: Introduction: The “Migrants’ Approached Self‐Learning Intervention in HIV/AIDS for Tajiks” (MASLIHAT) recruits and trains Tajik labour migrants who inject drugs as peer educators (PEs) in delivering HIV prevention information and encouragement to adopt risk‐reduction norms and practices within their diaspora social networks while reducing their own HIV risk. Methods: The MASLIHAT intervention was tested in Moscow in a cluster‐randomized controlled trial with 12 recruitment sites assigned to either the MASLIHAT intervention or an equal‐time peer‐educator training focused on other health conditions (TANSIHAT). From October 2021 to April 2022, 140 male Tajik migrants who inject drugs were recruited as PEs to attend the 5‐session MASLIHAT training or the TANSIHAT non‐HIV comparison condition. Each participant in both groups recruited two network members (NMs) who inject drugs with the intent to share with them the information and positive strategies for change they had learned (n = 280). All PEs and NMs (n = 420) participated in baseline and follow‐up interviews at 3‐month intervals for 1 year. All received HIV counselling and testing. Modified mixed effects Poisson regressions tested for group differences in injection practices, sexual risk behaviours and heavy alcohol use over time. Results: At baseline, across both groups, 75% of participants reported receptive syringe sharing (RSS), 42% reported condomless sex and 20% reported binge drinking at least once a month. In contrast to TANSIHAT where HIV risk behaviours remained the same, significant intervention effects that were sustained over the 12 months were observed for receptive syringe and ancillary equipment sharing among both MASLIHAT PEs and NMs (p < 0.0001). Significant declines in the prevalence of sexual risk behaviours were also associated with the MASLIHAT intervention (p < 0.01), but not the comparison condition. Binge alcohol use was not affected in either condition; the MASLIHAT intervention had a transitory effect on drinking frequency that dissipated after 9 months. Conclusions: The MASLIHAT peer‐education intervention proved highly effective in reducing HIV‐related injection risk behaviour, and moderately effective in reducing sexual risk behaviour among both PEs and NMs. Network‐based peer education is an important tool for HIV prevention among people who inject drugs, especially in environments that are not amenable to community‐based harm reduction., INTRODUCTION While significant progress has been made in addressing the AIDS pandemic throughout much of the world, new cases of HIV increased by 48% in eastern Europe and central Asia [...]

Background: Despite multiple years of government HIV educational efforts, the growing trend of new cases among women in Indonesia runs parallel with their seemingly overall lack of comprehensive knowledge about HIV. A major prevention challenge for the Indonesian government lies in delivering HIV prevention education across the world's largest archipelago. This study investigates comprehensive HIV knowledge among reproductive-age women in Southwest Sumba, Indonesia, and the sources through which they report having learned about HIV along with potential mediators of the relationship between socioeconomic status (SES) and HIV knowledge. Methods: This cross-sectional study using convenience sampling recruited 159 married women ages 15–49 years old living in 4 selected church parishes in Southwest Sumba on March 12–20, 2023. The HIV Knowledge Questionnaire-18 and items from the 2017 Indonesian Demographic and Health Survey (24 total) measured the women's comprehensive knowledge about HIV. Additional items asked about sources through which the women had received information about HIV. One-way ANOVA examined differences in the level of HIV knowledge obtained through one or more information sources (healthcare providers, media, schools, and social network members). Mediation analysis investigated potential mediators of the relationship between SES and HIV knowledge. Results: Comprehensive HIV knowledge scores ranged from 0 to 21 (mean = 10.6). About 65% of the 159 women could only correctly answer 50% or less of the questions. Most incorrect answers pertained either to Mother-to-Child HIV Transmission (MTCT) or misconceptions about the virus. One-way ANOVA revealed that the mean of HIV knowledge increased as the number of informational sources increased (p <.001). Structural Equation Modelling found that social network members, healthcare providers, and the media all had significant direct effects on HIV knowledge. In addition, an indirect positive relationship between SES and comprehensive HIV knowledge was mediated by information obtained through the media and marginally through healthcare providers. Conclusions: The study's results suggest that more extensive in-depth HIV education is greatly needed to help prevent transmission among women living in small and outlying islands in Indonesia. Wider promotion of HIV information through the media and more intensively through local healthcare providers appears a promising and likely effective means to reach these women. Theorists posit that personal decisions to disclose being HIV positive are made based on the perceived consequences of that disclosure. This study examines the perceived costs and benefits of self-disclosure among African American injection drug users (IDUs). A total of 80 African American IDUs were interviewed in-depth subsequent to testing HIV positive. Participants reported that interpersonal costs of self-disclosure included stigma, loss of sexual/romantic partners, emotional harming of family/friends, shattering of privacy, physical isolation, blame, and loss of income. The benefits of disclosure included social support, emotional catharsis, and income. Four factors that help to tip the scales in either direction were identified. Study findings have implications for the delivery of counseling, testing, and partner notification services to African American IDUs living with HIV.

Introduction: Assisted partner notification (APN) safely and effectively increases partner awareness of HIV exposure, testing and case identification in community settings. Nonetheless, it has not been specifically developed or evaluated for use in prison settings where people with HIV often are diagnosed and may have difficulty contacting or otherwise notifying partners. We developed Impart, a prison‐based APN model, and evaluated its efficacy in Indonesia to increase partner notification and HIV testing. Methods: From January 2020 to January 2021, 55 incarcerated men with HIV were recruited as index participants from six jail and prison facilities in Jakarta in a two‐group randomized trial comparing the outcomes of self‐tell notification (treatment as usual) versus Impart APN in increasing partner notification and HIV testing. Participants voluntarily provided names and contact information for sex and drug‐injection partners in the community with whom they had shared possible HIV exposure during the year prior to incarceration. Participants randomized to the self‐tell only condition were coached in how to notify their partners by phone, mail or during an in‐person visit within 6 weeks. Participants randomized to Impart APN could choose between self‐tell notification or anonymous APN by a two‐person team consisting of a nurse and outreach worker. We compared the proportion of partners in each group who were notified of exposure by the end of 6 weeks, subsequently tested and HIV diagnosed. Results: Index participants (n = 55) selected 117 partners for notification. Compared to self‐tell notification, Impart APN resulted in nearly a six‐fold increase in the odds of a named partner being notified of HIV exposure. Nearly two thirds of the partners notified through Impart APN (15/24) completed HIV testing within 6 weeks post notification compared to none of those whom participants had self‐notified. One‐third of the partners (5/15) who completed HIV testing post notification were diagnosed as HIV positive for the first time. Conclusions: Voluntary APN can be successfully implemented with a prison population and within a prison setting despite the many barriers to HIV notification that incarceration presents. Our findings suggest that the Impart model holds considerable promise to increase partner notification, HIV testing and diagnosis among sex and drug‐injecting partners of HIV‐positive incarcerated men. Despite the rising number of cases of HIV in adults over age 50, older persons rarely are considered to be at risk for HIV/AIDS, and even though they may be involved in risky behavior, such as unprotected penetrative sex, they may not consider themselves vulnerable to becoming infected. Informed awareness of risk is essential to making positive decisions about adopting preventive measures. We examined demographic, sociobehavioral, and contextual factors that predict urban, low-income older adults' perception of HIV/AIDS risk. Logistic regression results from 398 residents aged 50-93 living in six buildings in two American cities found that males, younger participants (aged 50-61), those living in higher risk buildings, and those who worried more about contracting HIV/AIDS were more likely to perceive themselves to be at HIV/AIDS risk. Findings accounted for 32% of the variance and the prediction success rate was 72%. Results point to the importance of considering sociodemographic characteristics and environmental (contextual) factors as they influence heuristic decision making in understanding HIV/AIDS risk perception among low-income urban older adults and when designing HIV/AIDS education and intervention strategies targeting this population.
