5. Palbociclib dose patterns in Swedish patients with metastatic breast cancer : 5-year update from the SIRI study

Abstract

Background: Although palbociclib combined with endocrine therapy is a well-established treatment option in patients with hormone receptor-positive (HR+), human epidermal growth factor receptor 2 (HER2)-negative metastatic breast cancer (MBC), there is limited evidence on patterns of dose reductions in real-world setting. The Swedish Ibrance Registries Insights (SIRI) study investigated real-world dose patterns using a nationwide cohort of palbociclib-treated MBC patients... Methods: This was a retrospective study utilizing population-based Swedish Health Data Registers. The cohort included all patients ≥ 18 years with ≥ 1 dispensation of palbociclib from January 2017 – June 2022. Minimum follow-up was 3 months. Starting dose and dose changes for the full population and for different age groups, in total and over time, was investigated... Results: 2058 patients were identified and the median (IQR) age at treatment initiation was 68.4 (15.7) years. Patients were stratified into three age groups: <50, 50-69, ≥70 years, with the following distribution: 9.9%, 46.1%, and 44.0%, respectively. Most patients were initiated on 125 mg (82.0%), with a lower share for older patients (≥70 years; 74.4%). In total, 45.5% of patients had ≥ 1 dose reduction, with a higher frequency for older patients (≥70 years; 48.5%). Median (IQR) time to first dose... Conclusions: Most Swedish palbociclib-treated patients were initiated on the recommended dose, but a lower starting dose and a higher frequency of dose reductions were observed for older patients. The time to dose reduction was equal across age groups, whereas the probability for dose reduction increased over time for older patients, and for patients with a start dose of 100 mg...

Abstract

This article describes a user-centred approach taken by a group of five procurers to set specifications for the procurement of value-based research and development services for IT-sup-ported integrated hypertension management. The approach considered the unmet needs of patients and health systems of the involved regions. The procurers established a framework for requirements and a solution design consisting of nine building blocks, divided into three domains: service delivery, devices and integration, and health care organisation. The approach included the development of questionnaires, capturing patients’ and professionals’ views on possible system functionalities, and a template collecting information about the organisation of healthcare, professionals involved and existing IT systems at the procurers’ premises. A total of 28 patients diagnosed with hypertension and 26 professionals were interviewed. The interviewees identified 98 functional requirements, grouped in the nine building blocks. A total of nine use cases and their corresponding process models were defined by the procurers’ working group. As result, a digitally enabled integrated approach to hypertension has been designed to allow citizens to learn how to prevent the development of hypertension and lead a healthy lifestyle, and to receive comprehensive, individualised treatment in close collaboration with healthcare professionals.

Abstract

Teacher education has developed rapidly over the recent decades. The structure of teacher education in Iceland has undergone frequent changes and it is therefore relevant to look more closely at how and why some of these changes have been implemented. In structuring teacher education it is important to account for the fact that teachers’ professionalism is based on both professional and personal aspects. Teacher education must, therefore, include subject studies, general pedagogy as well as pedagogical content knowledge and field studies. In that way it is possible to help student teachers connect theory and practice, and use that foundation to create opportunities for successful professional development. In the school year 2019–2020, student teachers were for the first time offered a whole-year field study, in the form of a paid internship, as part of a teacher education program at the University of Iceland, School of Education. This field work is part of the whole-year course Teaching and Learning – Becoming a Professional I and II , a total of 35 credits (ECTs), of which the field study accounts for 24 credits. This course is placed in the final year at master’s level, replacing two master’s courses: Teaching and Learning – Becoming a Professional (25 credits) and Subject Teaching (10 credits). The central aim of the course is to strengthen the connection between theory in the teacher education program and praxis in the field. Campus-based teaching takes place weekly, where the diverse aspects of teaching and pedagogy are introduced and discussed in small groups. The student teachers then explore these aspects in the field, test them in their own teaching and discuss the various issues and incidents that might occur with their mentor teacher. Alongside this process, the student teachers have to reflect critically on their practice and keep a journal in which they record their experiences during the course of the year. Through formative assessment the student teachers become increasingly able to critically reflect on their teaching and the learning of their students. This paper will examine the precursors, goals, structure, and basic ideas that support the development of the course, right from the inception of its predecessor in 2013. Moreover, the paper will discuss findings from a study that focused on the student teachers’ experiences of the new course during the school year 2019–2020. The aim of the study was to analyze the ways in which the content and structure of the course benefited the student teachers in their field practice. The study collected data from interviews conducted with eight student teachers, as well as written course material. The findings from the interviews indicate that the student teachers were generally content with the course. The students claimed that the course had supported them in their teaching, and noted that the small group seminars, where they discussed the course topics and their own teaching, had proven specifically useful. Furthermore, the students underlined the importance of practicing regular and purposeful critical reflection on their own work, and stated that the practice facilitated their professional development and the shaping of their personal professional theory. The structure and topics of the course were considered useful for building a stronger foundation for a learning community, both in the teacher education program and in the field. The findings also show that the organization of the course has been a complicated process involving the coordination of several agents. This process has produced a number of hindrances over the course of the first year, which calls for solutions and improvements. It is important to learn from one’s experience and to utilize that experience to further develop the teacher education program for everyone involved; that is, the student teachers, the field schools and the institution of teacher education.

Abstract

Stress-relieving heat treatment has been reported to deplete the corrosion resistance of new low-lead and lead-free brass alloys. How the heat treatment, processing and material composition relates to the microstructure and corrosion performance is not well understood. The present study aims to fill this knowledge gap by mapping stress-relieving annealing conditions and different standardised compositions to their respective microstructures and dezincification performance. It was found that loss of corrosion resistance was only the most severe for alloys with higher aluminium and iron content. These alloys displayed significant precipitation of intermetallic aluminium arsenide particles on grain boundaries, twins and lead particles, as well as the formation of β-phase along grain boundaries. Abstract

The interactions between nutritional supplementation and physical activity on changes in physical function among older adults remain unclear. The primary objective of this study was to examine the impact of nutritional supplementation plus structured physical activity on 400M walk capacity in mobility-limited older adults across two sites (Boston, USA and Stockholm, Sweden). All subjects participated in a physical activity program (3x/week for 24 weeks), involving walking, strength, balance, and flexibility exercises. Subjects were randomized to a daily nutritional supplement (150kcal, 20g whey protein, 800 IU vitamin D) or placebo (30kcal, non-nutritive). Participants were recruited from urban communities at 2 field centers in Boston MA USA and Stockholm SWE. Mobility-limited (Short Physical Performance Battery (SPPB) ae9) and vitamin D insufficient (serum 25(OH) D 9 - 24 ng/ml) older adults were recruited for this study. Primary outcome was gait speed assessed by the 400M walk. Results: 149 subjects were randomized into the study (mean age=77.5 +/- 5.4; female=46.3%; mean SPPB= 7.9 +/- 1.2; mean 25(OH)D=18.7 +/- 6.4 ng/ml). Adherence across supplement and placebo groups was similar (86% and 88%, respectively), and was also similar across groups for the physical activity intervention (75% and 72%, respectively). Both groups demonstrated an improvement in gait speed with no significant difference between those who received the nutritional supplement compared to the placebo (0.071 and 0.108 m/s, respectively (p=0.06)). Similar effects in physical function were observed using the SPPB. Serum 25(OH)D increased in supplemented group compared to placebo 7.4 ng/ml versus 1.3 ng/ml respectively. Results suggest improved gait speed following physical activity program with no further improvement with added nutritional supplementation.
