12. Guidelines on radioiodine therapy for differentiated thyroid carcinoma

Abstract

SummaryAim: For the examination of the impact on clinical practice of the guidelines for differentiated thyroid carcinoma (DTC), treatment data from the ongoing Multicenter Study Differentiated Thyroid Carcinoma (MSDS) were analyzed. Patients, methods: Patients were randomized to adjuvant external beam radiotherapy (RTx) or no RTx in addition to standard therapy in TNM stages pT4 pN0/1/x M0/x (UICC, 5th ed. 1997). All patients were to receive the same treatment regimen consisting of thyroidectomy, ablative radioiodine therapy (RIT), and a diagnostic 131I whole-body scintigraphy (WBS) 3-4 months after RIT. Results: Of 339 eligible patients enrolled between January 2000 and March 2004, 273 could be analyzed. Guideline recommendations by the German Society for Nuclear Medicine from 1999 and 1992 were complied with within 28% and 82% with regard to the interval between surgery and RIT (4 vs. 4–6 weeks), in 33% and 84% with regard to 131I activity for RIT (1-3 vs. 1-4 GBq; ± 10%), and in 16% and 60% with regard to 131I activity for WBS (15319-300 vs. 15319-400 MBq; ± 10%). Conclusions: The 1999 guideline revision appears to have had little impact on clinical practice. Further follow-up will reveal if guideline compliance had an effect on outcomes.

Abstract

Ziel, Methode: In einer retrospektiven Analyse wurden die wissenschaftlichen Veroffentlichungen der Zeitschrift Nuklearmedizin. Journal of Functional and Molecular Imaging der Jahrgange 2001 und 2002 hinsichtlich der inhaltlichen Zuordenbarkeit zu molekularer Bild-gebung/ Therapie ausgewertet. Ergebnisse: Von 82 Ori-ginalarbeiten und Kasuistiken wurden 57% als mole-kulare Bildgebung, 11% als molekulare Therapie, 12% als funktionelle Bildgebung und 2% als funktionelle Therapie klassifiziert. In der Summe verwendeten 83% der Arbeiten eine molekulare oder funktionelle methodi-sche Basis. Von den Veroffentlichungen mit molekularer oder funktioneller methodischer Basis verwendeten 91% ein etabliertes Verfahren. Der Anteil experimentel-ler Arbeiten betrug 3%. Unter Mitautorenschaft anderer medizinischer Fachrichtungen entstanden 71% der molekularen/funktionellen Arbeiten, dabei 16% mit Autoren von medizinischen Grundlagenfachern. Von den Originalarbeiten dienten 51% vorwiegend der metho-dischen Entwicklung, 49% waren prospektiv angelegt. Schlussfolgerung: Die Zeitschrift Nuklearmedizin. Jour-nal of Functional and Molecular Imaging wird dem im Untertitel dokumentierten Anspruch der molekularen und funktionellen Bildgebung gerecht. Die Integration anderer Fachrichtungen zeigt, dass molekulare Bild-gebung verwirklichte Integration der Disziplinen ist. Eine starkere Einbeziehung der medizinischen Grundlagen-facher ist jedoch unabdingbar. Die Nuklearmedizin tragt zum Verstandnis molekularer Bildgebung bei und damit zum molekular orientierten Denken, insbesondere in der Ubertragung und Anwendung am Menschen.

Abstract

In this work, n-type triple-crate metal-oxide-semiconductor field effect transistors (MOSFETs) are presented, where laser interference lithography (LIL) is integrated into a silicon-on-insulator (SOI) CMOS process to provide for the critical definition of the transistor channels. A mix and match process of optical contact lithography and LIL is developed to achieve device relevant structures. Abstract

The Pliocene lignite hypothesis is an environmental hypothesis that has been proposed to explain the etiology of Balkan endemic nephropathy (BEN). Aqueous leaching experiments were conducted on a variety of coal samples in order to simulate groundwater leaching of organic compounds, and to further test the role of the Pliocene lignite hypothesis in the etiology of BEN. Experiments were performed on lignite coal samples from endemic BEN areas in Romania and Serbia, and lignite and bituminous coals from nonendemic regions in Romania and the USA. Room temperature, hot water bath, and Soxhlet aqueous extraction experiments were conducted between 25 and 80 °C, and from 5 to 128 days in duration. A greater number of organic compounds and in higher concentrations were present in all three types of leaching experiments involving endemic area Pliocene lignite samples compared to all other coals examined. A BEN causing molecule or molecules may be among phenols, PAHs, benzenes, and/or lignin degradation compounds. The proposed transport pathway of the Pliocene lignite hypothesis for organic compound exposure from endemic area Pliocene lignite coals to well and spring drinking water, is likely. Aromatic compounds leached by groundwater from Pliocene lignite deposits in the vicinity of endemic BEN areas may play a role in the etiology of the disease. A better understanding of organic compounds leached by groundwater from Pliocene lignite deposits may potentially lead to the identification and implementation of effective strategies for the prevention of exposure to the causative agent(s) for BEN, and in turn, prevention of the disease. 50. Mutual Synchronization of Molecular Turnover Cycles in Allosteric Enzymes III. Intramolecular Cooperativity

Abstract

In small micrometer volumes typical of living cells, regulatory molecules of enzymic reactions can diffuse so fast from one enzyme molecule to another that the diffusion time is much shorter than the turnover time of an enzyme. Under these conditions, a special kinetic regime of a molecular network is realized. We consider molecular networks formed by allosteric enzymes with several functional subunits, interactions between which are described by a sequential model of Koshland et al. with heterotropic or homotropic (positive and negative) cooperativities. Simple product-activated and product-inhibited reactions are investigated. We show that allosteric cross-molecular regulation and intramolecular cooperativity result in the development of coherent dynamics of enzyme molecules in such networks. Intermolecular synchronization of turnover cycles of different enzymes is accompanied by intramolecular synchronization of the dynamics of subunits belonging to the same molecules. Strong intramolecular correlations persist even at high intensity of thermal molecular fluctuations, when intermolecular synchronization is already absent.
