Abstract

Summary: Opioid misuse is now considered a major public health epidemic in North America, with substantial social and financial consequences. As well as socio‐economic and commercial drivers, modifiable risk‐factors that have resulted in this crisis have been identified. The purpose of this study was to identify whether, within England, modifiable drivers for persistent postoperative opioid use were present. This was a retrospective cohort study of practice at 14 National Health Service hospitals across England. Data were collected retrospectively and validated for adult patients undergoing elective intermediate and major or complex major general surgical procedures between 1 and 31 March 2019. Of the 509 patients enrolled from 14 centres, 499 were included in the data analysis. In total, 31.5% (157/499) patients were in the intermediate surgery cohort and 68.5% (342/499) were in the major or complex major surgery cohort, with 21.0% (33/157) and 21.6% (74/342) discharged with opioid medicines to be taken at regular intervals, respectively. There were similar median oral morphine equivalent doses prescribed at discharge. Of patients prescribed regular opioid medicines, 76.6% (82/107) had a specified duration at discharge. However, 72.9% (78/107) had no written deprescribing advice on discharge. Similarly, of patients prescribed 'when required' opioids, 59.6% (93/156) had a specified duration of their prescription and 33.3% (52/156) were given written deprescribing advice. This study has identified a pattern of poor prescribing practices, a lack of guidance and formal training at individual institutions and highlights opportunities for improvement in opioid‐prescribing practices within England. Abstract

Introduction: The 2008 National Mastectomy and Breast Reconstruction Audit demonstrated marked variation in the practice and outcomes of breast reconstruction in the UK. To standardise practice and improve outcomes for patients, the British professional associations developed best-practice guidelines with specific guidance for newer mesh-assisted implant-based techniques. We explored the degree of uptake of best-practice guidelines within units performing implant-based reconstruction (IBBR) as the first phase of the implant Breast Reconstruction Evaluation (iBRA) study. Method(s): A questionnaire developed by the iBRA Steering Group was completed by trainee and consultant leads at breast and plastic surgical units across the UK. Simple summary statistics were calculated for each survey item to assess compliance with current best-practice guidelines. Result(s): 81 units from 79 NHS Trusts completed the questionnaire. Marked variation was observed in adherence to guidelines, especially those relating to clinical governance and infection prevention strategies. Less than half (n = 28, 47%) of units obtained local clinical governance board approval prior to offering new mesh-based techniques and prospective audit of the clinical, cosmetic and patient-reported outcomes of surgery was infrequent. Most units screened for methicillin-resistant staphylococcus aureus prior to surgery but fewer than 1 in 3 screened for methicillin-sensitive strains. Laminar-flow theatres (recommended for IBBR) were not widely-available with less than 1 in 5 units having regular access. Peri-operative antibiotics were widely-used, but the type and duration were highly-variable. Conclusion(s): The iBRA national practice questionnaire has demonstrated variation in reported practice and adherence to IBBR guidelines. Abstract

Acetophenone, ethyl butyrate and 2-phenyl ethanol were used as additives to media on which Tribolium castaneum (Herbst) and Lasioderma serricorne (Fabricius) were cultured for 65 days. Acetophenone and ethyl butyrate had a positive impact on the weight and multiplication of the insects. The weight and rate of multiplication was higher in feed media containing acetophenone alone. The amount of the flavour volatiles added was high enough to deter the Lasioderma from feeding well, which affected their growth. This attraction to specific flavour volatiles in some stored products could be used in the formulation of effective control measures for insect pests.

Abstract

The metabolism of deuterium-labelled analogues of ABA by normal and flacca mutant tomato plants was investigated. Comparison of the biological activity of ABA, ABA alcohol, ABA aldehyde and their 2-trans isomers was made in both mutant and non-mutant genotypes. While in normal plants ABA alcohol and ABA aldehyde were as effective as ABA in inducing stomatal closure, in the flacca mutants only ABA itself was biologically active. Both ABA alcohol and ABA aldehyde were converted to the inactive compound trans-ABA alcohol instead of ABA when fed to flacca plants. As trans-ABA aldehyde was also readily converted to trans-ABA alcohol by flacca plants, it was not possible to establish whether isomerization precedes reduction or vice versa in the synthesis of trans- ABA alcohol from ABA aldehyde.
