Sneddon syndrome may present with neurological findings such as transient ischemic stroke, strokes, seizures and/or headaches. However, a purplish, spider web-like skin finding called livedo reticularis may accompany the skin and precede neurological findings. Sneddon syndrome often affects women. Since it is vasculopathy affecting small and medium vessels, other organ findings may accompany. We present a 44-year-old Sneddon syndrome patient with monoparesis in her left lower extremity, livedo reticularis on her back and legs, and hypertension.

We present the case of a 76-year-old patient who complained of confusion, extreme irritability, and headache and was diagnosed with MSM based on examination, imaging, and test results.

Objective: Coronavirus 2019 disease (COVID-19), the cause of the ongoing pandemic, is an acute respiratory tract infection, which has made it necessary for healthcare personnel to use protective equipment such as N95 masks, protective goggles and visors. External compression headaches caused by the compression of the pericranial soft tissues by wearing hats, helmets, or goggles (worn during swimming or diving) have been previously described. In our study, we aimed to evaluate the presence and characteristics of personal protective equipment–associated headaches in healthcare workers during the COVID-19 pandemic period and to determine the effects of such headaches. Materials and methods: A face-to-face questionnaire was delivered to 300 male and female healthcare personnel between the ages of 18–56 working in healthcare units where COVID-19 patients were evaluated and treated. The data from 296 completed questionnaires was evaluated. Results: We included 296 participants (166 females and 130 males) in our study; the mean age was 33.98 ± 8.52 years (range 18–56). One hundred ninety-six (66.22%) participants indicated that they had new onset personal protective equipment–associated headaches. ABSTRACT Background: Few studies have explored the coexistence of migraine and disordered eating attitudes. Furthermore, the underlying pathophysiological mechanisms of migraine and disordered eating attitude comorbidity are not clearly understood. Objective: This study aimed to investigate the association between migraine and disordered eating attitudes in relation to personality traits, obesity, quality of life, migraine severity, depression, and anxiety. Methods: This study included 91 patients with episodic migraine and 84 healthy control subjects. Self-report questionnaires were used to evaluate anxiety, depression, migraine-related disability, personality traits, quality of life, and eating disorders. Results: The Eating Attitude Test (EAT) showed disordered eating attitudes in 21 patients (23.1%) in the migraine group and eight patients (9.5%) in the control group. Migraine-related disability, anxiety, depression, neuroticism, and quality of life scores were significantly worse in migraine patients with disordered eating attitudes compared to migraine patients without disordered eating attitudes. In migraine patients, eating attitude test scores were positively correlated with migraine-related disability, anxiety, depression, and neuroticism scores, and negatively correlated with quality of life scores. Conclusion: The association of migraine and disordered eating attitudes was shown to be related to depression, anxiety, quality of life and personality traits and may also indicate a more clinically severe migraine. To the best of our knowledge, there is no literature study that deals with all these relevant data together. However, neuropsychiatry-based biological studies are required to better understand this multifaceted association.

Abstract Objective: The effects of vitamin D on the central and peripheral nervous system continue to be investigated today. In the present study, we aimed to evaluate pain and electrophysiologic response in patients with carpal tunnel syndrome (CTS) who have undergone replacement therapy due to vitamin D deficiency. Methods: Fifty female patients diagnosed with mild and moderate CTS and accompanied by vitamin D deficiency were included in this study. Nerve conduction study (NCS) was performed before and after vitamin D replacement, and the patient’s pain was evaluated with Visual Analogue Scale (VAS). OBJECTIVE: Both neurological diseases and related disabilities are growing all over the world and in our country. Awareness of the neurological diseases is thought to be less than the expected. METHODS: The aim of this study is; to investigate the awareness of the neurological diseases in the secondary and tertiary neurological outpatient service attendies and to assess its correlations between the patient’s demographics. RESULTS: A total of 540 patients were included in the study. Alzheimer disease was found to be most known neurological disease and Multiple sclerosis was the least one. The awareness of the diseases correlated with the level of education. Younger patients were seem to be more aware about the neurological diseases than the older patients. Although currently widespread usage of internet/media, we found that patients prefer mostly to take advice from their family practitioners for being to attend to the neurological outpatient services. We comment that this result is a positive finding in terms of usage of health care system and patient-physician relationships. CONCLUSION: There is need for increasing the awareness of the neurological diseases for both early diagnosis of neurological diseases and to decrease the related disability

OBJECTIVE: The etiology and frequency of headache complaint were examined in patients diagnosed with idiopathic Parkinson’s disease. METHODS: 60 patients diagnosed with idiopathic Parkinson’s disease in policlinic and a healthy control group of 100 people of similar age and sex were included in the study. RESULTS: Headache was found in 26 of the patients (43,3%) with idiopathic Parkinson’s disease. There was not a significant difference between the ages of patients with and without headache (p=0,1). The duration of the disease was significantly longer in the patients without headache when compared to those with headache (p=0,02). CONCLUSION: The frequency of headache in patients with idiopathic Parkinson’s disease is not higher than the healthy population

Two siblings with myoclonic and generalized tonic clonic seizures, progressive dementia, ataxia and dysarthria, who were diagnosed as Lafora disease by sweat gland biopsy, are discussed.

Since then totaly few papers were published about this syndrome. The patient is 15 years old girl and she has been suffering from ipsilateral numbness and pain on the neck and tongue during sharp rotation of the neck for ten years. 13. Current Perspectives on the Impact of Chronic Migraine on Sleep Quality: A Literature Review

Recent studies have shown that sleep problems occur in migraineurs and poor sleep causes chronification, but the mechanisms by which chronic migraine affects sleep quality are still unknown. This review aims to analyze commonly reported sleep disturbances in chronic migraine (CM) and determine the effect of CM on sleep quality.We conducted a comprehensive review of all published articles on CM and sleep quality from inception to March 2022 in the literature. Clinical trials, observational studies, and case series (≥20 cases) were included. Two reviewers and a supervisor reviewed the titles and abstracts of all search results with predefined inclusion and exclusion criteria. PubMed search for randomized controlled trials and open studies on CM and sleep quality reported in English between 1983 and 2022 was conducted using the keywords including chronic migraine, sleep, insomnia, sleep quality, polysomnography, and Pittsburgh Sleep Quality Index.A total of 535 potentially relevant articles were found. A total of 455 articles and reviews, meta-analyses published in any language other than English, with other exclusion criteria, were excluded from the review. In the remaining articles, 36 clinical studies, reviewing sleep quality and its association with migraine, were identified and reviewed. Evidence from this review shows that poor sleep and migraine chronicity are intertwined with other accompanying comorbidities and dysregulation of circadian rhythm that innovative treatments promise to bring relief to both poor sleep as well as migraine.Sleep disorders are common in CM and the association between migraine chronification and sleep quality is bidirectional. Comorbid conditions with accompanying frequent attacks in migraine may impair sleep quality. While the maladaptive pain process worsens sleep, poor sleep quality also negatively affects migraine pain. Sleep disturbance, which is affected by worsening migraine attacks, causes deterioration in the quality of life, loss of workforce, and economic burden.

15. Addition of lesser occipital nerve blockade to greater occipital nerve blockade in patients with chronic migraine

Objectives: The aim of this study was to compare greater occipital nerve blockade (GONB) alone and GONB combined with lesser occipital nerve blockade (LONB) in chronic migraine patients. Methods: Patients were randomly divided into two groups: Group A consisted of 22 patients who received only GONB; while Group B consisted of 20 patients who underwent GONB and LONB. The demographics and clinical characteristics of the patients were evaluated. The injections were given unilaterally and to the side where the subjects experienced greater pain. GONB with or without LONB was performed on each patient once a week for 4 weeks and then two more times a month apart; 6 times in total. The number of headache days, severity of attacks and duration of headache episodes was recorded from headache diaries before treatment and the on the first, second and third month following the start of treatment. Treatment efficiencies were evaluated within and between the groups. Results: The duration of pretreatment headaches was significantly longer in Group B (p=0.032). There were no differences between the groups in terms of other demographic and clinical characteristics. When the treatments applied in group A and group B were evaluated separately compared to the control group, there was a statistically significant decrease in the number of headache days, VAS scores and headache duration (p 0.05). Conclusions: This study suggests that there is no difference in the number of headache days or headache intensity between GONB alone or in combination with LONB in chronic migraine patients. GONB combined with LONB in patients may be more effective than GONB alone when headaches of longer duration are present.

- Abstract
Background: Migraine (MIG) is a common cause of headache and a leading cause of morbidity in Turkey but comprehensive and comparative data evaluating its burden are scarce. The study aimed to describe clinical characteristics and management of MIG and compare MIG with tension type headache (TTH) regarding the burden of disease and healthcare resource utilization. Methods: A total of 1368 patients (aged 18-65 years) with MIG or TTH were surveyed regarding patients’ sociodemographics, headache characteristics, clinical management, burden of disease, quality of life and healthcare resource utilization within the previous 12 months. Data from 1053 patients with definite MIG (dMIG) (n: 924; 87.7%) or definite TTH (dTTH) (n: 129; 12.3%) criteria were analyzed.Results: The frequency and duration of attacks, the number of monthly headache days and of days with analgesic consumption and the severity of headaches were higher in dMIG than in dTTH (p:0.005 for the frequency of attacks and p14 days). Only 222 (36.8%) of definite migraineurs who experienced ≥4 monthly headache days were on preventive treatment. Migraine Disability Assessment (MIDAS) scores were higher in dMIG than in dTTH (24.7±40 vs 10.6±12; pp:0.016) but the number of physician visits were higher in dMIG (3.5 ±5 vs 1.7 ± 1; p pConclusions: It is crucial to timely and correctly diagnosing and optimally managing MIG due to its significant burden. Educational programmes for patients and healthcare providers, adherence and persistence to preventive medications may improve clinical outcomes.

Cervicogenic headache (CEH) is currently identified with different diagnostic criteria. The latest one is the International classification of headache disorders (ICHD), 3rd edition (2018). At the present time, there are not enough published articles with reliable sensitivity and specificity that may support a classification for clinical and research purposes. Current literature suggests improvement to the classification(s). The ICHD criteria should be modified to reach an optimal sensitive and specific level to identify CEH as a secondary headache. The B, C1, and C2 criteria should be implemented with proposed suggestions. The C3 criterion should be upgraded. Criteria such as mechanical precipitation of pain by digital pressure on neck trigger points and specific movements, strictly unilateral pain without side-shift, diffuse unilateral shoulder and arm pain, pain starting posteriorly and spreading anteriorly should be integral part of the classification.

Patients with multiple sclerosis (MS) often report fatigue, poor sleep and complaint of sleep disorders. Neurofilament light chain (NF-L) has been identified as a potential biomarker for disease progression in MS patients. In this study, we aimed to evaluate sleep characteristics in MS patients and its relationship with the level of serum NF-L. In the present study carried out as a prospective and cross-sectional study, 32 relapsing–remitting MS (RRMS) patients and 32 control subjects were included. Abstract Background Migraine is a disease characterized by headache attacks. The disease is multifactorial in etiology and genetic and environmental factors play role in pathogenesis. Migraine can also be accompanied by psychiatric disorders like neurotism and obsessive compulsive disorder. Stress, hormonal changes and certain food intake can trigger attacks in migraine. Previous studies showed that eating attitudes and disorders are prevalant in patients with migraine. Eating disorders are psychiatric disorders related to abnormal eating habits. Both migraine and eating disorders are common in young women and personality profiles of these patient groups are also similar. A possible relationship which shows that migraine and eating habits are related can lead to a better understanding of disease pathogenesis and subsequently new therapeutic options on both entities. Association of migraine in relation to severity, depression and anxiety and eating habits and disorders were aimed to be investigated in this study. Methods The study was designed as a prospective, multi-center, case control study. Twenty-one centers from Turkey was involved in the study. The gathered data was collected and evaluated at a single designated center. From a pool of 1200 migraine patients and 958 healthy control group, two groups as patient group and study group was created with PS matching method in relation to age, body-mass index, marital status and employment status. Eating Attitudes Test-26 (EAT-26), Beck’s Depression Inventory (BDI) and Beck’s Anxiety Inventory (BAI) were applied to both study groups. The data gathered was compared between two groups. Results EAT-26 scores and the requirement for referral to a psychiatrist due to symptoms related to eating disorder were both statistically significantly higher in patient group compared to control group (p = 0.034 and p = 0.0001 respectively). Patients with migraine had higher scores in both BDI and BAI compared to control group (p = 0.0001 and p = 0.0001 respectively). Severity of pain or frequency of attacks were not found to be related to eating attitudes (r:0.09, p = 0.055). Conclusions Migraine patients were found to have higher EAT-26, BDI and BAI scores along with a higher rate of referral to a psychiatrist due to symptoms. Results of the study showed that eating habits are altered in migraine patients with higher risk of eating disorders. Depression and anxiety are also found to be common amongst migraine patients.

Migraine is a disabling primary headache disorder that directly affects more than one billion people worldwide. Despite its widespread prevalence, migraine remains under-diagnosed and under-treated. To support clinical decision-making, we convened a European panel of experts to develop a ten-step approach to the diagnosis and management of migraine. Each step was established by expert consensus and supported by a review of current literature, and the Consensus Statement is endorsed by the European Headache Federation and the European Academy of Neurology. In this Consensus Statement, we introduce typical clinical features, diagnostic criteria and differential diagnoses of migraine. We then emphasize the value of patient centricity and patient education to ensure treatment adherence and satisfaction with care provision. Further, we outline best practices for acute and preventive treatment of migraine in various patient populations, including adults, children and adolescents, pregnant and breastfeeding women, and older people. In addition, we provide recommendations for evaluating treatment response and managing treatment failure. Lastly, we discuss the management of complications and comorbidities as well as the importance of planning long-term follow-up., In this Consensus Statement, which is endorsed by the European Headache Federation and the European Academy of Neurology, an expert panel provides recommendations for the diagnosis and management of migraine to support clinical decision-making by general practitioners, neurologists and headache specialists.

The relationship between sleep and brain lesions remains unknown in patients with multiple sclerosis (MS) and is currently under investigation. Sleep deprivation and reduction in the quality of sleep occur as a result of the direct involvement of the specific central nervous system structure. Therefore, we investigated the relationship between demyelination locations and sleep parameters such as sleep efficiency and respiratory disorders. The polysomnography measurements and brain magnetic resonance imaging (MRI) of 28 patients with relapsing–remitting MS were examined. The MRI results were evaluated using standardized maps to depict voxel-wise probability distribution of demyelination lesion load for patients. First, supratentorial and infratentorial lesion loads were evaluated. Subsequently, the procedure was completed by measuring the lobes and brainstem lesions loads in the brain. There was a negative correlation between the percentage of REM (%) and infratentorial and brainstem lesion loads ( $$\rho$$ : − 0.396, − 0.418; p 0.05). Although sleep-related breathing disorders are commonly seen in MS, this study shows that the polysomnography parameter most affected by brain lesion load distribution is REM sleep.

The objective of this study was to evaluate the spouses of people with epilepsy with and without a history of seizures during sleep in terms of depression, anxiety, and sleep quality.This prospective, cross-sectional study was conducted in three groups of 18-55 year olds, who were at least primary school graduates. The 1st group consisted of healthy spouses of 30 healthy volunteers with age and sex matched with the other groups. The 2nd group comprised spouses of 30 people with epilepsy who had been married for at least one year and had no history of seizures during sleep. The 3rd group consisted of spouses of 30 people with epilepsy who had been married for at least one year and had a history of at least one seizure during sleep in the course of the previous year. The questionnaire including demographic data, Pittsburgh Sleep Quality Index (PSQI), Beck Depression Inventory (BDI), and the Beck Anxiety Inventory (BAI) was applied to all participants.The mean age of the 1st group was 35.07 ± 8.33 years, that of the 2nd group was 36.47 ± 7.63 years, and that of the 3rd group was 35.33 ± 6.05 years. There was no significant age difference between the groups (p = 0.740). The depression scores of the 2nd and the 3rd groups were significantly higher than that of the 1st group (p 0.001, for both). The anxiety scores of the 3rd group were significantly higher than those of the 1st and the 2nd groups (p 0.001 and p = 0.001, respectively). Thirty percent (n = 9) of the 1st group, 40% (n = 12) of the 2nd group, and 70% (n = 21) of the 3rd group had poor sleep quality. The sleep disorder rate in the 3rd group was significantly higher than in the 1st and the 2nd groups (p = 0.002 and p = 0.020, respectively). When the PSQI subscales were examined, the sleep quality, sleep latency, usual sleep efficiency, daytime dysfunction, and the total sleep total score were significantly higher in the 3rd group than the 1st and the 2nd groups. The patients in the third group had significantly higher scores of sleep duration, sleep disturbance, and sleep medication use than those in the 1st group.We found out that the PSQI score, which reflected the sleep quality, was poor in the spouses of people with epilepsy, who had seizures during sleep. To the best of our knowledge, these findings are the first in the literature on this subject.

Individuals with diabetes can develop cognitive impairment due to dysfunction of glucose metabolism; however, it remains unclear whether cognition becomes altered in the prediabetic stage. Substantial evidence links cognitive impairment in diabetes to aberrant serum insulin-degrading enzyme (s[IDE]) levels. This relationship remains to be investigated in individuals with prediabetes.To investigate the relationship between cognitive function and s[IDE] levels in individuals with prediabetes.The study group consisted of 47 individuals who had been diagnosed with prediabetes and 41 healthy controls. Cognitive functions were evaluated using the Montreal Cognitive Assessment (MoCA), and s[IDE] levels were measured using enzyme-linked immunosorbent assay.The MoCA total scores and s[IDE] levels of the individuals with prediabetes were significantly lower (P = 0.001, 0.006) than those of the controls, and the MoCA Attention measure of the individuals with prediabetes was also very low (P = 0.001). To determine cognitive impairment, we divided the prediabetics into two subgroups according to the MoCA cutoff value. Scores on all of the MoCA tests were significantly lower in the group with mild cognitive impairment (P0.05). There was no correlation between MoCA scores and s[IDE] levels (P0.05), but serum-fasting glucose levels showed a negative correlation with MoCA scores (P0.05, ρ = -0.287).Evidence of mild cognitive impairment was high in the individuals with prediabetes and showed a negative correlation with serum-fasting glucose levels but not with s[IDE] levels.

Background: Few studies have explored the coexistence of migraine and disordered eating attitudes. Furthermore, the underlying pathophysiological mechanisms of migraine and disordered eating attitude comorbidity are not clearly understood. Objective: This study aimed to investigate the association between migraine and disordered eating attitudes in relation to personality traits, obesity, quality of life, migraine severity, depression, and anxiety. Methods: This study included 91 patients with episodic migraine and 84 healthy control subjects. Self-report questionnaires were used to evaluate anxiety, depression, migraine-related disability, personality traits, quality of life, and eating disorders. Results: The Eating Attitude Test (EAT) showed disordered eating attitudes in 21 patients (23.1%) in the migraine group and eight patients (9.5%) in the control group. Migraine-related disability, anxiety, depression, neuroticism, and quality of life scores were significantly worse in migraine patients with disordered eating attitudes compared to migraine patients without disordered eating attitudes. In migraine patients, eating attitude test scores were positively correlated with migraine-related disability, anxiety, depression, and neuroticism scores, and negatively correlated with quality of life scores. Conclusion: The association of migraine and disordered eating attitudes was shown to be related to depression, anxiety, quality of life and personality traits and may also indicate a more clinically severe migraine. To the best of our knowledge, there is no literature study that deals with all these relevant data together. However, neuropsychiatry-based biological studies are required to better understand this multifaceted association. RESUMO Introdução: Poucos estudos exploraram a coexistência de migrânea e hábitos alimentares inadequados. Além disso, os mecanismos fisiopatológicos subjacentes da migrânea e da comorbidade da atitude alimentar inadequada não são claramente entendidos. Objetivo: Este estudo teve como objetivo investigar a associação entre migrânea e atitudes alimentares inadequadas em relação aos traços de personalidade, obesidade, qualidade de vida, gravidade da migrânea, depressão e ansiedade. Métodos: Este estudo incluiu 91 pacientes com migrânea episódica e 84 indivíduos saudáveis. Questionários de autorrelato foram utilizados para avaliar ansiedade, depressão, incapacidade relacionada à enxaqueca, traços de personalidade, qualidade de vida e distúrbios alimentares. Resultados: O Teste de Hábito Alimentar (THA) mostrou hábitos alimentares inadequados em 21 pacientes (23,1%) no grupo com migrânea e 8 pacientes (9,5%) no grupo controle. Os índices de incapacidade, ansiedade, depressão, neuroticismo e qualidade de vida relacionados à migrânea foram significativamente piores em pacientes com migrânea com hábitos alimentares inadequados em comparação com pacientes com migrânea sem hábitos alimentares inadequados. Em pacientes com migrânea, os escores dos testes de hábito alimentar foram correlacionados positivamente com os escores de incapacidade, ansiedade, depressão e neuroticismo relacionados à migrânea e negativamente com os escores de qualidade de vida. Conclusão: A associação de migrânea e atitudes alimentares inadequadas mostrou-se relacionada à depressão, ansiedade, qualidade de vida e traços de personalidade, podendo também indicar uma migrânea mais clinicamente grave. Até onde sabemos, não há estudo de literatura que lide com todos esses dados relevantes juntos. No entanto, são necessários estudos biológicos baseados em neuropsiquiatria para entender melhor essa associação multifacetada.

The success of various therapy methods in the treatment of insomnia and poor quality sleep, both of which play an active role in the chronicization process of migraine, has been the subject of investigation. The aim of this research was to evaluate the success, acceptability, and efficacy of greater occipital nerve block (GON-B) therapy in chronic migraine (CM) patients in improving their sleep quality and developing their beliefs about sleep . The study included 40 patients with CM from the general population who agreed to receive blockade therapy. Before the injection treatment, 1-week sleep diaries as well as depression and anxiety symptoms were examined. The Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Pre-Sleep Arousal Scale (PSAS), Epworth Sleepiness Scale (ESS), and Dysfunctional Beliefs and Attitudes about Sleep (DBAS) scales were measured and evaluated both before and after the completion of 3 months of treatment. Thirty-seven patients with CM successfully completed our 12-week treatment period. The anxiety and depression scales of the patients decreased after 3 months (p < 0.001). PSQI and its subgroups ratings significantly improved after treatment (p < 0.001). Similarly, ISI, ESS, PSAS, and DBAS test scores also improved after treatment (p < 0.001). This study provides evidence as to the applicability and acceptability of GON-B in CM treatment in terms of increased sleep quality; improved sleep beliefs, attitudes, and behaviors; and arousals and decreased insomnia findings.

This study was designed to investigate the effect of apilarnil on neuronal damage and related mechanisms in a sepsis model in order to demonstrate whether or not apilarnil has neuroprotective effect.In this study, 64 adult male Sprague-Dawley species rats were randomly divided into eight groups. The rats were administered apilarnil and/or lipopolysaccharide (LPS). Superoxide dismutase (SOD), catalase (CAT), malondialdehyde (MDA), xanthine oxidase (XOD) and testican-1 levels were measured in the brain tissue. Proinflammatory cytokines (tumor necrosis factor alpha [TNF-α], interleukin 1 beta [IL-1β], interleukin 6 [IL-6]) were measured in brain tissue. Histological examinations were performed on hippocampus and cortex tissues in all groups. Apoptotic cell count was estimated using the Tunel method to observe the apilarnil's effect on apoptosis. Purkinje cells were counted in the hippocampus to measure the protective effect of apilarnil on the hippocampus.Apilarnil reduced the decrease in SOD and CAT levels in the brain developing sepsis. Apilarnil reduced the increase in MDA, XOD, and testican-1 levels in the septic brain. It was observed that the number of degenerated neurons due to sepsis decreased as apilarnil dose increased. Apilarnil reduced the elevated levels of proinflammatory cytokines (IL-6, TNF-α, IL-1β) induced by sepsis. Apilarnil prevented sepsis-related apoptosis in the brain.The neuroprotective potential of apilarnil against brain damage in the sepsis model was demonstrated and suggested that it has the potential to contribute to new therapeutic targets against various neurological disorders.

30. A comparison of the clinical efficacy of GON block at the C2 level and GON block at the classical distal occipital level in the treatment of migraine

The aim of this retrospective study was to compare the efficacy and complications of ultrasound-guided block (C2GON) at the level of C2 and block performed from the distal occiput level (DOGON) as the greater occipital nerve progresses superficially to the obliquus capitis inferior muscle in migraine patients.63 patients were included in this retrospective study. For C2GON blockade, 2 units of 4 ml 0.5% bupivacaine were prepared for bilateral administration to 32 patients. For DOGON, two 1.5 ml 0.5% bupivacaine was prepared bilaterally for each patient. The patients were evaluated in terms of pain intensity and frequency, analgesic use, and any side effects before the injection and at the 1st and 3rd months after the last treatment.When we compared C2GON block and DOGON block in our study, we found that both blocks were effective for 3 months. The reduction rates in the C2GON and DOGON groups were 75.3% and 59% in the number of severe attacks, 63.9% and 59.1% in the total analgesic use, 71.1% and 56.25% in the triptan use, 78% and 60% in the number of days with severe headache, 63.4% and 50.3% in the duration of headache. The decrease in the highest VAS score was 48.1% in the group with C2GON, while it was 53% in the group with DOGON.Current findings have shown that both C2GON and DOGON blockades are effective in the treatment of migraine for 3 months.

Osmophobia and headache triggered by odors are commonly seen in migraine, and these are symptoms that differentiate migraine from other primary headaches. Since these odor-related symptoms are disease-specific, we aimed to measure the volume of olfactory bulb and depth of olfactory sulcus in migraine patients. A total of 93 subjects, consisting of 62 episodic migraine (32 with osmophobia, 30 without osmophobia) patients and 31 healthy controls, were included in this study. Diagnosis and classification of migraine were performed according to the beta version criteria of International Classification of Headache Disorders (ICHD-3 Beta version). To better understand the etiology of the coexistence of MS and migraine and the outcomes of this relationship, the vitamin D, vitamin D-binding protein (VITDBP), vitamin D receptor (VITDR), high-sensitivity C-reactive protein (hs-CRP), superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-Px), total antioxidant status (TAS), total oxidant status (TOS), and Oxidative Stress Index (OSI) values were examined in patients with the coexistence of relapsing-remitting multiple sclerosis (RRMS) and migraine. Methods: This study was conducted between January 1, 2019, and July 25, 2019, at the neurology and biochemistry clinics of two different tertiary hospitals simultaneously. Overall, 50 RRMS patients with migraine, 50 RRMS patients without migraine, and 50 healthy volunteers were included in the study. The participants' vitamin D, VITDBP, VITDR, hs-CRP, SOD, CAT, GSH-Px, TAS, TOS, and OSI values were measured. Results: The vitamin D and VITDR values of the RRMS patients with migraine were lower than those of the RRMS patients without migraine (respectively, p = 0.014, p < 0.001). There was no significant difference between the RRMS patients with and without migraine in terms of their VITDBP values (p = 0.570). The SOD, CAT, GSH-Px, and TAS values of the RRMS patients with migraine were lower than those without migraine (all p < 0.001). The hs-CRP and TOS values of the RRMS patients with migraine were higher than those without migraine (all p < 0.001). Conclusion: To the best of our knowledge, this is the first study on this topic to date. Based on the results, our study may shed light on the etiopathogenesis of the coexistence of MS and migraine and new treatments. However, more studies are needed to better understand the etiology of this relationship and its negative effects.

Amaç: Bu çalışmada orta ve ağır evre Alzheimer hastalarına bakım veren aile üyelerinin depresyon, anksiyete ve uyku kalitelerinin sağlıklı kontrollerle karşılaştırılması amaçlandı. Materyal ve Metod: Bu prospektif, kesitsel çalışmaya orta veya ağır evre Alzheimer hastalarına bakım veren 18-65 yaş arası, en az ilkokul mezunu olan 29 kişi ile bakım verme öyküsü olmayan 30 sağlıklı kontrol grubu dahil edildi. Olgulara demografik verileri içeren anket soruları, Pitssburgh Uyku Kalitesi İndeksi (PUKİ), Beck Depresyon Ölçeği (BDÖ) ve Beck Anksiyete Ölçeği (BAÖ) uygulandı. Bulgular: Alzheimer hastalarının yaş ortalaması 74,52±8,23 yıl idi. Alzheimer hastalarının 14’ü (%48,3) kadın, 15’i (%51,7) erkekti. Alzheimer hastalarına bakım verenlerin yaş ortalaması 42,31±10,01, kontrol grubunun yaş ortalaması 37,87 ±11,08 idi. Yaşlar arasında anlamlı fark yoktu (p=0,111). Gruplar arasında cinsiyet açısından da anlamlı fark yoktu (p=0,322). Alzheimer hastalarına bakım verenlerin BDÖ median değeri 7 (min:2, max:19) yıl olup sağlıklı kontrol grubunun BDÖ median değeri 10 (min:6, max:36) yıl idi. Bu fark istatiksel olarak anlamlıydı (p=0,001). Bakım verenlerin BAÖ median değeri 5 (min:2, max:16) yıl olup sağlıklı kontrol grubunun BAÖ median değeri 6 (min:4, max:17) yıl idi. Bu fark istatiksel olarak anlamlıydı (p=0,026). PUKİ toplam puanlarına göre bakım verenlerin %51,7’sinde (n=15), sağlıklı kontrol grubunun %23,7’sinde (n=12) uyku kalitesi kötüydü ve bu fark anlamlıydı (p=0,047). Sonuç: Alzheimer hastalarına bakım verenlerde ölçek puanlarına göre anksiyete ve depresyon puanları daha yüksek bulunmuş olup ayrıca uyku kalitelerinin de daha kötü olduğu tespit edilmiştir.

ObjectivesThe importance and popularity of peripheral nerve block procedures have increased in the treatment of migraine. Greater occipital nerve (GON) block is a commonly used peripheral nerve block method, and there are numerous researches on its use in migraine treatment.Materials and methodsA search of PubMed for English-language randomized controlled trials (RCT) and open studies on greater occipital nerve block between 1995 and 2018 was performed using greater occipital nerve, headache, and migraine as keywords.ResultsIn total, 242 potentially relevant PubMed studies were found. A sum of 228 of them which were non-English articles and reviews, case reports, letters and meta-analyses were excluded. The remaining articles were reviewed, and 14 clinical trials, seven of which were randomized-controlled on greater occipital nerve block in migraine patients, were identified and reviewed.ConclusionsAlthough clinicians commonly use greater occipital nerve block in migraine patients, the procedure has yet to be standardized. The present study reviewed the techniques, drugs and dosages, the frequency of administration, side effects, and efficacy of greater occipital nerve block in migraine patients.

Objective: Trigeminal autonomic cephalalgias (TACs) are a group of primary headache syndromes. Although hemicrania continua (HC)is not included among the TACs according to the International Classification of Headache Disorders (ICHD-II), it also shows autonomic symptoms. In the literature, some authors suggest that TACs can be related with pituitary lesions. In this study, we have compared the pituitary lesions observed in the TAC, HC and migraine-type headache patientsMethods The cranial magnetic resonance images (MRI) of the patients with TAC, HC and migraine without aura (MWOA) unaccompanied by autonomic symptoms were retrospectively evaluated. The 33 TAC and HC patients were compared with the 30 migraine without aura (MWOA) patients. Results: The mean age of the TAC and HC patients was 43. 36±10. 72 years, whereas the mean age of the MWOA patients was 39.0±6.92 years. No statistically significant difference in terms of age was observed between the groups (p˃0.05). The MRI of the TACs and HC patients indicated that 62.5% (n:20) were normal, while 12.5% (n:4) had pituitary microadenomas, 6.3 % (n:2) had macroadenomas, 12.5% (n:4) had suspected pituitary microadenomas, and 6. 3% (n:2) had suspected heterogeneity. The MRI results of the MWOA patients were normal in 46.7% (n:14), whereas 23.3% (n:7) had pituitary microadenomas, 23.3% (n:7) had suspected pituitary microadenomas, and 6. 2% (n:2) had suspected pituitary heterogeneity. No statistically significant difference was observed between both groups (p>0. 05).Conclusion: In this study, no correlation was observed between the TACs, HC and pituitary abnormalities. In conclusion, the use of pituitary MRI has not been supported as a routine method in patients with this type of headache

Objective GON blockade with local anesthetics is an effective treatment for a group of headaches, such as cervicogenic headache, cluster headache, occipital neuralgia, migraine. Our aim was to evaluate the efficacy of greater occipital nerve (GON) blockade in patients with chronic migraine (CM) by using a control group. Materials and Methods We included 44 CM patients and randomly divide the patients into two groups, as group A (bupivacaine) and group B (placebo) to our study. GON blockade was administered four times (once per week) with bupivacaine or saline. After 4 weeks of treatment, patients were followed up for 3 months, and findings were recorded once every month for comparing each month's values with the pretreatment values. The primary endpoint was the difference in the frequency of headache (headache days/month). VAS pain scores were also recorded. Results A total of 44 patients had completed the study; no severe adverse effects had occurred. Group A showed a significant decrease in the frequency of headache and VAS scores at the first, second, and third months of follow-up. Similarly, group B showed a significant decrease in the frequency of headache and VAS scores at the first month of follow-up, but second and third months of follow-up showed no significant difference. Conclusion Our results suggest that GON blockade with bupivacaine was superior to placebo, has long-lasting effect than placebo, and was found to be effective for the treatment of CM. More studies are needed to better define the safety and cost-effectiveness of GON blockade in CM.

It has been hypothesized that chronic inflammation may play an important role in the development and progression of the neurodegeneration of Idiopathic Parkinson's disease (IPD). Neutrophil-lymphocyte ratio (NLR) is a marker that indicates the peripheral inflammation. There is only one study regarding NLR and IPD. In this study, we assessed to investigate NLR in patients with IPD, comparing the results with controls and to determine whether there is a difference in NLR levels in subgroups of IPD (akinetic-rigid and tremor-dominant) differ in their levels of NLR. Medical records of 200 IPD patients and 60 controls reviewed retrospectively. Forty-six IPD patients met the inclusion criteria. NLR was calculated by dividing neutrophil count to lymphocyte count. Thirteen akinetic-rigid (AR-IPD) and thirty-three tremor-dominant (TD-IPD) patients' and controls results were compared. There was not a statistically difference between the NLR levels of IPD patients, controls, and AR-IPD and TD-IPD patients. This result suggests that cerebral inflammation is in the forefront in the development of neurodegeneration in IPD, and that more evidence is needed for the role of peripheral inflammation in the development progression of disease.

PURPOSE Diabetes mellitus (DM) is a chronic metabolic disorder that often leads to complications. We aimed to correlate two complications of DM, polyneuropathy and hyperactive bladder syndrome, using noninvasive measures, such as screening tests. METHODS We included 80 female and 40 male type 2 diabetic patients in this prospective study. Diabetic polyneuropathy evaluations were conducted using the Douleur Neuropathique 4 Questions (DN4), and overactive bladder (OAB) evaluations were performed using the Overactive Bladder Questionnaire (OAB-V8). The patients were also evaluated for retinopathy and nephropathy. The diabetic male and female patients with or without OAB were chosen and compared for microvascular complications (polyneuropathy, retinopathy, and nephropathy). RESULTS There were no significant correlations between OAB and retinopathy as well as between OAB and nephropathy among diabetic patients (female patients, P>0.05; male patients, P>0.05 ). Parkinson's disease (PD) is a neurodegenerative disorder characterized by hyposmia in the preclinical stages. We investigated the relationships of olfactory bulb (OB) volume and olfactory sulcus (OS) depth with basal ganglia and hippocampal volumes. The study included 25 patients with PD and 40 age- and sex-matched control subjects. Idiopathic PD was diagnosed according to published diagnostic criteria. Conventional (Continuous) Radiofrequency thermocoagulation (CRF) and pulsed radiofrequency (PRF) are becoming treatment choices in severe headache cases. PRF differs from CRF as it maintains tissue temperature below 42 °C and prevents possible tissue destruction. We will focus on the PRF technique, mechanisms, and usage in cases of headache resistant to medical treatment. Its usage in trigeminal neuralgia is well known. We will therefore concentrate on the effectiveness of PRF of the sphenopalatine ganglion (SPG) in a resistant cluster headache patient, PRF of the greater occipital nerve (GON) in a cervicogenic headache patient, and PRF of the lesser occipital nerve (LON) in LON neuralgia.

AIM Paresthesia and personality disorders are common conditions among patients with fibromyalgia. However, no previous study has examined a possible relation of paresthesia with personality traits in fibromyalgia. This study investigates the frequency of paresthesia in fibromyalgia patients and its relation with personality traits. METHOD Female patients with fibromyalgia (n = 101) were divided into two groups according to the presence (n = 49; mean age 40.63 ± 7.62 years; range 23-55 years) or absence (n = 52; mean age 40.50 ± 7.12 years; range 27-53 years) of paresthesia. Also, a healthy control group (n = 53; mean age 39.34 ± 5.26 years; range 23-55 years) was included. The groups were evaluated by the Temperament and Character Inventory. Accordingly, temperament includes four dimensions: harm avoidance, novelty seeking, persistence, reward dependence; and character consists of three dimensions: cooperativeness, self-transcendence, self-directedness. RESULTS There were no significant differences among the three groups in the scores of novelty seeking, persistence, reward dependence and cooperativeness (for all P > 0.05). ÖZETMonoparezi tek ekstremitede kuvvet kaybı olarak tanımlanan nöroloji polikliniğine başvuru sebeplerinden biridir. Etyolojide; santral sinir sistemini etkileyen vasküler (iskemi, hemoraji) nedenler, yer kaplayıcı lezyonlar ve periferik sinir hastalıkları gibi nedenler yer almaktadır. Akut arteriyel oklüzyon ise nadir görülen ancak erken tanı ile sekelsiz iyileşebilen nedenlerdendir. Sol üst ekstremitede akut kuvvet kaybı yakınması ile başvuran akut arteriyel oklüzyona bağlı monoparezi olgusu; tedavi sonrasında normal kas gücü bulgularına ulaşmıştır. Monoparezi ile başvuran hastada, etyoloji araştırılırken nadir görülen sebeplerden biri olan arteryel oklüzyon acil ve nöroloji hekimlerince göz önünde bulundurulmalıdır.ABSTRACT: Monoparesis is that one extremity strength loss is defined as one of the reasons to neurology clinic. Etiology; includes vascular (ischemia, hemorrhage) causes affecting the central nervous system, locoregional lesions and peripheral nerve diseases. Acute arterial occlusion is rare, but with early diagnosis can be recovered without sequelae. Acute arterial occlusion due to monoparesis admitted with acute loss of strength in the left upper extremity symptoms after treatment of patients reached the normal muscle strength findings. In patients with arterial occlusion, which is one of the reasons monoparesis rare etiology should be considered when investigating and neurology by emergency physicians.

Objective: It is known that patients with multiple sclerosis have a high incidence of headache. Although there is increasing evidence to suggest that periaqueductal gray matter (PAG) plays a role in the pathophysiology of migraine headache, it is not known whether the type of headache may be a predictor of a MS relapse. Patients and Methods: The study enrolled 100 patients (68 females, 32 males) with clinically confirmed MS diagnosis established by McDonald diagnostic criteria. The type and duration of MS, MRI localization of lesions and cognitive status were recorded for all patients. Patients were questioned whether they experience headache during MS attacks. Results: Sixty-eight percent of the patients had headache and 32% of the patients were free of headache. Of the patients with headache, 16% had tension –type headache (TTH), 14% had migraine, 11% had primary stabbing headache (PSH), 8% had TTH+ migraine, 6% had PSH+ migraine, 6% had medication overuse headache , 2% had medication overuse headache + migraine, 2% had paroxysmal hemicrania, 1% had cervicogenic headache, 1% had chronic TTH, and 1% had unclassified headache. WOS: 000353003800007 PubMed ID: 25799899 Purpose: Behcet disease (BD) is a chronic relapsing inflammatory disorder. Neuro BD (NBD) is seen in approximately 5% of all patients. The aim of this study is to investigate the frequency, type and prognosis of epileptic seizures in different forms of NBD. Methods: All files of 42 patients with NBD were evaluated between 2006 and 2012, retrospectively. The demographic data, the presentation of NBD, clinical findings including seizures, EEG and neuroimaging findings were reviewed. Results: The mean age of patients was 35.02 +/- 8.43 years. Thirty (71.4%) patients were male; the remaining 12 of them were female. Twenty-four patients had brainstem lesions; 16 patients had cerebral venous thrombosis. Spinal cord involvement was seen in two patients. Seven patients had epileptic seizures (six partial onset seizures with or without secondary generalization). Six of them had cerebral sinus thrombosis (CVT). Four patients had a seizure as the first symptom of the thrombosis. One patient had late onset seizure due to chronic venous infarct. The other patient with seizure had brainstem involvement. The remaining was diagnosed as epilepsy before the determination of NBD. Conclusion: CVT seen in BD seems to be the main risk factor for epileptic seizures in patients with NBD. The prognosis is usually good especially in patients with CVT. Epileptic seizures in patients with brainstem involvement may be an indicator for poor prognosis. Superior sagittal thrombosis or cortical infarct would be predictor of seizures occurrence because of the high ratio in patients with seizures. (C) 2015 British Epilepsy Association. Amaç: Huzursuz bacaklar sendromu, özellikle bacaklarda ve ekstremitelerde uyuşma ve tanımlaması zor anormal bir his ile karakterizedir. Çalışmamızda kronik epilepsi hastalarında huzursuz bacaklar sendromu sıklığını saptamayı amaçladık.Yöntemler: Toplam 86 epilepsi hastası çalışmaya dahil edildi. Çalışma hastalarının epilepsi sınıflaması, 1981 International League Against Epilepsy sınıflamasına göre yapıldı. Hastaların demografik verileri toplandı ve diğer formlara ek olarak Uluslararası Huzursuz Bacaklar Sendromu Çalışma Grubu (IRLSSG) tarafından 2014 yılında yayınlanan 4 soruluk tanı kriterlerinden oluşan anketin Türkçe versiyonu hasta ile yüz yüze görüşülerek dolduruldu.Bulgular: Çalışmaya dahil edilen hastaların huzursuz bacaklar sendromu prevalansı % 5.81 (86 hasta üzerinden 5 hasta) bulundu. IRLSSG değerlendirme ölçeği ortalaması 17 ± 6,5 idi.Sonuç: Çalışmamızın sonuçları, yaygın nörolojik bir hastalık olan epilepsi hastalarında huzursuz bacaklar sendromunun prevalansının genel popülasyona göre daha düşük olduğunu gösterdi, Aim: Restless legs syndrome is characterized by an abnormal sensation, a difficult-to-define type of dysesthesia in the extremities, especially in the legs. In our study, we aimed to determine the frequency of restless legs syndrome in patients with chronic epilepsy.Methods: A total of 86 epilepsy patients were included in the study. Epilepsy grading of the study patients was performed based on the International League Against Epilepsy 1981 classification. Demographic data of the patients were collected and Turkish version of the questionnaire consisting of 4 questions of diagnostic criteria issued in 2014 by International Restless Legs Syndrome Study Group (IRLSSG), in addition to the other forms, were completed through face-to-face interviews.Results: Restless leg syndrome prevalence among the patients included in the study was found to be 5.81% (5 patients out of 86). Mean score of IRLSSG evaluation scale was 17±6.5.Conclusion: The results of our study showed that the prevalence of restless legs syndrome is lower in patients with epilepsy, a common neurologic disorder, compared to the general population.
