1. Non-invasive optical and laboratory hematologic biomarkers correlate in patients with sickle cell disease.

Abstract

The goal of this study is to identify non-invasive optical hemodynamic biomarkers that can index laboratory hematology measurements in sickle cell disease (SCD). We acquired frequency-domain NIRS (FD-NIRS) and diffuse correlation spectroscopy (DCS) data from the forearms and foreheads of 17 participants in a randomized, double-blind, placebo-controlled trial evaluating effects of isoquercetin (IQ) on thromboinflammation in SCD. We observed multiple, significant correlations between optical and hematology biomarkers including cerebral tissue oxygen saturation (StO2) and hematocrit (HCT); oxyhemoglobin ([O2Hb]) recovery rate and intercellular adhesion molecule 1 (ICAM-1); and blood flow index (BFI) reperfusion rate and coagulation index (CI). The potential of these non-invasive optical biomarkers for assessing vascular pathophysiology for the management of SCD warrants further exploration.

Abstract

International audience; Thanks to their membrane elasticity and fluidity, red blood cells (RBCs) are capable to pass through the smallest microcapillaries while exchanging oxygen and CO2 with organs. For several blood disorders like sickle cells disease (SCD), RBCs lose their deformability leading to vaso-occlusions. To assess the deformability of RBC, successive mechanical stresses are applied in our device mimicking blood microcapillaries network, while RBCs circulating being monitored optically by microscopy. Normal and sickled RBCs behaviors are compared using such approach. This device could be a promising tool for the diagnostic of different blood disorders.

Abstract

International audience; Red blood cell (RBC) microfluidic flow behaviours could be altered by varies of factors: the density of RBC, the cell composition, the level of cell dehydration as well as the RBC membrane damage. For the electrical monitoring of RBC microfluidic flow behaviours, the contact surface between RBC and mimetic microrestriction wall, whose width is smaller than the RBC diameter, is crucial. In this paper, we focus on cell rigidity and morphological alterations induced by hereditary spherocytosis, which play an important role for the analysis of RBC flow behaviours and the discrimination between RBCs of different pathological states.

International audience; For quantitative single cell analysis within vessel size microchannel, bioimpedance technology is considered to be important for future advanced biomedical applications. However, most of bioimpedance systems takes place and is not allowed to bring to biological institute. In this paper, two miniaturization systems for electrical biosensing and data acquisition will be introduced and compared. By considering the advantages of portable systems such as easy to take and use, simplicity and inexpensive, instead of impedance spectroscopy obtained by traditional equipment, the miniaturization bioimpedance systems might be helpful for 'Lab-on-a-Chip' device to be more complex.

Abstract

Combining microfluidic with sensors enables the development of smart analysis systems. Microelectrodes can be embedded within the microchannels network for electrical sensing, electrochemical analysis or impedance measurement. However, at the laboratory scale, the assembly between microfluidic network and electrical parts on the substrate remains an issue. This paper first discusses the principles of biosensing, and then proposes an original device integrating microfluidics with microelectrodes for the analysis of red blood cells transit in a structure mimicking micro-vascular flow. Some results concerning red blood cells discrimination of sickle cell disease are discussed with statistical analysis.Clinical relevance- This paper introduces a portable reusable device combining a microfluidic blood vessel mimicking network with microelectrodes for the biosensing of RBC.

Abstract

Vaso-occlusive crises are the hallmark of sickle cell disease (SCD). They are believed to occur in two steps, starting with adhesion of deformable low-dense red blood cells (RBCs), or other blood cells such as neutrophils, to the wall of post-capillary venules, followed by trapping of the denser RBCs or leukocytes in the areas of adhesion because of reduced effective lumen-diameter. In SCD, RBCs are heterogeneous in terms of density, shape, deformability and surface proteins, which accounts for the differences observed in their adhesion and resistance to shear stress. Sickle RBCs exhibit abnormal adhesion to laminin mediated by Lu/BCAM protein at their surface. This adhesion is triggered by Lu/BCAM phosphorylation in reticulocytes but such phosphorylation does not occur in mature dense RBCs despite firm adhesion to laminin. In this study, we investigated the adhesive properties of sickle RBC subpopulations and addressed the molecular mechanism responsible for the increased adhesion of dense RBCs to laminin in the absence of Lu/BCAM phosphorylation. We provide evidence for the implication of oxidative stress in post-translational modifications of Lu/BCAM that impact its distribution and cis-interaction with glycophorin C at the cell surface activating its adhesive function in sickle dense RBCs.

Abstract

The genetic and molecular basis of sickle cell disease (SCD) has long since been characterized but the pathophysiological basis is not entirely defined. How a red cell hemolytic disorder initiates inflammation, endothelial dysfunction, coagulation activation and eventually leads to vascular thrombosis, is yet to be elucidated. Recent evidence has demonstrated a high frequency of unprovoked/recurrent venous thromboembolism (VTE) in SCD, with an increased risk of mortality among patients with a history of VTE. Here, we thoroughly review the molecular basis for the prothrombotic state in SCD, specifically highlighting emerging evidence for activation of overlapping inflammation and coagulation pathways, that predispose to venous thromboembolism. We share perspectives in managing venous thrombosis in SCD, highlighting innovative therapies with the potential to influence the clinical course of disease and reduce thrombotic risk, while maintaining an acceptable safety profile.
