6. First report of an outbreak of "Q" fever IN an abattoir from Argentina.

Abstract

In late October 2021, one of the veterinarians and the occupational physician of a bovine and swine abattoir from Entre Ríos Province, Argentina were alerted about workers with atypical pneumonia symptoms, raising suspicious of a possible Q fever outbreak. An outbreak epidemiological investigation was carried out. Analysis was based on the description of the study population, according to gender, age, symptoms, and position within the abattoir, as well as on outbreak epidemic curve and its probable origin. Cases of Q fever in the workers were confirmed by serology. Measurements of the association between the evaluated variables and the risk of exposure were investigated and calculated as attack rates. The outbreak occurred between October and November 2021, symptomatically affecting 11 workers, out of a total exposed population of 49 individuals. The index case was a 33‐year‐old male who started with symptoms on 27 October 2021, and the outbreak extended for at least 17 days. Workers in the clean zone of the slaughter floor had a 4.68 times higher risk of contracting Q fever than people located in other areas. Importantly, two pregnant cows were slaughtered a few days before the outbreak began, which could have been the origin of the outbreak. The present study demonstrates the urgent need to consider Q fever when diagnosing abortive diseases of ruminants in Argentina, as well as in zoonotic disease epidemiological surveillance to inform all actors of the health system. Abstract

En Argentina el escorpionismo es el principal envenenamiento por animales ponzoñosos. Desde comienzos del siglo XX, Tityus trivittatus ha sido descripto en Buenos Aires. El escorpionismo es una intoxicación que produce un síndrome neurotóxico con síntomas locales y manifestaciones sistémicas. Puede desencadenar insuficiencia cardiaca, edema agudo de pulmón, shock y muerte. Afortunadamente, la mayoría de los accidentes con escorpiones en adultos en Buenos Aires son intoxicaciones leves. Se realizó un estudio retrospectivo, descriptivo y transversal basado en información recolectada de las historias clínicas desde 1982 hasta 2013. Se recolectó información sobre un total de 141 intoxicaciones por escorpiones y 115 capturas de artrópodos: 88 T. trivittatus, 10 Bothriurus bonariensis y 17 sin identificar. Los accidentes sucedieron más frecuentemente en los barrios del este y más antiguos de la ciudad. Dieciocho pacientes tuvieron síntomas sistémicos leves. No hubo ninguna muerte. Doce pacientes recibieron antiveneno. La mayoría de los envenenamientos fueron leves, aunque recientemente se produjo un caso grave en un niño de 5 años y en algunas provincias se han registrado muertes. Hemos observado un tratamiento inadecuado en algunos pacientes por lo que consideramos que sería importante capacitar en el correcto manejo y prevención de este envenenamiento. In Argentina scorpion stings are the leading cause of venom-related injury to human. Since the beginning of the 20th century Tityus trivittatus is found in Buenos Aires. Scorpion envenomation is a neurotoxic syndrome with local symptoms and systemic manifestations. It could develop cardiac failure, acute pulmonary edema, shock and death. Fortunately, most of the scorpion stings in adult people in Buenos Aires are mild envenomations. A retrospective, descriptive and cross sectional study based on data collected from medical records of patients followed between 1982 and 2013 were done. We compiled a total of 141 human scorpion stings and 115 arthropod captures: 88 T. trivittatus, 10 Bothriurus bonariensis and 17 missed data. The accidents occurred more frequently in the eastern and oldest neighborhoods of the city. Eighteen patients had mild systemic manifestation. There was not any death. Twelve patients received scorpion antivenom. Although recently there was a severe case in 5 years old boy and in some provinces in Argentina death have been reported, most of the envenomations are mild. We observed inadequate treatment in some patients; it is important training on the correct management and prevention of this envenomation.

Abstract

La enfermedad de Chagas, causada por el protozoo Trypanosoma cruzi, es una antropo-zoonosis endémica en Latinoamérica, vinculada con aspectos socio-económico-culturales deficitarios y considerada una de las enfermedades desatendidas. Presentamos un caso fatal de una reactivación de la enfermedad de Chagas con afectación del sistema nervioso central en un paciente con infección por VIH. El diagnóstico se confirmó por reacción de polimerasa en cadena (RPC) positiva en sangre. Tuvo una buena respuesta al tratamiento con benznidazol. Las dificultades en el manejo del tratamiento etiológico se debieron al número limitado de medicamentos antitripanosomiásicos y la aparición de efectos adversos graves. Chagas disease (ChD), caused by the protozoan Trypanosoma cruzi, is an endemic anthropozoonosis in Latin America, linked to deficients socio-economic and cultural aspects and is considered one of the neglected tropical diseases. We report a fatal case of Chagas disease reactivation with central nervous system involvement in a patient with HIV infection, whose diagnosis was confirmed by positive PCR (polymerase chain reaction) test of blood, with treatment response efficiency with benznidazol and management and etiologic treatment was difficult due to limited number of antitrypanosomal drugs and the occurrence of frequent and serious adverse effects.

Abstract

La infección por Plasmodium vivax se caracteriza por la formación de hipnozoítos que permanecen quiescentes en los hepatocitos del hospedero y son responsables de las recaídas de la malaria. Primaquina es el único fármaco en uso para la erradicación de los hipnozoítos y asociado a un agente esquizonticida, constituye el tratamiento radical. Las fallas al tratamiento radical están relacionados con una dosificación subóptima, adherencia inadecuada y reinfección. Sin embargo, cuando estos factores están ausentes, se han postulado mecanismos propios del metabolismo del hospedero y de tolerancia del parásito. Se describe un caso de malaria por P. vivax adquirido en la región amazónica asistido fuera de la zona endémica, con múltiples recaídas a pesar del tratamiento radical adecuado. Relapsing Plasmodium vivax malaria is due to activation of dormant intrahepatic parasitic forms known as hypnozoits. Primaquine is the only available drug effective against hypnozoits and, alongside a schizonticidal drug, constitutes the radical treatment of malaria. Failure of radical treatment is frequently attributed to inadequate dosing, poor adherence, or reinfection. However, several cases of radical treatment failure without these factors have been reported, inferring that metabolic properties of the host or tolerance mechanisms of the parasite may be implied. A case of malaria due to Plasmodium vivax acquired in the Amazonic region, treated outside endemic area, with multiple relapses despite adequate radical treatment is described.

La variable geografía, los atractivos turísticos y los múltiples sitios de interés histórico cultural, hacen que América del Sur sea un destino elegido por los viajeros. El continente tiene una amplia variación climática de norte a sur, lo que hace que la exposición al riesgo sea diferente entre las regiones tropicales y las templadas o frías. En los países que conforman América del Sur tropical, el mayor riesgo se asocia a la posibilidad de adquirir enfermedades de transmisión vectorial como fiebre amarilla, dengue, paludismo y leishmaniasis entre otras. El riesgo de adquirir diarrea del viajero y enfermedades de transmisión por alimentos es similar en todo el continente, con algunas variaciones de acuerdo al país y si se visitan áreas urbanas o rurales. Rabia, coqueluche y difteria han aparecido en forma epidémica en varios países y otras enfermedades como rickettsiosis, hantavirosis y encefalitis virales han ampliado su distribución. La diversidad geográfica y epidemiológica de América del Sur, plantea un importante desafío para los especialistas en Medicina del Viajero, ya que para asesorar adecuadamente a los viajeros al continente, hay que considerar no sólo el tipo de viaje, sino también los antecedentes personales, la exposición al riesgo y la dinámica de las enfermedades endémicas, emergentes y reemergentes en la región. The geography, tourist attractions and the multiple sites of historical and cultural interest make South America as an important destination chosen by travelers. The continent has a wide climatic variation from north to south, making exposure to risk different between the tropics and the temperate or cold regions. In the countries of tropical South America, the greatest risk is associated with the possibility of acquiring vector-borne diseases, like yellow fever, dengue, malaria and leishmaniasis. The risk of acquiring traveler's diarrhea and food-borne illness is similar across the continent, with some variations according to country and to visit urban or rural areas. Rabies, pertussis and diphtheria have appeared as epidemics in several countries and other diseases such as rickettsiosis, hantavirosis and viral encephalitis have expanded their distribution. The geographic and epidemiological diversity of South America, promotes a challenge for travel medicine specialists because during the pre-travel advice they have to take in account the kind of trip, traveller's medical history, exposure to risk and the dynamics of endemic emerging and reemerging diseases in the region.

Abstract

We communicate the diagnosis by microscopy of a pulmonary coinfection produced by Cryptococcus neoformans and Pneumocystis jiroveci, from a respiratory secretion obtained by bronchoalveolar lavage of an AIDS patient. Our review of literature identified this coinfection as unusual presentation. Opportunistic infections associated with HIV infection are increasingly recognized. It may occur at an early stage of HIV-infection. Whereas concurrent opportunistic infections may occur, coexisting Pneumocystis jiroveci pneumonia (PCP) and disseminated cryptococcosis with cryptococcal pneumonia is uncommon. The lungs of individuals infected with HIV are often affected by opportunistic infections and tumours and over two-thirds of patients have at least one respiratory episode during the course of their disease. Pneumonia is the leading HIV-associated infection. We present the case of a man who presented dual Pneumocystis jiroveci and cryptococcal pneumonia in a patient with HIV. Definitive diagnosis of PCP and Cryptococcus requires demonstration of these organisms in pulmonary tissues or fluid. In patients with < 200/microliter CD4-lymphocytes, a bronchoalveolar lavage should be performed. This patient was successfully treated with amphotericin B and trimethoprim sulfamethoxazole. After 1 week the patient showed clinical and radiologic improvement and was discharged 3 weeks later.

Abstract

Infectious diseases are among the leading cause of morbidity and mortality in Latin America. Despite the availability of safe, effective and relatively accessible vaccines, adult vaccination remains at a low level worldwide and especially in Latin America resulting in millions of adults every year suffering illness, hospitalization and even death from vaccine-preventable diseases. In response to this problem, the Americas Health Foundation facilitated the development of this document: Recommendations to Improve Adult Vaccination Coverage in Latin America. The 24 endorsing organizations are deeply committed to increasing adult vaccination coverage in the Region and believe that it should be considered an individual, community and governmental responsibility and that increasing vaccination coverage in adults is an essential investment in every country's future. The criteria for including a recommendation to improve adult vaccination coverage in Latin America are that it be timely, reasonable, effective, relevant and clear. While the challenges to adult vaccination coverage are multi-factorial, they can be overcome by a commitment from government, payers, health care professionals and others. Most important, we believe that every country should have a nationwide schedule for adult immunization. We believe that as a result of implementing the recommendations in this paper, all adults will be better informed about the severity of vaccine-preventable diseases, vaccine safety and efficacy, current recommendations, the fact that vaccines are as essential for adults as they are for children, and that vaccination is a life-long need. The aim of this work was to compare the epidemiological and clinical characteristics of Rickettsia parkeririckettsiosis related to Amblyomma tristeand Amblyomma tigrinumecological regions in Argentina. We reviewed cases of R. parkeririckettsiosis from 2007 to 2017 evaluated at Muñiz Hospital, directly or through referral. Univariate analysis was used to examine the association between different variables and the disease related by each vector species. The eighteen cases of R. parkeririckettsiosis included had fever, inoculation eschar and all except one had rash. Regional differences in epidemiological variables were identified, depending on the vector. There was a significantly increased risk of exposure to A. tigrinumin peri-domestic areas (odd ratio 12, p = 0.02), whereas an increased risk of exposure to A. tristewas evident in wildlife areas (odd ratio 12, p = 0.02). Seasonality of R. parkeririckettsiosis differed based on its vector. Cases associated with A. tristeoccurred predominantly during spring and summer, whereas those associated with A. tigrinumoccurred during fall, winter, and springtime. Exanthema was maculopapular (13/18), maculo-vesicular (3/18) or petechial (1/18). No clinical differences were identified depending on the vector.

47. Fatal journeys: causes of death in international travellers in South America.

Abstract

Background: Understanding mortality among travellers is essential for mitigating risks and enhancing travel safety. However, limited evidence exists on severe illnesses and injuries leading to death among travellers, particularly in low- and middle-income countries and remote regions., Methods: We conducted a retrospective census study using country-level observational data from death certificates of travellers of seven South American countries (Argentina, Brazil, Chile, Colombia, Ecuador, Peru, and Uruguay) from 2017 to 2021. Causes of death were evaluated using ICD-10 codes, categorised into non-communicable diseases (NCDs), communicable diseases, and injuries. We quantified causes of death by demographic characteristics (e.g. age, sex), and geographical variables. Chi-square tests were used to assess differences between categories. We calculated crude mortality rates and incidence rate ratios (IRRs) per country's subregions., Results: A total of 17 245 deaths were reported. NCDs (55%) were the most common cause of death, followed by communicable diseases (23.4%) and injuries (18.1%). NCD-associated deaths increased after age 55 years and were highest among ≥85 years. Communicable diseases were more common at younger age (<20 years). Injury-associated deaths were more common in men (79.9%) and 25-29-year-olds (17.1%). Most deaths (68.2%) could have been avoided by prevention or treatment. Mortality risk was higher among travellers in bordering regions between countries. In Roraima [Brazil] and Norte de Santander [Colombia], locations bordering Venezuela, the death incidence rate ratio was 863 and 60, respectively. These countries' reference mortality rates in those regions were much lower. More than 80% of the deaths in these border regions of Brazil and Colombia involved Venezuelan citizens. Conclusion: The study identified risk factors and high-risk locations for deaths among travellers in seven countries of South America. Our findings underscore the need for specific health interventions tailored to traveller demographics and destination to optimise prevention of avoidable deaths in South America., (© The Author(s) 2024. The first autochthonous case of rickettsiosis is reported here. The case occurred in the Costanera Sur Ecological Reserve, a protected area of the City of Buenos Aires, in August 2022, where 4 species of ticks were found, namely Amblyomma aureolatum, Ixodes auritulus sensu lato, Rhipicephalus sanguineus sensu stricto and Amblyomma triste. Abstract

We report an autochthonous case of Rickettsia parkeri rickettsiosis occurred in June 2018 in a forested area of the Urugua-í Provincial Park, Misiones, Argentina. No previous records of this disease in humans have been previously reported in this region. The epidemiological, ecological, clinical, and laboratory features required for a proper diagnosis and adequate treatment are described here.
