Abstract

Elexacaftor–tezacaftor–ivacaftor has been approved in Europe for people with cystic fibrosis with at least one F508del CFTRvariant. Additionally, it is approved by the US Food and Drug Administration (FDA) for people with cystic fibrosis with at least one of 177 rare variants. The aims of this study were to describe the clinical response to elexacaftor–tezacaftor–ivacaftor for people with cystic fibrosis without a F508del CFTRvariant in France and to determine CFTRvariant responsiveness to elexacaftor–tezacaftor–ivacaftor based on the observed clinical response.

Abstract

International audience; Household disinfectant and cleaning products (HDCPs) assessment is challenging in epidemiological research. We hypothesized that a newly-developed smartphone application was more objective than questionnaires in assessing HDCPs. Therefore, we aimed to compare both methods, in terms of exposure assessments and respiratory health effects estimates. The women of the SEPAGES birth cohort completed repeated validated questionnaires on HDCPs and respiratory health and used an application to report HDCPs and scan products barcodes, subsequently linked with an ingredients database. Agreements between the two methods were assessed by Kappa coefficients. Logistic regression models estimated associations of HDCP with asthma symptom score. The 101 participants (18 with asthma symptom score ≥1) scanned 617 different products (580 with available ingredients list). Slight to fair agreements for sprays, bleach and scented HDCP were observed (Kappa: 0.35, 0.25, 0.11, respectively). Strength of the associations between HDCP and asthma symptom score varied between both methods but all odds ratios (OR) were greater than one. The number of scanned products used weekly was significantly associated with the asthma symptom score (adjusted-OR [CI 95%]: 1.15 [1.00–1.32]). This study shows the importance of using novel tools in epidemiological research to objectively assess HDCP and therefore reduce exposure measurement errors.

Abstract

The types of financial responsibility centers are responsible for fulfilling a certain function in each center and facilitating the fulfillment of the objectives and strategies in a period established by the company. The centers of responsibility allow equity in the functions of each of the centers and take advantage of the intellect of the members that help to obtain and increase the economic benefit. There are three types of responsibility centers: Cost centers: This center is responsible for complying with the budget established for production normally and in turn trying to reduce the costs incurred in production through a constant update of the costs that exist in the market with the objective of meet the budget optimize expenses. Profit centers: This center is responsible for making the right decision about the income and expenses that generate benefits for the company through the study of internal and external factors that influence the economy of the company. Investment centers: This center is responsible for controlling through an analysis of the financial situation maintained by the country where the company's short and long-term investments are being made in order to safeguard the cash invested in order to obtain external profits At the turn of the business. The Study and research that leads each financial responsibility center to analyze the work periods in previous years, analyze internal and external factors and study the market costs at present. There are two types of methodology that help and facilitate the fulfillment of the objectives are: the descriptive and correctional method. The result of the study and the analysis of the methods help to implement controls that comply with the standards and policies indicated by the company for the fulfillment of the functions and objectives of the company. With the implementation of new controls, the results will be improved and the benefits of a production and investment period for the company increased. Los centros de responsabilidad permiten que exista equidad en las funciones de cada uno de los centros y aprovechar el intelecto de los miembros que ayudan a obtener e incrementar el beneficio económico. Existen tres tipos de centros de responsabilidad: Centros de costo: Este centro es responsable de cumplir con el presupuesto establecido para la producción con normalidad y a su vez tratar de reducir los costos que incurren en la producción a través de una constante actualización de los costos que existen en el mercado con el objetivo de cumplir con el presupuesto de optimizar los gastos. Centros de beneficios: Este centro es responsable de tomar la correcta decisión sobre los ingresos y gastos que generen beneficios para la empresa a través del estudio de los factores internos y externos que influyen dentro de la economía de la empresa. Centros de inversión: Este centro es responsable de controlar mediante un análisis la situación financiera que mantiene el país donde se están realizando las inversiones a corto y largo plazo de la empresa con la finalidad de salvaguardar el efectivo invertido con la finalidad de obtener ganancias externas al giro del negocio. Abstract

IF 1.710; International audience; Fungal respiratory colonization of cystic fibrosis (CF) patients emerges as a new concern; however, the heterogeneity of mycological protocols limits investigations. We first aimed at setting up an efficient standardized protocol for mycological analysis of CF sputa that was assessed during a prospective, multicenter study: “MucoFong” program (PHRC-06/1902). Sputa from 243 CF patients from seven centers in France were collected over a 15-month period and submitted to a standardized protocol based on 6 semi-selective media. After mucolytic pretreatment, sputa were plated in parallel on cycloheximide-enriched (ACT37), erythritol-enriched (ERY37), benomyl dichloran–rose bengal (BENO37) and chromogenic (CAN37) media incubated at 37 °C and on Sabouraud–chloramphenicol (SAB27) and erythritol-enriched (ERY27) media incubated at 20–27 °C. Each plate was checked twice a week during 3 weeks. Fungi were conventionally identified; time for detection of fungal growth was noted for each species. Fungal prevalences and media performances were assessed; an optimal combination of media was determined using the Chi-squared automatic interaction detector method. At least one fungal species was isolated from 81% of sputa. Candida albicans was the most prevalent species (58.8%), followed by Aspergillus fumigatus (35.4%). Cultivation on CAN37, SAB27, ACT37 and ERY27 during 16 days provided an optimal combination, detecting C. albicans, A. fumigatus, Scedosporium apiospermum complex and Exophiala spp. with sensitivities of 96.5, 98.8, 100 and 100%. Combination of these four culture media is recommended to ensure the growth of key fungal pathogens in CF respiratory specimens. The use of such consensual protocol is of major interest for merging results from future epidemiological studies.

Abstract

Objectives: Lumacaftor-ivacaftor is a cystic fibrosis transmembrane conductance regulator (CFTR) modulator known to improve clinical status in people with cystic fibrosis (CF). The aim of this study was to assess lung structural changes after 1 year of lumacaftor-ivacaftor treatment and to use unsupervised machine learning to identify morphological phenotypes of lung disease that are associated with response to lumacaftor-ivacaftor., Methods: Adolescents and adults with CF from a French multicentre real-world prospective observational study evaluating the first year of treatment with lumacaftor-ivacaftor were included if they had pre-therapeutic and follow-up chest computed tomography (CT) scans available. CT scans were visually scored using a modified Bhalla score. A k-means clustering method was performed based on 120 radiomics features extracted from unenhanced pre-therapeutic chest CT scans., Results: In total, 283 patients were included. The Bhalla score significantly decreased after 1 year of lumacaftor-ivacaftor (-1.40±1.53 points compared with pre-therapeutic CT, p<0.001). This finding was related to a significant decrease in mucus plugging (-0.58±0.88 points, p<0.001), bronchial wall thickening (-0.35±0.62 points, p<0.001) and parenchymal consolidations (-0.24±0.52 points, p<0.001). Cluster analysis identified three morphological clusters. Patients from cluster C were more likely to experience an increase in per cent predicted forced expiratory volume in 1 s (FEV 1 % pred) ≥5% under lumacaftor-ivacaftor than those in the other clusters (54% of responders versus 32% and 33%; p=0.02)., Conclusion: 1-year treatment with lumacaftor-ivacaftor was associated with a significant visual improvement of bronchial disease on chest CT.
