Clear cell renal cell carcinoma (ccRCC) is a challenging malignancy characterized by intricate biology and clinical characteristics. Despite advancements in treatment strategies, the molecular mechanisms underlying ccRCC initiation, progression, and therapeutic resistance remain elusive. Inflammasomes, multi-protein complexes involved in innate immunity and inflammation, have emerged as potential regulators in cancers. However, their involvement and mechanisms in ccRCC remain poorly understood. In this study, we conducted a systematic investigation into the expression patterns and clinical significance of inflammasome complexes in ccRCC. We found the perturbation of inflammasome complexes genes was related to patient’s prognosis and other clinical characteristics. By developing an Inflammasome Complexes (IFC) score and identifying IFC subtypes with distinct clinical characteristics and oncogenic roles, our study suggested that inflammasome activation could impact tumorigenesis and modulate the tumor immune landscape, particularly its positive correlations with immunosuppressive macrophages. Furthermore, our study revealed the potential of inflammasome complex genes as predictive markers for patient responses to various anti-tumor drugs, including Osimertinib, Ulixertinib, Telomerase Inhibitor IX, and GSK2578215A. These findings have significant clinical implications and offer opportunities for guiding treatment strategies and improving patient outcomes of ccRCC.

Abstract Anther development involves a series of important biological events that are precisely regulated by many genes. Although several important genes involved in rice anther development have been identified, the regulatory network involved in tapetal development and pollen wall formation is still largely unclear. PERSISTENT TAPETAL CELL 1 (PTC1) encodes a PHD-Finger protein, which plays a critical role in the regulation of tapetal cell death and pollen development in rice. Here, we report the isolation and characterization of a new allele ptc1-2 with 2-base deletion in the third exon, causing the absent of the PHD domain due to the sequence change. Cytological analysis revealed delayed tapetal PCD, defective pollen exine formation and abnormal ubisch bodies development. Transcriptome analysis revealed that genes related to pollen wall formation (secondary metabolism, phenylalanine synthesis, and cutin and wax biosynthesis pathways), cell death (cysteine and methionine metabolism and DNA repair pathways), and carbohydrate synthesis (starch and sucrose metabolism pathways) were significantly altered in ptc1-2 mutant. A total of 13 reported anther development genes exhibited significant expression changes in the ptc1-2 mutant. Yeast two-hybrid and BiFC analyses showed that PTC1 could interact with API5, an inhibitor of apoptosis, and the citrin-binding enzyme EDT1. This work is helpful in deepening the understanding of the regulatory network of male reproductive development in rice.

Objective Lower limb discrepancy (LLD) was frequently observed in patients with idiopathic scoliosis (IS), potentially associated with etiopathogenesis. Although sole lifts had been proposed as a conservative treatment for IS, evidence supporting their efficacy was limited. This study aimed to assess the effects of sole lift intervention on pediatric patients with mild IS, specifically focusing on thoracolumbar/lumbar (TL/L) curvature. Methods Twenty patients, with an average age of 12.3 ± 3.1 years and presenting mild TL/L curve (15.6° ± 6.2°), were selected from a pool of 267 pediatric IS patients in the outpatient of our spine center from February 2023 to August 2023. Inclusion criteria comprised a main TL/L curve ranging between 10° and 40°, the lower limb positioned at the convexity of the main curve, and LLD of less than 2 cm; individuals requiring bracing or surgical intervention were excluded. Custom sole lifts were used to address the shorter lower limb with the objective of leveling the pelvis. Radiographic evaluations were conducted both before and after intervention using standing full spine posteroanterior radiographs and full leg length radiographs. Statistical analysis was undertaken to evaluate curve correction and its associations with other influencing factors. Results The mean structural and functional LLD were 7.1 ± 4.5 mm and 7.1 ± 4.1 mm, respectively. Among the 20 patients, four exhibited structural LLD greater than 10 mm. The average follow‐up duration was 6.4 ± 1.9 months (range: 3–8 months). Following sole lift intervention (7.0 ± 3.0 mm), a significant reduction was observed in the TL/L curve compared to the pre‐sole lifting measurements (15.6° ± 6.2° vs. 12.1° ± 7.2°, p 0.05). Additionally, even after adjusting for other influencing factors, the TL/L Cobb angle remained significantly different between pre‐ and post‐sole lifting (p = 0.037). Conclusion This study confirmed the effectiveness of sole lift intervention in correcting TL/L and thoracic curves among the mild IS children with a main TL/L curve, providing a supplementary conservative treatment option for patients with the lower limb at the convexity of the main curve. Moreover, our findings underscored the active compensation of the lower limbs and the pelvis in the etiopathogenesis of IS, highlighting the importance of considering their influence in treatment strategies.

Abstract Objective Given the lack of global consensus on anesthesia selection for peritoneal dialysis catheter (PDC) placement via open surgery, this study investigates the safety and efficacy of combining local infiltration anesthesia (LIA) with monitored anesthesia care (MAC) in patients with end-stage renal disease (ESRD). Methods This retrospective analysis of ESRD patients who underwent open surgical placement of PDC in the Department of Nephrology, the First Affiliated Hospital of the Army Medical University from 1 August 2020 to 31 May 2022. Patients were categorized into two groups based on anesthesia method: LIA group and LIA + MA group. MA was defined as preoperative sedation (0.2–0.7 µg/kg/h) with dexmedetomidine injection (Huidrican trade name, DEX), and intraoperative analgesia with dexrazoxane injection (Garonin trade name, DEZ) as appropriate according to the patients’ pain scores. We compared the general clinical data of the two groups of patients, the changes in blood pressure and heart rate during the whole operation, the intraoperative and postoperative pain, the total duration of the operation and the immediate postoperative complications. Results The study included 123 patients (59 in the LIA + MAC group and 64 in the LIA group). Objective In urban rail transit, train departure intervals are generally short, leading to situations where two trains travel in the same direction within ultra-long tunnels. In the event of a fire, this scenario can result in significant property damage and casualties. Therefore, it is crucial to study the fire smoke propagation law and personnel evacuation strategies under dual-train tracking mode in ultra-long tunnels. Method Using FDS (fire dynamics simulation) software and Pathfinder software, a fire and personnel safe evacuation model for ultra-long tunnels is established to simulate and analyze the fire smoke propagation law and personnel evacuation under dual-train tracking mode. Result & Conclusion In this mode, if a fire occurs at leading train rear end, the longitudinal ventilation at critical wind speed can ensure the safety of leading train during the fire. When the downstream smoke front propagates at a speed of 3.20 m/s, it will reach the trailing train after 600 seconds and fully cover it after 620 seconds. The smoke temperature attenuates rapidly along tunnel longitudinal direction, which complies with the law of a bi-exponential model. In dual-train tracking mode, reducing the spacing between connecting passages can decrease the total personnel evacuation time, resulting in minimal impact on personnel time taken to leave the train. Opening train end doors and evacuation platform side doors can significantly reduce personnel evacuation time. In the most adverse fire scenario of dual-train tracking in an ultra-long tunnel, personnel evacuation is challenging, but their safety can be ensured by reducing connecting passage spacing.

In recent years, the application of digital twin technology in the field of intelligent management and control of bearer networks has become a new hotspot and research direction. Various research institutions both domestically and internationally are exploring relevant application scenarios and conducting standardization research. Based on the current research on digital twin technology in the field of information and communication, the new challenges faced by the management and control of 5G bearer networks were analyzed, and the application scenarios and requirements of digital twin technology in 5G bearer networks were explored. Based on these typical application scenarios, the design of digital twin application service scheme and the function and data description of emulator components were proposed. Finally, an application case of 5G network slicing resource allocation and change simulation function was described.

Abstract Background Acute closed midsubstance Achilles tendon rupture(ACMATR) is common, with various treatment methods developed over time. We retrospectively compared the two mini transverse-incision repair (2MTIR) with percutaneous repair (PR) to determine which method yields better results. Methods All cases meeting criteria from 2018 to 2021 in our hospital were included and followed up for 1 to 5 years. A final questionnaire with multiple indexes was conducted via phone call. Comparative analysis of these indexes between the two groups was performed using IBM SPSS Statistics (V.26). Continuous variables that passed tests for normality and equal variance were compared using the Student’s t-test. Ranked data were compared using the Mann-Whitney U test. Categorical variables were tested with the chi-square test or Fisher’s exact test. A p-value of less than 0.05 was considered statistically significant. Results There was one rerupture in the PR group. The final indexes for “Tightness Feeling”, “Heel Rising Strength”, and “Foot Numbness” were statistically different (P 0.05). Conclusions The 2MTIR technique provided a technically straightforward, minimally invasive procedure with well-preserved paratenon and direct end-to-end firm fixation in cases of ACMATR. It resulted in very low complications, easy rehabilitation, and full weight-bearing as early as 5–6 weeks postoperatively, yielding better functional outcomes compared to the PR technique in the 1–5 year follow-up. Abstract Objective To evaluate the efficacy and safety of hydroxychloroquine sulfate (HCQ) in the treatment of low risk phospholipase A2 receptor (PLA2R)-associated membranous nephropathy (MN). Methods A total of 110 patients with low risk PLA2R-associated MN were included in the study. Patients who met the inclusion and exclusion criteria were assigned randomly to two groups: the HCQ treatment group and the control group. The control group received standard supportive treatment according to the guidelines, while the HCQ treatment group received HCQ in addition to the supportive treatment. The clinical data of the patients were analyzed, with comparisons made at baseline and during the six-month follow-up period. Any adverse reactions were recorded. Results The baseline data were comparable between the HCQ treatment group and the control group. At the end of the six-month follow-up period, the reductions in urine protein excretion and serum PLA2R antibody titer were more notable in the HCQ treatment group than those in the control group, with these differences being statistically significant (p

Objective To investigate the law of surrounding soil displacement induced by construction under the arrangement of non-horizontal parallel double-line shield tunnels, a soil displacement calculation method based on modified Loganathan formula is proposed. Method The Loganathan formula and its relevant assumptions are introduced. Considering the comprehensive effects of ground loss, soil unloading rebound, and variation of soil loss rate along the excavation path caused by shield tunnel excavation disturbance, the Loganathan formula is modified, and a solution for soil displacement around a single-line shield tunnel is established. Then, a calculation method for soil displacement induced by construction in the case of non-horizontal parallel double-line shield tunnels is developed. The engineering applicability of the proposed method is validated through field monitoring and numerical simulation results of actual projects. Result & Conclusion The proposed calculation method can account for factors such as the angle (θ) between the tunnel axis line and the horizontal plane, the radius (R1,R2) of double-line tunnels, the spacing (D) between tunnel axes, and the buried depth (H1,H2) of double-line tunnels, and the influence of construction factors such as ground loss, soil unloading rebound, and variation of soil loss rate along the excavation path caused by shield tunnel construction disturbance on soil displacement. The average errors between theoretical calculation and field monitoring/numerical simulation results for land subsidence and horizontal soil displacement are 5.6% and 0.6%, and 5.5% and 5.3%, respectively. These errors all meet the 20% accuracy requirement specified by engineering experience, demonstrating that the proposed calculation method can effectively address the soil displacement calculation problem induced by the construction of non-horizontal parallel double-line shield tunnels and has good engineering applicability.

Abstract The sandstone is in a state of dry–wet cycle under the repeated action of rainfall, and its mechanical properties are deteriorated to varying degrees, which causes cracks in the sandstone. Therefore, it is of great significance to study the mechanical properties and fracture propagation of sandstone under the action of dry–wet cycles. Currently, there are limited studies using numerical simulation methods to study the fracture extension of rocks under various dry and wet cycling conditions.Therefore, in this paper, the effects of different amounts of dry and wet cycling on the mechanical properties and fracture behavior of sandstone are investigated through uniaxial compression tests and numerical simulations of fracture extension. The findings indicate that the deformation stage of sandstone remains unchanged by the dry–wet cycle. The uniaxial compressive potency and coefficient of restitution gradually diminish as the quantity of cycles rises, while the Poisson's ratio exhibits the opposite trend, and the impact on the mechanical performance of sandstone wanes with cycle increments, and the correlation coefficient surpasses 0.93, signifying a substantial influence of the dry–wet cycle on sandstone's mechanical performances. The discrepancy between the numerical simulation and experimental results is minimal, with a maximum error of only 3.1%, demonstrating the congruence of the simulation and experimental outcomes.The mesoscopic examination of the simulations indicates that the quantity of fractures in the sandstone specimens rises with the escalation of dry–wet cycles, and the steps of analysis linked to crack inception and fracture propagation are accelerated, and the analysis steps from fracture initiation to penetration are also reduced.

Temperature measurement of devices in working state is of critical importance. In this work, GaN-based thermistors and a thermopile are monolithically integrated with a III-nitride blue light-emitting diode (LED) for the on-chip temperature measurement of the LED. The thermistor is based on the temperature-dependent resistance, and the stacked thermopile is based on the Seebeck effect. The measured results indicate that both thermistors and the thermopile can monitor the temperature of the LED sensitively. Due to the advantages of higher sensitivity and easier signal processing, the thermopile is more suitable for real-time monitoring.

This study examines effects of mixing municipal solid waste incineration bottom ash (MSWI-BA) with reddish laterite clay (RLC), evaluating factors such as vertical stress, mixing ratio, curing period, and the addition of lime. A total of 153 direct shear tests were conducted to thoroughly assess the mixture's strength characteristics. Vertical stress levels of 85.5 kPa, 172.4 kPa, and 259.3 kPa were used to simulate varying stress conditions, while mixing ratios of 40 %, 80 %, 100 %, and 120 % were applied to explore potential applications of recycled MSWI-BA with clayey soils. A fast-curing approach was employed, with curing periods of 24, 48, and 72 h, to investigate the time-dependent strength development under controlled conditions. A three-way ANOVA analysis confirmed that mixing ratio, curing period, and vertical stress significantly impacted both peak and residual shear strength. The 100 % MSWI-BA mixture, with or without 1 % lime, exhibited optimal performance, providing the pronounced shear strengths and dilative behavior. The study found that MSWI-BA significantly improved shear strength ratios compared to the RLC, with improvement ratios ranging from 1.439 to 2.460 across stress levels. Additionally, upper and lower bound equations for peak and residual strength ratios were developed, providing predictive tools for mixture design. Cohesion values in the range of 8.3–128.9 kPa and friction angles from 40.6° to 44.1° were achieved, surpassing or matching those reported in similar research. The study employed Bolton's (1986) dilatancy model, finding α values between 0.61 and 0.71, comparable to those in studies of granular materials. These results highlight the effectiveness of adding MSWI-BA and lime in enhancing reddish laterite soil stabilization through both chemical and mechanical means, making it a sustainable and cost-effective approach for civil engineering projects by improving material strength, reusing local soils, recycling waste, and reducing carbon footprints.

Abstract Objective Restless legs syndrome (RLS) stands as a prevalent neurological complication within maintenance hemodialysis (MHD) patients. However, the alterations in cerebral blood flow (CBF) among MHD-RLS patients remain uncharted. Through the utilization of the arterial spin labeling (ASL) technique, we evaluated the fluctuations in CBF within distinct brain regions and analyzed the risk factors for the development of RLS in MHD patients in the context of the clinic. Methods Thirty-one MHD patients with concomitant RLS (MHD-RLS group) and thirty-one non-RLS patients matched based on age, gender, as well as cognitive function (MHD-nRLS group) were included. Through image preprocessing and data analysis, the changes in CBF values in distinct brain regions were obtained, and the CBF values of brain regions with substantial differences between the two groups were correlated with the RLS scores. Furthermore, the differences in baseline data were compared, and through the utilization of multifactorial logistic regression, the independent risk factors for the development of RLS were examined. Results Compared with the MHD-nRLS group, the MHD-RLS group had increased CBF in the right superior temporal gyrus, reduced CBF in the right hippocampus, left middle frontal gyrus, inferior frontal gyrus of right triangle, middle frontal gyrus of left orbit, left precentral gyrus, and left precuneus. Only left precentral gyrus CBF were negatively correlated with RLS scores after correction for dialysis duration(r = -0.436, P = 0.016). Accordingly, multifactorial regression analysis by stepwise method yielded that the left precentral gyrus CBF values(OR: 0.968, 95%CI: 0.944–0.993, P = 0.012) remained an independent risk factor for RLS in MHD patients. In addition, the results showed that hemodialysis duration (OR: 1.055, 95%CI: 1.014–1.098, P = 0.008) and serum iron levels (OR: 0.685, 95%CI: 0.551–0.852, P = 0.001) were also risk factors for the development of RLS. Conclusion Patients afflicted with MHD-RLS exhibit alterations in CBF across several brain regions. Notably, the left precentral gyrus might serve as a pivotal region influencing the onset of RLS among MHD patients. Furthermore, extended hemodialysis duration and a relative insufficiency in serum iron levels independently contribute as risk factors for RLS development within the MHD patient population.

Power Electronics (PE) is pivotal in fostering renewable energy. However, focusing on usage efficiency shifts environmental impacts across its life cycle, leading to excessive resource extraction and waste accumulation. Integrating environmental impact indicators in PE development is crucial for informed design decisions. Nevertheless, the current literature lacks the provision of adequate understanding of the PE product development process for successful ecodesign in PE industry. To effectively tackle the industrial integration aspect of ecodesign that enable functional and system innovation, this study employs empirical research, combining literature analysis with in-depth interviews with PE designers. Requirements for an ecodesign support to integrate ecodesign practices into the Product Development Process (PDP) of PCB-based Power Electronic Components (PECs) is proposed in this paper based on the interview results. These specifications are delineating the necessary working and decision-making steps. Subsequently, a parameterized Functional Block-based Life Cycle Analysis (FB-LCA) model is proposed under the requirement. A five-step methodology is proposed to operationalise the implementation of the FB-LCA model, aiming to supporting the definition of functional structure, configuration of technical solutions and choice of hardware components by considering environmental impacts generated all along life cycle, from manufacturing, transport, use, circularity scenario, end-of-life .The case study of a PCB-based boost converter design is conducted in collaboration with a PE engineer to test the practical implementation of FB-LCA model.

Abstract The continued miniaturization of chips demands highly thermally conductive materials and effective thermal management strategies. Particularly, the high‐field transport of the devices built with 2D materials is limited by self‐heating. Here a systematic control of heat flow in single‐side fluorinated graphene (FG) with varying degrees of fluorination is reported, revealing a superior room‐temperature thermal conductivity as high as 128 W m−1 K−1. Monolayer graphene/FG lateral heterostructures with seamless junctions are approached for device fabrication. Efficient in‐plane heat removal paths from graphene channel to side FG are created, contributing significant reduction of the channel peak temperature and improvement in the current‐carrying capability and power density. Molecular dynamics simulations indicate that the interfacial thermal conductance of the heterostructure is facilitated by the high degree of overlap in the phonon vibrational spectra. The findings offer novel design insights for efficient heat dissipation in micro‐ and nanoelectronic devices.

ContextDespite the recognition of attention deficit hyperactivity disorder (ADHD) as a multifaceted neurodevelopmental disorder, its core causes are still ambiguous. The objective of this study was to explore if the traits of circulating immune cells contribute causally to susceptibility to ADHD.MethodsBy employing a unified GWAS summary data covering 731 immune traits from the GWAS Catalog (accession numbers from GCST0001391 to GCST0002121), our analysis focused on the flow cytometry of lymphocyte clusters, encompassing 3,757 Sardinians, to identify genetically expected immune cells. Furthermore, we obtained summarized GWAS statistics from the Psychiatric Genomics Consortium to evaluate the genetic forecasting of ADHD. The studies employed ADHD2019 (20,183 cases and 35,191 controls from the 2019 GWAS ADHD dataset) and ADHD2022 (38,691 cases and 275,986 controls from the 2022 GWAS ADHD dataset). Through the examination of genome-wide association signals, we identified shared genetic variances between circulating immune cells and ADHD, employing the comprehensive ADHD2022 dataset. We primarily utilized inverse variance weighted (IVW) and weighted median methods in our Mendelian randomization research and sensitivity assessments to evaluate diversity and pleiotropy.ResultsAfter adjusting for false discovery rate (FDR), three distinct immunophenotypes were identified as associated with the risk of ADHD: CD33 in Im MDSC (OR=1.03, CI: 1.01~1.04, P=3.04×10−5, PFDR=0.015), CD8br NKT %T cell (OR=1.08, 95%CI: 1.04~1.12, P=9.33×10−5, PFDR=0.023), and CD8br NKT %lymphocyte (OR=1.08, 95%CI: 1.03~1.12, P=3.59×10−4, PFDR=0.066). Furthermore, ADHD showed no statistical effects on immunophenotypes. It’s worth noting that 20 phenotypes exist where ADHD’s appearance could diminish 85% of immune cells, including FSC-A in myeloid DC (β= -0.278, 95% CI: 0.616~0.931, P=0.008), CD3 in CD45RA- CD4+ (β= -0.233, 95% CI: 0.654~0.960, P=0.017), CD62L- monocyte AC (β=0.227, 95% CI: 0.038~1.518, P=0.019), CD33 in CD33br HLA DR+ CD14dim (β= -0.331, 95% CI: 0.543~0.950, P=0.020), and CD25 in CD39+ resting Treg (β=0.226, 95% CI: 1.522, P=0.022), and FSC-A in monocytes (β= -0.255, 95% CI: 0.621~0.967, P=0.234), among others.ConclusionStudies indicate that the immune system’s response influences the emergence of ADHD. The findings greatly improve our understanding of the interplay between immune responses and ADHD risk, aiding in the development of treatment strategies from an immunological perspective.

Introduction: Frailty has an important impact on the health outcomes of older patients, and frailty screening is recommended as part of perioperative evaluation. The Hospital Frailty Risk Score (HFRS) is a validated tool that highlights frailty risk using 109 International Classification of Diseases, 10th revision (ICD-10) codes. In this study, we aim to compare HFRS to the Charlson Comorbidity Index (CCI) and validate HFRS as a predictor of adverse outcomes in Asian patients admitted to surgical services. Method: A retrospective study of electronic health records (EHR) was undertaken in patients aged 65 years and above who were discharged from surgical services between 1 April 2022 to 31 July 2022. Patients were stratified into low (HFRS 15) risk of frailty. Results: Those at high risk of frailty were older and more likely to be men. They were also likely to have more comorbidities and a higher CCI than those at low risk of frailty. High HFRS scores were associated with an increased risk of adverse outcomes, such as mortality, hospital length of stay (LOS) and 30-day readmission. When used in combination with CCI, there was better prediction of mortality at 90 and 270 days, and 30-day readmission. Conclusion: To our knowledge, this is the first validation of HFRS in Singapore in surgical patients and confirms that high-risk HFRS predicts long LOS (≥7days), increased unplanned hospital readmissions (both 30-day and 270-day) and increased mortality (inpatient, 10-day, 30-day, 90-day, 270-day) compared with those at low risk of frailty.

GH2132 alloy， as one of the representative materials of high-temperature alloys based on Fe-25Ni-15Cr，has become a widely used high-temperature material below 650 ℃ due to its excellent comprehensive properties. At present，the research on GH2132 alloy mainly focuses on three aspects： solid solution heat treatment，solid solution + aging heat treatment，and direct aging heat treatment. This article summarizes recent research on the alloy in these three aspects and analyzes the impact of heat treatment processes on microstructure and properties. During the solid solution treatment process of GH2132 alloy，attention should be paid to the effect of the dissolution of Laves and M3B2 phases on grain size to prevent grain coarsening. The alloy after solid solution exhibits the characteristics of low strength and high plasticity. The formulation of aging treatment process needs to be combined with solid solution treatment process，taking into account the influence of coupling effect and preventing occurrence γ′ → η transform and reduce the reinforcement effect. The microstructure of GH2132 alloy is sensitive to deformation，and the larger the deformation，the lower the aging temperature or shorter the aging time should be chosen.

Objective: This study aimed to explore the effect of different ultrasonic enhancement methods on the quality characteristics of dried lotus root in heat pumps. Methods: The effects of US, CU, and USOD on the drying characteristics, appearance color, microstructure, texture characteristics, nutritional quality and antioxidant characteristics of the lotus root heat pump during the drying process were investigated. Ultrasonic (US), direct contact ultrasound (CU), USOD and heat pump drying (HPD) were treated in series and parallel. The coefficient of variation method was used to comprehensively evaluate the quality of lotus root dried by different ultrasonic enhancement methods. Results: US, CU and USOD treatments had positive effects on the HPD speed of lotus roots. The brightness (L*), polysaccharide content, and hydroxyl radical scavenging rate of lotus root obtained by USOD-HPD were the best, which were 77.98, 51.68 mg/g, and 45.94%, respectively. US-HPD treatment can better retain the content of flavonoids and vitamin C in lotus root. HPD lotus root had the highest polyphenol content and DPPH radical scavenging. The comprehensive evaluation results of the coefficient of variation method showed that the USOD-HPD lotus root had the best quality. Conclusion: The combined drying method of USOD-HPD can effectively improve the quality of lotus root.

Constipation is a common gastrointestinal disease that seriously affects human physical and mental health. Studies have reported that hemp seeds can improve constipation, however the specific mechanism is still unclear. This study investigates that hemp seed (HS) and its water-ethanol extract (HSE) attenuates loperamide-induced constipation in mice. The research results show that: the fecal water content and small intestinal transit rate of mice in the hemp seed group and hemp seed hydroalcoholic extract group were significantly increased compared with MC group, and the first red feces defecation time was significantly shortened; HS and HSE significantly influence serum levels of Gastrin (Gas), motilin (MTL), substance P (SP), and endothelin (ET), potentially mediating their effects on gastrointestinal motility. HS and HSE can improve colon inflammation in constipated mice with H&E staining. Compared with the model of constipation group, the content of short-chain fatty acids in the HS group and HSE group increased significantly. Gut microbiome studies have shown that the structure and abundance of intestinal flora are altered. HS and HSE changed the abundance of Odoribacter, Bacteroide, Lactobacillus and Prevotella. Together, these results suggest that HS have the potential to stimulate the proliferation of beneficial gut microbes and promote intestinal motility, thereby improving gut health and relieving symptoms of constipation.

Introduction Postoperative pain after thoracic surgery impairs patients’ quality of life and increases the incidence of respiratory complications. Optimised analgesia strategies include minimally invasive incisions, regional analgesia and early chest tube removal. However, little is known about the optimal analgesic regimen for uniportal video-assisted thoracoscopic surgery (uVATS).Methods and analysis We will conduct a single-centre, prospective, single-blind, randomised trial. The effects of postoperative analgesia will be tested using thoracic paravertebral block (PVB) in combination with patient-controlled intravenous analgesia (PVB+PCIA), erector spinae plane block (ESPB) in combination with patient-controlled intravenous analgesia (ESPB+PCIA) or PCIA alone; 102 patients undergoing uVATS will be enrolled in this study. Patients will be randomly assigned to the PVB group (30 mL of 0.33% ropivacaine with dexamethasone), ESPB group (40 mL of 0.25% ropivacaine with dexamethasone) or control groups. PCIA with sufentanil will be administered to all patients after surgery. The primary outcome will be total opioid consumption after surgery. Secondary outcomes include postoperative pain score; postoperative chronic pain at rest and during coughing; sensations of touch and pain in the chest wall, non-opioid analgesic consumption; length of stay; ambulation time, the total cost of hospitalisation and long-term postoperative analgesia. Adverse reactions to analgesics and adverse events related to the regional blocks will also be recorded. The statisticians will be blinded to the group allocation. Comparison of the continuous data among the three groups will be performed using a one-way analysis of variance to assess differences among the means.Ethics and dissemination The results will be published in patient education courses, academic conferences and peer-reviewed journals.Trial registration number NCT06016777.

Objectives: The Albumin-Bilirubin (ALBI) score has been widely used to assess the prognosis in patients with cirrhosis and hepatocellular carcinoma. This study aimed to analyze the relationship between ALBI score and all-cause mortality in patients with hepatitis B virus (HBV) infection in general. Methods: Patients aged ≥ 18 years with previous or current HBV infection from the National Health and Nutrition Examination Survey (NHANES) in the United States between 1999 and 2018 were enrolled in this retrospective cohort study. Weight univariate and multivariate Cox regression models were used to assess the relationship between ALBI score and all-cause mortality. The area under the receiver operating characteristic curve (AUC) was utilized to assess the predictive effect of ALBI score for all-cause mortality. Results: A total of 3,666 patients were included, of whom 925 (23.53 %) patients died. Compared with ALBI score ≤ -2.6, HBV-infected patients with ALBI score > -2.6 [hazard ratio (HR) = 1.75; 95 % confidence interval (CI): 1.43–2.14] were corrected with a higher all-cause mortality risk after adjusting for confounders. To summarize the nursing of a liver cancer patient with chemotherapy-induced oral ulcer, and provide reference for clinical practice. Traditional Chinese Medicine mouthwash (30 min per time, four times a day) was administrated for relieving external heat, moistening dryness, and promoting fluid production. After using ginger slice to embrocate local skin, the moxa stick moxibustion at Zusanli and Yongquan acupoints was carried out once a day for 30 minutes each time. The patient's oral ulcer pain decreased and the ulcer surface shrank after treatment. Traditional Chinese Medicine mouthwash therapy combined with moxa stick moxibustion after embrocation using ginger is effective to relieve chemotherapy-induced oral ulcers and improve patient quality of life. Introduction: The study aimed to determine if hepatic steatosis assessed by fatty liver index (FLI) was an independent risk factor for male low testosterone level and whether the FLI was the strongest risk factor for low testosterone level in two different age groups. Methods: Two cross-sectional studies were performed. A total of 3,443 male participants (aged 46–75) were recruited into study A (part of lONgitudinal study (REACTION)). Then a total of 267 male participants (aged 25–45) were recruited into study B. Serum total testosterone (TT) and sex hormone-binding globulin (SHBG) levels, indicators for assessing hepatic steatosis were measured. The Pearson correlation and regression analysis were performed to investigate the risk factors for low testosterone level. Results: The FLI had the strongest negative correlation with serum testosterone in the study A (r = −0.436) and B (r = −0.542). Compared with patients with a FLI lower than 30, the risk for low testosterone level increased by 3.48-fold in subjects with a FLI higher than 60 adjusted for potential risk factors in study A. In study B, the odds ratio of low testosterone level in patients with potential hepatic steatosis was 4.26 (1.57–11.60) after adjusted for age and homeostasis model assessment of insulin resistance (HOMA-IR) and 0.59 (0.14–2.60) after adjusted for age, HOMA-IR, waist circumference, body mass index, and SHBG. Conclusions: FLI was the strongest risk factor for male low testosterone level independent of insulin resistance in male populations of different ages; however, the association can be modulated by SHBG levels in the young. Significance Statement: In the study, FLI was the strongest negative risk factor for low testosterone level in the Chinese adult male population. The results suggested that hepatic steatosis assessed by the FLI was the main risk factor for male low testosterone level, independent of age, insulin resistance, smoking, and drinking status; however, the association of FLI and TT levels can be modulated by SHBG levels. Taken together these findings indicate that clinical physicians should pay more attention to the FLI index and hepatic steatosis, so that they can take advantage of them for assessing the risk of developing of low testosterone level in the male population.

Automated perimetrium segmentation of transvaginal ultrasound images is an important process for computer-aided diagnosis of uterine diseases. However, ultrasound images often contain various structures and textures, and these structures have different shapes, sizes, and contrasts; therefore, accurately segmenting the parametrium region of the uterus in transvaginal uterine ultrasound images is a challenge. Recently, many fully supervised deep learning-based methods have been proposed for the segmentation of transvaginal ultrasound images. Nevertheless, these methods require extensive pixel-level annotation by experienced sonographers. This procedure is expensive and time-consuming. In this paper, we present a bidirectional copy–paste Mamba (BCP-Mamba) semi-supervised model for segmenting the parametrium. The proposed model is based on a bidirectional copy–paste method and incorporates a U-shaped structure model with a visual state space (VSS) module instead of the traditional sampling method. A dataset comprising 1940 transvaginal ultrasound images from Tongji Hospital, Huazhong University of Science and Technology is utilized for training and evaluation. The proposed BCP-Mamba model undergoes comparative analysis with two widely recognized semi-supervised models, BCP-Net and U-Net, across various evaluation metrics including Dice, Jaccard, average surface distance (ASD), and Hausdorff_95. The results indicate the superior performance of the BCP-Mamba semi-supervised model, achieving a Dice coefficient of 86.55%, surpassing both U-Net (80.72%) and BCP-Net (84.63%) models. The Hausdorff_95 of the proposed method is 14.56. In comparison, the counterparts of U-Net and BCP-Net are 23.10 and 21.34, respectively. The experimental findings affirm the efficacy of the proposed semi-supervised learning approach in segmenting transvaginal uterine ultrasound images. The implementation of this model may alleviate the expert workload and facilitate more precise prediction and diagnosis of uterine-related conditions.

During the COVID-19 pandemic, many countries and regions experienced a surge in online learning, but the public complained about and questioned its effectiveness. One of the most important reasons for this was the inadequate metacognitive abilities of adolescents. Studies in learning sciences have identified various inefficient learning behaviors among students in online learning, including help abuse, help avoidance, and wheel spinning; all closely related to metacognition. Despite concerns about ecological validity, researchers in psychology have proposed the agenda-based regulation framework, the COPES model, and MAPS model, which may help explain the inefficient learning behaviors among adolescents in online learning. Future studies should aim to verify these theoretical frameworks within the context of online learning and elucidate the causes of inefficient learning behaviors; the design and optimization of online learning systems should be informed by theories in cognitive psychology.

Abstract Objective To explore the clinical features of renal damage related to pregnancy and pregnancy after chronic kidney disease (CKD), providing clinical evidence for the relationship between renal damage and pregnancy. Methods A retrospective analysis was performed on patients admitted to our hospital between March 2013 and February 2021 who had both pregnancy and kidney damage. The study collected pathology results from renal biopsies, 24-hour urinary protein quantity, albumin (Alb), serum creatinine (Scr), blood lipids, coagulation function, blood routine, and other indicators during and after pregnancy. Results This study included 82 cases, with 48 cases in the pregnancy-related renal damage group. Thirty-four cases were in the post-CKD pregnancy group. Of the patients, 30 cases (88.24%) had CKD stage 1–2. Results showed better pregnancy and fetal outcomes in the post-CKD pregnancy group compared to the pregnancy-related renal damage group (Ρ was 0.029 and 0.036, respectively). Renal biopsy pathology revealed that 16 cases (33.33%) in the pregnancy-related renal damage group mainly had focal segmental glomerulosclerosis (FSGS), while the post-CKD pregnancy group was dominated by 14 cases (43.75%) of IgA nephropathy. The first blood test indicators revealed that the pregnancy-related renal damage group had lower estimated glomerular filtration (eGFR) and Alb levels compared to the post-CKD pregnancy group (Ρ was 0.003 and 0.000, respectively). Additionally, 24-hour urinary protein quantity, total cholesterol (Tch), triglyceride (TG), and platelet (PLT) counts were higher in the pregnancy-related renal damage group compared to the post-CKD pregnancy group (Ρ was 0.005, 0.001, 0.008, and 0.031, respectively). The abnormal rate of Scr during pregnancy was 41.67% (20/48) in the pregnancy-related renal damage group and 17.39% (4/23) in the post-CKD pregnancy group, with a statistically significant difference (Ρ was 0.043). Conclusion The pregnancy-related renal damage group is mainly associated with FSGS, while the post-CKD pregnancy group is characterized by IgA nephropathy. Patients with CKD1-2 can have a successful pregnancy after achieving good control of eGFR, albumin, 24-hour urinary protein quantity and other indicators, resulting in better pregnancy and fetal outcomes. Abnormal Scr levels during pregnancy of pregnancy-related renal damage can be improved within 3 months after delivery.

Abstract Chronic inflammation is an important pathway for obesity to harm health, the aggravation of chronic inflammation occurs without clinical symptoms. BMI is closely related to chronic inflammation, and it is a predictive factor of chronic inflammation, but the following questions remain unanswered: Are the effects of chronic inflammation on different BMI intervals consistent? Are the effects of BMI on chronic inflammation consistent between male and female? This study aimed to explore the threshold effect, and sex characteristics of the relationship between chronic inflammation and BMI. Methods: People with normal weight, overweight, and obesity were selected as subjects for cross-sectional study. BMI, hs-CRP, adiponectin and irisin was tested. Multiple regression analysis and generalized additive models were used to examine the association between hs-CRP and BMI. Results: 119 adults were recruited (normal weight: n = 30, 28.1 ± 7.65 years, BMI: 22.04 ± 1.55; overweight: n = 29, 27.45 ± 7.47 years, BMI: 26.11 ± 1.22; and obesity: n = 60, 28.82 ± 6.05 years, BMI: 33.68 ± 3.57). After adjusting for age and sex, BMI was found to be positively associated with the chronic inflammatory marker hs-CRP (β = 0.45; P 24.6 (β = 0.54; P 0.05). The pro-inflammatory effect caused by BMI increase in female (β = 0.56; P 0.05). Conclusions: BMI has a threshold effect on chronic inflammation, BMI greater than 24.3 is positively correlated with hs-CRP. BMI in 18.5–24.3 is not correlated with hs-CRP. Furthermore, when the BMI greater than 33, hs-CRP is not positively correlated with BMI in male, whereas the pro-inflammatory effect of BMI increase becomes greater in female. Highlights: • BMI has a threshold effect on chronic inflammation. BMI in 18.5–24.3 is not correlated with chronic inflammation, and BMI greater than 24.3 is positively correlated with chronic inflammation. • The pro-inflammatory effect caused by BMI increase in female is higher than that in male. In particular, when the BMI is greater than 33, chronic inflammation is not positively correlated with BMI in male, whereas the pro-inflammatory effect of BMI increase becomes greater in female.

Objective To investigate the effect of Traditional Chinese Medicine(TCM) nursing technology combined with health education on gastrointestinal adverse reactions during chemotherapy in elderly patients with cancer. Methods Totally84 elderly patients with cancer who received chemotherapy from March 2021 to July 2022 were included and divided into two groups by the random number table method. The control group (n=42) received routine nursing during treatment, while the observation group (n=42) received TCM characteristic nursing technology combined with health education. The gastrointestinal adverse reactions during chemotherapy were observed and analyzed. The psychological state of patients was measured by Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). Results The incidences of constipation, loss of appetite, nausea and vomiting in the observation group were lower than those in the control group(P＜0. 05). Scores of SAS and SDS in the observation group were lower than those in the control group after implementation of nursing interventions(P＜0. 01). Conclusion The combination of TCM characteristic nursing techniques and health education has a positive impact on reducing gastrointestinal adverse reactions, and can also alleviate the negative emotions of patients with chemotherapy for cancer. Under the background of global carbon neutrality, soil carbon accumulation and transformation become a hot topic in academic circles. In the process of land reclamation and restoration, the change and its mechanism of soil carbon pool in coal mining subsidence area under the influence of groundwater are complicated. The study of carbon dynamics in reclaimed soil is an important basis for a scientific restoration of reclaimed soil ecosystem. In this study, the age chronosequense approach and the location-monitoring method were used to study the change of soil carbon pool after the reclamation of coal mining subsidence area in the area with higher level of underground water. Soil samples from the study areas were collected twice, respectively in 2012 and 2020. The content of soil total carbon (CTC, total organic carbon (CTOC), total inorganic carbon (CTIC) were measured, and soil carbon storage of TC pool (STC), TOC pool (STOC), TIC pool (STIC) and rates of carbon sequestration were calculated. The soil properties (i.e., soil bulk density, compaction, porosity and soil structure, etc.) associated with the change of soil carbon were also analyzed simultaneously. The results show that: ① the STC, STOC and STIC of soil in non-subsidence area (CK) were 268.34, 81.63, 186.70 t/hm2 respectively. Land subsidence caused the decrease of STC and STIC, but with no significant change of STOC. ② A quadratic equation curve was shown in the change of soil carbon density and carbon storage in the restoration process of reclaimed soil, the CTC and CTIC of 0−20 cm layer reaching the maximum in 14−18 years after reclamation, and exceeding CK. The STOC could exceed CK in 13−14 years of reclamation. ③ There was no significant difference in carbon density and carbon pool between layers in 0−40 cm of newly reclaimed soil. In the process of restoration after reclamation, the largest increase of carbon density and carbon storage was found in the surface layer (0−10 cm), and there was no regular variation of soil CTIC in the profile for study. ④ The soil CTOC, STOC and CTC were positively correlated with soil porosity, soil clay, fractal dimension (D) and content of aggregate larger than 3.2 mm, but negatively correlated with bulk density, compactness, pH and aggregate content of 2−0.50 mm. There was great potential for carbon sequestration in the reclaimed soil. Soil moisture affected the carbon fixation and transformation of reclaimed soil by influencing soil aeration, aggregate formation and particle change in the area with higher level of underground water. The cultivation could improve soil structure and ventilation condition, promote the growth of plants and soil microorganisms, increase the source of soil organic carbon, and promote the accumulation of reclaimed soil organic carbon by ploughing, crushing, applying organic fertilizer and water level adjustment.

Nuclear medicine is a novel technique of theranostics that uses radiopharmaceuticals. Nuclear medicine dated back to a century ago. Owing to the development of nuclear medicine devices and radiopharmaceuticals, increasing clinical demand, government support, and global cooperations, nuclear medicine has witnessed a rapid development. Nowadays, nuclear medicine has played an important and unique role in diagnosis and therapy. There are opportunities and challenges in future development of nuclear medicine, and more progress will be made in nuclear medicine imaging, radionuclide therapy, and artificial intelligence.

In this study, magnetic resonance imaging (MRI) based on a deep learning algorithm was used to evaluate the clinical effect of the small-incision approach in treating proximal tibial fractures. Super-resolution reconstruction (SRR) algorithm was used to reconstruct MRI images for analysis and comparison. The research objects were 40 patients with proximal tibial fractures. According to the random number method, patients were divided into a small-incision approach group (22 cases) and an ordinary approach group (18 cases). The peak signal-to-noise ratio (PSNR) and the structural similarity index (SSIM) of the MRI images before and after the reconstruction of the two groups were analyzed. The operative time, intraoperative blood loss, complete weight-bearing time, complete healing time, knee range of motion, and knee function of the two treatments were compared. The results showed that after SRR, the PSNR and SSIM of MRI images were 35.28 and 0.826 dB, respectively, so the MRI image display effect was better. The operation time in the small-incision approach group was 84.93 min, which was significantly shorter than that in the common approach group, and the intraoperative blood loss was 219.95 mL, which was significantly shorter than that in the common approach group (P < 0.05). The complete weight-bearing time and complete healing time in the small-incision approach group were 14.75 and 16.79 weeks, respectively, which were significantly shorter than those in the ordinary approach group (P < 0.05). The half-year knee range of motion and 1-year knee range of motion in the small-incision approach group were 118.27° and 128.72°, respectively, which were significantly higher than those in the conventional approach group (P < 0.05). After 6 months of treatment, the rate of good treatment was 86.36% in the small-incision approach group and 77.78% in the ordinary approach group. After 1 year of treatment, the rate of excellent and good treatment was 90.91% in the small-incision approach group and 83.33% in the ordinary approach group. The rate of good treatment for half a year and 1 year in the small incision group was significantly higher than that in the common approach group (P < 0.05). In conclusion, MRI image based on deep learning algorithm has a high resolution, good display effect, and high application value. The small-incision approach can be applied to the treatment of proximal tibial fractures, which showed good therapeutic effects and a high positive clinical application value.

Abstract Obesity-related chronic inflammation is closely related to the ability of immune cells to adapt to the body’s needs, research has shown that excess FAs can further activate pro-inflammatory transcription factors in the nucleus by interacting with various receptors such as CD36 and TLR4, thereby affecting the inflammatory state of cells. However, how the profile of various fatty acids in the blood of obese individuals is associated with chronic inflammation remains unclear. Objective The biomarkers associated with obesity were identified from 40 fatty acids (FAs) in the blood, and analyze the relationship between the biomarkers and chronic inflammation. Furthermore, by analyzing the difference in the expression of CD36, TLR4 and NF-κB p65 in peripheral blood mononuclear cells (PBMC) between obese and standard weight people, understand that immunophenotype PBMC is associated with chronic inflammation. Methods This study is a cross-sectional study. Participants were recruited from the Yangzhou Lipan weight loss training camp from May 2020 to July 2020. The sample size was 52 individuals, including 25 in the normal weight group and 27 in the obesity group. Individuals with obesity and controls of normal weight were recruited to identify biomarkers associated with obesity from 40 fatty acids in the blood; correlation analysis was conducted between the screened potential biomarkers FAs and the chronic inflammation index hs-CRP to identify FA biomarkers associated with chronic inflammation. Changes in the fatty acid receptor CD36, inflammatory receptor TLR4, and inflammatory nuclear transcription factor NF-κB p65 in PBMC subsets were used to further test the relationship between fatty acids and the inflammatory state in individuals with obesity. Results 23 potential FA biomarkers for obesity were screened, eleven of the potential obesity biomarkers were also significantly related to hs-CRP. Compared to the control group, in monocytes the obesity group expressed higher TLR4, CD36, and NF-κB p65 in lymphocytes, the obesity group expressed higher TLR4 and CD36; and in granulocytes the obesity group expressed higher CD36. Conclusion Blood FAs are associated with obesity and are associated with chronic inflammation through increased CD36, TLR4, and NF-κB p65 in monocytes.

Abstract Aim To explore the effect of information‐motivation‐behavioural skills (IMB) model on psychological and emotional state of caesarean section (CS) women. Design Control‐group patients got routine nursing during the preoperative, intraoperative and postoperative periods. Observation‐group patients received routine nursing combined with the IMB model that included specific informational, motivational and behavioural interventions. Methods Pregnant women (n = 102) who underwent CS in Hangzhou Women's Hospital were randomly divided into Control and Observation‐groups (n = 51 each). Solar radiation prediction research is a key area of interest in the realm of solar energy utilization and has garnered significant attention in recent times. In order to realize accurate prediction of solar radiation and make solar radiation prediction better serve photovoltaic (PV) power generation, this study proposes a solar radiation prediction method based on sequence model, which integrates two kinds of neural networks, namely, temporal convolutional network (TCN) and neural basis expansion analysis (N-BEATS). First, the dataset is preprocessed using Pearson’s correlation coefficient, outlier detection, and normalized to obtain valid and relevant data; second, the features of TCN feature extraction and N-BEATS flexible extension are integrated to predict the solar radiation; then, the model’s hyperparameters are fine-tuned using the grid search algorithm to ensure precise predictions; and last, the correctness of the method is verified by comparing the error metrics and the running time. Empirical findings indicate that the TCN-N-BEATS sequence model has high prediction accuracy and short time overhead, and it has certain application value in solar radiation prediction, and the model could offer valuable insights for predicting solar radiation.

In order to accurately detect the photovoltaic energy storage unit charge state, this paper selects the parameter charge state as the detection quantity in the equivalent model, establishes the PSO-ELM method to detect the charge state of photovoltaic energy storage unit, optimizes the limit learning machine network using the particle swarm optimization algorithm, and improves the problems such as redundancy of neurons in the implicit layer of the limit learning machine and the poor ability to identify the unknown input parameter, so as to increase the detection accuracy of the PSO-ELM method to improve the detection accuracy of photovoltaic energy storage unit charge state. The relative error between the method established in this paper and the results of the PV storage unit charge state detected by the definition method in the charging state is kept within ±1.9%, and the detection accuracy of the improved method in the dynamic working condition can reach about 97%. The PSO-ELM method established in this paper can accurately detect the charge state of PV energy storage units under various conditions, as demonstrated experimentally.

The special operation mode of high-speed railway will generate a large amount of harmonics. The harmonics will be transmitted to the 220kV side high-voltage cables through the traction transformer, and harmonic voltages will be generated on the cable sheath, threatening the safe operation of the cables. In this paper, on the basis of the measured harmonic data of the traction substation, a harmonic source model based on the measured data is established, which solves the complex problem of harmonic source modeling and can accurately reflect the actual operating conditions of high-speed trains. On this basis, the harmonic voltage characteristics of the cable sheath on the 220kV side of the high-speed railway under the three working conditions of traction, regenerative braking and inertia are simulated and calculated, and the effects of cable length, cable grounding method, and harmonic current parameters on the harmonic voltage are explored. Affect the law. The results show that under different operation modes of high-speed rail, the harmonic voltage of the sheath far exceeds the normal operation of the power frequency, and the maximum value of the harmonic voltage of the cable sheath can reach 454V under the traction condition; Under working conditions, the overvoltage of the metal sheath is greatly affected by the grounding method of the cable, the length of the cable and the harmonic current content. The above research results have important reference significance for the safe operation of electrified railways.

Introduction Postoperative laryngopharyngeal discomfort after extubation can lead to severe throat pain, dysphagia, or postoperative tongue oedema. Possible mechanisms include increased oral pressure, obstruction of venous and lymphatic return in the neck, and increased capillary hydrostatic pressure, which leads to oedema of the tongue and upper airway. However, real-time monitoring indicators of anaesthesia are lacking. Therefore, we designed this study to accurately measure the contact force of the tracheal tube on the tongue in different surgical positions during general anaesthesia.Methods and analysis This prospective single-centre observational study will enrol 54 patients undergoing elective surgery under general anaesthesia for>2 hours with endotracheal tube application from 1 July 2023 to 30 June 2024. Patients will be divided into the supine (Supine group) and high-risk (Flexion group) groups. Dynamic changes in the contact force between the tracheal tube and tongue will be measured using T-Scan technology. All patients will be followed up for 7 days postoperatively. The primary endpoint is postoperative laryngopharyngeal discomfort. Secondary outcomes include the time to the first successful recovery of oral intake of fluids and solid food, and airway-related events.Ethics and dissemination Ethical approval was obtained from the Ethics Committee of Clinical Research of China-Japan Friendship Hospital (2023-KY-219, approved on 14 September 2023). Informed consent will be obtained during anaesthesia evaluation. This study aims to explore the characteristics of the contact force on the tongue caused by endotracheal intubation in different surgical positions and to provide a better understanding of the risk factors and prevention of postoperative laryngopharyngeal discomfort. The findings of this study will be presented at our hospital, reported on ClinicalTrials.gov, and published in peer-reviewed journals.Trial registration number NCT05987293.

Background: Bone marrow mesenchymal stem cells (BMMSCs) possess the ability of adipogenic differentiation. Icariin (ICA) is a prenylated flavonol glycoside with diverse pharmacological activities and has been reported to promote osteogenic differentiation of BMMSCs. Nevertheless, the effects of ICA on BMMSC adipogenic differentiation into brown fat cells are still unclear. This study aimed to explore the effects and mechanistic basis of ICA on the differentiation of BMMSCs into brown fat cells. Methods: Oil Red-O staining assay was applied to detect the adipogenic differentiation of BMMSCs after induction. RT-qPCR and Western blot were conducted to detect the expression of lipogenic markers PPARγ and FABP4 as well as the brown fat biomarkers BMP7, PGC-1α, and UCP1 in BMMSCs. Moreover, phosphodiesterase-5A (PDE5A) expression in BMMSCs treated with ICA was measured by RT-qPCR and Western blot. Results: ICA promoted the adipogenic differentiation of BMMSCs and increased the expression levels of lipogenic markers PPARγ and FABP4 and the brown fat biomarkers BMP7, PGC-1α, and UCP1 during the adipogenic differentiation of BMMSCs. Furthermore, PDE5A was identified as a target of ICA, and its expression was reduced by ICA treatment. Moreover, PDE5A inhibition enhanced BMP7, PGC-1α, and UCP1 levels in BMMSCs. Additionally, overexpression of PDE5A notably reversed the effects of ICA in the differentiation of BMMSCs into brown fat cells. Conclusion: ICA induces the differentiation of BMMSCs into brown fat cells via PDE5A inhibition, highlighting the therapeutic value of ICA for treating obesity-related diseases.

Abstract Background Studies have shown that sperm-borne microRNAs (miRNAs) are involved in mammalian preimplantation embryonic development. In humans, spermatozoan miR-34c levels are correlated with in vitro fertilization outcomes, such as embryo quality and the clinical pregnancy and live birth rates. In rabbits and cows, miR-34c improves the developmental competence of embryos generated by somatic cell nuclear transfer. However, the mechanisms underlying the regulation of embryonic development by miR-34c remain unknown. Methods Female C57BL/6 mice (6–8 weeks old) were superovulated, and pronucleated zygotes were collected and microinjected with an miR-34c inhibitor or a negative-control RNA. The embryonic development of the microinjected zygotes was evaluated, and the messenger RNA (mRNA) expression profiles of the embryos at the two-cell, four-cell and blastocyst stages (five embryos per group) were determined by RNA sequencing analysis. Gene expression levels were verified by reverse transcription–quantitative polymerase chain reaction. Cluster analysis and heat map visualization were performed to detect differentially expressed mRNAs. Pathway and process enrichment analyses were performed using ontology resources. Differentially expressed mRNAs were systematically analyzed using the Search Tool for the Retrieval of Interacting Genes/Proteins database to determine their biological functions. Results Embryonic developmental potential was significantly reduced in zygotes microinjected with the miR-34c inhibitor compared with those microinjected with a negative-control RNA. Two-cell stage embryos microinjected with an miR-34c inhibitor presented altered transcriptomic profiles, with upregulated expression of maternal miR-34c target mRNAs and classical maternal mRNAs. Differentially expressed transcripts were mainly of genes associated with lipid metabolism and cellular membrane function at the two-cell stage, with cell-cycle phase transition and energy metabolism at the four-cell stage; and with vesicle organization, lipid biosynthetic process and endomembrane system organization at the blastocyst stage. We also showed that genes related to preimplantation embryonic development, including Alkbh4, Sp1, Mapk14, Sin3a, Sdc1 and Laptm4b, were significantly downregulated after microinjection of an miR-34c inhibitor. Conclusions Sperm-borne miR-34c may regulate preimplantation embryonic development by affecting multiple biological processes, such as maternal mRNA degradation, cellular metabolism, cell proliferation and blastocyst implantation. Our data demonstrate the importance of sperm-derived miRNAs in the development of preimplantation embryos.

Abstract Sodium–glucose cotransporter 2 inhibitors (SGLT2i) are recommended for type 2 diabetes mellitus patients with impaired renal function, but the actual situation of SGLT2i using is unclear. Therefore, in this real-world study, we analyzed the treatment scheme and clinical characteristics of SGLT2i in patients with diabetic kidney disease (DKD). We included DKD patients hospitalized in the First Affiliated Hospital of Zhengzhou University from October 2017 to March 2020. The Apriori algorithm of association rules was used to analysis treatment scheme prescribing SGLT2i and other different combinations of hypoglycemic drugs. SGLT2i was used in 781 (12.3%) of 6336 DKD patients, both number and proportion of patients using SGLT2i increased from 2017 to 2020 (1.9% to 33%). Nighty-eight percent of all DKD patients using SGLT2i were combined with other glucose-lowering agents, and insulin, metformin and alpha-glucosidase inhibitors are most commonly used in combination with hypoglycemic drugs. Multivariate analysis showed that compared with non-SGLT2i group, patients using SGLT2i were associated with younger age, higher BMI, higher HbA1c, preserved kidney function, dyslipidemia and combined with ACEI/ARB and statins. In this real-world study, use of SGLT2i in DKD patients is still low. Most patients performed younger age and in the early stages of chronic kidney disease with poor glycemic control. Clinical inertia should be overcome to fully exert the cardiorenal protective effects of SGLT2 inhibitors, with attention to rational drug use.

China’s increasing engagement in international governance has had significant ramifications in international rule-making and institutional build-up. The post-COVID-19 era has seen a rise of populism in China, as well as an elevation of China’s significance in international governance. This article offers a new perspective on China’s growing influence in the midst of resurgent populism, by focusing on three case scenarios: defending state liability in public health crisis lawsuits, championing financial multilateralism, and an emerging digital currency landscape. The argument made here is that China’s status as a rising power has led it to advocate for international rules, standards, and institutions in a de-Americanised and anti-populist manner.

Objective To explore the molecular mechanism underlying miR-9500 regulating the migration and invasion of lung adenocarcinoma cells by targeting SMAD2. Methods The core target genes of miR-9500 were screened out by bioinformatics analysis, and their GO function analysis, KEGG signaling pathway enrichment, and survival analysis were performed. The targeted binding sites between miR-9500 and SMAD2 were predicted, and the direct targeting relationship between miR-9500 and SMAD2 was verified by dual-luciferase reporter assay. qRT-PCR and Western blot were used to assess the effect of miR-9500 on the mRNA and protein expression levels of SMAD2. Wound healing assay, Transwell assay, and Matrigel invasion assay were used to determine the effect of miR-9500 on the migration and invasion of lung adenocarcinoma cells. Results The core target genes of miR-9500 were mainly enriched in the cancer pathway, TGF-β signaling pathway, and focal adhesion. However, only the expression levels of VAMP2, SMAD2, and RXRA among the top 10 core target genes were significantly correlated with the overall survival of patients with lung adenocarcinoma. miR-9500 targeted SMAD2 and down-regulated the expression levels of SMAD2, and overexpression of miR-9500 significantly inhibited the migration and invasion of lung adenocarcinoma cells and markedly decreased the expression levels of MMP2 and MMP9. Conclusion miR-9500 can inhibit the migration and invasion of lung adenocarcinoma cells by targeting SMAD2, which may play an important role in the tumorigenesis and development of lung adenocarcinoma as a tumor suppressor.

Canola expeller (CE) contains ~200 g/kg residual oil, but also fiber that impairs nutrient digestibility in weaned pigs. To study if feed enzymes increase digestibility, six diets containing either the basal or two CE samples mixed in at 250 g/kg (CE-A or CE-B) were formulated with or without a multi-enzyme blend containing cellulase, xylanase, glucanase, amylase, protease, invertase, and pectinase. The basal diet containing 620 g/kg wheat and 150 g/kg barley served as control. Twelve ileal-cannulated barrows (9–15 kg) were fed the six diets in a replicated 6 (pigs) × 3 (periods) Youden square. Ileal digestibility of gross energy and amino acids was 5% greater for basal than CE diets without differences between CE samples. Diet energy values were 4% greater for CE than basal diets due to residual oil in CE. Inclusion of the multi-enzyme blend increased total tract digestibility of energy of the basal but not CE diets by 2%. Net energy value was greater for CE-A than CE-B because CE-A contained more residual oil. In conclusion, feeding 250 g/kg CE increased diet energy values; thus, CE can substitute added fat in weaned pig diets. Feeding the multi-enzyme blend increased the energy digestibility of wheat and barley-based diets fed to weaned pigs. However, research is needed to identify enzyme combinations that increase the nutrient digestibility of CE.

Heavy metal pollution in soil is becoming more and more serious. LIBS is one of the most promising technologies for rapid detection of heavy metal contamination in soil. However, due to the wide variety of soils and complex matrices, accurate quantification remains a challenge. In total, 451 soil samples were prepared and detected by the portable detector of LIBS, which were divided into six categories based on the compactness of the soil pellets, and a separate quantitative model for each type of soil sample was used for quantitative analysis by external standard method. It did not need a lot of data to train the model, and only a small number of calibration samples could be used for quantitative analysis of a large number of samples. The results showed that 78 standard samples and 334 collected samples were quantitatively analyzed by 39 standard samples. Compared with the standard value, the correlation coefficients were all above 0.95. A comparative experiment indicated that the portable LIBS system combined with soil classification and calibration methods can achieve fast and accurate quantitative detection.
