More and more scholars have paid attention to traditional Chinese medicine since the concept of translational medicine was introduced into this field. Most scholars believe that it is of great benefit to the inheritance and development of traditional Chinese medicine by the integration of translational medicine concepts and traditional Chinese medicine research. By reviewing the connotation and development of translational medicine, this paper explained the similarities between translational medicine and the basic theories of traditional Chinese medicine, and further described the content and practice of translational research on traditional Chinese medicine. It points out that the research foundation of traditional Chinese medicine was weak, the evidence was scattered, and the overall evaluation of efficacy needed to be strengthened. Finally, the viewpoints and strategies of strengthening the transformation research of TCM were put forward, such as strengthening the integration of resources, focusing on the areas of advantage and improving the overall evaluation, so as to provide new ideas for the future research of TCM transformation.

Problem definition: This paper studies the single-warehouse assortment selection problem that aims to minimize the order fulfillment cost under the cardinality constraint. We propose two fulfillment-related cost functions corresponding to spillover fulfillment and order splitting. This problem includes the fill rate maximization problem as a special case. We show that although the objective function is submodular for a broad class of cost functions, the fill rate maximization problem with the largest order size being two is NP-hard. Methodology/results: To make the problem tractable to solve, we formulate the general warehouse assortment problem under the two types of cost functions as mixed integer linear programs (MILPs). We also provide a dynamic programming algorithm to solve the problem in polynomial time if orders are nonoverlapping. Furthermore, we propose a simple heuristic called the marginal choice indexing (MCI) policy that allows the warehouse to store the most popular products. This policy is easy to compute, and hence, it is scalable to large-size problems. Although the performance of MCI can be arbitrarily bad in some extreme scenarios, we find a general condition under which it is optimal. This condition is satisfied by many multi-purchase choice models. Managerial implications: Through extensive numerical experiments on a real-world data set from RiRiShun Logistics, we find that the MCI policy is surprisingly near optimal in all the settings we tested. Simply applying the MCI policy, the fill rate is estimated to improve by 9.18% on average compared with the current practice for the local transfer centers on the training data set. More surprisingly, the MCI policy outperforms the MILP optimal solution in 14 of 25 cases on the test data set, illustrating its robustness against demand fluctuations. Background: Optimal management of involved common carotid artery (CCA) with false-lumen thrombus remains unclear in aortic dissection patients. We aim to investigate outcomes and compare different surgical strategies., Methods: This is a retrospective cohort study and the institutional database of acute type A aortic dissection was reviewed. The patients with CCA involvement and extended false-lumen thrombus were enrolled and grouped according to the management of CCA: extra-thoracic carotid artery replacement (CAR) and reconstruction in situ (RIS). Multivariate logistic regression analysis was used to investigate the effect of management on neurological outcomes. Kaplan-Meier method was used for survival analysis and log-rank test was used to compare the difference on survival rate., Results: From March 2011 to December 2019, 68 patients were enrolled (24 in the CAR group and 44 in the RIS group). The overall operative mortality was 7.4% (5 patients) and 21 patients had the incidence of postoperative neurological deficit was (30.9%). The rates of main postoperative complications were similar between the two groups. Twenty-five (56.8%) patients in the RIS group had residual false-lumen thrombus at discharge. In multivariate analysis, CAR was the only independent protective factor of postoperative neurological deficit [odds ratio (OR) =0.03, 95% confidence interval (CI): 0.0-0.61, P=0.02] and age was the only risk factor (OR =1.34, 95% CI: 1.11-1.62, P=0.002). The median follow-up time was 40 (interquartile range, 24-69) months and some of the patients received imaging follow-up. The overall survival rates at 5 and 10 years were 95.8%, and 95.8% in the CAR group and 84.1%, and 76.4% in the RIS group, with no significant difference (P=0.22). No cerebrovascular accident and reintervention occurred and 20 (90.9%) patients with residual false-lumen thrombus had reabsorption of thrombus during the follow-up period., Conclusions: CAR was a thorough technique and could protect patients from postoperative neurological deficit than RIS. Patients in either group could have a satisfying long-term prognosis after surviving from perioperative period. Objective: To evaluate the therapeutic effect of ulinastatin combined with thymosin α1 in the treatment of severe sepsis in rats., Methods: Normal Wistar rats were subject to cecal ligation and puncture (CLP) to establish models of severe sepsis. The rats were then randomized into 4 groups for treatment with saline (control), ulinastatin, thymosin α1, or the combination of the latter two injected through the caudal vein or subcutaneously at 6, 24, 48 and 72 h after modeling. The mortality rate was recorded daily and the rats were executed at 24, 48, 72 and 96 h after CLP to harvest the heart, liver, spleen, lung, kidney and small intestines for pathological examination. The spleen of the rats were taken for detection of apoptosis of the spleen cells., Results: The mortality rate of the septic rats in the combined treatment group was decreased significantly (P=0.0325). The control group showed the most severe organ damage, which was moderate in single drug treatment group and the mildest in combined treatment group. Obvious spleen cell apoptosis was found in the control group, and was significantly ameliorated in the combined treatment group[(47.4∓10.9)% vs (39.3∓11.4)%, P=0.0000]., Conclusion: Combined treatment with ulinastatin and thymosin α1 can significantly improve the prognosis and ameliorate organ damage and spleen cell apoptosis in rats with sever sepsis.
