Pesticides are widely used globally. Due to their widespread use, exposure to pesticides is of concern. In addition to occupational exposure, residential exposure during pesticide application is a concern for those living in or near agricultural areas. The objective of this study was to analyze the association between residential proximity to agricultural pesticide use and birth outcomes. The association between residential proximity to agricultural land use as pesticide exposure and birth outcomes was explored in a birth cohort including 283 pregnant women from a medical center in Hualien during 2013-2016. In the third trimester, we collected demographic information and the residential addresses of pregnant women via structured questionnaires. After delivery, newborn birth weight, gestational age, and head circumference were collected from medical records. Maternal residential address during pregnancy was collected for geospatial mapping. The percentages of farmland area within circular buffers of different sizes were applied to estimate pesticide exposure. Associations between residential proximity to agricultural land use as pesticide exposure and birth outcomes were analyzed by multiple linear regression analyses. A significantly smaller head circumference was associated with agricultural farmland located within 400 m (β = - 0.51 [95% CI, - 0.99, - 0.03], P = 0.037) and 500 m (β = - 0.67 [95% CI, - 1.14, - 0.19], P = 0.006) radii of residences in the tertile 2 group. A significantly smaller head circumference was also associated with dry farming area within 400 m (β = - 0.70 [95% CI, - 1.17, - 0.24], P = 0.003) and 500 m (β = - 0.81 [95% CI, - 1.27, - 0.34], P = 0.001) radii of residences in the tertile 2 group. The multivariate linear regression analyses did not show any significant association between residential farmland area and birth weight or gestational age. In conclusion, residential proximity to agricultural land use as pesticide exposure was associated with negative infant birth outcomes, especially a small head circumference. In the future, agricultural land use information could be combined with biological samples to more accurately assess exposure in pregnant women.

Phthalic acid esters (PAEs) commonly used as plasticizers are distributed ubiquitously in the living environment. We conducted a field study to examine the associations between PAE residue in dust and home characteristics/living habits in 47 rural homes in Taiwan. A questionnaire regarding home characteristics/living habits and composite sampling of house dust were conducted in each participating home. Five PAEs were selected for analysis on gas chromatography-mass spectrometry with the limits of quantification being 0.5 ng/g or lower. Uni- and multivariate linear regression analyses were performed for examining the associations. The five PAEs were prevalently detected from the samples, and the concentrations were below 1000 ng/g; di(2-ethylhexyl) phthalate (DEHP) was the most frequently detected PAE (85%), whereas di-isobutyl phthalate (DiBP) appeared to the most abundant congener with the maximum concentration of 807.65 ng/g. Floor cleaning frequency and use of detergents for floor cleaning were significantly associated with DEHP in dust (P < 0.05), suggesting additives of plasticizers in detergent products. The factors of plastic wraps in storage and use of disposable cups were both significantly related to DiBP (P < 0.01), which could be extensively used in food packaging products. We confirmed that several home characteristics/living habits were related to certain PAE residue in dust.

Pesticide drift was reported in many international studies, but rarely studied in Taiwan. We conducted a study in a rural region of Taiwan to examine the associations between pesticides in house dust and nearby agricultural areas using geographic information system (GIS). A questionnaire regarding home characteristics and pesticide use, and indoor and outdoor dust samples were collected from 47 rural homes. Dust samples were analyzed for six pesticides, and agricultural land data for GIS analysis were retrieved from a national website. All but prallethrin were frequently detected from indoor dust samples (>50%), and the maximum concentrations were all below 1000 ng/g. Detection frequencies and concentrations of pesticides in outdoor dust samples were even lower than that in indoor dust samples. Only “work involving pesticides” in the questionnaire was significantly associated with four pesticides in house dust (p < 0.05). Carbofuran and tetramethrin in house dust were significantly correlated with rice cultivation area at certain buffer distances (ρ > 0.33, p < 0.05), and chlorpyrifos was found to be associated with abandoned cultivation area, suggesting the occurrence of pesticide drift. Despite the low levels of pesticides in house dust, residents in the rural region should be cautious of pesticide drift from nearby active or abandoned farmlands.

Allergic conjunctivitis (AC) is one of the common allergic diseases, which may be influenced by environmental factors. This study was to examine the association between the first occurrence of AC, air pollution and weather changes, and demographic factors in Taiwan. A total of 100,636 eligible subjects were identified from the systematic sampling cohort database containing 1,000,000 insureds of the National Health Insurance of Taiwan from 2004 to 2013, and were matched with data from the environmental monitoring stations adjacent to their locations of clinics. The case-crossover design, using the same subjects experiencing exposures at the time of diagnosis for cases and that on the days one and two weeks before and after diagnosis for controls, was applied in the study. We found that the first occurrences of AC were significantly the most for youngsters by age (42.6%), for women by sex (56.2%), and in spring by season (29.6%) (P

Allergic conjunctivitis (AC) and rhinitis (AR) are common allergic diseases that may be environmentally related. We used a systematic sampling cohort database, which was applied in an AC study previously, to examine the association of AR with air pollution and weather changes. A case-crossover design coupled with conditional logistic analysis was implemented in the analysis, we identified 140,365 eligible AR subjects, and matched their diagnoses with environmental monitoring data. Unlike AC, the descriptive statistics indicated that AR occurred the most in adults under 50 years old by age (44.7%), and in winter by season (28.7%) (p &lt, 0.001), similar to AC, AR occurred more in women than to men. Nitrogen dioxide (NO2) was found to be positively associated with AR (p &lt, 0.001), whereas relative humidity and temperature were negatively related (p &lt, 0.001). We found that the risk of AR increased with descending NO2 levels relative to AC (OR = 0.984, p = 0.003) after adjustment for covariates. It is suggested that AR could be triggered or exacerbated by lower levels of NO2 than is AC. We recommend that AR patients pay extra attention to air pollution and mitigate their allergic problem accordingly.

Dry eye disease (DED) has become a common eye disease in recent years and appears to be influenced by environmental factors. This study aimed to examine the association between the first occurrence of DED, air pollution and weather changes in Taiwan. We used the systematic sampling cohort database containing 1,000,000 insureds of the National Health Insurance of Taiwan from 2004 to 2013, and identified a total of 25,818 eligible DED subjects. Environmental data, including those of air pollutants, temperature and relative humidity, were retrieved from the environmental monitoring stations adjacent to subjects&rsquo, locations of clinics as exposure information. We applied the case-crossover design, which used the same subjects experiencing exposures on diagnosis days as cases and those on other days as controls. The descriptive statistics showed that the first occurrences of DED were the most for the elderly by age (53.6%), women by gender (68.9%), and spring by season (25.9%). Multivariate conditional logistic regression analyses indicated that carbon monoxide (CO), nitrogen dioxide (NO2), and temperature were positively associated with DED (p &lt, 0.05), while relative humidity was negatively related (p &lt, 0.001). Because CO and NO2 together are considered a surrogate of traffic emission, which is easier to control than the uprising temperature, it is suggested that efficient management and control of traffic emission may lower the probability of DED occurrence.

We conducted a pilot study to examine the relationship between organophosphate (OP) and pyrethroid (PYR) insecticides in blood and their metabolites in urine. A total of 30 pregnant women were enrolled in the study, and blood and urine was sampled from each subject during a regular clinic visit. Two OP and nine PYR insecticides were selected for blood sample analysis, while six OP and five PYR metabolites were analyzed for urine specimens. Both types of samples were processed and analyzed on gas chromatography-mass spectrometry. For OPs in blood, chlorpyrifos had a higher mean concentration (73.33 &micro, g/L) than terbufos. For PYRs in blood, cypermethrin and imiprothrin were the most frequently detected species with the highest mean concentrations (151.25 and 141.25 &micro, g/L). The concentrations of PYRs appeared to be higher than that of OPs, and the most frequently detected PYRs were commonly used in domestic products, suggesting that the exposure could mostly originate from use of domestic insecticides. The correlation between insecticides in blood and their metabolites in urine was significantly high (r = 0.795 for OPs and 0.882 for PYRs, p &lt, 0.001), indicating routine exposure at a steady state. Residents should be cautious with domestic use of insecticide products to lower their exposure.

We estimated the daily intakes of chlorpyrifos and cypermethrin via ingestion of indoor dust and outdoor soils using the Stochastic Human Exposure and Dose Simulation Model on a probabilistic approach for Taiwan’s homes. Variable information for the daily intake estimation, such as concentration, ingestion rate, body weight, was adopted from previous studies. Monte Carlo simulation was performed with 1,000,000 iterations to simulate a single daily intake, which was shown in terms of percentage of the Acceptable Daily Intake (ADI) of either insecticide. The daily intakes were minimal with a 99% probability; at 99.9th percentile, however, the total intakes leaped to 13.1% and 20.0% of the respective ADIs of chlorpyrifos and cypermethrin. The sensitivity analysis indicated that concentration was the most determinant variable. Compared to the data of daily intakes via dietary ingestion of vegetables derived from a previous study, the estimated intakes by this study were considerable at the highest percentile, which referred to insecticide residues few days after insecticide application. Consequently, the non-dietary ingestion exposure to either insecticide was negligible in most cases; nevertheless, for those with indoor insecticide applications, the daily intakes for young children could be of concern. Frequently home cleaning is recommended to reduce the exposure.

Consequently, the non-dietary ingestion exposure to either insecticide is commonly low for young children in Taiwan&rsquo, s homes, unless high contamination (e.g., indoor insecticide application) occurs in the environment. Care has to be taken to avoid high contamination indoors.

We conducted a pilot study to examine pesticides in dust of homes in a rural county of Taiwan. A total of 56 homes of pregnant women were included in the study. Indoor and outdoor dust was collected by a vacuum sampler and a dustpan/brush set, respectively. Nine pesticides were selected for analysis on gas chromatography-mass spectrometry with the detection limits being 0.088 ng/g or lower. The most detected pesticides were cypermethrin and chlorpyrifos, which appeared in 82.7 and 78.8% of indoor samples and 48.2 and 39.3% of outdoor samples, respectively. The detection of pesticides from indoor and outdoor dust, however, was not consistent, indicating different sources of pesticides. In addition to those two most detected, permethrin, prallethrin, and tetramethrin, which were common ingredients of insecticide products for indoor use, were also frequently found in indoor dust, suggesting that indoor use of such pesticide products may have been a major source. Fewer pesticides were found in outdoor dust, but the outdoor detection of chlorpyrifos was significantly associated with farms present inside the circles with radii of 50 and 100 m surrounding the homes (P = 0.021, 0.016). It is suggested that pesticide drift from agricultural areas to residential environments may have occurred. No seasonal effect on distribution of pesticides in dust was found, indicating that pesticides could be routinely used in Taiwan regardless of season. Compared with other international studies, this study shows relatively high levels of chlorpyrifos but low levels of pyrethroids (i.e., cypermethrin), reflecting a different pattern of pesticide use in Taiwan. Further studies need to be warranted for a better understanding of exposure to pesticides and the associated health effects.

A quick and cost-effective method to analyze leafy vegetable nitrate on spectrometry was developed and compared with a standard method using high performance liquid chromatography (HPLC). This method was designed to use ion-exchange solid phase extraction (SPE) cartridges in reducing interference from organic matrices to meet the criterion of an existing method for analyzing nitrate in wastewater. Nine vegetables (bok coy, cabbage, celery, Chinese cabbage, Chinese kale, lettuce, mustard green, pak choi, and water spinach) were selected for the method testing with three replicates being conducted for each vegetable. The nitrate contents ranged from 800 to 4,300 μg/g, with bok coy, celery, and pak choi being the highest. Data derived from spectrometry and HPLC were close to each other with most relative errors being within ±10% and were highly correlated with an R square value of 0.969. Stability testing and spike analysis resulted in a mean coefficient of variation lower than 6% and a mean recovery rate of 83.7%, suggesting reliability of the method. In addition, both the cost and time consumption for using this method were lower than the standard method using HPLC or ion chromatography, making this spectrometric method a good alternative for analysis of leafy vegetable nitrate.

Mutual enhancement of dermal absorption of N,N-diethyl-m-toluamide (DEET) and oxybenzone (OBZ) has been reported recently with DEET and OBZ being active ingredients of insect repellent and sunscreen, respectively. To assess the reported enhancing effect directly, we used human urinary metabolites as biomarkers; besides, we also sought to determine the best way for concurrent use of these two products without extra absorption of either. Four dermal application methods were used: DEET only (S1), OBZ only (S2), DEET on top of OBZ (S3), and OBZ on top of DEET (S4). Among the study methods, there was a significant difference (p = 0.013), which was attributed to the difference between S1 and S4, suggesting that applying OBZ over DEET on the skin lead to significantly higher absorption of DEET. Using both products in reverse order, (S3) did not result in extra DEET absorption significantly. As for OBZ permeation, no significant difference was observed among the methods. In summary, the enhancement of DEET absorption is confirmed for OBZ being applied over DEET on the skin; should concurrent use of both be necessary, applying sunscreen (OBZ) first and then insect repellent (DEET) with a 15-min interval is recommended.

Airborne Staphylococcus aureus causes a significant proportion of nosocomial infections. The purpose of this study was to combine real-time quantitative polymerase chain reaction with the DNA-binding agent propidium monoazide (PMA-qPCR) to assess exposures to airborne S. aureus. In this work, we generated a S. aureus aerosol and used our assay to detect viable, airborne S. aureus in a study chamber. The biological collection efficiencies of three samplers (the AGI-30 impinger, BioSampler, and Nuclepore filter sampler) were evaluated using the S. aureus aerosols. The effects of storage in collection fluid on S. aureus sampled by the AGI-30 impinger and BioSampler were evaluated. Furthermore, air samples from an intensive care unit (ICU) and a gymnasium (GYM) were subsequently used to test the performance of a BioSampler combined with our PMA-qPCR technique. Acinetobacter baumannii represents a significant cause of nosocomial infections. Therefore, we combined real-time quantitative polymerase chain reaction (PCR) with the propidium monoazide (PMA-qPCR) to assess the feasibility of detecting viable, airborne A. baumannii. The biological collection efficiencies of three samplers for collecting airborne A. baumannii were evaluated by PMA-qPCR in a chamber study. After sampling, the effects of storage in collection fluid on A. baumannii were evaluated. The results showed that the culturable ratio of A. baumannii measured using the culture method was significantly correlated with the viable ratio measured using PMA-qPCR, but was not significantly correlated with the qPCR results. It was indicated that the AGI-30 impinger and the BioSampler were much more effective than the Nuclepore filter sampler for collecting airborne A. baumannii. The storage temperature was critical for aerosol samples, as the loss of viable A. baumannii was minimized when the PMA-bound DNA was stored at -20°C or if the collected cells were stored at 4°C and subsequently processed by PMA-qPCR within 1 month. The PMA-qPCR method was also to distinguish between colistin-sensitive and colistin-resistant A. baumannii, and no colistin-sensitive A. baumannii was detected by PMA-qPCR upon treatment of the BioSampler collection medium with 2 μg/ml colistin for 5 min.

Borneol is a monoterpene that is widely used in traditional Chinese medicine. There are two different products sold in Taipei's traditional Chinese medicine market, natural and chemically synthesized borneol. Chemically synthesized borneol contains four stereoisomers, (+)-isoborneol, (−)-isoborneol, (−)-borneol, and (+)-borneol. The ratio of these four isomers in chemically synthesized and natural borneol products was determined by gas chromatography mass spectrometry. A huge variation between these products is highlighted in this survey. The results suggest that the Food and Drug Administrations in Asian countries should establish a regulatory standard regarding the ratio of the four different borneol isomers in both natural and chemically synthesized borneol.

We have developed a rapid, sensitive, and reliable method for simultaneous determination of the urinary metabolites of common insecticides in a single analytical run using gas chromatography&ndash, mass spectrometry (GCMS). Thirteen metabolites, one originating from carbamate, six from organophosphates, and seven from pyrethroids, were selected for method validation. Samples at different concentrations (0.5&ndash, 15 &micro, g/L) were prepared by mixing working solutions containing the analytes with blank urine. After acid hydrolysis for 45 min at 90 &deg, C, samples were processed with liquid&ndash, liquid extraction and derivatization by N-tert-butyldimethylsilyl-N-methyltrifluoroacetamide (MTBSTFA) before analysis on GCMS. The limits of detection for all thirteen analytes were below 0.1 &micro, g/L. The recovery rates, evaluated at two concentrations (1, 10 &micro, g/L), were found to be 90.48%, on average. The precision of multiple analyses at three different concentrations (0.5, 5, 15 &micro, g/L) within one day or between 10 days was evaluated, and the resultant relative standard deviations were 8.1% or under. We also applied this method to analyze genuine urine samples collected from 30 human subjects, and successfully detected all the metabolites, with detection frequencies more than 50% for pyrethroid metabolites. In summary, this method is not only as good as others in performance, but is advantageous in terms of cost effectiveness and multiplicity of analytes.

Vancomycin-sensitive and vancomycin-resistant Enterococcus (VSE and VRE) species have become a significant health problem. CHROMagar medium, which permits direct, color-based identification of target pathogens, could potentially be used to rapidly monitor airborne VSE and VRE. In this study, the efficiency of CHROMagar VRE medium without vancomycin supplementation (CVSE) for collecting airborne vancomycin-sensitive Enterococcus faecalis was evaluated in a chamber study. Subsequently, the performance of bioaerosol samplers combined with CVSE and CHROMagar VRE (CVRE) was evaluated in a hospital environment, a wastewater treatment plant, and a pig-rearing facility. Our results demonstrated that an Andersen impactor was much more effective than a Nuclepore filter for collecting airborne E. faecalis at relative humidity levels of 30% and 55%. “Jinchuang ointment” is a traditional Chinese herbal medicine complex for treatment of incised wounds. For more than ten years, it has been used at China Medical University Hospital (Taichung, Taiwan) for the treatment of diabetic foot infections and decubitus ulcers. Three different cases are presented in this study. “Jinchuang” ointment is a mixture of natural product complexes from nine different components, making it difficult to analyze its exact chemical compositions. To further characterize the herbal ingredients used in this study, the contents of reference standards present in a subset of the ointment ingredients (dragon’s blood, catechu, frankincense, and myrrh) were determined by HPLC. Twoin vitrocell based assay platforms, wound healing and tube formation, were used to examine the biological activity of this medicine. Our results show that this herbal medicine possesses strong activities including stimulation of angiogenesis, cell proliferation, and cell migration, which provide the scientific basis for its clinically observed curative effects on nonhealing diabetic wounds.

Aim The purpose was to examine the effects of non-commercial music on quality of sleep and relaxation indices, including heart rate, mean arterial pressure, and respiratory rate in patients in intensive care units. Background There is currently insufficient scientific knowledge for the effects of music on quality of sleep in critically ill patients. Design A randomized controlled trial. Methods Between January–December 2010, 28 patients aged 39–78 years were randomly assigned to music and control groups. Participants in the music groups listened to non-commercial music for 45 minutes at nocturnal sleep time. In the control group, participants slept with no music. Participants were assessed using vital signs monitors, standardized questionnaire, and polysomnography. Polysomnography sleep was recorded for the first 2 hours of the nocturnal sleep. General estimating equation was applied to analyse data. Findings Participants in the music group had shorter stage N2 sleep and longer stage N3 sleep in the first 2 hours of the nocturnal sleep and improved self-reported sleep quality, compared with those in the control group. The music group patients also had significantly lower heart rates than those in the control group. The intensive care units patients experienced fragmented sleep with a high frequency of awakenings and severe reduction in random eye movement sleep during the first 2 hours of the nocturnal sleep. Conclusion The findings provided evidence for nurses to use soothing music as a research-based nursing intervention for intensive care unit patients' sleep improvement.

Dry steam cleaning, which has gained recent attention as an effective method to reduce house dust mite (HDM) allergen concentration and loading in carpets, was evaluated in this study for its efficacy in lowering levels of polycyclic aromatic hydrocarbons (PAHs) as well as HDM allergens. Fifty urban homes with wail-to-wall carpets, mostly low-income and with known lead contamination, were studied in 2003 and 2004. Two carpet-cleaning interventions were compared: Repeated HEPA (High Efficiency Particulate Air filtered) vacuuming alone and repeated HEPA vacuuming supplemented with dry steam cleaning. Vacuum samples were collected to measure carpet loading of dust and contaminants immediately before and after cleaning. Paired comparisons were conducted to evaluate the effectiveness of the cleaning protocols in reducing the levels of PAHs and HDM allergens in carpets. The results indicated that both cleaning methods substantially reduced the loading of PAHs and HDM allergens as well as dust in carpets (p < 0.0001). The reductions in loading of dust (64.4%), PAHs (69.1%), and HDM allergens (85.5%), by dry steam cleaning plus repetitive HEPA vacuuming were larger than the reductions by regular HEPA vacuuming alone: dust (55.5%), PAHs (58.6%), and HDM allergens (80.8%), although the difference was statistically significant only for dust and PAHs. We conclude that intensive HEPA vacuum cleaning substantially reduced the loading of PAHs and HDM allergens in carpets in these urban homes and that dry steam cleaning added modestly to cleaning effectiveness.

We conducted a cleaning trial in 40 northern New Jersey homes where home renovation and remodeling (RR) activities were undertaken. Two cleaning protocols were used in the study: a specific method recommended by the US Department of Housing and Urban Development (HUD), in the 1995 "Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing," using a high-efficiency particulate air (HEPA)-filtered vacuum cleaner and a tri-sodium phosphate solution (TSP); and an alternative method using a household vacuum cleaner and a household detergent. Eligible homes were built before the 1970s with potential lead-based paint and had recent RR activities without thorough cleaning. The two cleaning protocols were randomly assigned to the participants' homes and followed the HUD-recommended three-step procedure: vacuuming, wet washing, and repeat vacuuming. Wipe sampling was conducted on floor surfaces or windowsills before and after cleaning to evaluate the efficacy. All floor and windowsill data indicated that both methods (TSP/HEPA and non-TSP/non-HEPA) were effective in reducing lead loading on the surfaces (P0.001). When cleaning was applied to surfaces with initial lead loading above the clearance standards, the reductions were even greater, above 95% for either cleaning method. The mixed-effect model analysis showed no significant difference between the two methods. Baseline lead loading was found to be associated with lead loading reduction significantly on floors (P0.001) and marginally on windowsills (P = 0.077). Such relations were different between the two cleaning methods significantly on floors (P0.001) and marginally on windowsills (P = 0.066), with the TSP/HEPA method being favored for higher baseline levels and the non-TSP/non-HEPA method for lower baseline levels. For the 10 homes with lead abatement, almost all post-cleaning lead loadings were below the standards using either cleaning method. Based on our results, we recommend that contractors or homeowners can use a household vacuum cleaner and a household detergent to clean lead-contaminated environments after RR activities when HUD-recommended equipment is not available.

Five methods of sampling lead-contaminated dust on carpets were evaluated and compared in 33 New Jersey homes of children with elevated blood lead levels. The five sampling methods were (1) wipe, (2) adhesive label, (3) C18 sheet, (4) vacuum, and (5) hand rinse. Samples were collected side by side on the same carpets within the homes. Among the five methods the wipe and vacuum methods showed high percentages of detectable samples, good reproducibility, and significant correlations with other methods. C18 sheets and adhesive labels collected the least quantity of lead dust, with high percentages of undetectable samples. Because of the limited ability of sampling lead on carpets and the relatively high cost for laboratory analysis, C18 sheets or adhesive labels are not considered feasible sampling techniques. The hand rinse method also was not feasible for carpet sampling, because it was difficult to conduct in the field and laboratory, and it was subject to inconsistency and cross contamination. Wipes, which collected lead dust from carpet surfaces, were believed to be the most appropriate method for measuring lead from carpets accessible to children. However, because of the low pickup from carpets, wipes may not be an appropriate measuring tool to assess the levels of total lead contamination in carpets. The authors recommend using surface wipe sampling to measure accessible lead from carpets for exposure assessment, and vacuum sampling to obtain the information on total lead accumulation.

High-efficiency particulate air (HEPA) filtered vacuum cleaners are recommended by the U.S. Department of Housing and Urban Development for cleaning lead-contaminated house dust. We performed a randomized field study to determine whether a conventional (non-HEPA) vacuum cleaner could achieve cleaning results comparable with those of a HEPA vacuum cleaner. We compared the lead loading reductions of these two vacuum cleaners in a total of 127 New Jersey homes of lead-exposed children. We used wet towelettes and a vacuum sampler to collect lead dust from carpets and upholstery before and after vacuum cleaning. The vacuum sampling data showed that the HEPA and non-HEPA vacuum cleaners resulted in 54.7% (p = 0.006) and 36.4% (p = 0.020) reductions in lead loading, respectively, when used on soiled carpets, although the overall difference in lead loading reduction between the two vacuum cleaners was not statistically significant (p = 0.293). The wipe sampling data did not show any significant lead loading reduction for either of the vacuum cleaners, suggesting that both vacuum cleaners fail to clean the surfaces of carpet effectively, considering that wipe sampling media simulate surface contact. On upholstery, the wipe sampling data showed a significant reduction in lead loading for the non-HEPA vacuum cleaner (22.2%, p = 0.047). Even with the significant reduction, the postcleaning lead loadings on upholstery were similar to those on carpets. The similar lead loading results for carpets and upholstery indicate that soiled upholstery may be as important a source of childhood lead exposure as carpets.

Urinary metabolites of DEET of 17 children (5-7 years of age) and 9 adults (23-25 years of age) were examined in the study described in this article. Urine samples were collected from each subject within eight hours after a single dermal application of 10 mL 12% DEET-containing insect repellent. Two metabolites, m-diethylaminocarbonyl benzoic acid (R3N0) and N-ethyl-m-toluamide (RON1), with unchanged DEET, were identified in the urine. The major metabolite was R3NO, which was 78.2% and 46.1% of the total DEET metabolites from children and adults, respectively, indicating that the pathway of ring methyl oxidation predominated. The recovered DEET metabolites were observed significantly more from children (1,116 pg) than from adults (446.2 pg) (p.001). The difference in dermal absorption, albeit primarily attributed to DEET loading, was found to be related to height by regression analysis. The inverse association between height and dermal absorption of DEET suggests that shorter individuals (i.e., children) are subjected to dermal uptake of DEET. To avoid unnecessary exposure, parents need to be cautious when applying DEET-containing insect repellent on children.

We conducted a study to examine seasonal changes in residential dust lead content and its relationship to blood lead in preschool children. We collected blood and dust samples (floors, windowsills, and carpets) to assess lead exposure. The geometric mean blood lead concentrations are 10.77 and 7.66 microg/dL for the defined hot and cold periods, respectively (p < 0.05). Lead loading (milligrams per square meter) is the measure derived from floor and windowsill wipe samples that is most correlated with blood lead concentration, whereas lead concentration (micrograms per gram) is the best variable derived from carpet vacuum samples. The variation of dust lead levels for these three dust variables (floor lead loading, windowsill lead loading, and carpet lead concentration) are consistent with the variation of blood lead levels, showing the highest levels in the hottest months of the year, June, July, and August. The regression analysis, including the three representative dust variables in the equations to predict blood lead concentration, suggests that the seasonality of blood lead levels in children is related to the seasonal distributions of dust lead in the home. In addition, the outdoor activity patterns indicate that children are likely to contact high leaded street dust or soil during longer outdoor play periods in summer. Consequently, our results show that children appear to receive the highest dust lead exposure indoors and outdoors during the summer, when they have the highest blood lead levels. There is no significant difference in the bond lengths of Mn-S bonds among three species in their crystal structures [2.325(2) A in 1; 2.358(4) in 2; 2.380(2) in 4], but the better donating ability of thiolate in complex 4 appears on the lower frequencies of its carbonyl stretching absorptions.

Treatment of fac-[ (P,SPh-P,P’)Mn(CO),Br] and fac-[ (PS2PhP,S)Mn(CO),Br] [P2SPh: Z = PPh,; PS2Ph: Z = SPh in CH3C(CH,PPh,)(CH,SPh)(CH,Z)] with silver hexafluorophosphate yields the q3-tripodal manganese complexes [ (P2SPhP,P’,S)Mn(C0)3]PF6 (7) and [(PS,Ph-P,S,S’)Mn(CO),]PF, (9), respectively, whereas the reaction of CH3C(CH2PPh2),(CH,SH) with Mn(CO)5Br gives directly [CH3C(Ph2PCH,),(CH2S)-P,P’,S]Mn(CO)3 (11). Both ionic complexes 7 and 9 undergo stereospecifically ligand substitution upon treatment with Br-, I-, NF, SCN- and CN- to give syn-fac[(P,SPh-RP‘)Mn(CO),X] and syn-fac-[(PS,Ph-P,S)Mn(CO),X] (X = Br, I, SCN, N3, CN) respectively, but the neutral complex 11 fails to undergo such a substitution reaction with these ions. However, reaction of I- with the complex [(PSMe)-P,P’,S]Mn(CO),]BF, (20) (P2SMe = CH3C(CH2PPh2)2(CH2SCH3)), which is obtained from the S-alkylation of 11 with (CH3),0BF4, affords syn-fac-[(P2SMe)P,P’]Mn(CO)J (22s) exclusively. The occurrence of allergic rhinitis (AR) may be significantly influenced by air pollution. This study examined the association between the first occurrence of AR in preschool children and the pre-incident levels of air pollutants in Taiwan. We identified 9960 eligible subjects from a systematic sampling cohort database containing 400,000 insureds of the National Health Insurance from 2007 to 2011 and matched them with the environmental monitoring data from 2006 to 2011 according to the locations of their clinics. Pre-incident levels were determined using the average concentrations of air pollutants one or two weeks prior to the AR diagnoses. Logistic regression analyses were performed to determine any significant relationships between AR and specific air pollutants. The first AR incidence for Taiwanese preschool children, which increased with age, was 10.9% on average; boys appeared to have a higher percentage (14.2%) than girls (8.27%). Among the air pollutants, carbon monoxide (CO) and nitrogen oxides (NOX) were significantly related to AR after adjusting for age and gender (p < 0.05). Because both pollutants are considered to be traffic emissions, this study suggests that traffic emissions in Taiwan need to be controlled to lower the prevalence of children’s AR.

Risk assessments for toxicants in environmental media via oral exposure often rely on measurements of total concentration in a collected sample. However, the human digestive system cannot dissolute all of a toxicant present in the binding matrix, and cannot absorb it with nearly 100% efficiency. In vitro bioaccessibility has been developed as a method to estimate oral bioavailability of a toxicant using a physiologically-based extraction procedure. Bioaccessibility measurements are more physiologically relevant than strong acid leaching measurements of concentration. A method for measuring bioaccessible lead in house dust was derived from the bioaccessibility method currently used for heavy metals in contaminated soils. The efficacy of the Community Reinvestment Act (CRA) in assisting homeowners or landlords in urban areas of New Jersey to finance lead abatement was evaluated in the study reported here, as was the effectiveness of the abatement. The study involved working with homeowners, banks, local health departments, and communities to facilitate financing for lead abatement, and collecting environmental-dust specimens from some of the participants' homes before and after abatement. Of the 113 interested subjects, 59 (52 percent) completed abatement of their homes. Of the 58 who applied for CRA loans, 21 received approvals and had the work completed. Thirty-nine of the homeowners found alternate means of financing abatement. The environmental data showed a significant decrease on the windowsills in the 4aated homes, but not on the floors. In conclusion, one-on-one counseling of homeowners about funding was associated with more than 50 percent success in completing abatement, The CRA loans' accounted for about one-third of the abated units. The lead abatement was effective in removing lead paint from the windows, but not in reducing dust lead levels on the floors.

The collapse of the World Trade Center (WTC) on September 11, 2001, generated large amounts of dust and smoke that settled in the surrounding indoor and outdoor environments in southern Manhattan. Sixteen dust samples were collected from undisturbed locations inside two uncleaned buildings that were adjacent to Ground Zero. These samples were analyzed for morphology, metals, and organic compounds, and the results were compared with the previously reported outdoor WTC dust/smoke results. We also analyzed seven additional dust samples provided by residents in the local neighborhoods. The morphologic analyses showed that the indoor WTC dust/smoke samples were similar to the outdoor WTC dust/smoke samples in composition and characteristics but with more than 50% mass in the53-microm size fraction. This was in contrast to the outdoor samples that contained50% of mass above53 microm. Elemental analyses also showed the similarities, but at lower concentrations. Organic compounds present in the outdoor samples were also detected in the indoor samples. Conversely, the resident-provided convenience dust samples were different from either the WTC indoor or outdoor samples in composition and pH, indicating that they were not WTC-affected locations. In summary, the indoor dust/smoke was similar in concentration to the outdoor dust/smoke but had a greater percentage of mass53 microm in diameter.

The explosion and collapse of the World Trade Center (WTC) was a catastrophic event that produced an aerosol impacting many residents, workers, and commuters after September 11, 2001. In all, 12 bulk samples of the settled dust were collected at indoor locations surrounding the epicenter of the disaster, including one sample from a residence that had been cleansed and was once again occupied. Additionally, one sample was collected from just outside a fifth story window on the sill. These samples were analyzed for many components, including inorganic and organic constituents as well as morphology of the various particles. The results of the analyses for persistent organic pollutants on dusts that settled at indoor locations are described herein, including polycyclic aromatic hydrocarbons, polychlorinated biphenyls, and select organo-chlorine pesticides. The Sigma(86)-PCB concentrations, comprising less than one part per million by mass of the bulk in the two samples analyzed, indicated that PCBs were of limited significance in the dust that settled at indoor locations across lower Manhattan. Likewise, organo-chlorine pesticides, Hexachlorobenzene, Heptachlor, 4,4'-DDE, 2,4'-DDT, 4,4'-DDT and Mirex were found at even lower concentrations in the bulk samples. Conversely, Sigma(37)-PAHs comprised up to 0.04% (0.005-0.036%) by mass of the bulk indoor dust in the 11 WTC impacted bulk indoor samples. Analysis of one sample of indoor dusts collected from a vacuum cleaner of a rehabilitated home shows markedly lower PAH concentrations (0.0005 mass%), as well as differing relative contributions for individual compounds. In addition to similar concentrations, comparison of PAH concentration patterns (i.e. chemical fingerprints) shows that dusts that settled indoors are chemically similar to previously measured WTC dusts found at outdoor locations and that these PAH analyses may be used in identifying dusts of WTC origin at indoor locations, along with ascertaining further needs for cleaning.

Five methods of sampling lead-contaminated dust on carpets were evaluated and compared in 33 New Jersey homes of children with elevated blood lead levels. The five sampling methods were (1) wipe, (2) adhesive label, (3) C18 sheet, (4) vacuum, and (5) hand rinse. Samples were collected side by side on the same carpets within the homes. Among the five methods the wipe and vacuum methods showed high percentages of detectable samples, good reproducibility, and significant correlations with other methods. C18 sheets and adhesive labels collected the least quantity of lead dust, with high percentages of undetectable samples. Because of the limited ability of sampling lead on carpets and the relatively high cost for laboratory analysis, C18 sheets or adhesive labels are not considered feasible sampling techniques. The hand rinse method also was not feasible for carpet sampling, because it was difficult to conduct in the field and laboratory, and it was subject to inconsistency and cross contamination. High efficiency particulate air filter (HEPA) vacuums, which collect particles > 0.3 micro m, and trisodium phosphate (TSP), a detergent claimed to selectively remove lead, have been included in the HUD Guidelines for the Evaluation and Control of Lead Based Paint Hazards in Housing without systematic validation of their effectiveness. At the time the study was initiated, both HEPA vacuums and TSP were relatively expensive, they were not readily found in urban retail centers, and there were environmental concerns about the use and disposal of high-phosphate detergents. A randomized, controlled trial was conducted in urban high-risk homes in northern New Jersey to determine whether a more readily available and less expensive low-phosphate, non-TSP detergent and non-HEPA vacuum could perform as well as TSP and a HEPA vacuum in a cleaning protocol. Homes were randomized to one of three cleaning methods: TSP/HEPA vacuum, TSP/non-HEPA vacuum, or non-TSP/non-HEPA vacuum. Change in log-transformed lead loading was used in mixed models to compare the efficacy of the three cleaning techniques separately for uncarpeted floors, window sills, and window troughs. After we adjusted for baseline lead loading, the non-HEPA vacuum produced larger reductions on hard floors [19%; 95% confidence interval (CI), 3-38%], but the HEPA vacuum produced larger reductions on window sills (22%; 95% CI, 11-32%) and larger reductions on window troughs (16%; 95% CI, -4 to 33%). The non-TSP produced larger reductions on window troughs (21%; 95% CI, -2 to 50%), but TSP produced larger reductions on hard floors (5%; 95% CI, -12 to 19%) and window sills (8%; 95% CI, -5 to 20%). TSP/HEPA produced larger reductions on window sills (28%; 95% CI, 18-37%) and larger reductions on window troughs (2%; 95% CI, -24 to 23%), whereas the non-TSP/non-HEPA method produced larger reductions on hard floors (13%; 95% CI, -5 to 34%). Because neither vacuum nor detergent produced consistent results across surface types, the use of low-phosphate detergents and non-HEPA vacuums in a temporary control measure is supported.

Comparability of dust lead measurements has been a difficult problem due to different sampling and analysis techniques. This paper compares two dust sampling techniques, the U.S. Department of Housing and Urban Development (HUD) dust wipe method and the Lioy, Wainman, Weisel (LWW) sampler. The HUD method specifies using a moist towelette to pick up as much dust as possible in a specified area and estimates total lead loading. The LWW sampler collects the dust on preweighed wetted filter media, and provides greater standardization of the sampling path and pressure applied. LWW samples were analyzed using inductively coupled plasma mass spectronomy (no samples below minimum detection limit), while HUD samples were analyzed using flame atomic absorption (32% of samples below minimum detection limit). A bootstrapping technique was used in the analysis to contend with those HUD samples below the minimum detection limit. Mixed model equations were generated to predict HUD values from LWW results, and to examine the effects of sampling location, time, and method. The results indicate that the two samplers performed similarly under field conditions, although the LWW sampler produced consistently lower lead loading estimates. LWW values that predicted HUD lead clearance values of 100 micrograms/ft2 for floors and 500 micrograms/ft2 for window sills were 72 micrograms/ft2 and 275 micrograms/ft2, respectively. To examine internal reproducibility, duplicate samples were taken using both the HUD and LWW methods. Correlation results within paired samples indicated a statistically significantly higher (p < 0.001) internal reproducibility for lead loading, for the LWW sampler (r = 0.87), than for the HUD method (r = 0.71). Some of the differences appeared to be related to the analytical methods.
