Objective To study the epidemiology of mold infections in China. Methods Based on the surveillance data of 19 hospitals participating in the China Hospital Invasive Fungal Surveillance Net from Jan 2019 to Jun 2022, the general information of patients and the epidemiological characteristics such as the proportion of different strains and clinical infection were analyzed by WHONET software. Results A total of 16 285 mold infection cases were included in the analysis, of which 49.3% were patients aged 61 and over, with the median age of 60 years old. The proportion of males was significantly higher than that of females (62.1% vs. 37.9%). The patients were mainly from the internal medicine, ICU and surgical wards. Most strains were isolated from lower respiratory tract, accounting for 81.7%, followed by pus and secretions (7.8%). In terms of species distribution, Aspergillus spp. accounted for the highest proportion (84.8%), with Penicillium spp., Fusarium spp., order Mucorales and Sedosporium spp. accounting for 5.1%, 3.0%, 1.3% and 0.4%, respectively. For species distribution among different mold infection, 88.7% of lower respiratory tract mold infections were caused by Aspergillus spp., and Aspergillus fumigatus(47.8%) was the most common species. Otomycosis was mainly caused by Aspergillus spp.(98.7%), of which Aspergillus terreus accounted for 39.7%, and ophthalmomycosis was mainly caused by Fusarium spp.(54.6%). Conclusions By the retrospective analysis of mold isolation from multicenter in China, we found that Aspergillus fumigatus, Aspergillus terreus, and Fusarium spp. were the most common species causing pneumonomycosis, otomycosis, and ophthalmomycosis, respectively. Therefore, it is necessary to pay attention to the differences in species distribution among different mold infections in clinical empirical treatment of fungal infections.

Interaction between light and magnetism in magneto-optical (MO) nanophotonic devices has been actively studied in the past few years. The recent development of MO all-dielectric resonators and metasurfaces has led to the emergence of various novel MO phenomena that were not observed in their bulk counterparts. For example, a large s-polarized transverse MO Kerr effect can be observed at magnetic resonance wavelength, which cannot exist in the bare MO films. We review recent developments in nanophotonic devices based on MO materials and focus on different modes and related MO effects in nanophotonic structures with emphasis on recently discovered new MO phenomena in magnetoplasmonics and all-dielectric nanostructures, such as dark mode, all-dielectric Mie resonance and waveguide mode. Further, we discuss the potential applications of these nanostructures for biological/chemical sensing, magnetic field sensing, and magnetic field-controlled active and nonreciprocal metasurfaces.

Malignant mesothelioma (MM) is a rare aggressive tumor, and bone metastasis often occurs in later stages of this disease. This study aimed to establish a nomogram to predict the prognosis of bone metastasis of patients with MM. Data from the Surveillance, Epidemiology, and End Results database were screened and retrieved. This study included 311 patients with MM with bone metastases. Prognostic factors were analyzed using the Kaplan–Meier method and Cox proportional hazards model. A nomogram for overall survival (OS) was established and evaluated using statistically significant prognostic factors, and cancer-specific survival (CSS) analysis was performed to investigate its prognostic factors. In addition, the metastasis patterns of patients with MM were investigated, and the effects of different sites of metastasis on survival were compared using the Kaplan–Meier method. Age, sex, histological type, and chemotherapy were identified as the independent risk factors for OS. The 1-, 2-, and 3-year areas under the curve of the nomogram were 0.792, 0.774, and 0.928, and 0.742, 0.733, and 0.733 in the training and validation sets, respectively. Compared to OS, histological type, radiotherapy, and chemotherapy were independent risk factors for CSS. Different metastatic sites in MM have significantly different effects on prognosis. Introduction: Tuberculous meningitis (TBM) is still a great challenge to global public health. As conventional diagnostic methods for TBM are unsatisfactory, interferon-γ release assays (IGRAs) have been introduced for TBM diagnosis tentatively. However, the role of IGRAs for diagnosing TBM remains unclear. Thus, we systematically evaluated the diagnostic performance of cerebrospinal fluid (CSF) and peripheral blood (PB) IGRAs in TBM to fill this blank.Material and Methods: Relevant studies were systematically searched in both foreign and Chinese databases up to March 2018. Studies in which TBM diagnosis was based on microbiological or clinical criteria were included. The quality of the included studies was assessed through the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. Main outcome measures, including sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR) and diagnostic odds ratio (DOR), were pooled statistically using random effects models. The potential heterogeneity was explored by threshold effect analysis, subgroup analyses and meta-regression. Funnel plots and Egger's test were used to test the potential publication bias. Statistical analyses were performed using Stata and Meta-DiSc software.Results: Twenty-six out of 656 publications were eligible for meta-analysis, including 1892 participants in total. The pooled estimates of PB IGRAs for TBM diagnosis are as follows: sensitivity: 0.81 (95% CI: 0.78-0.84); specificity: 0.76 (95% CI: 0.73-0.78); PLR: 4.23 (95% CI: 2.95-6.07); NLR: 0.24 (95% CI: 0.19-0.32) and DOR: 21.06 (11.91-37.24). The corresponding estimates for CSF IGRAs were obtained: sensitivity: 0.81 (95% CI: 0.76-0.85); specificity: 0.89 (95% CI: 0.86-0.92); PLR: 7.87 (95% CI: 4.98-12.46); NLR: 0.19 (95% CI: 0.13-0.29); and DOR: 47.74 (25.02-91.12).Conclusions: The diagnostic performance of IGRAs is suboptimal. In terms of cost, turn-around time and accessibility, these assays are unsuitable for use as biomarkers for TBM diagnosis. [ABSTRACT FROM AUTHOR]

Hospital-acquired pneumonia (HAP) is a significant nosocomial infection; data on the distribution and antimicrobial resistance profiles of HAP in China are limited. We included 2827 adult patients with HAP from the Chinese Antimicrobial Resistance Surveillance of Nosocomial Infections network admitted in 15 Chinese teaching hospitals between 2007 and 2016. Clinical data and antimicrobial susceptibility of isolated pathogens were obtained from the medical records and central laboratory, respectively. Multivariable logistic regression was performed to determine the risk factors for mortality and multidrug resistance (MDR). A total of 386 (13.7%) patients died in the hospital, while 1181 (41.8%) developed ventilator-associated pneumonia (VAP). Active immunosuppressant therapy (OR 1.915 (95% CI 1.475–2.487)), solid tumor (OR 1.860 (95% CI 1.410–2.452)), coma (OR 1.783 (95% CI 1.364–2.333)), clinical pulmonary infection score ≥7 (OR 1.743 (95% CI 1.373–2.212)), intensive care unit stay (OR 1.652 (95% CI 1.292–2.111)), age ≥65 years (OR 1.621 (95% CI 1.282–2.049)), and tracheal cannula insertion (OR 1.613 (95% CI 1.169–2.224)) were independent risk factors for in-hospital mortality. Liver cirrhosis (OR 3.120 (95% CI 1.436–6.780)) and six other variables were independent predictors of MDR. Acinetobacter baumannii (25.6%), Pseudomonas aeruginosa (20.1%), Klebsiella pneumoniae (15.4%), and Staphylococcus aureus (12.6%) were the most common pathogens (MDR prevalence 64.9%). Isolates from VAP patients showed more A. baumannii and less K. pneumoniae and E. coli strains (p < 0.001, respectively) than those from patients without VAP. The proportion of methicillin-resistant S. aureus strains decreased; that of carbapenem-resistant A. baumannii and Enterobacterales strains increased. There had been changes in the antibiotic resistance profiles of HAP pathogens in China. Risk factors for mortality and MDR are important for the selection of antimicrobials for HAP in China. Carbapenem-resistant Acinetobacter baumannii has been known as an opportunistic pathogen that causes a variety of illness worldwide. In this study, we characterize the molecular epidemiology of 174 non-repetitive clinical isolates of A. baumannii collected from January to June 2009 from Xiangya Hospital, in Hunan Province, China, including an outbreak period of A. baumannii. These 174 isolates harbored A. baumannii intrinsic gene OXA-51. They were resistant to multiple antibiotics with resistance rates as 49.4% to imipenem, 48.3% to meropenem, 46.6% to ampicillin-sulbactam, 6.9% to cefoperazone-sulbactam, and 6.3% to minocycline. 74 out of 174 isolates were identified as carbapenemase-producing strains, among which blaOXA-23 gene was found in 71 isolates. These 74 carbapenemase expression strains could be divided into four genotypes by enterobacterial repetitive intergenic consensus (ERIC)-PCR, with 19, 17, 33 and 5 clones in each group. We also found four imipenem resistant isolates carrying OXA-23 gene without showing carbapenemase phenotype. Our findings show that the blaOXA-23 gene is the common carbapenemase gene among carbapenem-resistant Acinetobacter spp. isolates, suggesting that clonal spread of carbapenemase-producing isolates may be one important factor which results in the high carbapenem resistance rate in the local hospital in Hunan Province, China.
