Abstract

Surgery on the temporal bone is technically challenging due to its complex anatomy. Precise anatomical dissection of the human temporal bone is essential and is fundamental for middle ear surgery. We assessed the possible application of a virtual reality temporal bone surgery simulator to the education of ear surgeons. Seventeen ENT physicians with different levels of surgical training and 20 medical students performed an antrotomy with a computer-based virtual temporal bone surgery simulator. The ease, accuracy and timing of the simulated temporal bone surgery were assessed using the automatic assessment software provided by the simulator device and additionally with a modified Final Product Analysis Scale. Trained ENT surgeons, physicians without temporal bone surgical training and medical students were all able to perform the antrotomy. However, the highly trained ENT surgeons were able to complete the surgery in approximately half the time, with better handling and accuracy as assessed by the significant reduction in injury to important middle ear structures. Trained ENT surgeons achieved significantly higher scores using both dissection analysis methods. Surprisingly, there were no significant differences in the results between medical students and physicians without experience in ear surgery. The virtual temporal bone training system can stratify users of known levels of experience. This system can be used not only to improve the surgical skills of trained ENT surgeons for more successful and injury-free surgeries, but also to train inexperienced physicians/medical students in developing their surgical skills for the ear.

22. Environmental Education Research Project, Content Analysis Criteria, Report on First Evaluation Trial.

Abstract

Ten criteria for use in assessing the emphasis on environmental education in textbooks and similar resource materials were developed and given to 30 members of the Australian Conservation Foundation Education and Training Committees throughout the country. Each rater applied the criteria to three chapters of a biology textbook "The Web of Life," the Australian adaptation of the Biological Sciences Curriculum Study materials) and three units of the Australian Science Education Project materials (see ED 061 106 and ED 063 167 for two of the units). The results of the rating are presented, and, where appropriate, the maximum deviation from a cumulative reference distribution based on random rating (Kolmogorov-Smirnov one-sample test) is presented. The reliability of each index, and difficulties encountered in its use, are discussed. Copies of the scales, instructions and explanations provided to raters, and the rationale for each criterion are included in an appendix to the report. (AL)

Abstract

An experimental course covering some of the fundamental principles and terminology associated with environmental science and the application of these principles to various contemporary problems is summarized in this report. The course involved a series of lectures together with a program of specific seminar and discussion topics presented by the students, and a limited selection of field and laboratory exercises. Content dealt with the biosphere; energy flow on the earth and in the biosphere; biogeochemical cycles--carbon, oxygen, water, nitrogen, and mineral; general principles of ecology; general population studies; human population studies; basic principles of genetics--biochemical genetics, genetic variation, and natural selection and evolution; and conservation and pollution. Evaluations conducted during the course are reported in detail. These include analysis of affective objectives, cognitive objectives, and the total course. (BL)

Abstract

Objective: Regulation of postprandial (pp) plasma glucose excursions is complex. Insulin and glucagon are thought to play the predominant role. Nevertheless, only 50% of the variation in pp plasma glucose excursions is explained by variations by the latter. Theoretically, gastric emptying (GE) should be another important factor. However, its impact on pp glucose homeostasis is unknown. Research Design and Methods: We examined the consequences of pramlintide-induced delay in GE on pp glycemia and glucose fluxes, determined isotopically. GE was recorded by scintigraphy. Fourteen healthy subjects (8 men, 6 women; age 40 [+ or -] 3 yr, body mass index 27.8 [+ or -] 1.1 kg/[m.sup.2]) ate a mixed meal, and 30 [micro]g of pramlintide (PRAM) or placebo (PBO) were injected subcutaneously. Results: At 60 min, greater proportions of the initial gastric contents remained in the stomach (PBO vs. PRAM). Thereafter, GE slopes paralleled until 240 min. Fifty percent retention times were lower when PBO was given (P < 0.001). GE was greater from 240 min to the end of the PRAM experiments, so that only slightly greater proportions of the meal remained in the stomach at 330 min. Reductions of GE lowered pp glucose (7.5 [+ or -] 0.3 vs. 6.0 [+ or -] 0.2 mmol/1, P < 0.001), even though plasma insulin was lower with PRAM (164 [+ or -] 13 vs. 138 [+ or -] 13 pmol/ml, both P < 0.01). Reduction in total glucose appearance (P < 0.001) was due to reduced meal-derived glucose appearance (10.2 [+ or -] 0.5 vs. 7.0 [+ or -] 0.4 [micro]mol x [kg.sup.-1] x [min.sup.-1], P < 0.001). Endogenous glucose appearance was greater with PRAM (P < 0.001). Splanchnic glucose uptake was greater with PRAM (26.5 [+ or -] 1.6 vs. 32.5 [+ or -] 2.1%, P = 0.014). Conclusions: These data support the concept that GE is an important physiological regulator of pp glucose homeostasis in humans.
