Abstract

The cross sections of the pd -> 3He eta, pd -> 3He pi0 pi0 pi0 and pd -> 3He pi+ pi- pi0 reactions have been measured at beam kinetic energies T_p= 1360 MeV and T_p= 1450 MeV using the CELSIUS/WASA detector setup. At both energies, the differential cross section dsigma/dOmega of the eta meson in the pd -> 3He eta reaction shows a strong forward-backward asymmetry in the CMS. The ratio between the pd -> 3He pi+ pi- pi0 and the pd -> 3He pi0 pi0 pi0 cross sections has been analysed in terms of isospin amplitudes. The reconstructed invariant mass distributions of the pi-pi, 3He-pi and 3He-2pi systems provide hints on the role of nucleon resonances in the 3pi production process., Comment: Shorter version accepted to EPJA 10 pages 14 figures

Abstract

The cross section for the pd --> ^3He eta pi0 reaction has been measured at a beam energy of 1450 MeV using the WASA detector at the CELSIUS storage ring and detecting one ^3He and four photons from the decays of the two photons. The data indicate that the production mechanism involves the formation of the Delta(1232) isobar. Although the beam energy does not allow the full peak of this resonance to be seen, the invariant masses of all three pairs of final state particles are well reproduced by a phase space Monte Carlo simulation weighted with the p-wave factor of the square of the pi^0 momentum in the ^3Hepi^0 system., Comment: 10 pages, 5 figures

Abstract

The results from the first kinematically complete measurement of the dd --> 4Hepipi reaction are reported. The aim was to investigate a long standing puzzle regarding the origin of the peculiar pipi-invariant mass distributions appearing in double pion production in light ion collisions, the so-called ABC effect. The measurements were performed at the incident deuteron energies of 712 MeV and 1029 MeV, with the WASA detector assembly at CELSIUS in Uppsala, Sweden. We report the observation of a characteristic enhancement at low pipi-invariant mass at 712 MeV, the lowest energy yet. At the higher energy, in addition to confirming previous experimental observations, our results reveal a strong angular dependence of the pions in the overall centre of mass system. The results are qualitatively reproduced by a theoretical model, according to which the ABC effect is described as resulting from a kinematical enhancement in the production of the pion pairs from two parallel and independent NN--> dpi sub-processes., Comment: 20 pages including 18 figures

Abstract

The production of $\omega$ mesons in the $pd \to{}^3$He$ \omega$ reaction has been studied at two energies near the kinematic threshold, $T_p=1450$ MeV and $T_p=1360$ MeV. The differential cross section was measured as a function of the $\omega$ cm angle at both energies over the whole angular range. Whereas the results at 1360 MeV are consistent with isotropy, strong rises are observed near both the forward and backward directions at 1450 MeV. Calculations made using a two-step model with an intermediate pion fail to reproduce the shapes of the measured angular distributions and also underestimate the total cross sections., Comment: 10 pages, 13 figures

Abstract

The ABC effect - a puzzling low-mass enhancement in the $\pi\pi$ invariant mass spectrum - is well-known from inclusive measurements of two-pion production in nuclear fusion reactions. Here we report on first exclusive and kinematically complete measurements of the most basic double pionic fusion reaction $pn \to d \pi^0\pi^0$ at 1.03 and 1.35 GeV. The measurements, which have been carried out at CELSIUS-WASA, reveal the ABC effect to be a $(\pi\pi)_{I=L=0}$ channel phenomenon associated with both a resonance-like energy dependence in the integral cross section and the formation of a $\Delta\Delta$ system in the intermediate state. A corresponding simple s-channel resonance ansatz provides a surprisingly good description of the data.

Abstract

The tensor polarisation of omega mesons produced in the pd-->3He+omega reaction has been studied at two energies near threshold. The 3He nuclei were detected in coincidence with the pi0pi+pi- or pi0gamma decay products of the omega. In contrast to the case of phi meson production, the omega mesons are found to be unpolarised. This brings into question the applicability of the Okubo-Zweig-Iizuka rule when comparing the production of vector mesons in low energy hadronic reactions., Comment: 11 pages, 4 figures

Abstract

A search for rare lepton decays of the eta meson was performed using the WASA detector at CELSIUS. Two candidates for double Dalitz decay eta->e+e-e+e- events are reported with a background of 1.3+/-0.2 events. This allows to set an upper limit to the branching ratio of 9.7E-5 (90% CL). The branching ratio for the decay eta->e+e-gamma is determined to (7.8+/-0.5 stat+/-0.8 syst)E-3. An upper limit (90% CL) for the branching ratio for the eta->e+e- decay is 2.7E-5 and a limit for the sum of the eta->mu+mu-mu+mu- and eta->pi+pi-mu+mu- decays is 3.6E-4., Comment: 9 pages, 10 Postscript figures, uses revtex4.sty, revised version after referee comments; conclusions unchanged

Abstract

The ABC effect - a puzzling low-mass enhancement in the $\pi\pi$ invariant mass spectrum - is known from inclusive measurements of two-pion production in nuclear collisions, where it always showed up, if the participating nucleons fused to a bound nuclear system in the final state. The first exclusive measurements on the ABC effect have been carried out very recently at CELSIUS-WASA for the fusion reactions leading to d, $^3$He and $^4$He nuclei in the final state. The data analyzed so far for the fusion processes to d and $^3$He reveal this effect to be a $\sigma$ channel phenomenon associated with the formation of a strongly attractive $\Delta\Delta$ system. The data for the strictly isospin-selective double-pionic fusion to $^4$He, where we expect the largest effect, are currently still analyzed. All inclusive data on this system are well described by our model, too. This case also constitutes the heaviest nuclear system, where exclusive measurements of double-pionic fusion can be carried out with present-day instruments. Surprisingly, the $pp \to pp\pi^0\pi^0$ reaction in the $\Delta\Delta$ region is observed to also show a ABC-like low-$\pi\pi$ mass enhancement, a phenomenon, which deserves special attention.

Abstract

Background There are many well-documented obstacles to routine laparoscopy, but the contribution of healthcare workers' (HCWs') attitudes has not been examined. Methods A questionnaire study was performed to evaluate HCWs' attitudes toward laparoscopic cholecystectomy (LC) in operating rooms of all public hospitals in Jamaica. Results There were 285 HCWs interviewed from 12 hospitals across the nation. All HCWs were aware of the theoretic advantages of LC, but 56% opposed a suggestion to routinely offer LC to elective patients and 89% opposed LC for acute cholecystitis. Those in opposition were more commonly members of the nursing profession (71.8% vs 25.8%; P 20 years ago (72% vs 42.5%; P Although some HCWs cited valid reasons to oppose LC, some made erroneous statements such as LC was for patients who were overly concerned with esthetics (48%), LC was unsafe when a conversion was required (35%), LC compromised patient safety for esthetics (25%), LC was inferior to open surgery (21%), LC brought no advantage over open surgery (21%), and LC was harmful (20%). These beliefs are not aligned with local or international data, and they add drag to the lethargic pace of laparoscopy in Jamaica. Conclusion A change in HCWs' attitudes is necessary to overcome the stagnation in laparoscopic practice. We propose short-term and long-term strategies that may potentially change HCWs' attitudes. This study identified the HCW population to be targeted.

18. Stump Cholecystitis: Laparoscopic Completion Cholecystectomy with Basic Laparoscopic Equipment in a Resource Poor Setting

Abstract

Introduction. Stump cholecystitis is a recognised condition in which a large gallbladder remnant becomes inflamed after subtotal cholecystectomy. When this occurs, a completion cholecystectomy is indicated. Traditionally, these patients were subjected to open surgery because the laparoscopic approach was anticipated to be technically difficult. We present a case of completion cholecystectomy using basic laparoscopic equipment in a resource poor setting to demonstrate that the laparoscopic approach is feasible. Case Description. A 57-year-old woman presented with right upper quadrant pain and vomiting. She had an elective open cholecystectomy seven years before but reported remarkably similar symptoms. Abdominal ultrasound suggested calculous acute cholecystitis. MRCP confirmed the presence of a large gallbladder remnant with stones. Gastroduodenoscopy excluded other differentials. She had an uneventful laparoscopic completion cholecystectomy performed. Discussion. Although traditional dogma suggested that a completion cholecystectomy should be performed through the open approach, several small studies have demonstrated that laparoscopic completion cholecystectomy is feasible and safe. This report adds to the existing data in support of the laparoscopic approach.

23. Simulating heat load profiles in buildings using mixed effects models

Abstract

The landscape of buildings is a diverse one and long-term energy system planning requires simulation tools that can capture such diversity. This work proposes a model for simulating the space-heating consumption of buildings using a linear mixed-effects model. This modelling framework captures the noise caused by the differences that are not being measured between individual buildings; e.g. the preferences of their occupants. The proposed model uses outdoor temperature and space-heating consumption measured at hourly resolution; thus, the model is able to predict the intra-day variations as well as longer effects. Given the stochastic nature of the simulation, the prediction interval of the simulation can be estimated, which defines a region where the consumption of any unobserved building will fall in. A whole year has been simulated and compared to out-of-sample measurements from the same period. The results show that the out-of-sample data is virtually always inside the estimated 90% prediction interval. This work uses data from Norwegian schools, although the model is general and can be built for other building categories. This amount of detail allows energy planners to draw a varied and realistic map of the future energy needs for a given location.

Abstract

Nature-based tourism is a highly dynamic sector and subject to many trends and changes. Based on a comprehensive literature review, this chapter identifies broad global changes, so-called megatrends, that affect tourism in general and NBT in particular. Data from an online panel survey with experts from five countries who were asked to recognize the most prominent nature-based tourism trends in a 10 years’ perspective, is analyzed in the light of global drivers. The chapter concludes that issues related to sustainability, health and well-being, increased specialization and segmentation are the most noticeable and recurrent trends in nature-based tourism today.

Abstract

Objective Removal of a gallbladder remnant occasionally becomes necessary when retained stones become symptomatic. Although the laparoscopic approach has been described, it is not yet considered the standard of care. We sought to determine the outcomes after completion cholecystectomies in the resource-poor setting within the Caribbean. Methods We carried out an audit of the databases from all hepatobiliary surgeons in the Anglophone Caribbean. We identified all patients who had completion cholecystectomy over the five-year period from July 1, 2012 to June 30, 2018. Retrospective chart review was performed to extract the following data: patient demographics, diagnoses, presenting complaints, operative details, morbidity, mortality, and clinical outcomes. Descriptive statistics were generated using Statistical Packaging for Social Sciences (SPSS), version 12.0 (SPSS Inc., Chicago IL) Results There were 12 patients who were subjected to laparoscopic completion cholecystectomy for acute cholecystitis (7), severe biliary pancreatitis (3), and chronic cholecystitis (2) secondary to stones in a gallbladder remnant. There were 10 women and two men at a mean age of 47.4 years (range 32-60; standard deviation (SD) +/-7.81; median 48; mode 52) and a mean body mass index (BMI) of 30.8 Kg/M2 (SD +/-3.81; range 26-38; median 29.5). The mean interval between the index operation and the completion operation was 14.8 months (SD +/- 12.3; range 1-48; median 13; mode 18). Five (42%) patients had their original cholecystectomy using the open approach. Five (42%) index operations were done on an emergent basis and the gallbladder remnant was deliberately left behind in three (25%) index operations. The completion cholecystectomies were all completed laparoscopically in 130.5 minutes (SD +/- 30.5; range 90-180, median 125; mode 125) without any conversions or mortality. There were two minor bile leaks that resolved without intervention through an indwelling drain. Discussion Completions cholecystectomy can be completed via the laparoscopic approach with good outcomes and acceptable morbidity and mortality rates. The patients derive the same advantages as elective cholecystectomies. Therefore, the laparoscopic approach, when performed by hepatobiliary surgeons with advanced laparoscopic expertise in specialized centers, should be the new standard of care.
