Introduction: At the end of 2019 a new virus, called SARS-CoV-2, emerged in Wuhan, China. The aim of the present study was to assess the impact of the first wave of the COVID-19 pandemic on the health system of the Tuscany Region and the response implemented by the South-Eastern Local Health Unit, also in view of the new reform of territorial healthcare established by Ministerial Decree No. 77 of 2022., Methods: Data were taken from the "OpenToscana" database beginning when the first case was recorded in Italy (18 February 2020) until July 2020. We analyzed infections and deaths in each Local Heal.th Unit in the Tuscany Region and calculated the fatality rate (number of deaths/cases x 100) following COVID-19 infection. We subsequently compared the fatality rates among the Local Health Units by means of the Kruskal Wallis test., Results: During the first wave, the South-Eastern Local Health Unit had fewer infections (a total of 1,532 by July) and fewer deaths (total: 107 by July) than the other Local Health Units. In the South-Eastern Local Health Unit, the fatality rate in July was 6.98%. The comparison of the fatality rates among the various LHUs and the whole Region showed statistically significant differences (p<0.001)., Conclusions: The organizational models promptly implemented by the South-Eastern Local Health Unit for good territorial care and the management of COVID-19-positive patients limited the spread of infection, and consequently the deaths, thus reducing the fatality rate in the first wave of the pandemic.

Introduction: UV-C has proven to be an effective virucide and microbicide, and its cost-effectiveness allowed it to spread as a disinfecting procedure in different environments., Methods: The study aims to determine the microbicide activity on Staphylococcus aureus, Escherichia coli and SARS-CoV-2 of the UV-C Boxer by Cartoni S.p.A. Three separate experiments were performed to assess the effectiveness of the UV-C disinfection device on different materials, directly on surfaces of a video camera and on a specific carrier for SARS-CoV-2., Results: In all three experiments, a significant abatement of bacterial and viral contamination was reached after 60 seconds on carriers and after 3 minutes on all examined surfaces of the video camera, with a higher reduction on glass carriers., Conclusions: UV-C devices may be a valuable tool to implement in the working routine to achieve a higher level of safety in work environments., Competing Interests: Cartoni S.p.A. financed the University of Siena, with G.M. as the principal investigator. Chronic obstructive pulmonary disease (COPD) and atrial fibrillation (AF) are characterized by increased oxidative stress, but the impact of the coexistence of COPD and AF on systemic oxidative stress is unclear. We performed a cross-sectional study including 157 outpatients to investigate the Nox2-related oxidative stress in patients with AF and COPD. COPD was defined by an FEV 1 /FVC <0.70. Oxidative stress was measured by sNox2-dp, a marker of Nox2 activation, and urinary isoprostanes. We divided patients into four groups: Group 0: hypertension ( n  = 49, controls); Group 1: COPD ( n  = 42); Group 2: AF ( n  = 33); and Group 3: COPD and AF ( n  = 33). Mean age was 68.3 ± 11.0 years, and 46.5% were women. Patients with COPD or AF showed increased levels of sNox2-dp as compared with group 0; sNox2-dp further increased in patients with COPD + AF. In these patients, sNox2-dp was higher than in those with COPD ( p  < 0.001) or AF ( p  = 0.003). At multivariable logistic regression analysis, chronic kidney disease, COPD, and AF were associated with sNox2-dp above median. Similar results were observed for urinary isoprostanes. We hypothesize that the coexistence of COPD in AF patients may be associated with an increased systemic oxidative stress by the upregulation of Nox2. Antioxid. Redox Signal. 31, 786-790.
