Abstract

Accurate abundance estimates are essential for the development of effective conservation management strategies, yet they are difficult to produce for small populations that are elusive and sparsely distributed throughout their range. For such populations it is challenging to collect a representative dataset sufficient for robust estimation of detectability and abundance. Over a one‐year study, we used two methods to estimate abundance of a Nationally Critical, widely dispersed Bryde's whale population in the Hauraki Gulf, Aotearoa/New Zealand; (i) distance sampling from systematic line‐transect aerial surveys (n = 22 surveys, 9,944 km, total sightings 21–24 whales), and (ii) mark‐recapture (MR) using photo‐identification images collected from a platform‐of‐opportunity and small‐boat surveys (218 sampling occasions, 27 whales). From the aerial surveys, we estimated an average of 15 whales (95% CI = 6, 30; CV = 37%) at the sea‐surface at any time. For the boat‐based surveys, we developed a custom MR model to address seasonal and individual heterogeneity in capture probabilities and obtained an estimate of 72 distinct whales (95% CI = 38, 106; CV = 24%) in the population. These two approaches provide different perspectives on the abundance and dynamics of Bryde's whales. The aerial surveys estimate the average number of individuals present at any one time, whereas the MR model estimates the total number of animals that used the Gulf during the study. Although neither sampling method is optimal for estimating the abundance of this small, dispersed population, the use of two complementary approaches informs conservation managers about patterns of abundance and distribution over different temporal and spatial scales. It is common to have limited resources for marine research where model assumptions cannot be met. Abstract

INTRODUCTION: Our aim was to gain insight into the effect of COVID-19 measures on SARS-CoV-2 incidence in secondary schools and the association with classroom CO 2 concentration and airborne contamination. METHODS: Between October 2020-June 2021, 18 schools weekly reported SARS-CoV-2 incidence and completed surveys on school-initiated COVID-19 measures (e.g. improving hygiene or minimizing contacts). CO 2 was measured in occupied classrooms twice, and SARS-CoV-2 air contamination longitudinally using electrostatic dust collectors (EDC) and analyzed using RT-qPCR. National COVID-19 policy measures varied during pre-lockdown, lockdown and post-lockdown periods. During the entire study, schools were recommended to improve ventilation. SARS-CoV-2 incidence rate ratios (IRR) were estimated by Generalized Estimating Equation (GEE) models. RESULTS: During 18 weeks follow-up (range: 10-22) SARS-CoV-2 school-incidence decreased during national lockdown (adjusted IRR: 0.41, 95%CI: 0.21-0.80) and post-lockdown (IRR: 0.60, 0.39-0.93) compared to pre-lockdown. School-initiated COVID-19 measures had no additional effect. Pre-lockdown, IRRs per 10% increase in time CO 2 exceeded 400, 550 and 800 ppm above outdoor level respectively, were 1.08 (1.00-1.16), 1.10 (1.02-1.19), and 1.08 (0.95-1.22). Post-lockdown, CO 2-concentrations were considerably lower and not associated with SARS-CoV-2 incidence. No SARS-CoV-2 RNA was detected in any of the EDC samples. CONCLUSION: During a period with low SARS-CoV-2 population immunity and increased attention to ventilation, with CO 2 levels most of the time below acceptable thresholds, only the national policy during and post-lockdown of reduced class-occupancy, stringent quarantine, and contact testing reduced SARS-CoV-2 incidence in Dutch secondary schools. Widespread SARS-CoV-2 air contamination could not be demonstrated in schools under the prevailing conditions during the study.

Abstract Background In the pivotal phase III RECOURSE trial, trifluridine/tipiracil (FTD/TPI) improved progression-free and overall survival (PFS, OS) of patients with pre-treated metastatic colorectal cancer (mCRC). Subsequently, the TALLISUR trial provided post-authorisation efficacy and safety data and patient-reported outcomes on quality of life (QoL) in a German patient cohort. The present analysis reports the final data on efficacy, safety and QoL and investigates the impact of baseline characteristics and associated prognostic subgroups on outcome. Methods In this prospective, multi-centre, Germany-wide, phase IV study, patients with pre-treated mCRC were given the choice to receive either FTD/TPI or best supportive care (BSC). To assess the primary endpoint, QoL, EORTC QLQ-C30 questionnaires were employed. Secondary endpoints included QoL assessed through EQ-5D-5L questionnaires, OS, PFS and safety. Abstract

There is an ongoing debate on airborne transmission of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) as a risk factor for infection. In this study, the level of SARS-CoV-2 in air and on surfaces of SARS-CoV-2 infected nursing home residents was assessed to gain insight in potential transmission routes. During outbreaks, air samples were collected using three different active and one passive air sampling technique in rooms of infected patients. Oropharyngeal swabs (OPS) of the residents and dry surface swabs were collected. Additionally, longitudinal passive air samples were collected during a period of 4 months in common areas of the wards. Presence of SARS-CoV-2 RNA was determined using RT-qPCR, targeting the RdRp- and E-genes. This article synthesizes research on auditors’ ‘bright’ and ‘dark’ personality traits. Our systematic literature review identified 34 studies that met our inclusion criteria. These studies provide insights into auditors’ personality traits, their impact on audit quality, and their association with various professional behaviors and outcomes. Our review emphasizes the importance of understanding personality traits and considering them in the recruitment phase and throughout an auditor’s career. Recognizing the variation in auditors’ personalities and understanding their interplay within audit practices are relevant for improving and refining the profession.

Abstract Axon diameter influences the conduction properties of myelinated axons, both directly, and indirectly through effects on myelin. However, we have limited understanding of mechanisms controlling axon diameter growth in the central nervous system, preventing systematic dissection of how manipulating diameter affects myelination and conduction along individual axons. Here we establish zebrafish to study axon diameter. We find that importin 13b is required for axon diameter growth, but does not affect cell body size or axon length. Using neuron-specific ipo13b mutants, we assess how reduced axon diameter affects myelination and conduction, and find no changes to myelin thickness, precision of action potential propagation, or ability to sustain high frequency firing. However, increases in conduction speed that occur along single myelinated axons with development are tightly linked to their growth in diameter. This suggests that axon diameter growth is a major driver of increases in conduction speeds along myelinated axons over time.

Abstract

AIM: There is little evidence to support choice of technique and configuration for stapled anastomoses after right hemicolectomy and ileocaecal resection. This study aimed to determine the relationship between stapling technique and anastomotic failure.METHOD: Any unit performing gastrointestinal surgery was invited to contribute data on consecutive adult patients undergoing right hemicolectomy or ileocolic resection to this prospective, observational, international, multicentre study. Patients undergoing stapled, side-to-side ileocolic anastomoses were identified and multilevel, multivariable logistic regression analyses were performed to explore factors associated with anastomotic leak.RESULTS: One thousand three hundred and forty-seven patients were included from 200 centres in 32 countries. The overall anastomotic leak rate was 8.3%. Upon multivariate analysis there was no difference in leak rate with use of a cutting stapler for apical closure compared with a noncutting stapler (8.4% vs 8.0%, OR 0.91, 95% CI 0.54-1.53, P = 0.72). Oversewing of the apical staple line, whether in the cutting group (7.9% vs 9.7%, OR 0.87, 95% CI 0.52-1.46, P = 0.60) or noncutting group (8.9% vs 5.7%, OR 1.40, 95% CI 0.46-4.23, P = 0.55) also conferred no benefit in terms of reducing leak rates. Surgeons reporting to be general surgeons had a significantly higher leak rate than those reporting to be colorectal surgeons (12.1% vs 7.3%, OR 1.65, 95% CI 1.04-2.64, P = 0.04).CONCLUSION: This study did not identify any difference in anastomotic leak rates according to the type of stapling device used to close the apical aspect. In addition, oversewing of the anastomotic staple lines appears to confer no benefit in terms of reducing leak rates. Although general surgeons operated on patients with more high-risk characteristics than colorectal surgeons, a higher leak rate for general surgeons which remained after risk adjustment needs further exploration.

PhD (Occupaional Hygiene), North-West University, Potchefstroom Campus Background: Outdoor agricultural workers such as farmers and farmworkers have been identified as an occupational group that is at high risk of exposure to solar ultraviolet radiation (UVR) during their work activities. Outdoor workers’ exposure to solar UVR need to be managed to prevent negative health effects such as skin cancer and ocular diseases. Skin colour and melanin content of skin provides a measure of genetic protection against exposure to high levels of solar UVR. Photoprotective measures such as wearing long sleeve clothing, broad-brimmed hats and sunglasses have been identified as measures that can reduce an individual’s exposure to solar UVR if used correctly. The solar UVR exposure of outdoor farmworkers has not previously been investigated in South Africa. Aims and objectives: The general aim of this thesis was to quantify the differences in skin colour and melanin content of facultative and constitutive skin of farmworkers on a macadamia nut and avocado farm in the Limpopo Province of South Africa, as well assessing the existing control measures and practices of those farmworkers together with their occupational exposure to solar UVR to determine if the exposure poses a risk to their health. The specific objectives were: (i) to determine the differences in skin colour (individual typology angle, ITA°) and melanin content (melanin index, MI) between the facultative and constitutive skin of farmworkers using skin bioengineering measurements, (ii) to assess the solar UVR practices, in particular the use of ocular protective measures, of the farmworkers on the farm using questionnaires, (iii) to determine the photoprotection provided by the clothing worn by farmworkers using polysulphone (PSF) badges on the shoulder both under and on top of the clothing and (iv) to assess the personal solar UVR exposure of outdoor farmworkers on the farm during autumn, spring and summer while carrying out their different work activities through polysulphone badges placed on their arms, top of the shoulder and top of any head covering worn. Methods: Outdoor and indoor farmworkers on the farm participated in this study. The skin colour and melanin content of the skin of farmworkers were classified using both a subjective method, namely questionnaires and objective methods namely measurement of ITA°) and MI. Outdoor farmworkers’ use of ocular photoprotective measures was determined with the use of a questionnaire. The photoprotection provided by the clothing worn by farmworkers was measured by placing a PSF badge on the shoulder beneath the clothing and on the outside of clothing and classified according to the requirements of the AS/NZS 4399:2017 standard. The solar UVR exposure of outdoor and indoor farmworkers was measured by placing PSF badges on the shoulder, arm and top of the head during autumn, spring and summer during different work activities. Workers’ exposure was compared with the highest value of the International Commission of Illumination (CIE) occupational exposure limit, namely 1.3 SED (standard erythemal dose). Research ethics approval for this study was obtained from the North-West University Health Research Ethics Committee (NWU-00101-17-A1). Results: Significant differences were found between both the ITA° and MI measured on the constitutive (inner upper arm) skin and facultative (back of the hand) of farmworkers. The skin colour of the majority of farmworkers were objectively classified as falling in the Fitzpatrick Phototype V/Brown. A strong correlation was found between ITA° and MI which indicates that both objective methods are appropriate to determine the skin colour of darkly-pigmented skin. The majority of outdoor farmworkers (80%) never wore sunglasses while working outside. Although more than 70% of outdoor farmworkers wore a hat while working, they preferred to wear a cap rather than a broad-brimmed hat which provides better ocular photoprotection. Statistically significant differences were found between the solar UVR underneath and outside of clothing worn by farmworkers during autumn, spring and summer. Only 5% of clothing did not provide the minimum required photoprotection. More than 75% of farmworkers reported that they removed the long sleeve overall jacket they wear as part of their uniform at some point during the day which exposes their arms to solar UVR. Statistically significant differences were found between the solar UVR measured on the shoulder, arm and top of the head of outdoor farmworkers during spring and summer. The arm received the highest solar UVR exposure during autumn (7.8 SED) while the top of the head received the highest levels during spring (11.6 SED) and summer (13.9 SED). The solar UVR exposure of all body sites exceeded the daily CIE exposure limit for more than 80% of measurements during all three seasons. Conclusion: Skin colour and melanin content of farmworkers in this study was found to provide a high level of genetic photoprotection. It was also found that objective and subjective measurement of skin colour and can both be used to classify the skin colour of an occupational group in order to identify genetic photoprotection in workers. The use of ocular photoprotective measures by the outdoor farmworkers in this study was inadequate. The clothing worn by farmworkers in the study provided sufficient photoprotection, but removal of clothing during the day decreased its effectivity. The exposure of farmworkers to solar UVR exceeded the recommended CIE exposure limit during not only summer, but autumn and spring as well which indicates that photoprotective measures should not only be in use during summer but also during the other two seasons. The solar UVR exposure of the outdoor farmworkers included in our study was generally higher than the exposure of farmers and farmworkers in other countries. Abstract

BACKGROUND: Right hemicolectomy or ileocecal resection are used to treat benign conditions like Crohn's disease (CD) and malignant ones like colon cancer (CC).AIM: To investigate differences in pre- and peri-operative factors and their impact on post-operative outcome in patients with CC and CD.METHODS: This is a sub-group analysis of the European Society of Coloproctology's prospective, multi-centre snapshot audit. Adult patients with CC and CD undergoing right hemicolectomy or ileocecal resection were included. Primary outcome measure was 30-d post-operative complications. Secondary outcome measures were post-operative length of stay (LOS) at and readmission.RESULTS: Three hundred and seventy-five patients with CD and 2,515 patients with CC were included. Patients with CD were younger (median = 37 years for CD and 71 years for CC (P < 0.01), had lower American Society of Anesthesiology score (ASA) grade (P < 0.01) and less comorbidity (P < 0.01), but were more likely to be current smokers (P < 0.01). Patients with CD were more frequently operated on by colorectal surgeons (P < 0.01) and frequently underwent ileocecal resection (P < 0.01) with higher rate of de-functioning/primary stoma construction (P < 0.01). Thirty-day post-operative mortality occurred exclusively in the CC group (66/2515, 2.3%). In multivariate analyses, the risk of post-operative complications was similar in the two groups (OR 0.80, 95%CI: 0.54-1.17; P = 0.25). Patients with CD had a significantly longer LOS (Geometric mean 0.87, 95%CI: 0.79-0.95; P < 0.01). There was no difference in re-admission rates. The audit did not collect data on post-operative enhanced recovery protocols that are implemented in the different participating centers.CONCLUSION: Patients with CD were younger, with lower ASA grade, less comorbidity, operated on by experienced surgeons and underwent less radical resection but had a longer LOS than patients with CC although complications' rate was not different between the two groups.

Abstract

In opdracht van de gemeente Utrecht heeft het RIVM in het project GO! Utrecht onderzocht hoe de leefomgeving in de Utrechtse wijken gezonder kan worden gemaakt. Per wijk worden combinaties van maatregelen voor de leefomgeving voorgesteld die extra gezondheidswinst kunnen opleveren. De voorstellen zijn per wijk afgestemd op de omstandigheden en samengevat in factsheets. Zo geeft het de gemeente handvatten om keuzes te maken. Het gaat onder andere om maatregelen voor meer groen waar mensen iets mee kunnen doen (zoals buurtmoestuinen, buurtparkjes en onderhoud door bewoners van gemeentelijke groenstroken), beweegvriendelijke wijken, minder lawaai, schonere lucht en meer sociaal contact. Ook kan worden ingezet op verkeersvrije en leefbare straten ('leefstraten'), meer plekken om fietsen te stallen, het stimuleren van zogeheten deelauto's en een betere handhaving op asociaal rijgedrag. De aanpak van GO! Utrecht is erop gericht om maatregelen voor het milieu, de fysieke inrichting en sociale samenhang tegen elkaar af te wegen en te vertalen in integrale strategieën om de wijk te verbeteren. Het zijn maatregelen die in de praktijk al hebben bewezen te kunnen bijdragen aan een gezondere leefomgeving in brede zin. De meerwaarde van dit onderzoek zit vooral in de voorgestelde combinaties van maatregelen die op meerdere onderdelen positief uitwerken: lucht en geluid, gezondheid en leefstijl, actief en sociaal, veiligheid en schoon, voorzieningen en inrichting, en groen. De benodigde informatie is uit beschikbare Utrechtse databestanden, wijkgesprekken en (inter)nationale bronnen met leefomgevingsmaatregelen verzameld. Aanvullend heeft het RIVM luchtkwaliteitsberekeningen uitgevoerd om op wijkniveau een beeld te krijgen welke stoffen en bronnen een aandeel leverden aan de totale luchtkwaliteit. Dit rapport biedt de achtergrondinformatie bij de factsheets om de resultaten van GO! Utrecht te kunnen duiden en de voorgestelde maatregelencombinaties te kunnen beoordelen en uit te voeren.

Abstract

A right hemicolectomy is among the most commonly performed operations for colon cancer, but modern high-quality, multination data addressing the morbidity and mortality rates are lacking.This study reports the morbidity and mortality rates for right-sided colon cancer and identifies predictors for unfavorable short-term outcome after right hemicolectomy.This was a snapshot observational prospective study.The study was conducted as a multicenter international study.The 2015 European Society of Coloproctology snapshot study was a prospective multicenter international series that included all patients undergoing elective or emergency right hemicolectomy or ileocecal resection over a 2-month period in early 2015. This is a subanalysis of the colon cancer cohort of patients.Predictors for anastomotic leak and 30-day postoperative morbidity and mortality were assessed using multivariable mixed-effect logistic regression models after variables selection with the Lasso method.Of the 2515 included patients, an anastomosis was performed in 97.2% (n = 2444), handsewn in 38.5% (n = 940) and stapled in 61.5% (n = 1504) cases. The overall anastomotic leak rate was 7.4% (180/2444), 30-day morbidity was 38.0% (n = 956), and mortality was 2.6% (n = 66). Patients with anastomotic leak had a significantly increased mortality rate (10.6% vs 1.6% no-leak patients; p0.001). At multivariable analysis the following variables were associated with anastomotic leak: longer duration of surgery (OR = 1.007 per min; p = 0.0037), open approach (OR = 1.9; p = 0.0037), and stapled anastomosis (OR = 1.5; p = 0.041).This is an observational study, and therefore selection bias could be present. For this reason, a multivariable logistic regression model was performed, trying to correct possible confounding factors.Anastomotic leak after oncologic right hemicolectomy is a frequent complication, and it is associated with increased mortality. PREDICTORES DE FUGA ANASTOMóTICA, COMPLICACIONES POSTOPERATORIAS Y MORTALIDAD DESPUéS DE LA COLECTOMíA DERECHA POR CáNCER: RESULTADOS DE UNA AUDITORíA INTERNACIONAL DE CORTO PLAZO: La hemicolectomía derecha se encuentra entre las operaciones más frecuentemente realizadas para cáncer de colon, pero faltan datos modernos multinacionales de alta calidad, que aborden las tasas de morbilidad y mortalidad.Reportar la tasa de morbilidad y mortalidad para cáncer de colon del lado derecho, e identificar predictores de resultados desfavorables a corto plazo, después de la hemicolectomía derecha.Estudio prospectivo observacional de corto plazo.Estudio multicéntrico internacional.El estudio de corto plazo de la Sociedad Europea de Coloproctología de 2015, fue una serie prospectiva multicéntrica internacional, que incluyó a todos los pacientes sometidos a hemicolectomía derecha electiva, de emergencia o resección ileocecal, por un período de dos meses y a principios de 2015. Este es un subanálisis, cohorte de pacientes con cáncer de colon.Los predictores de fuga anastomótica, morbilidad y mortalidad postoperatorias a los 30 días, se evaluaron usando modelos de regresión logística de efectos multivariables mixtos, después de la selección de variables con el método Lasso.De los 2,515 pacientes incluidos, se realizó una anastomosis en el 97,2% (n = 2,444); sutura manual en 38.5% (n = 940) y por engrapadora en 61.5% (n = 1504) casos. La tasa global de fuga anastomótica fue del 7,4% (180/2,444), morbilidad a los 30 días fue del 38,0% (n = 956) y la mortalidad fue del 2,6% (n = 66). Los pacientes con fuga anastomótica tuvieron una tasa de mortalidad significativamente mayor (10,6% frente al 1,6% de pacientes sin fuga, p0,001). En el análisis multivariable, las siguientes variables se asociaron con la fuga anastomótica: mayor duración de la cirugía (OR 1.007 por minuto, p = 0.0037), abordaje abierto (OR 1.9, p = 0.0037) y anastomosis por engrapadora (OR 1.5, p = 0.041).Este es un estudio observacional y por lo tanto podría estar presente el sesgo de selección. Por esta razón, se realizó un modelo de regresión logística multivariable, tratando de corregir posibles factores de confusión.La fuga anastomótica después de la hemicolectomía derecha oncológica, es una complicación frecuente y asociada a mayor mortalidad. Los factores quirúrgicos clave que contribuyeron a la fuga anastomótica, fueron la técnica anastomótica, abordaje quirúrgico y duración de la cirugía.
