Abstract

The self-expandable metallic stents have been widely used in tracheobronchial obstruction or fistulation, including the J-shaped and Y-shaped stents, named after the shape of the branch-stem junction of the stent. However, there is scarce data on the mechanical performance of these tracheobronchial stents, which is essential for optimal stent implantation. In this work, eight self-expandable metallic tracheobronchial stents in three types (i.e., straight, J-shaped, and Y-shaped), with or without cover, were characterized. The compression resistance of the stems was investigated through both compression and indentation tests. The bending resistance of the branches in the J-shaped and Y-shaped stents was assessed through the bending test. Our results demonstrated that the covered stents exhibited a significantly higher compression resistance and bending resistance than the uncovered ones. The branches had a minimal impact on the compression resistance of the stem. The branch of the J-shaped stent showed a significantly lower bending resistance than the Y shaped one. This work provides a testing framework for the J-shaped and Y-shaped stents,which could shed some light on the optimal design of stent with branches.

Abstract

Background: To investigate the safety and effectiveness of trans-oral trans-sheath forceps biopsy (TTFB) for patients with severe esophageal obstruction under fluoroscopy.Methods: From November 2016 to November 2019, 35 patients with level Ⅲ or Ⅳ dysphagia and a Karnofsky score of less than 60 were enrolled to undergo TTFB and esophageal nutrition tube insertion or stenting simultaneously. Data on diagnostic performance, early complications, and radiation dose were collected, and Karnofsky scores before and after the procedures were compared.Results: The technical success of TTFB was 100%. The sensitivity, specificity and accuracy were 92.3% (24/26), 77.8% (7/9), and 88.6% (31/35), respectively. Complications occurred in two cases (5.7%). The mean procedure duration and irradiation dose were 23.2 min and 7.2 mSv, respectively. The Karnofsky scores significantly increased after 2-4 weeks (t = -8.9, P < 0.01).Conclusions: TTFB is a safe and effective method for patients with severe esophageal obstruction under fluoroscopy, especially in those who cannot undergo or refuse endoscopy.

Abstract

The multimedia micro-course is a professional basic course, having a very strong practicality and requiring the teacher to combine the theory with the practice to give lessons, this paper proposed one kind of evaluation of multimedia micro-Course based on the augmented reality technology Bayesian network. In this paper, the concept of the multi-branches tree of attribute in DDMB and the idea of reflecting the general characteristics of the data sets in the distributed envi-ronment through the multi-branches tree of attribute are expounded. This paper has introduced the method of constructing Multi-branches tree of attribute based on mobile agent access distribution Data Set, and specifically described the algo-rithm of generating integrated Bayesian network through multibranches tree of attribute The structure Learning and parameter learning of Bayesian networks oriented to multibranches tree of attribute and the method of determining the minimum threshold of dependent coefficient between attributes have been expounded. The experimental results show that, the model can effectively solve the problem of heavy learning burden, high storage cost and low executive efficiency of the original distributional environment Bayesian network, and has realized a more effective multimedia micro-course evaluation.
