13. Empirical Bayes estimation of posterior probabilities of enrichment

Abstract

To interpret differentially expressed genes or other discovered features, researchers conduct hypothesis tests to determine which biological categories such as those of the Gene Ontology (GO) are enriched in the sense of having differential representation among the discovered features. We study application of better estimators of the local false discovery rate (LFDR), a probability that the biological category has equivalent representation among the preselected features. We identified three promising estimators of the LFDR for detecting differential representation: a semiparametric estimator (SPE), a normalized maximum likelihood estimator (NMLE), and a maximum likelihood estimator (MLE). We found that the MLE performs at least as well as the SPE for on the order of 100 of GO categories even when the ideal number of components in its underlying mixture model is unknown. However, the MLE is unreliable when the number of GO categories is small compared to the number of PMM components. Thus, if the number of categories is on the order of 10, the SPE is a more reliable LFDR estimator. The NMLE depends not only on the data but also on a specified value of the prior probability of differential representation. It is therefore an appropriate LFDR estimator only when the number of GO categories is too small for application of the other methods. For enrichment detection, we recommend estimating the LFDR by the MLE given at least a medium number (~100) of GO categories, by the SPE given a small number of GO categories (~10), and by the NMLE given a very small number (~1) of GO categories., Comment: exhaustive revision of Zhenyu Yang and David R. Abstract

Supplemental Digital Content is available in the text, Background: Currently, the nonsmall-cell lung cancer (NSCLC) is a worldwide disease, which has very poor influence on life quality, whereas the therapeutic effects of drugs for it are not satisfactory. The aim of our PRISMA-compliant systematic review and meta-analysis was to compare the efficacy and safety of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) with Taxanes in patients with lung tumors. Methods: We collected randomized controlled trials (RCTs) of EGFR-TKIs (gefitinib, erlotinib) versus Taxanes (docetaxel, paclitaxel) for the treatment of NSCLC by searching PubMed, EMbase, and the Cochrane library databases until April, 2016. The extracted data on progression-free survival (PFS), progression-free survival rate (PFSR), overall survival (OS), overall survival rate (OSR), objective response rate (ORR), disease control rate (DCR), quality of life (QoL), and adverse event rates (AEs) were pooled. Disease-relevant outcomes were evaluated using RevMan 5.3.5 software and STATA 13.0 software. Results: We systematically searched 26 RCTs involving 11,676 patients. The results showed that EGFR-TKIs could significantly prolong PFS (hazard ratio [HR] = 0.78, 95% confidence interval [CI]: 0.66–0.92) and PFSR (risk ratio [RR] = 2.10, 95% CI: 1.17–3.77), and improve ORR (RR = 1.62, 95% CI: 1.38–1.91) and QoL. EGFR-TKIs had similar therapeutic effects to taxanes with respect to OS (HR = 1.00, 95% CI: 0.95–1.05) and OSR (RR = 1.03, 95% CI: 0.94–1.14). Furthermore, there were no significant differences between them in DCR (RR = 0.95, 95% CI: 0.88–1.03). Finally, EGFR-TKIs were superior to taxanes in most of all grades or grade ≥3 AEs. Conclusion: In the efficacy and safety evaluation, EGFR-TKIs had an advantage in the treatment of NSCLC, especially for patients with EGFR mutation-positive. 29. Longitudinal data analysis using generalized linear models

Abstract

In this work we examine various conditions under which the usual asymptotic results (i.e. the weak consistency, the asymptotic normality and the strong consistency) hold for the regressor parameter beta which arises in a linear model (Chapter 2), a generalized linea model (GLM) with a fully specified likelihood (Chapter 3) or as a root of the generalized estimating equation (GEE) associated with a sequence of longitudinal observations (Chapter 4). Our main references for each of these chapters are [12], [9], respectively [20]. We provide detailed proofs of the results found in the above-mentioned references, and we extend the results of [9] to, the case of stochastic regressors (Section 3.4). Finally, in Chapter 5, we identify a fundamental mistake appearing in the recent article [4], which examines the strong consistency of the regressor parameter beta in a GLM for which the likelihood of the density is not specified. In Section 5.2, we give a correction to the main theorem of [4], as well as some new results concerning the weak consistency and asymptotic normality of beta.

Abstract

Introduction: The use of metformin for treating gestational diabetes mellitus (GDM) remains controversial because it can pass through the placenta. This meta-analysis aimed to compare the effects of metformin and insulin on maternal and neonatal outcomes in patients with GDM., Methods: We conducted a comprehensive search of the PubMed, Embase, and Cochrane Library databases, focusing on randomized controlled trials (RCTs) that evaluated the impacts of metformin and insulin on both maternal and neonatal outcomes in patients with GDM., Results: Twenty-four RCTs involving 4934 patients with GDM were included in this meta-analysis. Compared with insulin, metformin demonstrated a significant reduction in the risks of preeclampsia (RR 0.61, 95% CI 0.48 to 0.78, p  < .0001), induction of labor (RR 0.90, 95% CI 0.82 to 0.98, p  = .02), cesarean delivery (RR 0.91, 95% CI 0.85 to 0.98, p  = .01), macrosomia (RR 0.67, 95% CI 0.53 to 0.83, p  = .0004), neonatal intensive care unit (NICU) admission (RR 0.75, 95% CI 0.66 to 0.86, p  < .0001), neonatal hypoglycemia (RR 0.55, 95% CI 0.48 to 0.63, p  < .00001), and large for gestational age (LGA) (RR 0.80, 95% CI 0.68 to 0.94, p  = .007). Conversely, metformin showed no significant impact on gestational hypertension (RR 0.84, 95% CI 0.67 to 1.06, p  = .15), spontaneous vaginal delivery (RR 1.13, 95% CI 1.00 to 1.08, p  = .05), emergency cesarean section (RR 0.94, 95% CI 0.77 to 1.16, p  = .58), shoulder dystocia (RR 0.65, 95% CI 0.31 to 1.39, p  = .27), premature birth (RR 0. 92, 95% CI 0.61 to 1.39, p  = .69), polyhydramnios (RR 1.11, 95% CI 0.54 to 2.30, p  = .77), birth trauma (RR 0.87, 95% CI 0.54 to 1.39, p  = .56), 5-min Apgar score < 7 (RR 1.13, 95% CI 0.76 to 1.68, p  = .55), small for gestational age (SGA) (RR 0.93, 95% CI 0.71 to 1.22, p  = .62), respiratory distress syndrome (RDS) (RR 0.74, 95% CI 0.50 to 1.08, p  = .11), jaundice (RR 1.09, 95% CI 0.95 to 1.25, p  = .24) or birth defects (RR 0.80, 95% CI 0.37 to 1.74, p  = .57)., Conclusions: The findings suggest that metformin can reduce the risk of certain maternal and neonatal outcomes compared with insulin therapy for GDM. However, long-term follow-up studies of patients with GDM taking metformin and their offspring are warranted to provide further evidence.

Abstract

Objective: To assess the long-term effectiveness of Huangqi ( Radix Astragali Mongolici , HQ)-based Traditional Chinese Medicine (TCM) in the treatment of diabetic peripheral neuropathy (DPN)., Methods: Nine databases were searched to retrieve available randomized controlled trials that compared HQ-based TCM and Western Medicines in the treatment of DPN. The methodological quality of the included studies was assessed using the Cochrane bias risk tool, and RevMan 5.4 was used for data analysis. The effect estimates of interest were risk ratio ( RR ), mean difference ( MD ) or standardized mean difference ( SMD ) with 95% confidence interval ( CI )., Results: The results from 48 available studies assessing 3759 patients demonstrated that cases administered HQ-based TCM [ RR = 1.30, 95% CI (1.21, 1.40), P < 0.000 01] or HQ-based TCM combined with Western Medicines [ RR = 1.25, 95% CI (1.19, 1.31), P < 0.000 01] exhibited higher total efficacy rates than individuals who received Western Medicine alone. The results showed that the HQ-based TCM group had decreased Toronto Clinical Scoring System scores [ MD =－1.50, 95% CI (－1.83, －1.17), P < 0.000 01], and reduced serum interleukin 6 [S MD = －0.57, 95% CI (－0.87, －0.27), P = 0.0002] and tumor necrosis factors-α levels [S MD = －0.60, 95% CI (－0.95, －0.25), P = 0.0009]. In addition, both HQ-based TCM and HQ-based TCM combined with Western Medicine increased nerve conduction velocity and decreased glycaemia compared with Western Medicine alone. In terms of blood lipids, oxidative stress and adverse drug reactions, there were no significant differences between the HQ-based TCM groups and the Western Medicine control group., Conclusion: The current Meta-analysis revealed that HQ-based TCM yields higher efficacy and safety than Western Medicine alone for the treatment of DPN, although further well-designed RCTs are required to validate these findings.
