The objective of this study was to compare the lesion detection performance of human observers between thin-section computed tomography images of the breast, with thick-section (>40 mm) simulated projection images of the breast. Three radiologists and six physicists each executed a two alterative force choice (2AFC) study involving simulated spherical lesions placed mathematically into breast images produced on a prototype dedicated breast CT scanner. The breast image data sets from 88 patients were used to create 352 pairs of image data. Spherical lesions with diameters of 1, 2, 3, 5, and 11 mm were simulated and adaptively positioned into 3D breast CT image data sets; the native thin section (0.33 mm) images were averaged to produce images with different slice thicknesses; average section thicknesses of 0.33, 0.71, 1.5 and 2.9 mm were representative of breast CT; the average 43 mm slice thickness served to simulate simulated projection images of the breast.The percent correct of the human observer's responses were evaluated in the 2AFC experiments. Radiologists lesion detection performance was significantly (p < 0.05) better in the case of thin-section images, compared to thick section images similar to mammography, for all but the 1 mm lesion diameter lesions. For example, the average of three radiologist's performance for 3 mm diameter lesions was 92% correct for thin section breast CT images while it was 67% for the simulated projection images. A gradual reduction in observer performance was observed as the section thickness increased beyond about 1 mm. While a performance difference based on breast density was seen in both breast CT and the projection image results, the average radiologist performance using breast CT images in dense breasts outperformed the performance using simulated projection images in fatty breasts for all lesion diameters except 11 mm. The average radiologist performance outperformed that of the average physicist observer, however trends in performance were similar. Human observers demonstrate significantly better mass-lesion detection performance on thin-section CT images of the breast, compared to thick-section simulated projection images of the breast.

ObjectiveThe stress experienced by women undergoing surveillance mammography for benign lesions was compared with that experienced by women undergoing core biopsy of benign lesions.Materials and methodsA retrospective survey was mailed to women in whom were diagnosed breast abnormalities that were likely to be benign and that were stable at short-interval follow-up mammography (n = 174) or in whom a core biopsy of the breast was performed, with benign findings (n = 116). The survey included questions about stress related to the diagnostic experience as well as questions about who advised the women of the results of their breast imaging studies and about the information provided.ResultsEighty eligible surveys were received from the women who underwent short-interval follow-up mammography, and 58 surveys were received from the women who underwent biopsies. The self-reported overall stress experienced by the women who underwent core biopsies was significantly greater (p < .001) than that reported by the group who were followed up with mammography. The level of anxiety was not affected by the patient's perception of the probability that the lesion represented carcinoma. We found no significant differences in stress between women who discussed their mammogram results with a radiologist and women who were notified by their primary care provider.ConclusionShort-interval follow-up mammography continues to be acceptable for the evaluation of lesions that are probably benign, even when patient anxiety is considered.

Objective: Compare conspicuity of suspicious breast lesions on contrast-enhanced dedicated breast CT (CEbCT), tomosynthesis (DBT) and digital mammography (DM)., Methods: 100 females with BI-RADS 4/5 lesions underwent CEbCT and/or DBT prior to biopsy in this IRB approved, HIPAA compliant study. Two breast radiologists adjudicated lesion conspicuity scores (CS) for each modality independently. Data are shown as mean CS ±standard deviation. Two-sided t -test was used to determine significance between two modalities within each subgroup. Multiple comparisons were controlled by the false-discovery rate set to 5%., Results: 50% of studied lesions were biopsy-confirmed malignancies. Malignant masses were more conspicuous on CEbCT than on DBT or DM (9.7 ±0.5, n = 25; 6.8 ± 3.1, n = 15; 6.7 ± 3.0, n = 27; p < 0.05). Malignant calcifications were equally conspicuous on all three modalities (CEbCT 8.7 ± 0.8, n = 18; DBT 8.5 ± 0.6, n = 15; DM 8.8 ± 0.7, n = 23; p = NS). Benign masses were equally conspicuous on CEbCT (6.6 ± 4.1, n = 22); DBT (6.4 ± 3.8, n = 17); DM (5.9 ± 3.6, n = 24; p = NS). Benign calcifications CS were similar between DBT (8.5 ± 1.0, n = 17) and DM (8.8 ± 0.8, n = 26; p = NS) but less conspicuous on CEbCT (4.0 ± 2.9, n = 25, p < 0.001). 55 females were imaged with all modalities. Results paralleled the entire cohort. 69%( n = 62) of females imaged by CEbCT had dense breasts. Benign/malignant lesion CSs in dense/non-dense categories were 4.8 ± 3.7, n = 33, vs 6.0 ± 3.9, n = 14, p = 0.35; 9.2 ± 0.9, n = 29 vs. 9.4 ± 0.7, n = 14; p = 0.29, respectively., Conclusion: Malignant masses are more conspicuous on CEbCT than DM or DBT. Malignant microcalcifications are equally conspicuous on all three modalities. Benign calcifications remain better visualized by DM and DBT than with CEbCT. We observed no differences in benign masses on all modalities. CS of both benign and malignant lesions were independent of breast density., Advances in Knowledge: CEbCT is a promising diagnostic imaging modality for suspicious breast lesions.

Breast density notification laws, passed in 19 states as of October 2014, mandate that patients be informed of their breast density. The purpose of this study is to assess the impact of this legislation on radiology practices, including performance of breast cancer risk assessment and supplemental screening studies. A 20-question anonymous web-based survey was emailed to radiologists in the Society of Breast Imaging between August 2013 and March 2014. Statistical analysis was performed using Fisher's exact test. Around 121 radiologists from 110 facilities in 34 USA states and 1 Canadian site responded. About 50% (55/110) of facilities had breast density legislation, 36% of facilities (39/109) performed breast cancer risk assessment (one facility did not respond). Risk assessment was performed as a new task in response to density legislation in 40% (6/15) of facilities in states with notification laws. However, there was no significant difference in performing risk assessment between facilities in states with a law and those without (p < 0.831). In anticipation of breast density legislation, 33% (16/48), 6% (3/48), and 6% (3/48) of facilities in states with laws implemented handheld whole breast ultrasound (WBUS), automated WBUS, and tomosynthesis, respectively. The ratio of facilities offering handheld WBUS was significantly higher in states with a law than in states without (p < 0.001). Purpose: Compare conspicuity of ductal carcinoma in-situ (DCIS) to benign calcifications on unenhanced (bCT), contrast-enhanced dedicated breast CT (CEbCT) and mammography (DM)., Methods and Materials: The institutional review board approved this HIPAA-compliant study. 42 women with Breast Imaging Reporting and Data System 4 or 5 category micro-calcifications had breast CT before biopsy. Three subjects with invasive disease at surgery were excluded. Two breast radiologists independently compared lesion conspicuity scores (CS) for CEbCT, to bCT and DM. Enhancement was measured in Hounsfield units (HU). Mean CS ± standard deviations are shown. Receiver operating characteristic analysis (ROC) measured radiologists' discrimination performance by comparing CS to enhancement alone. Statistical measurements were made using ANOVA F-test, Wilcoxon rank-sum test and robust linear regression analyses., Results: 39 lesions (17 DCIS, 22 benign) were analyzed. DCIS (8.5 ± 0.9, n=17) was more conspicuous than benign micro-calcifications (3.6 ± 2.9, n=22; p<0.0001) on CEbCT. DCIS was equally conspicuous on CEbCT and DM (8.5 ± 0.9, 8.7 ± 0.8, n=17; p=0.85) and more conspicuous when compared to bCT (5.3 ± 2.6, n=17; p<0.001). All DCIS enhanced; mean enhancement (90HU ± 53HU, n=17) was higher compared to benign lesions (33 ± 30HU, n=22) (p<0.0001). ROC analysis of the radiologists' CS showed high discrimination performance (AUC=0.94) compared to enhancement alone (AUC=0.85) (p<0.026)., Conclusion: DCIS is more conspicuous than benign micro-calcifications on CEbCT. DCIS visualization on CEbCT is equal to mammography but improved compared to bCT. Radiologists' discrimination performance using CEBCT is significantly higher than enhancement values alone. Purpose: The purpose of this study is to measure the effectiveness of local curvature measures as novel image features for classifying breast tumors., Methods: A total of 119 breast lesions from 104 noncontrast dedicated breast computed tomography images of women were used in this study. Volumetric segmentation was done using a seed-based segmentation algorithm and then a triangulated surface was extracted from the resulting segmentation. Total, mean, and Gaussian curvatures were then computed. Normalized curvatures were used as classification features. In addition, traditional image features were also extracted and a forward feature selection scheme was used to select the optimal feature set. Logistic regression was used as a classifier and leave-one-out cross-validation was utilized to evaluate the classification performances of the features. The area under the receiver operating characteristic curve (AUC, area under curve) was used as a figure of merit., Results: Among curvature measures, the normalized total curvature (CT) showed the best classification performance (AUC of 0.74), while the others showed no classification power individually. Five traditional image features (two shape, two margin, and one texture descriptors) were selected via the feature selection scheme and its resulting classifier achieved an AUC of 0.83. Among those five features, the radial gradient index (RGI), which is a margin descriptor, showed the best classification performance (AUC of 0.73). A classifier combining RGI and CT yielded an AUC of 0.81, which showed similar performance (i.e., no statistically significant difference) to the classifier with the above five traditional image features. Additional comparisons in AUC values between classifiers using different combinations of traditional image features and CT were conducted. The results showed that CT was able to replace the other four image features for the classification task., Conclusions: The normalized curvature measure contains useful information in classifying breast tumors. Using this, one can reduce the number of features in a classifier, which may result in more robust classifiers for different datasets.

Purpose: To investigate primary physician awareness of the California Breast Density Notification Law and its impact on primary care practice., Methods: An online survey was distributed to 174 physicians within a single primary care network system 10 months after California's breast density notification law took effect. The survey assessed physicians' awareness of the law, perceived changes in patient levels of concern about breast density, and physician comfort levels in handling breast density management issues., Results: The survey was completed by 77 physicians (45%). Roughly half of those surveyed (49%) reported no knowledge of the breast density notification legislation. Only 32% of respondents noted an increase in patient levels of concern about breast density compared to prior years. The majority were only "somewhat comfortable" (55%) or "not comfortable" (12%) with breast density questions, and almost one-third (32%) had referred patients to a breast health clinic for these discussions. A total of 75% of those surveyed would be interested in more specific education on the subject., Conclusions: Awareness among primary care clinicians of the California Breast Density Notification Law is low, and many do not feel comfortable answering breast density-related patient questions. Breast imagers and institutions may need to devote additional time and resources to primary physician education in order for density notification laws to have significant impact on patient care., (Copyright © 2015 American College of Radiology. Evaluation of segmentation algorithms usually involves comparisons of segmentations to gold-standard delineations without regard to the ultimate medical decision-making task. We compare two segmentation evaluations methods-a Dice similarity coefficient (DSC) evaluation and a diagnostic classification task-based evaluation method using lesions from breast computed tomography. In our investigation, we use results from two previously developed lesion-segmentation algorithms [a global active contour model (GAC) and a global with local aspects active contour model]. Although similar DSC values were obtained (0.80 versus 0.77), we show that the global + local active contour (GLAC) model, as compared with the GAC model, is able to yield significantly improved classification performance in terms of area under the receivers operating characteristic (ROC) curve in the task of distinguishing malignant from benign lesions. This is mainly because the GLAC model yields better detailed information required in the calculation of morphological features. Based on our findings, we conclude that the DSC metric alone is not sufficient for evaluating segmentation lesions in computer-aided diagnosis tasks.

We present and evaluate a method for the three-dimensional (3-D) segmentation of breast masses on dedicated breast computed tomography (bCT) and automated 3-D breast ultrasound images. The segmentation method, refined from our previous segmentation method for masses on contrast-enhanced bCT, includes two steps: (1) initial contour estimation and (2) active contour-based segmentation to further evolve and refine the initial contour by adding a local energy term to the level-set equation. Segmentation performance was assessed in terms of Dice coefficients (DICE) for 129 lesions on noncontrast bCT, 38 lesions on contrast-enhanced bCT, and 98 lesions on 3-D breast ultrasound (US) images. For bCT, DICE values of 0.82 and 0.80 were obtained on contrast-enhanced and noncontrast images, respectively. The improvement in segmentation performance with respect to that of our previous method was statistically significant ( p = 0.002 ). Moreover, segmentation appeared robust with respect to the presence of glandular tissue. For 3-D breast US, the DICE value was 0.71. Dedicated breast CT (bCT) produces high-resolution 3D tomographic images of the breast, fully resolving fibroglandular tissue structures within the breast and allowing for breast lesion detection and assessment in 3D. In order to enable quantitative analysis, such as volumetrics, automated lesion segmentation on bCT is highly desirable. In addition, accurate output from CAD (computer-aided detection/diagnosis) methods depends on sufficient segmentation of lesions. Thus, in this study, we present a 3D lesion segmentation method for breast masses in contrast-enhanced bCT images. The segmentation algorithm follows a two-step approach. First, 3D radial-gradient index segmentation is used to obtain a crude initial contour, which is then refined by a 3D level set-based active contour algorithm. The data set included contrast-enhanced bCT images from 33 patients containing 38 masses (25 malignant, 13 benign). The mass centers served as input to the algorithm. In this study, three criteria for stopping the contour evolution were compared, based on (1) the change of region volume, (2) the average intensity in the segmented region increase at each iteration, and (3) the rate of change of the average intensity inside and outside the segmented region. Lesion segmentation was evaluated by computing the overlap ratio between computer segmentations and manually drawn lesion outlines. For each lesion, the overlap ratio was averaged across coronal, sagittal, and axial planes. The average overlap ratios for the three stopping criteria ranged from 0.66 to 0.68 (dice coefficient of 0.80 to 0.81), indicating that the proposed segmentation procedure is promising for use in quantitative dedicated bCT analyses.

In anticipation of breast density notification legislation in the state of California, which would require notification of women with heterogeneously and extremely dense breast tissue, a working group of breast imagers and breast cancer risk specialists was formed to provide a common response framework. The California Breast Density Information Group identified key elements and implications of the law, researching scientific evidence needed to develop a robust response. In particular, issues of risk associated with dense breast tissue, masking of cancers by dense tissue on mammograms, and the efficacy, benefits, and harms of supplementary screening tests were studied and consensus reached. National guidelines and peer-reviewed published literature were used to recommend that women with dense breast tissue at screening mammography follow supplemental screening guidelines based on breast cancer risk assessment. The goal of developing educational materials for referring clinicians and patients was reached with the construction of an easily accessible Web site that contains information about breast density, breast cancer risk assessment, and supplementary imaging. This multi-institutional, multidisciplinary approach may be useful for organizations to frame responses as similar legislation is passed across the United States. Background: Computer-aided detection (CAD) has rapidly diffused into screening mammography practice despite limited and conflicting data on its clinical effect., Objective: To determine associations between CAD use during screening mammography and the incidence of ductal carcinoma in situ (DCIS) and invasive breast cancer, invasive cancer stage, and diagnostic testing., Design: Retrospective cohort study., Setting: Medicare program., Participants: Women aged 67 to 89 years having screening mammography between 2001 and 2006 in U.S. SEER (Surveillance, Epidemiology and End Results) regions (409 459 mammograms from 163 099 women)., Measurements: Incident DCIS and invasive breast cancer within 1 year after mammography, invasive cancer stage, and diagnostic testing within 90 days after screening among women without breast cancer., Results: From 2001 to 2006, CAD prevalence increased from 3.6% to 60.5%. Use of CAD was associated with greater DCIS incidence (adjusted odds ratio [OR], 1.17 [95% CI, 1.11 to 1.23]) but no difference in invasive breast cancer incidence (adjusted OR, 1.00 [CI, 0.97 to 1.03]). Among women with invasive cancer, CAD was associated with greater likelihood of stage I to II versus III to IV cancer (adjusted OR, 1.27 [CI, 1.14 to 1.41]). In women without breast cancer, CAD was associated with increased odds of diagnostic mammography (adjusted OR, 1.28 [CI, 1.27 to 1.29]), breast ultrasonography (adjusted OR, 1.07 [CI, 1.06 to 1.09]), and breast biopsy (adjusted OR, 1.10 [CI, 1.08 to 1.12])., Limitation: Short follow-up for cancer stage, potential unmeasured confounding, and uncertain generalizability to younger women., Conclusion: Use of CAD during screening mammography among Medicare enrollees is associated with increased DCIS incidence, the diagnosis of invasive breast cancer at earlier stages, and increased diagnostic testing among women without breast cancer., Primary Funding Source: Center for Healthcare Policy and Research, University of California, Davis.

Background: While Medicare claims are a potential resource for clinical mammography research or quality monitoring, the validity of key data elements remains uncertain. Claims codes for digital mammography and computer-aided detection (CAD), for example, have not been validated against a credible external reference standard., Methods: We matched Medicare mammography claims for women who received bilateral mammograms from 2003 to 2006 to corresponding mammography data from the Breast Cancer Surveillance Consortium (BCSC) registries in four U.S. states (N = 253,727 mammograms received by 120,709 women). We assessed the accuracy of the claims-based classifications of bilateral mammograms as either digital versus film and CAD versus non-CAD relative to a reference standard derived from BCSC data., Results: Claims data correctly classified the large majority of film and digital mammograms (97.2% and 97.3%, respectively), yielding excellent agreement beyond chance (κ = 0.90). Claims data correctly classified the large majority of CAD mammograms (96.6%) but a lower percentage of non-CAD mammograms (86.7%). Agreement beyond chance remained high for CAD classification (κ = 0.83). From 2003 to 2006, the predictive values of claims-based digital and CAD classifications increased as the sample prevalences of each technology increased., Conclusion: Medicare claims data can accurately distinguish film and digital bilateral mammograms and mammograms conducted with and without CAD., Impact: The validity of Medicare claims data regarding film versus digital mammography and CAD suggests that these data elements can be useful in research and quality improvement., (©2012 AACR.)

Purpose: The purpose of this investigation was to assess the anatomical noise in breast images using a mathematically derived parameter β as a surrogate for detection performance, across the same patient cohort but in different imaging modalities including mammography, tomosynthesis, and breast CT., Methods: Women who were scheduled for breast biopsy were approached for participation in this IRB and HIPPA-compliant investigation. A total of 23 women had all views of each modality and represent the cohort studied in this investigation. Image data sets across all modalities were analyzed using 1000 regions of interest per image data set, and the anatomical noise power spectrum, NPS(a)(f), was computed and averaged for each breast image data set. After windowing the total noise power spectrum NPS(t)(f) to a specific frequency range corresponding to anatomical noise, the power-law slope (β) of the NPS(a)(f) was computed where NPS(a)(f) = α f(-) (β)., Results: The value of β was determined for breast CT data sets, and they were 1.75 (0.424), 1.83 (0.352), and 1.79 (0.397), for the coronal, sagittal, and axial views, respectively. For tomosynthesis, β was 3.06 (0.361) and 3.10 (0.315) for the craniocaudal (CC) and medial lateral oblique (MLO) views, respectively. For mammography, these values were 3.17 (0.226) and 3.30 (0.236), for the CC and MLO views, respectively. The values of β for breast CT were significantly different than those for tomosynthesis and mammography (p < 0.001, all 12 comparisons)., Conclusions: Based on the parameter β which is thought to describe anatomical noise in breast images, breast CT was shown to have a statistically significant lower β than mammography or tomosynthesis. It has been suggested in the literature that a lower β may correspond to increased cancer detection performance; however, this has yet to be demonstrated unequivocally.

Purpose: To develop an automated method to detect breast masses on dedicated breast CT (BCT) volumes and to conduct a preliminary evaluation of its performance. This method can be used in a computer-aided detection (CADe) system for noncontrast enhanced BCT., Methods: The database included patient images, which were acquired under an IRB-approved protocol. The database in this study consisted of 132 cases. 50 cases contained 58 malignant masses, and 23 cases contained 24 benign masses. 59 cases did not contain any biopsy-proven lesions. Each case consisted of an unenhanced CT volume of a single breast. First, each breast was segmented into adipose and glandular tissues using a fuzzy c-means clustering algorithm. The glandular breast regions were then sampled at a resolution of 2 mm. At each sampling step, a 3.5-cm(3) volume-of-interest was subjected to constrained region segmentation and 17 characteristic features were extracted, yielding 17 corresponding feature volumes. Four features were selected using step-wise feature selection and merged with linear discriminant analysis trained in the task of distinguishing between normal breast glandular regions and masses. Detection performance was measured using free-response receiver operating characteristic analysis (FROC) with leave-one-case-out evaluation., Results: The feature selection stage selected features that characterized the shape and margin strength of the segmented region. CADe sensitivity per case was 84% (std = 4.2%) at 2.6 (std = 0.06) false positives per volume, or 6 × 10(-3) per slice (at an average of 424 slices per volume in this data set)., Conclusions: This preliminary study demonstrates the feasibility of our approach for CADe for BCT.

Purpose: Accurate anatomical characterization of the breast is useful in breast phantom development and computer modeling of breast imaging technologies. Capitalizing on the three-dimensional capabilities of dedicated breast CT (bCT), a number of parameters which describe breast shape and fibroglandular distribution are defined., Methods: Among 219 bCT data sets, the effective diameter and length of the pendant breast as well as the breast volume were measured and characterized for each bra cup size. The volume glandular fraction (VGF) was determined as a function of patient age, BIRADS density, bra cup size, and breast diameter. The glandular fraction was examined in coronal and sagittal planes of the breast, and the radial distribution of breast glandular fraction within a coronal bCT image was examined for three breast regions. The areal glandular fraction (AGF) was estimated from two-dimensional projections of the breast (simulated by projecting bCT data sets) and was compared to the corresponding VGF., Results: The effective breast diameter and length increase with increasing bra cup size. The mean breast diameters (+/- standard error) of bra cup sizes A/AA, B, C, and D/DD were 11.1 +/- 0.5, 11.4 +/- 0.3, 13.0 +/- 0.2, and 13.7 +/- 0.2 cm, respectively. VGF was lower among older women and those with larger breast diameter and larger bra cup size. VGF increased as a function of the reported BIRADS density. AGF increased with VGF. Fibroglandular tissue was distributed primarily in the central portion of the breast., Conclusions: Breast metrics were examined and a number of parameters were defined which may be useful for breast modeling. The reported data may provide researchers with useful information for characterizing the breast for various imaging or dosimetry tasks.

Dedicated breast computed tomography (DBCT) is a burgeoning technology that has many advantages over current breast-imaging systems. Three-dimensional visualization of the breast mitigates the limiting effects of superimposition noted with mammography. Postprocessing capabilities will allow application of advanced technologies, such as creation of maximum-intensity projection and subtraction images, and the use of both computer-aided detection and possible computer-aided diagnosis algorithms. Excellent morphologic detail and soft tissue contrast can be achieved, due in part to the isotropic image data that DBCT produces. The expected cost should be more reasonable than magnetic resonance imaging. At present, because the breast is not compressed, patients find it more comfortable than mammography. Physiologic information can be obtained when intravenous contrast material is used and/or when DBCT is combined with single photon emission-computed tomography or positron emission tomography. DBCT provides an excellent platform for multimodality systems including integration with interventional and therapeutic procedures. Purpose: To quantify contrast material enhancement of breast lesions scanned with dedicated breast computed tomography (CT) and to compare their conspicuity with that at unenhanced breast CT and mammography., Materials and Methods: Approval of the institutional review board and the Radiation Use Committee and written informed consent were obtained for this HIPAA-compliant study. Between September 2006 and April 2009, 46 women (mean age, 53.2 years; age range, 35-72 years) with Breast Imaging Reporting and Data System category 4 or 5 lesions underwent unenhanced breast CT and contrast material-enhanced breast CT before biopsy. Two radiologists independently scored lesion conspicuity for contrast-enhanced breast CT versus mammography and for contrast-enhanced breast CT versus unenhanced breast CT. Mean lesion voxel intensity was measured in Hounsfield units and normalized to adipose tissue intensity on manually segmented images obtained before and after administration of contrast material. Regression models focused on conspicuity and quantified enhancement were used to estimate the effect of pathologic diagnosis (benign vs malignant), lesion type (mass vs calcifications), breast density, and interradiologist variability., Results: Fifty-four lesions (25 benign, 29 malignant) in 46 subjects were analyzed. Malignant lesions were seen significantly better at contrast-enhanced breast CT than at unenhanced breast CT (P < .001) or mammography (P < .001). Malignant calcifications (malignant lesions manifested mammographically as microcalcifications only, n = 7) were seen better at contrast-enhanced breast CT than at unenhanced breast CT (P < .001) and were seen similarly at contrast-enhanced breast CT and mammography. Malignant lesions enhanced 55.9 HU +/- 4.0 (standard error), whereas benign lesions enhanced 17.6 HU +/- 6.1 (P < .001). Ductal carcinoma in situ (n = 5) enhanced a mean of 59.6 HU +/- 2.8. Receiver operating characteristic curve analysis of lesion enhancement yielded an area under the receiver operating characteristic curve of 0.876., Conclusion: Conspicuity of malignant breast lesions, including ductal carcinoma in situ, is significantly improved at contrast-enhanced breast CT. Mammography has served the population of women who are at-risk for breast cancer well over the past 30 years. While mammography has undergone a number of changes as digital detector technology has advanced, other modalities such as computed tomography have experienced technological sophistication over this same time frame as well. The advent of large field of view flat panel detector systems enable the development of breast CT and several other niche CT applications, which rely on cone beam geometry. The breast, it turns out, is well suited to cone beam CT imaging because the lack of bones reduces artifacts, and the natural tapering of the breast anteriorly reduces the x-ray path lengths through the breast at large cone angle, reducing cone beam artifacts as well. We are in the process of designing a third prototype system which will enable the use of breast CT for image guided interventional procedures. This system will have several copies fabricated so that several breast CT scanners can be used in a multi-institutional clinical trial to better understand the role that this technology can bring to breast imaging.

Unlabelled: We have constructed a dedicated breast PET/CT scanner capable of high-resolution functional and anatomic imaging. Here, we present an initial characterization of scanner performance during patient imaging., Methods: The system consisted of a lutetium oxyorthosilicate-based dual-planar head PET camera (crystal size, 3 x 3 x 20 mm) and 768-slice cone-beam CT. The position of the PET heads (separation and height) could be adjusted for varying breast dimensions. For scanning, the patient lay prone on a specialized bed and inserted a single pendent breast through an aperture in the table top. Compression of the breast as used in mammography is not required. PET and CT systems rotate in the coronal plane underneath the patient sequentially to collect fully tomographic datasets. PET images were reconstructed with the fully 3-dimensional maximum a posteriori method, and CT images were reconstructed with the Feldkamp algorithm, then spatially registered and fused for display. Phantom scans were obtained to assess the registration accuracy between PET and CT images and the influence of PET electronics and activity on CT image quality. We imaged 4 women with mammographic findings highly suggestive of breast cancer (breast imaging reporting and data system, category 5) in an ongoing clinical trial. Patients were injected with (18)F-FDG and imaged for 12.5 min per breast. From patient data, noise-equivalent counting rates and the singles-to-trues ratio (a surrogate for the randoms fraction) were calculated., Results: The average registration error between PET and CT images was 0.18 mm. PET electronics and activity did not significantly affect CT image quality. For the patient trial, biopsy-confirmed cancers were visualized on dedicated breast PET/CT on all patient scans, including the detection of ductal carcinoma in situ in 1 case. The singles-to-trues ratio was found to be inversely correlated with breast volume in the field of view, suggesting that larger breasts trend toward increased noise-equivalent counting rates for all other things equal., Conclusion: Scanning of the uncompressed breast with dedicated breast PET/CT can accurately visualize suspected lesions in 3 dimensions.

Rationale and Objectives: Improvements in the diagnosis of early breast cancers depend on a physician's ability to obtain the information necessary to distinguish nonpalpable malignant and benign tumors. Viscoelastic features that describe mechanical properties of tissues may help to distinguish these types of lesions., Materials and Methods: Twenty-one patients with nonpalpable, pathology-confirmed Breast Imaging Reporting and Data System (BIRADS) 4 or 5 breast lesions (10 benign, 11 malignant) detected by mammography were studied. Viscoelastic parameters were extracted from a time sequence of ultrasonic strain images, and differences in the parameters between malignant and benign tumors were compared. Parametric data were color coded and superimposed on sonograms., Results: The strain retardance time parameter, T(1), provided the best discrimination between malignant and benign tumors (P < .01). T(1) measures the time required for tissues to fully deform (strain) once compressed; therefore, it describes the time-varying viscous response of tissue to a small deforming force. Compared to the surrounding background tissues, malignant lesions have smaller average T(1) values, whereas benign lesions have higher T(1) values. This tissue-specific contrast correlates with known changes in the extracellular matrix of breast stroma., Conclusion: Characterization of nonpalpable breast lesions is improved by the addition of viscoelastic strain imaging parameters. The differentiation of malignant and benign BI-RADS 4 or 5 tumors is especially evident with the use of the retardation time estimates, T(1).

Differences in breast tissue composition are important determinants in assessing risk, identifying disease in images and following changes over time. This paper presents an algorithm for tissue classification that separates breast tissue into its three primary constituents of skin, fat and glandular tissue. We have designed and built a dedicated breast CT scanner. Fifty-five normal volunteers and patients with mammographically identified breast lesions were scanned. Breast CT voxel data were filtered using a 5 pt median filter and the image histogram was computed. A two compartment Gaussian fit of histogram data was used to provide an initial estimate of tissue compartments. After histogram analysis, data were input to region-growing algorithms and classified as to belonging to skin, fat or gland based on their value and architectural features. Once tissues were classified, a more detailed analysis of glandular tissue patterns and a more quantitative analysis of breast composition was made. Algorithm performance assessment demonstrated very good or excellent agreement between algorithm and radiologist observers in 97.7% of the segmented data. We observed that even in dense breasts the fraction of glandular tissue seldom exceeded 50%. For most individuals the composition is better characterized as being a 70% (fat)-30% (gland) composition than a 50% (fat)-50% (gland) composition.

Purpose: To prospectively and intraindividually compare dedicated breast computed tomographic (CT) images with screen-film mammograms., Materials and Methods: All patient studies were performed according to protocols approved by the institutional review board and Radiation Use Committee; informed consent was obtained. A breast CT scanner prototype was used to individually scan uncompressed breasts in 10 healthy volunteers (mean age, 52.1 years) and 69 women with Breast Imaging Reporting and Data System category 4 and 5 lesions (mean age, 54.4 years). In women with lesions, breast CT images were compared with screen-film mammograms by an experienced mammographer and ranked with a continuous scale of 1-10 (score 1, excellent lesion visualization with CT and poor visualization with mammography; score 5.5, equal visualization with both modalities; and score 10, poor visualization with CT and excellent visualization with mammography). A Wilcoxon signed rank procedure was used to test the null hypothesis that ratings were symmetric at about a score of 5.5 for the entire group and for distinguishing microcalcifications versus masses and other findings and benign versus malignant lesions and for effect of breast density on lesion visualization. Women were asked to compare their comfort during CT with that during mammography on a continuous scale of 1-10. With a Wilcoxon signed rank procedure, the null hypothesis that comfort ratings were symmetric about a score of 5.5 (equal comfort with CT and mammography) was tested., Results: Overall, CT was equal to mammography for visualization of breast lesions. Breast CT was significantly better than mammography for visualization of masses (P = .002); mammography outperformed CT for visualization of microcalcifications (P = .006). No significant differences between CT and mammography were seen among benign versus malignant lesions or for effect of breast density on lesion visualization. Subjects found CT significantly more comfortable than mammography (P < .001)., Conclusion: Some technical challenges remain, but breast CT is promising and may have potential clinical applications., ((c) RSNA, 2008.)

Although screening mammography has been shown to be effective in reducing breast cancer mortality, a new technique called breast computed tomography (CT) is being studied in the hope that breast cancer can be detected even earlier. A prototype unit has been designed, fabricated and tested at the University of California, Davis, USA, and is currently being used in a Phase II clinical trial to study the feasibility of breast CT for breast cancer detection and diagnosis. A total of 46 volunteers and patients have been imaged, and the breast CT images show impressive anatomical detail of the breast that is not appreciated in mammography. The radiation dose levels needed to produce excellent image quality are equal to two-view mammography. Further study of breast CT is needed to better understand its potential role in breast cancer screening and diagnosis.

Purpose: To analyze the cost-effectiveness of adding computer-aided detection (CAD) to a screening mammography program., Materials and Methods: A Markov model was developed to compare three hypothetical groups of women aged 40-79 years. The first group was composed of women undergoing mammographic screening without CAD; the second, of women undergoing mammographic screening with CAD; and the third, of women undergoing observation without screening. Cost-effectiveness was expressed as the marginal cost per year of life saved (MCYLS). MCYLS was calculated for screening mammography with CAD compared with screening mammography alone and for screening mammography alone compared with observation. Sensitivity analyses were performed by varying the cost of CAD, the rates of cancer detection with CAD, and the stage distribution of breast cancers diagnosed with CAD., Results: Adding CAD to a mammographic screening program resulted in a MCYLS of $19,058. The MCYLS of screening mammography alone compared with observation was $16,023. CAD increases the marginal effectiveness of screening by 29%; however, the marginal cost of screening is also increased by 34%. Varying the cost of CAD yields a linear increase in MCYLS from $8937 with CAD at $9 per case to $24,924 with CAD at $25 per case. The cost-effectiveness of CAD is dependent on the magnitude of the increase in cancer detection rates with CAD but is also affected by the stage distribution of cancers diagnosed with CAD., Conclusion: The MCYLS is 19% greater for CAD added to screening versus screening mammography alone but is still within the accepted range for cost-effectiveness., (Copyright (c) RSNA, 2006.)

Despite the success of screening mammography contributing to the reduction of cancer mortality, a number of other imaging techniques are being studied for breast cancer screening. In our laboratory, a dedicated breast computed tomography (CT) system has been developed and is currently undergoing patient testing. The breast CT system is capable of scanning the breast with the woman lying prone on a tabletop, with the breast in the pendant position. A 360 degrees scan currently requires 16.6 s, and a second scanner with a 9-second scan time is nearly operational. Extensive effort was placed on computing the radiation dose to the breast under CT geometry, and the scan parameters are selected to utilize the same radiation dose levels as two-view mammography. A total of 55 women have been scanned, ten healthy volunteers in a Phase I trial, and 45 women with a high likelihood of having breast cancer in a Phase II trial. The breast CT process leads to the production of approximately three hundred 512 x 512 images for each breast. Subjective evaluation of the breast CT images reveals excellent anatomical detail, good depiction of microcalcifications, and exquisite visualization of the soft tissue components of the tumor when contrasted against adipose tissues. The use of iodine contrast injection dramatically enhances the visualization of tumors. While a thorough scientific investigation based upon observer performance studies is in progress, initial breast CT images do appear promising and it is likely that breast CT will play some role in breast cancer imaging.

We investigate and extend the ideal observer methodology developed by Smith and Wagner to detection and discrimination tasks related to breast sonography. We provide a numerical approach for evaluating the ideal observer acting on radio frequency (RF) frame data, which involves inversion of large nonstationary covariance matrices, and we describe a power-series approach to computing this inverse. Considering a truncated power series suggests that the RF data be Wiener-filtered before forming the final envelope image. We have compared human performance for Wiener-filtered and conventional B-mode envelope images using psychophysical studies for 5 tasks related to breast cancer classification. We find significant improvements in visual detection and discrimination efficiency in four of these five tasks. We also use the Smith-Wagner approach to distinguish between human and processing inefficiencies, and find that generally the principle limitation comes from the information lost in computing the final envelope image.

The use of breast computed tomography (CT) as an alternative to mammography in some patients is being studied at several institutions. However, the radiation dosimetry issues associated with breast CT are markedly different than in the case of mammography. In this study, the spectral properties of an operational breast CT scanner were characterized both by physical measurement and computer modeling of the kVp-dependent spectra, from 40 to 110 kVp (Be window W anode with 0.30 mm added Cu filtration). Previously reported conversion factors, normalized glandular dose for CT-DgN(ct), derived from Monte Carlo methods, were used in concert with the output spectra of the breast scanner to compute the mean glandular dose to the breast based upon different combinations of x-ray technique factors (kVp and mAs). The mean glandular dose (MGD) was measured as a function of the compressed breast thickness (2-8 cm) and three different breast compositions (0%, 50%, and 100% glandular fractions) in four clinical mammography systems in our institution. The average MGD from these four systems was used to compute the technique factors for breast CT systems that would match the two-view mammographic dose levels. For a 14 cm diameter breast (equivalent to a 5 cm thick compressed breast in mammography), air kerma levels at the breast CT scanner's isocenter (468 mm from the source) of 4.4, 6.4, and 9.0 mGy were found to deliver equivalent mammography doses for 0%, 50%, and 100% glandular breasts (respectively) at 80 kVp. At 80 kVp (where air kerma was 11.3 mGy/100 mAs at the isocenter), 57 mAs (integrated over the entire scan) was required to match the mammography dose for a 14 cm 50% glandular breast. At 50 kVp, 360 mAs is required to match mammographic dose levels. Tables are provided for both air kerma at the isocenter and mAs for 0%, 50%, and 100% glandular breasts. Other issues that impact breast CT technique factors are also discussed.

Over the past several years, digital mammography systems have been installed clinically across North America in small but growing numbers. A photostimulable phosphor-based full-field digital mammography image was evaluated in this investigation. Commonly known as computed radiography (CR), its use closely mimics the screen-film mammography paradigm. System performance using modulation transfer function (MTF) and detective quantum efficiency (DQE) metrics show MTF(2.5 mm(-1)) = 0.5, DQE(2.5 mm(-1)) = 0.3, and MTF(5.0 mm(-1)) = 0.2, DQE(5.0 mm(-1)) = 0.05, for a 26 kVp beam, 0.03 mm molybdenum tube filtration, 4.5 cm tissue attenuation, and 15 mR incident exposure to the detector. Slightly higher DQE values were measured at 32 kVp with 0.025 mm rhodium tube filtration. CR mammography advantages include the ability to use existing mammography machines, where multiple rooms can be converted to "digital" operation, which allows overall cost savings compared to integrated digital mammography systems. Chief disadvantages include the labor-intensive handling of the cassettes prior to and after the imaging exam, lack of a direct interface to the x-ray system for recording technique parameters, and relatively slow processing time. Clinical experience in an IRB-approved research trial has suggested that digital mammography with photostimulable storage phosphors and a dedicated CR reader is a viable alternative to conventional screen-film mammography.

Imaging systems are most effective for detection and classification when they exploit contrast mechanisms specific to particular disease processes. A common example is mammography, where the contrast depends on local changes in cell density and the presence of microcalcifications. Unfortunately the specificity for classifying malignant breast disease is relatively low for many current diagnostic techniques. This paper describes a new ultrasonic technique for imaging the viscoelastic properties of breast tissue. The mechanical properties of glandular breast tissue, like most biopolymers, react to mechanical stimuli in a manner specific to the microenvironment of the tissue. Elastic properties allow noninvasive imaging of desmoplasia while viscous properties describe metabolism-dependent features such as pH. These ultrasonic methods are providing new tools for studying disease mechanisms as well as improving diagnosis.

Objectives: A mathematical model is presented that allows the computation of the sensitivity and specificity of breast screening based on receiver-operating characteristic (ROC) curve shape, the positive predictive value (PPV) of screening mammography, and the cancer incidence, f., Methods: The normal and cancer populations are modeled as normal distributions with independent means and standard deviations. The distributions are scaled such that the area of the normal population is equal to 1-f and that of the cancer population is f. The PPV for screening mammography is used to determine the operating point on the ROC curve. Knowing this leads directly to the computation of sensitivity and specificity. The derivation is general and is applicable to both symmetrical and asymmetrical ROC curves., Results: For symmetric ROC curves and typical values for the PPV of mammography (about 8%) and cancer incidence (f= 0.003), an A(z) value of 0.95 was required to achieve 63% sensitivity and an A. value of 0.98 led to 86% sensitivity., Conclusion: A model was developed that should allow researchers to deduce sensitivity and specificity for screening mammography based on ROC curve measurements and using realistic values of PPV and f. This model allows A. values to be related to the probability of breast cancer detection.

Purpose: To evaluate the feasibility of breast computed tomography (CT) in terms of radiation dose and image quality., Materials and Methods: Validated Monte Carlo simulation techniques were used to estimate the average glandular dose (AGD). The calculated photon fluence at the detector for high-quality abdominal CT (120 kVp, 300 mAs, 5-mm section thickness) was the benchmark for assessing the milliampere seconds and corresponding radiation dose necessary for breast CT. Image noise was measured by using a 10-cm-diameter cylinder imaged with a clinical CT scanner at 10-300 mAs for 80, 100, and 120 kVp. A cadaveric breast was imaged in the coronal plane to approximate the acquisition geometry of a proposed breast CT scanner., Results: The AGD for 80-kVp breast CT was comparable to that for two-view mammography of 5-cm breasts (compressed breast thickness). For thicker breasts, the breast CT dose was about one-third less than that for two-view mammography. The maximum dose at mammography assessed in 1-mm(3) voxels was far higher (20.0 mGy) than that at breast CT (5.4 mGy) for a typical 5-cm 50% glandular breast. CT images of an 8-cm cadaveric breast (AGD, 6.3 mGy) were subjectively superior to digital mammograms (AGD, 10.1 mGy) of the same specimen., Conclusion: The potential of high signal-to-noise ratio images with low anatomic noise, which are obtainable at dose levels comparable to those for mammography, suggests that dedicated breast CT should be studied further for its potential in breast cancer screening and diagnosis.

Purpose: To compare the stress experienced by women who had false-positive screening mammograms and had undergone immediate on-site diagnostic imaging evaluation with that experienced by those who had been recalled for subsequent imaging., Materials and Methods: A retrospective survey was mailed to women with false-positive screening mammograms that had proved negative or benign at diagnostic imaging. The women were divided into (a) those who had undergone diagnostic imaging during the same appointment as their screening examination and (b) those who had returned at a later date for work-up. The survey included questions about stress that was related to the screening and diagnostic experiences, how subjects had been notified about screening results, and their breast health histories., Results: From the group that had undergone immediate work-up (n = 100), 50 eligible surveys were received; 71 were received from women who had undergone later work-ups (n = 176). The self-reported overall stress was significantly greater (P =.027) in the group recalled for subsequent diagnostic imaging. Among all respondents, stress from a false-positive screening result was greatest in women younger than 50 years of age with a positive first-degree family history of breast cancer., Conclusion: Providing immediate on-site diagnostic evaluation can reduce the stress of a false-positive screening mammogram.

Monte Carlo procedures using the SIERRA code (validated in a companion article) were used to investigate the scatter properties in mammography. The scatter to primary ratio (SPR) was used for quantifying scatter levels as a function of beam spectrum, position in the field, air gap, breast thickness, tissue composition, and the area of the field of view (FOV). The geometry of slot scan mammography was also simulated, and SPR values were calculated as a function of slot width. The influence of large air gaps (to 30 cm) was also studied in the context of magnification mammography. X-ray energy and tissue composition from 100% adipose to 100% glandular demonstrated little effect on the SPR. Air gaps over a range from 0 to 30 mm showed only slight effects. The SPR increased with increased breast thickness and with larger fields of view. Measurements from 82 mammograms provided estimates of the range of compressed breast thickness (median: 5.2 cm, 95% range: 2.4 cm to 7.9 cm) and projected breast area onto the film (left craniocaudal view, median: 146 cm2, 95% range: 58 cm2 to 298 cm2). SPR values for semicircular breast shapes, Mo/Mo spectra, and a 15 mm air gap were parametrized as a function of breast thickness and (semicircular) breast diameter. With the coefficients a = - 2.35452817439093, b = 22.3960980055927, and c = 8.85064260299289, the equation SPR= [a + b x (diameter in cm)--(-1.5) + c x (thickness in cm) --(-0.5)]-- -1 produces SPR data from 2 to 8 cm and from 3 to 30 cm breast diameters with an average error of about 1%.

The purpose of this study was to develop and evaluate a computerized method of calculating a breast density index (BDI) from digitized mammograms that was designed specifically to model radiologists' perception of breast density. A set of 153 pairs of digitized mammograms (cranio-caudal, CC, and mediolateral oblique, MLO, views) were acquired and preprocessed to reduce detector biases. The sets of mammograms were ordered on an ordinal scale (a scale based only on relative rank-ordering) by two radiologists, and a cardinal (an absolute numerical score) BDI value was calculated from the ordinal ranks. The images were also assigned cardinal BDI values by the radiologists in a subsequent session. Six mathematical features (including fractal dimension and others) were calculated from the digital mammograms, and were used in conjunction with single value decomposition and multiple linear regression to calculate a computerized BDI. The linear correlation coefficient between different ordinal ranking sessions were as follows: intraradiologist intraprojection (CC/CC): r = 0.978; intraradiologist interprojection (CC/MLO): r = 0.960; and interradiologist intraprojection (CC/CC): r = 0.968. A separate breast density index was derived from three separate ordinal rankings by one radiologist (two with CC views, one with the MLO view). The computer derived BDI had a correlation coefficient (r) of 0.907 with the radiologists' ordinal BDI. A comparison between radiologists using a cardinal scoring system (which is closest to how radiologists actually evaluate breast density) showed r = 0.914. A breast density index calculated by a computer but modeled after radiologist perception of breast density may be valuable in objectively measuring breast density. Such a metric may prove valuable in numerous areas, including breast cancer risk assessment and in evaluating screening techniques specifically designed to improve imaging of the dense breast.

Objective: The stress experienced by women undergoing surveillance mammography for benign lesions was compared with that experienced by women undergoing core biopsy of benign lesions., Materials and Methods: A retrospective survey was mailed to women in whom were diagnosed breast abnormalities that were likely to be benign and that were stable at short-interval follow-up mammography (n = 174) or in whom a core biopsy of the breast was performed, with benign findings (n = 116). The survey included questions about stress related to the diagnostic experience as well as questions about who advised the women of the results of their breast imaging studies and about the information provided., Results: Eighty eligible surveys were received from the women who underwent short-interval follow-up mammography, and 58 surveys were received from the women who underwent biopsies. The self-reported overall stress experienced by the women who underwent core biopsies was significantly greater (p < .001) than that reported by the group who were followed up with mammography. The level of anxiety was not affected by the patient's perception of the probability that the lesion represented carcinoma. We found no significant differences in stress between women who discussed their mammogram results with a radiologist and women who were notified by their primary care provider., Conclusion: Short-interval follow-up mammography continues to be acceptable for the evaluation of lesions that are probably benign, even when patient anxiety is considered.

Background: Several recent studies have added significant information regarding the benefit of screening mammography, especially in the 40-49-years age group. This new information makes it important to reassess the cost-effectiveness of screening., Methods: A Markov model was used to study the cost-effectiveness of 4 age-related screening strategies: 1) annually from ages 40-79 years; 2) annually from ages 40-64 years and biennially from ages 65-79 years; 3) annually from ages 40-49 years and biennially from ages 50-79 years; and 4) annually from ages 40-79 years in high risk women (10%) and biennially from ages 40-49 years followed by annually from ages 50 -79 years in normal risk women (90%). An additional strategy simulating hormone status and estrogen exposure was evaluated. Cost-effectiveness was expressed as marginal cost per year-life saved (MCYLS)., Results: The MCYLS varied from $18,800 to $16,100. For all strategies this was within the range of other generally acceptable diagnostic and therapeutic medical procedures. There was a 14% decrease in MCYLS from the least cost-effective to the most cost-effective strategy., Conclusions: Cost-effectiveness of four age-related mammographic screening strategies was evaluated. The MCYLS for all strategies was within a generally accepted range. With increasing concerns regarding the cost of health care, this information may be useful in health policy decision-making.

Objective: To compare and analyze the cost-effectiveness of different mammographic screening strategies., Design: A computer simulation model was developed to compare mammographic screening with observation without screening. Cost-effectiveness was expressed as marginal cost per year of life saved (MCYLS) and was calculated for the following mammographic screening strategies: (1) annual for ages 40 to 79 years; (2) annual for ages 50 to 79 years; (3) biennial for ages 50 to 79 years; (4) annual for ages 40 to 49 years with biennial for ages 50 to 79 years; (5) annual for ages 40 to 64 years with biennial for ages 65 to 79 years; (6) biennial for ages 40 to 49 years with annual for ages 50 to 79 years; and (7) annual for high-risk and biennial for normal-risk women aged 40 to 49 years with annual for ages 50 to 79 years., Data Sources: The probability and cost of all outcomes were established from previously published data or community experience., Results: The most cost-effective screening strategy is biennial mammography for women aged 50 to 79 years, with an MCYLS of $16,000. Adding annual mammography for women aged 40 to 49 years increases the MCYLS to $20,200, but is more cost-effective than other tested protocols that included women in their 40s; annual mammography for ages 40 to 49 years with biennial for ages 50 to 79 years is also more cost-effective than annual mammography for ages 50 to 79 years., Conclusion: Screening programs that include women in their 40s can be as cost-effective as some that exclude such women. Choice of a screening strategy depends on financial resources and desired effectiveness.

Purpose: To evaluate the cost-effectiveness of screening mammography in women 40-49 years old., Materials and Methods: A Markov model compared two hypothetical groups; one underwent screening mammography and the other, observation without mammography. Variables tested included frequency of mammography, reduction in breast cancer mortality, and cost-effectiveness by age group. Results were expressed as marginal cost per year of life saved., Results: If the estimated mortality reduction from mammographic screening was 15% or greater, the marginal cost per year of life saved was comparable to that of other generally accepted medical procedures. Annual screening with an assumed 30% reduction in mortality had a cost-effectiveness similar to that of biennial screening with a 20% reduction. Screening mammography was less cost-effective in the 40-49-year-old age group than in women aged 50-79 years but more cost-effective than in those aged 80-84 years., Conclusion: Although the parameters for screening mammography in women aged 40-49 years are not known with certainty, the results of this analysis may help establish priorities for utilization of medical resources.

Purpose: To analyze the cost-effectiveness of needle core biopsy (NCB) in a breast cancer screening program., Materials and Methods: A computer model was used to compare two hypothetical groups of women: those who undergo annual mammographic screening and those observed without screening. All clinically significant outcomes for each group were included in the model, which reflected the state of a woman's health each year beginning at age 40 years and ending at age 85 years., Results: The marginal cost per year of life saved by screening was reduced a maximum of 23% (from $20,770 to $15,934) with use of NCB instead of surgical biopsy. The cost of NCB had less effect on cost-effectiveness than the number of women who underwent NCB., Conclusion: When used for the same indications as surgical biopsy, NCB substantially reduces the cost of a breast cancer screening program.

The measurement of resistive index (RI = [peak systolic velocity--end diastolic velocity]/peak systolic velocity) by Doppler sonography has demonstrated variable reliability as an indicator of pediatric urinary obstruction. By modifying Doppler studies with the addition of furosemide (diuretic Doppler sonography), we previously found significant differences between 10 nonobstructed and 10 obstructed kidneys in children (median age 7 months). The obstructed kidneys have since undergone surgical repair, and postoperative reevaluation has been performed by diuretic Doppler sonography and diuretic renography. Diuretic Doppler sonography was performed on well hydrated catheterized patients, with resistive index measurement of the renal interlobar and arcuate arteries obtained before and 10 minutes after 1 mg./kg. furosemide infusion. Following surgical repair of obstruction all 10 kidneys had stable glomerular filtration rate with improved pelvic emptying times as demonstrated by half-time. Of 6 kidneys evaluated by diuretic Doppler sonography before 3 months 2 had resistive index levels greater than 75. Of the 9 kidneys measured at 3 months or more postoperatively all had resistive index values of less than 75, even after furosemide infusion (5 kidneys underwent repeat evaluation). In our study the previously demonstrated post-diuretic elevation of resistive index in pediatric urinary obstruction was eventually reversed following surgical repair. Diuretic Doppler sonography appears to be a promising noninvasive method for evaluating pediatric hydronephrosis, providing an alternative physiological parameter with which to measure renal obstruction.
