Abstract. Introduction:. Global prevalence of knee osteoarthritis is more than 300 million. Uncontrollable risk factors include age, sex, and height. Controllable risk factors include trauma, weight, and waist circumference. Objectives:. Our goal was to determine the association between knee osteoarthritis and anthropometric measures that include weight, height, and waist circumference. Methods:. Using 4,602 participants (45–79 years) from the Osteoarthritis Initiative, we analyzed the association between knee osteoarthritis and anthropometry collectively and by sex. We calculated female and male tertiles (3 groups) for anthropometry. Results:. Anthropometric measures were correlated with knee osteoarthritis (P ≤ 0.05) except the correlation between height and activities and height and quality of life. When comparing female weight tertiles, there were associations (P's < 0.001) between knee osteoarthritis and weight, but when comparing male weight tertiles, these associations were primarily between the lowest weight and highest weight groups. There were significant associations between knee osteoarthritis and height among female tertiles, with no differences among male tertiles. There were knee osteoarthritis/waist circumference tertile associations (P's < 0.001) for the lowest and highest waist circumference groups. Conclusion:. Higher weight in female participants was a stronger predictor of increases in knee osteoarthritis discomforts when compared to waist circumference, while weight and waist circumference were almost equivalent in predicting increases in knee osteoarthritis for male participants. Height did not predict increases in knee osteoarthritis with the exception of female symptoms and quality of life. Quality of life for both sexes was the most unfavorable with female participants reporting a more unfavorable quality of life than male participants.

Abstract Introduction: Previous research indicates ethnic/race group differences in pain and neurodegenerative diseases. Accounting for socioenvironmental factors reduces ethnic/race group differences in clinical and experimental pain. In the current study sample, we previously reported that in individuals with knee pain, ethnic/race group differences were observed in bilateral temporal lobe thickness, areas of the brain associated with risk for Alzheimer's disease, and related dementias. The purpose of the study was to determine if socioenvironmental factors reduce or account for previously observed ethnic/race group differences and explore if a combined effect of socioenvironmental risk and chronic pain severity on temporal lobe cortices is evident. Methods: Consistent with the prior study, the sample was comprised of 147 adults (95 women, 52 men), 45–85 years of age, who self‐identified as non‐Hispanic Black (n = 72) and non‐Hispanic White (n = 75), with knee pain with/at risk for osteoarthritis. Measures included demographics, health history, pain questionnaires, cognitive screening, body mass index, individual‐ and community‐level socioenvironmental factors (education, income, household size, marital and insurance status, and area deprivation index), and brain imaging. Knee pain, the most common cause of musculoskeletal pain (MSK), constitutes a severe public health burden. Its neurobiological causes, however, remain poorly understood. Among many possible causes, it has been proposed that sleep problems could lead to an increase in chronic pain symptomatology, which may be driven by central nervous system changes. In fact, we previously found that brain cortical thickness mediated the relationship between sleep qualities and pain severity in older adults with MSK. We also demonstrated a significant difference in a machine-learning-derived brain-aging biomarker between participants with low-and high-impact knee pain. Considering this, we examined whether brain aging was associated with self-reported sleep and pain measures, and whether brain aging mediated the relationship between sleep problems and knee pain. Exploratory Spearman and Pearson partial correlations, controlling for age, sex, race and study site, showed a significant association of brain aging with sleep related impairment and self-reported pain measures. Moreover, mediation analysis showed that brain aging significantly mediated the effect of sleep related impairment on clinical pain and physical symptoms. Our findings extend our prior work demonstrating advanced brain aging among individuals with chronic pain and the mediating role of brain-aging on the association between sleep and pain severity. Future longitudinal studies are needed to further understand whether the brain can be a therapeutic target to reverse the possible effect of sleep problems on chronic pain.

Background: As part of a coordinated effort to expand our research activity at the interface of Aging and Energetics a team of investigators at The University of Alabama at Birmingham systematically assayed and catalogued the top research priorities identified in leading publications in that domain, believing the result would be useful to the scientific community at large. Objective: To identify research priorities and opportunities in the domain of aging and energetics as advocated in the 40 most cited papers related to aging and energetics in the last 4 years. Design: The investigators conducted a search for papers on aging and energetics in Scopus, ranked the resulting papers by number of times they were cited, and selected the ten most-cited papers in each of the four years that include 2010 to 2013, inclusive. Results: Ten research categories were identified from the 40 papers. These included: (1) Calorie restriction (CR) longevity response, (2) role of mTOR (mechanistic target of Rapamycin) and related factors in lifespan extension, (3) nutrient effects beyond energy (especially resveratrol, omega-3 fatty acids, and selected amino acids), 4) autophagy and increased longevity and health, (5) aging-associated predictors of chronic disease, (6) use and effects of mesenchymal stem cells (MSCs), (7) telomeres relative to aging and energetics, (8) accretion and effects of body fat, (9) the aging heart, and (10) mitochondria, reactive oxygen species, and cellular energetics. Conclusion: The field is rich with exciting opportunities to build upon our existing knowledge about the relations among aspects of aging and aspects of energetics and to better understand the mechanisms which connect them.

Background: Recent trends suggest that physicians will diagnose thousands of children in the United States with a brain or central nervous system tumor in 2020. Malignant brain tumors are difficult to treat, with low life expectancy rates in children and adults. Convection-enhanced delivery (CED) shows promise for the treatment of brain tumors, yet remains in clinical trials despite being developed more than 20 years ago. To advance CED to standard of care status and help improve survival rates, this study group developed a quantitative computer simulation model to determine and optimize therapy distribution in brain tumors based on the catheter infusion locations for CED. Methods: The simulations resulted in the identification of four infusion reference locations, which were used to conduct an optimization study to identify the optimal locations for CED. Patient-specific T1-weighted images and diffusion-weighted images provided information regarding tumor shape and size and the approximate rate at which therapy distributes at spatial locations within the tumor. Using the images, the researchers in this study developed a model which allowed the calculation of therapy distribution within the tumor while considering its permeability, porosity, and interstitial fluid pressure characteristics. We divided the tumor into regions and calculated distribution for four infusion locations per region. Using the location from each region with the highest volume distribution allowed our study group to conduct the response surface optimization. Results: Twelve optimal locations emerged from the optimization with volume percentage distributions ranging from 7.92% to 9.09%, compared to 2.87% to 6.32% coverage for non-optimal locations. This optimization method improved distribution from 27.80% to 45.95%, which may improve therapeutic value. Conclusions: Catheter placement appears to influence volume therapy distribution percentages. The selection of the highest percentages per region may provide optimal therapy for the entire tumor region.

Background: Convection-enhanced delivery (CED) of therapeutic agents to brain tumors allows clinicians to bypass the blood-brain barrier (BBB) to infuse virus therapy, biological, or chemotherapy directly into a brain tumor through convection. However, the effectiveness of infusions via CED may depend on catheter placement. Methods: This study used diffusion maps from magnetic resonance imaging (MRI) of human brain tumors and computational fluid dynamics (CFD) simulations to assess therapy volume distribution percentages based on catheter placement locations. Results: The primary outcome showed differences in volume distribution based on the catheter placement location. Total tumor volume filled ranged from 144.40 mm 3 to 317.98 mm 3. Percent filled of tumor volume ranged from 2.87% to 6.32%. Conclusions: The selection of the location for catheter placement using the region with the highest volume filled may provide optimal therapeutic effect. The researchers conclude that CFD may provide guidance for catheter placement in CED of therapeutic agents.
