Abstract

Background Defects after total pharyngolaryngectomy for hypopharyngeal cancer often require reconstruction via free tissue transfer. Recently, anterolateral thigh (ALT) flap has become the gold standard in many centers because of its advantages with respect to versatility, minimal donor-site morbidity, good speech quality, and relatively low fistula and anastomotic leakage rates. Moreover, ALT allows 2 surgical teams to work simultaneously. However, the height of the parallelogram in the ALT design for neoesophagus reconstruction is usually set at a minimum of 9.4 cm (circumference, 2πr) for smooth food passage. Because this height exceeds 8 cm, the donor site may not be closed primarily, which highly depends on the patient's body habitus and the skin tone or quality and requires other methods, such as local flap or skin graft for wound closure, which subsequently increase operating time and donor-site complication rate. Objectives Thus, we aimed to construct a simple and modified ALT design that will not only include the advantages described earlier but also provide adequate donor-site primary closure without jeopardizing complication rates. Methods Ten patients with hypopharyngeal cancer underwent reconstructive surgery using our modified ALT design after total pharyngolaryngectomy between 2010 and 2017. Our modified ALT design converts this "classical" shape into a parallelogram so that the height of the modified design is always less than 8 cm, thus allowing for easy primary closure of the wound. Results The donor-site defects of all 10 patients were closed primarily. No donor-site complications and partial or total flap loss were observed. One patient experienced persistent wound infection with dehiscence, for which debridement was performed. The stricture and fistula rates were 10% (n = 1) and 20% (n = 2), respectively. The mean follow-up time is approximately 1 year. Conclusions Minimizing donor-site morbidity is an important goal in reconstructive surgery. Our modified ALT flap design is simple, enabling easy primary closure of the donor-site defect, with improved results for the patient and operators. Furthermore, this design is also suitable for ALT flaps with widths larger than 8 cm.

Abstract

Two new labdane-type diterpene acids, (12 S)-12-hydroxylabda-8(17),13(16),14-trien-18-oic acid ( 1) and (12 R)-12-hydroxy-15,16-epoxylabda-8(17),13-dien-18-oic acid ( 2), along with a known labdane-type diterpene, (11 E)-15,16-bisnor-13-oxolabda-8(17),11-dien-19-oic acid ( 3), were isolated from the MeOH extract of the wood of Cunninghamia konishii. 48. Silicon-induced ene-type reaction in the thermal conversion of enolates to beta-silyl enones with molecular dioxygen.

Abstract

Reaction of alkyl, acetoxy, and silyl enol ethers of 3-(organosilyl)cyclohexanone with molecular dioxygen in toluene at 110 degrees C produced the corresponding conjugated enones in yields up to 88% yield. The reaction of the same type failed on replacement of the silyl group at the C-3 position with an isopropyl group. These results indicate the existence of an unprecedented silicon-induced ene-type reaction. Its reaction mechanism, generality, limitations, and exceptions are discussed.

Abstract

One of the keys for using deoxyribonucleic acid (DNA) as a nanomaterial relies on how the individual DNA chain can be aligned and how a multitude of DNA chains can be packed into ordered nanostructures. Here we present a simple method for constructing a 2-D densely packed DNA nanostructure using the electrostatic complex of DNA with a poly(amidoamine) (PAMAM) dendrimer of generation two. Ordered DNA arrays are formed by drop-casting an aqueous solution containing positively overcharged complexes onto mica followed by a prolonged incubation. During the incubation, the complexes tend to adsorb onto the negatively charged mica surface through electrostatic attraction. The rodlike complexes organize to form ordered arrays to increase the surface density of the adsorbed complexes and hence the attractive free energy of adsorption. The densely packed nanostructure obtained here is distinguished from the previously reported spheroid or toroid structure derived from DNA complexations with the higher-generation dendrimers.

Abstract

Polyanionic DNA interacts with cationic amphiphiles to form electrostatic complexes exhibiting rich self-assembled structures. This type of complex has been considered as a nonviral carrier in gene therapy and as a template for nanostructure construction. Here we report a thermally-induced phase transition of the complexes of DNA with the mixtures of a cationic surfactant, dodecyltrimethyl bromide (DTAB), and a neutral lipid, dioleoylphosphatidylethanolamine (DOPE), in fully hydrated state. An order-order transition between a multilamellar (L(c)alpha) phase and an inverted hexagonal (H(c)II) phase was found to occur with the transition temperature adjustable by the DTAB-to-DNA base pair molar ratio (x) and DOPE-to-DTAB molar ratio (m). The stability of the L(c)alpha phase was enhanced at lower m and x, as the L(c)alpha-to-H(c)II transition temperature increased with the decreases of these two parameters. The suppression of -to- transition at lower x was attributed to the lower entropic gain from the counterion release due to the presence of uncomplexed DNA in the bulk solution.
