Abstract

Spreading depolarizations (SD) are waves of abrupt, near-complete breakdown of neuronal transmembrane ion gradients, are the largest possible pathophysiologic disruption of viable cerebral gray matter, and are a crucial mechanism of lesion development. Spreading depolarizations are increasingly recorded during multimodal neuromonitoring in neuro-critical care as a causal biomarker providing a diagnostic summary measure of metabolic failure and excitotoxic injury. Focal ischemia causes spreading depolarization within minutes. Further spreading depolarizations arise for hours to days due to energy supply-demand mismatch in viable tissue. Spreading depolarizations exacerbate neuronal injury through prolonged ionic breakdown and spreading depolarization-related hypoperfusion (spreading ischemia). Local duration of the depolarization indicates local tissue energy status and risk of injury. Regional electrocorticographic monitoring affords even remote detection of injury because spreading depolarizations propagate widely from ischemic or metabolically stressed zones; characteristic patterns, including temporal clusters of spreading depolarizations and persistent depression of spontaneous cortical activity, can be recognized and quantified. Here, we describe the experimental basis for interpreting these patterns and illustrate their translation to human disease. We further provide consensus recommendations for electrocorticographic methods to record, classify, and score spreading depolarizations and associated spreading depressions. These methods offer distinct advantages over other neuromonitoring modalities and allow for future refinement through less invasive and more automated approaches.

Abstract

Background Erythropoietin (Epo) is an ubiquitous cytokine. It has endocrine, paracrin and autocrin functions. It improves antiapoptotic mechanisms on all tissues subjected to hypoxic stress. In many animal models of brain trauma but also in other brain injury models, and some human clinical studies, recombinant Human Epo (Epo-rh) has proven neuro-protective properties. The main goal of this work was to improve and incorporate Epo-rh in the pharmacological arsenal of treatment in brain and spinal cord traumatic injuries in human. Material and methods In a first part, to explore the reasons of failure of inclusion in a Spinal Cord Injury (SCI) study to test the thrombo-embolic tolerance and efficacy of Epo-rh, we studied the epidemiology of SCI using the road crash Rhône registry in the period 1997-2006. Then we compared the epidemiological trends of the SCI incidence, associated trauma, mortality and fatality rates in two periods of 6 years: 1995-2001 and 2003-2008. In a second part, due to the 20-fold higher incidence of traumatic brian injury (TBI) in comparison to SCI, we characterized the effects of a moderate (1.6-1.8 atm) lateral fluid percussion injury (LFPI) in order to understand and characterize the pharmacological, antiinflammatory and neuroprotective mechanisms of action of Epo-rh in such a brain injury. Results : The incidence of SCI was 5/100 000 inhabitants in the Rhône area during the 10 year period. However, 40% of the patients died before or immediatly after arriving in hospital facility. This means that, in the 10 years studied, only 57 patients with SCI without associated lesions could be included in the study. Although incidence of severe road crash trauma decreased by 33%, no change in the SCI incidence was observed in the later 6-year period. SCI victims were significantly more aged, polytraumatized, and more frequently using motorcycle, despite mortality increased, non significantly, by 17%. In the experimental LFPI model, we noted histological and genic modifications without significant changes in behavior and MRI-DTI data obtained in the dorsum thalamus and the corpus callosum. If Epo-rh decreased IL-1! and MIP-2 protein concentrations in the cortex, it had no effect either on the 15- to 50-fold increase of the transcrits of IL-1! , TNF" and IL-6 or on the Epo and EpoR genic responses whatever the brain structures analyzed ipsilaterally to the trauma. Bioavailability of Epo-rh in the traumatic cortex was significant 4 hours after intraperitoneal or intravenous injection using at least 3000 UI/kg, was increased in the trauma side and remained elevated for 24h while it disappeared in the non trauma side. Conclusion : it should be difficult to include SCI patients without associated body lesions in Lyon due to the change of the incidence of the main target population. Experimental LFPI of 1.6-1.8 atm corresponds to a mild brain trauma because neither behavioral changes nor MRI- DTI modifications were observed. Such a LFPI induced i) neuronal loss and astroglial reaction in the ipsilateral side, ii) an increase in the inflammatory protein concentrations and associated transcrits, and iii) an increase in Epo and EpoR transcrits. Epo-rh crossed easier the Blood Brain Barrier in the itraumatized side, decreased the IL-1! and MIP-2 protein concentrations in the cortex and had no effect on associated mRNA levels and on Epo system reactivity.; Situation du sujet : L'érythropoiétine (Epo) est une protéine ubiquitaire dans les tissus de l'organisme. Elle est dotée d'une fonction endocrine, autocrine et paracrine. Elle favorise les activités anti-apoptotiques des tissus soumis à un stress hypoxique. Dans de nombreux modèles animaux de traumatisme ou d'agression du système nerveux central et quelques études cliniques, l'Epo recombinante humaine (Epo-rh) a révélé des propriétés neuroprotectrices. L'objectif principal de ce travail est d'améliorer les connaissances sur l'Epo afin de favoriser l'inclusion de l'Epo-rh dans l'arsenal thérapeutique chez l'homme après traumatisme du système nerveux central. Matériel et méthodes : Deux axes de travail ont été explorés : dans un première partie, pour expliquer l'échec de la mise en œuvre d'une étude de la tolérance et d'efficacité biologioque de l'Epo-rh dans le Traumatisme Médullaire Déficitaire (TMD) chez l'homme, nous avons étudié l'épidémiologie des TMD sur la période 1997-2006, à partir du Registre des accidents du Rhône. Ensuite nous avons étudié l'évolution de cette incidence entre 2 périodes de 6 ans : 1995-2001 et 2003-2008. Parallèlement, compte tenu d'une incidence de traumatisme crânien (TC) 20 fois plus élevée que celle du TMD chez l'homme, nous avons entrepris de caractériser les effets d'un TC expérimental par percussion fluide latérale (LFPI) chez le rat afin de mieux comprendre la pharmacologie et les mécanismes d'action moléculaires, en particulier anti-inflammatoires et neuroprotecteurs, de l'Epo-rh. Résultats : Nous avons montré que l'incidence du TMD était d'environ 5/100 000 habitants. Néanmoins, près de 40% des TMD décédaient dans les premières heures suivant l'accident, souvent avant l'arrivée des secours. D'autre part, cette incidence correspondait à une inclusion potentielle de 57 TMD isolés sur 7 ans. L'évolution de l'incidence des TMD ne changeait pas au cours de la période récente alors que l'accidentologie grave s'effondrait de 33%. Dans la période récente, les victimes de TMD étaient plus souvent polytraumatisées, motocyclistes et plus âgées avec une augmentation non significative de la mortalité de 17%. Parallèlement, un LFPI de 1,6-1,8 atm induit des modifications histologiques et en expression génique mais est sans effet sur le comportement ou sur les paramètres mesurés en IRM-DTI au niveau du corps calleux ou du thalamus dorsal. Si l'Epo-rh diminue les concentrations d' IL-1! et de MIP-2 au niveau du cortex traumatisé, elle est sans effet sur l'augmentation des transcrits de l'IL-1! , TNF" et IL-6. Elle est également sans action sur l'expression des transcrits de l'Epo et de son récepteur (EpoR). L'Epo-rh a une biodisponibilité à 4h équivalente entre les voies intrapéritonéale et intraveineuse, et se concentre plus facilement dans le cerveau du côté traumatisé ou elle reste détectable pendant 24h. Conclusion : Il sera difficile d'inclure des patients dans un contexte de TMD sans autre lésion associée compte tenu de l'évolution de l'incidence du TMD dans la principale population cible à Lyon. Un LFPI de 1,6-1,8 atm n'induit pas de troubles neurocomportementaux ni de signaux IRM détectables en DTI. Ce LFPI génère une perte neuronale au niveau du cortex pariéto-temporal et de l'hippocampe correspondant à des signes évoquant un TC mineur. L'Epo-rh passe la barrière hémo-méningée plus facilement du côté traumatisé, réduit les concentrations d'IL-1! et de MIP-2 mais est sans action d'une part sur les transcrits de ces molécules inflammatoires et d'autre part sur les transcrits de l'Epo et de l'Epo-R, pour ce niveau de traumatisme.
