Objective To observe the clinical efficacy of Redo-Bentall surgery in the reoperation of aortic root lesions. Methods A retrospective analysis was performed on 46 patients who underwent Redo-Bentall surgery for aortic root lesions in our department from June 2010 to April 2022. They were 35 males and 11 females, at a mean age of 43.37±12.79 years, in 4.96±6.76 years since the last operation. General clinical data in perioperative period and during follow-up were collected and analyzed. Kaplan-Meier survival analysis was used to compare the survival rates of each etiological group. Results There were 9 cases of central end otitis, 12 cases of Behset's disease, and 25 cases of other causes. After operation, 4 cases (8.70%) experienced cardiac arrest, 4 cases (8.70%) renal failure, 2 cases (4.35%) gastrointestinal bleeding, 2 cases (4.35%) new third-degree atrioventricular block and 2 cases (4.35%) permanent pacemaker placement. In perioperative period, 3 cases (6.52%) died in hospital. During a mean follow-up of 5.03±3.27 years after discharge, 5 cases (11.63%) were lost to follow-up, 1 case died (2.33%), 1 case had lacunar infarction (2.33%), and no severe bleeding or embolism complications was observed in the rest patients. The long-term survival rate was significantly lower in the endocarditis group (62.3%) and the Behcet's disease group (70%) than the other etiological groups (80%, P < 0.05). Conclusion The application of Redo-Bentall in the reoperation of aortic root lesions is safe and effective, but the survival rate is quite lower in the patients with infective endocarditis and Behcet's disease.

The TRPA1 ion channel is activated by electrophilic compounds through the covalent modification of intracellular cysteine residues. How non-covalent agonists activate the channel and whether covalent and non-covalent agonists elicit the same physiological responses are not understood. Here, we report the discovery of a non-covalent agonist, GNE551, and determine a cryo-EM structure of the TRPA1-GNE551 complex, revealing a distinct binding pocket and ligand-interaction mechanism. Unlike the covalent agonist allyl isothiocyanate, which elicits channel desensitization, tachyphylaxis, and transient pain, GNE551 activates TRPA1 into a distinct conducting state without desensitization and induces persistent pain. Furthermore, GNE551-evoked pain is relatively insensitive to antagonist treatment. Thus, we demonstrate the biased agonism of TRPA1, a finding that has important implications for the discovery of effective drugs tailored to different disease etiologies.

Objective To investigate the clinical characteristics and surgical treatment experience of reoperation of aorta caused by connective tissue disease. Methods The perioperative and follow-up data of 45 patients with reoperation of aorta in our department from January 2010 to December 2019 were collected and retrospectively analyzed. They were 33 males and 12 females, at an age from 9~73 (46.0±14.1) years. There were 19 cases of connective tissue disease, including 9 cases of Behcet's disease, 4 cases of Marfan's syndrome and 6 cases of bicuspid aortic valve. Bentall operation was performed in 24 cases, Bentall+total aortic arch replacement+descending aortic stent elephant trunk operation in 6 cases, Bentall+mitral valve replacement in 5 cases, and other types of operation in 10 cases. All the patients were followed up for 17~113 (58.5±25.6) months. Results Reoperation of aorta was quite rare, only accounting for 7.3% of reoperation of cardiovascular surgeries in our department (45/617), while connective tissue disease took 42.2% (19/45) among the reoperation. There were 5 patients who died during perioperative period, including 2 cases due to cardiac arrest, 2 cases to persistent low cardiac output syndrome and 1 case to refractory hypoxemia. During the follow-up, 7 cases died, including 4 cases due to Behcet's disease, 1 case to Marfan's syndrome and 2 cases to aortic dissection. The outcomes of other cases were satisfactory. Kaplan-Meier survival analysis indicated the mortality rate of Behect's disease was the highest during follow-up period (Log rank test, P=0.0082). Conclusion Connective tissue disease is an important cause of aortic reoperation. Behcet's disease has the highest mortality and the worst prognosis.

Objective To investigate the differences and factors influencing the medium-term outcomes of aortic valve replacement (AVR) between bicuspid aortic valve (BAV) and tricuspid aortic valve (TAV) patients. Methods A total of 245 patients who underwent AVR in our hospital from January 2014 to June 2019 and met our inclusion criteria were retrospectively recruited in this study, and were divided into TAV group (n=158) and BAV group (n=87), including 15 cases of type 0 Ap, 21 cases of type 0 Lat, 45 cases of type 1, and 6 cases of type 2. The 1:1 propensity score matching method (PSM) was used to screen the matching cohort, and the intraoperative, perioperative and follow-up outcomes were compared. Multivariate Logistic regression analysis was performed to evaluate the factors affecting the continued dilation of the ascending aorta. Results After PSM, 69 pairs of matching cohorts were selected and the baseline data of the 2 groups were basically balanced. Intraoperative and perioperative outcomes did not differ between the 2 groups. During the median follow-up time of 45 months, there were no significant differences in the diameter of the ascending aorta, diameter of the aortic sinus and its changes, incidence of adverse aortic events, functional status of the prosthetic valve, and classification of cardiac function between the 2 groups, while the difference of ascending aorta change, annual change rate, and proportion of continuous dilation of ascending aorta were significantly higher in the BAV group than the TAV group (P=0.016, 0.010, 0.041). Multivariate Logistic regression analysis of PSM cohort showed that BAV types (OR=3.296, 95%CI: 1.473~7.372), aortic regurgitation (OR=6.491, 95%CI: 2.691~15.657) and aortic valve stenosis with regurgitation (OR=8.233, 95%CI: 2.565~26.429) were positively correlated with the continued dilatation of the ascending aorta after AVR. Furthermore, after the BAV cohort was screened out from the PSM cohort, multivariate Logistic regression analysis indicated that type 1 in BAV (OR=10.378, 95%CI: 1.587~67.853), smoking (OR=4.959, 95%CI: 1.158~21.231) and aortic regurgitation (OR=8.070, 95%CI: 1.729~37.674) were positively correlated with the continued dilatation of the ascending aorta after AVR in BAV patients. Conclusion BAV patients are more likely to continue to dilate the ascending aorta after AVR when compared with TAV patients, and type 1 in BAV, smoking and aortic regurgitation are independent risk factors in BAV patients.

This study aims to comprehensively investigate the effects of hot-air dehydration on the quality of blue honeysuckle berries (Lonicera caerulea L.). The results demonstrated that drying with hot air at 40–65 °C for 7–72 h resulted in blue honeysuckle berries with a moisture content of 0.21–1.10 g H2O/g dry weight. Generally, low to medium temperatures (40–55 °C) showed a better effect on the quality than high temperatures (60–65 °C). Specifically, drying at 40 °C exclusively resulted in better retention of cuticular wax, the best sensory appearance, and the highest total phenolic content. Drying at 45 °C and 50 °C resulted in the highest antioxidant capacity and the optimal sensory flavor. Drying at 55 °C led to the highest soluble solid/acid ratio, ascorbic acid concentration, total flavonoid, and total anthocyanin. The work introduces an innovative raw berry product and provides a comprehensive practical and theoretical framework for convective dehydration of blue honeysuckle berries. [ABSTRACT FROM AUTHOR]

Objective To evaluate the clinical efficiency of intra-aortic balloon pump (IABP) in the perioperative period for the patients undergoing heart valve replacement by propensity score matching (PSM). Methods During January 2014 to December 2016, 43 patients treated with IABP in perioperative period in our institute were recruited and assigned into observation group. They were 21 males and 22 females, and at an average age of 52.5±9.8 (33~73) years. Another 43 patients (20 males and 23 females, aged 31~72 years, averagely 52.3±11.7 years) who underwent valve replacement but not IABP were selected from 1 487 cases of heart valve replacement patients in 2013 by PSM to balance intraoperative baseline data. The perioperative mortality, duration of assisted ventilation, length of ICU stay and incidences of postoperative complications were analyzed and compared between the 2 groups. Results There were no statistical differences in perioperative mortality [16.3% (7/43) vs 11.6% (5/43), P=0.137] and cases needing dialysis [7% (3/43) vs 14% (6/43), P=0.062] between the 2 groups. The observation group had statistically shorter time of endotracheal intubation (66.02±31.2 vs 79.27±17.1 h, P < 0.05) and lower incidence of low cardiac output syndrome (4.7% vs 37.2%, P < 0.05) than the other group. Conclusion IABP decreases the incidence rates of postoperative complications, but has no effect on the mortality in the patients undergoing heart valve replacement
