The efficacy of the percutaneous transluminal balloon angioplasty in patients with critical ischemia of the lower limbs and diabetes mellitus was analyzed. The study included 102 patients with septic complications of the diabetic foot syndrome. The main group consisted of 37 patients, who received both the traditional conservative therapy and the endovascular angioplasty. The study proves that the endovascular interventions by diabetic foot syndrome are effective and justified. Revascularisation permits the minimal resectional interventions on the foot, stop the ischemia and minimize the number of repeated necrectomies and high amputations. The diabetic foot syndrome is represented as a multidisciplinary problem.

The results of surgical treatment of patients with the fourth stage of the upper limbs' lymphedema was studied. Of all 170 patients with lymphedema, 16 (9.4%) had the fourth stage of the disease. The debulking procedures were performed in 15 patients (8 had standart operations, 7 - simultaneous rwo-stage operations). Beeing a preliminary stage of the radical surgical excision of lymphoedematously changed tissues, the liposuction allows a more precisional hemostasis for the accurate vessel visualization. It led to the 4,4 times decrease of the postoperative morbidity rate and shortened the time of the operation.

Using transmission electron microscopy, the ultrastructural organization of the endothelium of blood and lymphatic capillaries and the structures of the interstitium of synovial membrane were studied in the patients with osteoarthrosis of a knee joint of I-II and II-III stages. Material, obtained from the patients with traumatic lesions of the knee joint components, was used as a control. The structural signs indicative of the microcirculation and lymphatic drainage disturbances were detected in the synovial membrane and were most expressed in II-III stages of osteoarthrosis. These included: the decrease of the volumetric density of all the types of micropinocytotic vesicles and the cisterns of rough endoplasmic reticulum, the decrease of the numerical density of attached and free polysomal ribosomes, the accumulation of erythrocytes and thrombocytes in the lumen of blood capillaries, the increase of the number of open-type interendotheliocyte contacts in the lymph capillaries, the increase of the size and the decrease of the electron density of the pericapillary and the interstitial spaces.

Capillaries and tissue leukocyte cytograms in soft gingival tissues were examined by optic microscopy in patients with acute purulent periostitis of the jaws treated by different methods. Disorders in the lymph outflow from gingival tissues were detected, which were due to dilatation of the lymph vessels and interstitial spaces in the mucosal lamina propria and stasis with leukocyte aggregation and purulent clots in the lumina of many lymph vessels. Less pronounced dilatation of interstitial spaces and lymph vessels, higher counts of lymphocytes, monocytes, and macrophages in the tissues of the gingival mucosa lamina propria, observed 2 days after the beginning of adsorption therapy of the subperiosteal abscess cavity, indicate that this therapy is more effective in acute purulent periostitis than traditional treatment.

Microcirculation and tissue leukocytes in transplanted on vascular peduncle tissue complex were studied with light microscopy. Lymph outflow disorders were the causes of transplant edema. Lymph outflow and hemocirculation were restored through new vessels formation that was revealed in all transplant tissues over 2-8 months after transplantation. With time, processes of sclerotic transformation in tissues of transplanted flap progress due to denervation and function loss of transplanted tissues.

Light microscopy was used to study microvascular bed and tissue leukocytes in transplanted tissue complex on the vascular pedicle. The cause of the transplant edema was disturbed lymph flow manifesting in a significant dilation of lymph vessels and interstitial spaces. Restoration of the lymph flow and hemocirculation takes place by formation of new vessels registered in all the transplant tissue 2-8 months after transplantation. Late after the transplantation the tissues of the graft underwent progressive sclerotic transformation due to denervation and loss of specific functions of the transplanted tissues.

The use of the enterosorbent SUMS-1 in the combined therapy for pulmonary tuberculosis patients with drug-induced toxic hepatic lesion arrests the clinical and laboratory manifestations of endotoxicosis, removes antibiotic intolerance, increases the elimination rate of isoniazid, and reduces its distribution scope, the area beneath the kinetic curve being unchanged. The plasma concentrations of primary products and after-products of lipid peroxidation products during enterosorbent therapy decline and the levels of ceruloplasmin and alpha-tocopherol remain unchanged.

Morphological changes in inguinal lymph nodes, lymphatic collector and subcutaneous space were studied up in patients with the lower extremities lymphoedema. While the lower extremities lymphoedema occurs in inguinal lymph nodes the connective tissues spreading in two variants is observed. In the first one fibroblasts and collagen fibres appears in medullar substance (chordae medullaris and medullar sinuses) and in the second the sclerosis process begins from cortical plateau and paracortical zone. It is expedient to include the intraarterial infusion of the xenospleen preparations in complex of treatment of patients with initial stage of the lower extremities lymphoedema, especially in the absence of possibility to proceed with microsurgical intervention and the erysipelas presence in anamnesis.

Adsorption of lymphocytes on the sorbents in correction of inflammatory lesions has significant clinical perspectives as the cells and active biological substances they secrete function actively in inflamed tissues acting in the site of its application: every cell adsorbs and inactivates foreign substances, every cytokin affects certain target cells. The products of these cells functioning and dissociation as well as the products of immune system reaction to them remain on the sorbent and are eliminated together with granules during bandaging or by the end of the treatment thus do not entering the organism. Lymphocytic monolayer on the sorbent does not prevent its specific action in pathologic nidus. In this case sorbent does not only drain the tissue duplicating certain primitive non specific functions of regional lymph node (barrier, filtrating, transport, draining and protective functions) but also performs certain highly specialized functions of immunocompetent organs--selective adsorption and inactivation of antigenic substances. It is reasonable to use sorbents for adsorbing lymphocytes in kapron containers which exclude the preparation leak into wounds and cavities and act as a filter preventing fast inactivation of the preparation by large fragments of tissue detritus.

The changes in the peritoneal cavity at different intervals after direct administration of the sorbent in purulent peritonitis are investigated. Great quantity of granular sorbent administered directly into the peritoneal cavity provides a stronger therapeutic effect than kanamycin. However, the probability of commissure formation and commissural ileus is greater too. The presence of separate granules of the sorbent in the peritoneal cavity does not produce pathologic changes for a long time.

Regional lymphatic bed in local application of mineral sorbent SUMS-2p in 80 white noninbred rats with experimental generalized purulent peritonitis is studied with the use of indirect intravital lympho-roentgenography. The correlation between lympho-dynamics and generalized purulent peritonitis stage was observed. It is shown that the peritoneo-sorption with the mineral sorbent SUMS-2p led to early restoration of lympho-drainage.

In the lymphoid tissue of the iliac lymph node at an aseptic inflammation in the uterus the greatest changes have been revealed in 48 h from the moment, when the foreign body has been introduced. They are manifested at the tissue level (increasing content of macrophages, lymphoblasts, large lymphocytes, plasmoblasts, immature plasmocytes, mast cells, neutrophils), as well as at the organic level (increasing amount of secondary lymph noduli, increasing area of the paracortical zone, decreasing area of the cortical plateau, of the medullary intermediate sinus, medullary cords). Use of carbomineral sorbents ensures correction of the inflammation; this is demonstrated as reduction of reactive changes in the regional lymph node.

Morphometrical investigation of the blood microcirculatory bed has been performed using the material from nonpregnant rats. Changes in volumetric density of the arterial and venous parts of the microcirculatory bed in the ovary and uterine tube after application of the spherical carbonate-mineral sorbent (SUMS-1) demonstrate a corregating effect of the sorption applied.

We studied the volume compactness of the arterial, capillary and venous parts of the microcirculation channel after 24, 48, 72 and 96 hours of the aseptic inflammation and after local application of the carbon-mineral sorbents in the rat's uterus. The local sorption decreased the reaction of the inflammation in the endo- and myometrium.
