5. Susceptibility to lenacapavir, fostemsavir and broadly neutralizing antibodies in French primary HIV‐1 infected patients in 2020–2023.

Surveillance studies of Transmitted Drug Resistance (TDR) are crucial in tracking the evolution of HIV epidemiology. Our aim was to investigate TDR to nucleoside reverse transcriptase inhibitors (NRTIs), non‐nucleoside reverse transcriptase inhibitors (NNRTIs), protease inhibitors (PIs), integrase inhibitors (INIs), as well as to new drugs: lenacapavir, fostemsavir. Predictive sensitivity was evaluated for maraviroc and broadly neutralizing antibodies (bNAbs) (zinlirvimab and teropavimab). Between 2020 and 2023, 85 people with HIV (PWH) were diagnosed with primary HIV‐1 infection (PHI). Pol and env sequences were analyzed and TDR was characterized according to the French ANRS algorithm. The genotypic‐based prediction of bNAbs sensitivity was based on HIV env amino acid signatures I108, I201, F353 for teropavimab and N325, N332, H330 for zinlirvimab. TDR to NRTIs, NNRTIs, PIs and INIs was evidenced in 8.2%, 12.9%, 4.7%, and 5.9% strains, respectively. Ten viruses were CXCR4/dual mix. All viruses were susceptible to lenacapavir (100%) and 52% harbored resistance to fostemsavir. The genotypic profile was associated with a predictive positive value (PPV) > 83% of susceptibility to both teropavimab and zinlirvimab for 23 viruses (31%), while 22 (29%) had a PPV between 62% and 75%, suggesting reduced susceptibility to both bNAbs as soon as primary infection. The surveillance of TDR evidenced at the time of PHI is important with regard to new strategies for HIV patients with virological failure and global implementation of PrEP using NRTI, INI such as recently approved injectable cabotegravir, and future long‐acting drugs such as lenacapavir and bNAbs. Background: Awareness and uptake of human immunodeficiency virus (HIV) pre-exposure prophylaxis (PrEP) remains low among Black/African American cisgender women, partly due to low self-reported PrEP knowledge and comfort among primary care providers. Ensuring providers are trained on PrEP is crucial, as increased PrEP knowledge is associated with higher rates of PrEP prescription. Objective: We aimed to develop a PrEP training for providers to improve their self-efficacy in discussing and prescribing PrEP for Black women, with the ultimate goal of increasing PrEP awareness and utilization among Black women. Design: In this qualitative study, we conducted focus groups with medical providers at three federally qualified health centers in the Southern and Midwestern United States to identify themes informing the development of a provider PrEP training. Methods: Providers were asked for input on content/design of PrEP training. Transcripts underwent rapid qualitative analysis using the Stanford Lightning Report Method. Themes were identified and presented under the domains of the Consolidated Framework for Implementation Research. Results: Ten providers completed four focus groups. Themes included the individual characteristics of providers (low comfort initiating PrEP discussions, particularly among White providers) and the outer setting of client attitudes (perceptions of potential provider bias/racism, varying levels of concern about HIV acquisition). Opportunities were identified to maximize the benefit of training design (e.g., developing case scenarios to enhance providers' cultural competency with Black women and PrEP knowledge). Conclusion: This comprehensive PrEP training features both didactic material and interactive role-plays to equip providers with the clinical knowledge for prescribing PrEP while building their competency discussing PrEP with Black women. This training is particularly important for providers who have racial or gender discordance with Black women and express lower comfort discussing PrEP with these clients. Provider training could lead to minimizing racial- and gender-based inequities in PrEP use. Introduction: Daily pre-exposure prophylaxis (PrEP) with tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) is associated with a small but statistically significant decrease in estimated glomerular filtration rate (eGFR). We assessed the renal safety of on-demand PrEP with TDF/FTC in HIV-1 uninfected men. Methods: We used data from the randomized double-blind placebo-controlled ANRS-IPERGAY trial and its open-label extension conducted between February 2012 and June 2016 among HIV-uninfected MSM starting on-demand PrEP. Using linear mixed model, we evaluated the mean eGFR decline from baseline over time and determined risks factors associated with eGFR decline during the study. Results: During the blind phase, with a median follow-up of 9.4 months, the mean decline slope of eGFR from baseline was -0.88 and -1.53 mL/min/1.73 [m.sup.2] per year in the placebo (n = 201) and the TDF/FTC group (n = 198) respectively, with a slope difference of 0.65 mL/min/1.73 [m.sup.2] per year (p = 0.27). Including both phases, 389 participants started on-demand TDF/FTC with a median follow-up of 19.2 months and a mean decline of eGFR from baseline of -1.14 mL/min/1.73 [m.sup.2] per year (p < 0.001). The slope of eGFR reduction was not significantly different in participants with baseline eGFR [less than or equal to] 90 mL/min/1.73 [m.sup.2] (p = 0.44), age >40 years (p = 0.24) or hypertension (p = 0.21). There was a dose-response relationship between recent tenofovir exposure and lower eGFR when considering the number of pills taken in the two months prior the visit (eGFR difference of -0.88 mL/min/1.73 [m.sup.2] between >15 pills/month vs. [less than or equal to]15 pills/month, p < 0.01) or plasma tenofovir concentrations at the visit (eGFR difference compared to [less than or equal to]2 ng/mL: >2 to [less than or equal to]10ng/mL: -0.98 mL/min/1.73 [m.sup.2], >10 to [less than or equal to]40ng/mL: -1.28 mL/min/1.73 [m.sup.2], >40 ng/mL: -1.82 mL/min/1.73 [m.sup.2], p < 0.001). Three participants discontinued TDF/FTC for eGFR < 60 mL/min/1.73 [m.sup.2] during the OLE phase. No case of Fanconi syndrome was reported. Objectives: We conducted a survey to evaluate HIV pre‐exposure prophylaxis (PrEP) practices in a European clinical research network on HIV, hepatitis, and global infectious diseases (NEAT ID). Methods: An online survey comprising 22 questions was sent via a secure electronic tool to the investigating physician of each of the 342 NEAT ID study centres across 15 European countries in November 2020. Results: In total, 50 sites from 12 countries responded (15% response rate). Most sites were in Western Europe, two were in Poland, and one was in Hungary. Of the responding sites, 45 provided PrEP services for a total of 27 416 PrEP users, with 1361 new PrEP initiators each month. These centres supplied PrEP for men who have sex with men (100%), people who inject drugs (84%), sex workers (84%), women (62%), and transgender women (31%). PrEP persistence after 1 year was >90%, 75%–90%, and 40%–75% in 17, 24, and 4 centres, respectively. In total, 32/45 (71%) centres reported strong community‐based organization commitment at their site, and 15/45 (33%) centres developed task‐shifting processes to deliver PrEP through nurses (11/15), pharmacists (5/15), and key‐population peers (2/15). The biggest barriers to implementation of PrEP were low awareness of and knowledge about PrEP (47%), unwillingness to disclose sexual identity or at‐risk behaviour (36%), and lack of administrative support (29%). Of the 45 centres, 32 (71%) have already been involved in PrEP research and 43 (96%) were interested in participating in such studies. Conclusions: The few NEAT ID centres that responded to the survey showed disparities in PrEP deployment and practices despite a common interest in participating in research in this field. 15. Incidence and Management of Complex Kidney Situations Among On-Demand and Daily HIV Pre-Exposure Prophylaxis Users.

- Abstract
Background: We evaluated complex pre-exposure prophylaxis (PrEP) situations linked to kidney issues in a cohort of on-demand and daily PrEP users. Setting: We conducted a single-center retrospective cohort study in France including all PrEP users who received a tenofovir disoproxil (TD)--emtricitabine (FTC) prescription between January 1, 2012 and December 31, 2019 with at least 1 creatinine measurement available before and after PrEP initiation. Methods: A complex kidney situation (CKS) was defined as an estimated glomerular filtration rate (eGFR) <60 mL/minute/1.73m² on 2 consecutive measurements. We estimated the incidence of this event, described case management, and identified associated factors using a Cox model. Results: Three thousand one hundred and fourteen individuals were included in this study. Almost all were men (99%) with a median age of 35 years, 25% had an eGFR <90 mL/minute/1.73m² at baseline, and 65% used on-demand PrEP. Nine users (0.29%) had a CKS at baseline; 8/9 initiated on-demand PrEP without renal function worsening after a median (interquartile range [IQR]) follow-up time of 14 months (7-31). Thirteen cases of CKS occurred during the follow-up for a 0.25 per 100 person-years incidence (95% confidence interval [CI]: [0.14; 0.45]). On-demand PrEP was used in 7/13 participants with no further episode of confirmed eGFR <60 mL/ minute/1.73m² after a 17-month median follow-up (IQR 4-18). CKS was associated with an age ≥50 years (hazard ratio [HR] 13, 95% CI: [4-39]) or with a baseline eGFR <90 mL/minute/1.73m² (HR 34, 95% CI: [4-261]). 9/22 CKS were linked to high-protein intake for weight training. Conclusions: CKS were rare in our cohort. On-demand PrEP did not result in subsequent renal function worsening in these few situations. [ABSTRACT FROM AUTHOR]

Black cisgender women (hereafter referred to as "women") have disproportionately high rates of HIV infection yet low rates of pre-exposure prophylaxis (PrEP) utilization. Barriers to PrEP uptake exist at the system, provider, and individual/client level. To learn how existing training and advertising can be adapted to address race- and sex-based gaps within PrEP service delivery, we conducted focus groups with providers and Black women. Participants were recruited at three health care organizations in the Midwest and South, screened for eligibility, and consented verbally. Focus groups occurred from August 2022 to February 2023. Women were asked about their knowledge and thoughts on PrEP. Providers were asked about factors influencing their decision-making about PrEP. A codebook was developed based on the Consolidated Framework for Implementation Research. Transcripts were coded using the Stanford Lightning Report Method. We completed four focus groups with 10 providers and 9 focus groups with 25 women. Three major themes emerged: (1) low comfort level and limited cultural sensitivity/competency among providers discussing HIV risk and PrEP with Black women, (2) women's concerns about PrEP's side effects and safety during pregnancy, and (3) lack of Black women representation in PrEP advertisement/educational materials. In addition, women in the South reported general medical mistrust and specific misconceptions about PrEP. PrEP trainings for providers need detailed information about the safety of PrEP for women and should include role-playing to enhance cultural competency. Likewise, PrEP advertisements/materials should incorporate information regarding side effects and images/experiences of Black women to increase PrEP awareness and uptake among this population. International audience; Objectives - Persistent post-acute coronavirus disease 2019 (COVID-19) symptoms (PACSs) have been reported up to 6 months after hospital discharge. Herein we assessed the symptoms that persisted 12 months (M12) after admission for COVID-19 in the longitudinal prospective national French coronavirus disease cohort. Methods - Hospitalized patients with a confirmed virological diagnosis of COVID-19 were enrolled. Follow-up was planned until M12 after admission. Associations between persistence of ≥3 PACSs at M12 and clinical characteristics at admission were assessed through logistic regression according to gender. Results - We focused on participants enrolled between 24 January 2020 and 15 July 2020, to allow M12 follow-up. The M12 data were available for 737 participants. Median age was 61 years, 475 (64%) were men and 242/647 (37%) were admitted to intensive care units during the acute phase. At M12, 27% (194/710) of the participants had ≥3 persistent PACS, mostly fatigue, dyspnoea and joint pain. Among those who had a professional occupation before the acute phase, 91 out of 339 (27%) were still on sick leave at M12. Number studies have shown that the prophylactic use of tenofovir disoproxil (TDF) can significantly reduce the number of new HIV and hepatitis B infections. However, the risk of TDF-associated severe renal adverse events may hamper the implementation of these new prevention strategies. This thesis aims to evaluate the impact on renal function of TDF in the setting of HIV pre-exposure prophylaxis (PrEP) and prevention of mother-to-child transmission (PMTCT) of HBV.The cohort study conducted among 3000 PrEP users followed at the Saint Louis Hospital in Paris demonstrate that the risk of acute renal failure was low and mainly concerned users with an age > 50 years and/or with an estimated glomerular filtration rate < 90mL/min at the time of prophylaxis initiation. All these users were able to continue using PrEP with a low risk of relapse. The analysis conducted in the randomized placebo-controlled Ipergay clinical trial, as well as in the ANRS-Prévenir cohort study in the Ile de France area, highlights that the use of "on-demand" HIV PrEP in men who have sex with men improved the renal safety of the prophylaxis by reducing individuals' exposure to TDF. This dosing regimen can reduce the risk of renal toxicity among individuals with risk factors of kidney injury. Data from these studies allowed the development of recommendations for renal function monitoring among PrEP users. These recommendations emphasize the need to individualize renal function monitoring and support the shift towards differentiated PrEP service delivery and models involving non-medical healthcare providers. This approach will be crucial to significantly increasing the number of PrEP users and should facilitate adherence and persistence in treatment.Concerning the PMTCT of HBV, we specifically assessed the risk of TDF-associated tubulopathy, given the harmful effect of tubular dysfunction on maternal and infant health. Using early kidney injury markers on urine samples collected in a randomized, double-blind, placebo-controlled clinical trial conducted in Thailand (iTAP), we show that TDF is not associated with a significant increase of tubulopathy and that TD can be used safely during the third trimester of pregnancy and breastfeeding. This confirms that the PMTCT of hepatitis B among women at high risk of transmission can be implemented by non-specialists in many settings.Overall, this thesis highlights that the risk of TDF-associated renal toxicity in the setting of HIV and hepatitis B prevention does not stand as a barrier to the implementation of these new preventive strategies in low-income countries that cannot perform regular monitoring of renal function.; De nombreuses études montrent que l'utilisation du ténofovir disoproxil (TDF) à titre prophylactique permet de réduire très significativement le nombre de nouvelles contaminations par le VIH et par le virus de l'hépatite B. Cependant, le risque de survenue d'évènements indésirables rénaux sévères peut ralentir la mise en œuvre de ces nouvelles stratégies de prévention. Ce travail de thèse s'est donné pour objectif d'évaluer l'impact rénal du TDF utilisé dans le cadre de la prophylaxie pré-exposition (PrEP) du VIH et dans la prévention de la transmission mère-enfant (PTME) du VHB afin d'élaborer des recommandations de suivi et de surveillance de la fonction rénale sous cette prophylaxie.L'étude de cohorte menée sur les 3000 usagers de PrEP suivis à l'hôpital Saint-Louis à Paris démontre que le risque d'insuffisance rénale aigue sous TDF est faible et se concentre spécifiquement dans le groupe des utilisateurs âgés de plus de 50 ans et/ou ayant un débit de filtration glomérulaire estimé (DFGe) inférieur à 90 mL/min au moment d'initier la prophylaxie. Ces utilisateurs ont pu continuer à utiliser la PrEP, suite ou non à un arrêt temporaire, avec un faible taux de récidive de l'insuffisance rénale. L'analyse des données issues de l'essai clinique contrôlé randomisé contre placebo Ipergay et de la cohorte d'usagers de PrEP ANRS-Prévenir en Ile de France montrent que la PrEP « à la demande » chez les hommes ayant des rapports sexuels avec des hommes améliore la tolérance rénale de cette prophylaxie. Ce type de schéma intermittent peut permettre de réduire le risque de toxicité rénale chez les utilisateurs ayant des facteurs de risque d'insuffisance rénale. Les données de ces trois études combinées aux données bibliographiques permettent de suggérer des recommandations simples de suivi de la fonction rénale chez les usagers PrEP. Ces recommandations soulignent la nécessité d'individualiser la surveillance de la fonction rénale. Elles permettent d'envisager des stratégies de démédicalisation visant à augmenter significativement le nombre d'usagers en élargissant leur suivi à des professionnels de santé non médicaux tout en diversifiant les lieux d'obtention de la PrEP. Dans le cadre de la PTME de l'hépatite B, nous avons évalué spécifiquement le risque de tubulopathie maternelle, compte-tenu de l'effet potentiellement très délétère qu'une telle atteinte aurait sur la santé maternelle et infantile. En utilisant des marqueurs précoces d'atteinte tubulaire sur des prélèvements d'urines recueillis dans un essai clinique contrôlé randomisé en double insu contre placebo conduit en Thaïlande (iTAP), nous montrons que l'utilisation du TDF n'est pas associée à une augmentation significative du risque de tubulopathie et que le TDF peut être utilisé sans risque durant le troisième trimestre de grossesse et l'allaitement. Cela confirme que la PTME de l'hépatite B chez les femmes infectées à haut risque de transmission peut être mise en œuvre par des non-spécialistes dans de nombreux contextes.En conclusion, le risque de toxicité rénale lié à l'utilisation du TDF dans la prévention du VIH et de l'hépatite B ne doit pas être considéré comme un obstacle à la mise en œuvre partout dans le monde de ces stratégies de prévention très prometteuses notamment dans les pays à faibles ressources où la surveillance régulière de la fonction rénale n'est pas possible.

Background: Long‐acting injectable cabotegravir (CAB‐LA) represents a new additional option for HIV prevention in people at substantial risk of HIV infection that may fill the gaps in pre‐exposure prophylaxis (PrEP) uptake, adherence, and retention in users having difficulty with oral PrEP. Data from clinical trials demonstrated that CAB‐LA was safe, highly effective, and well‐accepted for HIV prevention. However, the occurrence of breakthrough HIV infections despite timely injections, HIV seroconversion timing and patterns, risk of selection and dissemination of resistance‐associated mutations to integrase inhibitors, complexity of follow‐up, logistical considerations, and its cost effectiveness compared with oral PrEP constitute significant issues for the integration of CAB‐LA into clinical routine. Findings: These concerns need to be addressed before moving forward with large‐scale implementation programmes. Pilot and implementation projects are required in the following areas: HIV testing algorithms, patient education, clinic procedures, protocols for switching and discontinuation, efficacy and safety in populations not included in clinical trials, and demedicalization processes. The development of models to increase the uptake of, adherence to, and persistence with and after CAB‐LA injections will also be of paramount importance for success. Lessons learned from these projects will increase experience, staff expertise, and organizational and training capacities to support the roll‐out of this new agent as part of HIV prevention programmes. Conclusion: CAB‐LA has not yet achieved its full potential in HIV prevention, and strong commitment from all stakeholders is required to push CAB‐LA as a game‐changer in HIV response. Background: We assessed whether on-demand PrEP could increase the renal safety of TDF/FTC based PrEP among MSM. Methods: We used data from the randomized double-blind placebo-controlled ANRS-IPERGAY trial conducted among HIV-uninfected MSM with normal kidney function at baseline starting on-demand TDF/FTC based PrEP. Using linear mixed model, we evaluated the mean eGFR change from baseline during both the blind and the open-label extension phase of the study.Results: During the blind phase, with a median of follow-up of 9.4 months, the mean decline slope of eGFR from baseline was -0.07mL/min/1.73m² per month in the placebo group (n=201) (P=0.04) and -0.13 mL/min/1.73m² per month in the TDF/FTC group (n=198) (P40 years or hypertension. Background: Tenofovir diphosphate (TFV-DP) concentration in dried blood spots (DBSs) is a reliable pharmacokinetics biomarker of adherence to tenofovir disoproxil fumarate (TDF). We aimed to use DBSs to estimate pill intake among participants using on-demand pre-exposure prophylaxis (PrEP) and to identify predictive factors associated with higher TFV-DP concentrations.Methods: DBSs were collected at the last study visit of the open-label phase of the ANRS IPERGAY study, assessing on-demand oral TDF/emtricitabine for PrEP among MSM and transgender female participants. We quantified TFV-DP in DBSs centrally. We assessed correlation between pill count and TFV-DP concentration by Spearman correlation and explored associations between participant demographics, sexual behaviour and PrEP use during sexual intercourse (SI) with TFV-DP concentrations by univariate and multivariate logistic regression models.Results: The median age of the 245 participants included in this study was 40 years, with a median body weight of 73 kg. Median (IQR) TFV-DP concentration reached 517 (128-868) fmol/punch, corresponding to an estimated intake of 8-12 tablets per month (2-3 doses per week). Only 39% of participants had a TFV-DP concentration above 700 fmol/punch. TFV-DP concentrations were moderately correlated with pill count (r: 0.59; P < 0.001). In multivariate analysis, only systematic use of PrEP during SI and more frequent episodes of SI in the past 4 weeks were significantly associated with higher TFV-DP levels [OR (95% CI): 11.30 (3.62-35.33) and 1.46 (1.19-1.79), respectively; P < 0.001].Conclusions: Among participants using on-demand PrEP, estimated pill intake reached 8-12 tablets per month and was correlated with frequency and systematic use of PrEP for SI. - Abstract
Laboratory diagnosis of histoplasmosis is based on various methods, including microscopy, culture, antigen, and DNA detection of Histoplasma capsulatumvar. capsulatumor Histoplasma capsulatumvar. duboisii. To improve sensitivity of existing real-time quantitative PCR (qPCR) assays, we developed a new RT-qPCR assay that allows amplification of whole nucleic acids of Histoplasmaspp. validated on suspected cases. The limit of detection was 20 copies, and the specificity against 114 fungal isolates/species was restricted to Histoplasmaspp. Whole nucleic acids of 1319 prospectively collected consecutive samples from 907 patients suspected of having histoplasmosis were tested routinely between May 2015 and May 2019 in parallel with standard diagnostic procedures performed in parallel. Forty-four had proven histoplasmosis attributable to H. capsulatumvar. capsulatum(n= 40) or H. capsulatumvar. duboisii(n= 4) infections. The results of RT-qPCR were positive in 43 of 44 patients (97.7% sensitivity) in at least one specimen. Nine of 863 cases (99% specificity) were RT-qPCR positive and therefore classified as possible cases. RT-qPCR was positive in 13 of 30 (43.3%) blood samples tested in proven cases. A positive RT-qPCR result in blood was significantly associated with H. capsulatumvar. capsulatumprogressively disseminated histoplasmosis with a positive RT-qPCR result in 92.3% of the immunocompromised patients with disseminated disease. This new HistoplasmaRT-qPCR assay enabling amplification of H. capsulatumvar. capsulatumand H. capsulatumvar. duboisiiis highly sensitive and allows the diagnosis of histoplasmosis advantageously from blood and bronchoalveolar lavage fluid.

Background: We evaluated complex pre-exposure prophylaxis (PrEP) situations linked to kidney issues in a cohort of on-demand and daily PrEP users., Setting: We conducted a single-center retrospective cohort study in France including all PrEP users who received a tenofovir disoproxil (TD)-emtricitabine (FTC) prescription between January 1, 2012 and December 31, 2019 with at least 1 creatinine measurement available before and after PrEP initiation., Methods: A complex kidney situation (CKS) was defined as an estimated glomerular filtration rate (eGFR) <60 mL/minute/1.73m 2 on 2 consecutive measurements. We estimated the incidence of this event, described case management, and identified associated factors using a Cox model., Results: Three thousand one hundred and fourteen individuals were included in this study. Almost all were men (99%) with a median age of 35 years, 25% had an eGFR <90 mL/minute/1.73m 2 at baseline, and 65% used on-demand PrEP. Nine users (0.29%) had a CKS at baseline; 8/9 initiated on-demand PrEP without renal function worsening after a median (interquartile range [IQR]) follow-up time of 14 months (7-31). Thirteen cases of CKS occurred during the follow-up for a 0.25 per 100 person-years incidence (95% confidence interval [CI]: [0.14; 0.45]). On-demand PrEP was used in 7/13 participants with no further episode of confirmed eGFR <60 mL/minute/1.73m 2 after a 17-month median follow-up (IQR 4-18). 34. Atypical presentation of Mycobacterium xenopi pulmonary infection in a kidney transplant recipient: A case report and literature review.

- Abstract
Background: Mycobacterium xenopi is one of the most common pathogens responsible for non-tuberculosis mycobacteria (NTM) pulmonary diseases, which are associated with poor prognosis in immunocompromised patients., Case Presentation: We report the unusual case of a 44-year-old kidney transplant recipient with multiple pulmonary nodules revealing M. xenopi pulmonary disease with atypical presentation. A three drug-regimen containing moxifloxacin, ethambutol and azithromycin was prescribed, with careful monitoring of the immunosuppressive therapy. The outcome was favorable., Discussion and Conclusion: Although infrequent in kidney transplant recipients, NTM can cause pulmonary infection several years after transplantation. Treatment of M. xenopi infection relies on a multidrug regimen with at least 3 antimycobacterial drugs.
