01 juni 2023, Item does not contain fulltext, BACKGROUND: Patients with serrated polyposis syndrome (SPS) have multiple and/or large serrated colonic polyps and higher risk for colorectal cancer. SPS inherited genetic basis is mostly unknown. We aimed to identify new germline predisposition factors for SPS by functionally evaluating a candidate gene and replicating it in additional SPS cohorts. METHODS: After a previous whole-exome sequencing in 39 SPS patients from 16 families (discovery cohort), we sequenced specific genes in an independent validation cohort of 211 unrelated SPS cases. Additional external replication was also available in 297 SPS cases. The WNK2 gene was disrupted in HT-29 cells by gene editing, and WNK2 variants were transfected using a lentiviral delivery system. Cells were analysed by immunoblots, real-time PCR and functional assays monitoring the mitogen-activated protein kinase (MAPK) pathway, cell cycle progression, survival and adhesion. RESULTS: We identified 2 rare germline variants in the WNK2 gene in the discovery cohort, 3 additional variants in the validation cohort and 10 other variants in the external cohorts. Variants c.2105C>T (p.Pro702Leu), c.4820C>T (p.Ala1607Val) and c.6157G>A (p.Val2053Ile) were functionally characterised, displaying higher levels of phospho-PAK1/2, phospho-ERK1/2, CCND1, clonogenic capacity and MMP2. CONCLUSION: After whole-exome sequencing in SPS cases with familial aggregation and replication of results in additional cohorts, we identified rare germline variants in the WNK2 gene. Functional studies suggested germline WNK2 variants affect protein function in the context of the MAPK pathway, a molecular hallmark in this disease.

- Abstract
Background: Tuberculosis diagnosis might be delayed or missed in children with severe pneumonia because this diagnosis is usually only considered in cases of prolonged symptoms or antibiotic failure. Systematic tuberculosis detection at hospital admission could increase case detection and reduce mortality. Methods: We did a stepped-wedge cluster-randomised trial in 16 hospitals from six countries (Cambodia, Cameroon, Côte d'Ivoire, Mozambique, Uganda, and Zambia) with high incidence of tuberculosis. Children younger than 5 years with WHO-defined severe pneumonia received either the standard of care (control group) or standard of care plus Xpert MTB/RIF Ultra (Xpert Ultra; Cepheid, Sunnyvale, CA, USA) on nasopharyngeal aspirate and stool samples (intervention group). Clusters (hospitals) were progressively switched from control to intervention at 5-week intervals, using a computer-generated random sequence, stratified on incidence rate of tuberculosis at country level, and masked to teams until 5 weeks before switch. We assessed the effect of the intervention on primary (12-week all-cause mortality) and secondary (including tuberculosis diagnosis) outcomes, using generalised linear mixed models. The primary analysis was by intention to treat. We described outcomes in children with severe acute malnutrition in a post hoc analysis. This study is registered with ClinicalTrials.gov (NCT03831906) and the Pan African Clinical Trial Registry (PACTR202101615120643). Findings: From March 21, 2019, to March 30, 2021, we enrolled 1401 children in the control group and 1169 children in the intervention group. In the intervention group, 1140 (97·5%) children had nasopharyngeal aspirates and 942 (80·6%) had their stool collected; 24 (2·1%) had positive Xpert Ultra. At 12 weeks, 110 (7·9%) children in the control group and 91 (7·8%) children in the intervention group had died (adjusted odds ratio [OR] 0·986, 95% CI 0·597–1·630, p=0·957), and 74 (5·3%) children in the control group

O presente estudo teve o objetivo de descrever o perfil socioeconômico, produtivo e aspectos de mercado dos apicultores de Remanso (Bahia, Brasil), assim como analisar a influência da assistência técnica extensionista sobre a produtividade apícola na região. A pesquisa foi realizada no município de Remanso, devido sua localização na região onde a atividade apícola está difundida entre os pequenos produtores familiares. As entrevistas foram realizadas com quatorze apicultores, a partir de questionários semiestruturados. Foi observado que a maioria dos apicultores pertencem ao sexo masculino e são experientes na atividade apícola. A apicultura na região ainda é realizada de forma secundária, simultaneamente com outras atividades agrícolas e pecuárias. A maioria dos apicultores estão inseridos em cooperativas e associações, de forma que há melhor facilitação no beneficiamento e venda do mel produzido. Os apicultores vêm recebendo capacitações a partir de atividades extensionistas, contribuindo para ampliação e crescimento na produtividade de mel por colmeia. No mais, a flora apícola da região favorece a atividade no município, assim como o incremento na renda dos apicultores, além dos benefícios ecológicos, devido a polinização da flora local realizada pelas abelhas africanizadas. The bees were very efficient in the pollination of flowers (71.08%), compared to spontaneous self-pollination (2.86%). A. leptopus has mellitophilous flowers, attracting a considerable diversity of species, being characterized as a plant with potential for honey bees and stingless bees, and for providing resources for wild bees. - Abstract
A new mixed turbidite-contourite system is described in the northern Campos Basin, southeastern Brazilian margin. This system is developed in a middle slope setting and was formed through non-synchronous interaction between the turbidity current and a contour current in the same stratigraphic interval (Miocene). Different depositional cycles were accounted based on their diagnostic seismic features. Seismic attributes, seismic facies, and isochron maps were used to identify alternating cycles of downslope and alongslope processes in the study area, along with the intermediate stage with features from both processes (mixed system). The depositional processes resulted from alongslope current activity can be distinguished from the downslope current activity, based on the acoustic characteristics (root-mean-square (RMS) amplitude), internal architecture, and external geometry pattern. While alongslope currents deposits consist of mainly low RMS amplitude values clinoforms with an alongslope trend; the downslope gravity deposits present high-amplitude or chaotic seismic facies, usually higher values of RMS amplitude, channel or channel-lobe features, erosive surfaces, and a basinward depositional trend. In this study, five seismic units are described and later associated with their dominant type of current. Based on the main depositional diagnostic features, it was possible to determine which were the dominant processes that controlled the sedimentation by indicating periods where the margin was mostly submitted to sediment transfer from continent to the basin and periods wherethe oceanic currents prevailed by redistributing sediments along the isobaths and replacing the axis of downslope transfer conduits. Important information on the paleocurrents' direction was also made based on the final deposits display (e.g. terraces, sediment waves, paleochannels), a northward-flowing bottom current was assumed. Para este estudio se utilizaron las siguientes bases de datos: (I) registros de derechos de extracción y uso de agua de la Autoridad Nacional del Agua (ANA) en el ámbito de la cuenca (en la que se incorporaron variables de uso anual de agua); (II) registros de clientes industriales y comerciales de SEDAPAL (con el consumo mensual entre los años 2016 a octubre 2019); (III) valores de producción, valor agregado y empleo del censo económico 2008 (año base 2007) para Lima Metropolitana. En las dos primeras bases fue necesario generar manualmente el código CIIU de una cantidad importante de unidades productivas que no contaban con esta información. Para la integración con el censo económico se agregaron las otras dos bases al nivel de División CIIU2 (Revisión 4) y por distrito. En base a esto se cuenta con datos bastante detallados por rama de producción y ubicación distrital de las empresas, con el valor de producción, valor agregado, empleo y consumo de agua. La base permite diferenciar el uso de agua subterránea y el uso de agua provista por SEDAPAL, generándose indicadores de riesgo diferenciado por cada fuente. Plantea identificar los niveles de riesgo hídrico que enfrentan las empresas de Lima. El nivel de riesgo se mide por rama de producción, por fuente de abastecimiento y por ubicación de las empresas en el territorio de la ciudad. El riesgo hídrico hace referencia a dos elementos de las empresas: 1. Su nivel de dependencia en el uso del agua en un contexto de creciente escasez y volatilidad hídrica; 2. El valor económico generado. Además de identificar niveles de riesgo hídrico. También presenta una cuantificación de los impactos potenciales de una crisis hídrica (reducción significativa de la oferta) en variables económicas como el valor de producción y el empleo de la ciudad.

O cajueiro (Anacardium occidentale L.) é uma frutífera de grande relevância no cenário socioeconômico do Brasil. Seus produtos (principalmente a castanha) possuem alto valor nutritivo e comercial. Devido a sua importância para o país e, principalmente, para o Nordeste, a Embrapa Agroindústria Tropical conta com o Banco de Germoplasma do Caju, o qual é composto por mais de 600 acessos diferentes. A fim de se ter um maior conhecimento da variabilidade metabólica dos acessos contidos no Banco de Germoplasma, genótipos do Banco de Germoplasma e do Programa de Melhoramento Genético do cajueiro foram analisados por meio de Cromatografia Líquida acoplada a Espectrometria de Massas (LC-MS) e Espectroscopia de Infravermelho Próximo. Foi criada uma matriz numérica utilizando os dados cromatográficos e as ferramentas quimiométricas de Análise de Grupamento Hierárquico (HCA), Análise de Componentes Principais (PCA) e análise discriminante por mínimos quadrados parciais (PLS-DA) foram aplicadas. Em HCA e PCA, observou-se a formação de quatro grupamentos, e os eixos PC1 e PC2 explicaram 36,7% da variância. O método PLS-DA foi empregado para melhorar a associação da variabilidade química de acordo com o genótipo do caju e, com três Variáveis Latentes (LVs), o modelo apresentou uma capacidade de predição de 88,05%. Os principais compostos responsáveis pela separação dos grupos foram identificados. São eles: galoilhexose I, digaloilhexosídeo I, éster etílico do ácido hidroxibutanóico-hexosídeo, miricetina-3-O-glucosídeo, e miricetina-3-O-rhamnosídeo. O espectrômetro portátil de Infravermelho Próximo (NIR) foi utilizado a fim de avaliar seu pontencial para criar um modelo de classificação utilizando os teores de vitamina C, sólidos solúveis e acidez total. Porém, os resultados mostraram que outros parâmetros devem ser levados em conta para uma melhor compreensão da variabilidade química e física do caju do banco de germoplasma da Embrapa. Background. In 2004 the World Health Organization (WHO) released the Interim Policy on Collaborative TB/HIV activities. According to the policy, for people living with HIV (PLWH), activities include intensified case finding, isoniazid preventive therapy (IPT) and infection control. For TB patients, activities included HIV counselling and testing (HCT), prevention messages, and cotrimoxazole preventive therapy (CPT), care and support, and antiretroviral therapy (ART) for those with HIV-associated TB. While important progress has been made in implementation, targets of the WHO Global Plan to Stop TB have not been reached. Objective. To quantify TB/HIV integration at 3 primary healthcare clinics in Johannesburg, South Africa. Methods. Routinely collected TB and HIV data from the HCT register, TB ‘suspect’ register, TB treatment register, clinic files and HIV electronic database, collected over a 3-month period, were reviewed. Results. Of 1 104 people receiving HCT: 306 (28%) were HIV-positive; a CD4 count was documented for 57%; and few received TB screening or IPT. In clinic encounters among PLWH, 921 (15%) had documented TB symptoms; only 10% were assessed by smear microscopy, and few asymptomatic PLWH were offered IPT. Infection control was poorly documented and implemented. HIV status was documented for 155 (75%) of the 208 TB patients; 90% were HIV-positive and 88% had a documented CD4 count. Provision of CPT and ART was poorly documented. Conclusion. The coverage of most TB/HIV collaborative activities was below Global Plan targets. The lack of standardised recording tools and incomplete documentation impeded assessment at facility level and limited the accuracy of compiled data.

BackgroundAedes aegypti is a vector for the transmission of diseases such as dengue fever, chikungunya, Zika fever, and yellow fever. In 2016, over 1 million cases of these diseases were reported in Brazil, which is an alarming public health issue. One of the ways of controlling this disease is by inspecting and neutralizing the places where A. aegypti lays its eggs. The Ministry of Planning, Development, and Administration of Brazil maintains the inspection statistics. ObjectiveWe propose a multi-criteria analysis to create an index for A. aegypti inspections reported through the Ministry of Planning, Development, and Administration system of Brazil. MethodsBased on the repository from urban cleaning services combined with data on inspections conducted by government agencies in several Brazilian cities and municipalities, we selected and combined metrics, which we further ranked using the analytic hierarchy process methodology. We also developed risk maps based on the analytic hierarchy process ranking of the A. aegypti breeding sites. ResultsBased on our analysis and the available data, the priority for inspections should consider the number of sick people (weight 0.350), medical evaluations (weight 0.239), inspections (weight 0.201), mosquito breeding sites (weight 0.126), and days of absence from work (weight 0.096). ConclusionsThe proposed index could aid public health practitioners in preventing the appearance of new A. aegypti breeding sites. This information technology application can help solve such public health challenges.

The article focuses on the usefulness of a geographic information system (GIS) in dengue epidemiology. To analyze the dengue epidemics that occurred in the city of Campinas, Brazil, during summers of 2001 /2002 and 2002/2003, GIS was used. State and municipal health authorities used a GIS to plot geographically dengue cases that occurred within the time frame. Several information sources were used: mandatory reporting dengue cases databank, digital maps of city streets and census 2000 data. And it was found that spatial distribution of dengue cases was similar to the city's urban area population distribution. In 2001/2002 transmission level was higher, but only a few area presented higher case density and those areas have a worse socioeconomic situation.

- Abstract
The aim of the present study was to evaluate the characteristics of match injury in male beach soccer players. Video recordings of all official beach soccer tournaments in which the European national male beach soccer teams participated from 2018 to 2021 were analysed by two sports medicine specialists retrospectively. Regarding each injury, data including the mechanism, location of the injury, whether the injury led to time-loss, and the relationship of the injury to the bicycle kick (BK), etc. were documented. A total of 632 injuries were documented, corresponding to 234.9 injuries/1000 player hours. Video footage was available for 580 injuries. Whilst 79.8% of medical attention injuries occurred due to opponent contact, 19.5% of time-loss injuries occurred due to non-contact, and 12.2% of indirect opponent contact ( p < 0.01). The most common location of the BK related injury was the head/neck (68.7%), whereas most common location of the BK unrelated injury was the lower extremity (54.1%) ( p < 0.01). The findings demonstrated that beach soccer injury incidence was quite high; the most common injury location was head/neck and head/neck injuries were associated with BK. In light of these results, some rule regulations, particularly those associated with BK, and the use of protective equipment should be considered to prevent these injuries.

This study aimed to evaluate depression, anxiety, and stress prevalence and correlates among professional football team medical and allied health staff (MAHS) in Turkey. An online survey was sent to all MAHS ( n = 865) attending the professional development accreditation course organized at the end of the 2021-2022 football season in Turkey. Three standardized scales assessed depression, anxiety, and stress. A total of 573 staff participated (response rate = 66.2%). 36.7% of MAHS reported at least moderate severity depression, 25% anxiety, and 80.5% stress. Younger (26-33 years) and less experienced (6-10 years) MAHS reported higher stress scores than their older (50-57 years) and more experienced (>15 years) counterparts ( p = 0.02 and p = 0.03, respectively). Masseurs (compared to team doctors) and staff without a second job (compared to those with a second job) reported higher depression and anxiety scores ( p = 0.02, p = 0.03, p = 0.03, p = 0.02, respectively). Depression, anxiety, and stress scores of MAHS whose monthly income was less than $519 were higher than those whose income was more than $1036 (all p < 0.01). Findings demonstrated that professional football team MAHS suffer from symptoms of mental-ill-health at substantial rates. In light of these results, organizational policies should be implemented to proactively support the mental health of MAHS working in professional football.

Background: CDH1 and CTNNA1 remain as the main genes for hereditary gastric cancer. However, they only explain a small fraction of gastric cancer cases with suspected inherited basis. In this study, we aimed to identify new hereditary genes for early-onset gastric cancer patients (EOGC; < 50 years old)., Methods: After germline exome sequencing in 20 EOGC patients and replication of relevant findings by gene-panel sequencing in an independent cohort of 152 patients, CTNND1 stood out as an interesting candidate gene, since its protein product (p120ctn) directly interacts with E-cadherin. We proceeded with functional characterization by generating two knockout CTNND1 cellular models by gene editing and introducing the detected genetic variants using a lentiviral delivery system. We assessed β-catenin and E-cadherin levels, cell detachment, as well as E-cadherin localization and cell-to-cell interaction by spheroid modeling., Results: Three CTNND1 germline variants [c.28_29delinsCT, p.(Ala10Leu); c.1105C > T, p.(Pro369Ser); c.1537A > G, p.(Asn513Asp)] were identified in our EOGC cohorts. Cells encoding CTNND1 variants displayed altered E-cadherin levels and intercellular interactions. In addition, the p.(Pro369Ser) variant, located in a key region in the E-cadherin/p120ctn binding domain, showed E-cadherin mislocalization., Conclusions: Defects in CTNND1 could be involved in germline predisposition to gastric cancer by altering E-cadherin and, consequently, cell-to-cell interactions. The present study evaluated the mental health (MH) of athletes according to a range of variables, including SARS-CoV-2 infection status and sport-specific factors. An online survey was sent to 1636 Turkish professional athletes incorporating standardized scales assessing athlete-specific stress, depression, and anxiety. A total of 579 professional athletes (29% SARS-CoV-2 positive) participated in the study. While female athletes reported higher anxiety scores than males (p < 0.01), the depression scores of the athletes who had been infected with SARS-CoV-2 were significantly higher than athletes who were not infected (p = 0.03). Higher athlete-specific stress, depression, and anxiety scores were associated with participating in an individual sport, experiencing chronic pain, severe sports injury history, and performance concerns (all p < 0.01). MH symptoms in athletes were associated with being infected with SARS-CoV-2, female gender, participating in an individual sport, and performance concerns. MH assessments should be undertaken with athletes to detect vulnerabilities and enable appropriate and timely MH interventions.

Objectives: This study evaluated the mental health correlates of athletic shame and guilt among elite-level youth athletes and provided psychometric data on the Turkish version of the Athletic Perceptions of Performance Scale., Design: Cross-sectional., Methods: An online survey was sent to 645 Turkish elite-level youth athletes incorporating standardized scales assessing athletic shame, guilt, and symptoms of high prevalence mental disorders. A subset of participants (n = 45) provided test-retest data at 30 days., Results: A total of 301 elite youth athletes participated (M = 16.42 ± 0.49 years; 48.7 % response rate) equally representing team and individual sports. Confirmatory factor analysis validated the Turkish version of the Athletic Perceptions of Performance Scale and test-retest data supported temporal stability. Female athletes (compared to male athletes) and athletes participating in an individual sport (compared to participating in a team sport) reported higher athletic shame-proneness scores (p < 0.01 and p = 0.04; respectively). The Turkish version of the Athletic Perceptions of Performance Scale shame-proneness and no-concern scores were associated with athlete-specific stress, anxiety, and depression scores. The Turkish version of the Athletic Perceptions of Performance Scale guilt-proneness was associated with athlete-specific stress and anxiety scores., Conclusions: Findings provide cross-cultural validation of the Athletic Perceptions of Performance Scale assessing athletic shame and guilt, demonstrating that female athletes and athletes participating in an individual sport were more likely to experience athletic shame-proneness. The COVID-19 pandemic, a transformative event in modern society, has disrupted routine, work, behavior, and human relationships. Organizations, amidst the chaos, have innovatively adapted to the evolving situation. However, many countries were unprepared for the magnitude of the challenge, revealing the fragility of health responses due to inadequate leadership, insufficient resources, and poor information system integration. Structural changes in health systems are imperative, particularly in leadership, governance, human resources, financing, information systems, technology, and health service provision. This research utilizes the Technological Roadmapping method to analyze the health sector, focusing on public health, drawing on articles from SCOPUS and PubMed databases, and creating a roadmap extending to 2050. The research presents three long-term scenarios based on the literature-derived roadmap and explores various alternatives, including integrated care, telemedicine, Big Data utilization, nanotechnology, and Big Tech's AI services. The results underscore the anticipation of post-pandemic public health with high expectations, emphasizing the importance of integrating health history access, encouraging self-care, and leveraging technology for streamlined treatment. Practical implications include insights for decision makers and stakeholders to inform strategic planning and adapt to evolving industry demands, recognizing the significance of preventive services and the humanizing potential of technology.

41. Mental health symptoms and correlates among amateur football players: a cross-sectional study.

The objective of this study was to evaluate mental health problems reported by Turkish amateur football players. A total of 4516 Turkish amateur football players were invited to complete an anonymous online survey including demographic and COVID-19 pandemic related questions, the Patient Health Questionnaire-9, General Anxiety Disorder-7 scale, and Athlete Psychological Strain Questionnaire. A total of 1263 players participated in the study (27.9% response rate). Single players reported higher depression and anxiety scores than married players. Higher depression, anxiety, and athletes-specific stress scores were reported by players with an annual income <$5,000 and those reporting an income loss >75% during the COVID-19 pandemic. Players with a sports injury history, psychiatric illness, sleep problems, and perceived inadequate social and/or organizational support reported poorer indices relative to their counterparts. Implementation of mental health screening among amateur football players may assist in detecting vulnerabilities and initiating appropriate timely intervention.

42. Estimating the effect of a rifampicin resistant tuberculosis diagnosis by the Xpert MTB/RIF assay on two-year mortality.

- Abstract
Studies assessing patient-centred outcomes of novel rifampicin resistant tuberculosis (RR-TB) diagnostics are rare and mostly apply conventional methods which may not adequately address biases. Even though the Xpert MTB/RIF molecular assay was endorsed a decade ago for simultaneous diagnosis of tuberculosis and RR-TB, the impact of the assay on mortality among people with RR-TB has not yet been assessed. We analysed data of an observational prospective cohort study (EXIT-RIF) performed in South Africa. We applied a causal inference approach using inverse odds of sampling weights to rectify survivor bias and selection bias caused by differing screening guidelines. We also adjusted for confounding using a marginal structural model with inverse probability of treatment weights. We estimated the total effect of an RR-TB diagnosis made by the Xpert assay versus the pre-Xpert diagnostic algorithm (entailing a targeted Line Probe Assay (LPA) among TB-confirmed patients) on two-year mortality and we assessed mediation by RR-treatment initiation. Of the 749 patients diagnosed with RR-TB [247 (33%) by the pre-Xpert diagnostic algorithm and 502 (67%) by the Xpert assay], 42.7% died. Of these, 364 (48.6%) patients died in the pre-Xpert group and 200 (39.8%) in the Xpert group. People diagnosed with RR-TB by the Xpert assay had a higher odds of RR-TB treatment initiation compared to those diagnosed by the targeted LPA-based diagnostic process (OR 2.79; 95%CI 2.19-3.56). Receiving an RR-TB diagnosis by Xpert resulted in a 28% reduction in the odds of mortality within 2 years after presentation to the clinic (ORCI 0.72; 95%CI 0.53-0.99). Causal mediation analysis suggests that the higher rate of RR-TB treatment initiation in people diagnosed by the Xpert assay explains the effect of Xpert on 2-year mortality [natural indirect effect odds ratio 0.90 (95%CI 0.85-0.96). By using causal inference methods in combination with high quality observational data, we could demonstrate that the introduction of the Xpert assay caused a 28% reduction in 2-year odds of mortality of RR-TB. This finding highlights the need for advocacy for a worldwide roll-out of rapid molecular tests. Objective: The purpose of the study was to evaluate the mental health (MH) status of referees who officiate in the Turkish professional football leagues., Method: An online survey was sent to all referees in the Turkish professional football leagues (n = 630) incorporating standardized scales assessing depression, anxiety, and stress., Results: A total of 433 referees participated in the study, yielding a response rate of 68.7%. Younger referees (18-27 years) reported higher depression (p = 0.01), anxiety (p < 0.01), and stress (p < 0.01) scores than older (>38 years) refereees. Depression, anxiety, and stress scores of single referees were higher compared to married referees (all p < 0.01). Lower-level referees reported higher depression (p < 0.01), anxiety (p = 0.01), and stress (p < 0.01) scores than their higher-level counterparts. Higher depression, anxiety, and stress scores were also associated with less income, performance concerns, severe injury history, and inadequate social support., Conclusion: MH problems in referees were associated with a wide range of variables including younger age, being single, refereeing at lower-levels, performance concerns, and inadequate social support. In light of these results, MH assessments should be undertaken with referees to detect which officials are at greater risk of MH problems. Doing so will help to enable appropriate and timely MH interventions.

Deficits in movement patterns during cutting while running might place soccer players at risk of injury. The objective was to compare joint angles and intersegment coordination between sexes and ages during an unanticipated side-step cutting task in soccer players. This cross-sectional study recruited 11 male (four adolescents and seven adults) and 10 female (six adolescents and four adults) soccer players. Three-dimensional motion capture was used to measure lower-extremity joint and segment angles as participants performed an unanticipated cutting task. Hierarchical linear models examined relationships between joint angle characteristics with age and sex. Continuous relative phase was used to quantify intersegment coordination amplitude and variability. These values were compared between age and sex groups using analysis of covariance. Adult males had greater hip flexion angle excursions than adolescent males, while adult females had smaller excursions than adolescent females (p = .011). Females had smaller changes in hip flexion angles (p = .045), greater hip adduction angles (p = .043), and greater ankle eversion angles (p = .009) than males. Adolescents had greater hip internal rotation (p = .044) and knee flexion (p = .033) angles than adults, but smaller changes in knee flexion angles at precontact compared with stance/foot off (p < .001). For intersegment coordination, females were more out-of-phase than males in the foot/shank segment in the sagittal plane. There were no differences in intersegment coordination variability between groups. Differences in joint motion during an unanticipated cutting task were present between age groups and sexes. Injury prevention programs or training programs may be able target specific deficits to lower injury risk and improve performance.

Background & Aims: The association of nonalcoholic fatty liver disease (NAFLD) with dietary factors is well established but not thoroughly investigated. This systematic review and meta-analysis synthesizes available evidence regarding the effect of nutrition on the presence and severity of NAFLD., Methods: A literature search was conducted identifying studies published between January 1985 and May 2021. We included studies with a dietary assessment and anthropometry based on validated tools, performed by a qualified dietitian or a trained health professional. We examined differences between patients with NAFLD and healthy controls as well as patients with NAFLD and nonalcoholic steatohepatitis (NASH). Risk of bias was assessed with the Risk Of Bias In Non-randomised Studies of Interventions (ROBINS-I) tool., Results: There were 60 eligible studies with 100,621 patients. The risk of bias was moderate for the majority of studies (41/60; 68%). According to meta-analyses, total caloric intake was higher in patients with NAFLD compared with controls (mean difference, 78.08; 95% confidence interval, 41.03-115.13). Macronutrient (protein, fat, and carbohydrate) consumption as proportion of total caloric intake and daily intake of fiber, caffeine and vitamins E, A, and C did not significantly differ between patients with NAFLD and controls. Soft drink consumption had a trend towards association with the presence of NAFLD. However, the odds ratio was 4.4 and the confidence intervals very wide. Finally, there was no significant difference in any comparison between patients with NAFLD and NASH, although the number of patients was relatively small. All meta-analyses had significant heterogeneity., Conclusions: Overall, despite high heterogeneity among studies, this meta-analysis demonstrated that higher caloric intake is positively associated with NAFLD, whereas diet composition in macronutrients was not associated with the presence or severity of the disease., (Copyright © 2023 AGA Institute. - Abstract
Background: Patients with serrated polyposis syndrome (SPS) have multiple and/or large serrated colonic polyps and higher risk for colorectal cancer. SPS inherited genetic basis is mostly unknown. We aimed to identify new germline predisposition factors for SPS by functionally evaluating a candidate gene and replicating it in additional SPS cohorts., Methods: After a previous whole-exome sequencing in 39 SPS patients from 16 families (discovery cohort), we sequenced specific genes in an independent validation cohort of 211 unrelated SPS cases. Additional external replication was also available in 297 SPS cases. The WNK2 gene was disrupted in HT-29 cells by gene editing, and WNK2 variants were transfected using a lentiviral delivery system. Cells were analysed by immunoblots, real-time PCR and functional assays monitoring the mitogen-activated protein kinase (MAPK) pathway, cell cycle progression, survival and adhesion., Results: We identified 2 rare germline variants in the WNK2 gene in the discovery cohort, 3 additional variants in the validation cohort and 10 other variants in the external cohorts. Variants c.2105C>T (p.Pro702Leu), c.4820C>T (p.Ala1607Val) and c.6157G>A (p.Val2053Ile) were functionally characterised, displaying higher levels of phospho-PAK1/2, phospho-ERK1/2, CCND1, clonogenic capacity and MMP2., Conclusion: After whole-exome sequencing in SPS cases with familial aggregation and replication of results in additional cohorts, we identified rare germline variants in the WNK2 gene. 47. Unraveling the impact of a germline heterozygous POLD1 frameshift variant in serrated polyposis syndrome.

- Abstract
Serrated polyposis syndrome (SPS) is one of the most frequent polyposis syndromes characterized by an increased risk for developing colorectal cancer (CRC). Although SPS etiology has been mainly associated with environmental factors, germline predisposition to SPS could also be relevant for cases with familial aggregation or a family history of SPS/CRC. After whole-exome sequencing of 39 SPS patients from 16 families, we identified a heterozygous germline frameshift variant in the POLD1 gene (c.1941delG, p.(Lys648fs*46)) in a patient with SPS and CRC. Tumor presented an ultra-hypermutated phenotype and microsatellite instability. The POLD1 germline variant segregated in three additional SPS-affected family members. We attempted to create yeast and cellular models for this variant but were no viable. Alternatively, we generated patient-derived organoids (PDOs) from healthy rectal tissue of the index case, as well as from a control donor. Then, we challenged PDOs with a DNA-damaging agent to induce replication stress. No significant differences were observed in the DNA damage response between control and POLD1 -Lys648fs PDOs, nor specific mutational signatures were observed. Our results do not support the pathogenicity of the analyzed POLD1 frameshift variant. One possible explanation is that haplosufficiency of the wild-type allele may be compensating for the absence of expression of the frameshift allele. Overall, future work is required to elucidate if functional consequences could be derived from POLD1 alterations different from missense variants in their proofreading domain. 48. Competing against COVID-19: what about the mental health problems of athletes with disabilities?

- Abstract
Objectives: Although there is growing interest in athletes' mental health (MH), there is a paucity of research exploring MH in athletes with disabilities (AWD). The purpose of this study was to evaluate the impact of the COVID-19 pandemic on the psychological state of AWD., Materials and Methods: An online survey including demographic questions, SARS-CoV-2-related questions, and standardized scales assessing depression, anxiety, stress, coronavirus anxiety (CAS), and nonspecific psychological distress was distributed to AWD., Results: A total of 362 athletes (183 AWD) participated in the study (response rate 31.2%). AWD infected with SARS-CoV-2 reported higher depression, anxiety, and CAS scores than AWD not infected with SARS-CoV-2 (Z = -2.10, ES = 0.15 Z = -3.45, ES = 0.25; Z = -2.43, ES = 0.18; respectively). Female AWD reported higher anxiety scores compared to males (Z = -2.25, ES = 0.16). Depression (Z = -2.80, ES = 0.20), stress (Z = -2.12, ES = 0.15) and K-10 (Z = -2.02, ES = 0.14 scores of individual sport AWD were higher than team sports AWD., Conclusion: MH problems in AWD were associated with a wide range of variables such as being infected with SARS-CoV-2, female gender, competing in an individual sport, and having performance concerns. In light of these results, MH assessment should be undertaken for AWD to ensure appropriate and timely intervention.

49. Mental health problems and risk assessment in football players infected with SARS-CoV-2: a cross-sectional study.

- Abstract
Background: Although the negative psychological impact of the pandemic on athletes is well known, there is currently insufficient evidence regarding mental health (MH) problems among athletes infected with SARS-CoV-2. The aim of the present study was to evaluate the association between MH problems, demographics, and SARS-CoV-2 related variables in football players infected with SARS-CoV-2., Methods: An online survey included demographic questions, SARS-CoV-2 related questions, the Athlete Psychological Strain Questionnaire, and the Depression Anxiety Stress Scales-21 was distributed to male and female professional football players aged between 18-38 and infected with SARS-CoV-2., Results: A total of 261 football players participated in the study. The odds of athlete-specific distress in lower-level football players were up to 5.9 times more than those competing at the highest level (OR=3.06-5.93, 95% CI: 1.27-14.43, P<0.01). The odds of depression and stress in lower-level football players were up to 6.7 times more than those competing at the highest level (both OR=3.70-6.77, 95% CI: 1.18-19.20, P<0.01). In players who reported sleep problems, there was increased risk of athlete-specific distress (OR=2.43, 95% CI: 1.31-4.52, P<0.01), depression (OR=3.71, 95% CI: 1.98-6.95, P<0.01), anxiety (OR=2.93, 95% CI: 1.62-5.28, P<0.01), and stress (OR=7.08 95% CI: 3.52-14.02, P<0.01). There was increased risk of depression and stress in female players (OR=5.78, 95% CI: 2.03-16.4, P<0.01; OR=4.91, 95% CI: 1.57-15.4, P<0.01; respectively). The odds of anxiety in players who reported severe SARS-C0V-2 symptoms were up to 2.2 times more than asymptomatic players (OR=2.28, 95% CI: 1.08-4.82, P=0.03)., Conclusions: MH problems in football players infected with SARS-CoV-2 were associated with female gender, participation in football in the lower-level, performance concerns, and sleep problems. MH assessment is indicated in athletes infected with SARS-CoV-2 to support detection, and appropriate and timely intervention.
