Abstract

In dit onderzoek naar de gevolgen van een eventuele suikerbelasting worden twee hoofdvragen in de conclusie aan het eind beantwoord: 1. Is een suikerbelasting effectief voor het verminderen van de consumptie van suiker via voedingsmiddelen? 2. Hoe kan een belasting worden vormgegeven en geïmplementeerd die bijdraagt aan het verminderen van de consumptie van suiker via voedingsmiddelen?

Abstract

Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

4. PERCEIVED CHALLENGES IN BUSINESS DEVELOPMENT OF SMALLHOLDER DAIRY FARMERS IN THREE MALAWIAN REGIONS

Abstract

Dairy farmers face various challenges in developing their businesses. The current literature identifies several constraints towards a more efficient dairy sector that relate mostly to on-farm management practices. The available studies analyze constraints mostly from the objective viewpoint of the researcher, whereas very little is known about what farmers themselves perceive to be challenging. To better understand the farmers' perspective and what they perceive to be challenging, and how these challenges differ across regions, this paper builds on a survey of 529 dairy farmers in Malawi. In the survey, respondents were asked in an open question to name their three main challenges. Based on the responses, a three-level coding scheme was elaborated. Data were then coded by two researchers to ensure reliability of coding. Codes were then used to calculate relative frequencies for the different challenges and to perform chi-square tests to check for regional differences in frequencies. Results suggest that farmers perceive low milk price and milk yield, animal health, availability of feedstock, and the costs for drugs as the main challenges. The analysis also revealed that the challenges vary strongly between the three main regions in the country--Blantyre (south), Lilongwe (central), and Mzuzu (north). The perceived challenges reflect the different production systems in the regions. In Blantyre, dairy farming is low in intensity, and farmers thus perceive inputs, in particular the availability of fodder, as well as outputs, in particular milk yields more frequently as a challenge than farmers in Lilongwe and Mzuzu. In contrast, dairy farming in Lilongwe is the most intense. Accordingly, farmers in Lilongwe perceive costs, in particular for inputs such as mash and concentrates as major constraints. Farmers in Mzuzu, which is a rather remote region, consider a lack of assistance, in particular concerning extension services significantly more challenging than the farmers in the other two regions do. Considering these insights, development work and extensions services may be able to increase outcomes in the dairy sector by targeting the regional challenges. Abstract

The secure digitalisation of distributed workflows with different stakeholders (and trust relationships) using systems from different stakeholder domains is of increasing interest. Just one example is the workflow/policy area of student mobility. Others are from public administration and from economic sectors. According to the eIDAS regulation, eID and trust services (TS) are available across EU - upcoming also EUid & wallets (eIDAS 2.0) - to improve security aspects (providing interoperability or standards). We present some security enhancements to maintainhigher trustworthiness in Identity and Access Management (IAM) services for different policy areas with mandatory, owner-based and self-sovereign control aspects - based on eIDAS and different standards and the integration of views/results from deployed or ongoing projects (EMREX/ELMO, Europass/ EDCI, eIDAS, EUid, Verifiable Credentials, NBP initiative, OZG implementation, Self-Sovereign Identities SSI, RBAC, ABAC, DAC/MAC, IPv6) and a trustistor.

Abstract

In het algemeen zijn de klanten van Vitens erg tevreden over hun drinkwater. Toch zijn er verschillen. Het duidelijkst komt dit naar voren in de mate van kalkafzetting van het water. Hoe meer kalkafzetting klanten ervaren, hoe minder positief ze zijn over het drinkwater. Uit de klantonderzoeken is duidelijk naar voren gekomen dat investeren in waterkwaliteitsverbeteringen voor klantgerichte parameters leidt tot een hogere klanttevredenheid. Gerichte communicatiecampagnes rondom deze investeringsprojecten kunnen deze verbetering in klanttevredenheid ondersteunen.

14. Influence of milk yield on profitability a quantile regression analysis

Abstract

The paper analyses factors influencing the economic success of Swiss dairy farms, measured by annual income per family work unit, using panel-data regression techniques. Based on more than 5400 observations, the analysis focusses on annual milk yield per cow as key explanatory variable, adjustable by the farm manager in the medium term. We apply a random-effects model and a quantile regression based on deciles, which allows us to study the heterogeneity of the sample in more detail. Consistently with literature, the random-effects model shows a positive contribution of milk yield: an additional ton per cow results in an increase of CHF 2660, i.e. 6% of annual income. The quantile regression reveals that the impact of milk yield differs between deciles: a high milk yield is most beneficial for the best performing farms, accounting for up to 7210 CHF per ton. Our analysis further shows the influence of milk yield on profitability to be highly heterogeneous among Swiss dairy farms, indicating the demand for business-specific consulting services and not indicating the requirement for increased milk yield at each level of economic success. Abstract

Objectives/HypothesisPerform a systematic literature search to provide an overview of today's literature regarding the different aspects that can cause dysphagia after supracricoid laryngectomy. Study DesignA systematic literature review. Review MethodsThe inclusion criteria were laryngeal cancer, supracricoid laryngectomy, and swallowing. Thirty-one qualifying articles were included and analyzed describing swallowing after supracricoid laryngectomy. ResultsIncluded studies examined the incidence of dysphagia and discussed various factors that will or will not contribute to dysphagia after supracricoid laryngectomy, type of reconstruction, swallow training, radiation, arytenoid cartilage resection, extended procedures, and age. ConclusionA high incidence of dysphagia was reported after supracricoid laryngectomy. However, good recovery rates were observed with low incidence of severe complications. The included studies used different methods and standards to start oral intake, remove the nasogastric feeding tube, and observe swallow function. Homogenous study population and standardized guidelines on how to handle the pre- and postoperative course after supracricoid laryngectomy and how to measure swallow function could improve further research. Abstract

OBJECTIVE: We sought evidence of interaction between single-nucleotide polymorphisms (SNP) in the calcium-sensing receptor (CASR) gene and early life in determination of bone mineral density (BMD) among individuals from the Hertfordshire Cohort Study. METHODS: Four hundred ninety-eight men and 468 women aged 59-71 years were recruited. A lifestyle questionnaire was administered and BMD at lumbar spine and femoral neck was measured. DNA was obtained from whole blood samples using standard extraction techniques. Five SNP of the CASR gene termed CASRV1 (rs1801725, G-->T, S986A), CASRV2 (rs7614486, T-->G, untranslated), CASRV3 (rs4300957, untranslated), CASRV4 (rs3804592 G-->A, intron), and CASRV5 (rs1393189, T-->C, intron) were analyzed. RESULTS: Among women the 11 genotype of the CASRV3 SNP was associated with higher lumbar spine BMD within the lowest birthweight tertile, while the opposite pattern was observed among individuals in the highest birthweight tertile (test for interaction on 1 df, p = 0.005, adjusted for age, body mass index, physical activity, dietary calcium intake, cigarette and alcohol consumption, social class, menopausal status, and hormone replacement therapy use). Similar relationships were seen at the total femur (p = 0.042, fully adjusted) with birthweight and at the total femur according to weight at 1 year tertile among women (p < 0.001, fully adjusted). One haplotype was associated with lumbar spine BMD in women (p = 0.008, fully adjusted); these findings were replicated in a second cohort. CONCLUSION: We have found evidence of an interaction between a SNP of the CASR gene and birthweight in determination of bone mass in a UK female population.

Abstract

Dairy farmers face various challenges in developing their businesses. The current literature identifies several constraints towards a more efficient dairy sector that relate mostly to on-farm management practices. The available studies analyze constraints mostly from the objective viewpoint of the researcher, whereas very little is known about what farmers themselves perceive to be challenging. To better understand the farmers' perspective and what they perceive to be challenging, and how these challenges differ across regions, this paper builds on a survey of 529 dairy farmers in Malawi. In the survey, respondents were asked in an open question to name their three main challenges. Based on the responses, a three-level coding scheme was elaborated. Data were then coded by two researchers to ensure reliability of coding. Codes were then used to calculate relative frequencies for the different challenges and to perform chi-square tests to check for regional differences in frequencies. Results suggest that farmers perceive low milk price and milk yield, animal health, availability of feedstock, and the costs for drugs as the main challenges. The analysis also revealed that the challenges vary strongly between the three main regions in the country - Blantyre (south), Lilongwe (central), and Mzuzu (north). The perceived challenges reflect the different production systems in the regions. In Blantyre, dairy farming is low in intensity, and farmers thus perceive inputs, in particular the availability of fodder, as well as outputs, in particular milk yields more frequently as a challenge than farmers in Lilongwe and Mzuzu. In contrast, dairy farming in Lilongwe is the most intense. Accordingly, farmers in Lilongwe perceive costs, in particular for inputs such as mash and concentrates as major constraints. Farmers in Mzuzu, which is a rather remote region, consider a lack of assistance, in particular concerning extension services significantly more challenging than the farmers in the other two regions do. Abstract

This report analyses the potential impact of the EU CAP reforms that follow the Mid Term Review and the Harbinson Proposal for negotiation modalities in the WTO Doha Round on the Netherlands, EU14 and the accession countries. In welfare terms, the MTR has a rela-tively small impact on the Netherlands, the other member countries of the EU as well as the accession countries, while agricultural income decreases add up to around 2, 3 and 5 percent respectively. Under the Harbinson Proposal the Netherlands and the other EU member countries face losses of their agricultural incomes of 8 and 10 percent respec-tively, mainly due to reduced outputs in the cereals, oilseeds, and cattle sectors. The agricultural income in the accession countries is also decreasing. As a conclusion, the MTR does not make the European Agriculture compatible for the WTO Doha Round. The border protection measures, which are hardly affected in the MTR, still play a mayor role. Assumptions about the effects of decoupling and the difference between applied and bound tariff rates influence model results.

Abstract

Subclinical vitamin B-12 deficiency is common in the elderly. Encouraged by early indications, we investigated the plasma vitamin B-12 status in association with bone mineral content (BMC) and bone mineral density (BMD) in frail elderly people. Data of 194 free-living Dutch frail elderly (143 women and 51 men) were available. BMC and BMD were measured by dual energy X-ray analysis. Biochemical analyses were performed on plasma or serum including vitamin B-12, methylmalonic acid, homocysteine, 25-hydroxy vitamin D and parathyroid hormone. Women had higher plasma vitamin B-12 (288 and 238 pmol/L, respectively) and lower plasma homocysteine levels (15.8 and 21.3 μmol/L, respectively) than men. Of the total explained variance of BMC and BMD in women (46 and 22%, respectively), 1.3-3.1% was explained by plasma vitamin B-12, in addition to weight and height or energy intake. In men, the variance of BMC and BMD was explained by weight, smoking and/or height (total R2 was 53 and 25%, respectively), but not by plasma vitamin B-12. Osteoporosis occurred more often among women whose vitamin B-12 status was considered marginal or deficient than in women with a normal status, i.e., the prevalence odds ratios (after adjustment for weight, age and calcium intake) (95% confidence intervals) were 4.5 (0.8; 24.8) and 6.9 (1.2;39.4), respectively. These results suggest that vitamin B-12 status is associated with bone health in elderly women. Future studies on bone health should take into account a possible role of vitamin B-12 status in different populations. Abstract

Dit rapport verkent de gevolgen van een aantal mogelijke wijzigingen in het EU-suikerbeleid. Onderzocht zijn drie opties. De eerste optie behelst een volledige en wereld-wijde afschaffing van alle exportondersteunende en grensbeschermende maatregelen. De tweede optie richt zich op de gevolgen van een forse reductie met 20% van de EU-suikerquota. De derde en laatste optie verkent de gevolgen van een forse verlaging met 36% van de institutionele suikerprijzen. De opties worden doorgerekend voor de EU-15 en voor Nederland met behulp van twee economische modellen: GTAP en DRAM. Uit de si-mulaties van de beleidsalternatieven blijkt dat de consequenties van de drie opties voor zowel de Europese als de Nederlandse suikersector aanzienlijk zijn.
