A 74-year-old female patient was admitted to hospital following a road accident with pains in the chest, abdomen, waist, back, nose, left wrist and lower limbs. After 1 week, the patient presented with gastrointestinal bleeding, and thus was treated with protein pump inhibitors (PPIs), including lansoprazole, esomeprazole and omeprazole enteric-coated tablets, in order to inhibit acid secretion and attenuate bleeding. However, the patient developed skin rashes on the chest and right lower limb and foot 28 days following treatment initiation. The skin rashes spread and ulcerated after 3 days, and were associated with tracheal mucosal injury and hemoptysis. Subsequently, treatment of the patient with PPIs was terminated, after which the tracheal hemoptysis and skin rashes markedly improved. In addition, no new skin rashes appeared following termination of the PPI treatment. In the present case, long-term treatment of an elderly patient with PPIs may have induced exfoliative dermatitis, due to hepatic ischemia, hypoxia and acute renal failure, which may have decreased the metabolism of PPIs, resulting in the accumulation of PPI metabolites.

In the current digital era, telecommunications industry technology is growing rapidly, impacting the demands for innovation in the telecommunications operator business. One of them is the change in the size of the Subscriber Identity Module (SIM) card model, which is getting smaller, and the use of embedded SIM (eSIM) technology on smartphones. This study aims to identify operational risk factors from the change in SIM card technology to eSIM. The research method used is the Systematic Literature Review (SLR) method. This study documents and reviews scientific journal papers from scientific databases published from 2015 to 2022 on risk management in the information technology field, following this research's objectives. The results obtained from this study showed that there were 43 journals studied, of which four had the theme of technology-embedded subscriber identity module (eSIM), and 13 discussed risk operations technology

4. A Case Report of One Patient with Allergic Purpura Induced by Voriconazole

Voriconazole is a second-generation triazole antifungal drug, which exhibits characteristics of wide antibacterial spectru, high biological utilization and passing through plasma brain barrier. The most common adverse reactions of voriconazole include neurological dysfunction, visual disturbance, abnormal liver function and renal dysfunction, here, this is the report to show that voriconazole can induce allergic purpura. The present report mainly describes one patient with rare adverse reactions, namely mixed type allergic purpura induced by high concentration of plasma drug caused by slow metabolism of voriconazole. Aafter using voriconazole, the patient was diagnosed as mixed type allergic purpura, methylprednisolone sodium succinate was given for treatment. After treatment, abdominal pain and abdominal distension of the patient was significantly alleviated, and the color of rash faded. Edemas of both lower extremities were significantly improved, and the color of stool turned into yellow. The measured concentration of plasma drug of voriconazole was 15 μg/ml on admission of the patient. Genetic testing result demonstrated that the patient belonged to heterozygous mutation of CYP2C19*1*2, and the metabolism of voriconazole of the patient was relatively slow. The report prompted the clinicians that there were significant metabolic differences of voriconazole in different populations. Therefore, monitoring of drug concentration should be conducted timely in the clinical application to reduce the risk of medication and achieve individualized administration.

Total Sulfur Analyzer (TS Analyzer) is one of the machines in the laboratory to test the total sulfur content contained in coal samples. Technically, specifications and use are regulated by international standards. In 2020, the downtime of these tools was very high, causing the productivity of surveyors to decrease. Low productivity will have an impact on the company's losses. The productivity of this equipment and machines is crucial to maintaining the company's success in its business processes. This study aims to measure the effectiveness of the operational performance of the TS Analyzer machine in the laboratory, analyze the causative factors and make improvements. This research uses the integration method of Failure Mode And Effect Analysis (FMEA) and Overall Equipment Effectiveness (OEE). Based on the analysis of the causes of the problem, the low OEE value is caused by the high breakdown loss. Based on the FMEA analysis, it was found that the operator was wrong in operating the machine with RPN 343, electrical instability with RPN 343, unstable temperature with RPN 343 and improper thermocouple installation. The results showed that the OEE value of the TS Analyzer machine has increased after the improvement.

PT Nusantara Totalindo Logistics (PT NTL) is a company engaged in the logistics transportation sector with fast business growth but is currently in the business introduction and business turbulence phase. In facing the business introduction phase and business turbulence, it is necessary to develop business resilience and adaptation strategies. This study aims to identify PT NTL's business potential and advantages, build business resilience for PT NTL by establishing PT NTL's vision and mission as a corporate statement to support the completeness of organizational documents and designing business resilience formulations for PT NTL through creating work programs, organizational structure, business model canvas, business processes, and business risk mitigation to build a business adaptation strategy. The data for this research were sourced from in-depth interviews and risk management research questionnaires. Based on the results of the study that has been done, a time-framed business adaptation-bearing formulation has been formed through six processes, starting from vision and mission formation to risk management that has adaptation and resilience properties. Business in the face of business turbulence. The management implications of a business resilience strategy are the establishment of a vision and mission according to the resilience and business adaptation strategy, a comprehensive organizational structure, work programs for the next five years according to business turbulence and disruption, a kanas business model that supports work programs up to logistics risk management. 7. Spare Parts Supplier Selection Design: A Case Study of A Railway Company

Spare parts support is essential for rolling stock maintenance management. The current supplier selection model determines the selected supplier based on evaluating one aspect of the criteria (product aspect). The selection of suppliers with poor performance occurred between 2018-2020 related to the delivery of goods that exceeded the deadline and goods that did not meet specifications. The first objective of this research is to analyze and determine the relevant priority criteria for selecting suppliers of rolling stock spare parts for railway companies. The second objective is to determine the rolling stock spare parts supplier by using the evaluation criteria determined in the previous process. The method used in this research is the integration of the Fuzzy Delphi Method (FDM), Analytical Hierarchy Process (AHP), and Technique for Others Preference by Similarity to Ideal Solutions (TOPSIS). FDM is used to select important criteria for the selection of suppliers of rolling stock parts. AHP is used to assist in choosing various criteria through evaluation in determining the criteria's weight. TOPSIS is used to assess supplier ratings. A total of 13 criteria from 19 alternative criteria have been selected for railway companies, especially in selecting rolling stock spare parts suppliers. Furthermore, the selection becomes the basis for bidding. Finally, Supplier A is the supplier with the highest relative closeness value (0.591), followed by Supplier B (0.545), and the lowest is Supplier C (0.282).

Permasalahan pokok yang menjadi fokus penelitian ini adalah rendahnya Hasil Belajar Materi Apresiasi puisi siswa Kelas VIII-F SMP Negeri 1 Rancaekek. Dari studi awal yang dilakukan di sekolah, ditemukan beberapa indikator yang menunjukkan rendahnya Hasil Belajar Materi Apresiasi siswa di sekolah. Secara umum tujuan penelitian ini adalah mendapatkan kajian tentang upaya meningkatkan Hasil Belajar Materi Apresiasi puisi siswa kelas VIII-F SMP Negeri 1 Rancaekek menggunakan Model Think Talk Write (TTW). Subjek penelitian ini dilaksanakan dikelas VIII-F SMP N 1 Rancaekek dengan siswa berjumlah 24 siswa dalam penelitian ini. Penelitian ini yang melalui 2 siklus perbaikan. Berdasarkan hasil penelitain Pada siklus I nilai siswa yang berhasil mendapatkan nilai KKM yaitu sebanyak 32%. Sedangkan pada perbaikan pembelajaran di siklus II siswa yang berhasil mencapai KKM 90%. Dengan kesimpulan bahwa hasil pembelajaran Bahasa Indonesia materi apresiasi puisi dapat meningkat dengan menggunakan metode think Talk Write.

Enhancing the process capability is a must in quality improvement of process manufacturing in the industry. Usually, Statistical Process Control (SPC) is applied in measure the activity of quality improvement. In this case, the statistical process control used to measure the process capability of molding machine regarding of force result of inner contact rubber button in plastic industry. The forces of inner contact rubber button which produce by molding machine already become the critical point as a judgment of malfunctions or not. Meanwhile, the forces problem still found in molding machine that affected to the inner contact rubber button as defect product. Furthermore, as shown of SPC, the process capability of molding machine was 0.63. This mean the process capability is still need the improvement. As the result of quality improvement was made by applied of Poka-Yoke, the process capability of molding machine was improved to 1.65.

Introducción: como consecuencia de la acción de la caries dental sobre los dientes de la cavidad oral, la pulpa se convierte en un albergue de bacterias y sus subproductos, lo que origina la enfermedad pulpar. La respuesta a estas infecciones puede dar lugar a enfermedades agudas o crónicas que constituye la mayoría de las urgencias estomatológicas en Cuba. Objetivo: caracterizar el comportamiento de las enfermedades pulpares; así como el proceder terapéutico frente a las mismas en la escuela "Arides Estévez". Material y Métodos: se realizó un estudio descriptivo transversal con 162 estudiantes quienes acudieron a la consulta estomatológica de dicha escuela y fueron diagnosticados con algún tipo de enfermedad pulpar, en el período comprendido entre octubre de 2008 a diciembre de 2009. Resultados: el 69,1% de los afectados por enfermedades pulpares fueron del sexo masculino y el grupo etario más afectado fue el de 19-25 años con 65,4%. La pulpitis transitoria afectó a 50% de los casos, mientras que la caries dental estuvo presente en 46,9% de los mismos. El 60,4% de los pacientes recibieron tratamiento conservador. Conclusiones: la enfermedad pulpar más frecuente en la población estudiada es la pulpitis reversible y de esta la transitoria; las edades más afectadas están entre los 19 y 25 años, y los varones son los más aquejados. La caries dental, seguida de las obturaciones deficientes, es la causa de la mayor cantidad de lesiones pulpares. El tratamiento conservador es la terapia más usada en estos casos. Introducción: la caries dental afecta la integridad de los dientes, pudiendo provocar lesiones endodónticas irreversibles que le confieren un carácter incurable a los mismos. El periodonto está anatómicamente interrelacionado con la pulpa dental. Las lesiones resultantes de la interacción entre la enfermedad pulpar y periodontal son conocidas como lesiones endoperiodontales. Objetivo: evaluar el comportamiento de las lesiones endoperiodontales en el área de salud de la Facultad de Estomatología de enero de 1998 a diciembre de 2002. Material y Métodos: para ello se realizó un estudio descriptivo, retrospectivo y transversal con las historias clínicas de pacientes atendidos con este diagnóstico en la Facultad de Estomatología de La Habana, en el período comprendido entre enero de 1998 y diciembre del 2002. La muestra quedó constituida por 124 historias clínicas pertenecientes a pacientes con dientes afectados por estas lesiones. Resultados: el 15,6% de los pacientes presentaron lesión endoperiodontal, 60,5% tuvieron la causa endodóntica como origen de las lesiones. El 42 % de los dientes afectados tuvo que ser extraído debido a causas periodontales. Conclusiones: dentro de las causas que favorecieron el origen del objeto de estudio, el endodóntico ocupó el lugar cimero. Las terapéuticas más realizadas fueron la endodóntica y combinadas ocupando el primero y segundo lugares respectivamente. El porcentaje de lesiones endoperiodontales fue bajo, aunque la mortalidad dentaria por este tipo de afecciones fue alta y estuvo fundamentalmente relacionada con la causa periodontal, y con el grupo de 35 a 59 años de edad. 13. Prediction consistency and clinical presentations of breast cancer molecular subtypes for Han Chinese population

Abstract Background Breast cancer is a heterogeneous disease in terms of transcriptional aberrations; moreover, microarray gene expression profiles had defined 5 molecular subtypes based on certain intrinsic genes. This study aimed to evaluate the prediction consistency of breast cancer molecular subtypes from 3 distinct intrinsic gene sets (Sørlie 500, Hu 306 and PAM50) as well as clinical presentations of each molecualr subtype in Han Chinese population. Methods In all, 169 breast cancer samples (44 from Taiwan and 125 from China) of Han Chinese population were gathered, and the gene expression features corresponding to 3 distinct intrinsic gene sets (Sørlie 500, Hu 306 and PAM50) were retrieved for molecular subtype prediction. Results For Sørlie 500 and Hu 306 intrinsic gene set, mean-centring of genes and distance-weighted discrimination (DWD) remarkably reduced the number of unclassified cases. Regarding pairwise agreement, the highest predictive consistency was found between Hu 306 and PAM50. In all, 150 and 126 samples were assigned into identical subtypes by both Hu 306 and PAM50 genes, under mean-centring and DWD. Luminal B tended to show a higher nuclear grade and have more HER2 over-expression status than luminal A did. No basal-like breast tumours were ER positive, and most HER2-enriched breast tumours showed HER2 over-expression, whereas, only two-thirds of ER negativity/HER2 over-expression tumros were predicted as HER2-enriched molecular subtype. For 44 Taiwanese breast cancers with survival data, a better prognosis of luminal A than luminal B subtype in ER-postive breast cancers and a better prognosis of basal-like than HER2-enriched subtype in ER-negative breast cancers was observed. Conclusions We suggest that the intrinsic signature Hu 306 or PAM50 be used for breast cancers in the Han Chinese population during molecular subtyping. For the prognostic value and decision making based on intrinsic subtypes, further prospective study with longer survival data is needed.

Background: Precise preoperative risk classification of endometrial cancer is crucial for treatment decisions. Existing clinical markers often fail to accurately predict lymph node metastasis and recurrence risk. Loss of vimentin expression has emerged as a potential marker for predicting recurrence in low-risk endometrial cancer patients. We assessed whether vimentin expression in preoperative biopsies predicts poor prognosis and lymph node metastasis in a large multicentre cohort., Methods: Vimentin expression was evaluated using immunohistochemistry in 1483 patients diagnosed with endometrial cancer across 14 hospitals in Europe. Expression levels of vimentin were analyzed in conjunction with clinical characteristics for predicting disease-specific survival and lymph node metastases., Results: Vimentin loss was significantly associated with aggressive disease and poor survival. Adjusted for clinicopathological variables, vimentin remained independently prognostic with a hazard ratio (HR) of 1.68 (95% CI 1.16-2.42, P = 0.006). Vimentin expression remained independently prognostic in endometrioid endometrial cancer- and FIGO staged 1 patient. Interestingly, vimentin loss independently predicted lymph node metastases, with an HR of 1.83 (95% CI 1.13-2.95, P = 0.014)., Conclusions: Loss of vimentin in preoperative biopsies serves as an independent predictor of poor prognosis and lymph node metastases. Background: Identification of aggressive low-stage endometrial cancers is challenging. So far, studies have failed to pinpoint robust features or biomarkers associated with risk of recurrence for these patients., Methods: Imaging mass cytometry was used to examine single-cell expression of 23 proteins in 36 primary FIGO IB endometrial cancers, of which 17 recurred. Single-cell information was extracted for each tumor and unsupervised clustering was used to identify cellular phenotypes. Distinct phenotypes and cellular neighborhoods were compared in relation to recurrence. Cellular differences were validated in a separate gene expression dataset and the TCGA EC dataset. Vimentin protein expression was evaluated by IHC in pre-operative samples from 518 patients to validate its robustness as a prognostic marker., Findings: The abundance of epithelial, immune or stromal cell types did not associate with recurrence. Clustering of patients based on tumor single cell marker expression revealed distinct patient clusters associated with outcome. A cell population neighboring CD8+ T cells, defined by vimentin, ER, and PR expressing epithelial cells, was more prevalent in non-recurrent tumors. Importantly, lower epithelial vimentin expression and lower gene expression of VIM associated with worse recurrence-free survival. Loss and low expression of vimentin was validated by IHC as a robust marker for recurrence in FIGO I stage disease and predicted poor prognosis also when including all patients and in endometrioid patients only., Interpretation: This study reveals distinct characteristics in low-stage tumors and points to vimentin as a clinically relevant marker that may aid in identifying a here to unidentified subgroup of high-risk patients., Funding: A full list of funding that contributed to this study can be found in the Acknowledgements section., Competing Interests: Declaration of interests The authors declare no competing interests., (Copyright © 2023 The Author(s). 19. Mismatch repair markers in preoperative and operative endometrial cancer samples; expression concordance and prognostic value.

- Abstract
Background: The endometrial cancer mismatch repair (MMR) deficient subgroup is defined by loss of MSH6, MSH2, PMS2 or MLH1. We compare MMR status in paired preoperative and operative samples and investigate the prognostic impact of differential MMR protein expression levels., Methods: Tumour lesions from 1058 endometrial cancer patients were immunohistochemically stained for MSH6, MSH2, PMS2 and MLH1. MMR protein expression was evaluated as loss or intact to determine MMR status, or by staining index to evaluate the prognostic potential of differential expression. Gene expression data from a local (n = 235) and the TCGA (n = 524) endometrial cancer cohorts was used for validation., Results: We identified a substantial agreement in MMR status between paired curettage and hysterectomy samples. Individual high expression of all four MMR markers associated with non-endometrioid subtype, and high MSH6 or MSH2 strongly associated with several aggressive disease characteristics including high tumour grade and FIGO stage, and for MSH6, with lymph node metastasis. In multivariate Cox analysis, MSH6 remained an independent prognostic marker, also within the endometrioid low-grade subgroup (P < 0.001)., Conclusion: We demonstrate that in addition to determine MMR status, MMR protein expression levels, particularly MSH6, may add prognostic information in endometrial cancer., (© 2022. - Abstract
Obesity is a disease characterized by chronic low-grade systemic inflammation and has been causally linked to the development of 13 cancer types. Several studies have been undertaken to determine whether tumors evolving in obese environments adapt differential interactions with immune cells and whether this can be connected to disease outcome. Most of these studies have been limited to single-cell lines and tumor models and analysis of limited immune cell populations. Given the multicellular complexity of the immune system and its dysregulation in obesity, we applied high-dimensional suspension mass cytometry to investigate how obesity affects tumor immunity. We used a 36-marker immune-focused mass cytometry panel to interrogate the immune landscape of orthotopic syngeneic mouse models of pancreatic and breast cancer. Unanchored batch correction was implemented to enable simultaneous analysis of tumor cohorts to uncover the immunotypes of each cancer model and reveal remarkably model-specific immune regulation. In the E0771 breast cancer model, we demonstrate an important link to obesity with an increase in two T-cell-suppressive cell types and a decrease in CD8 T cells., Competing Interests: Competing interests The authors declare no competing or financial interests., (© 2021.
