BACKGROUND:Heatwaves have been linked to increased risk of mortality and morbidity and are projected to increase in frequency and intensity due to climate change. The current study uses emergency department (ED) data from Australia, Botswana, Netherlands, Pakistan, and the United States of America to evaluate the impact of heatwaves on ED attendances, admissions and mortality. METHODS:Routinely collected time series data were obtained from 18 hospitals. Two separate thresholds (≥4 and ≥7) of the acclimatisation excess heat index (EHIaccl) were used to define "hot days". Analyses included descriptive statistics, independent samples T-tests to determine differences in case mix between hot days and other days, and threshold regression to determine which temperature thresholds correspond to large increases in ED attendances. FINDINGS:In all regions, increases in temperature that did not coincide with time to acclimatise resulted in increases in ED attendances, and the EHIaccl performed in a similar manner. During hot days in California and The Netherlands, significantly more children ended up in the ED, while in Pakistan more elderly people attended. Hot days were associated with more patient admissions in the ages 5-11 in California, 65-74 in Karachi, and 75-84 in The Hague. During hot days in The Hague, patients with psychiatric symptoms were more likely to die. The current study did not identify a threshold temperature associated with particularly large increases in ED demand. INTERPRETATION:The association between heat and ED demand differs between regions. A limitation of the current study is that it does not consider delayed effects or influences of other environmental factors. Given the association between heat and ED use, hospitals and governmental authorities should recognise the demands that heat can place on local health care systems. These demands differ substantially between regions, with Pakistan being the most heavily affected within our study sample.

Parallaxes for 331 classical Cepheids, 31 Type II Cepheids and 364 RR Lyrae stars in common between Gaia and the Hipparcos and Tycho-2 catalogues are published in Gaia Data Release 1 (DR1) as part of the Tycho-Gaia Astrometric Solution (TGAS). In order to test these first parallax measurements of the primary standard candles of the cosmological distance ladder, that involve astrometry collected by Gaia during the initial 14 months of science operation, we compared them with literature estimates and derived new period-luminosity ($PL$), period-Wesenheit ($PW$) relations for classical and Type II Cepheids and infrared $PL$, $PL$-metallicity ($PLZ$) and optical luminosity-metallicity ($M_V$-[Fe/H]) relations for the RR Lyrae stars, with zero points based on TGAS. The new relations were computed using multi-band ($V,I,J,K_{\mathrm{s}},W_{1}$) photometry and spectroscopic metal abundances available in the literature, and applying three alternative approaches: (i) by linear least squares fitting the absolute magnitudes inferred from direct transformation of the TGAS parallaxes, (ii) by adopting astrometric-based luminosities, and (iii) using a Bayesian fitting approach. TGAS parallaxes bring a significant added value to the previous Hipparcos estimates. The relations presented in this paper represent first Gaia-calibrated relations and form a "work-in-progress" milestone report in the wait for Gaia-only parallaxes of which a first solution will become available with Gaia's Data Release 2 (DR2) in 2018., Comment: 29 pages, 25 figures. Context. The first Gaia Data Release contains the Tycho-Gaia Astrometric Solution (TGAS). This is a subset of about 2 million stars for which, besides the position and photometry, the proper motion and parallax are calculated using Hipparcos and Tycho-2 positions in 1991.25 as prior information. Aims. We investigate the scientific potential and limitations of the TGAS component by means of the astrometric data for open clusters. Methods. Mean cluster parallax and proper motion values are derived taking into account the error correlations within the astrometric solutions for individual stars, an estimate of the internal velocity dispersion in the cluster, and, where relevant, the effects of the depth of the cluster along the line of sight. Internal consistency of the TGAS data is assessed. Results. Values given for standard uncertainties are still inaccurate and may lead to unrealistic unit-weight standard deviations of least squares solutions for cluster parameters. Reconstructed mean cluster parallax and proper motion values are generally in very good agreement with earlier Hipparcos-based determination, although the Gaia mean parallax for the Pleiades is a significant exception. We have no current explanation for that discrepancy. Most clusters are observed to extend to nearly 15 pc from the cluster centre, and it will be up to future Gaia releases to establish whether those potential cluster-member stars are still dynamically bound to the clusters. Conclusions. The Gaia DR1 provides the means to examine open clusters far beyond their more easily visible cores, and can provide membership assessments based on proper motions and parallaxes. Aim: To review the international evidence base on interventions to support the mental health of family carers of children with brain injuries in low and middle income countries (LMIC). Methods: Searches were conducted with five electronic databases (Pubmed, Web of Science, Embase, PsycINFO, CINAHL) using search terms related to "family carers", "brain injury", "children" and "low and middle income countries". Studies were independently screened using predetermined eligibility criteria by two authors. Data were extracted from included studies using standardised data extraction and quality appraisal tools. These data were then subjected to narrative synthesis. The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines were used to govern the review process. Findings: One study met our inclusion criteria and described an acquired brain injury called nodding syndrome which occurs in Sub-Saharan Africa. The study was conducted in Ghana and provided group-based psychotherapy to carers and their children. As such we found no study which sought to solely support the mental health of family carers. Conclusions: There has been a lack of focus in the literature on the mental health of family carers of children with brain injuries in LMIC. Considering the vital importance of caregivers in supporting their children it is imperative that service providers and researchers devise programmes to better meet their needs. The mental health of family carers should be better supported to improve their overall wellbeing, which will in turn improve the wellbeing of their children. [ABSTRACT FROM AUTHOR]

Het is een botanisch soepje, de naamgeving van de plant die in het Nederlands 'zilverkaars' genoemd wordt. Is het nu Cimicifuga of Actaea? Volgens Hans Kramer van kwekerij De Hessenhof worden ze in de landen waar ze van nature vooral voorkomen, China en de VS, nog Cimicifuga genoemd. Ook in tuincentra en catalogi wordt deze naamgeving nog steeds gevolgd, maar in de Naamlijst voor Houtige Gewassen en Vaste Planten - een standaardlijst - wordt Actaea aangehouden. Daarom houd ik daar in dit artikel ook aan vast. Want het is maar net wat je afspreekt; voor de plant maakt het niet uit: hij wordt er niet mooier of minder mooi van. In de vorige top 10 heb ik het over grijsbladigen gehad. Deze keer gaat het over vaste planten die ook tegen droogte kunnen, maar niet grijsbladig zijn. Grijsbladigen zijn voor 99 procent droogteminnend, maar dat houdt niet in dat er geen andere planten zijn die deze omstandigheden aankunnen. Ze durven zelfs hun 'nek' uit te steken waardoor ze, ondanks de beperkende factor water en volop zon, toch een aardige lengte krijgen. Gelukkig maar, want dan kunnen we toch afwisselende borders ontwerpen voor deze omstandigheden.

Waarom grijs? Wat zou de reden zijn voor een plant om grijs blad te hebben? Groen blad assimileert toch veel makkelijker? Het bekende verhaal over survival of the fittest geldt ook in dit geval. Er zijn gebieden in de wereld waar de omstandigheden zo extreem zonnig of droog zijn, dat alleen planten met een aanpassing hier kunnen overleven. Groen blad zou verbranden of verdrogen. Planten met heel fijne beharing op het blad filteren/reflecteren het zonlicht waardoor minder verdamping plaatsvindt. Ook een waslaagje op het blad houdt dit tegen. Een bijkomend voordeel is dat planten hier ook door beschermd worden tegen de zoute zeewind. Zout onttrekt vocht uit het blad, maar door de beharing of waslaag waait het zout bij wijze van spreken over het blad en kan de plant op deze manier toch overleven. Het begrip 'grijsbladig' is overigens een verzamelnaam; het 'grijs' kan variëren van zilverkleurig tot het blauwgrijs van bijvoorbeeld Eucalyptus.

Zoals op de foto te zien is, staat er een weelderig siergras te bloeien onder de twee dikke dennen. Grassen zijn dus niet alleen geschikt voor open vlakten, maar ook voor schaduwplekken. Er zijn siergrassen die schaduw tolereren, maar toch wel van vochtige of in ieder geval niet te droge voeten houden. Maar er zijn ook grassen die het zelfs onder droge omstandigheden prima doen onder bijvoorbeeld bomen en heesters.

25. Effecten van waterstofexplosies

- Abstract
RIVM rapport:Gevaarlijke stoffen, zoals waterstof, kunnen branden of ontploffen. Voor de veiligheid van omwonenden kunnen gemeenten eisen stellen aan nieuwbouw als er in de buurt bedrijven liggen die met gevaarlijke stoffen werken. Dat geldt ook voor transportroutes van dit soort stoffen. Het RIVM beschrijft welke effecten te verwachten zijn bij een explosie van tanks waar waterstof in zit. De effecten zijn beschreven bij tanks van verschillende groottes. Op basis daarvan is berekend tot welke afstanden de opslag van waterstof en het transport ervan veilig zijn. Gemeenten en omgevingsdiensten hebben deze informatie nodig om veiligheidsmaatregelen te kunnen bedenken en kiezen. Zij beoordelen of extra veiligheidsmaatregelen nodig zijn, bijvoorbeeld door gebouwen sterker te maken of er een andere plek voor te kiezen. Ook kunnen gebouwen worden afgeschermd van branden of ontploffingen (explosies) door wallen of muren aan te leggen. Voor een goed advies is het nodig om te weten waartegen een maatregel moet beschermen. Het onderzoek beschrijft daarom wat explosies zijn, hoe een explosie van waterstof verloopt en welke schade explosies kunnen veroorzaken. De effecten zijn berekend met een internationaal model dat ook in Nederland wordt gebruikt. De rekenresultaten komen grofweg overeen met ervaringen met ongevallen met waterstof en veldproeven in de praktijk. In vergelijking met explosies van LNG(Liquefied Natural Gas) (aardgas) en LPG(Liquefied Petroleum Gas) (propaan) zijn de effecten van een waterstofexplosie zijn per kilo waterstof krachtiger. Toch is een ontploffing van een LNG of LPG-opslagtank krachtiger dan een explosie van een even grote tank waterstof. Dat komt omdat waterstof moeilijker is samen te persen. Daarom zit er in opslagtanks die even groot zijn minder kilo waterstof dan kilo LNG of LPG. Dit onderzoek is gedaan omdat het gebruik van waterstof relatief nieuw is en belangrijk is om de overgang van fossiele naar duurzame brandstof mogelijk te maken. Het RIVM heeft de i, Hazardous substances, like hydrogen, have the potential to ignite or explode. In consideration of residents' safety, municipalities may impose specific requirements on new construction projects in areas with nearby businesses handling such substances. This also extends to transportation routes used for these substances. RIVM outlines the expected effects of explosions involving tanks containing hydrogen, considering various tank sizes. Based on this, calculations have been used to determine safe distances for both storing and transporting hydrogen. Municipalities and environmental services rely on this data to develop and select suitable safety measures. They assess whether additional precautions, such as reinforcing buildings or choosing alternative locations, are necessary. Additionally, structures can be protected from fires or explosions by constructing barriers such as embankments or walls. To provide effective guidance, it is crucial to understand the potential threats that measures aim to mitigate. Therefore, the research explains the nature of explosions, the process of a hydrogen explosion and the resulting damages. Calculations are conducted using an international model that is also utilised in the Netherlands, offering results that align closely with hydrogen accident experiences and practical field trials. Comparatively, hydrogen explosions yield more powerful effects per kilogramme than those of LNG (natural gas) or LPG (propane). However, despite this, explosions from LNG or LPG storage tanks are more forceful than those from equally sized hydrogen tanks. This discrepancy arises from the difficulty in compressing hydrogen, resulting in fewer kilogrammes stored in hydrogen tanks compared to LNG or LPG tanks. This research serves to address the relatively new use of hydrogen, which is crucial for facilitating the transition from fossil fuels to sustainable alternatives. - Abstract
Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

- Abstract
Background: Because Americans commonly consume restaurant foods with poor dietary quality, effective interventions are needed to improve food choices at restaurants. Purpose: The purpose of this study was to design and evaluate a restaurant-based intervention to help customers select and restaurants promote heart healthy menu items with healthful fats and high-quality carbohydrates. Methods: The intervention included table tents outlining 10 heart healthy eating tips, coupons promoting healthy menu items, an information brochure, and link to study website. Pre- and postintervention surveys were completed by restaurant managers and customers completed a brief "intercept" survey. Results: Managers (n = 10) reported that the table tents and coupons were well received, and several noted improved personal nutrition knowledge. Overall, 4214 coupons were distributed with 1244 (30%) redeemed. Of 300 customers surveyed, 126 (42%) noticed the table tents and, of these, 115 (91%) considered the nutrition information helpful, 42 (33%) indicated that the information influenced menu items purchased, and 91 (72%) reported that the information will influence what they order in the future. Discussion: The intervention was well received by restaurant managers and positively influenced menu item selection by many customers. Translation to Health Education Practice: Further research is needed to assess effective strategies for scaling up and sustaining this intervention approach.

The study of the exploitation of animals for traction in prehistoric Europe has been linked to the 'secondary products revolution '. Such an approach, however, leaves little scope for identification of the less specialised exploitation of animals for traction during the European Neolithic. This study presents zooarchaeological evidence--in the form of sub-pathological alterations to cattle foot bones--for the exploitation of cattle for the occasional pulling of heavy loads, or 'light' traction.
