Abstract

We obtained Wisconsin Card Sorting Test (WCST) scores on 91 (35 male and 56 female) healthy, well-educated individuals aged 45 to 83. Women scored better than men on six WCST scores (categories, errors, perseverative responses, % perseverative errors, % conceptual level responses, and trials to first category), while subjects with more than 16 years of education outscored those limited to high school education on four measures (perseverative responses, errors, % perseverative errors, % conceptual level responses). Individuals older than 70 years of age scored less well than younger subjects on only two indices (errors, % conceptual level responses). Full Scale IQ was not related to WCST scores. Our findings suggest that WCST scores should be interpreted within the context of patient gender, education, and age.

Abstract

Objective: The authors' goal was to evaluate the relationship between plasma concentrations of alprazolam and both treatment response and side effects in patients with panic disorder and agoraphobia. Method: Ninety-six patients with panic disorder and agoraphobia were treated at three sites in a 6-week, fixed-dose, double-blind, placebo-controlled, dose-response study of 2 mg/day or 6 mg/day of alprazolam. Assessments were made of panic attacks, avoidance behavior, generalized anxiety, and global response. Blood samples were collected throughout the study and analyzed for alprazolam and other benzodiazepines. Results: Patient compliance with the protocol was judged to be good on the basis of plasma concentrations. According to logistic regression analysis, the relationships between plasma alprazolam concentration and response, as reflected by number of panic attacks reported, phobia ratings, physicians' and patients' ratings of global improvement, and the emergence of side effects, were significant. However, there was no significant relationship between plasma alprazolam concentration and the degree of generalized anxiety symptoms. Conclusions: The authors conclude that plasma concentration of alprazolam is related to treatment response, particularly in panic attacks. The alprazolam concentration associated with treatment response or with emergence of a given side effect varied widely among individuals, highlighting the necessity for individualized dose adjustment to obtain optimal treatment response while minimizing side effects. Abstract

Whether or not true psychosis (e.g., schizophrenia) can develop later in life remains controversial. There have been many studies during this century but the phenomenon remains poorly understood. With more advanced technology, evidence is accumulating that brain disease or structural damage may contribute to, if not cause, what has been called late-life psychosis (LLP). LLP is not considered a form of schizophrenia or other psychiatric illness. It is possible that brain lesions may not have been detected in prior research with less reliable diagnostic techniques. To study the incidence of structural brain damage in those with LLP, psychiatric, neurological, and neuroimaging (e.g. magnetic resonance imaging, or MRI) tests were performed on 24 patients whose first psychotic episode occurred after the age of 45. There were 72 elderly controls as well, who received the same battery of tests. It was found that 42 percent of the subjects with LLP had structural brain abnormalities and 58 percent suffered from neuromedical illness (nerve diseases). Intellectual and memory deficits were common as well. There was nothing in the findings that would have predicted the structural abnormalities found with MRI. It is important that this type of assessment be made on patients who appear to develop psychotic disorders late in life, as some causes of structural brain abnormalities may be reversible.
