Abstract

BackgroundCyclin E1 (CCNE1) is a potential predictive marker and therapeutic target in tubo-ovarian high-grade serous carcinoma (HGSC). Smaller studies have revealed unfavorable associations for CCNE1 amplification and CCNE1 overexpression with survival, but to date no large-scale, histotype-specific validation has been performed. The hypothesis was that high-level amplification of CCNE1 and CCNE1 overexpression, as well as a combination of the two, are linked to shorter overall survival in HGSC.MethodsWithin the Ovarian Tumor Tissue Analysis consortium, amplification status and protein level in 3029 HGSC cases and mRNA expression in 2419 samples were investigated.ResultsHigh-level amplification (>8 copies by chromogenic in situ hybridization) was found in 8.6% of HGSC and overexpression (>60% with at least 5% demonstrating strong intensity by immunohistochemistry) was found in 22.4%. CCNE1 high-level amplification and overexpression both were linked to shorter overall survival in multivariate survival analysis adjusted for age and stage, with hazard stratification by study (hazard ratio [HR], 1.26; 95% CI, 1.08-1.47, p = .034, and HR, 1.18; 95% CI, 1.05-1.32, p = .015, respectively). This was also true for cases with combined high-level amplification/overexpression (HR, 1.26; 95% CI, 1.09-1.47, p = .033). CCNE1 mRNA expression was not associated with overall survival (HR, 1.00 per 1-SD increase; 95% CI, 0.94-1.06; p = .58). CCNE1 high-level amplification is mutually exclusive with the presence of germline BRCA1/2 pathogenic variants and shows an inverse association to RB1 loss.ConclusionThis study provides large-scale validation that CCNE1 high-level amplification is associated with shorter survival, supporting its utility as a prognostic biomarker in HGSC.

Abstract

To mitigate the significant biases/errors in research studying the associations between PM and health, which are introduced by the coarse/inadequate assessments of PM exposure from conventional PM monitoring paradigm, a personalized monitoring system consisting of a low-cost wearable PM device is proposed. However, due to the absence of a unifying evaluation protocol for low-cost PM sensors, the evaluation results/performance specifications from existing studies/datasheets are of limited reference values when attempting to determine the best candidate for the proposed system. In this regard, the authors appeal to the research community to develop a standardized evaluation protocol for low-cost PM sensors/devices, and a unifying attempt is established in this manuscript by adopting the definitive terminology from international documents and the evaluation metrics regarded as best practices. Collocated on the rooftop of the HKUST Supersite, four empirically selected PM sensors were compared against each other and calibrated against two reference monitors. They were then evaluated against the reference following the protocol. The PlanTower PMS-A003 sensor was selected for the wearable device as it outperformed the others in terms of affordability, portability, detection capability, data quality, as well as humidity and condensation insusceptibility. An automated approach was proposed to identify and remove the condensation associated abnormal measurements. The proposed device has better affordability and portability as well as similar usability and data accessibility compared to those existing devices recognized. The first 10 devices were also evaluated and calibrated at the Supersite. Additional 120 units were manufactured and delivered to the subjects to acquire their daily PM2.5 exposures for investigating the association with subclinical atherosclerosis.

3. Towards Personalized Healthcare in Cardiac Population: The Development of a Wearable ECG Monitoring System, an ECG Lossy Compression Schema, and a ResNet-Based AF Detector

Abstract

Cardiovascular diseases (CVDs) are the number one cause of death worldwide. While there is growing evidence that the atrial fibrillation (AF) has strong associations with various CVDs, this heart arrhythmia is usually diagnosed using electrocardiography (ECG) which is a risk-free, non-intrusive, and cost-efficient tool. Continuously and remotely monitoring the subjects' ECG information unlocks the potentials of prompt pre-diagnosis and timely pre-treatment of AF before the development of any life-threatening conditions/diseases. Ultimately, the CVDs associated mortality could be reduced. In this manuscript, the design and implementation of a personalized healthcare system embodying a wearable ECG device, a mobile application, and a back-end server are presented. This system continuously monitors the users' ECG information to provide personalized health warnings/feedbacks. The users are able to communicate with their paired health advisors through this system for remote diagnoses, interventions, etc. The implemented wearable ECG devices have been evaluated and showed excellent intra-consistency (CVRMS=5.5%), acceptable inter-consistency (CVRMS=12.1%), and negligible RR-interval errors (ARE<1.4%). To boost the battery life of the wearable devices, a lossy compression schema utilizing the quasi-periodic feature of ECG signals to achieve compression was proposed. Compared to the recognized schemata, it outperformed the others in terms of compression efficiency and distortion, and achieved at least 2x of CR at a certain PRD or RMSE for ECG signals from the MIT-BIH database. To enable automated AF diagnosis/screening in the proposed system, a ResNet-based AF detector was developed. For the ECG records from the 2017 PhysioNet CinC challenge, this AF detector obtained an average testing F1=85.10% and a best testing F1=87.31%, outperforming the state-of-the-art.

Abstract

PurposeAdvanced-stage mucinous ovarian carcinoma (MOC) has poor chemotherapy response and prognosis and lacks biomarkers to aid stage I adjuvant treatment. Differentiating primary MOC from gastrointestinal (GI) metastases to the ovary is also challenging due to phenotypic similarities. Clinicopathologic and gene-expression data were analyzed to identify prognostic and diagnostic features.Experimental designDiscovery analyses selected 19 genes with prognostic/diagnostic potential. Validation was performed through the Ovarian Tumor Tissue Analysis consortium and GI cancer biobanks comprising 604 patients with MOC (n = 333), mucinous borderline ovarian tumors (MBOT, n = 151), and upper GI (n = 65) and lower GI tumors (n = 55).ResultsInfiltrative pattern of invasion was associated with decreased overall survival (OS) within 2 years from diagnosis, compared with expansile pattern in stage I MOC [hazard ratio (HR), 2.77; 95% confidence interval (CI), 1.04-7.41, P = 0.042]. Increased expression of THBS2 and TAGLN was associated with shorter OS in MOC patients (HR, 1.25; 95% CI, 1.04-1.51, P = 0.016) and (HR, 1.21; 95% CI, 1.01-1.45, P = 0.043), respectively. ERBB2 (HER2) amplification or high mRNA expression was evident in 64 of 243 (26%) of MOCs, but only 8 of 243 (3%) were also infiltrative (4/39, 10%) or stage III/IV (4/31, 13%).ConclusionsAn infiltrative growth pattern infers poor prognosis within 2 years from diagnosis and may help select stage I patients for adjuvant therapy. High expression of THBS2 and TAGLN in MOC confers an adverse prognosis and is upregulated in the infiltrative subtype, which warrants further investigation. Anti-HER2 therapy should be investigated in a subset of patients. MOC samples clustered with upper GI, yet markers to differentiate these entities remain elusive, suggesting similar underlying biology and shared treatment strategies.

Abstract

Tubo-ovarian high-grade serous carcinomas (HGSC) are highly proliferative neoplasms that generally respond well to platinum/taxane chemotherapy. We recently identified minichromosome maintenance complex component 3 (MCM3), which is involved in the initiation of DNA replication and proliferation, as a favorable prognostic marker in HGSC. Our objective was to further validate whether MCM3 mRNA expression and possibly MCM3 protein levels are associated with survival in patients with HGSC. MCM3 mRNA expression was measured using NanoString expression profiling on formalin-fixed and paraffin-embedded tissue (N = 2355 HGSC) and MCM3 protein expression was assessed by immunohistochemistry (N = 522 HGSC) and compared with Ki-67. Kaplan-Meier curves and the Cox proportional hazards model were used to estimate associations with survival. Abstract

Bayesian approach, as a useful tool for quantifying uncertainties, has been widely used for solving inverse problems of partial differential equations (PDEs). One of the key difficulties for employing Bayesian approach for the issue is how to extract information from the posterior probability measure. Variational Bayes' method (VBM) is firstly and broadly studied in the field of machine learning, which has the ability to extract posterior information approximately by using much lower computational resources compared with the conventional sampling type methods. In this paper, we generalize the usual finite-dimensional VBM to infinite-dimensional space, which makes the usage of VBM for inverse problems of PDEs rigorously. We further establish general infinite-dimensional mean-field approximate theory, and apply this theory to abstract linear inverse problems with Gaussian and Laplace noise assumptions. Abstract

The 3-D total variation (3DTV) is a powerful regularization term, which encodes the local smoothness prior structure underlying a hyper-spectral image (HSI), for general HSI processing tasks. This term is calculated by assuming identical and independent sparsity structures on all bands of gradient maps calculated along spatial and spectral HSI modes. This, however, is always largely deviated from the real cases, where the gradient maps are generally with different while correlated sparsity structures across all their bands. Such deviation tends to hamper the performance of the related method by adopting such prior term. To this is- sue, this paper proposes an enhanced 3DTV (E-3DTV) regularization term beyond conventional 3DTV. Instead of imposing sparsity on gradient maps themselves, the new term calculated sparsity on the subspace bases on the gradient maps along their bands, which naturally encode the correlation and difference across these bands, and more faithfully reflect the insightful configurations of an HSI. The E-3DTV term can easily replace the previous 3DTV term and be em- bedded into an HSI processing model to ameliorate its performance. Abstract

Hyperspectral images (HSIs) are often corrupted by a mixture of several types of noise during the acquisition process, e.g., Gaussian noise, impulse noise, dead lines, stripes, and many others. Such complex noise could degrade the quality of the acquired HSIs, limiting the precision of the subsequent processing. In this paper, we present a novel tensor-based HSI restoration approach by fully identifying the intrinsic structures of the clean HSI part and the mixed noise part respectively. Specifically, for the clean HSI part, we use tensor Tucker decomposition to describe the global correlation among all bands, and an anisotropic spatial-spectral total variation (SSTV) regularization to characterize the piecewise smooth structure in both spatial and spectral domains. For the mixed noise part, we adopt the $\ell_1$ norm regularization to detect the sparse noise, including stripes, impulse noise, and dead pixels. Despite that TV regulariztion has the ability of removing Gaussian noise, the Frobenius norm term is further used to model heavy Gaussian noise for some real-world scenarios. Then, we develop an efficient algorithm for solving the resulting optimization problem by using the augmented Lagrange multiplier (ALM) method. Finally, extensive experiments on simulated and real-world noise HSIs are carried out to demonstrate the superiority of the proposed method over the existing state-of-the-art ones., Comment: 15 pages, 20 figures

18. Multifractal analyses of daily rainfall time series in Pearl River basin of China

Abstract

The multifractal properties of daily rainfall time series at the stations in Pearl River basin of China over periods of up to 45 years are examined using the universal multifractal approach based on the multiplicative cascade model and the multifractal detrended fluctuation analysis (MF-DFA). The results from these two kinds of multifractal analyses show that the daily rainfall time series in this basin have multifractal behavior in two different time scale ranges. It is found that the empirical multifractal moment function $K(q)$ of the daily rainfall time series can be fitted very well by the universal mulitifractal model (UMM). The estimated values of the conservation parameter $H$ from UMM for these daily rainfall data are close to zero indicating that they correspond to conserved fields. After removing the seasonal trend in the rainfall data, the estimated values of the exponent $h(2)$ from MF-DFA indicate that the daily rainfall time series in Pearl River basin exhibit no long-term correlations. It is also found that $K(2)$ and elevation series are negatively correlated. Abstract

A new method is proposed in this paper to learn overcomplete dictionary from training data samples. Differing from the current methods that enforce similar sparsity constraint on each of the input samples, the proposed method attempts to impose global sparsity constraint on the entire data set. This enables the proposed method to fittingly assign the atoms of the dictionary to represent various samples and optimally adapt to the complicated structures underlying the entire data set. By virtue of the sparse coding and sparse PCA techniques, a simple algorithm is designed for the implementation of the method. The efficiency and the convergence of the proposed algorithm are also theoretically analyzed. Based on the experimental results implemented on a series of signal and image data sets, it is apparent that our method performs better than the current dictionary learning methods in original dictionary recovering, input data reconstructing, and salient data structure revealing., Comment: 27 pages, 9 figures, 1 table

Abstract

Pregnancies with a complete hydatidiform mole and co‐existing fetus (CMCF) are rare, but increasingly common due to the rising prevalence of assisted reproductive technology. They are frequently associated with adverse obstetric outcomes, providing women with the challenge of pregnancy termination or continuing the pregnancy at the risk of maternal‐fetal morbidity and fetal mortality. This report demonstrates two cases of CMCF pregnancy with excellent maternal‐fetal outcomes, including spontaneous resolution of the molar tissue antenatally. It is helpful in counselling women who are diagnosed with this rare and frequently morbid condition in considering how to proceed with their pregnancy. [ABSTRACT FROM AUTHOR]
