Cutaneous lichen planus is a skin disorder characterized by the development of inflammatory, itching skin lesions. This condition can be treated by various types of retinoids given orally. These include all-trans-retinoic acid (tretinoin), etretinate, and acitretin. The effectiveness of these retinoids in treating lichen planus has not been appropriately assessed. Hence, the effectiveness of acitretin was examined in patients with lichen planus, with or without involvement of the mucous membranes. Acitretin, the primary metabolic product of etretinate, is as effective as etretinate, but is removed from the body faster than etretinate. The study involved 65 patients treated with 30 milligrams of acitretin over an eight-week period. Remission or significant improvement in lichen planus was observed in 64 percent of patients treated with acitretin and only 13 percent of patients given an inert placebo. Eighty-three percent of patients initially given a placebo responded to treatment with acitretin. Retinoid treatment caused adverse effects in all patients. The medical literature concerning the effectiveness of topical cyclosporine for treating disorders of the mucous membranes of the mouth is reviewed. In general, topical cyclosporine has not been effective against conditions known to respond to systemic (for instance, orally administered) cyclosporine, but direct injection into skin lesions can produce results. This implies that increased skin absorption of the drug could improve its effectiveness. Absorption by the oral mucosa is known to be considerably better than by skin, thus presumably increasing drug action. The results are presented from cyclosporine treatment of oral lichen planus (red, itchy bumps and scales), bullous disorders (in which blisters form), and aphthous stomatitis (severe canker sores). Most patients with lichen planus of the mouth were helped by the drug. Many patients with oral bullous disorders are elderly, and the potential toxicity of cyclosporine is a concern. However, in studies involving small numbers of patients, several experienced reduction in lesion size as well as pain relief. Treatment with cyclosporine of aphthous stomatitis was effective in some cases but not others, for reasons that may be linked to the various causes of this disorder. Lichen planus is a skin disorder characterized by inflammatory, pruritic, or itching, skin lesions. The lesions start as pinhead-sized papules, or skin elevations, that become red and glistening, and fuse to form rough, scaly patches. These patches may occur on the inner arm, trunk, genitals, and oral and vaginal mucosal membranes. There are six subtypes of lichen planus. The cause of this skin disorder is not known, but it may be related to exposure to certain chemicals. Erythema dyschromicum perstans is a skin disorder, characterized by a bluish gray discoloration, and sharply outlined skin lesions, that may be elevated or itching. Erythema dyschromicum resembles some forms of lichen planus. A case is described of a 53-year-old woman with lichen planus on her extremities. A year later, her skin lesions resembled those of erythema dyschromicum perstans. Both diseases resolved after treatment with griseofulvin, an antifungal agent, but recurred after drug treatment was discontinued. Retreatment with griseofulvin cleared the recurrent skin lesions. It was concluded that the patient initially had the typical form of lichen planus, but later developed the classic lesions of erythema dyschromicum perstans, which may also be referred to as lichen planus pigmentosus, a subtype of lichen planus. Lichen planus is a skin disorder characterized by itching and inflamed papules (elevations) which may become scaly patches in more extreme cases. The results of treatment of lichen planus with acitretin are reported. Eleven patients received acitretin daily for 16 weeks, with evaluation of their clinical signs according to a four-point scale at each clinic visit. Six of the eight patients who completed the study experienced complete remission or marked improvement in their symptoms. The level of improvement stabilized after four weeks of treatment. Two patients had reactions to the drug that were so severe that the treatment had to be stopped. Overall, the results indicate that acitretin may be more effective than etretinate or isotretinoin, drugs commonly used for treating lichen planus, with a more rapid elimination from the body. One important consequence of this is that in pregnant women, teratogenic effects (damage to a developing fetus) are limited to the period when the drug is being used and do not continue for weeks thereafter.
