Mitochondrial carrier homologs 1 (MTCH1) and 2 (MTCH2) are orphan members of the mitochondrial transporter family SLC25. Human MTCH1 is also known as presenilin 1‐associated protein, PSAP. MTCH2 is a receptor for tBid and is related to lipid metabolism. Both proteins have been recently described as protein insertases of the outer mitochondrial membrane. We have depleted Mtch in Drosophila and show here that mutant flies are unable to complete development, showing an excess of apoptosis during pupation; this observation was confirmed by RNAi in Schneider cells. These findings are contrary to what has been described in humans. We discuss the implications in view of recent reports concerning the function of these proteins.

La tuberculosis es una enfermedad infectocontagiosa granulomatosa causada por Micobacterium Tuberculosis. La tuberculosis genitourinaria es la segunda forma más frecuente de tuberculosis extrapulmonar. Sin historia manifiesta de tuberculosis, representa un dilema diagnóstico. Se presenta este caso para divulgar la importancia del pesquisaje e interrogatorio adecuado para un diagnóstico precoz de tuberculosis genitourinaria. Paciente masculino de 59 años, con padecimiento de Orquiepididimitis de larga data, rebelde a tratamiento urológico, caracterizada por síntomas urinarios repetitivos acompañados en la última etapa de dolor intenso espontáneo con aumento palpable de volumen escrotal izquierdo, interpretado como neoplasia en estudio imagenológico. que es intervenido quirúrgicamente. Es importante investigar, realizar entrevista médica precisa, estudios imagenológicos; así como la inclusión de la tuberculosis como posible diagnostico diferencial de masas escrotales en adultos mayores con enfermedad genitourinaria, por su diferentes presentaciones, seguimiento, pronóstico y tratamiento.

Objective The purpose of the study will be to evaluate the effect of pulmonary rehabilitation on the improvement of patients with post-COVID-19 musculoskeletal symptoms, as well as to quantify the impact of telemedicine that evaluates the evolution of pain, functionality, and quality of life. Methods We will carry out a case-control study in post-COVID-19 musculoskeletal symptoms patients who will undergo pulmonary rehabilitation, together with an intervention and a follow-up using programmed telemedicine sessions. Data will be collected on the improvement of functional capacity and quality of life, in addition to assessing the evolution of musculoskeletal symptomatology, as well as pain and psychological variables. The approaches of face-to-face rehabilitation and telerehabilitation will also be compared. The telemedicine sessions will improve user adherence and follow-up, and the results are expected to be disseminated to the scientific community during and after the end of the study.

Abstract Background Muscle strength and dexterity impairments are common among patients with multiple sclerosis (MS) producing limitations in activities of daily living related to the upper limb (UL). This study aimed to evaluate the effectiveness of serious games specifically developed for the MYO Armband® capture sensor in improving forearm and wrist mobility, UL muscle strength, dexterity, fatigue, functionality, quality of life, satisfaction, adverse effects and compliance. Methods A double-blinded (allocation concealment was performed by a blinded investigator and by blinding for assessors) randomised controlled trial was conducted. The sample was randomised into two groups: an experimental group that received treatment based on UL serious games designed by the research team and controlled by the MYO Armband® gesture capture sensor, along with conventional rehabilitation and a control group that received the same conventional rehabilitation for the UL. Both groups received two 60-min sessions per week over an eight-week period. Wrist range of motion (goniometry), grip muscle strength (Jamar® dynamometer), coordination and gross UL dexterity (Box and Block Test), fatigue (Fatigue Severity Scale), functionality (ABILHAND), quality of life (Multiple Sclerosis Impact Scale-29), adverse effects (Simulator Sickness Questionnaire, SSQ), perceived workload (NASA-Task load index), satisfaction (Client Satisfaction Questionnaire-8 (CSQ-8), Satisfaction with Technology Scale, System Usability Scale (SUS) and QUEST 2.0) and compliance (attendance) were assessed in both groups pre-treatment, post-treatment and during a follow-up period of 2 weeks without receiving any treatment. Results Significant differences were observed in the experimental group compared to the control group in the assessment of forearm supination (p = .004) and grip strength (p = .004). Adverse effects were minimal (SSQ: 7/100 points) and perceived workload was low (NASA-Task Load Index: 25/100 points) in the experimental group. The MYO Armband® technology proved to be useful for the participants (SUS: 80.66/100) and the satisfaction scales received high scores (QUEST 2.0: 59.4/70 points; Satisfaction with Technology: 84.36/100 points). There were significant differences between the groups in terms of attendance percentage (p = .029). Conclusions An experimental protocol using MYO Armband®-based serious games designed for UL rehabilitation showed improvements in active wrist range of motion and handgrip strength in patients with MS, with high satisfaction, minimal adverse effects and workload and excellent compliance. Introduction: Suicidal behaviour and self-harm are increasing in children and adolescents. Non-suicidal self-harm are a dysfunctional method of emotional regulation, and it must be distinguished from suicidal behaviours. Methods: Narrative review of the current situation on suicide and self-harm in Spain. Descriptive study of suicidal behaviours in paediatric emergencies. Results: Mental health consultations were analysed (March-2019 to March-2020 and March-2020 to March-2021) in a multicentric study of the SEUP (Spanish Society of Pediatric Emergencies), finding a 122 % increase of the diagnosis of ''non-accidental drug intoxication” and 56 % of “suicide/suicide attempt/suicidal ideation”. In another prospective analysis, 281 attempts were recorded, with the patient profile being: female (90.1 %), 14.8 years old, 34.9 % without previous psychiatric diagnosis; 57.7% with previous suicidal behaviour. The presence of psychiatric disorders, especially depression, and previous attempts, are the best-known risk factors for suicidal behaviour, although other factors are involved (family, personal or social). Pediatricians should be trained to deal with questions about suicide and acquire the skills to conduct an interview with a supportive and empathetic attitude. In Spain, suicide prevention plans are heterogeneous among communities, and there is not a unified national suicide prevention plan. Conclusions: Primary, hospital and mental health care resources for paediatric population should be strengthened to prevent suicidal behaviours. Specific training for school staff, and child and adolescent psychiatry training for paediatricians and nurses are crucial in the prevention of suicide in children and adolescent population. Resumen: Introducción: Las conductas suicidas y las autolesiones están aumentando en niños y adolescentes. Las autolesiones no suicidas suponen un método disfuncional de regulación emocional. Es importante aprender a distinguirlas de las conductas suicidas. Material y métodos: Revisión narrativa de la situación actual sobre suicidio y autolesiones en España. Estudio descriptivo conductas suicidas en urgencias pediátricas. Resultados: En un estudio multicéntrico de la SEUP (sociedad Española de Urgencias Pediátricas) se analizaron las consultas por salud mental (marzo-2019 a marzo-2020 y marzo-2020 a marzo-2021), encontrándose un aumento de 122% del diagnóstico “intoxicación no accidental por fármacos” y de 56% en “suicidio/ intento de suicidio/ideación autolítica”. En otro análisis prospectivo, se registraron 281 tentativas, siendo el perfil de paciente: sexo femenino (90,1%), 14,8 años, 34,9% sin diagnóstico psiquiátrico previo, 57,7% con conductas suicidas anteriormente. La presencia de trastornos psiquiátricos, en especial de depresión, y de intentos previos, son los factores de riesgo más implicados en la conducta suicida, aunque existen otros de índole diversa (familiares, personales o sociales). Los pediatras deben estar formados para atender consultas sobre suicidio, y adquirir habilidades para realizar una entrevista con actitud de apoyo y empatía. En España los planes de prevención de suicidio son heterogéneos y no existe un plan nacional de prevención del suicidio. Conclusiones: Se deben reforzar los recursos de atención primaria, hospitalaria y de salud mental de población pediátrica. Los centros escolares y la formación en psiquiatría infantil y adolescente para médicos y enfermeras resultan cruciales en la prevención del suicidio en niños y adolescentes.

Resumen: Objetivo: Mostrar el proceso de elaboración, dentro del Grupo de Trabajo de Intoxicaciones de la Sociedad Española de Urgencias de Pediatría (GTI-SEUP), de la lista de recomendaciones de «no hacer» ante un paciente pediátrico que ha contactado con una sustancia potencialmente tóxica. Método: El proceso de elaboración de la lista se realizó en tres fases. Primera: «Lluvia de ideas» abierta a todos los miembros del GTI-SEUP. Segunda: Selección de las recomendaciones, siguiendo una metodología Delphi-modificada. Se solicitó a todos los participantes que puntuasen las propuestas (del 1 = totalmente en desacuerdo al 9 = totalmente de acuerdo). Se aceptaron aquellas con una puntuación media superior a 8 (siempre que al menos 2/3 de los participantes le hubieran otorgado una puntuación ≥ 7) y se realizó una segunda consulta para las recomendaciones con una puntuación media entre 6 y 8. Tercera: Redacción y consenso del documento final. Resultado: Inicialmente se obtuvieron 11 propuestas; 32 de los 57 participantes del GTI-SEUP respondieron al cuestionario de puntuación. En la primera consulta, fueron aceptadas siete recomendaciones de «no hacer» y cuatro obtuvieron una puntuación media dudosa (entre 6 y 8). Tras la segunda consulta, la lista quedó formada por ocho recomendaciones. Dos hacen referencia al manejo general, cuatro a técnicas de descontaminación digestiva y dos a la administración de antídotos. Conclusión: La lista de acciones que no hay que hacer ante un niño que ha contactado con un posible tóxico es una herramienta consensuada, dentro del GTI-SEUP, para promover una mejora de la calidad asistencial ofrecida a estos pacientes. Dicha mejora se basa en evitar medidas innecesarias, que en ocasiones pueden resultar nocivas para el niño. Method: The preparation process of the list was carried out in three phases. First: «Brainstorming» that was open to all members of the GTI-SEUP. Second: Recommendations were selected by following modified-Delphi methodology. All participants were asked to rate the proposals (from 1 = strongly disagree to 9 = strongly agree). Those with an average score greater than 8 were accepted (provided that at least two-thirds of the participants had given them a score ≥ 7), and a second consultation was made for the recommendations with an average score between 6 and 8. Third: Writing and creating a consensus of the final document was done. Result: A total of 11 proposals were initially obtained. Thirty-two of the 57 GTI-SEUP participants completed the scoring questionnaire. In the first consultation, seven «not to do» recommendations were accepted, and four obtained a doubtful average score (between 6 and 8). After the second consultation, the list was made up of eight recommendations. Two refer to general management, four to gastrointestinal decontamination techniques, and two to the administration of antidotes. Conclusion: The list of actions that should not be taken with a child that has been exposed to a possible poison is a consensus tool, within the GTI-SEUP, to promote improvement in the quality of care offered to these patients. This improvement is based on avoiding unnecessary measures, which can sometimes be harmful to the child.

Resumen: Introducción: Los pacientes con enfermedad renal crónica (ERC) tiene alto riesgo de morbimortalidad cardiovascular. Las alteraciones estructurales cardiacas subclínicas tienen valor pronóstico en estos pacientes. El objetivo fue estudiar la calcificación valvular, su evolución y relación con diferentes factores de riesgo. Material y métodos: Se seleccionó aleatoriamente parte de la muestra del estudio NEFRONA analizando la calcificación valvular aórtica y mitral en ecocardiogramas de la visita basal y a los 24 meses. Resultados: Se estudiaron 397 pacientes con filtrado glomerular estimado (FGE) basal de 33 ml/min/1.73 m2 con disminución significativa hasta 30.9 ml/min/1.73 m2 Se produjo aumento del área de placa carotidea y femoral, así como aumento de los pacientes con calcificación valvular a los 24 meses. Se observó asociación positiva de la calcificación mitral a 24 meses con la edad, el índice tobillo brazo (ITB) y el producto calcio fósforo (CaxP) basal, sin asociación con el FGE. La calcificación aórtica a los 24 meses presentó asociación positiva con el área total de placa carotidea, el fósforo y la edad basal, sin relación con el FGE. Conclusiones: Se objetivó en pacientes con ERC, sin enfermedad cardiovascular conocida progresión de la calcificación valvular a dos años independientemente del FGE. Presentaron mayor calcificación valvular mitral aquellos de mayor edad, mayor ITB y producto CaxP. Presentaron mayor calcificación valvular aórtica aquellos de mayor edad, mayores niveles de fósforo y mayor área total de placa carotidea. La identificación de estos pacientes con mayor riesgo, podría ayudar a evitar eventos cardiovasculares futuros intensificando el seguimiento. Subclinical cardiac structural alterations have prognostic value in these patients. The aim was to analyse the prevalence of valvular calcification, the evolution and the relationship with different risk factors. Material and methods: Part of the sample of the NEFRONA study was randomly selected. Aortic and mitral valve calcification were analysed in echocardiograms performed at the baseline visit and at 24 months. Results: We included 397 patients, the estimated basal glomerular filtrate (eGFR) was 33 ml/min with significant decrease to 30.9 ml/min. There was an increase in the area of carotid and femoral plaque, as well as an increase in patients with aortic and mitral calcification at 24 months. A positive association of mitral calcification at 24 months with age, ankle-brachial index (ABI) and calcium-phosphorus product (CaxP) at baseline visit was observed, without association with eGFR. Aortic calcification at 24 months was positively associated with age, phosphorous and total carotid plaque area at baseline, with no relationship to eGFR. Conclusions: A significant prevalence of valvular calcification was observed in patients with CKD without known cardiovascular disease.Two-year progression was observed independently of the eGFR. Patients with higher risk of mitral valve calcification were those with older age, higher ABI and CaxP product. Patients with a higher risk of aortic calcification were those with older age, higher phosphorous levels and larger area of carotid plaque. Identifying these higher risk patients would help to avoid future cardiovascular events intensifying follow-ups.

Objetivo: Comparar las creencias de enfermedad y las creencias de medicación con la adherencia al tratamiento en pacientes con hipertensión arterial. Método: Diseño transversal comparativo en población hipertensa sin diabetes mellitus, grupo de 137 pacientes adherentes y grupo de 129 pacientes no adherentes. Se evaluaron las creencias de la enfermedad y las creencias de la medicación. El análisis estadístico incluyó prueba de ji al cuadrado, razón de momios e intervalo de confianza para razón de momios. Resultados: En el grupo de pacientes adherentes, el 71.5% tienen creencias adecuadas de la enfermedad, y en el grupo de pacientes no adherentes el 43.4% también tienen creencias adecuadas de la enfermedad (p = 0.000). Cuando se agrupan las creencias de la enfermedad y las creencias sobre la medicación existe una asociación con la adherencia al tratamiento en los pacientes hipertensos. Conclusiones: Existe asociación entre las creencias de la enfermedad y las creencias de la medicación con la adherencia al tratamiento antihipertensivo.

Objective: To prepare a list of highly toxic drugs in infants (HTDs) marketed in Spain, comparing those that reach the lethal dose in a child of 10 kg with the ingestion of one to three units. Method: HTDs are defined as those capable of causing severe or lethal poisoning in children less than 8-year-old. Severe poisoning is considered as that corresponding to Grade 3 in the Poisoning Severity Score classification and to the “major effects” category in publications in the American Association of Poison Control Centers (AAPCC). A literature review was carried out on the annual reports of the AAPCC, as well as in PubMed, between January 2000 and February 2019 (key words: “severe”, “fatal”, “life-threatening”, “poisoning”, “child”, “paediatric”, “toxicological emergency”). An observational, retrospective study was also conducted on infants less than 8-year-old that were seen in a Paediatric Emergency Department due to suspected drug poisoning between July 2012 and June 2018.The active ingredients responsible marketed in Spain were selected, and the lethal or highly toxic doses were determined. The number of units (pills) necessary to reach this dose in children of 10 kg was calculated. Results: A total of 7 HTD groups were identified: analgesics; psychotropics and other medication used in neurological disorders; catarrh decongestants – cough –antihistamine – asthma drugs; cardiovascular drugs; antibiotics, topical preparations, and other drugs. In 29 active ingredients, the ingestion of a single pill could cause death in 10 kg infant, in another 13, the ingestion of 2 pills could cause death, as well as the ingestion of 3 pills in 10 cases. Conclusion: There are numerous HTDs marketed in Spain, some of which are available in potentially fatal presentations with few pills. Resumen: Objetivo: Elaborar un listado de medicamentos altamente tóxicos en la infancia (MAT), comercializados en España, diferenciando aquellos que alcanzan la dosis letal para un niño de 10 kg con la ingesta de una a tres unidades. Método: Se definió MAT como aquellos capaces de producir intoxicaciones graves o letales en niños menores de 8 años. Se consideró toxicidad grave la correspondiente al grado 3 en la clasificación Poisoning Severity Score y a la categoría “major effects” en las publicaciones de la American Association of Poison Control Centers (AAPCC). Se realizó una revisión bibliográfica de los informes anuales de la AAPCC y de PubMed entre enero 2000 y febrero 2019 (palabras clave: “severe”, “fatal”, “life-threatening”, “poisoning”, “child”, “pediatric”, “toxicological emergency”). Además, se realizó un estudio observacional retrospectivo de menores de 8 años que consultaron en un servicio de urgencias pediátrico por sospecha de intoxicación farmacológica entre julio 2012 y junio 2018.Se seleccionaron los principios activos responsables comercializados en España y se determinó la dosis letal o la dosis altamente tóxica. Se calculó el número de unidades necesarias para alcanzarla en niños de 10 kg. Resultados: Se identificaron 7 grupos de MAT: analgésicos; psicofármacos y medicamentos neuromusculares; anticatarrales descongestivos-antitusígenos-antihistamínicos-antiasmáticos; medicamentos cardiovasculares; antimicrobianos; preparados tópicos y otros medicamentos. En 29 principios activos, la ingesta de una única unidad podría causar la muerte en un lactante de 10 kg de peso, en 13 podría causarla la ingesta de dos unidades y en 10 la ingesta de tres unidades. Conclusión: Existen numerosos MAT comercializados en España, algunos de ellos disponibles en presentaciones potencialmente letales con pocas unidades.

Resumen: Objetivo: Elaborar un listado de medicamentos altamente tóxicos en la infancia (MAT), comercializados en España, diferenciando aquellos que alcanzan la dosis letal para un niño de 10 kg con la ingesta de 1-3 unidades. Método: Se definió MAT como aquellos capaces de producir intoxicaciones graves o letales en niños menores de 8 años. Se consideró toxicidad grave la correspondiente al grado 3 en la clasificación Poisoning Severity Score y la categoría «major effects» en las publicaciones de la American Association of Poison Control Centers. Se realizó una revisión bibliográfica de los informes anuales de la American Association of Poison Control Centers y de PubMed entre enero 2000 y febrero 2019 (palabras clave: «severe», «fatal», «life-threatening», «poisoning», «child», «pediatric», «toxicological emergency»). Además, se realizó un estudio observacional retrospectivo de menores de 8 años que consultaron en un servicio de urgencias pediátrico por sospecha de intoxicación farmacológica entre julio 2012 y junio 2018.Se seleccionaron los principios activos responsables comercializados en España y se determinó la dosis letal o la dosis altamente tóxica. Abstract: Objective: To prepare a list of highly toxic drugs in infants (HTDs) marketed in Spain, comparing those that reach the lethal dose in a child of 10 kg with the ingestion of 1 to 3 units. Method: HTDs are defined as those capable of causing severe or lethal poisoning in children less than 8-year-old. Severe poisoning is considered as that corresponding to Grade 3 in the Poisoning Severity Score classification and to the “major effects” category in publications in the American Association of Poison Control Centers. A literature review was carried out on the annual reports of the American Association of Poison Control Centers, as well as in PubMed, between January 2000 and February 2019 (Keywords “severe”, “fatal”, “life-threatening”, “poisoning”, “child”, “paediatric”, “toxicological emergency”). An observational, retrospective study was also conducted on infants less than 8-year-old that were seen in a Paediatric Emergency Department due to suspected drug poisoning between July 2012 and June 2018.The active ingredients responsible marketed in Spain were selected, and the lethal or highly toxic doses were determined. The number of units (pills) necessary to reach this dose in children of 10 kg was calculated. Results: A total of 7 HTD groups were identified: analgesics; psychotropics and other medication used in neurological disorders; catarrh decongestants – cough –antihistamine – asthma drugs; cardiovascular drugs; antibiotics, topical preparations, and other drugs. In 29 active ingredients, the ingestion of a single pill could cause death in 10 kg infant, in another 13, the ingestion of 2 pills could cause death, as well as the ingestion of 3 pills in 10 cases. Conclusion: There are numerous HTDs marketed in Spain, some of which are available in potentially fatal presentations with few pills.

The study of the optical properties of the Cd1-xZnxTe1-ySey (CZTS) crystal provides a clear idea about its response to incident X-ray or gamma radiation. This is important for selecting a proper composition of CZTS to achieve superior quality and high-resolution X-ray and gamma radiation detectors at room temperature and reduce their production cost. This article’s novelty is in lowering the cost of the optical and compositional characterisation of CZTS using the ellipsometry technique. The most significant successes achieved are the composition ellipsometry model determination of CZTS based on the Effective Medium Approximation (EMA) substrate of the binary compound CdTe and ZnSe with an oxide layer of CdTe and the experimental verification that the bandgap moves to lower energies with the addition of Se.

Background: Pressure ulcers are a public health problem given the impact that they have on morbidity, mortality and the quality of life and participation of patients who suffer from them. Therefore, the main objective of this study was to evaluate the presence of differences in the radiofrequency parameters applied to complex pressure ulcers throughout the sessions and between the right and left leg. As a secondary objective, the subjective perceptions of the effects of the treatment by both the patients and the practitioner were analyzed. Methods: We performed a secondary analysis of data from a prospective study involving 36 patients from the Hospital de Guadarrama in Madrid, Spain, who presented ulcers in the lower limbs. Ten treatment sessions of radiofrequency were administered with a frequency of one session/week, collecting the data referring to the variables in each of the sessions. The main outcome variables were the radiofrequency parameters automatically adjusted in each session and that referred to the frequency (Hz), maximum and average power (W), absorbed energy by the ulcer (J/cm2) and temperature (°C) reached by the tissues. On the other hand, the subjective perception of the results was evaluated using the Global Response Assessment (GRA), a Likert-type scale that scores the treatment results from 1 (significantly worse) to 5 (significantly better). Likewise, the satisfaction of both the patients and the professional were evaluated using a 10-point numerical scale. Results: The ANOVA test showed significant differences (p < 0.05) throughout the sessions except in patient satisfaction. The ANOVA test showed significant differences (p < 0.05) between both legs and over time in all parameters except for frequency. The presence of significant differences (p < 0.05) was observed over time between legs compared to the initial values in the absorbed energy and in temperature, with higher final values in the absorbed energy in the left leg compared to the right (26.31 ± 3.75 W vs. 17.36 ± 5.66 W) and a moderate effect on both (R2 = 0.471 and 0.492, respectively). The near absence of changes in the satisfaction of both the patients and the professional was observed, while the score in the GRA decreased continuously throughout the sessions. Conclusions: Radiofrequency parameters are indicative of an improved clinical response to ulcers. In addition, higher radiofrequency exposure increases healing capacity. However, the subjective perception of treatment outcomes worsened, which may be related to the chronic nature of the ulcers, leading to patients’ expectations not being met.

Gasdermins (GSDM) genes play complex roles in inflammatory diseases and cancer. Gasdermin-B (GSDMB) is frequently upregulated in human cancers, especially in HER2-amplified breast carcinomas, and can promote diverse pro-tumor functions (invasion, metastasis, therapy-resistance). In particular, the GSDMB shortest translated variant (isoform 2; GSDMB2) increases aggressive behavior in breast cancer cells. Paradoxically, GSDMB can also have tumor suppressor (cell death induction) effects in specific biological contexts. However, whether GSDMB has inherent oncogenic, or tumor suppressor function in vivo has not been demonstrated yet in preclinical mouse models, since mice lack GSDMB orthologue. Therefore, to decipher GSDMB cancer functions in vivo we first generated a novel knock-in mouse model (R26-GB2) ubiquitously expressing human GSDMB2. The comprehensive histopathological analysis of multiple tissues from 75 animals showed that nucleus-cytoplasmic GSDMB2 expression did not clearly affect the overall frequency nor the histology of spontaneous neoplasias (mostly lung carcinomas), but associated with reduced incidence of gastric tumors, compared to wildtype animals. Next, to assess specifically the GSDMB2 roles in breast cancer, we generated two additional double transgenic mouse models, that co-express GSDMB2 with either the HER2/NEU oncogene (R26-GB2/MMTV-NEU mice) or the Polyoma middle-T antigen (R26-GB2/MMTV-PyMT) in breast tumors. Consistent with the pro-tumor effect of GSDMB in HER2+ human breast carcinomas, R26-GB2/MMTV-NEU GSDMB2-positive mice have double breast cancer incidence than wildtype animals. By contrast, in the R26-GB2/MMTV-PyMT model of fast growing and highly metastatic mammary tumors, GSDMB2 expression did not significantly influence cancer development nor metastatic potential. In conclusion, our data prove that GSDMB2 in vivo pro-tumor effect is evidenced only in specific biological contexts (in concert with the HER2 oncogene), while GSDMB2 alone does not have overall intrinsic oncogenic potential in genetically modified mice. Our novel models are useful to identify the precise stimuli and molecular mechanisms governing GSDMB functions in neoplasias and can be the basis for the future development of additional tissue-specific and context-dependent cancer models.

Objective: To describe the characteristics of paediatric patients with suspected poisoning treated by advanced life support (ALS) units, and to evaluate quality indicators (QI) for the prehospital emergency care of these patients. Method: A one-year observational study of patients under 18 years of age exposed to poisoning and treated by an ALS unit of the Medical Emergency System in Catalonia. Severe clinical criteria were defined, with 8 QI being evaluated for prehospital emergency care of poisoned paediatric patients. Results: The study included a total of 254 patients, with a median age of 14 years old (p25-75 = 7–16), with intentional poisoning in 50.8% of cases. The most frequently involved toxic agent was carbon monoxide (CO) (33.8%). Poisoning was found in 48.8% of those patients, being serious in 16.5%. Intentionally (OR 5.1; 95% CI: 1.9–13.8) and knowledge of the time of exposure (OD 3.1; 95% CI: 1.3–7.3) were independent risk factors associated with the appearance of severe clinical symptoms. Five QI did not reach the quality standard and included, availability of specific clinical guidelines, activated charcoal administration in selected patients, oxygen therapy administration at maximum possible concentration in carbon monoxide poisoning, electrocardiographic assessment in patients exposed to cardiotoxic substances, and recording of the minimum data set. Conclusions: Paediatric patients attended by ALS units showed specific characteristics, highlighting the involvement of CO and adolescents with voluntary poisoning. The QI assessment was useful to detect weak points in the quality of care of these patients and to develop strategies for improvement. Introduction: With a prevalence of 2-8%, preemclampsia constitutes a scenario that may imply the beginning of future vascular and metabolic alterations, among which we could mention chronic kidney disease. Objective: To establish preeclampsia as a factor associated with chronic kidney disease. Methods: Case-control study covering women who had been pregnant. The population sample included 60 subjects in the case group and 40 in the control group. The case group included patients with chronic kidney disease (glomerular filtration rate lower than 60 ml/min/1.73m2, CKD-EPI formula); the control group included patients who did not suffer from chronic kidney disease. Preeclampsia diagnosis was established according to medical history. The statistical analysis comprised the chi-squared test (X2), odds ratio, logistic regression and event occurrence probability. Results: Among the chronic kidney disease patients, 80% reported a history of preeclampsia; 35% of subjects who did not suffer from chronic kidney disease also had a similar history (p=0.00). The time of evolution of preeclampsia and the diagnosis of chronic kidney disease was 7.5 years (95% CI: 7.10-7.89). The model which best explains chronic kidney disease includes preeclampsia and patient’s age (p=0.000), y=-3.444 + 1.794 (history of preeclampsia) + 0.082 (patient’s age). If the patient is 35 years old and has a history of preeclampsia, the probability of chronic kidney disease is 0.7721. Conclusion: Preeclampsia is a factor associated with chronic kidney disease.

A group of patients could be considered low risk and not require aggressive procedures such a corticosteroid administration and endoscopy, especially in the paediatric population. Objective: To evaluate the safety and benefit of a less aggressive protocol for patients defined as low risk. Material and methods: An analytical–observational study conducted on patients who consulted for caustic ingestion between January 2011 and December 2015. Two periods were differentiated according to the current protocol. Period-1: usual protocol (which included admission and parenteral corticosteroid and antibiotic administration) and Period-2: less aggressive protocol for the low risk patients (oral intake test after 6 h and discharged if they remained asymptomatic). Low risk patients were considered as those who met the following criteria: unintentional intake, absence of symptoms and oral lesions. In the rest of the patients the usual protocol was performed. Re-admission with a diagnosis of digestive lesions was considered as a complication. Results: Forty-eight patients were included in period 1, and 35 in period 2. In period 2, thirteen patients met low risk criteria. The adherence to the less aggressive protocol was 100%. None of the low risk patients required admission or endoscopy after discharge. In period 1 the adherence to the usual protocol was 60.4%. Six patients would have benefited from the application of the less aggressive protocol. Conclusions: Adopting a more conservative attitude in low risk patients is safe. These patients benefit from clinical observation, without performing more aggressive measures with their possible iatrogenic adverse effects. Size, shape and hot spots are crucial to optimize Raman amplification from metallic nanoparticle (NPs). The amplification from radius = 1.8 ± 0.4 nm ultra-small silver NPs was explored. Increasing NP density redshifts and widens their plasmon that, according to simulations for NPs arrays, is originated by the reduction of the interparticle distance, d, becoming remarkable for d ≤ R. Inter-particle interaction red-shifts (>130 nm) and widens (>90 nm) the standard plasmon of non-interacting spherical particles. Graphene partly delocalizes the carriers enhancing the NIR spectral weight. Raman amplification of graphene phonons is moderate and depends smoothly on d while that of Rhodamine 6G (R6G) varies almost exponentially due to their location at hot-spots that depend strongly on d. The experimental correlation between amplification and plasmon position is well reproduced by simulations. Objetivo: Determinar la asociación entre sobrepeso-obesidad y trastornos del estado de ánimo en adolescentes, específicamente depresión, ansiedad, baja autoestima y dismorfia corporal. Metodología: Diseño transversal comparativo en adolescentes de 12 a 14 años, se integraron dos grupos el primero con sobrepeso-obesidad y el segundo con normopeso. Los trastornos del estado de ánimo fueron evaluados con Inventario de Depresión de Beck-II, Escala de ansiedad de Hamilton, Escala de autoestima de Rosenberg y Cuestionario de forma corporal de Cooper. El análisis estadístico incluyó chi cuadrada y razón de momios. Resultados: Se encontró asociación de sobrepeso-obesidad con depresión RM 17,94 (IC 95%; 8,17-39,38), ansiedad RM 11,11 (IC 95%; 4,75-25,97), autoestima RM 8,40 (IC 95%; 4,08-17,24) y dismorfia corporal RM 9,64 (IC 95%; 2,14-43,17). Conclusión: Existe asociación entre sobrepeso-obesidad y trastornos del estado de ánimo en adolescentes.

Cuando la población de estudio está integrada por un grupo de pacientes expuestos y se les observa durante un determinado periodo de tiempo para medir la presencia o ausencia de la enfermedad, el diseño se ubica en el contexto de los estudios observacionales, no obstante, no cumple con todos los requisitos de un diseño de cohorte, en consecuencia se podía decir que se trata de un diseño de investigación llamado cuasi cohorte.

Nanostructured silver (Ag) and gold (Au) are widely known to be potent biocidal and cytotoxic agents as well as biocompatible nanomaterials. It has been recently reported that combining both metals in a specific chemical composition causes a significant enhancement in their antibacterial activity against antibiotic-resistant bacterial strains, as well as in their anticancer effects, while preserving cytocompatibility properties. In this work, Ag/Au bimetallic nanoparticles over a complete atomic chemical composition range were prepared at 10 at% through a green, highly reproducible, and simple approach using starch as a unique reducing and capping agent. The noble metal nanosystems were thoroughly characterized by different analytical techniques, including UV-visible and FT-IR spectroscopies, XRD, TEM/EDS, XPS and ICP-MS. Moreover, absorption spectra simulations for representative colloidal Ag/Au-NP samples were conducted using FDTD modelling. The antibacterial properties of the bimetallic nanoparticles were determined against multidrug-resistant Escherichia coli and methicillin-resistant Staphylococcus aureus, showing a clear dose-dependent inhibition even at the lowest concentration tested (5 µg/mL). Cytocompatibility assays showed a medium range of toxicity at low and intermediate concentrations (5 and 10 µg/mL), while triggering an anticancer behavior, even at the lowest concentration tested, in a process involving reactive oxygen species production per the nanoparticle Au:Ag ratio. In this manner, this study provides promising evidence that the presently fabricated Ag/Au-NPs should be further studied for a wide range of antibacterial and anticancer applications.

n-Eicosane-melamine formaldehyde microcapsules of an average size of 1.1 μm and latent heat of fusion of 146.2 ± 5.3 J/g have been prepared. They have been characterized by scanning electron microscopy, FTIR spectroscopy, calorimetric techniques, and thermogravimetric analyses. Under processing conditions, the microcapsules apparently preserved their properties, also maintaining their n-eicosane loading and heat storage capacity under washing conditions (water with detergent at 60 °C). The microcapsules synthesis has been scaled up for the fabrication of functional films by extrusion. For that, polymer films containing 10 wt.% of microcapsules were prepared at a pilot plant level. In those films, even though a fraction of the n-eicosane loading was lost during the extrusion process, the microcapsules showed good compatibility within the polyamide. The percentage of PCM in the polyamide 6 films was estimated by TGA, verifying also the heat storage capacity predicted by DSC (2.6 ± 0.7 J/g).

Resumen: Objetivo: Describir las situaciones en las que se solicita cribado toxicológico en orina desde un servicio de urgencias pediátricas. Determinar si la prueba es potencialmente útil, si conlleva un cambio en el manejo del paciente y si los resultados se comprueban mediante técnicas específicas. Metodología: Estudio retrospectivo de los pacientes menores de 18 años atendidos en urgencias durante el año 2014 a los que se solicitó cribado de tóxicos en orina. Se definieron 2 grupos en función de la potencial capacidad de modificar el manejo del paciente (potencial utilidad y ausencia de utilidad). Resultados: Se recogieron 161 pacientes. En 87 casos (54,0%) el cribado de tóxicos se consideró sin potencial utilidad. En 55 pacientes (34,1%) la falta de utilidad fue debida a que la anamnesis ya explicaba la sintomatología presente, en 29 (18,0%) a que el paciente se encontraba asintomático y en 3 (1,9%) a la sospecha de intoxicación por una sustancia no detectable mediante esta técnica. El resultado ocasionó un cambio de manejo en 5 casos (3,1%). Se detectó algún tóxico en 44 pacientes (27,3%). Se solicitó confirmación con técnicas específicas en 2 (1,2%). Ambos fueron falsos positivos. Conclusiones: La mayor parte de los cribados de tóxicos solicitados no están justificados y es infrecuente que condicionen un cambio en el manejo del paciente. La confirmación mediante técnicas específicas es inusual. Su uso debe restringirse a casos concretos y, siempre que pueda tener repercusiones legales o el paciente niegue el consumo, debe seguirse de un estudio toxicológico específico que aporte un resultado concluyente. Objetive: To design a virtual antidote network between hospitals that could help to locate on-line those hospitals that stocked those antidotes with the highest difficulty in terms of availability, and ensured that the medication was loaned in case of necessity.Methods: The application was developed by four hospital pharmacists and two clinical toxicologists with the support of a Healthcare Informatics Consultant Company.Results: The antidotes network in Catalonia, Spain, was launched in July 2015. It can be accessed through the platform: www.xarxaantidots.org. The application has an open area with overall information about the project and the option to ask toxicological questions of non-urgent nature. The private area is divided into four sections: 1) Antidotes: data of interest about the 15 antidotes included in the network and their recommended stock depending on the complexity of the hospital, 2) Antidote stock management: virtual formulary, 3) Loans: location of antidotes through the on-line map application Google Maps, and virtual loan request, and 4) Documentation: As of June, 2016, 40 public and private hospitals have joined the network, from all four provinces of Catalonia, which have accessed the private area 2 102 times, requested two loans of silibinin, one of hydroxocobalamin, three of antiophidic serum and three of botulism antitoxin. Thirteen toxicological consultations have been received.Conclusions: The implementation of this network improves the communication between centers that manage poisoned patients, adapts and standardizes the stock of antidotes in hospitals, speeds up loans if necessary, and improves the quality of care for poisoned patients.

(3-aminopropyl)triethoxysilane (APTES) is a commonly used organosilane on surface functionalization of silicon oxide surfaces. However, its deposition process from solution-phase usually involves the use of toluene, which has often been identified as crucial for the formation of an aminopropylsilane monolayer. Toluene is ranked as a problematic solvent in the guide developed by a group referred to as the solvent sub-team of CHEM21. In this work, we propose a facile synthetic route for functionalizing a silicon substrate with APTES via solution-phase approach using only solvents that are classified as recommended. The influence of the APTES concentration, reaction times and different post-deposition conditions using acetic acid and methanol were studied in order to evaluate the quality and thickness of the organosilane layers. • The method uses ethanol as APTES solvent for functionalizing silicon dioxide surfaces and only uses solvents classified as recommended. • The method uses a solution phase approach, does not require complicated equipment and can be prepared at room temperature.

The present work is focused on the study of the effect that the casting solution concentration has on the morphology and gas separation performance of poly(ether-block-amide) copolymer membranes (Pebax® MH 1657). With this aim, three different concentrations of Pebax® MH 1657 in the casting solution (1, 3 and 5 wt%) were used to prepare dense membranes with a thickness of 40 µm. The morphology and thermal stability of all membranes were characterized by scanning electron microscopy, X-ray diffraction, differential scanning calorimetry, rotational viscometry and thermogravimetric analyses. An increase in crystallinity was notable when the amount of solvent in the Pebax® MH 1657 solution was higher, mainly related to the polymer chains arrangement and the solvent evaporation time. Such characteristic seemed to play a key role in the thermal degradation of the membranes, confirming that the most crystalline materials tend to be thermally more stable than those with lower crystallinity. To study the influence of their morphology and operating temperature on the CO2 separation, gas separation tests were conducted with the gas mixture CO2/N2. Results indicated that a compromise must be found between the amount of solvent used to prepare the membranes and the crystallinity, in order to reach the best gas separation performance. In this study, the best performance was achieved with the membrane prepared from a 3 wt% casting solution, reaching at 35°C and under a feed pressure of 3 bar, a CO2 permeability of 110 Barrer and a CO2/N2 selectivity of 36.

Cancer and antimicrobial resistance to antibiotics are two of the most worrying healthcare concerns that humanity is facing nowadays. Some of the most promising solutions for these healthcare problems may come from nanomedicine. While the traditional synthesis of nanomaterials is often accompanied by drawbacks such as high cost or the production of toxic by-products, green nanotechnology has been presented as a suitable solution to overcome such challenges. In this work, an approach for the synthesis of tellurium (Te) nanostructures in aqueous media has been developed using aloe vera (AV) extracts as a unique reducing and capping agent. Te-based nanoparticles (AV-TeNPs), with sizes between 20 and 60 nm, were characterized in terms of physicochemical properties and tested for potential biomedical applications. A significant decay in bacterial growth after 24 h was achieved for both Methicillin-resistant Staphylococcus aureus and multidrug-resistant Escherichia coli at a relative low concentration of 5 µg/mL, while there was no cytotoxicity towards human dermal fibroblasts after 3 days of treatment. AV-TeNPs also showed anticancer properties up to 72 h within a range of concentrations between 5 and 100 µg/mL. Consequently, here, we present a novel and green approach to produce Te-based nanostructures with potential biomedical applications, especially for antibacterial and anticancer applications.

Objetivo: Determinar el estado de salud en el adulto mayor con 60 o más años de edad en atención primaria a partir de una valoración geriátrica integral. Diseño: Estudio transversal descriptivo. Emplazamiento: Cinco unidades de atención primaria, Instituto Mexicano del Seguro Social; México. Participantes: Adultos mayores con 60 o más años que acuden a la consulta de atención primaria. Previo consentimiento informado, con criterios de eliminación, pacientes que no completaran la valoración geriátrica integral. Técnica muestral por conglomerados y cuota. Medidas principales: Variables de dimensión médica: visual, auditiva (Hearing Handicap Inventory for the Elderly), incontinencia urinaria (Consultation on Incontinence Questionarie), estado nutricional (Mini Nutritional Assessment), antecedentes personales patológicos, polifarmacia; mental cognitivo (Mini Mental State Examination), depresión (Yesavaje); funcional: actividades básicas (Katz) e instrumentales de vida diaria (Lawton y Brody), marcha (Up and go) y social (Escala de recursos social). El análisis incluyó porcentajes e intervalos de confianza. Resultados: En dimensión médica: 42,3% presentó disminución visual, 27,7% auditiva, 68,3% incontinencia urinaria, 37,0% desnutrición, 54,7% polifarmacia; dimensión mental: 4,0% deterioro cognitivo grave y 11% depresión; dimensión funcional: 2,0% dependencia total de actividades básicas de la vida diaria; 14,3% deterioro en actividades instrumentales; 29,0% alteración en la marcha; y 48,0% presentan recursos sociales medianamente deteriorados. Conclusión: El estado de salud en el adulto mayor que acude a atención primaria se caracteriza por pacientes independientes con diferente grado de alteraciones en la dimensión médica, bajo grado de alteración mental y recursos sociales medianamente deteriorados.

La necrosis recidivante posterior a quimioterapia no es frecuente; se relaciona habitualmente con la extravasación de citostáticos y esta se define como la salida no intencionada del fármaco durante su administración intravenosa hacia los espacios perivascular y subcutáneo, ya que una de las vías de administración de la quimioterapia es la endovenosa (EV), por tanto, es una de las complicaciones que puede ocurrir y que puede ser tratada con diferentes antídotos, en dependencia del citostático extravasado;1-4 y recidiva, porque pasado un tiempo aparecen los síntomas de igual forma en que ocurrió la extravasación pero, además, infectada. Sus consecuencias clínicas, si no es tratada a tiempo, pueden ir desde el dolor local hasta provocar endurecimiento en todo el trayecto del vaso sanguíneo, necrosis o fibrosis tisular, hasta llegar al acortamiento del miembro y causar pérdida de su función

A student essay for the Special Student Issue of the Journal of Extreme Anthropology accompanying the art exhibition 'Artist's Waste, Wasted Artists', which opened in Vienna on the 19th of September 2017 and was curated by the students of social anthropology at the University of Vienna. This essay investigates the structure of the art world, as well as the role of museums, likening them to prisons of taste, thus pointing to structural limitations imposed upon the artist, while also showing, using the case of the Viennese photographer Sabine Hauswirth, that even within such system, artistic freedom, or at least its semblance, are possible.

El adenoma velloso del apéndice cecal es una entidad patológica rara que tiene una forma clínica particular de presentación diferente a los demás adenomas vellosos de otras partes del tubo digestivo. Se presenta paciente femenina octogenaria con dolor abdominal en fosa ilíaca derecha que fue, desde el inicio, interpretada como apendicitis aguda del anciano por ausencia de antecedentes patológicos personales o asociación con otras entidades más frecuentes a la edad de la paciente. Fue intervenida y se envía la pieza al servicio de Anatomía Patológica para estudio, encontrándose un diagnóstico poco común e infrecuente.

In­tro­duc­ción La lumbalgia afecta de manera frecuente a la población económicamente activa. Es la primera causa de pérdida de días laborales en trabajadores menores de 55 años de edad y la segunda de ausentismo laboral. Objetivo Determinar el costo institucional del paciente con incapacidad temporal para el trabajo por lumbalgia mecánica. Material­ y­ métodos Se realizó un estudio de costos de expedientes de trabajadores con lumbalgia de 20 a 60 años con incapacidad temporal para el trabajo. El tamaño de la muestra (228 pacientes) se calculó con la fórmula de promedios para población infinita con nivel de confianza del 95%. La técnica muestral fue por cuota empleando como marco muestral el listado de pacientes con incapacidad temporal para el trabajo y diagnóstico de lumbalgia mecánica. las variables estudiadas fueron las características sociodemográficas, el perfil de uso, costo unitario y costo promedio de los servicios otorgados. El análisis estadístico incluyó porcentajes, promedios, intervalo de confianza y proyección. Resultados El costo total promedio por paciente con lumbalgia mecánica es de 1744,08 uSD, la incapacidad es el costo más elevado (1083,71 uSD), seguido de los estudios de gabinete (394,89 uSD), las consultas (180,52 uSD), los medicamentos y el laboratorio. Conclusión El costo institucional del paciente con incapacidad temporal para el trabajo con lumbalgia mecánica resulta en miles de dólares y, proyectado a una población de pacientes, el costo se incrementa a millones.

El carcinoma del tiroides se caracteriza por su baja mortalidad y poca incidencia. Las formas metastásicas regionales son las formas de metástasis más frecuentes que ocurren generalmente en ganglios cervicales. Se presenta un caso de metástasis inusual y poco frecuente de cáncer de tiroides aunque con caracteres de presentación común en cuanto a sexo y edad en estas lesiones quísticas del cuello.

Se realizó un estudio descriptivo, retrospectivo y transversal en el Hospital Universitario “Dr. Miguel Enríquez”, de La Habana, en el período comprendido desde enero del 2000 hasta diciembre del 2007, con el objetivo de caracterizar los diferentes tipos de pólipos colorrectales mediante el diagnóstico clínico-endoscópico en pacientes sometidos a colonoscopia terapéutica (polipectomía). El universo estuvo constituido por todos los pacientes que se les identificó la presencia de pólipo colorrectal, que fueron un total 361. La muestra estuvo conformada por los 290 pacientes sometidos a colonoscopía terapéutica que representaron el 80.33 % del total de pacientes y de estos se extrajeron 595 pólipos. En la serie predominaron de forma significativa los mayores de 70 años y el sexo masculino. Las manifestaciones clínicas más frecuentes que llevaron a indicar la colonoscopia fueron: cambios en el hábito intestinal, dolor abdominal y la rectorragia. Los pólipos de tipo sésiles fueron los más usuales, se localizaron con más frecuencia en la región del sigmoide y del recto.

Introducción: el Cáncer tiroideo se caracteriza por su baja incidencia, comportamiento maligno infrecuente y baja mortalidad. La mayoría de ellos procede de bocios multinodulares de larga duración por lo que es muy importante tener en cuenta factores que constituyen riesgos potenciales como: tiempo de establecimiento de la enfermedad, tamaño, así como las características particulares personales y la evolución de la entidad en cada caso. Si tenemos en cuenta el gran avance tecnológico que constituye la Biopsia Aspirativa con Aguja Fina (BAAF), que nos permite no solamente evitar su transformación neoplásica maligna, sino que también podemos prolongar la vida con la calidad que humanamente se requiere que es en definitiva la meta principal de la Salud Pública Cubana. Objetivo: destacar la importancia del adecuado seguimiento evolutivo del Bocio multinodular como elemento significativo en la prevención de su transformación maligna. Presentación del caso: Paciente femenina de 80 años de edad con aumento de volumen de la región anterior del cuello y antecedentes patológicos personales de Bocio Multinodular (BMN) con tratamiento inconstante e irregular de años de evolución, quien no solamente pudo haber mejorado la calidad de su vida sino que no la hubiera truncado, considerando las condiciones atenuantes en la forma de presentación que tuvo su caso. Al examen físico: Aumento de la región anterior del cuello, palpándose masa de superficie irregular, consistencia firme, dolorosa, de aproximadamente 6x5 cm que había aumentado considerablemente en los últimos meses sin fecha precisada. La paciente fue ingresada por el gran agobio respiratorio y la toma del estado general, pudiéndose realizar únicamente la Biopsia Aspirativa con Aguja Fina (BAAF). Desde su ingreso presenta evolución tórpida con incremento del compromiso respiratorio requiriendo la práctica de la traqueotomía que sólo ayuda como medida de soporte al estadio terminal de esta enfermedad neoplásica maligna. (Carcinoma Anaplásico del Tiroides). A pesar de ello la paciente fallece y los familiares autorizan la necropsia. Conclusiones: los BMN deben tener seguimiento evolutivo que permita precisar una tiroidectomía a tiempo como prevención en enfermedades benignas del Tiroides, por los cambios malignos que pueden producirse cuando la glándula es irradiada. La tiroidectomía a tiempo permite evitar no solamente la transformación maligna que puede producirse, sino también previene las metástasis que ocurren en los Carcinoma Anaplásicos.Introduction: the thyroid carcinoma is characterized by low incidence, low malignant behaviour as well as low mortality. The most of they are developed from multinodular goitres of long duration so is important to be into account the risk factors such as: time of the establishment of the disease, size and the particular and personal characterists and the evolution of the each case. If we take into account the great technological advance of the BAAF from its performance in the past century to be united to the National Health System of our country ,we have to say that the multinodular goitres in any group of age are easily manageable and accessible which may us to avoid the malignant transformation besides we can do that the life continues with the quality humanely requested . Finally, this is the first goal of the Cuban Public Health. Objective: to highlight the importance of an appropriate follow-up of multinodular goitre as a significant element in the prevention of its multinodular malignant transformation. Case presentation: an 80 year- old, female patient presents an increase of the volumen of the anterior section of the neck, with pathologic antecedents of a multinodular goitre which the evolution has been characterized by an irregular and non systematic treatment along of years .She could have better quality of life as well as to avoid to death taking into account the wild conditions of that disease was presented herself. Physical examination: It has been observed an increase of the anterior area of the neck. On palpation ,it was felt a mass of irregular surface , painful and of a hard consistency with 6x5cms. approximately that had increased notably in the course of the last months . On the admission, the patient presented a serious respiratory process and poor general status so we only achieved a Fine Needle Aspirative Biopsy (FAAB).She continuos with reapiratory compromised so a surgical racheotomy was ejecuted that only help her as a way of supporting in the patient terminal stage in this malignant disease (Anaplastic Carcinoma of Thyroid). Despite the recommended procedure the patient died. The family accepted the autopsy. Conclusions: the multinodular goitres have to exam periodically in order to just may to do the tiroidectomy as the preventive measure in benign diseases of thyroid and avoid the malignant changes that could occurred when the gland is irradiated.The tiroidectomy ejecuted at just time avoid the malignant transformation as well as the metastasis in Anaplastic Carcinoma Thyroid.

Objetivo: Determinar el costo oportunidad de la embarazada que demanda atención en primer nivel de atención. Método: Estudio de costo oportunidad en la embarazada atendida en consulta de Medicina Familiar, Laboratorio y Farmacia, que acudieron solas o acompañadas a 3 unidades de Medicina Familiar. Se estudiaron 807 embarazadas. La técnica muestral contempló conglomerados (Unidad Médica), estratos (consultorio) y al interior por cuota. Se estudió edad, estado civil, ocupación y número de acompañantes. El costo oportunidad se evaluó a través de tiempo invertido y el costo por minuto; se estimó para el traslado, espera y atención en los tres servicios señalados, para la paciente y acompañante. Se utilizó porcentajes, promedios e intervalos de confianza del 95%. Resultados: El costo oportunidad de la embarazada sola en consulta de Medicina Familiar es $46,48 (IC95% 41,68-51,27), del acompañante 1 $49,91 (IC 95% 41,49-58,39) y acompañante 2 $9,84 (IC95% 4,05-15,99); cuando la embarazada acude con un acompañante el costo oportunidad es de $95,62 y con dos acompañantes $106,23. El costo oportunidad de la embarazada sola en Farmacia es $22,14 y en Laboratorio $34,82. El costo oportunidad ajustado por número de acompañantes en consulta de Medicina Familiar es $77,14; si utiliza en la misma visita la consulta de Medicina Familiar, Laboratorio y Farmacia es de $104,20. Conclusión: El costo oportunidad en la paciente embarazada que demanda atención en primer nivel tiene variaciones importantes determinadas por el número de servicios que utilice y la presencia de acompañantes.Objective: To determine the cost opportunity for pregnant women who receive first-level health care. Methods. Cost opportunity study including 807 pregnant tamales who received first-level care in the family medicine, laboratory and pharmacy areas. The sample was distributed by medical unit and stratified by office. Variables included age, marital status, occupation, number of accompanying; cost opportunity was estimated for transfers, waiting and care time in the mentioned service areas. Cost opportunity was calculated for the patient and for those persons who accompanied the patient and included the time invested and cost per minute. Statistical analysis included averages and 95% confidence intervals. Results. Cost opportunity for pregnant alone in family medicine is $46.48 (IC 95% 41.68-51.27), companion 1 $49.91 (IC 95% 41.49-58.39) and companion 2 $9.84 (IC 95% 4.05-15.99); for pregnant women and one companion, cost opportunity is $95.62 and whit two companions $106.23. Cost opportunity for pregnant women alone in pharmacy is $22.14 and laboratory $34.82. Cost opportunity adjusted by companions in family medicine is $77.14, if used in the same visit family medicine, laboratory and pharmacy is $104.20. Conclusion. Cost opportunity for a pregnant woman who receives first-level care it has significant variations determined by the number of services that use and the presence of companions.

Objetivo: El propósito de la investigación fue evaluar la evolución a un año los efectos adversos en una cohorte de pacientes con implante subdérmico de desogestrel. Método: Se incluyeron pacientes portadoras de implante que tenían como mínimo tres meses de uso, previo consentimiento informado. Se realizaron 4 mediciones en forma trimestral, registrándose los efectos más frecuentes referidos por la paciente. El análisis incluyó porcentajes, promedios e intervalos de confianza al 95%. Resultados: Se estudiaron 50 pacientes, los efectos adversos presentados con mayor frecuencia en el primer trimestre fueron alteraciones del ciclo 48%, mastalgia 46%, amenorrea 40%, mareo 40%, cefalea 40%, náuseas 30%, acné 28%, aumento de peso 26% y dolor local 10%. Introducción:el Cáncer tiroideo se caracteriza por su baja incidencia,comportamiento maligno infrecuente y baja mortalidad. La mayoría de ellosprocede de bocios multinodulares de larga duración por lo que es muy importantetener en cuenta factores que constituyen riesgos potenciales como: tiempo deestablecimiento de la enfermedad, tamaño,así como las características particularespersonales y la evolución de la entidad encada caso. Si tenemos en cuenta el granavance tecnológico que constituye la Biopsia Aspirativa con Aguja Fina (BAAF), quenos permite no solamente evitar su transformación neoplásicamaligna, sino quetambién podemos prolongar la vida con lacalidad que humanamente se requiereque es en definitiva la meta principal de la Salud Pública Cubana.Objetivo:destacar la importancia del adecuado seguimiento evolutivo del Bociomultinodular como elemento significativo en la prevención de su transformaciónmaligna.Presentación del caso:paciente femenina de 80 años de edad con aumento devolumen de la región anterior del cuelloy antecedentes patológicos personales deBocio Multinodular (BMN) con tratamiento inconstante e irregular de años deevolución, quien no solamente pudo haber mejorado la calidad desu vida sino queno la hubiera truncado, considerando lascondiciones atenuantes en la forma depresentación que tuvo su caso. Al examen físico: Aumento de la región anterior delcuello, palpándose masa de superficie irregular, consistencia firme, dolorosa, deaproximadamente 6x5 cm que había aumentado considerablemente en los últimosmeses sin fecha precisada. La paciente fueingresada por el gran agobio respiratorioy la toma del estado general, pudiéndoserealizar únicamente la Biopsia Aspirativacon Aguja Fina (BAAF). Desde su ingreso presenta evolución tórpida conincremento del compromiso respiratorio requiriendo la práctica de la traqueotomíaque sólo ayuda como medida de soporte alestadio terminal de esta enfermedadneoplásica maligna. A pesar de ello la pacientefallece y los familiares autorizan la necropsia.Conclusiones: los BMN deben tener seguimientoevolutivo que permita precisaruna tiroidectomía a tiempo como prevención en enfermedades benignas delTiroides, por los cambios malignos que pueden producirse cuando la glándula esirradiada.
