However, much remains unknown about the structure of this important ion channel. Here, we have purified a heterologously expressed TRPM4-eGFP fusion protein and investigated the oligomeric state of TRPM4-eGFP in detergent micelles using crosslinking, native gel electrophoresis, multi-angle laser light scattering and electron microscopy. Our data indicate that TRPM4 is tetrameric, like other TRP channels studied to date. Furthermore, the functionality of liposome reconstituted TRPM4-eGFP was examined using electrophysiology. Single-channel recordings from TRPM4-eGFP proteoliposomes showed inhibition of the channel using Flufenamic acid, a well-established inhibitor of TRPM4, suggesting that the channels are functional upon reconstitution. Our characterisation of the oligomeric structure of TRPM4 and the ability to reconstitute functional channels in liposomes should facilitate future studies into the structure, function and pharmacology of this therapeutically relevant channel.

INTRODUCTION: Periostin (Postn) is a secreted cell adhesion protein that activates signaling pathways to promote cancer cell survival, angiogenesis, invasion, and metastasis. Interestingly, Postn is frequently overexpressed in numerous human cancers, including breast, lung, colon, pancreatic, and ovarian cancer.METHODS: Using transgenic mice expressing the Neu oncogene in the mammary epithelium crossed into Postn-deficient animals, we have assessed the effect of Postn gene deletion on Neu-driven mammary tumorigenesis.RESULTS: Although Postn is exclusively expressed in the stromal fibroblasts of the mammary gland, Postn deletion does not affect mammary gland outgrowth during development or pregnancy. Furthermore, we find that loss of Postn in the mammary epithelium does not alter breast tumor initiation or growth in mouse mammary tumor virus (MMTV)-Neu expressing mice but results in an apocrine-like tumor phenotype. Surprisingly, we find that tumors derived from Postn-null animals express low levels of Notch protein and Hey1 mRNA but increased expression of androgen receptor (AR) and AR target genes. We show that tumor cells derived from wild-type animals do not proliferate when transplanted in a Postn-null environment but that this growth defect is rescued by the overexpression of active Notch or the AR target gene prolactin-induced protein (PIP/GCDFP-15).CONCLUSIONS: Together our data suggest that loss of Postn in an ErbB2/Neu/HER2 overexpression model results in apocrine-like tumors that activate an AR-dependent pathway. This may have important implications for the treatment of breast cancers involving the therapeutic targeting of periostin or Notch signaling.

Abstract

Lighting experiments were performed in a real context populated with dummies and objects. Using the King’s College Chapel in Cambridge as a case study, two field surveys of concert lighting were performed, one with the chapel empty and one with it occupied. In each survey, photometric data were collected under three electric lighting conditions and from six different viewing positions. A comparative analysis indicates that the data gathered from the occupied space represent the luminances more accurately, present a more detailed description of the light distribution, and provide a more extensive set of variables characterising the geometrical details of the visual scene. This study demonstrates the importance of using occupied spaces and considering the presence of occupants in field studies, which could be useful for obtaining a more complete understanding of complex luminous environments. 37. Productivity differences, technology adoption and economic growth: the case of India

Abstract

When the acronym of ëBRICí was coined in 2001 by Jim OíNeill of Goldman Sachs, it was expected that economic growth rates in India, Brazil and Russia would eventually catch up with that of China. However, China has continued to outperform the other economies in the group, even after it was renamed ëBRICSí to reflect the inclusion of South Africa in 2010. The focus of this chapter is on one of the BRICS economies, namely India. Its aim is to examine from an economic perspective, why Indiaís performance has not lived up to expectations, and comment on the key challenges it faces in meeting them. We begin with some descriptive statistics regarding the progress of the Indian economy since 1990. While it has been growing at a rapid rate since the reforms it introduced in the1990s, there has been a slowdown in its overall GDP growth rates since 2008. The rate of growth experienced in the period 2003ñ07 was an average of 10.5 per cent. However, since the recession following the Global Financial Crisis (GFC) of 2008, the growth rate has fallen. From the period 2008ñ12 it has only registered an average growth rate of 6.5 per cent (World Bank, 2013). This chapter suggests that one of the major factors underpinning this slowdown is the performance of Indiaís agricultural sector. The importance of the agricultural sector is highlighted by the following stylized facts.

Abstract

Das Förderprogramm INNO-WATT des Bundeswirtschaftsministeriums unterstützt KMU sowie externe Industrieforschungseinrichtungen in benachteiligten Regionen der neuen Bundesländer bei der Entwicklung neuer Erzeugnisse und Verfahren. Die Leistungsfähigkeit der Zuwendungsempfänger soll dadurch gestärkt und die Zahl der Arbeitsplätze ausgeweitet werden. Das Fraunhofer ISI zeigt mit seiner Programmevaluierung, dass die Förderung wesentlich zur positiven wirtschaftlichen Entwicklung der Zuwendungsempfänger beiträgt. Die Analyse von Einflussfaktoren des Erfolgs der geförderten FuE-Projekte belegt zudem, dass die konsequente Ausrichtung der Förderung auf Wachstumsträger die Effizienz der Förderung erhöht.

Abstract

Das Förderprogramm PRO INNO des Bundeswirtschaftsministeriums zielte auf eine Verbesserung der Innovationskraft kleiner und mittlerer Unternehmen (KMU) zur Sicherung ihrer Wettbewerbsfähigkeit auf internationalen Märkten, um damit einen Beitrag zu mehr Wachstum und neuen Beschäftigungsmöglichkeiten zu leisten. Gespräche des Fraunhofer ISI mit 60 Experten aus Wirtschaft und Wissenschaft sowie die Auswertung der Förderdaten zeigen, dass diese Ziele weitgehend erreicht wurden. Ein Indiz ist die große Anzahl geförderter Unternehmen und Forschungseinrichtungen. Die Programmevaluation unterstreicht den breiten, technologieunspezifischen Förderansatz von PRO INNO sowie die Passfähigkeit des Förderinstrumentariums für die Zielgruppe. Die Förderung von FuE-Kooperationen hat zu einer stärkeren Vernetzung der KMU untereinander und mit Forschungseinrichtungen im Bereich von FuE beigetragen und ist damit ein geeignetes Instrument zur Verbesserung der Innovationskraft von KMU. Die Ausgestaltung des Förderinstrumentariums stärkt die internen Kompetenzen der geförderten Unternehmen und erweitert ihre quantitativen wie qualitativen FuE-Kapazitäten für die Durchführung anspruchsvoller Innovationsvorhaben.

Abstract

Trotz aller Erfolge stellt sich den neuen Bundesländern nach wie vor eine ganze Reihe von Schwierigkeiten in ihrer wirtschaftlichen Entwicklung (v.a. geringer Industriebesatz, hohe Abwanderung). Inzwischen setzt die Bundesregierung verstärkt auf die Innovationsförderung, um der wirtschaftlichen Situation Ostdeutschlands eine nachhaltige Wendung zu geben. Ungeachtet einzelner Erfolge konnten mit den eingesetzten Fördermaßnahmen die Wachstumsraten bisher allerdings nicht auf ein zufrieden stellendes Maß angehoben werden. Das Buch zeigt den derzeitigen Stand der Innovationsaktivitäten und das Spektrum der bisherigen Innovationspolitik in Ostdeutschland auf. Auf der Basis dieser Untersuchung und regionaler Fallbeispiele werden Leitlinien für eine Weiterentwicklung der Innovationsförderung in den neuen Bundesländern entwickelt.
