A lesão apical está relacionada a progressão de lesões de cárie ou traumatismos dentários, que levam a polpa à inflamação e necrose, resultando, em alguns casos, em perda dentária. Quando persistente, o processo patológico não se reverte sem tratamento adequado. Objetivo: O estudo discorre sobre como, após ocorrer os fenômenos de necrose pulpar, acontece o processo de reabsorção e reparo ósseo apical, com a finalidade de compreender como a instrumentação e a limpeza do sistema de canais radiculares podem contribuir para a neoformação óssea na lesão periapical. Material e métodos: Utilizou-se o método de revisão integrativa da literatura. O estudo descreve o processo de reabsorção óssea e os seus estágios, destacando a importância dos receptores RANK, RANKL e a OPG, os quais possuem significativa importância na regulação do metabolismo dos tecidos duros. Resultados: A limpeza do sistema de canais radiculares é relevante para a supressão de agentes estimuladores de reação imunoinflamatória, como microrganismos e citocinas pró-inflamatórias. Nesse processo, o hidróxido de cálcio possui alta eficiência para eliminação de bactérias, principalmente anaeróbicas. Conclusão: Para alcançar o reparo da lesão periapical, é necessário atuar modulando a reação imunoinflamatória no local, removendo os agentes patogênicos, mediante técnicas e produtos que estimulem a ligação de RANKL/OPG, de modo a favorecer a estimulação de células blásticas, que promoverão o reparo do tecido ósseo.

O SARS-CoV-2 é um vírus que causou grande impacto no mundo. Possui alta transmissibilidade, infectando muitas pessoas em pouco tempo e causando a covid-19, doença que pode ou não ser assintomática. Os sintomas mais comuns são: febre, tosse, dor de cabeça, dor no corpo (muscular), náusea, fadiga, falta de ar ou dificuldade para respirar, diarreia, cansaço, perda do olfato (anosmia) e do paladar (disgeusia). A disgeusia ocorre por meio da atração da proteína spike (S), que o vírus possui, pelas enzimas conversoras de angiotensina-2 (ACE2), presentes na mucosa oral, língua e regiões envolvendo o trato respiratório. Essa invasão causa uma inflamação nas células, fazendo com que haja perda, ou não, de suas funções, como a sensação do paladar. Objetivo: Avaliar os sintomas apresentados por pessoas que testaram positivo para SARS-CoV2, com enfoque especial na ocorrência de disgeusia e suas manifestações clínicas. Material e métodos: Foi realizado um estudo, por meio de um questionário produzido pelo Google Forms, com 17 perguntas, para avaliar os sintomas apresentados por pessoas infectadas pelo SARS-CoV-2. Obtiveram-se 261 respostas. Resultados: Mulheres e pessoas mais jovens são mais propensas a apresentarem manifestações causadas pela variação delta do vírus SARS-CoV-2, incluindo a perda no paladar. Os sintomas mais relatados foram cansaço, dor no corpo e dor de cabeça. A disgeusia foi relatada por 172 (65,90%) pacientes. As disfunções olfatórias e gustativas foram encontradas simultaneamente em 138 (52,87%) infectados. Conclusão: A disgeusia é um sintoma comum relacionado à infecção por SARS-CoV-2 e causa um grande impacto na vida das pessoas, sendo, portanto, parte das responsabilidades do cirurgião-dentista identificá-la e orientar seus pacientes. Além disso, foi possível notar que, apesar de anosmia e disgeusia se manifestarem simultaneamente em diversos casos, uma disfunção não depende da outra.

This study aimed to compare the survival of replanted teeth that followed the 2012 or the 2020 International Association of Dental Traumatology (IADT) guidelines. Sixty-two permanent replanted teeth were retrospectively assessed (IADT 2012, n = 45; IADT 2020, n = 17). Five years after replantation (from January 2017 to December 2021), clinical and radiographic examinations were performed. A significance level of 95% was considered to evaluate the outcomes. Thirty-one teeth (50.0%) remained in their sockets and 31 (50.0%) were lost due to external root resorption. Of the 25 (40.3%) teeth replanted within one hour, 16 (64.0%) remained in their sockets, and 9 (36.0%) were lost. Twenty-two (71.0%) of all 31 lost teeth had an extra-alveolar time of more than one hour. Twelve teeth remained in their sockets without resorption: 8 (66.7%) were replanted within one hour, 2 (16.7%) followed the 2012 IADT, and 2 (16.7%) the 2020 IADT guidelines for late replantation. There was a significant difference (p 0.05). Replanted teeth following both, 2012 or 2020 IADT guidelines, have similar clinical outcomes. The extra-alveolar time of less than one hour was demonstrated to be important to keep the permanent tooth in its socket. Resumo Este estudo teve como objetivo comparar a sobrevida, por cinco anos, de dentes reimplantados que seguiram as diretrizes de 2012 ou 2020 da International Association of Dental Traumatology (IADT). Sessenta e dois dentes permanentes reimplantados foram avaliados retrospectivamente (IADT 2012, n = 45; IADT 2020, n = 17). Cinco anos após o reimplante, foram realizados exames clínicos e radiográficos. Foi considerado um nível de significância de 95% para avaliar os desfechos. Trinta e um dentes (50,0%) permaneceram em seus alvéolos e 31 (50,0%) foram perdidos por reabsorção radicular externa. Dos 25 (40,3%) dentes reimplantados em uma hora, 16 (64,0%) permaneceram em seus alvéolos e 9 (36,0%) foram perdidos. Vinte e dois (71,0%) de todos os 31 dentes perdidos tiveram um tempo extra-alveolar superior a uma hora. Doze dentes permaneceram em seus alvéolos sem reabsorção: 8 (66,7%) foram reimplantados em uma hora, 2 (16,7%) seguiram a IADT de 2012 e 2 (16,7%) as diretrizes da IADT de 2020 para reimplante tardio. Houve diferença significativa (p 0,05). Dentes reimplantados seguindo as diretrizes de 2012 ou 2020 da IADT, tiveram taxas de sucesso semelhantes. O tempo extra-alveolar inferior a uma hora demonstrou ser importante para manter o dente permanente em seu alvéolo.

O autocuidado em saúde bucal dos preparadores de alimentos/merendeiras pode ser considerado um fator de grande importância para garantir a saúde bucal das crianças que consomem os alimentos por eles preparados. É de suma importância que esses profissionais, que atuam na instituição escolar, estejam capacitados e atentos à adoção de práticas de higiene bucal, evitando a proliferação de doenças bucais. Objetivo: O objetivo deste trabalho foi avaliar o conhecimento sobre saúde bucal e sua relação com a transmissibilidade de doenças dos preparadores de alimentos/ merendeiras de escolas de Curitiba (PR), observando se suas ações trazem maior possibilidade de causar danos à saúde bucal das crianças das escolas/creches da cidade. Material e métodos: Da amostra, constaram 60 merendeiras, as quais responderam a questionários durante seu período normal de trabalho. A maior parte das merendeiras (68% da amostra) afirmou não ter o conhecimento sobre como evitar a doença cárie, reforçando a necessidade de educação em saúde bucal. Na questão da transmissibilidade da doença cárie e demais doenças bucais para as crianças, 72% delas alegaram que não podem transmitir nem/ou influenciar na doença cárie, evidenciando a falta de informação sobre o processo de transmissibilidade das doenças bucais. A questão do autocuidado em saúde bucal se torna fundamental para um programa preventivo em saúde. Resultados: Do total, 95% das merendeiras relataram nunca ter participado de reuniões organizadas pela escola/creche abordando o tema saúde bucal, uma vez que essas reuniões nunca ocorreram. Percebeu-se claramente que há falta de informação no sistema de ensino em geral. Quanto à necessidade de tratamento odontológico, 65% disseram precisar de tratamento. Essa informação reforça a importância do autocuidado de saúde bucal, mesmo quando esses profissionais não relatam um real impedimento em buscar atendimento odontológico. Conclusão: Quando bem informadas sobre o assunto de educação em saúde bucal, as merendeiras podem desenvolver atividades preventivas que buscam alcançar os conhecimentos, as atitudes e as práticas desejáveis.

Durante o atendimento odontológico, há dispersão de respingos e aerossóis contendo microrganismos patogênicos, oriundos da saliva, sangue e secreções bucais. Esse bioaerossol formado pode permanecer suspenso por um período antes de decair, contaminando as superfícies próximas. Objetivo: Avaliar o alcance da contaminação por bioaerossóis produzidos durante um atendimento dentário na clínica odontológica de uma universidade. Material e métodos: Para o experimento, foram utilizadas dez placas de Petri contendo ágar Sabouraud e ágar Mueller Hinton, espalhadas em três áreas internas da clínica, para avaliar o alcance dos bioaerossóis, e duas áreas externas, na sala de raio X e no lado de fora da entrada da clínica, sem presença de bioaerossóis. Cinco placas ficaram expostas por 30 minutos antes do atendimento clínico e cinco 30 minutos durante o atendimento clínico, nas mesmas posições. Depois, elas foram lacradas e permaneceram por 48 h em estufa incubadora a 37ºC. Em seguida, realizou-se avaliação visual do crescimento microbiano das placas. Resultados: Todas as placas apresentaram contaminação; nas placas de Petri perto da janela houve maior crescimento de microrganismos. Não houve diferença na contaminação das placas de Petri, antes e durante o atendimento dentário, que permaneceram na sala de raio X e fora da clínica. Conclusão: Este estudo destaca a contaminação biológica disseminada por aerossóis durante atendimento odontológico em ambiente clínico universitário, alertando para os cuidados de desinfecção de superfícies, e demais ambientes internos de clínicas universitárias

Because of the COVID-19 pandemic, people all over the world had to interrupt their face-to-face interactions. Health professionals were most affected, as they were at the forefront of combating this disease. Biosecurity measures had to be re-doubled to prevent the spread of the virus and thus help control the pandemic. Procedures in dentistry that generated aerosols were avoided, as saliva is one of the means of COVID-19 transmission. Complementary Personal Protective Equipment (PPE) was required to ensure protection, and environmental disinfection protocols had been modified. Thus, to assess the level of knowledge about biosafety and these new changes, particularly regarding COVID19, a questionnaire was published for the community related to health courses in the city of Curitiba (PR, Brazil). This study included 159 students (dentistry, 107 (67.3%); medicine, 28 (17.6%); biomedicine, 17 (10.7%); and nursing, seven (4.4%)). Approximately 63.55% of dentistry, 100% of medicine, 64.71% of biomedical, and 85.71% of nursing students believe that undergraduate students are unprepared to provide urgent care to COVID-19 patients. Despite all the guidance on biosafety, students in healthcare must still develop skills in correct procedures for caring for patients with or without communicable diseases. A pandemia de COVID-19 impôs a interrupção de atividades presenciais coletivas e os profissionais de saúde foram os mais afetados, por estarem na linha de frente no combate desta doença. Medidas de biossegurança tiveram que ser redobradas para evitar a propagação do vírus e assim ajudar a controlar a pandemia. Procedimentos que geram aerossóis na Odontologia foram evitados, pois é de conhecimento que um dos meios de transmissão é a saliva. Alguns Equipamentos de Proteção Individual (EPs) foram adicionados para garantir a proteção, e protocolos de desinfecção do ambiente foram modificados. O objetivo deste estudo foi avaliar o grau de conhecimento sobre biossegurança entre estudantes da área da saúde. Foi aplicado um questionário eletrônico para a comunidade relacionada aos cursos da área da saúde na cidade de Curitiba/PR. Cento e cinquenta e nove alunos responderam ao questionário, sendo 107 (67,3%) estudantes da Odontologia, 28 (17,6%) da Medicina, 17 (10,7%) da Biomedicina e 7 (4,4%) da Enfermagem. Entre os participantes, 63,55% dos alunos de Odontologia, 100% de Medicina, 64,71% de Biomedicina e 85,71% de Enfermagem acreditam que os alunos da graduação não estão preparados para fazer atendimentos de urgência em pacientes com COVID-19. Apesar de toda a orientação sobre biossegurança, os alunos da área da saúde ainda devem desenvolver habilidades quanto aos corretos procedimentos para atendimento de pacientes portadores ou não de doenças transmissíveis.

A erva-mate (Ilex paraguariensis) é uma planta que possui propriedade antimicrobiana, por isso sua aplicabilidade como medicação intracanal na Endodontia. Objetivo: Considerando que ser permeável é requisito fundamental da medicação intracanal, o presente trabalho teve como objetivo avaliar a difusão do extrato de erva-mate através da dentina radicular. Material e métodos: Sessenta raízes de dentes permanentes humanos foram preparadas com instrumentos Protaper até a lima F3. Os canais foram irrigados com hipoclorito de sódio 2,5% e, ao final, com ácido etilenodiaminotetracético (EDTA) 17%. Os ápices foram selados com resina composta. Criaramse sete grupos, de acordo com a medicação intracanal utilizada: G1 – canais vazios; G2 – hidróxido de cálcio 500 mg/ml; G3 – extrato de erva-mate 500 mg/ml; G4 – extrato de erva-mate 250 mg/ml; G5 – extrato de erva-mate 125 mg/ml; G6 – hidróxido de cálcio 500 mg/ml + extrato de erva-mate 125 mg/ml e G7 – água destilada. Os espécimes foram acondicionados em tubos de ensaio tampados, contendo 20 ml de água destilada. Todas as amostras tiveram seu pH aferido nos dias 0, 3, 7, 14 e 30. Resultados: O G6 obteve resultado significante no dia 3; o G3, no dia 30. Não houve diferença estatística entre os demais grupos. Conclusão: O extrato de erva-mate 500 mg/ml apresentou permeabilidade dentinária em longo período de contato. Observou-se também que o hidróxido de cálcio permeou a dentina mais rapidamente quando associado à erva-mate 125 mg/ml.

The absence or presence of root resorption on the root surface of a replanted tooth indicates an immune-inflammatory reaction. Since interleukin-6 (IL-6) is considered an inflammatory marker in bone resorption, this study aimed to investigate the association between clinical variables and polymorphisms in IL6, with the outcome of replanted teeth at 1-year follow-up. Multiple signaling molecules have been shown to play crucial roles in dental root development. Therefore, we aimed to investigate the prevalence of S-shaped roots and also to investigate, if single nucleotide polymorphisms (SNPs) in BMP2, BMP4 and SMAD6 are associated with this phenotype in humans.This is a cross-sectional phenotype-genotype association study that used radiographs to determine the phenotypes and DNA to investigate SNPs in candidate genes. During the radiographic exam, teeth presenting root canal(s) doubly curved were considered S-shaped roots. SNPs in BMP2 (rs1005464 and rs235768), BMP4 (rs17563) and SMAD6 (rs2119261 and rs3934908) were blindly genotyped by real-time PCR using TaqMan assay. The relative and absolute frequency of S-shaped roots were calculated. Chi-square test was used to compare the genotype distributions between control and S-shaped groups.Among the 578 subjects, 61 (10.6 %) presented at least one tooth with an S-shaped root. The most commonly affected type of tooth was the premolar. rs1005464 in BMP2 was statistically associated with an S-shaped root (p = 0.036). rs235768 in BMP2 was associated with an S-shaped root also in mandibular teeth (p = 0.017). A statistical significance was observed for the rs3934908 in SMAD6 (p = 0.049) for S-shaped root in the mandible. In the analysis stratified according to the type of tooth, rs235768 in BMP2 was associated with S-shaped roots in premolars (p = 0.029).The prevalence of S-shaped roots is 10.6 % in permanent teeth. SNPs in BMP2 and SMAD6 could be involved in a higher chance to present S-shaped roots.

Introduction: The World Health Organization (WHO) warned of a “Public Health Emergency of International Concern” at the end of 2019. Due to Covid-19, preventive and limiting strategies had to be adhered to, such as isolation and social distancing. This new global public health problem has affected several areas, including education. Because of the above, the objective of this study was to evaluate the perception of professors and students of the School of Dentistry of a private university about teaching in a remote regime. Methodology: Two different questionnaires were made available to professors and students. The data were tabulated and a descriptive analysis was performed because of the perception of remote teaching. Results: One hundred and four students and 26 professors answered the questionnaires. 76% of students and 100% of professors responded that they agreed with the remote method during a pandemic. However, 74% of students and 61.5% of professors disagree with the implementation of a blended learning model in the future. 74% of students reported difficulty focusing during remote classes, despite 74% of students rated their professors' performance as good or excellent. 56.7% of professors and 60.6% of students agreed with the improvement of learning with the use of tools such as Google Forms® and Kahoot®. Conclusion: students and professors felt safe with the remote teaching regime during the pandemic. However, it is estimated that after the pandemic period, face-to-face teaching will be encouraged again. Keywords: Distance learning. Remote learning. Alternative learning techniques. Introdução: A Organização Mundial de Saúde (OMS) alertou uma “Emergência de Saúde Pública de Importância Internacional” ao final do ano de 2019. Devido à Covid-19 estratégias preventivas e limitantes precisaram ser aderidas, como o isolamento e distanciamento social. Este novo problema de saúde pública global que afetou diversas áreas, dentre elas a educação. Diante o exposto, o objetivo deste trabalho foi avaliar a percepção dos professores e alunos do curso de Odontologia de uma universidade privada sobre o ensino em regime remoto. Metodologia: Dois questionários distintos foram disponibilizados para professores e alunos. Os dados foram tabulados e uma análise descritiva foi realizada perante a percepção sobre ensino remoto. Resultados: Cento e quatro alunos e 26 professores responderam os questionários. 76% dos alunos e 100% dos professores responderam que concordavam com o método remoto em época de pandemia. Entretanto, 74% dos alunos e 61,5% dos professores discordam com a implementação de um modelo de ensino híbrido no futuro. 74% dos alunos relataram dificuldade de foco durante aulas remotas, apesar de 74% dos alunos terem considerado o desempenho dos professores como bom ou excelente. 56,7% dos professores e 60,6% dos alunos concordaram com a melhoria do aprendizado com a utilização de ferramentas como o Google Forms® e Kahoot®. Conclusão: alunos e professores se sentiram seguros com o regime de ensino remoto durante a pandemia. Contudo, estima-se que após o período pandêmico, o ensino presencial seja novamente encorajado. The study aimed to explore the influence of genetic polymorphisms in ANKK1 and DRD2 on the signs and symptoms of temporomandibular disorder (TMD) in construction workers. This cross-sectional study included only male subjects. All construction workers were healthy and over 18 years age. Illiterate workers and functionally illiterate workers were excluded. The diagnosis of TMD was established according to the Research Diagnostic Criteria for TMD (RDC/TMD). Genomic DNA was used to evaluate the genetic polymorphisms ANKK1 (rs1800497) and DRD2 (rs6275; rs6276) using Real-Time PCR. Chi-square or Fisher exact tests were used to evaluate genotypes and allele distribution among the studied phenotypes. The established alpha of this study was 5%. The sample included a total of 115 patients. The age of the patients ranged from 19 to 70 years (mean age 38.2; standard deviation 11.7). Chronic pain (87.7%), disc displacement (38.2%), and joint inflammation (26.9%) were the most frequently observed signs and symptoms. The genetic polymorphism rs6276 in DRD2 was associated with chronic pain (p=0.033). In conclusion, our study suggests that genetic polymorphisms in DRD2 and ANKK1 may influence TMD signs and symptoms in a group of male construction workers. Resumo O objetivo do estudo foi explorar a influência de polimorfismos genéticos em ANKK1 e DRD2 sobre os sinais e sintomas da disfunção temporomandibular (DTM) em trabalhadores da construção civil. Este estudo transversal incluiu apenas indivíduos do sexo masculino. Todos os trabalhadores da construção civil eram saudáveis e maiores de 18 anos. Foram excluídos os trabalhadores analfabetos e analfabetos funcionais. O diagnóstico de DTM foi estabelecido de acordo com o Research Diagnostic Criteria para DTM (RDC/TMD). O DNA genômico foi usado para avaliar os polimorfismos genéticos ANKK1 (rs1800497) e DRD2 (rs6275; rs6276) usando PCR em tempo real. Testes qui-quadrado ou exato de Fisher foram utilizados para avaliar genótipos e distribuição de alelos entre os fenótipos estudados. O alfa estabelecido deste estudo foi de 5%. A amostra incluiu um total de 115 pacientes. A idade dos pacientes variou de 19 a 70 anos (média de idade 38,2; desvio padrão 11,7). Dor crônica (87,7%), deslocamento de disco (38,2%) e inflamação articular (26,9%) foram os sinais e sintomas mais observados. O polimorfismo genético rs6276 em DRD2 foi associado a dor crônica (p=0,033). Em conclusão, nosso estudo sugere que polimorfismos genéticos em DRD2 e ANKK1 podem influenciar sinais e sintomas de DTM em um grupo de trabalhadores da construção civil do sexo masculino.

To investigate the genetic association in a sample of replanted teeth, it is necessary to observe the extreme phenotypes, such as, teeth that underwent functional healing and those extracted due to severe external root resorption. Thus, this study aimed to investigate the association of age of the patients, root development, storage media, and polymorphisms in the interleukin 4 (IL4) and interleukin 6 (IL6) genes with teeth that presented extreme outcomes, as functional healing or extraction, in a group whose replantation techniques did not follow the International Association of Dental Traumatology (IADT) 2012 guidelines. Forty-three avulsed and replanted teeth that did not follow IADT 2012 guidelines and underwent functional healing or were extracted were included. Periapical radiographs employed for this study were taken soon after tooth replantation and after 1 year. For genotypic IL4 and IL6 genes analysis, DNA of oral mucosa cells was extracted. Real-time- PCR performed for genotyping polymorphisms in IL4 and IL6 genes. Clinical and genetic variables were analyzed by the Chi-square test and the “Z” test. P values < .05 were considered significant. The results showed that functional healing and extraction were associated with storage media and with the rs2243268 of IL- 4 gene polymorphisms. As conclusion, the C rs2243268 allele of IL4 gene may have a positive relationship with functional healing teeth that were replanted not following the 2012 IADT guidelines. Keeping the tooth dry is associated to a fast loss of avulsed and replanted teeth after 1-year follow-up. Resumo Para investigar a influência genética em uma amostra de dentes reimplantados, é necessário observar os fenótipos extremos, como os dentes que sofreram cura funcional e os extraídos devido à reabsorção radicular externa severa. Assim, este estudo teve como objetivo investigar a associação da idade dos pacientes, desenvolvimento radicular dos dentes, assim como o meio de transporte até o reimplante e polimorfismos nos genes da interleucina 4 (IL4) e da interleucina 6 (IL6) com dentes que apresentaram cura funcional ou extração, em um grupo cujas técnicas de reimplante não seguiu a International Association of Dental Traumatology (IADT) 2012. Foram incluídos 43 dentes avulsionados e reimplantados que não seguiram as diretrizes do IADT, e tiveram cura funcional ou foram extraídos. As radiografias periapicais utilizadas para este estudo foram feitas logo após o reimplante dentário e após 1 ano. Para a análise genotípica dos genes IL4 e IL6, foi extraído DNA de células da mucosa oral. PCR em tempo real realizou a análise dos polimorfismos dos genes. The simple bone cyst is a benign osteolytic lesion, uncommon in the maxillomandibular region. Its etiology remains unknown, although traumatic injuries are often associated with this injury. Young patients, men and women alike are the most affected. It is asymptomatic in most cases, being diagnosed on routine radiographs. The radiolucent image, uni or multilocular, is well defined, with irregular margins, mainly in the posterior region of the mandible. Bilateral presentation is unusual. The final diagnosis is made during surgical procedures when an empty bone cavity is detected. Simple curettage is the treatment of choice and it consists of causing bleeding and filling the cavity with a blood clot. Bone repair will be observed in the follow-up procedures. The aim of this study was to present a rare case of bilateral simple bone cyst in a young male, treated with curettage of the bone walls.

Introdução: A avulsão é caracterizada pelo deslocamento do dente para fora da cavidade alveolar, ocasionando danos no ligamento periodontal e na polpa dentária. Seu prognóstico depende das ações imediatas ao acidente, e o reimplante é a melhor forma de devolver função e estética ao paciente. Objetivo: Comparar os três últimos protocolos da International Association of Dental Traumatology (IADT) e avaliar a evolução do tratamento, destacando a terapêutica preconizada. Material e Métodos: Foram utilizadas diretrizes da IADT publicadas nos anos 2007, 2012 e 2020 e artigos relacionados à avulsão dentária, pesquisados nas plataformas digitais Pubmed e Google Acadêmico. Uma análise de classificação dos artigos encontrados, pelo indicador Scimago Journal Rank (SJR) foi utilizada. Foram incluídos apenas estudos que constaram classificações de Q1 e Q2. Resultados: atualizações importantes em meios de armazenamento com osmolaridade fisiológica, limpeza do dente avulsionado, terapia antibiótica, tratamento do canal radicular, tratamento da superfície radicular e retirada do coágulo da cavidade alveolar foram implementadas. Conclusão: A atualização dos profissionais em relação aos protocolos terapêuticos relacionados à avulsão se faz necessária tendo em vista a ampla atualização quanto as estratégias de manejo preconizadas.

Many studies address the quality of root canal treatments, but few refer to endodontics using rotatory techniques performed by novice operators. This study evaluated the performance of undergraduates in their first contact with rotary root canal instrumentation concerning the findings of the final periapical radiograph, and the post endodontic treatment pain. A longitudinal observational study was performed on periapical radiographs of 491 teeth from 450 patients submitted to root canal treatment by undergraduate students from 2015 to 2018. The analysis of the length of root canal filling followed the criteria: (i) acceptable, if periapical radiograph presents root filling ending 0 - 1 mm short of radiographic apex; (ii) over, if periapical radiograph presents root filling ending beyond the radiographic apex; and (iii) under, if periapical radiograph presents root filling ending > 1 mm short of radiographic apex. Evaluation of postendodontic treatment pain was categorized into either absence or presence of pain. Adequate length root canal filling was observed in 65.9% of the cases (324 teeth). Periapical lesions presence and dimensions did not interfere to the obturation quality. Statistical relation was found between the pulp condition and postendodontic treatment pain. The presence of pain was observed in 4.7% of the vital teeth and 0.3% of non-vital teeth. The presence of periapical lesion did not influence postoperative pain. Adequate length root canal filling was observed in most cases and NitTi rotary instrumentation had applicability in undergraduate programs, even with novice operators. Besides that, pulp condition had an effect on postendodontic pain. Muitos estudos abordam a qualidade dos tratamentos de canal radicular, mas poucos se referem à endodontia por meio de técnicas rotatórias realizada por operadores novatos. Este estudo avaliou o desempenho de graduandos no primeiro contato com a instrumentação endodôntica em relação aos achados da radiografia periapical final e à dor pós-tratamento endodôntico. Um estudo observacional longitudinal foi realizado em radiografias periapicais de 491 dentes de 450 pacientes submetidos a tratamento endodôntico por estudantes de graduação no período de 2015 a 2018. A análise do comprimento da obturação de canais radiculares obedeceu aos critérios: (i) aceitável, se a radiografia periapical apresentasse término de obturação 0 - 1 mm aquém do ápice radiográfico; (ii) acima, se a radiografia periapical apresentasse obturação que termina além do ápice radiográfico; e (iii) abaixo, se a radiografia periapical apresentasse obturação, terminando > 1 mm aquém do ápice radiográfico. A avaliação da dor pós-operatória foi categorizada em ausência ou presença de dor. A obturação de canais radiculares de comprimento adequado foi observada em 65,9% dos casos (324 dentes). A presença e as dimensões das lesões periapicais não interferiram na qualidade da obturação. Foi encontrada relação estatística entre a condição pulpar e a dor pós-tratamento endodôntico. Presença de dor foi observada em 4,7% dos dentes vitais e 0,3% dos não vitais. A presença de lesão periapical não influenciou na dor pós-operatória. O comprimento de trabalho adequado foi observado na maioria dos casos e a instrumentação rotatória NitTi teve aplicabilidade em programas de graduação, mesmo com operadores iniciantes. Além disso, a condição pulpar afetou a dor pós-operatória.

Introduction The goal of replantation after dental avulsion is to maintain the tooth in its socket. Presence of vital periodontal ligament cells on the root surface of the replanted tooth and the immunologic status of the patient are factors that protect against root resorption. It is known that dental constituents play an active role in root resorption by inducing specific and non-specific immune responses; however, little information exists regarding the influence of the acquired immune response on replantation. Therefore, the aim of this study was to evaluate the 5-year outcome of replantation in non-atopic and atopic patients. Methods Clinical and radiographic examinations were performed in 62 replanted teeth during a 5-year period. The evaluation of atopy was based on the patients’ personal and family histories and skin prick test results. Introduction The absence or presence of root resorption on the surface of a replanted tooth indicates an immune-inflammatory reaction. Recent research even suggests the participation of host predominant immunologic profile on types of resorptions detected on the root surface. Because interleukin 4 (IL-4) is an important anti-inflammatory cytokine, this study aimed to investigate the association of clinical variables and polymorphisms in IL4 with types of resorption of replanted teeth after 1 year of follow-up. Methods One hundred twenty-seven avulsed teeth that were replanted were selected. Periapical radiographs were taken after replantation and for 1 year to detect the types of root resorption. Real-time polymerase chain reaction was used to genotype IL4 polymorphisms. The χ2 and Z tests were performed to verify the association of clinical and genetic variables with the outcomes of replanted teeth (P Results An association was observed of extra-alveolar time, storage medium, and development of the root (P .05). Conclusions Extraoral time, storage medium, and development of the root, but not IL4 polymorphisms, may influence the types of resorption of avulsed and replanted teeth in the first year after trauma.

Replantation is the most indicated procedure for traumatic dental avulsion, but it invariably results in a greater or lower degree of root resorption. The outcomes of these cases can be affected by some well-known factors. Because studies have shown the importance of the innate and acquired immune response in the mechanisms involved in the resorption of bone tissues, an endeavor was made to verify if such an influence applies to dental resorption.Fifty-seven avulsed, endodontically treated teeth were assessed in the Endodontics Clinic, Pontifical Catholic University of Paraná. Follow-up of the replanted teeth included periodical clinical examinations and radiographs, in accordance with the control rules of the International Association of Dental Traumatology. The evaluation of atopy was based on the patient's personal and family history in conjunction with the skin-prick test for five allergen extracts.Of the 46 teeth with a favorable outcome, 33 (71.74%) were in atopic patients, and 13 (28.26%) were in non-atopic patients. Of the 11 teeth with an unfavorable outcome, four (36.36%) were in atopic patients, and seven (63.64%) were in non-atopic patients; a greater prevalence of unfavorable prognoses occurred in non-atopic patients.The outcome after 1 year for avulsed and replanted teeth is more favorable in atopic patients.

Introduction The aim of this study was to evaluate any potential influence of a patient’s Th2 immunologic profile on the occurrence of root resorption in avulsed and replanted teeth; tooth replantation cases in which different post-trauma management protocols were examined. Materials and Methods In this study, the presence or absence of root resorption was evaluated in 88 avulsed and replanted teeth. In the International Association of Dental Traumatology (IADT) group, the replantation technique and endodontic treatments were performed in accordance with the IADT guidelines. In the non-IADT group, the teeth were replanted using techniques and protocols that deviated from the IADT guidelines in some way. Follow-up of the replanted teeth included monthly clinical and radiographic examinations over the course of 1 year. Evaluation of patients’ atopy was based on a medical examination, personal and family allergic history, and a skin prick test. Results Of 47 teeth of atopic patients, 39 (82.98%) did not exhibit root resorption, whereas 8 (17.02%) did. Of the 41 teeth of nonatopic patients, 24 (58.54%) did not exhibit resorption, whereas 17 (41.46%) did. Conclusions These results show the importance of a predominantly Th2 immunological profile, which was represented by atopic patients, in replantation after dental avulsion despite differences in post-trauma management.

Background The aim of this in vivo study was to evaluate the correlation between the occurrence of root resorption in avulsed and replanted teeth, extra-alveolar time, and the possible influence of Th2 immunologic profile, represented by patient's atopy and its influence in the prognosis of the tooth. Methods This study evaluated the presence or absence of root resorption in 57 avulsed and replanted teeth. The replantation technique and endodontic treatments were performed following the International Association of Dental Trauma guidelines. Follow-up of the replanted teeth included monthly clinical and radiographic examinations during 1 year. The patient's medical examination, including the personal and family allergic history, in conjunction with the skin prick test based the evaluation of patient's atopy. Results Of the 37 teeth of atopic patients, 33 (89.2%) teeth did not present root resorption. Of these, 27 (72.9%) had an extra-alveolar time of 1 hour. Four (10.8%) teeth presented root resorption and an extra-alveolar period of time >1 hour. Conclusion These results showed the importance of the extra-alveolar time and the predominantly Th2 immunologic profile, represented by atopic patients, that suffered replantation after dental avulsion in a 1-year follow-up.

OBJECTIVES: The aim of this study was to evaluate cases of dental trauma treated at the specialized center of Pontifical Catholic University of Paraná, Curitiba, Brazil, during a period of 2 years. MATERIAL AND METHODS: A total of 647 patients were evaluated and treated between 2003 and 2005. Data obtained from each patient were tabulated and analyzed as to gender, age, etiology, time elapsed after the injury, diagnosis (type of trauma), and affected teeth. RESULTS: The results revealed that male individuals aged 7 to 13 years presented the highest prevalence of injury, and falling was the main causal factor. In most cases, the time elapsed between the accident and the first care ranged from 4 to 24 h. A total of 1,747 teeth were affected, with higher incidence of concussion/subluxation and coronal fracture, followed by lateral luxation and avulsion. The permanent maxillary central incisors were the most commonly affected teeth. CONCLUSION: The frequency and causes of dentoalveolar trauma should be investigated for identification of risk groups, treatment demands and costs in order to allow for the establishment of effective preventive measures that can reduce the treatment duration and costs for both patients and oral health services.
