Background. The uterine sarcoma is a rare tumor with the unpredictable, aggressive clinical behavior. Medical science relies on the development of reliable tumor markers, on the basis of which the optimal treatment program can be chosen, and will be also possible to make a prognosis. The hyperexpression of PD-L1 in many cases correlates with unfavorable prognosis of the disease and is an important prognostic biomarker for some types of tumors: melanoma, kidney cancer, non-small cell lung cancer. The role of PD-L1 expression, as a tumor marker in sarcoma, remains unclear. Objective. The investigation of PD-L1 expression as a prognostic tumor marker for uterine sarcoma. Methods. There have been selected 30 uterine sarcoma patients stage I-II (T1-2NxM0), for immunohistochemistry analyze of PD-L1 expression. Depending on the morphological tumor types all the patients were distributed: leiomyosarcoma (LMS) - 20.0%, endometrial stromal sarcoma (ESS) - 46.7%, undifferentiated sarcoma (HC) - 33.3%. Results. Our results showed that 73.3 % of patients with uterine sarcoma exhibited low expression level of PD-L1. The moderate level and overexpression of PD-L1 were observed in undifferentiated and endometrial stromal sarcoma - 13.3 and 6.7 %, respectively. At further follow-up of patients with PD-L1 expression, the relapse of the disease was detected in 50.0 % of cases. Conclusion. The PD-L1 expression in tumor tissue, regardless of its level, is considered to be an unfavorable prognostic factor for uterine sarcoma patients. In case of moderate expression level of PD-L1, so as at its overexpression, the tumor progression was detected in 83.3% of uterine sarcoma patients., Саркома матки является редкой опухолью с непредсказуемым агрессивным клиническим течением. Гиперэкспрессия PD-L1 на опухолевых клетках в большинстве случаев коррелирует с неблагоприятным прогнозом заболевания и является важным прогностическим биомаркером при некоторых типах опухолей: меланома, почечно-клеточный, немелкоклеточный рак легких. Роль экспрессии PD-L1, как опухолевого маркера при саркоме, остается до конца не определенной. Целью данного исследования является изучение экспрессии белка PD-L1 как прогностического опухолевого маркера при саркоме матки. Материалы и методы. Нами отобрано 30 больных саркомой матки I-II стадий (T1-2NxM0) для проведения иммуногистохимического исследования экспрессии маркера PD-L1. В зависимости от морфологического типа опухоли всех больных распредилили: лейомиосаркома (ЛМС) – 20,0 %, эндометриальная стромальная саркома (ЭСС) – 46,7 %, недифференціированная саркома (НС) – 33,3 %. Результаты. У 73,3 % больных отмечен низкий уровень экспрессии PD-L1. Умеренный уровень и гиперэкспрессия PD-L1 наблюдаются при недифференцированной и эндометриальной стромальной саркоме – у 13,3 и 6,7 % пациенток, соответственно. При дальнейшем мониторинге больных, у у которых отмечено экспрессию PD-L1, выявлен рецидив заболевания в 50,0 % случаев. Заключение. Независимо от уровня экспрессии PD-L1, ее наличие является неблагоприятный прогностический фактор при саркоме матки. При умеренном уровне экспрессии та гиперэкспресии PD-L1 прогрессию опухоли отмечено у 83,3 % больных саркомой матки., Обгрунтування. Було селекційовано 30 хворих на саркому матки I-II стадії (T1-2NxM0) для проведення імуногістохімічного дослідження експресії маркера PD-L1. Залежно від морфологічного типу пухлини усiх хворих було розподілено: лейоміосаркома (ЛМС) – 20,0 %, ендометріальна стромальна саркома (ЕСС) – 46,7 %, недиференційована саркома (НС) – 33,3 %. Результати. У 73,3% хворих спостерігається низький рівень експресії PD-L1. Помірний рівень і гіперекспресія PD-L1 спостерігаються при недиференційованих і ендометріальних стромальних саркомах – у 13,3 і 6,7%, відповідно. При подальшому моніторингу хворих, у яких відмічено експресію PD-L1, виявлено рецидив захворювання у 50,0 % випадків. Підсумок. Незалежно від рівня експресії PD-L1, її наявність виступає несприятливим прогностичним фактором при саркомі матки. При помірному рівні експресії та гіперекспресії PD-L1 прогресію пухлини відмічено у 83,3 % хворих на саркому матки.

A total of 308,036 women were selected from the European Prospective Investigation into Cancer and Nutrition (EPIC) study to evaluate the association between tobacco smoking and the risk of cervical intraepithelial neoplasia of grade 3 (CIN3)/carcinoma in situ (CIS) and invasive cervical cancer (ICC). At baseline, participants completed a questionnaire and provided blood samples. During a mean follow-up time of 9 years, 261 ICC cases and 804 CIN3/CIS cases were reported. In a nested case-control study, the baseline sera from 609 cases and 1,218 matched controls were tested for L1 antibodies against HPV types 11, 16, 18, 31, 33, 35, 45, 52, 58, and antibodies against Chlamydia trachomatis (CT), and Human Herpes Virus 2 (HHV-2). Cervical samples were not available for HPV-DNA analysis in this study. Multivariate analyses were used to estimate associations between smoking and risk of CIN3/CIS and ICC in the cohort and the case-control studies. In the cohort analyses smoking status, duration and intensity showed a two-fold increased risk of CIN3/CIS and ICC, while time since quitting was associated with a two-fold reduced risk. In the nested case-control study, consistent associations were observed after adjustment for HPV, CT and HHV-2 serostatus, in both HPV seronegative and seropositive women. Results from this large prospective study confirm the role of tobacco smoking as an important risk factor for both CIN3/CIS and ICC, even after taking into account HPV exposure as determined by HPV serology. The strong beneficial effect of quitting smoking is an important finding that will further support public health policies for smoking cessation. What's new? Tobacco smoking is a cited cause of cervical cancer, but whether it causes cervical malignancy independent of human papillomavirus (HPV) infection is unclear. Here, strong associations were found between most measures of tobacco smoking and the risk of cervical intraepithelial neoplasia of grade 3/carcinoma in situ and invasive cervical cancer, after taking into account past exposure to HPV infection. Quitting smoking was associated with a 2-fold risk reduction. The findings confirm the role of tobacco smoking in cervical carcinogenesis and show that quitting the habit has important benefits for cancer protection. © 2013 UICC.

Mastitis remains a critical disease in the dairy industry and the use of intramammary antibiotics plays a critical role in mastitis treatment. Hetacin-K® (Boehringer Ingelheim Vetmedica, Inc) is currently approved as an intramammary antibiotic that is used to treat mastitis in dairy cows. It is approved for once daily intramammary administration and can be used for a total of three days. More and more dairy farms are milking three times per day instead of the traditional two times per day; however, there is very little pharmacokinetic data on the use of intramammary drugs such as Hetacin-K® in a 3X system.
