It is well documented that most trauma survivors recover from adversity and only a number of them go on to develop posttraumatic stress disorder (PTSD). In addition, survivors of interpersonal trauma (IPT) appear to be at heightened risk for developing PTSD in comparison to survivors of noninterpersonal trauma (NIPT). Despite a robust association between IPT exposure and attachment disruptions, there is a dearth of research examining the role of attachment-related processes implicated in predicting PTSD. Using a sample of college undergraduates exposed to IPT and NIPT, this study explores the mediating effect of self-worth in the relationship between attachment and PTSD. It is hypothesized that insecure attachment will be related to posttraumatic symptomatology via a reduced sense of self-worth in IPT survivors but not in NIPT survivors. Mediation analyses provide support for this hypothesis, suggesting the importance of considering negative cognitions about the self in therapeutic interventions, particularly those offered to IPT survivors. The association between trauma exposure and mental health-related challenges such as depression are well documented in the research literature. The assumptive world theory was used to explore this relationship in 97 female survivors of intimate partner violence (IPV). Participants completed self-report questionnaires that assessed trauma history, world assumptions, and depression severity. Regression analyses revealed that diminished world assumptions mediate the relationship between trauma exposure and depression severity. As predicted, this relationship held for interpersonal forms of trauma, whereas noninterpersonal forms of trauma were related neither to diminished world assumption nor to depression severity. This suggests that our conceptual system of relating to the world, our core beliefs that comprise our assumptive world, may be challenged in the face of human-induced trauma, increasing our risk for developing adverse psychological outcomes such as depression. The present study uses a feminist theoretical framework to explore risk factors for the development of posttraumatic stress symptoms following intimate partner violence, with a community sample of 120 low-income European American and African American women. Hierarchical regression analyses were used to examine demographic, violence, and mental health variables that predict posttraumatic stress symptoms. The data reveal that African American women report lower levels of posttraumatic stress symptoms than do their European American peers. This difference was observed despite the presence of more empirically identified risk factors for African American women. Regression analyses show that symptoms of depression increase risk for posttraumatic stress for both groups. However, a difference was observed such that past victimization increase risk for only European American women, whereas amount of psychological violence in the previous year increases risk for only African American women. Potential explanations for observed ethnic differences are offered. (Contains 3 tables.)

Study Objectives Fear of sleep contributes to insomnia in some individuals with posttraumatic stress disorder (PTSD) but remains uncharacterized in first responders, a population with high rates of insomnia and PTSD. We evaluated the clinical relevance of fear of sleep in first responders by (1) examining its relationship with trauma types and clinical symptoms and (2) assessing differences in fear of sleep severity between those reporting provisional PTSD, insomnia, or both. Methods A cross-sectional study of 242 first responders across the United States (59.2% male, 86.4% white, 56.2% law enforcement officers, 98.7% active duty, and M years of service = 17). Participants completed the Fear of Sleep Inventory-Short Form and measures of trauma history, psychopathology (e.g. PTSD), and sleep disturbances (insomnia and trauma-related nightmares). Results Fear of sleep was associated with trauma types characterized by interpersonal violence and victimization, as well as symptoms of PTSD, depression, anxiety, stress, alcohol use problems, insomnia, and trauma-related nightmares. Fear of sleep was most pronounced among first responders reporting provisional PTSD comorbid with insomnia compared to those with PTSD or insomnia only. Post hoc analyses revealed PTSD hyperarousal symptoms and trauma-related nightmares were independently associated with fear of sleep, even after adjusting for the remaining PTSD clusters, insomnia, sex, and years of service. Conclusions Fear of sleep is a clinically relevant construct in first responders that is associated with a broad range of psychopathology symptoms and is most severe among those with cooccurring PTSD and insomnia. Interest in the mechanisms by which childhood maltreatment can lead to adult intimate partner victimization (IPV) is growing, though limited research has examined these mechanisms from the direct perspective of the victims. Using qualitative methods to examine childhood histories in a sample of 23 IPV survivors, we identified two trajectories, childhood emotional trauma and childhood physical trauma, which lead to revictimization in adulthood in the form of IPV. The emotional trauma trajectory was associated with a desire for intimacy and deficits in navigating interpersonal relationships. Problematic interpersonal schemas and a fear of loneliness swayed many of these women to stay with an abusive partner. The physical trauma trajectory was associated with desensitization and normalization of violence. Problematic interpersonal schemas, and the belief that the experience of violence is normal, promoted tolerance of IPV. Implications for research and intervention are discussed. Intimate partner violence (IPV) affects millions of women every year, often resulting in posttraumatic stress disorder (PTSD). The strength of the relationship between IPV and PTSD has been shown to be affected by factors such as the amount of violence exposure and the style of coping in which the individual engages. For example, emotion-focused coping has been shown to be strongly related to IPV exposure and the expression of PTSD symptoms. This topic was explored in IPV survivors, with the finding that more frequent use of emotion-focused coping was associated with both higher violence exposure and heightened PTSD symptoms. Regression analyses revealed that emotion-focused coping moderated the relationship between IPV exposure and PTSD symptoms. More specifically, the results suggest that while individuals low on emotion-focused coping had fewer PTSD symptoms than women who frequently used emotion-focused coping, these individuals reported higher PTSD symptoms in the presence of frequent violence exposure. For individuals who frequently engaged in emotion-focused coping, violence exposure was less strongly associated with symptoms of PTSD. Journalists are often exposed to potentially traumatic content through their profession. High levels of exposure have been found to predict high levels of psychopathology among journalists, including posttraumatic stress, alcohol use, and depressive symptoms. Predictors of such outcomes have not been extensively investigated. The current study assessed the roles of world views, institutional betrayal, and work-related trauma exposure on symptoms of posttraumatic stress disorder (PTSD), alcohol use, and depression. One hundred and fifteen journalists completed the online study and a portion subsequently completed a phone clinical interview. Greater exposure to work-related trauma was positively correlated with PTSD. Regression analyzes revealed that experiences of institutional betrayal moderated the relationship between shattered world assumptions and PTSD and alcohol use, but not depression. Specifically, among individuals who reported greater institutional betrayal, the relationship between benevolence of the world and PTSD and alcohol use was stronger. Results highlight the role of organizational factors in exacerbating symptoms of pathology. Implications include conducting research on micro- and macro-level factors that contribute to pathology in this group. Recommendations include destigmatizing disclosure of psychological needs to editors and news agencies, introducing more trainings for journalists and editorial staff focused on trauma, mental health, and how to improve and maintain psychological well-being. Research has identified two subtypes of posttraumatic stress disorder (PTSD); the traditional form that includes hyperarousal reactions, and the other involving ongoing dissociation. Dissociation has been reflected in the new diagnostic criteria for PTSD with the dissociative specification focused on the experience of high levels of depersonalization and/or derealization. PTSD that presents with ongoing dissociation appears to characterize complex cases of PTSD for individuals exposed to protracted trauma. Yet, our understanding of the role of dissociation in relation to PTSD symptoms is limited with inconsistent empirical findings. This study aims to elucidate the complex associations between trauma, dissociation, and trauma-relevant symptomatology. In total, 60 female interpersonal trauma survivors completed baseline measures of trauma and dissociation, and at least a week later, they completed self-report measures of state trauma intrusions, guilt, and anxiety before and after participating in a trauma recall task to examine dissociative tendencies on analogue trauma-related symptoms. After data collection, participants were categorized into two groups; individuals with multiple forms of lifetime interpersonal trauma versus individuals with trauma isolated to single-type trauma in childhood and/or adulthood. Results revealed trauma intrusions, guilt, and anxiety increased after trauma recall overall, though trauma groups differed in the manifestation of these outcomes. In addition, less depersonalization and greater derealization predicted increases in anxiety overall, though this only remained true for those exposed to multiple-type lifespan traumas. Results reveal dissociative tendencies may be most relevant in the processing of trauma-related material for those exposed to multiple types of trauma throughout the lifespan. Recent research has demonstrated that first responders may report posttraumatic growth (PTG), positive psychological changes that arise in the aftermath of a trauma. Less is known regarding the perception of PTG among 9-1-1 telecommunicators, a group of first responders exposed to a high degree of lifetime trauma, including duty-related trauma as well as early and non-duty-related trauma. Moreover, the impact of childhood trauma on the processes involved in the perception of growth is less clear. While some distress is needed to facilitate processes that lead to the perception of PTG, it has been suggested that positive associations between PTG and pathology reflect avoidant coping or represent an illusory component of PTG. Structural equation models were used to examine early trauma exposure, coping, and pathology in predicting PTG among 9-1-1 telecommunicators (N = 788). In separate models using active and avoidant forms of coping, childhood trauma exposure had an indirect effect on PTG through coping. In a model considering both forms of coping, childhood trauma had an indirect effect on PTG through psychopathology, but not through coping. The results show that early trauma exposure leads to the perception of growth through pathways indicative of both adaptive and maladaptive coping processes. [ABSTRACT FROM AUTHOR]

Objective: The Moms' Empowerment Program (MEP) provides affordable services to address the effects of women experiencing intimate partner violence (IPV) and to enhance their mental health. In past evaluations with mothers of school-age children this ten-session program was successful in reducing women's symptoms of posttraumatic stress. Method: A new efficacy trial compares outcomes for women with preschool-age children who received the MEP to those randomly assigned to a waitlist comparison condition. Women with young children (N = 120) who had experienced IPV living in Southeast Michigan and Ontario, Canada, were assessed at baseline, 5 weeks later (at post-intervention) and at approximately 8-month follow-up. Using standardized measures, this evaluation compared rates of change in women's symptoms of posttraumatic stress. Results: Approximately 46% of the women were diagnosed with posttraumatic stress disorder (PTSD); 94% reported re-experiencing symptoms, 85% physiological arousal symptoms, and 69% avoidance/numbing symptoms. Hierarchical Linear Modeling (HLM) results showed that the MEP was somewhat successful in reducing women's symptoms of posttraumatic stress symptoms. However, the extent of improvement was related to both age and the number of sessions attended during the program. Conclusion: This finding indicates that mothers with young children, who have experienced IPV and who are significantly distressed, may be successfully treated by attending this group intervention designed specifically to address their unique experiences and needs. Clinical impact: High rates of traumatic stress and PTSD are found in women who experience intimate partner violence. Cognitive theories of PTSD argue that poor recall of trauma memories results from a stress-induced shift toward perceptual processing during encoding. The present study assessed the extent to which self-reported state anxiety affects event segmentation and its subsequent impact on memory performance (recall and recognition). Event segmentation is the cognitive process of condensing continuous streams of spatiotemporal information into discrete elements. In this study, undergraduates without PTSD used a computer programme to segment a stressful film and a non-stressful film and then they completed memory tasks for each film. For the stressful film, low memory performance was associated with high segmentation performance. A meditational analysis revealed high segmentation performance mediated a negative relationship between state anxiety and memory performance. Additionally, ad-hoc analyses suggest perceptual processing primarily drives segmentation of the stressful film and conceptual processing primarily drives segmentation of the non-stressful film. Objective Trauma-related rumination (i.e., repetitive and recurrent thinking about trauma and its consequences) has shown to predict the development and maintenance of posttraumatic stress disorder symptoms, though little is known about its characteristics. The purpose of this study was to examine trauma-related ruminative content, correlates, and processes during a trauma-specific repetitive thinking interview. Method A total of 63 female survivors of violence completed questionnaires assessing trauma-related pathology and participated in a trauma-specific repetitive thinking interview, which was qualitatively coded. Results Most participants expressed problematic (i.e., assimilated and overaccommodated) trauma beliefs during the interview, which were associated with baseline posttraumatic sequelae. Reexperiencing symptoms mediated the relation between a brooding response style and expressed problematic trauma beliefs. State negative emotions were associated with ruminative processes during the interview and predicted negative emotions after the interview. Conclusion Maladaptive trauma-related rumination is characterized by perseveration on problematic trauma beliefs. Implications for treatment are discussed. [ABSTRACT FROM AUTHOR]
