Frente a la pandemia el gobierno argentino estableció el aislamiento social, preventivo y obligatorio que imponía la restricción de movimientos de toda la población, el cual se prolongó durante ocho meses. Esta medida provocó un cambio súbito y drástico en la vida social e individual con impacto directo en el bienestar emocional de las personas. El propósito en este artículo es explorar la experiencia subjetiva de 319 personas de 65 años y más residentes en la ciudad de Buenos Aires, focalizando en sus sentimientos y emociones y en su evaluación de las ganancias experimentadas durante el aislamiento. Utilizando información obtenida mediante un diseño longitudinal de panel fijo, examinamos las variabilidades temporales de las respuestas. La información se recogió en tres ondas, mediante un cuestionario online entre los meses de junio y agosto de 2020, período en el cual la curva de contagios en Buenos Ares alcanzó su mayor expresión2. Para el análisis de la información se aplica un enfoque de análisis de contenido poniendo atención en los elementos explícitos presentes en las respuestas. Se desarrolló una tipología básica de trayectorias de las emociones que informan tanto características estables como oscilantes que dan cuenta de valencias emocionales de tipo positivo o negativo. La mayor parte de los casos siguen trayectorias oscilantes que muestran un desplazamiento hacia los valores negativos. Se observa que las mujeres experimentan malestar emocional en mayor medida que los hombres.

Liver cancer is the second leading cause of cancer-related deaths globally and has an incidence of approximately 850,000 new cases per year. Hepatocellular carcinoma (HCC) represents approximately 90% of all cases of primary liver cancer. The main risk factors for developing HCC are well known and include hepatitis B and C virus infection, alcohol intake and ingestion of the fungal metabolite aflatoxin B1. Additional risk factors such as non-alcoholic steatohepatitis are also emerging. Advances in the understanding of the molecular pathogenesis of HCC have led to identification of critical driver mutations; however, the most prevalent of these are not yet druggable targets. The molecular classification of HCC is not established, and the Barcelona Clinic Liver Cancer staging classification is the main clinical algorithm for the stratification of patients according to prognosis and treatment allocation. Surveillance programmes enable the detection of early-stage tumours that are amenable to curative therapies - resection, liver transplantation or local ablation. At more developed stages, only chemoembolization (for intermediate HCC) and sorafenib (for advanced HCC) have shown survival benefits. There are major unmet needs in HCC management that might be addressed through the discovery of new therapies and their combinations for use in the adjuvant setting and for intermediate- and advanced-stage disease. Moreover, biomarkers for therapy stratification, patient-tailored strategies targeting driver mutations and/or activating signalling cascades, and validated measurements of quality of life are needed. Recent failures in the testing of systemic drugs for intermediate and advanced stages have indicated a need to refine trial designs and to define novel approaches.

BACKGROUND & AIMS: A subset of patients with hepatocellular carcinoma (HCC) beyond Milan criteria might obtain acceptable survival outcomes after liver transplantation. Living donor liver transplantation (LDLT) has emerged as a feasible alternative to overcome the paucity of donors. METHODS: In 2001 we started a protocol for LDLT in Child A-B patients with HCC fulfilling a set of criteria - the BCLC expanded criteria- that expanded the conventional indications of transplantation: 1 tumor ≤ 7cm, 5 tumors ≤ 3cm, 3 tumors ≤ 5cm without macrovascular invasion or down-staging to Milan after loco-regional therapies. RESULTS: We present a prospective cohort of 22 patients with BCLC extended indications based on size/number (17) or down-staging (5) treated with LDLT between 2001 and 2014. [eng] 1. HYPOTHESES Primary liver cancer is the sixth most commonly diagnosed cancer and the second leading cause of cancer-related death worldwide2,3. Around 90% of cases correspond to hepatocellular carcinoma (HCC)2,3, which arises almost unfailingly in the setting of chronic liver diseases. The worldwide incidence of HCC cases is heterogeneous, reflecting the distribution of risk factors, with Mongolia exhibiting the highest HCC burden worldwide2. Well-established HCC risk factors are hepatitis B virus, hepatitis C virus, and non-alcoholic fatty liver disease (NAFLD). Other agents have also been reported to promote hepatocarcinogenesis, including adeno-associated virus (AAV) integration3,4, but more studies are needed to determine in which conditions this virus can induce HCC. Approximately, 50–60% of HCC patients are exposed to systemic therapies in their lifespan, particularly in advanced stages of the disease3. These mostly include multikinase inhibitors such as lenvatinib and immune checkpoint inhibitors (ICI) such as anti-PD1 antibodies. Although ICIs are revolutionizing HCC treatment, monotherapy approaches elicit responses in only 15% of patients, while the majority are primarily resistant5,6. Thus, identifying existing multikinase inhibitors that can effectively synergize with ICI is urgently needed. During the last decade, molecular profiling of tumors using translational approaches has significantly contributed to the understanding of the molecular pathogenesis of HCC3,83. This knowledge has provided important opportunities in clinical oncology by 1) identifying novel genetic and environmental features associated with hepatocarcinogenesis in specific patient populations; and 2) unraveling new treatment strategies. Considering all this, the hypotheses of this thesis are that: 1. Performing a comprehensive analysis of the molecular and immunological features of HCC will provide relevant information about the genetic and molecular determinants associated with HCC in specific populations and will unveil novel potential therapeutic approaches. This could result in fundamental implications for the clinical decision- making in HCC. 2. Assessing the distinct genomic and transcriptomic alterations of Mongolian HCC could provide relevant information that may help unveil the genetic factors and environmental exposures underlying the high incidence in this population. 67 HYPOTHESES AND AIMS 3. Hepatocyte replication in response to liver injury and NAFLD could promote oncogenic AAV integration and HCC development, which could be a concern for the use of AAV gene therapy in patients with chronic liver. 4. The multikinase inhibitor lenvatinib has immune-modulating potential and its combination with anti-PD1 checkpoint inhibitors could improve its anti-tumoral effect in HCC. 2. AIMS. Considering the background and hypotheses exposed above, the specific aims of this doctoral thesis were the following: 1. To provide a molecular characterization of Mongolian HCC and identify its unique genomic features compared to Western HCC. 2. To assess whether NAFLD-associated liver damage increase the risk of AAV integration inducing HCC. 3. To identify the immunomodulatory effects of lenvatinib in combination with anti-PD1 and provide a mechanistic rationale for this treatment in advanced HCC., [cat] INTRODUCCIÓ: El carcinoma hepatocel·lular (CHC) representa un important problema de salut pública degut a la seva elevada incidència i mortalitat1,2. La incidència mundial de CHC és heterogènia, sent Mongòlia el país amb més casos per habitant, gairebé 10 vegades per sobre de la mitjana global2. Els principals factors de risc del CHC són la infecció pel virus de l'hepatitis B o pel virus de l'hepatitis C i la malaltia del fetge gras no alcohòlica (NAFLD, de les seves sigles en anglès). També s’han proposat altres agents que podrien estar associats a hepatocarcinogènesi, inclosa la infecció per virus adenoassociat (VAA)3,4, però és necessària més informació per determinar en quines condicions aquest virus pot promoure el CHC. Al voltant del 40% dels pacients amb CHC són diagnosticats en etapes avançades de la malaltia, en les quals són elegibles per a teràpies sistèmiques incloent inhibidors multiquinases (p. ex., lenvatinib) i inhibidors de punts de control immunitaris (ICI; p. ex., anticossos anti-PD1). Tot i que els ICI estan revolucionant el tractament del CHC, només aconsegueixen respostes en el ~15% dels pacients en monoteràpia5,6. Per contra, noves teràpies combinades estan incrementant la taxa de resposta fins al ~30%. Considerant això, és necessari identificar noves teràpies que puguin fer sinergia amb ICI per tal d’augmentar la supervivència dels pacients. Durant els últims anys, la caracterització molecular i immunològica de tumors mitjançant estudis translacionals ha proporcionat una millor comprensió de la patogènesi molecular del CHC5,6. Aquest coneixement té una gran rellevància clínica, ja que, entre altres coses, permet 1) identificar determinants genètics i moleculars que afavoreixen l'hepatocarcinogènesi en poblacions específiques de pacients; i 2) dissenyar nous tractaments incloent teràpies combinades. HIPÒTESIS: Les hipòtesis d’aquesta tesi són les següents: - L’anàlisi exhaustiva de les característiques moleculars i immunològiques del CHC proporcionarà nova informació sobre els determinants genètics i moleculars associats al CHC en poblacions d’alt risc, afavorint l’estudi de noves estratègies terapèutiques. Aquestes dades podrien tenir implicacions fonamentals per a la presa de decisions clíniques en CHC. - L'avaluació de les alteracions genòmiques i transcripcionals del CHC a Mongòlia pot proporcionar nova informació que ajudi a identificar factors genètics i ambientals propis associats a l’elevada incidència en aquesta població. - La malaltia hepàtica crònica i NAFLD promouen la integració oncogènica del VAA i el desenvolupament de CHC, la qual cosa podria ser un condicionant per a l'ús de la teràpia gènica mitjançant VAA en pacients. - L'inhibidor multiquinasa lenvatinib té potencial immunomodulador i la seva combinació amb ICI anti-PD1 podria incrementar l’efecte anti-tumoral en models experimentals i en pacients amb CHC. OBJECTIUS: Els objectius específics d'aquesta tesi doctoral són: - Realitzar una caracterització molecular del CHC a Mongòlia i identificar trets moleculars específics en comparació amb tumors de pacients occidentals. - Analitzar si el NAFLD incrementa el risc d’integració del VAA en hepatòcits, induint així el CHC. - Investigar l'efecte immunomodulador de lenvatinib en combinació amb l'anticòs anti-PD1 per proporcionar una base racional pel seu ús com a tractament pel CHC avançat. MÈTODES: - Estudi #1 (Torrens et al., en revisió): Un total de 192 mostres de tumors hepàtics CHC de pacients provinents de Mongòlia i 187 mostres de CHC de pacients occidentals (Europa i EEUU) van ser analitzades mitjançant seqüenciació completa d'exoma i d’ARN. Per identificar les característiques moleculars úniques del CHC de Mongòlia, es van comparar les característiques clinico-patològiques dels tumors de pacients mongols i occidentals, així com els seus perfils mutacionals i transcripcionals. - Estudi #2 (Dalwadi et al., Mol Ther 2021): Per tal de determinar si la infecció pel virus VAA dels hepatòcis promou l’hepatocarcinogènesi en condicions de malaltia hepàtica, es va fer servir un model animal murí on ratolins adults i nounats de la soca C57BL/6 van ser infectats amb un vector VAA dirigit contra el locus genètic Rian (VAA-Rian). A continuació, els animals van ser a) alimentats amb una dieta rica en greixos per generar NAFLD o b) sotmesos a hepatectomia parcial per generar dany hepàtic i regeneració. Es va monitoritzar el desenvolupament tumoral i els tumors generats es van caracteritzar mitjançant seqüenciació d'ARN. - Estudi #3 (Torrens et al., Hepatology 2021): Per avaluar l'activitat anti-tumoral de la combinació de l’inhibidor multiquinasa lenvatinib amb anti-PD1 es van generar tres models murins singènics de CHC. L’impacte dels tractaments sobre el perfil molecular i immunitari dels tumors van ser analitzats per citometria de flux i perfilats a nivell transcripcional i immunohistoquímic. Finalment, es va explorar el perfil d'expressió gènica de 228 tumors humans per identificar pacients amb CHC que es podrien beneficiar de la teràpia combinada de lenvatinib i anti-PD1. RESULTATS: - Estudi #1 (Torrens et al., en revisió): Els resultats del nostre estudi indiquen que els tumors CHC de pacients de Mongòlia presenten les següents particularitats: 1. Una elevada taxa de mutacions, que gairebé dobla la taxa de mutacions de la cohort occidental (121 davant de 70 mutacions per tumor). 2. Un perfil mutacional diferent, amb una major freqüència de mutacions de TP53, APOB, TSC2 i la família de gens KMT2. 3. La presència d'una nova firma mutacional (SBS Mongòlia) al 25% dels casos, que està associada a la firma del genotòxic dimetil sulfat derivat de la combustió del carbó. 4. A nivell d’expressió gènica, els tumors de la cohort de Mongòlia es classifiquen en tres classes moleculars (MGL1, MGL2 i MGL3), dues de les quals (MGL2 al 26% dels CHC i MGL3 al 30%) i presenten característiques moleculars i clínico-patològiques no observades en mostres de CHC occidental. - Estudi #2 (Dalwadi et al., Mol Ther 2021): 1. Només el 5% dels ratolins adults infectats amb VAA-Rian van desenvolupar CHC. Per contra, l’elevada proliferació cel·lular en nounats va promoure un increment en la integració viral i generació de tumors en el 100% dels animals. 2. Tant la lesió hepàtica per la dieta rica en greix com la derivada de l’hepatectomia parcial van conduir a un increment en l’aparició de CHC en ratolins adults infectats amb VAA-Rian en comparació amb animals infectats però no sotmesos a estrès hepàtic. 3. L'anàlisi transcriptòmica de tumors de CHC murins de ratolins originats degut a la infecció amb VAA-Rian va revelar similituds moleculars amb tumors de CHC humans amb sobreexpressió del locus Rian, associats amb proliferació, fenotip agressiu i mal pronòstic. 4. El fetge de ratolins alimentats amb una dieta rica en greixos presentaven un perfil immunològic protumorigènic, que podria ser el motiu de l’increment del risc de CHC induït per la infecció de VAA. - Estudi #3 (Torrens et al., Hepatology 2021): 1. La combinació de lenvatinib i anti-PD1 aconsegueix una major taxa de resposta, un temps de resposta més curt i una reducció de la viabilitat del tumor en comparació amb els tractaments administrats en monoteràpia. 2. Lenvatinib exerceix un potent efecte immunomodulador sobre l'infiltrat tumoral caracteritzat per una reducció en la proporció de cèl·lules T reguladores i inhibició de la senyalització immunosupressora mitjançant TGFß. 3. El tractament anti-PD1 modifica l'infiltrat immunològic del tumor augmentant les cèl·lules T i la subpoblació de cèl·lules dendrítiques de tipus 1 al tumor. 4. La combinació de lenvatinib amb anti-PD1 és l’únic dels tractaments avaluats capaç de generar una resposta immune anti-tumoral activada. 5. El 22% dels pacients amb CHC presenten un perfil transcripcional que podria estar associat a resistència primària a anti-PD1 i resposta al règim combinat. Aquests pacients podrien beneficiar-se de l'efecte de reforç del tractament de combinació. CONCLUSIONS: - El CHC a Mongòlia presenta característiques moleculars úniques comparat amb els tumors de pacients occidentals. El seu perfil mutacional podria ser degut a exposició a factors ambientals responsables de l'elevada incidència en aquest país. - La infecció amb el virus VAA promou el desenvolupament de CHC en models murins amb fetge gras degut a un augment en la proliferació d’hepatòcits i a la presència d'un infiltrat protumorogènic. Per tant, la teràpia gènica amb VAA podria resultar oncogènica en pacients amb inflamació hepàtica crònica. - La combinació de lenvatinib i anti-PD1 té un efecte immunomodulador al CHC caracteritzat per la generació d’un perfil immunològic activat. Aquest tractament podria maximitzar el benefici clínic en un subgrup de pacients de CHC. - Els estudis translacionals basats en models preclínics i anàlisis òmiques tenen la capacitat de revelar les característiques moleculars del CHC, potencials factors de risc i nous enfocaments terapèutics.

[eng] Insulin-like growth factor (IGF) signaling is frequently altered in HCC, including activation of IGF1R, IGF2 overexpression, IGFBP3 downregulation and IGF2R allelic losses1,2. A previous study from our group identified activation of IGF1R in 20% of human HCCs and demonstrated the efficacy of a fully human mAb against this receptor, IMC-A12 or cixutumumab, to delayed tumor growth in vivo1. This and other studies in different malignancies pointed to IGF1R as a promising therapeutic target. However, the molecular impact of IGF1R activation in HCC progression and the actual functional implications of its inhibition in human disease were not fully understood. This may explain the unsuccessful results obtained in subsequent clinical trials blocking this target in HCC, due to lack of efficacy or metabolic toxicity3. To overcome this issue, novel therapeutic strategies to neutralize IGF1 and 2 ligands have been developed. IGF2 is an imprinted gene highly overexpressed in HCC1,2, which can be partially explained by the reactivation of IGF2 transcription from the fetal-specific promoters (P2, P3, and P4)1,4. Therefore, IGF2 represents a potential driver in hepatocarcinogenesis but further studies are needed to address the precise mechanisms of deregulation in this neoplasm, the specific contribution of IGF2 overexpression to HCC development and the viability of this protein as a therapeutic molecular target. To this aim, we characterized a cohort of 228 resected human HCCs at the DNA, RNA, miRNA, protein and methylation level in order to perform an integrative genomic analysis with a focus on the IGF pathway. In order to assess the contribution of IGF2 to HCC development we established HCC cell lines with knock-down and ectopic overexpression and three different mouse models of HCC, including a chemically-induced model and two genetically-engineered mosaic mouse models (GEMM). The therapeutic potential of neutralizing IGF2 in HCC was evaluated in a xenograft model of HCC treated with Xentuzumab (BI 836845). This mAb has been designed to abrogate IGF1R and INSR-A activation by binding IGF1 and 2, without affecting insulin metabolic functions through INSR-B5. Our results showed that: 1. IGF2 overexpression >20-fold, as observed by qRT-PCR analysis, was observed in 15% of HCC patients. Immunohistochemical staining showed that it is mainly expressed by hepatocytes. This overlapped with increased mRNA expression of INSR-A isoform, a pro-proliferative receptor with high affinity for IGF2. 2. DNA methylation de-regulation, including hypomethylation of fetal promoters (P3 and P4), increased methylation of the adult promoter P1 and hypomethylation of the imprinting IGF2/H19 locus, is the predominant cause of IGF2 overexpression in human HCC samples. 4. A genetically engineered mosaic mouse model (GEMM) of HCC overexpressing IGF2 in the liver presented a significant reduction in survival compared to control mice due to accelerated tumor progression, but it did not promote tumor initiation. 5. As observed in in vitro studies using engineered HCC cell lines, IGF2-driven tumorigenesis in HCC relies on the increase in cell proliferation rather than antiapoptotic or enhanced migratory effects. 6. Genomic analysis revealed that IGF2-upregulated tumors display a more proliferative, undifferentiated and aggressive phenotype. 7. The in vitro results with xentuzumab suggest that IGF1/2-mAb-based treatments, contrary to IGF1R inhibitors, are able to block HCC cell proliferation specifically induced by IGF2-dependent activation of IGF1R and INSR-A, without affecting the metabolic pathway. 8. Xentuzumab efficiently inhibited tumor growth and significantly increased survival in an HCC xenograft model through antiproliferative and antiangiogenic effects. In conclusion, in this study we demonstrated the contribution of IGF2 to the development of hepatocarcinogenesis and defined it as an actionable epi-driver, whose blocking could benefit a subset of HCC patients. STUDY 2 Advances in the understanding of tumor immunity have allowed the development of immunotherapies that harness the body’s own immune system to attack tumors by targeting regulatory pathways in T cells1. In recent years, monoclonal antibodies directed against immune checkpoints such as CTLA-4 or the PD-1/ PD-L1 axis have shown remarkable efficacy in advanced stage malignancies such as melanoma or lung cancer (i.e., pembrolizumab and nivolumab)2. However not all treated patients benefit similarly3, hence the identification of a potential predictor of response to immunotherapy is an urgent medical need. In HCC, promising responses have been recently reported with nivolumab, a monoclonal antibody directed against PD-1, in a phase I/II trial6. Unfortunately, little is known about the immunological profile of HCC tumors and how to leverage this information to maximize response to immune-based therapies. In this study, we aimed to characterize the immune component of HCC tumors in order to provide knowledge that may improve the use of immunotherapies in this neoplasm. To this end, we have performed sophisticated molecular analysis of a total of 956 human HCC samples with a focus on the immune-related components. The main results of this study are: 1. Virtual microdissection of the molecular signals deriving from the distinct cell types present in HCC tumors through the non-negative matrix factorization (NMF) method was able to isolate the signal released from the inflammatory infiltrates. This has allowed us to define an immune-specific class in ~25% of HCC patients, that can be identified using a 112-genes Immune Classifier. 2. Our newly identified Immune subclass was characterized by significant enrichment of signatures identifying immune cells (i.e. T cells, TLS, cytotox, macrophages), IFN-α and γ signaling, signatures of response to immune checkpoint therapy, higher immune infiltration and PD-1 and PD-L1 expression. 3. Integration with previously reported molecular classification revealed an enrichment of the interferon-related and S1 subclasses and a significant exclusion of the CTNNB1 subclass. 4. Pathological examination revealed that patients belonging to the Immune class showed a high degree of immune infiltration, higher immunohistochemical expression of PD-1/PD-L1, and presence of TLS. 5. The immune class contains two distinct microenvironment-based components: a) Active Immune Response Subtype (~65%) characterized by overexpression of adaptive immune response genes and b) Exhausted Immune Response Subtype (~35%) characterized by the presence of immunosuppressive signals (i.e. TGF-ß, M2 macrophages). 6. Molecular characteristics of the Immune class and the presence of the two microenvironment-based subtypes were successfully recapitulated in 7 additional datasets (956 samples) tested regardless of the platform and type of samples used. 7. Immune class tumors show lower burden of chromosomal aberrations compared to the rest of tumors but no differences in the expression of neo-antigens or tumoral mutational burden. 8. Supervised analysis of whole genome methylation data revealed that the Immune class has a differential methylation profile mostly associated with immune related genes. 9. HCC samples within the CTNNB1 subclass, showed lower scores for immune signatures and over-expression of PTK2, suggesting that multiple oncogenic pathways could cooperate to modify the immune profile of the tumor. 10. The immune profile of the surrounding liver tumor does not reflect the intra-tumoral profile and is mostly characterized by immunosuppressive components associated with survival of HCC patients. In conclusion, our study represents a comprehensive characterization of the immunological profile of human HCC tumors and introduces a novel immune-specific class in ~25% of HCC cases. The identification of this class, which comprises two robust microenvironment-based clusters with either active or exhausted immune responses, might have implications in future immune therapeutic approaches in HCC., [spa] BASE RACIONAL Y OBJETIVOS DEL ESTUDIO: El carcinoma hepatocelular (CHC) es uno de los tumores más agresivos, cuya alta incidencia y mortalidad representan un importante problema de salud pública a nivel mundial1,2. Dos terceras partes de los pacientes de CHC son diagnosticados en etapas avanzadas de la enfermedad3, cuando las opciones terapéuticas se limitan a sorafenib (primera línea de tratamiento)4 y regorafenib (segunda línea de tratamiento)5. Sin embargo, aun contando con los beneficios que suponen estos fármacos, la esperanza de vida de los pacientes de CHC es menor a dos años3. Así pues, el desarrollo de nuevas estrategias terapéuticas para tratar casos avanzados de CHC representa una necesidad médica urgente6. Durante los últimos años, el análisis molecular de los tumores ha adquirido gran relevancia en el campo de la oncología clínica, permitiendo la utilización de biomarcadores con el objetivo de identificar el subgrupo de pacientes con mayor sensibilidad a un tratamiento específico7. Esta estrategia ha dado lugar al desarrollo de nuevas terapias antitumorales en varios tipos de neoplasmas. Desafortunadamente, no existen terapias dirigidas a bloquear ninguna de las alteraciones moleculares que se han identificado hasta la fecha en los tumores de CHC6,8. Por lo tanto, la extensión de nuestro conocimiento acerca de las alteraciones críticas que controlan la progresión tumoral del CHC es esencial para desarrollar nuevas terapias más eficaces y seguras que mejoren la supervivencia de los pacientes y permitan avanzar hacia un tipo de medicina de precisión. La presente tesis doctoral se focaliza en el estudio de la desregulación y potencial terapéutico de la vía de señalización de IGF en CHC y en la caracterización del perfil inmunológico de este tipo de tumores. El papel de la vía de señalización del IGF en CHC ha sido descrito en varios estudios independientes9,10. Además, varios fármacos dirigidos a bloquear el receptor de la vía (IGF1R) han sido evaluados en ensayos clínicos con pacientes de CHC, dando lugar a resultados insatisfactorios debidos a una falta de eficacia o a la toxicidad metabólica producida11,12. Así pues, el oncogen clave de esta vía de señalización sigue siendo desconocido. IGF2 es un gen imprintado que está altamente sobreexpresado en CHC9,10. En este sentido, no existe ningún estudio robusto que evalue su contribución a la hepatocarcinogenesis o su potencial como diana terapéutica9,10. Por otra parte, recientemente se han observado prometedoras respuestas con agentes immunoterapéuticos como el nivolumab en pacientes de CHC13. Sin embargo, este tipo de terapias parecen solo tener impacto en una baja proporción de pacientes. Así pues, es necesario entender la compleja relación existente entre las células immunitarias y las células tumorales en CHC para maximizar la respuesta a este tipo de terapias. Los objetivos de esta tesis son: 1) Evaluar el potencial oncogénico y los mecanismos de desregulación del IGF2 y determinar la eficacia anti-tumoral en CHC de terapias que bloquean este ligando. (Studio 1, Martínez-Quetglas I. et al, Gastroenterology 2016) 2) Descifrar el microambiente immunológico de los tumores de CHC mediante un análisis integrativo multi-ómico para mejorar el beneficio clínico de las inmunoterapias (Studio 2, Sia D. et al, Gastroenterology 2017) MÉTODOS Y RESULTADOS: ESTUDIO 1: IGF2 como oncogen epigenético y tratable en el CHC Nuestros resultados mostraron que: 1. La caracterización de una cohorte de 228 muestras humanas de CHC a nivel de DNA, RNA, miRNA, proteína y metilación demostró que el 15% de pacientes de CHC presentan sobreexpresión de IGF2. Esta alteración se vio asociada con altos niveles de la isoforma pro-proliferativa INSR-A, la cual tiene gran afinidad por el IGF2. 2. La causa predominante de la sobreexpresión de IGF2 que se observa en CHC es una desregulación de la metilación que incluye la hipometilación de los promotores fetales (P3 y P4), un aumento de la metilación del promotor adulto P1 y una pérdida de la imprintación génica. 3. La sobreexpresión de IGF2 en el hígado del modelo murino modificado genéticamente redujo significativamente la supervivencia de estos ratones comparados con los controles mediante un mecanismo de aceleración de la progresión tumoral. 4. Los estudios con líneas celulares de CHC indicaron que el mecanismo tumorogénico del IGF2 se basa en un incremento de la proliferación de las células tumorales y no en un mecanismo antiapoptótico o promigratorio. 5. Los análisis genómicos revelaron que los tumores con altos niveles de IGF2 presentan un fenotipo proliferativo, desdiferenciado y más agresivo. 6. Los resultados in vitro con xentuzumab, un anticuerpo monoclonal dirigido contra IGF1 y 2, sugieren que este nuevo tipo de terapias anti-IGF, al contrario que los inhibidores de IGF1R, son capaces de bloquear la proliferación de las células de CHC inducida por el IGF2 a través de la activación de los receptores IGF1R y INSR-A, sin afectar a la vía metabólica. 7. En un modelo de ratón con CHC subcutáneo, xentuzumab inhibió eficazmente el crecimiento tumoral y alargó la supervivencia de los ratones con CHC subcutáneo mediante un mecanismo anti-proliferativo y anti-angiogénico. ESTUDIO 2: Identificación de una subclase Inmune de CHC basada en características moleculares Los principales resultados de este estudio fueron: 1. La microdisección virtual de las señales moleculares derivadas de los distintos tipos celulares presentes en los tumores de CHC mediante el método de non-negative matrix factorization (NMF) fue capaz de aislar la señal producida por los infiltrados inflamatorios. Esto nos ha permitido definir una subclase Inmune en el 25% de los pacientes de CHC, que puede identificarse mediante un clasificador Inmune de 112 genes. 2. La nueva subclase Inmune se caracteriza por un enriquecimiento significativo de firmas genéticas que identifican la presencia de células inmunitarias (ej: células T, células citotóxicas, macrófagos), activación de la vía del interferón y firmas de respuesta a terapias inhibitorias de checkpoint inmunitarios. 3. La integración con otras clases moleculares previamente definidas reveló un enriquecimiento en la subclase de interferón y la S1, así como una exclusión significativa de la subclase de CTNNB1. 4. El examen patológico reveló que los pacientes pertenecientes a la clase Immune muestran un alto grado de infiltración inmunitaria, más expresión de PD-1/PD-L1 detectada por immunohistoquímica y presencia de estructuras linfoides terciarias. 5. La clase Inmune contiene dos componentes microambientales diferentes: a) la subclase con respuesta inmune activa (~65%) caracterizada por la sobreexpresión de los genes de respuesta inmune adaptativa y b) la subclase con respuesta inmune exhausta (~35%) caracterizada por la presencia de señales inmunosupresoras (ej: TGF-β, macrófagos M2). 6. Las características moleculares de la subclase Inmune, así como la presencia de los dos componentes microambientales fueron reproducidos con éxito en siete cohortes adicionales de CHC, independientemente de la plataforma o el tipo de muestra utilizados. 7. Los tumores de la subclase Inmune tienen un menor número de alteraciones cromosomales en comparación con el resto, pero no muestran diferencias a nivel del número de neo-antígenos o carga mutacional. 8. El análisis de los datos de la metilación indica que la clase Inmune tiene un perfil de metilación diferencial, asociado mayoritariamente a genes relacionados con el sistema inmune. 9. Las muestras de CHC clasificadas dentro de la subclase de CTNNB1 muestran menores niveles de infiltración inmunitaria y una sobreexpresión de PTK2, la cual cosa sugiere que diversas vías oncogénicas podrían estar cooperando para modificar el perfil inmunitario del tumor. 10. El perfil inmunitario del tejido adyacente al tumor no refleja el perfil intratumoral y está caracterizado mayormente por señales inmunosupresoras asociadas con la supervivencia de los pacientes de CHC. CONCLUSIONES: 1. Nuestro primer estudio demuestra la contribución del IGF2 al proceso de hepatocarcinogenesis y lo define como un oncogén epigenético, cuyo bloqueo podría beneficiar a un subgrupo de pacientes de CHC. 2. Nuestro segundo estudio representa una caracterización integral del perfil inmunológico de los tumores humanos de CHC e introduce una nueva subclase Inmune, a la que pertenecerían el 25% de los pacientes. La identificación de esta subclase podría tener implicaciones en las futuras estrategias terapéuticas que impliquen inmunoterapias en CHC. BIBLIOGRAFÍA: 1.
