The present research demonstrated that an integrated multi‐system based on the assays of lipid‐lowering and expectorant effects was used to screen quality markers of an edible and medical material—the blossom of Citrus aurantium L. var. amara Engl. (BCAVA)—and a portion of active constituents were quantified in multiple batches to provide scientific data to establish a quality standard for BCAVA. Mouse models were developed to evaluate the lipid‐lowering and expectorant effects, facilitating the investigation of medicinal parts through different polar extractions of BCAVA. Subsequently, ultra‐high‐performance liquid chromatography coupled with quadrupole time‐of‐flight mass spectrometry was utilized for the in vivo and in vitro identification of chemical profiles within the medicinal parts of BCAVA. This methodological approach led to the selection and quantification of several active compounds from 21 batches of BCAVA sourced from different geographical regions samples. Notably, the ethanol extract of BCAVA exhibited significant lipid‐lowering and expectorant effects while 183 compounds were identified in vitro and 109 in vivo, respectively. Then, five key ingredients were quantified, and the quantitative data were subjected to statistical analysis to discriminate between samples from various geographical regions. 旨在为动物源性与植物源性脂质复配保健品的研发提供参考，以高脂饮食诱导建立SD大鼠混合型高脂血症动物模型，揭示蛋黄磷脂与紫苏籽油复配对维持大鼠血脂健康水平的影响。将50只雄性SD大鼠随机分为空白对照组，模型对照组和蛋黄磷脂与紫苏籽油复配受试物低、中、高剂量组，分析大鼠的体质量、肝脏指数、血脂水平、体成分以及肝脏中脂滴含量。结果表明：给予蛋黄磷脂与紫苏籽油复配受试物45 d，各组大鼠的体质量无显著差异，但受试物中、高剂量组大鼠的肝脏指数，各剂量组大鼠的血清总胆固醇（TC）水平以及高剂量组大鼠血清的甘油三酯（TG）水平均显著低于模型对照组，且TC和TG水平的降低趋势与受试物剂量之间存在显著的剂量-效应关系，而各剂量组大鼠血清的高密度脂蛋白胆固醇（HDL-C）和低密度脂蛋白胆固醇（LDL-C）无显著变化；各剂量组大鼠体内的脂肪占比较模型对照组均下降，瘦体质量占比上升，但均无显著差异；各剂量组大鼠的肝脏脂滴含量较模型对照组均极显著降低。综上，蛋黄磷脂与紫苏籽油复配受试物有助于维持大鼠的血脂健康水平。In order to provide reference for the research and development of health care products with animal and plant derived lipids, the mixed hyperlipidemia animal model of SD rats was established by the high-fat diet, and the effects of the combination of egg yolk phospholipid and perilla seed oil on blood lipid health level was revealed. 50 Male SD rats were divided into blank control group, model control group, high, medium and low dose groups of combination of egg yolk phospholipid and perilla seed oil, and the changes of body weight, liver index, blood lipid level, body composition and liver lipid droplet content in rats were analyzed. The results showed that after 45 d of administration of the test substance, the body weight of the rats in each group did not change significantly, but the liver index of the medium and high dose groups, the TC levels in each dose group, and the TG levels in the high dose group were significantly lower than those in the model control group. There was a significant dose-effect relationship between the decreasing trend of TC and TG levels and the dose of the test substance, while there was no significant change in HDL-C and LDL-C levels in each dose group. The proportion of body fat in the body composition of rats in each dose group decreased compared with the model control group, and the proportion of lean body mass increased, but there was no significant difference between them. The content of liver lipid droplets in each dose group was very significantly lower than that in the model control group. In summary, the combination of egg yolk phospholipid and perilla seed oil is helpful in maintaining the healthy blood lipid level of rats.

• Despite many patients being on statins before optimisation most were under treated. • A pharmacist led service was implemented to optimise lipid lowering therapy. • Goal was secondary prevention target of non-HDL cholesterol < 97 mg/dl. • High intensity statin use increased from 39% before optimisation to 76% after review. • 41/216 (19%) of patients were at non-HDL target at baseline which improved to 92/137 (67%) after intervention. Patients attending vascular or diabetic foot clinics commonly have atherosclerotic disease, are at increased risk of cardiovascular disease (CVD), merit high-intensity lipid-modifying therapy to maintain secondary prevention targets and are often sub optimally treated in primary care. We set out to assess the impact of a pharmacist led lipid optimisation clinic in these patients in an area with high levels of social deprivation. We performed a clinical cohort study to assess the effectiveness of a pharmacist led clinic to optimise lipid lowering therapy by optimising of statin therapy and commencing additional lipid lowering therapy if applicable with monitoring of blood lipid profiles. Of the 216 patients (166 [77%] on statins) triaged by the pharmacist, 175 (81%) had non-high-density lipoprotein (non-HDL) cholesterol levels above the target value of 97 mg/dL (2.5 mmol/L) with a mean non-HDL cholesterol level of 135.73 mg/dL (3.51 mmol/L). Pre optimisation by the prescribing clinical pharmacist 41/216 (19%) patients were at target with a mean non-HDL cholesterol of 135.5 mg/dL improving to 92/137 (67%) patients achieving the target non-HDL cholesterol level with a mean post optimisation non-HDL cholesterol of 94.35 mg/dL (2.44 mmol/L), odds ratio (OR) for being at target 8.67 (95% CI 5.30–14.20). The calculated low-density lipoprotein cholesterol levels (Friedewald) demonstrated a mean reduction of 35.19 (95% CI 29.23–41.38) mg/dL (0.91 [95% CI 0.76–1.07] mmol/L). Proportion on high intensity statin increased from 65 out of 166 (39%) to 129 of 170 (76%) at follow up (OR 4.89 [3.06–7.82]), equivalent to an number needed to treat = 3. A pharmacist led service in undertreated and clinically challenging vascular and diabetic foot patients in an area of high social deprivation produced significant improvements in utilization of high intensity statin and other lipid lowering therapies and attainment of lipid goals. Lowing blood lipid levels with probiotics has good application prospects. This study aimed to isolate probiotics with hypolipidemic efficacy from homemade na dish and investigate their mechanism of action. In vitro experiments were conducted to determine the cholesterol-lowering ability of five isolates, with results showing that Lactiplantibacillus plantarum N4 exhibited a high cholesterol-lowering rate of 50.27% and significant resistance to acid (87%), bile salt (51.97%), and pepsin (88.28%) in simulated gastrointestinal fluids, indicating promising application prospects for the use of probiotics in lowering blood lipids. The findings from the in vivo experiment demonstrated that the administration of N4 effectively attenuated lipid droplet accumulation and inflammatory cell infiltration in the body weight and liver of hyperlipidemic rats, leading to restoration of liver tissue morphology and structure, as well as improvement in lipid and liver biochemical parameters. 16S analysis indicated that the oral administration of N4 led to significant alterations in the relative abundance of various genera, including Sutterella, Bacteroides, Clostridium, and Ruminococcus, in the gut microbiota of hyperlipidemia rats. Additionally, fecal metabolomic analysis identified a total of 78 metabolites following N4 intervention, with carboxylic acids and their derivatives being the predominant compounds detected. The transcriptomic analysis revealed 156 genes with differential expression following N4 intervention, leading to the identification of 171 metabolic pathways through Kyoto Encyclopedia of Genes and Genomes enrichment analysis. Notably, the glutathione metabolism pathway, PPAR signaling pathway, and bile secretion pathway emerged as the primary enrichment pathways. The findings from a comprehensive multi-omics analysis indicate that N4 influences lipid metabolism and diminishes lipid levels in hyperlipidemic rats through modulation of fumaric acid and γ-aminobutyric acid concentrations, as well as glutathione and other metabolic pathways in the intestinal tract, derived from both the gut microbiota and the host liver. This research offers valuable insights into the therapeutic potential of probiotics for managing lipid metabolism disorders and their utilization in the development of functional foods. There is a direct relationship between the duration and level of exposure to low density lipoprotein cholesterol (LDL-C) levels over one's lifespan and cardiovascular events. Early treatment to lower elevated LDL-C is crucial for better outcomes with multiple therapies currently available to reduce atherogenic lipoproteins. Statins remain the foundation of LDL-C lowering therapy as one of the most cost-effective drugs to reduce atherosclerotic events (ASCVD) and mortality. Nonetheless, LDL-driven goal attainment remains suboptimal globally, highlighting a considerable need for non-statin therapies to address residual risk related to statin intolerance, non-adherence, and inherited lipoprotein disorders. LDL-C lowering interventions beyond statins include ezetimibe, PCSK9 monoclonal antibodies, inclisiran and bempedoic acid with specific guideline recommendations as to when to consider each. For patients with homozygous familial hypercholesterolemia requiring more advanced therapy, lomitapide and evinacumab are available, providing mechanisms that are not LDL receptor dependent. Lipoprotein apheresis remains an effective option for clinical familial hypercholesterolemia as well as elevated lipoprotein (a). There are investigational therapies being explored to add to our current armamentarium including CETP inhibitors, a third-generation PCSK9 inhibitor (small recombinant fusion protein oral PCSK9 inhibitor) and gene editing which aims to directly restore or disrupt genes of interest at the DNA level. This article is a brief review of the pharmacotherapy options beyond statins for lowering LDL-C and their impact on ASCVD risk reduction. Our primary aim is to guide physicians on the role these therapies play in achieving appropriate LDL-C goals, with an algorithm of when to consider each based on efficacy, safety and outcomes.

IntroductionThis study aims to investigate the effects of Polygonatum fermented liquor (PFL) on improving lipid metabolism and oxidative stress in mice by regulating the gut microbiota.MethodsForty SPF-grade male Kunming mice were randomly divided into four groups: normal control group (NC), general liquor group (GC), fresh Polygonatum fermented liquor group (FPC), and nine-steam-nine-bask Polygonatum fermented liquor group (NPC). Each group was administered with sterile water, general liquor, fresh Polygonatum fermented liquor, and nine-steam-nine-bask Polygonatum fermented liquor, respectively, by gavage. The mice's liver, brain tissue, serum, and intestinal contents were collected. The indicators of oxidative stress in the liver, four blood lipid indicators, gamma-aminobutyric acid (GABA), and brain-derived neurotrophic factor (BDNF) levels in the brain tissue were measured, liver hematoxylin and eosin (HE) staining was performed, and the gut microbiota in the small intestine were analyzed using 16S rRNA second-generation sequencing technology.ResultsCompared with the NC group, the NPC group showed significantly increased liver glutathione peroxidase (GSH-Px) content in mice (p < 0.05), reduced number of lipid droplets in the liver cells, and increased GABA and BDNF content in the brain tissues. The NPC group regulated lipid metabolism by lowering low-density lipoprotein cholesterol (LDL-C) and increasing high-density lipoprotein cholesterol (HDL-C) content in the mouse serum. Gut microbiota analysis showed significant changes in the gut microbiota of mice in the FPC and NPC groups, with increased richness and species diversity. These two groups increased the abundance of beneficial bacteria such as Lactobacillus, unclassified Muribaculaceae, unclassified Bacilli, and uncultured Bacteroidales bacterium while reducing the abundance of harmful bacteria such as Candidatus Arthromitus, and Staphylococcus, with a particularly significant reduction in Staphylococcus (p < 0.05). It is speculated that the two types of PFL may exert lipid-lowering and antioxidant effects by modulating the abundance of these dominant bacteria. Further studies showed that various environmental factors are closely related to the dominant gut bacteria. Malondialdehyde (MDA) was significantly negatively correlated with Lactobacillus and unclassified Bacilli, superoxide dismutase (SOD) was significantly negatively correlated with Staphylococcus (p < 0.01) and significantly negatively correlated with Candidatus Arthromitus (p < 0.05), and HDL-C was significantly negatively correlated with Staphylococcus and Facklamia (p < 0.05).DiscussionThe two types of PFL chosen in this study may exert lipid-lowering and antioxidant effects by modulating the composition and function of the gut microbiota, providing guidance for the industrial application of Polygonatum.

The aim of this study is to evaluate the protective effect of Armillaria mellea polysaccharides (AMP) against ethanol-induced vascular endothelial cell injury in rats. Altogether 40 SD rats were randomly divided into 4 groups: normal control, injury, low- and high-dose AMP. The rats from all groups except the normal control group were gavaged with 40% ethanol at 10 mL/kg mb to induce endothelial cell injury. The rats in the low- and high-dose groups were gavaged with AMP at 100 and 400 mg/kg mb, respectively, while the remaining groups were given an equal volume of physiological saline. The administration period lasted for 4 weeks. The histopathological changes of carotid arteries were observed by hematoxylin-eosin (HE) staining. Serum triglycerides (TG), total cholesterol (T-CHO), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), endothelial nitric oxide synthase (eNOS), inducible nitric oxide synthase (iNOS), nitric oxide (NO), endothelin 1 (ET-1), superoxide dismutase (SOD), and malondialdehyde (MDA) levels were detected. In addition, the effects of different doses (100 and 400 μg/mL) of AMP on the levels of reactive oxygen species (ROS), mitochondrial transmembrane potential, apoptosis, and the expression of apoptosis-associated proteins such as B cell lymphoma 2 (Bcl-2), Bcl-2 associated X protein (Bax), and caspase-3 in human umbilical vein endothelial cell injured by 600 μmol/mL ethanol were investigated. Results: AMP attenuated ethanol-induced endothelial cell injury, decreased T-CHO, TG, LDL-C, iNOS, NO, ET-1, and MDA levels, and increased HDL-C and SOD activity. In vitro, AMP reduced cellular ROS levels, inhibited the ethanol-induced decrease in mitochondrial transmembrane potential and apoptosis, increased Bcl-2/Bax ratio and down-regulated the expression of cleaved caspase-3. Conclusion: AMP have a protective effect against ethanol-induced endothelial cell injury in rats, and the mechanism may be related to their lipid-lowering, antioxidant, and anti-apoptotic effects.

Patients with multiple cerebral vascular stenosis and aortic arch ulcer plaques have a significantly increased risk of recurrent ischemic stroke. This paper introduces a case of probucol combined with atorvastatin in the treatment of multiple cerebral vascular stenosis with aortic arch ulcer plaque. This acute ischemic stroke patient presented with mild neurological impairment. During his hospitalization, multiple cerebral vascular stenosis and aortic arch ulcer plaques were discovered. After 3 months of sequential dual antiplatelet therapy, aspirin therapy was continued and combined with probucol and atorvastatin throughout the course for enhanced lipid-lowering antioxidant therapy. After 25 months of follow-up, there was no recurrence of stroke in this patient, and all imaging and hematological indicators remained stable.

Lycopene is a kind of natural carotenoid that could achieve antioxidant, anticancer, lipid-lowering and immune-improving effects by up-regulating or down-regulating genes related to antioxidant, anti-cancer, lipid-lowering and immunity. Furthermore, lycopene is natural, pollution-free, and has no toxic side effects. The application of lycopene in animal production has shown that it could improve livestock production performance, slaughter performance, immunity, antioxidant capacity, intestinal health, and meat quality. Therefore, lycopene as a new type of feed additive, has broader application prospects in many antibiotic-forbidden environments. This article serves as a reference for the use of lycopene as a health feed additive in animal production by going over its physical and chemical characteristics, antioxidant, lipid-lowering, anticancer, and application in animal production. Liver disorders represent a significant global health burden, necessitating the exploration of innovative therapeutic interventions. Vaccaria pyramidata, a perennial plant with rhizomatous roots, has a rich history in traditional medicine, exhibiting diverse pharmacological activities. This study delved into the hepatoprotective potential of an ethanolic extract derived from Vaccaria pyramidata roots using a CCl4-induced liver injury model in Wistar albino rats. Pretreatment with the ethanolic extract of Vaccaria pyramidata roots demonstrated remarkable hepatoprotective effects, significantly attenuating CCl4-induced hepatotoxicity. Notably, the extract exhibited superior efficacy in reducing elevated serum enzyme levels, including serum glutamic oxaloacetic transaminase, serum glutamic pyruvic transaminase and alkaline phosphatase, compared to untreated toxic rats. Furthermore, administration of the extract led to a substantial decrease in direct bilirubin levels, highlighting its potential in ameliorating liver dysfunction. Intriguingly, the ethanolic extract of Vaccaria pyramidata roots exhibited potent antioxidant activity, as evidenced by its ability to scavenge free radicals in DPPH and ABTS assays, along with a high total flavonoid content. Additionally, the extract demonstrated efficacy in reducing triglyceride levels, suggesting potential lipidlowering effects. These findings underscore the promising therapeutic potential of Vaccaria pyramidata root ethanolic extract as a novel hepatoprotective agent. Lycium barbarum contains a variety of phytochemicals, primarily polysaccharides, and other potent compounds. L. barbarum is a plant that has several uses in contemporary society, including food processing and everyday eating. However, through traditional processing methods the medicinal or edible value of L. barbarum cannot be fully exploited, to further improve the utilisation rate of L. barbarum and enrich the variety of L. barbarum market. The L. barbarum compound beverage, which uses L. barbarum as its primary ingredient, was created through a series of trials, including the creation of tea beverage recipes and research on the most suitable content of stabilisers, flavouring agent formulations and so on. In addition, several validation tests, including those involving hypoglycaemic and lipid-lowering were conducted. The results indicated that 0.1 g mL−1 stabiliser CMC-Na, 0.2 g mL−1 erythritol, 3 g mL−1 citric acid, 8% L. barbarum juice, and 6% chrysanthemum tea cold brew extract were the best combinations for food additive formulations. Additionally, compound beverages have hypoglycemic and lipid-lowering effects. In conclusion, the research and development of L. barbarum compound beverages can not only increase the rate at which L. barbarum is utilised but also offer fresh ideas for the research and development of L. barbarum as they transition from cash crop agricultural products to market-oriented standardised processed products. Obesity has become a serious global public health risk threatening millions of people. In this study, the astaxanthin–anthocyanin nanoparticles (AXT-ACN NPs) were used to investigate their effects on the lipid accumulation and antioxidative capacity of the high-sugar-diet-induced high-fat Caenorhabditis elegans (C. elegans). It can be found that the lifespan, motility, and reproductive capacity of the high-fat C. elegans were significantly decreased compared to the normal nematodes in the control group. However, treatment of high-fat C. elegans with AXT-ACN NPs resulted in a prolonged lifespan of 35 days, improved motility, and a 22.06% increase in total spawn production of the nematodes. Furthermore, AXT-ACN NPs were found to effectively extend the lifespan of high-fat C. elegans under heat and oxidative stress conditions. Oil-red O staining results also demonstrated that AXT-ACN NPs have a remarkable effect on reducing the fat accumulation in nematodes, compared with pure astaxanthin and anthocyanin nanoparticles. Additionally, AXT-ACN NPs can significantly decrease the accumulation of lipofuscin and the level of reactive oxygen species (ROS). The activities of antioxidant-related enzymes in nematodes were further measured, which revealed that the AXT-ACN NPs could increase the activities of catalase (CAT), superoxidase dismutase (SOD), and glutathione peroxidase (GSH-Px), and decrease the malondialdehyde (MDA) content. IntroductionThe incidence of metabolic disorders during pregnancy is increasing year by year, with diseases including hypertension and hyperlipidemia. Statins are the primary drugs for treating hyperlipidemia or atherosclerosis, yet some patients remain unresponsive to them, and pregnant women are prohibited from taking statins. Curculigoside is the major biologically active natural product present in Curculigo orchioides.MethodsIn this study, A high-fat mice model was developed to study the lipid-lowering effect of curculigoside. Using intestinal Caco-2 cell monolayer, the curculigoside transport properties at two temperatures and possible transporters were systemically studied.ResultsCurculigoside at concentrations used during the experiments have no toxic effect to Caco-2 cells. The curculigoside transfer from the apical to the basolateral side was strongly influenced by temperature. P-glycoprotein, breast cancer resistance protein, and efflux transporters are crucial components of the human intestinal cell line Caco-2. The curculigoside can significantly affect the contents of total cholesterol, triglycerides, high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol in mice.DiscussionThe transport properties and potential mechanism of curculigoside offer valuable insights for the design of development of hypolipidemic drugs like anti-atherosclerotic drugs and also be helpful to the further study of the pharmacological activity of curculigoside.

In the last few decades, atherosclerotic cardiovascular disease (ASCVD) risk has decreased dramatically among individuals affected by familial hypercholesterolaemia (FH) as a result of the early initiation of statin treatment in childhood. Contemporaneously important improvements in care for people with diabetes have also been made, such as the prevention of mortality from acute diabetic complications. However, individuals with type 1 diabetes still have a two to eight times higher risk of death than the general population. In the last 20 years, a few landmark studies on excess mortality in people with type 1 diabetes, in particular young adults, have been published. Although these studies were carried out in different populations, all reached the same conclusion: individuals with type 1 diabetes have a pronounced increased risk of ASCVD. In this review, we address the role of lipid abnormalities in the development of ASCVD in type 1 diabetes and FH. Although type 1 diabetes and FH are different diseases, lessons could be learned from the early initiation of statins in children with FH, which may provide a rationale for more stringent control of dyslipidaemia in children with type 1 diabetes. Aging and obesity make humans more prone to cardiovascular and metabolic syndrome diseases, leading to several serious health conditions, including hyperlipidemia, high blood pressure, and sleep disturbance. This study aimed to explore the hypolipidemic effect of fermented citrus lemon juice using a hyperlipidemic hamster model. The sugar-free lemon juice's fermentation was optimized, and the characteristics of fresh and fermented lemon juice (FLJ) were evaluated and compared, which contained polyphenols and superoxide dismutase-like activity. Results showed that the absorption and utilization efficiency of FLJ was higher compared with the unfermented lemon juice. This study's prefermentation efficiency evaluation found that 21–30 days of bacterial DMS32004 and DMS32005 fermentation of fresh lemon juice provided the best fermentation benefits, and 21-day FLJ was applied as a remedy after the efficiency compassion. After six weeks of feeding, the total cholesterol (TC) and triglyceride (TG) values in the blood and liver of the FLJ treatment groups were decreased compared with the high-fat diet (HFD) group. In addition, the blood low-density lipoprotein cholesterol (LDL-C) levels were significantly reduced in the FLJ treatment groups compared with the HFD group. In contrast, the blood high-density lipoprotein (HDL-C) to LDL-C ratio increased considerably in the FLJ treatment groups, and the total to HDL ratio was significantly lower than in the HFD group. Compared with the HFD group, the TC content in the FLJ treatment groups' feces increased significantly. This study demonstrated that the sugar-free fermentation method and fermentation cycle management provided FLJ with the potential to regulate blood lipids. Purpose of Review: To discuss and review the technical considerations, fundamentals, and guideline-based indications for coronary artery calcium scoring, and the use of other non-invasive imaging modalities, such as extra-coronary calcification in cardiovascular risk prediction. Recent Findings: The most robust evidence for the use of CAC scoring is in select individuals, 40–75 years of age, at borderline to intermediate 10-year ASCVD risk. Recent US recommendations support the use of CAC scoring in varying clinical scenarios. First, in adults with very high CAC scores (CAC ≥ 1000), the use of high-intensity statin therapy and, if necessary, guideline-based add-on LDL-C lowering therapies (ezetimibe, PCSK9-inhibitors) to achieve a ≥ 50% reduction in LDL-C and optimally an LDL-C < 70 mg/dL is recommended. In patients with a CAC score ≥ 100 at low risk of bleeding, the benefits of aspirin use may outweigh the risk of bleeding. Other applications of CAC scoring include risk estimation on non-contrast CT scans of the chest, risk prediction in younger patients (< 40 years of age), its value as a gatekeeper for the decision to perform nuclear stress testing, and to aid in risk stratification in patients presenting with low-risk chest pain. There is a correlation between extra-coronary calcification (e.g., breast arterial calcification, aortic calcification, and aortic valve calcification) and incident ASCVD events. However, its role in informing lipid management remains unclear. Summary: Identification of coronary calcium in selected patients is the single best non-invasive imaging modality to identify future ASCVD risk and inform lipid-lowering therapy decision-making. Atherosclerosis, the key pathogenesis of cardiovascular disease, is a leading cause of death and disability worldwide. Statins are first-line lipid-lowering drugs, which have been demonstrated to be powerful agents for anti-atherosclerosis. Numerous studies have confirmed the cardiovascular benefits and long-term safety of statins in a wide range of patients. Statins play an indispensable and irreplaceable part in the prevention and treatment of atherosclerotic cardiovascular disease (ASCVD). In this article, we summarize the evolution of statins and their role in the treatment of cholesterol. The anti-atherosclerotic mechanism of statins, its efficacy, safety and clinical outcomes in secondary and primary prevention of ACSVD in different patient populations, the combination treatment effects, and guideline recommendations are also detailed. Mulberry leaf has potential in lipid-lowering. Here, we compared the chemical composition and lipid-lowering effects of different mulberry leaf-derived teas, and investigated their underlying gut microbiome-related mechanisms. The main components of mulberry leaf-derived tea were flavonoids, phenolic acids and alkaloids, with green tea (0.60% polyphenols, 3.35% polysaccharides, 0.20% DNJ) containing higher levels than black tea (0.40% polyphenols, 2.95% polysaccharides, 0.15% DNJ). Accordingly, the lipid-lowering efficacy of green tea was superior to that of black tea in high-fat (HFD)-induced hyperlipidemic mice. Black tea increased 2 genera and 130 metabolites, decreased 6 genera and 90 metabolites, while green tea increased 5 genera and 127 metabolites, decreased 15 genera and 64 metabolites. Correlation analysis showed that the alterations of gut microbiota and metabolic profiles caused by mulberry leaf tea were tightly associated with its lipid-lowering effect. Altogether, this study provided a promising functional food targeting gut microbiota for the management of lipid metabolism.

Quercetin is one of the most common flavonoids. More and more studies have found that quercetin has great potential utilization value in cardiovascular diseases (CVD), such as antioxidant, antiplatelet aggregation, antibacterial, cholesterol lowering, endothelial cell protection, etc. However, the medicinal value of quercetin is mostly limited to animal models and preclinical studies. Due to the complexity of the human body and functional structure compared to animals, more research is needed to explore whether quercetin has the same mechanism of action and pharmacological value as animal experiments. In order to systematically understand the clinical application value of quercetin, this article reviews the research progress of quercetin in CVD, including preclinical and clinical studies. We will focus on the relationship between quercetin and common CVD, such as atherosclerosis, myocardial infarction, ischemia reperfusion injury, heart failure, hypertension and arrhythmia, etc. By elaborating on the pathophysiological mechanism and clinical application research progress of quercetin's protective effect on CVD, data support is provided for the transformation of quercetin from laboratory to clinical application.

Berberine possesses a wide spectrum of lipid regulation, and yet it has poor physicochemical property and cytotoxicity as a drug candidate. In order to alleviate the problems, a total of twenty-one 9-O-cinnamoylberberines and twenty 9-O-cinnamoyltetrahydroberberines were designed, synthesized, and evaluated by in vitro cell viability experiment and four classical lipid-lowering assays involving with total cholesterol, triglyceride, low density lipoprotein cholesterol, and high density lipoprotein cholesterol. A structure–activity relationship study of these compounds resulted in the discovery of two promising candidate molecules (5p and 7u). Compound 5p displayed the most potent inhibitory effect for TG formation, with the inhibitory rates of 40.5% and 76.8% in 3T3-L1 cells and HepG2 cells, respectively. Compound 7u exhibited the most promoting activity for the production of HDLC, with the increasing rates of 52.6% and 70.5% in both models, respectively. Purpose: Low-density lipoprotein cholesterol (LDL-C) recommendations differ between the 2018 American College of Cardiology/American Heart Association (ACC/AHA) and 2019 European Society of Cardiology/European Atherosclerosis Society (ESC/EAS) guidelines for patients with atherosclerotic cardiovascular disease (ASCVD) (< 70 vs. < 55 mg/dl, respectively). In the DA VINCI study, residual cardiovascular risk was predicted in ASCVD patients. The extent to which relative and absolute risk might be lowered by achieving ACC/AHA versus ESC/EAS LDL-C recommended approaches was simulated. Methods: DA VINCI was a cross-sectional observational study of patients prescribed lipid-lowering therapy (LLT) across 18 European countries. Ten-year cardiovascular risk (CVR) was predicted among ASCVD patients receiving stabilized LLT. For patients with LDL-C ≥ 70 mg/dl, the absolute LDL-C reduction required to achieve an LDL-C of < 70 or < 55 mg/dl (LDL-C of 69 or 54 mg/dl, respectively) was calculated. Relative and absolute risk reductions (RRRs and ARRs) were simulated. Results: Of the 2039 patients, 61% did not achieve LDL-C < 70 mg/dl. For patients with LDL-C ≥ 70 mg/dl, median (interquartile range) baseline LDL-C and 10-year CVR were 93 (81–115) mg/dl and 32% (25–43%), respectively. Median LDL-C reductions of 24 (12–46) and 39 (27–91) mg/dl were needed to achieve an LDL-C of 69 and 54 mg/dl, respectively. Attaining ACC/AHA or ESC/EAS goals resulted in simulated RRRs of 14% (7–25%) and 22% (15–32%), respectively, and ARRs of 4% (2–7%) and 6% (4–9%), respectively. Conclusion: In ASCVD patients, achieving ESC/EAS LDL-C goals could result in a 2% additional ARR over 10 years versus the ACC/AHA approach. [ABSTRACT FROM AUTHOR]

Background Dyslipidemia is closely related to the occurrence of acute myocardial infarction (AMI) and affects the prognosis of patients. Objective: To explore the lipid-lowering effect of compound seabuckthorn concentrate in hyperlipidemia model rats. Methods: Sixty Wistar rats were randomly divided into blank-control group, high-fat diet group, positive control group (lovastatin capsules 1.80 mg/kg·bw), compound seabuckthorn concentrate low (3.57 mL/kg·bw), medium (7.14 mL/kg·bw), high (14.28 mL/kg·bw) dose groups. This research aimed to investigate the hypolipidemic and antioxidant effects of fermented rice buckwheat. A high-fat model of Caenorhabditis elegans (C. elegans) was established by high sugar (10 mmol/L) induction. After the treatment of fermented rice buckwheat, its effects on the longevity, motility, egg production, reactive oxygen species (ROS) levels, resistance to oxidative stress, and antioxidant enzyme activity of high-fat C. elegans were determined. At the same time, its effect on the triglyceride and free fatty acid content in high-fat C. elegans were examined. Obesity has become a serious global public health risk threatening millions of people. In this study, the astaxanthin–anthocyanin nanoparticles (AXT-ACN NPs) were used to investigate their effects on the lipid accumulation and antioxidative capacity of the high-sugar-diet-induced high-fat Caenorhabditis elegans (C. elegans). It can be found that the lifespan, motility, and reproductive capacity of the high-fat C. elegans were significantly decreased compared to the normal nematodes in the control group. However, treatment of high-fat C. elegans with AXT-ACN NPs resulted in a prolonged lifespan of 35 days, improved motility, and a 22.06% increase in total spawn production of the nematodes. Furthermore, AXT-ACN NPs were found to effectively extend the lifespan of high-fat C. elegans under heat and oxidative stress conditions. Oil-red O staining results also demonstrated that AXT-ACN NPs have a remarkable effect on reducing the fat accumulation in nematodes, compared with pure astaxanthin and anthocyanin nanoparticles. Additionally, AXT-ACN NPs can significantly decrease the accumulation of lipofuscin and the level of reactive oxygen species (ROS). The activities of antioxidant-related enzymes in nematodes were further measured, which revealed that the AXT-ACN NPs could increase the activities of catalase (CAT), superoxidase dismutase (SOD), and glutathione peroxidase (GSH-Px), and decrease the malondialdehyde (MDA) content. The astaxanthin and anthocyanin in AXT-ACN NPs showed sound synergistic antioxidation and lipid-lowering effects, making them potential components in functional foods.

Summary: Background: The impact of the stepwise implementation of the 2019 European Society of Cardiology (ESC)/European Atherosclerosis Society (EAS) treatment algorithm on low-density lipoprotein cholesterol (LDL-C) goal attainment was simulated in patients from the DA VINCI study. Methods: Monte Carlo simulation was used to evaluate treatment optimisation scenarios, based on a patient's risk category: statin intensification (step 1), addition of ezetimibe (step 2), and addition of a proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor (step 3). Residual cardiovascular risk and predicted relative and absolute risk reduction (RRR and ARR) in cardiovascular events were assessed. Findings: In DA VINCI, 2482 patients did not achieve their 2019 ESC/EAS LDL-C goals and were included in the simulation. In patients without atherosclerotic cardiovascular disease (ASCVD) (n = 962), 27.0% (n = 259) and 57.0% (n = 548) are likely to achieve their LDL-C goals at step 1 and step 2, respectively. Of those at very high risk without ASCVD (n = 74), 88.1% (n = 65) are likely to achieve their LDL-C goals at step 3. In patients with ASCVD (n = 1520), 12.0% (n = 183), 42.1% (n = 641) and 93.2% (n = 1416) are likely to achieve their LDL-C goals at steps 1, 2 and 3, respectively. In patients with and without ASCVD, treatment optimisation may result in mean simulated RRR of 24.0% and 17.7%, respectively, and ARR of 8.1% and 2.6%, respectively. Interpretation: Most patients at high cardiovascular risk are unlikely to achieve LDL-C goals through statin optimisation and ezetimibe, and will require a PCSK9 inhibitor, leading to greater reduction in cardiovascular risk. The management of low-density lipoprotein cholesterol (LDL-C) levels is a central strategy for the prevention of atherosclerotic cardiovascular disease. Current United States (2018 American Heart Association/American College of Cardiology/Multisociety) and European (2019 European Society of Cardiology/European Atherosclerosis Society) guidelines endorse statin therapy as the first-line therapy for pharmacologic LDL-C lowering. However, in clinical practice up to 30% of patients report partial or complete intolerance to statin therapy. While the nocebo effect with statins is well described, perceived statin intolerance prevents many patients from achieving LDL-C thresholds associated with clinical benefit. Bempedoic acid is a novel, oral, non-statin lipid-lowering therapy that works by inhibiting adenosine triphosphate-citrate lyase, an enzymatic reaction upstream of 3-hydroxy-3-methylglutaryl coenzyme A reductase in the hepatic cholesterol synthesis pathway. Bempedoic acid confers reduction in LDL-C of ~18% on background statin therapy, ~21% in patients with statin intolerance, and ~38% when given in fixed-dose combination with ezetimibe. The CLEAR Outcomes trial, which enrolled high-risk primary and secondary prevention patients with reported statin intolerance and LDL-C levels ≥100 mg/dL, showed that bempedoic acid compared with placebo reduced 4-component major adverse cardiovascular events (MACE) by 13% (hazard ratio 0.87, 95% confidence interval 0.79-0.96). Bempedoic acid also reduced 3-component MACE by 15%, myocardial infarction by 23% and coronary revascularization by 19%. The benefit was even greater in the primary prevention cohort (hazard ratio 0.70, 95% confidence interval 0.55-0.89) for 4-component MACE. Bempedoic acid was associated with increases in uric acid levels and cholelithiasis, but numerically fewer events of myalgia and new-onset diabetes. These findings confirm that bempedoic acid is an effective approach to reduce cardiovascular outcomes in high-risk patients with statin intolerance who require further reduction in LDL-C. Background: Ezetimibe has been recommended to add on statin therapy when maximally tolerated statin cannot achieve low-density lipoprotein cholesterol (LDL-C) target. However, the safety and efficacy of generic formulation of ezetimibe has rarely been reported. Objective: To examine efficacy of branded generic ezetimibe in patients with high cardiovascular risk receiving high-potency statin. Materials and Methods: The present study was a prospective cohort study in adult patients with high cardiovascular risk receiving atorvastatin 40 mg/day, but the LDL-C level was still higher than 70 mg/dL. Branded generic ezetimibe 10 mg once daily (MiBEAZ™) was added on the high-potency statin therapy as a combination treatment. Lipid parameters were evaluated three months after ezetimibe treatment. Results: Of the 61 patients, mean age was 62.4±11.5 years, and 33 (54.1%), were male. Coronary artery disease was reported in 58 patients 95.1%. The median baseline of LDL-C was 95.0 mg/dL (IQR 79.5, 108.5). At 3-month follow-up, total cholesterol and LDL-C levels were significantly decreased by branded generic ezetimibe add-on therapy. The relative change of LDL-C reduction was 26.3% (IQR -38.8, -11.1, p<0.001). The LDL-target under 70 mg/dL was attained in 60% of the patients. Triglyceride and high-density lipoprotein cholesterol levels remained unchanged. After ezetimibe medication, there were no differences in renal or hepatic function. Conclusion: The authors demonstrated that branded generic ezetimibe significantly reduced levels of LDL-C in high cardiovascular risk patients with uncontrolled LDL-C level despite the high-potency statin treatment. Background: To assess the causal role of lipid traits and lipid-lowering agents in inflammatory bowel disease (IBD). Methods: Univariable mendelian randomization (MR) and multivariable MR (MVMR) analyses were conducted to evaluate the causal association between low-density lipoprotein cholesterol (LDL-C), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C) and IBD. Drug-targeted MR analyzed the effects of lipid-lowering drugs on IBD, and network MR was used to analyze potential mediation effects. Results: The levels of HDL-C had an inverse relationship with the risk of Crohn's disease (CD, OR: 0.85, 95% CI: 0.73-0.98, P = 0.024). In MVMR, the inverse relationships were found in all three outcomes. Drug-targeted MR analyses showed that with one-SD LDL-C decrease predicted by variants at or near proprotein convertase subtilisin/kexin type 9 (PCSK9), the OR values of people diagnosed with IBD, ulcerative colitis (UC) and CD were 1.75 (95%CI: 1.13-2.69, P = 0.011), 2.1 (95%CI: 1.28-3.42, P = 0.003) and 2.24 (95%CI: 1.11-4.5, P = 0.024), respectively. With one-SD LDL-C decrease predicted by variants at or near cholesteryl ester transfer protein (CETP), the OR value of people diagnosed with CD was 0.12 (95%CI: 0.03-0.51, P = 0.004). Network-MR showed that HDLC mediated the causal pathway from variants at or near CETP to CD. Conclusion: Our study suggested a causal association between HDL-C and IBD, UC and CD. Genetically proxied inhibition of PCSK9 increased the risk of IBD, UC and CD, while inhibition of CETP decreased the risk of CD. Eurotium cristatum had important application value in the development of hypoglycemic and lipid-lowering functional foods and natural medicines. The analysis of the overall metabolite information and the hypoglycemic and lipid-lowering active ingredients produced by Eurotium cristatum could provide theoretical basis and data support for the development and application of the bacterial strain. By taking the strain Ys-Ec-01 of Eurotium cristatum as the experimental material, the non-targeted metabolomics based on the ultra performance liquid chromatography-quadrupole electrostatic field orbitrap mass spectrometry (UHPLC-QE-MS) technology was used to analyze the fermentation metabolic profile Ys-Ec-01 by combining with the principal component analysis (PCA) and the orthogonal partial least squares-discriminant analysis (OPLS-DA), in order to find the hypoglycemic and lipid-lowering active products. The results showed that the metabolite profile of Ys-Ec-01 changed significantly after the fermentation. From the metabolite profile, a total of 365 fermentation products were identified, and then 205 differential fermentation products and 151 main fermentation products were screened, thus to identify 13 hypoglycemic and lipid-lowering active ingredients, which were chrysoeriol, kaempferol, isorhamnetin, epicatechin, capsaicin, nicotinic acid, trigonelline, 1-deoxynojirimycin, allantoin, protocatechuic acid, linoleic acid, choline and L-carnitine. Objective: This study focused on exploring the in vitro lipid-lowering efficacy of Puerarin lobata extract. Methods: The adsorption ratios of oil, cholesterol, sodium glycocholate and sodium taurocholate were evaluated by a multi index comprehensive scoring method to optimize the P. lobata alcohol extraction process using Box-Behnken response surface methodology with extraction time, extraction times and ethanol concentration as influencing factors. In vitro lipid-lowering activity was further validated by comparing the levels of TG and TC accumulation and the extent of intracellular lipid droplet accumulation in a model of human hepatoma (HepG2) fatty liver cells induced by palmitic acid (PA). Results: The optimal alcohol extraction process of P. lobata was 70% by volume of ethanol, 1 h of extraction time and 3 times of extraction time; P. lobata extract could obviously reduce the contents of TG and TC in cells and effectively reduce the intracellular lipid droplet aggregation. Conclusion: The optimized P. lobata extraction process was stable and reliable, manipulable, and showed good lipid-lowering activity in vitro.

Abstract Dyslipidemia is an independent risk factor of cardiovascular diseases (CVDs), which lead to the high mortality, disability, and medical expenses in the worldwide. Based on the previous researches, the improvement of dyslipidemia could efficiently prevent the occurrence and progress of cardiovascular diseases. Medicinal and edible plants (MEPs) are the characteristics of Chinese medicine, and could be employed for the disease treatment and health care mostly due to their homology of medicine and food. Compared to the lipid-lowering drugs with many adverse effects, such as rhabdomyolysis and impaired liver function, MEPs exhibit the great potential in the treatment of dyslipidemia with high efficiency, good tolerance and commercial value. In this review, we would like to introduce 20 kinds of MEPs with lipid-lowering effect in the following aspects, including the source, function, active component, target and underlying mechanism, which may provide inspiration for the development of new prescription, functional food and complementary therapy for dyslipidemia.

The lipid-lowering effects of a water extract of Jasminum grandiflorum L. (WE) and its chemical components on 3T3-L1 preadipocytes was investigated. To determine the inhibitory effects of J. grandiflorum samples on lipid droplet development, oil red O staining was used to quantify and detect triglyceride (TG) contents following the induction of 3T3-L1 preadipocytes to produce mature adipocytes. Liquid chromatography-mass spectrometry was used to analyze the chemical components in the water extract of J. grandiflorum, an MTT assay was used to determine the effects of J. grandiflorum and its components on the proliferation of 3T3-L1 preadipocytes, and western blotting was performed to determine the expression of AMPK, C/EBPα, FAS, ACC, CPT1, Bax, and Bcl-2 proteins. The results revealed that the WE can significantly inhibit the formation of lipid droplets in a dose-dependent manner. A high concentration of WE (500 µg/mL) was found to reduce the contents of neutral lipids and TG by 23.59% and 30.20%, respectively. These effects were found to be most significant during the early stage of fat accumulation, and it was established that the active constituent contributing most to this lipid-lowering effect was oleuropein, with a content of 13.77%. Furthermore, WE and oleuropein were shown to activate the phosphorylation of AMPK (123.19% and 115.98%, respectively) and its downstream target ACC (451.06% and 1 050.0%), and to downregulate the protein expression of its downstream target FAS (81.72% and 60.50%). This had the effect of reducing adipogenesis and increasing the expression of CPT1 (164.84% and 292.19%), and accelerating fatty acid oxidation. Moreover, it inhibited the expression of C/EBPα (68.97% and 34.57%), thereby affecting the differentiation of 3T3-L1 cells, and also upregulated the expression of Bax (154.60% and 139.37%) and downregulated the expression of Bcl-2 (41.10% and 45.62%), thus inducing 3T3-L1 cell apoptosis. These findings indicate that a WE of J. grandiflorum can inhibit cell growth, differentiation, and lipid synthesis, and promote fatty acid oxidation during the early stage of 3T3-L1 cell differentiation, thereby effectively inhibiting lipid accumulation, the underlying mechanism of which may involve the regulation of AMPK and Bax/Bcl-2 pathways. Lipid-lowering is one of the most effective methods of prevention and treatment for cardiovascular diseases. However, most clinical lipid-lowering drugs have adverse effects and cannot achieve the desired efficacy in some complex hyperlipidemia patients, so it is of great significance to develop safe and effective novel lipid-lowering drugs. In the course of our project aimed at discovering the chemical novelty and bioactive natural products of marine-derived actinomycetes, we found that the organic crude extracts (OCEs) of Nocardiopsis sp. ZHD001 exhibited strong in vivo efficacies in reducing weight gain, lowering LDL-C, TC, and TG levels, and improving HDL-C levels in high-fat-diet-fed mice models. Chemical investigations of the active OCEs led to identifying two new sphydrofuran-derived compounds (1–2) and one known 2-methyl-4-(1-glycerol)-furan (3). Their structures were elucidated by the analysis of HRESIMS, 1D and 2D NMR spectroscopic data, and ECD calculations. Among these compounds, compound 1 represents a novel rearranged sphydrofuran-derived derivative. Bioactivity evaluations of these pure compounds showed that all the compounds exhibited significant lipid-lowering activity with lower cytotoxicity in vitro compared to simvastatin. Aging and obesity make humans more prone to cardiovascular and metabolic syndrome diseases, leading to several serious health conditions, including hyperlipidemia, high blood pressure, and sleep disturbance. This study aimed to explore the hypolipidemic effect of fermented citrus lemon juice using a hyperlipidemic hamster model. The sugar-free lemon juice’s fermentation was optimized, and the characteristics of fresh and fermented lemon juice (FLJ) were evaluated and compared, which contained polyphenols and superoxide dismutase-like activity. Results showed that the absorption and utilization efficiency of FLJ was higher compared with the unfermented lemon juice. This study’s prefermentation efficiency evaluation found that 21–30 days of bacterial DMS32004 and DMS32005 fermentation of fresh lemon juice provided the best fermentation benefits, and 21-day FLJ was applied as a remedy after the efficiency compassion. After six weeks of feeding, the total cholesterol (TC) and triglyceride (TG) values in the blood and liver of the FLJ treatment groups were decreased compared with the high-fat diet (HFD) group. In addition, the blood low-density lipoprotein cholesterol (LDL-C) levels were significantly reduced in the FLJ treatment groups compared with the HFD group. In contrast, the blood high-density lipoprotein (HDL-C) to LDL-C ratio increased considerably in the FLJ treatment groups, and the total to HDL ratio was significantly lower than in the HFD group. Compared with the HFD group, the TC content in the FLJ treatment groups’ feces increased significantly. This study demonstrated that the sugar-free fermentation method and fermentation cycle management provided FLJ with the potential to regulate blood lipids. Further research and verification will be carried out to isolate specific substances from the FLJ and identify their mechanisms of action.

Aim: This online interactive survey investigated lipid-lowering approaches of French cardiologists in highand very high-cardiovascular risk patients with hypercholesterolemia. Materials & methods: Physicians assessed three hypothetical patients at three clinic visits, and selected the patients’ cardiovascular risk category, target low-density lipoprotein cholesterol (LDL-C) and treatment. Results: A total of 162 physicians completed 480 risk assessments; 58% of assessments correctly categorized the hypothetical patients. Most physicians chose the correct LDL-C target for one of the very high-risk patients, but higherthan- recommended targets were selected for the other very high-risk patient and the high-risk patient. Statins were the most commonly chosen treatment. Conclusion: French cardiologists often underestimate cardiovascular risk in patients with hypercholesterolemia, select a higher-than-recommended LDL-C target and prescribe less intensive treatment than that recommended by guidelines.

Jellyfish peptides are of significant interest, due to their potential application as a source of bioactive peptides in pharmaceutical domains. Here, we investigated the action of Nemopilema nomurai peptides (NNPs) on lipids reduction in 3 T3-L1 adipocytes. We demonstrated for the first time that NNPs prepared by papain enzymolysis were able to reduce lipid accumulation in 3 T3-L1cells. Differentially expressed metabolites among control group, MDI group, GW9662 treatment group, and NNPs treatment group were identified using non-targeted liquid chromatography-mass spectrometry (LC-MS)-based metabolomics. A total of 105 differentially expressed metabolites were identified between MDI group and NNPs treatment group. These metabolites were predominantly enriched in amino acid, carbohydrate and lipid metabolism. Metabolomics analysis indicated that NNPs leads to imbalance of lipid, which represent as down-regulated glycerolipid and glycerophospholipid, up-regulated sphingolipid metabolism and sphingolipid signaling pathway. Additionally, ATP level was significantly decreased in TCA cycle and oxidative phosphorylation pathway, which correlated with glucagon signaling pathway and fatty acid synthesis. The lipid-lowering activity of NNPs may associate with lipid and energy-related metabolism in 3 T3-T1 cell. ATP may be a potential biomarker for lipid-lowering, with a fold change of 0.534.

Non-alcoholic fatty liver disease (NAFLD), defined in recent years as metabolic-associated fatty liver disease (MAFLD), is one of the most common liver diseases in the world, with no drugs on market. Esculetin (ESC) is an active compound discovered in a variety of natural products that modulates a wide range of metabolic diseases and is a potential drug for the treatment of NAFLD. In this study, we used an HCD-induced NAFLD larval zebrafish model in vivo and an FFA-induced BRL-3A hepatocyte model in vitro to evaluate the anti-NAFLD effect of ESC. Lipid lowering, anti-oxidation and anti-inflammation effects were revealed on ESC and related gene changes were observed. This study provides a reference for further study and development of ESC as a potential anti-NAFLD/MAFLD drug. [ABSTRACT FROM AUTHOR]

Cardiovascular disease (CVD) caused by atherosclerosis is the leading cause of death worldwide. The level of low-density lipoprotein cholesterol (LDL-C), considered as the initiator of atherosclerosis, is the most widely used predictor for CVD risk and LDL-C has been the primary target for lipid-lowering therapies. However, residual CVD risk remains high even with very low levels of LDL-C. This residual CVD risk may be due to remnant cholesterol, high triglyceride levels, and low high-density lipoprotein cholesterol (HDL-C). Non-high density lipoprotein cholesterol (non-HDL-C), which is calculated as total cholesterol minus HDL-C (and represents the cholesterol content of all atherogenic apolipoprotein B-containing lipoproteins), has emerged as a better risk predictor for CVD than LDL-C and an alternative target for CVD risk reduction. Major international guidelines recommend evaluating non-HDL-C as part of atherosclerotic CVD risk assessment, especially in people with high triglycerides, diabetes, obesity, or very low LDL-C. A non-HDL-C target of <130 mg/dL (3.4 mmol/L) has been recommended for patients at very high risk, which is 30 mg/dL (0.8 mmol/L) higher than the corresponding LDL-C target goal. Non-HDL-C lowering approaches include reducing LDL-C and triglyceride levels, increasing HDL-C, or targeting multiple risk factors simultaneously. However, despite the growing evidence for the role of non-HDL-C in residual CVD risk, and recommendations for its assessment in major guidelines, non-HDL-C testing is not routinely done in clinical practice. Despite the availability of effective therapies that lower low‐density lipoprotein cholesterol (LDL‐C) levels in patients with atherosclerotic cardiovascular disease, many eligible patients are inadequately treated and their LDL‐C levels remain suboptimal. Patient nonadherence to lipid‐lowering therapy (LLT) is a major contributor to the failure of LDL‐C goal attainment. Several factors have been identified as contributing to LLT nonadherence, including healthcare disparities due to socioeconomic status, age, race, sex, and cost; limited access to healthcare; perceived side effects associated with LLT; health literacy; and the presence of comorbidities. Suboptimal LLT use has also been associated with clinician factors, including failure to identify patients who require LDL‐C reassessment, insufficient LDL‐C monitoring, and clinical inertia such as a lack of therapy intensification. Several strategies to enhance LLT adherence have been shown to be effective, including the implementation of educational initiatives and tools for both patients and physicians, the use of clinical protocols and algorithms to identify patients at risk and optimize treatment, and improvements in electronic healthcare records. Pharmacy‐based programs designed to help patients with prescription refills, including reminders or the use of prescription delivery by mail, have also proven effective. Drugs requiring frequent administration can represent a barrier to treatment adherence; therefore, newer, more effective LLTs with lower frequency of administration and lower potential for polypharmacy may improve patient adherence to LLT. Implementation of strategies to identify patients at risk for LLT nonadherence and the use of flexible tools such as telemedicine to overcome geographical barriers may improve LLT adherence.
