The 2019 novel coronavirus disease (COVID-19) pandemic produced an abrupt and near shutdown of nonemergent patient care. Children's National Hospital (CNH) mounted a multidisciplinary, coordinated ambulatory response that included supply chain management, human resources, risk management, infection control, and information technology. To ensure patient access, CNH expanded telemedicine and instituted operational innovations for outpatient procedures. While monthly in-person ambulatory subspecialty visits decreased from 25 889 pre-COVID-19 to 4484 at nadir of the COVID-19 pandemic, telemedicine visits increased from 70 to 13 539. We designed this study to clarify the role of work stress on long-term blood pressure control and in particular to investigate whether perceived work stress directly affected resting blood pressure levels or whether there were indirect effects mediated by coping mechanisms and lifestyle. Men (n = 337) and women (n = 317) working in a government tax office completed questionnaires for assessment of work-related stress, coping strategies, and lifestyle. Seven resting blood pressure measurements were recorded serially on each of two occasions a week apart. Men had higher blood pressures (119.6/68.6 versus 110.9/65.6 mm Hg) than women; they used more "maladaptive" coping strategies, drank more alcohol, and ate less healthily but exercised more than women. There were no direct associations between measures of work stress and blood pressure. In univariate and regression analyses, both body mass index and lifestyle factors in the form of alcohol consumption, exercise, and diet were related to blood pressure in men and women. Various "adaptive" or "maladaptive" coping mechanisms were identified and independently related to both job stress and blood pressure levels. Women were more likely to use "healthier" or adaptive coping mechanisms than men. Thus, work stress per se had no direct effect on blood pressure, but the ways that individuals reported coping with stress were significantly related to blood pressure, with blood pressure elevation effects appearing to be mediated largely by dietary and drinking habits and physical inactivity. The results point to the need to target individual coping strategies and lifestyle as much as the working environment in workplace cardiovascular health promotion programs.

The relative importance of perceived stress compared with coping behaviours and 'lifestyle' characteristics known to influence blood pressure were studied in 337 male and 317 female office workers. 2. Males had significantly higher mean systolic (P < 0.01; t-test, 652 d.f.) and diastolic (P < 0.01; t-test, 652 d.f.) blood pressure and unhealthier lifestyles than females, particularly in the areas of alcohol intake (P < 0.01; t-test, 653 d.f.) and diet (P = 0.01; t-test, 663 d.f.) 3. In males drinking alcohol was correlated to job and home/work stress (P < 0.05), and eating more atherogenic foods was correlated to home/work stress (P < 0.05). 4. Coping by food and drug consumption was correlated with job and home/work stress (P < 0.05) in males; avoidance/denial coping was correlated with job stress in both males and females (P < 0.001) and to home/work stress (P < 0.001) in males only. Blood pressure was not correlated with stress in males or females. 5. In age-adjusted regression analyses body mass index (BMI) and lifestyle (physical inactivity, alcohol consumption and diet) made significant contributions to systolic (P = 0.02) and diastolic (P < 0.01) blood pressure and, in separate analyses, coping contributed significantly to diastolic blood pressure (P < 0.01) in males. Stress made no additional contribution to blood pressure in either analysis. 6. After including age, BMI, lifestyle and coping in males 'lifestyle' still contributed to systolic and diastolic blood pressure and coping made a significant additional contribution to diastolic blood pressure.(ABSTRACT TRUNCATED AT 250 WORDS)

The relationship between cardiovascular risk factors and urine albumin excretion were studied in 474 healthy office workers. Albumin concentration was measured fresh in first morning midstream urines. Lifestyle details, oscillometric BP and lipids were assessed. Subjects with urine albumin concentration above the median (5.30 mg/l) were compared with those with albumin concentration below the median. Subjects with above median urinary albumin concentration had higher systolic blood pressure (mean 115.2 vs. 113.1 mm Hg for above median, respectively, P = 0.06), were more likely to be male (56.8 vs. 45.0%, respectively, P = 0.01) and to have lower levels of high density lipoprotein (HDL)-cholesterol (mean 1.34 vs. 1.41 mmol/l, P = 0.006). Multivariate analysis following adjustment for urine creatinine concentration to allow for urine volume confirmed the relationship with systolic blood pressure (P = 0.01) and sex (P = 0.02), and in addition revealed a relationship with alcohol intake approaching significance (mean intake 70.8 and 76.0 g/week, respectively, P = 0.06). The univariate finding of increased albuminuria with lower HDL-cholesterol appeared to be attributable to the associated relationships with male sex and lower alcohol intake. The relationships between albumin excretion and BP, male sex and alcohol intake may reflect the effects of asymptomatic developing arterial disease. The relationship with BP may also be a consequence of effects on glomerular hydrostatic or interstitial renal pressure on albumin filtration or resorption. Very low level urine albumin excretion in healthy subjects is associated with factors which predict arterial disease. Urine albumin excretion may prove to be a useful early marker of cardiovascular disease in population studies.
