The World Oral Maxillofacial Trauma (WORMAT) project was performed to analyze the causes and characteristics of maxillofacial fractures managed in 14 maxillofacial surgery divisions over a 1-year period.The following data were collected: age, sex, cause and mechanism of maxillofacial fracture, alcohol and/or drug abuse at the time of trauma, fracture site, Facial Injury Severity Scale score (FISS), associated injury, day of trauma, timing and type of treatment, and length of hospitalization. Statistical analyses were performed using SPSS software.Between 30 September 2019 and 4 October 2020, 2,387 patients (1,825 males and 562 females [ratio 3.2:1], 47.6% aged 20-39 years [mean age 37.2 years, median 33.0 years]) were hospitalised. The main cause of maxillofacial fracture was road traffic accidents (RTA), which were statistically associated with male adults as like as assault, sport, and work (p0,05). Half of the fractures involved the middle third of the face, statistically associated with fall and assault (p0.05). Trauma in multiple locations was significantly associated with longer hospital stay (p0.05). The mean length of hospitalization was 3.9 days (95% Confidence Interval 3.7-4.2).This prospective, multicenter epidemiological study confirmed that young adult males were the ones most commonly affected by maxillofacial fracture. RTAs and assaults are statistically associated with the adult population, while falls are associated with females and older population.

Editorial Caro(a) Leitor(a), O 10º Congresso de Odontologia da Faculdade de Odontologia de Araçatuba – Unesp, a 40ª Jornada Acadêmica, 16º Simpósio de Pós-graduação e o 12º Encontro do CAOE ocorreram nos dias 12,13 e 14 de agosto de 2021, pela primeira vez no formato remoto, por meio da plataforma Google Meet. Os homenageados foram o Professor Associado Marcos Rogério de Mendonça emprestando seu nome para o Congresso, o professor Titular Célio Percinoto como nome da Jornada e, o professor Titular Valdir Gouveia Garcia como nome do Simpósio de Pós-Graduação. Os eventos contaram com a participação de alunos da graduação, pós-graduação e professores nas comissões organizadoras. O público-alvo do evento incluiu estudantes da graduação em odontologia e áreas afins, cirurgiões-dentistas, técnicos e assistentes em saúde bucal e pós-graduação na área de Odontologia e demais áreas afins, docentes e pesquisadores de todo o Brasil. Foram realizadas 453 adesões ao Congresso e 401 inscrições de trabalhos científicos para apresentação durante o congresso, incluindo as regiões sudeste, sul, centro-oeste, nordeste e norte do país. Foram oferecidos 23 de cursos. Para todas as categorias, painel ou oral, houve menções honrosas premiando os trabalhos. Além disso, os três melhores trabalhos foram premiados com uma caixa básica de instrumentos básicos para cirurgia bucal fornecidos pela empresa LM. O Prêmio Eugênio Zerlotti, em que os participantes apresentaram e arguiram seus trabalhos na língua inglesa foram premiados com valores em dinheiro para as categorias clínico e pesquisa, graduação e pós-graduação. The purpose of this prospective multicenter study was to analyze the epidemiology, patterns, and management of maxillofacial fractures due to road traffic accidents (RTAs) worldwide.Between Monday September 30, 2019 and Sunday October 4, 2020,1066 patients with RTAs related fractures were admitted to 14 maxillofacial surgery departments. The following data were analyzed: age, gender, mechanism of injury, alcohol or drug abuse at the time of trauma, maxillofacial fracture site, facial injury severity scale (FISS) score, associated injuries, day and month of trauma, time of treatment, type of treatment and length of hospital stay. Data were analyzed using bivaried and multivaried statistical analysis.Eight hundred seventy patients were male, and 196 were female. The most common mechanism of injury was motorcycle accidents (48%). More than half of the patients had fractures of the middle third of the maxillofacial skeleton. In total, 59% of the study sample underwent open reduction internal fixation. The median facial injury severity scale (3 points) and the medial hospital stay (3 days) were significantly lower in patients with seatbelts and helmet (P 0.001).This first prospective, multicenter epidemiological study shows that motorcycle accidents are the leading cause of RTAs related fractures, mostly in young males. Particularly in Australia and Europe, the incidence of RTAs was significantly lower. Moreover, this study found that the severity of maxillofacial lesions was significantly higher in patients without safety devices, with consequent longer hospital stay demonstrating the efficacy of road safety policies in preventing maxillofacial injury.

Este estudo teve como objetivo, apresentar por meio de um relato de caso a interação entre odontologia restauradora e prótese dentária, orientando ao clínico uma opção de conduta clínica minimamente invasiva no tratamento de reabilitação oral. Paciente do sexo masculino, 49 anos, com queixa principal de mobilidade nos dentes anteriores superiores que sustentavam uma prótese fixa e insatisfação estética do sorriso. O exame clínico foi realizado e constatou-se situação periodontal comprometida de todos os dentes superiores, periodontite com grau 2 de mobilidade e reabsorção óssea vertical severa nos dentes 31, 32, 37 e 42, ausência dos elementos 36 e 41 e mobilidade grau 2 nos demais dentes inferiores em função da má oclusão. Foi planejada extração dentária dos dentes superiores, seguida de levantamento de seio bilateral e instalação de seis implantes para prótese do tipo protocolo de Banemärk. No arco inferior, raspagem e alisamento radicular, extração dos dentes 31, 32, 37 42, instalação de implantes nas regiões dos dentes 32, 36 e 42 e instalação de prótese implantosuportada. Considerando a necessidade de reestabelecer a oclusão, a condição periodontal comprometida dos dentes inferiores posteriores e a necessidade de melhorias estéticas, foi optado pela realização de restaurações em resina composta do tipo BulkFill® através da técnica do carimbo modificado, reproduzindo a anatomia oclusal dos elementos. Dessa forma, podemos concluir que a interação da odontologia restauradora e a prótese dentária pode ser uma alternativa interessante para casos em que há necessidade de tratamento reabilitador conservador, visando obter resultado de excelência na reabilitação oral

La Lipectomía Oral es un procedimiento médico-odontológico que tiene como objetivo corregir el volumen del cuerpo adiposo de la mejilla que de alguna manera está generando implicaciones clínicas desfavorables para el sistema estomatognático, siendo por motivos estéticos faciales, causa de traumatismo en la mucosa juiciosa, aparición de hernia contenido de grasa en la cavidad oral, o como resultado de patologías como los lipomas. Así, este procedimiento quirúrgico puede realizarse con el objetivo de eliminar parte del contenido de grasa en la región indicada para el procedimiento quirúrgico. Sin embargo, en muchos casos este procedimiento puede estar acompañado de complicaciones intraoperatorias como hemorragias o traumatismos en el conducto de la glándula parótida o estructuras nerviosas; así como, pueden presentarse complicaciones postoperatorias como: edema, trismo, enfisema, seroma, hematoma e infecciones. A Lipectomia Bucal é um procedimento médico-odontológico que visa a correção do volume do corpo adiposo da bochecha que esteja de alguma forma gerando implicações clínicas desfavoráveis ao aparelho estomatognático, sendo por motivos estéticos faciais, causa de traumas em mucosa julgal, ocorrência de herniação traumática do conteúdo gorduroso para a cavidade bucal, ou em decorrência de patologias como caso de lipomas. Desse modo, esse procedimento cirúrgico pode ser realizado visando a remoção de parte do conteúdo gorduroso na região indicada para o procedimento cirúrgico. Contudo, em muitos casos esse procedimento pode ser acompanhado de intercorrências trans-operatórias como hemorragias ou traumatismos como ao ducto da glândula parótida ou estruturas nervosas; assim como, podem ocorrer complicações pós-operatórias como: edema, trismo, enfisema, seroma, hematoma e infecções. Desse modo, o cirurgião deve estar familiarizado com a técnica cirúrgica utilizada e esses procedimentos devem ser realizados e acompanhados com extrema cautela para minimizar as possíveis complicações e otimizar seus tratamentos.

Introdução: O cisto ósseo traumático possui diversas nomenclaturas e é classificado como um pseudocisto, por não apresentar cápsula e revestimento epitelial. Acomete principalmente pacientes jovens na região posterior de mandíbula, podendo este ter histórico de trauma. Por serem assintomáticos, muitas vezes são diagnosticados em exames radiográficos de rotina. A exploração da cavidade é indicada tanto para diagnóstico, após analise histopatológica, quanto para terapêutica. Objetivo: O objetivo desse trabalho é apresentar as características de cisto ósseo traumático para diagnostico e tratamento, a partir do relato de um caso de dois cistos ósseos em regiões de menor incidência na mandíbula. Relato de caso: Paciente de 15 anos foi diagnosticado com duas lesões de cisto ósseo traumático, após uma radiografia de rotina para tratamento ortodôntico. As lesões eram de aproximadamente 10 mm sendo duas cavidades no mento e de 15mm na região do côndilo esquerdo e assintomáticas. Para o diagnóstico e tratamento, foi realizado uma aspiração na lesão do mento e posteriormente exploração cirúrgica em ambas lesões; e mesmo observando certa escassez de tecido para a exploração, uniu-se o exame com as características clinicas do paciente e obteve-se um diagnóstico definitivo de cisto ósseo traumático. O mesmo passou por acompanhamento clínico-radiográfico e pode ser observada regressão da lesão. Conclusão: Se faz importante o conhecimento das características dessa lesão para correto diagnóstico e tratamento, mesmo que o tratamento seja feito somente com exploração cirúrgica e proservação.

O terço médio da face é funcional e esteticamente importante. De acordo com a classificação Le Fort, existem três níveis mais fracos desta região da face quando traumatizados a partir de uma direção frontal, sendo que os acidentes motociclísticos, atualmente, correspondem a causa de aproximadamente 29% destes traumas. O presente trabalho tem como objetivo relatar um caso clínico de tratamento cirúrgico de fraturas do tipo Le Fort I e Le Fort II em um paciente de 29 anos de idade, sexo masculino, vítima de acidente motociclístico, atendido no Hospital de Emergência e Trauma Senador Humberto Lucena (João Pessoa – PB). Ao exame físico observou-se mobilidade de maxila, degrau palpável em pilar zigomático e pilar canino, alteração oclusal com leve mordida aberta e degrau em rebordo infraorbitário direito, entretanto o paciente não apresentava nenhuma alteração ocular. Foi solicitada tomografia computadorizada como exame complementar para confirmação do diagnóstico e planejamento cirúrgico, o qual se deu como fratura Le Fort I e Le Fort II no lado direito. O paciente foi submetido à cirurgia sob anestesia geral para fixação dos pilares zigomático e canino através do acesso vestibular maxilar e rebordo infraorbitário através do acesso subciliar. Inicialmente foi feito o bloqueio maxilo – mandibular para a utilização da oclusão como ponto de referência, seguido da redução das fraturas e fixação com placas e parafusos do sistema 2.0. Sob acompanhamento pós – operatório o paciente apresentou retorno da oclusão dentro dos padrões de normalidade, recuperou a projeção da região zigomática fraturada e então recebeu alta. The aim of this study was to perform a systematic review and meta-analysis to evaluate the efficacy of laser therapy on medication-related osteonecrosis of the jaw (MRONJ) treatment. This study followed PRISMA standards, and an electronic search was performed on the PubMed/MEDLINE, Scopus, and Cochrane databases. Eighty-nine articles were found. After reading the manuscripts, 15 articles remained for the review. Three of them were selected for meta-analysis. Female gender was predominant (72.5%), and mean age was 66.5 years. Follow-up varied between 3 and 80 months, and the most used bisphosphonate was zoledronic acid (71.6%). Stage II of MRONJ was the most prevalent (68.9%), and the mandible was the most affected site (64.5%). Qualitative data showed that treatment with laser surgery (Er:YAG) was most effective regarding complete healing of the lesion (90%) compared with other treatments. Meta-analysis data showed that low-level laser therapy (LLLT) was more effective than medical treatment (P = 0.006), and surgical treatment was more effective than LLLT (P = 0.008). Surgical management of impacted third molars is a treatment performed by dentists and its indicated for various conditions, among the indications has the prevention of mandibular fracture, but this is a factor that has been discussed and is controversial. Fractures caused by a third molar can occur, and on the other hand, the fractures after or during the management of impacted third molar are a possible complication. Surgical planning as the indication to the surgery has to be performed to avoid complications as the fracture. Thus this study aims to report a case about a mandible fracture after third molar extraction, presenting edema, pain, and restriction in mandible mobility in 01-day follow-up. A 27-year-old female was referred to the oral and maxillofacial surgery service of the Santa Casa Hospital of Araçatuba (Araçatuba, Brazil), reporting dental history of third molar removal, with immediately chief complaints. In the assessment, the patient presented edema, pain, malocclusion, and restriction in mandible mobility, and the panoramic X-ray showed a mandible fracture with displacement. The patient underwent surgery under general anesthesia to open reduction with internal fixation (ORIF) through an intraoral approach. On the follow-up, the patient showed an improvement in mouth opening and mandibular mobility without pain or infection signs. The panoramic X-Rays showed us the bone fracture healing. El manejo quirúrgico de los terceros molares impactados es un tratamiento común que realizan los dentistas y está indicado para diversas afecciones, entre las indicaciones tiene la prevención de la fractura mandibular, pero este es un factor que ha sido discutido y es controvertido. Pueden ocurrir fracturas causadas por la presencia del tercer molar por otro lado, las fracturas después o durante el manejo del tercer molar impactado son una posible complicación. Por tanto, la planificación quirúrgica, como la indicación de la cirugía, debe realizarse para evitar complicaciones como la fractura. El objetivo de este estudio es reportar un caso sobre una fractura de mandíbula luego después de la extracción del tercer molar, presentando en el seguimiento de 01 día edema, dolor y restricción en la movilidad mandibular. Una mujer de 27 años fue remitida al servicio de cirugía oral y maxilofacial del Hospital Santa Casa de Araçatuba (Araçatuba, Brasil), informando que se sometió a la extracción del tercer molar, e inmediatamente presentó las quejas principales y mantuvo el edema, dolor, malloclusión y restricción de la movilidad mandibular. En radiografía presentó fractura de mandíbula con desplazamiento. Tras el diagnóstico, el paciente fue intervenido quirúrgicamente bajo anestesia general para reducción abierta con fijación interna (ORIF) mediante abordaje intraoral. En el seguimiento, la paciente presenta una mejora en su capacidad para abrir la boca y la movilidad mandibular sin signos de dolor o infección y la radiografía panorámica nos muestra una curación de la fractura ósea. O tratamento cirúrgico dos terceiros molares inclusos é um tratamento comumente realizado pelos dentistas e indicado em diversas condições. A prevenção de fratura mandibular está entre as indicações para a realização da exodontia, mas este é um fator que vem sendo discutido e é controverso. Fraturas causadas pela presença do terceiro molar podem ocorrer e, por outro lado, as fraturas após ou durante o procedimento cirúrgico do terceiro molar incluso são uma possível complicação. Portanto, o planejamento cirúrgico, assim como a indicação da cirurgia tem que ser realizado cuidadosamente para evitar complicações como a fratura. O objetivo deste estudo é relatar um caso de fratura de mandíbula após exodontia de terceiros molares, apresentando no acompanhamento edema, dor e restrição dos movimentos mandibulares. Paciente do gênero feminino, 27 anos, foi encaminhada ao serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial da Santa Casa de Araçatuba (Araçatuba, Brasil), relatando ter sido submetida a exodontia de terceiro molar, e imediatamente apresentou queixas importantes. Na avaliação a paciente apresentou edema, dor, maloclusão e restrição nos movimentos mandibulares, e na radiografia apresentou fratura de mandíbula com deslocamento. Após o diagnóstico, a paciente foi submetida à cirurgia sob anestesia geral para redução aberta com fixação interna (ORIF) por acesso intra-oral. No acompanhamento a paciente apresentou melhora da abertura bucal e dos movimentos mandibulares sem sinais de dor ou infecção e as radiografias panorâmicas nos mostraram sinais de reparo da região da fratura óssea.

The aim of this multicentric prospective study was to understand and evaluate the incidences, causes and patterns of oral and maxillofacial injuries in patients aged over 60 years admitted to 14 maxillofacial surgical departments around the world. Methods: The following data were collected: gender, cause and mechanism of maxillofacial fracture, alcohol and drug abuse at the time of trauma, fracture site, Facial Injury Severity Scale score, associated injury, day of trauma, timing and type of treatment and length of hospitalization. Statistical analyses were performed using non-parametric and association tests, as well as linear regression. Results: Between 30 September 2019 and 4 October 2020, 348 out of 2387 patients (14.6%), 197 men and 151 women (ratio 1.3:1; mean age 72.7 years), were hospitalized. The main causes of the maxillofacial fractures were falls (66.4%), followed by road traffic accidents (21.5%) and assaults (5.2%). Of the 472 maxillofacial fractures, 69.7% were in the middle third of the face, 28% in the lower third and 2.3% in the upper third. Forty 129/Sv female mice (64–68 weeks old), 20 wild type (WT) and 20 5-LO knockout (5LOKO) were equally distributed according to ZL treatment: WT Control, WT ZL, 5LOKO Control, and 5LOKO ZL. ZL groups were treated with an intraperitoneal injection of 250 µg/Kg of ZL, while controls were treated with saline. Treatments were administered once a week, starting four weeks before surgery for tooth extraction and until 7 and 21 days post-surgery. Mice were euthanized for a comprehensive microscopic analysis (microCT, histomorphometry and immunohistochemistry). WT ZL mice presented intense inflammatory infiltrate (7 days), delayed bone formation (21 days), reduced collagenous matrix quality, and a deficiency in Runx-2 + , TRAP + , and macrophages as compared to controls. 5LOKO ZL animals presented decreased number of Runx-2 + cells in comparison to 5LOKO Control at 7 days, but no major changes in bone healing as compared to WT or 5LOKO mice at 21 days. The knockout of 5LO favored intramembranous bone healing in aged female mice, with a direct impact on inflammatory response and bone metabolism on the development of ONJ-like lesions. Ti6Al4V alloy disks (n = 5) were dynamically exposed to mouthwashes (0.12% chlorhexidine digluconate [CHX], cetylpyridinium chloride [CC], and hydrogen peroxide [HP]) by immersion 3 times a day for 1 min. Artificial saliva [AS] is a control solution. The electrochemical behavior of the Ti6Al4V (n = 5) was tested at the baseline, and after 7 and 14 days of mouthwashes dynamic action. Surfaces characteristics were investigated using scanning electron microscopy (SEM), energy-dispersive spectroscopy, surface roughness, and Vickers microhardness. CC had the highest values of polarization resistance (Rp), and CHX reduced the capacitance values (Q) compared to the other groups (p < 0.05). HP generated the lowest Rp values in all periods, and the highest Q values after 14 days immersion (p < 0.05). CC and CHX did not alter the alloy potentiodynamic curves when compared to AS. HP increased the corrosion and passivation current densities, and the corrosion rate in relation to the other groups (p < 0.05). SEM micrographs showed minors surface changes with the presence of pitting corrosion for HP. No differences were found for roughness and microhardness after all immersion periods (p > 0.05). Dynamic mouthwash simulation with HP impairs the corrosion stability of Ti6Al4V alloy, and CC presents the greatest electrochemical stability over time. Our data shed light on how the dynamic action of mouthwashes affects the Ti electrochemical behavior aiming at ensuring a safer indication for patients receiving dental implant. (1) Background: Hypersensitivity reactions to metals may arise in predisposed patients chronically exposed to metallic materials, including dental implants made of titanium alloys. The purpose of this article was to systematically review titanium allergy manifestations in patients treated with dental implants and report a clinical case; (2) Methods: A systematic electronic search was performed for articles published in the English language until July 2021. The following eligibility criteria were adopted: (1) Population: individuals undergoing titanium and/or titanium alloy implant-supported rehabilitations; (2) Exposure: peri-implant soft tissue reactions attributable to implant insertion; (3) Outcome: evidence of titanium allergy, diagnostic methods, and forms of resolution; (3) Results: The included studies, in summary, presented evidence that titanium should not be considered an inert material, being able to trigger allergic reactions, and may be responsible for implant failure. A 55-year-old male patient received 3 implants in the posterior region of the left mandible and presented an epulis-like lesion developed from the peri-implant mucosa. The immunohistochemical analysis of the biopsy specimen confirmed the initial diagnosis of allergic reaction to titanium; (4) Conclusions: Although the evidence is weak, and titanium allergy has a low incidence, hypersensitivity reactions should not be underestimated. A rapid and conclusive diagnosis is mandatory to prevent further complications.

The management of orbital fractures in children and adolescents is little reported in the literature, considering that orbital fractures can cause functional problems such as enophthalmia, diplopia and aesthetic deformities. The aim of this study is to review the literature and report three clinical cases, corroborating with professional experience, about the different management of orbital fractures in adolescents, approaching the advantages, disadvantages and complications. Among the different fractures of the orbital floor, the techniques of interposition of autograft of the anterior wall of the maxillary sinus, suture for anchoring the periosteum, and reduction and stabilization of the fracture by means of titanium mesh fixation were performed. The choice of material and technique depends on the surgeon's preference, access and availability of materials. Regardless of which technique and material is used, and complete removal of the herniated tissue is fundamental to obtain satisfactory results. El manejo de las fracturas orbitarias en niños y adolescentes está poco reportado en la literatura, considerando que las fracturas orbitarias pueden causar problemas funcionales como enoftalmia, diplopía y deformidades estéticas. Nuestro objetivo es revisar la literatura y reportar tres casos clínicos, corroborando con la experiencia profesional, sobre los diferentes manejos de las fracturas orbitarias en adolescentes, abordando las ventajas, desventajas y complicaciones. Entre las diferentes fracturas del suelo orbitario, realizamos las técnicas de interposición de autoinjerto de la pared anterior del seno maxilar, la sutura para el anclaje del periostio y la reducción y estabilización de la fractura mediante la fijación de malla de titanio. La elección del material y de la técnica depende de la preferencia del cirujano, del acceso y de la disponibilidad de los materiales. Independientemente de la técnica y el material que se utilicen, es esencial eliminar por completo el tejido herniado para obtener resultados satisfactorios. O manejo das fraturas orbitais em crianças e adolescentes, são pouco relatadas na literatura, considerando que as fraturas orbitais podem ocasionar problemas funcionais como enoftalmia, diplopia e deformidades estéticas. Objetivando revisar a literatura e relatar três casos clínicos, corroborando com a experiencia profissional, sobre diferentes manejos de fraturas orbitárias em adolescentes, abordando as vantagens, desvantagens e complicações. Dentre as diferentes fraturas de assoalho orbitário, foi realizado as técnicas de interposição de enxerto autógeno de parede anterior de seio maxilar, a sutura de ancoragem do periósteo e a redução e estabilização da fratura através da fixação de tela de titânio. A escolha do material e técnica dependem da preferência do cirurgião, do acesso e da disponibilidade dos materiais. Independente de qual técnica e material seja utilizado, e a remoção completa do tecido herniado é fundamental para a obtenção de resultados satisfatórios.

This study evaluated the effect of disinfection procedures and exposure to 0.9% NaCl solution cycles during sterilization, on the electrochemical, physical and chemical properties of different stainless steel alloys used in the biomedical field. Discs of F899 XM16, F899 303, and F899 420B alloys were cleaned with enzymatic detergent followed by 20 cycles of autoclave sterilization. Electrochemical tests were performed including open circuit potential, electrochemical impedance spectroscopy, and potentiodynamic polarization tests in 0.9% NaCl (n=5). Surface characterization was performed using scanning electron microscopy (SEM), energy dispersive spectroscopy, and surface roughness and microhardness both before and after the electrochemical test. Quantitative data were analyzed with a significance level of 5%. The corrosion current density and corrosion potential of the alloys did not present statistical differences. The capacitance values of the three oxide layers, polarization resistance, and impedance of Warburg were similar among the alloys. Considering the topography, surface roughness increased for all alloys in the post-corrosion period. The stainless steel alloys analyzed were negatively affected at the electrochemical behavior, corrosion kinetics, and surface characterization after the 0.9% NaCl solution and the autoclave sterilization cycles, confirming the need for regular exchanges of surgical instruments and drills used in medical and dental implantology. Este estudio evaluó el efecto de los procedimientos de desinfección y la exposición a ciclos de solución de NaCl al 0,9% durante la esterilización, sobre las propiedades electroquímicas, físicas y químicas de diferentes aleaciones de acero inoxidable utilizadas en el campo biomédico. Los discos de aleaciones F899 XM16, F899 303 y F899 420B se limpiaron con detergente enzimático seguido de 20 ciclos de esterilización en autoclave. Se realizaron pruebas electroquímicas que incluyeron potencial de circuito abierto, espectroscopia de impedancia electroquímica y pruebas de polarización potenciodinámica en NaCl al 0,9% (n = 5). La caracterización de la superficie se realizó mediante microscopía electrónica de barrido (SEM), espectroscopía de dispersión de energía y rugosidad y microdureza de la superficie antes y después de la prueba electroquímica. Los datos cuantitativos se analizaron con un nivel de significancia del 5%. La densidad de la corriente de corrosión y el potencial de corrosión de las aleaciones no presentaron diferencias estadísticas. Los valores de capacitancia de las tres capas de óxido, la resistencia a la polarización y la impedancia de Warburg fueron similares entre las aleaciones. Teniendo en cuenta la topografía, la rugosidad de la superficie aumentó para todas las aleaciones en el período posterior a la corrosión. Las aleaciones de acero inoxidable analizadas se vieron afectadas negativamente en el comportamiento electroquímico, cinética de corrosión y caracterización de la superficie después de la solución de NaCl al 0,9% y los ciclos de esterilización en autoclave, lo que confirma la necesidad de intercambios regulares de instrumentos quirúrgicos y fresas utilizados en implantología médica y dental. Este estudo avaliou o efeito de procedimentos de desinfecção e exposição a ciclos de solução de NaCl a 0,9% durante a esterilização, nas propriedades eletroquímicas, físicas e químicas de diferentes ligas de aço inoxidável utilizadas na área biomédica. Os discos das ligas F899 XM16, F899 303 e F899 420B foram limpos com detergente enzimático seguido de 20 ciclos de esterilização em autoclave. Testes eletroquímicos foram realizados incluindo potencial de circuito aberto, espectroscopia de impedância eletroquímica e testes de polarização potenciodinâmica em NaCl a 0,9% (n = 5). A caracterização da superfície foi realizada por meio de microscopia eletrônica de varredura (MEV), espectroscopia de energia dispersiva e rugosidade e microdureza de superfície antes e após o teste eletroquímico. Os dados quantitativos foram analisados com nível de significância de 5%. A densidade de corrente de corrosão e o potencial de corrosão das ligas não apresentaram diferenças estatísticas. Os valores de capacitância das três camadas de óxido, resistência de polarização e impedância de Warburg foram semelhantes entre as ligas. Considerando a topografia, a rugosidade superficial aumentou para todas as ligas no período pós-corrosão. As ligas de aço inoxidável analisadas foram afetadas negativamente no comportamento eletroquímico, cinética de corrosão e caracterização da superfície após a solução de NaCl a 0,9% e os ciclos de esterilização em autoclave, confirmando a necessidade de trocas regulares de instrumentos cirúrgicos e brocas usadas em implantologia médica e dentária.

Bichectomy surgery has been increasingly widespread and performed as an aesthetic, functional, or aesthetic-functional treatment. This study aims to describe the surgical technique of bichectomy with the application of the Kinesio tape to verify its effectiveness in controlling edema formation, with the measurement in four postoperative periods. Through a split-mouth, double-blind, randomized study, 13 participants who had an indication for the surgery were selected and divided into two groups - Control Group: bichectomy surgery and cryotherapy for a period of 12 hours, without external application of the Kinesio tape (KT) and Experimental Group: bichectomy surgery, cryotherapy for a period of 12 hours, and application of KT in the outer cheek area of the face, for 2 days. A millimeter measuring tape was used to measure three reference points on the face before and after the surgical procedure (preoperative, 24 hours, 48 hours, and 7 days postoperatively). Quantitative data were subjected to statistical analysis using the Sigma Plot 12.0 program (Exakt graphs and data analysis, San Jose, USA). The comparisons were made according to the edema measurements for the two experimental groups (2 levels: Kinesio tape versus cryotherapy) and the periods of analysis (4 levels: Preoperative, 24 hours, 48 hours and 7 days postoperatively). After obtaining the data, and the statistical analysis, the application of Kinesio Tape proved to be more effective when compared to the use of cryotherapy alone, in the control of postoperative edema in patients who underwent bichectomy surgery. The study contributes to the literature regarding the applicability of the KT and expands the therapeutic possibilities in an aesthetic-functional procedure that is increasingly sought after by patients. La cirugía de bichectomía está cada vez más extendida y se realiza como tratamiento estético, funcional o estético-funcional. Este estudio tiene como objetivo describir la técnica quirúrgica de la bichectomía con la aplicación de la cinta Kinesio para verificar su efectividad en el control de la formación de edemas, con la medición en cuatro postoperatorios. A través de un estudio aleatorizado, doble ciego, de boca partida, se seleccionaron 13 participantes que tenían una indicación para la cirugía y se dividieron en dos grupos - Grupo Control: cirugía de bichectomía y crioterapia por un período de 12 horas, sin aplicación externa del Kinesio tape (KT) y Grupo Experimental: cirugía de bichectomía, crioterapia por un período de 12 horas y aplicación de KT en la zona externa de la mejilla del rostro, durante 2 días. Se utilizó una cinta métrica milimétrica para medir tres puntos de referencia en el rostro antes y después del procedimiento quirúrgico (preoperatorio, 24 horas, 48 horas y 7 días del postoperatorio). Los datos cuantitativos se sometieron a análisis estadístico utilizando el programa Sigma Plot 12.0 (gráficos y análisis de datos Exakt, San José, EE. UU.). Las comparaciones se realizaron según las medidas de edema para los dos grupos experimentales (2 niveles: Kinesio tape versus crioterapia) y los periodos de análisis (4 niveles: Preoperatorio, 24 horas, 48 horas y 7 días del postoperatorio). Tras la obtención de los datos, y el análisis estadístico, la aplicación de Kinesio Tape demostró ser más eficaz en comparación con el uso de crioterapia sola, en el control del edema postoperatorio en pacientes sometidos a cirugía de bichectomía. El estudio aporta a la literatura sobre la aplicabilidad del KT y amplía las posibilidades terapéuticas en un procedimiento estético-funcional cada vez más buscado por los pacientes. A cirurgia de bichectomia tem sido cada vez mais difundida e realizada como tratamento estético, funcional ou estético-funcional. Este estudo tem como objetivo descrever a técnica cirúrgica da bichectomia com a aplicação da fita Kinesio para verificar sua eficácia no controle da formação de edema, com a mensuração em quatro períodos pós-operatórios. Por meio de um estudo duplo-cego, randomizado e duplo-cego, foram selecionados 13 participantes que tinham indicação para a cirurgia e divididos em dois grupos - Grupo Controle: cirurgia de bichectomia e crioterapia por um período de 12 horas, sem aplicação externa do Kinesio fita adesiva (KT) e Grupo Experimental: cirurgia de bichectomia, crioterapia por um período de 12 horas e aplicação de KT na região externa da bochecha, por 2 dias. Uma fita métrica milimetrada foi utilizada para medir três pontos de referência na face antes e após o procedimento cirúrgico (pré-operatório, 24 horas, 48 horas e 7 dias de pós-operatório). Os dados quantitativos foram submetidos à análise estatística por meio do programa Sigma Plot 12.0 (gráficos Exakt e análise de dados, San Jose, EUA). As comparações foram feitas de acordo com as medidas de edema para os dois grupos experimentais (2 níveis: Kinesio tape versus crioterapia) e os períodos de análise (4 níveis: pré-operatório, 24 horas, 48 horas e 7 dias de pós-operatório). Após a obtenção dos dados e análise estatística, a aplicação do Kinesio Tape mostrou-se mais eficaz quando comparada ao uso da crioterapia isolada, no controle do edema pós-operatório em pacientes submetidos à cirurgia de bichectomia. O estudo contribui com a literatura a respeito da aplicabilidade do KT e amplia as possibilidades terapêuticas em um procedimento estético-funcional cada vez mais procurado pelos pacientes.

O objetivo do presente artigo é relatar o caso de um paciente jovem do sexo masculino que compareceu ao ambulatório de Cirurgia da Faculdade de Odontologia de Araçatuba-Unesp com a presença de um dente supranumerário, 4º molar, fusionado a um terceiro molar inferior esquerdo. Foi solicitada radiografia panorâmica, evidenciando duas coroas unidas e também a união radicular, totalizando 3 raízes. Assim, o tratamento proposto foi a extração com seccionamento dentário em 3 partes, considerando o tamanho do elemento dentário e a quantidade de raízes. A cirurgia transcorreu sem intercorrências e o pós-operatório foi realizado após 7 dias para retirada da sutura. Assim, podemos concluir que, para definir o diagnóstico e o plano de tratamento, é necessário um exame clínico completo e uma anamnese criteriosa, tendo sempre à mão os exames de imagem. El objetivo del presente artículo fue reportar el caso de un paciente joven de sexo masculino que acudió a la Clínica de Cirugía de la Facultad de Odontología de Araçatuba-Unesp con la presencia de un diente supernumerario, 4º molar, fusionado con un tercer molar inferior izquierdo. Se solicitó una radiografía panorámica en la que se objetivaron dos coronas unidas y unión radicular, totalizando 3 raíces. Así, el tratamiento propuesto fue la extracción con seccionamiento dentario en 3 partes, considerando el tamaño del elemento dentario y la cantidad de raíces. La cirugía transcurrió sin incidentes y el postoperatorio se realizó a los 7 días para retirar la sutura. Así, podemos concluir que, para definir el diagnóstico y plan de tratamiento, es necesario realizar una exploración clínica completa y una historia cuidadosa, y tener siempre a mano las pruebas de imagen. The aim of the present article was to report the case of a young male patient who attended the Surgery clinic of the School of Dentistry of Araçatuba-Unesp with the presence of a supernumerary tooth, 4th molar, fused with a lower left third molar. A panoramic radiograph was requested, showing two united crowns as well as root union, totaling 3 roots. Thus, the proposed treatment was extraction with 3-part dental sectioning, considering the size of the dental element and the amount of roots. The surgery was uneventful and postoperative was performed after 7 days for suture removal. Thus, we can conclude that, to define the diagnosis and treatment plan, it is necessary to perform a complete clinical examination and careful history, and always have imaging tests at hand.

A expansão rápida da maxila cirurgicamente assistida (ERMCA) é uma combinação de procedimentos cirúrgicos e ortodônticos, indicada para o tratamento de verdadeiras deficiências esqueléticas transversais em indivíduos esqueleticamente maduros. O presente relato apresenta um caso clínico de ERMCA, realizado sob anestesia local, por equipe multidisciplinar, em ambiente ambulatorial. O plano de tratamento começou com a colocação de um aparelho ortodôntico tipo Hyrax. No procedimento cirúrgico foram realizadas osteotomias horizontais e verticais, seguidas de disjunção da sutura palatina mediana. A expansão foi realizada pela ativação diária do aparelho disjuntor. Após a disjunção, iniciou-se o tratamento ortodôntico para alinhar e nivelar os dentes. Ao término do tratamento, observou-se expansão efetiva da arcada superior, com correção da atrofia maxilar e melhora anatomofuncional do sistema estomatognático. Diante do resultado obtido, concluiu-se que o ERMCA é um tratamento multidisciplinar eficaz para a correção de deficiências transversais da maxila. La expansión maxilar rápida asistida quirúrgicamente (EMRAQ) es una combinación de procedimientos quirúrgicos y de ortodoncia, indicada para el tratamiento de verdaderas deficiencias esqueléticas transversales en individuos esqueléticamente maduros. El presente informe presenta un caso clínico de EMRAQ, realizado bajo anestesia local, por un equipo multidisciplinario, de forma ambulatoria. El plan de tratamiento comenzó con la colocación de un aparato de ortodoncia tipo Hyrax. En el procedimiento quirúrgico se realizaron osteotomías horizontales y verticales, seguidas de disyunción de la sutura palatina media. La expansión se realizó mediante la activación diaria del dispositivo disyuntor. Después de la disyunción, el tratamiento de ortodoncia comenzó a alinear y nivelar los dientes. Al finalizar el tratamiento, se observó una expansión efectiva del arco maxilar, con corrección de la atrofia maxilar y mejoría anatomofuncional del sistema estomatognático. A la vista del resultado obtenido, se concluyó que EMRAQ es un tratamiento multidisciplinario eficaz para la corrección de deficiencias transversales del maxilar. Surgically assisted rapid maxillary expansion (SARME) is a combination of surgical and orthodontic procedures, indicated for the treatment of true transverse skeletal deficiencies in skeletally mature individuals. The present report presents a clinical case of SARME, performed under local anesthesia, by a multidisciplinary team, in an outpatient setting. The treatment plan began with the placement of a Hyrax type of orthodontic appliance. In the surgical procedure, horizontal and vertical osteotomies were performed, followed by median palatine suture disjunction. Expansion was performed by daily activation of the disjunctor appliance. After disjunction, orthodontic treatment began to align and level the teeth. On conclusion of treatment, effective expansion of the maxillary arch was observed, with correction of maxillary atrophy and anatomofunctional improvement of the stomatognathic system. In view of the result obtained, it was concluded that SARME is an effective multidisciplinary treatment for the correction of transverse deficiencies of the maxilla.

Objectives The aim of this study is to evaluate the preemptive analgesic effects of dexamethasone (DEX) alone or combined with non-steroidal anti-inflammatory drugs (NSAIDs) in third molar surgeries.Materials and Methods The subjects were divided into five groups (n = 20 teeth/group); subjects received only 8 mg of dexamethasone 1 hour before the surgical procedure (DEX group), or in combination with etodolac (DEX + ETO), ketorolac (DEX + KET), ibuprofen (DEX + IBU), loxoprofen (DEX + LOX). Paracetamol 750 mg was provided as the number of rescue analgesics (NRA). Salivary PGE2 expression was measured preoperatively and at 48 hours. Edema and Maximum mouth opening (MMO) were measured postoperatively at 48h and 7 days. A visual analog scale (VAS) was performed postoperatively at 6, 12, 24, 48, 72 hours, and 7 days.Results Salivary expression of PGE2 showed a decrease only for the DEX group. Edema and MMO and NRA consumption showed no significant differences among the groups (P > 0.05). The VAS showed a significantly lower pain perception at 6 hours after the surgery for the DEX + ETO and DEX + KET groups (P ).Conclusions The combination of DEX and NSAIDS should be considered for preemptive acute postsurgical pain management in third molar surgery. Clinical Relevance: In some drug associations such as dexamethasone 8 mg + NSAIDS (ETO and KET), and others, the isolated administration of a single dose of dexamethasone in the pre-operative time, only a few rescue analgesics are necessary.

The management of patients with neurodegenerative conditions such as Alzheimer's disease presents a challenge in promoting oral health, which is mainly linked to the motor and cognitive limitations that these individuals have, making them susceptible to the development of a series of oral diseases, such as oral myiasis. The presence of larvae in the oral cavity in patients with Alzheimer's disease seems to be related to poor oral hygiene condition, to the long periods that the patient remains immobile and with the mouth ajar, facilitating the deposition of eggs. Therefore, this study aimed to report a clinical experience of a patient with Alzheimer's disease in an advanced stage and with manifestation of oral myiasis on the oral floor, vestibule bottom and inserted gum. The 81-year-old patient was debilitated, bedridden, unable to perform his activities alone, was taken to the emergency room by his caregiver, who reported having noticed the presence of larvae in the oral cavity. The patient was hospitalized and immediately administered Ivermectin, ceftriaxone and clindamycin. On the second day of hospitalization, he presented a systemic worsening due to bronchoaspiration, requiring immediate removal of the larvae under local anesthesia. Only after significant improvement in the general condition, thorough surgery was performed to debridement of the injured areas and remnants of larvae. After hospital discharge with 2-month follow-up, the patient did not have any complications related to his oral health, demonstrating that periodic follow-up to debilitated patients is an important tool for preventing opportunistic manifestations such as oral myiasis. El manejo de pacientes con afecciones neurodegenerativas como la enfermedad de Alzheimer presenta un desafío en la promoción de la salud bucal, que está principalmente ligado a las limitaciones motoras y cognitivas que tienen estos individuos, haciéndolos susceptibles al desarrollo de una serie de enfermedades bucodentales. La presencia de larvas en la cavidad oral en pacientes con enfermedad de Alzheimer parece estar relacionada con un mal estado de higiene bucal, con los largos periodos que el paciente permanece inmóvil y con la boca entreabierta, facilitando el depósito de huevos. Este estudio tuvo como objetivo reportar una experiencia clínica de un paciente con enfermedad de Alzheimer en estadio avanzado y con manifestación de miasis oral en el piso bucal, fondo del vestíbulo y encía insertada. El paciente de 81 años se encontraba debilitado, postrado en cama, incapaz de realizar sus actividades solo, fue llevado a urgencias por su cuidador, quien refirió haber notado la presencia de larvas en la cavidad bucal. El paciente fue hospitalizado y se le administró inmediatamente ivermectina, ceftriaxona y clindamicina. Al segundo día de hospitalización presentó un agravamiento sistémico por broncoaspiración, requiriendo la extracción inmediata de las larvas bajo anestesia local. Solo después de una mejora significativa en el estado general, se realizó una cirugía completa para el desbridamiento de las áreas lesionadas y los restos de larvas. Tras el alta hospitalaria con seguimiento a 2 meses, el paciente no presentó complicaciones relacionadas con su salud bucal, demostrando que el seguimiento periódico a pacientes debilitados es una herramienta importante para prevenir manifestaciones oportunistas como la miasis oral. O manejo de pacientes com condições neurodegenerativas como a doença de Alzheimer apresenta-se um desafio na promoção de saúde bucal, o que está atrelado principalmente as limitações motoras e cognitivas que tais indivíduos possuem, tornando-os susceptíveis ao desenvolvimento de uma série de doenças bucais, como a miíase oral. A presença de larvas na cavidade oral em pacientes com doença de Alzheimer parece estar relacionada a condição de higiene bucal deficiente, aos longos períodos que o paciente permanece imóvel e com a boca entreaberta, facilitando a deposição dos ovos. Diante disso, este trabalho teve o objetivo de relatar uma experiência clínica de paciente com doença de Alzheimer em estágio avançado e com manifestação de miíase oral em assoalho bucal, fundo de vestíbulo e gengiva inserida. O paciente de 81 anos encontrava-se debilitado, acamado, sem condições de realizar suas atividades sozinho, foi levado ao pronto atendimento por sua cuidadora, que relatou ter notado presença de larvas em cavidade bucal. O paciente foi internado e imediatamente instituiu-se administração de Ivermectina, ceftriaxona e clindamicina. No segundo dia de internação, apresentou uma piora sistêmica devido broncoaspiração, necessitando de remoção imediata das larvas sob anestesia local. Somente após significativa melhora da condição geral, realizou-se cirurgia minuciosa para debridamento das áreas lesionadas e de remanescentes de larvas. Após alta hospitalar com acompanhamento de 2 meses, o paciente não apresentou nenhuma intercorrência relacionada a sua saúde bucal, demonstrando que o acompanhamento periódico a pacientes debilitados é uma ferramenta importante para prevenção de manifestações oportunistas como a miíase oral.

Orotracheal intubation is considered the most delicate maneuver performed by anesthesiologists, and failure to manage the airways is one of the main causes of possible complications that can be catastrophic during its performance, thus requiring good team planning. Anatomical and / or physiological characteristics, such as limited mouth opening, a condition often found in patients suffering from fractures of the zygomatic arch or who have some abnormality in the temporomandibular joint (TMJ), can characterize a difficult airway, which demands attention greatest of the professional. Therefore, in order to overcome the obstacles that may occur during a difficult or unexpected intubation, new tools have been used, such as videolaryngoscopy in which an intubation device containing miniature video cameras is used in order to allow the operator has an indirect view of the glottis to increase success rates and decrease intubation time. The aim of the present study is to report a clinical case of a trauma victim, with limited mouth opening, who underwent intubation with the aid of a video laryngoscope after failed attempts at conventional intubation. La intubación orotraqueal se considera la maniobra más delicada que realizan los anestesiólogos, y la falta de manejo de la vía aérea es una de las principales causas de posibles complicaciones que pueden ser catastróficas durante su realización, por lo que requieren una buena planificación del equipo. Las características anatómicas y / o fisiológicas, como la apertura bucal limitada, una condición que se encuentra a menudo en pacientes que sufren fracturas del arco cigomático o que tienen alguna anomalía en la articulación temporomandibular (ATM), pueden caracterizar una vía aérea difícil, que requiere atención. el más grande del profesional. Características anatômicas e/ou fisiológicas como por exemplo a limitação da abertura bucal, condição frequentemente encontrada em pacientes vítimas de fratura do arco zigomático ou que possuem alguma anormalidade na articulação temporomandibular (ATM), podem caracterizar uma via aérea difícil, o que demanda uma atenção maior do profissional. Sendo assim, a fim de superar os obstáculos que podem ocorrer durante uma intubação difícil ou inesperada, novas ferramentas vêm sendo utilizadas, como a videolaringoscopia na qual utiliza-se um dispositivo de intubação que contém câmeras de vídeo em miniatura com o objetivo de permitir que o operador tenha uma visão indireta da glote para aumentar as taxas de sucesso e diminuir o tempo de intubação. O objetivo do presente trabalho é relatar um caso clínico de um paciente vítima de trauma, com limitação de abertura bucal, submetido à intubação com o auxílio de um videolaringoscópio após tentativas falhas na realização da intubação convencional.

This infection invades not only the paranasal sinuses, but also the maxillae, zygoma, and rhinocerebral structures, which results in massive destruction of the facial skeleton and soft tissue. It progresses in patients with underlying comorbidities, such as diabetes mellitus, haematologic malignancy, cerebral malignancy, renal failure, and systemic immunosuppression. Most cases occur in tropical countries. The relationship between mucormycosis and these underlying conditions has been widely discussed in the literature. This study sought to report the case of a patient diagnosed with glioblastoma and mucormycosis in the maxilla, who was successfully treated with surgery and isavuconazonium sulfate (Cresemba). No. of bitstreams: 0 Previous issue date: 2021-01-01 Background: This study aimed to conduct a randomized prospective study about the efficacy of etodolac and ibuprofen on trismus, pain and edema regarding gender of patients submitted to impacted lower third molar teeth extraction. Material and Methods: Thirty patients aging between 16 and 35 year-old were submitted to the exodontia of impacted lower third molars. During the postoperative period, patients received nine ibuprofen (600 mg) or etodolac (300 mg) pills via oral administration immediately after surgery and repeated doses every eight hours during three days. Patients were evaluated regarding pain, trismus and edema. Results: Sixteen men and fourteen women participated of the study. No statistical difference was established regarding gender according to the evaluated parameters. However, etodolac use showed better results regarding pain, trismus and edema. Conclusions: Pain, edema and trismus after impacted third molars extraction were not influenced by gender. No. of bitstreams: 0 Previous issue date: 2020-12-01 Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Commercially available preparations of methionyl-human BMP-2 and CHO-derived hBMP-2, which belongs to the transforming growth factor β (TGF-β) superfamily, were used for a complete characterization. This protein is an extremely efficient osteoinductor that plays an important role during bone regeneration and embryonic development. Characterization was carried out via SDS-PAGE and Western blotting, followed by reversed-phase HPLC, size-exclusion HPLC and MALDI-TOF-MS. The classical in vitro bioassay, based on the induction of alkaline phosphatase activity in C2C12 cells, confirmed that hBMP-2 biological activity is mostly related to the dimeric form, being ~ 4-fold higher for the CHO-derived glycosylated form when compared with the E. coli counterpart. The E. coli-derived met-hBMP-2 has shown, by MALDI-TOF-MS, a large presence of the bioactive dimer. A more complex molecular mass (MM) distribution was found for the CHO-derived product, whose exact MM has never been reported because of its variable glycosylation. A method based on RP-HPLC was set up, allowing a quantitative and qualitative hBMP-2 determination even directly on ongoing culture media. Considering that hBMP-2 is highly unstable, presenting moreover an extremely high aggregate value, we believe that these data pave the way to a necessary characterization of this important factor when synthesized by DNA recombinant techniques in different types of hosts. Although the posterior region of the mandible is more frequently affected, lesions observed in the maxilla should receive special attention due to the risk of expansion to the nasal, paranasal and orbital structures. This report shows a successful case of a maxillary ameloblastoma treated by combining therapies in a young patient. The treatment strategy adopted to reduce and redefine the lesion dimensions enabled enucleation in a safe way, without destruction of adjacent noble structures. It also allowed complementary approach with peripheral ostectomy and sclerosing agent, efficiently, without evidence of recurrence or complications after 2 years. Objective: to describe the clinical case of a patient with multiple myeloma, who developed maxillary osteonecrosis induced by anti-resorptive medication (bisphosphonate) submitted to a conservative and surgical therapeutic approach and subsequent prosthetic rehabilitation. Patients and methods: the case report is presented based on a simple review of the literature and clinical experience for the diagnosis, clinical staging of the lesion, carrying out the excision of the necrotic bone, debridement, lesion control and making a obturator prosthesis. Results: Despite the difficulty offered by the pathological condition and continuous use of medication, the treatment employed was successful, restoring health and providing a better quality of life to the patient, which allowed the obturation prosthesis to be made in the 8-month postoperative period. Conclusion: The result obtained emphasizes the importance of knowledge of the staging of the lesion for the application of the treatment protocol, in addition to recognizing patients at risk for the practice of prevention in dental treatments. Objetivo: describir el caso clínico de un paciente con mieloma múltiple, que desarrolló osteonecrosis maxilar inducida por medicación antirresortiva (bisfosfonatos) sometido a un abordaje terapéutico conservador y quirúrgico y posterior rehabilitación protésica. Pacientes y métodos: se presenta el caso clínico en base a una simple revisión de la literatura y experiencia clínica para el diagnóstico, estadificación clínica de la lesión, realización de la exéresis del hueso necrótico, desbridamiento, control de la lesión y realización de una prótesis de obturación. Resultados: A pesar de la dificultad que ofrecía la condición patológica y el uso continuado de la medicación, el tratamiento empleado fue exitoso, devolviendo la salud y proporcionando una mejor calidad de vida al paciente, lo que permitió realizar la prótesis de obturación en el postoperatorio de 8 meses. Conclusión: El resultado obtenido enfatiza la importancia del conocimiento de la estadificación de la lesión para la aplicación del protocolo de tratamiento, además de reconocer a los pacientes en riesgo para la práctica de la prevención en los tratamientos dentales. Pacientes e métodos: o relato do caso é apresentado com base em uma revisão simples da literatura e experiência clínica para o diagnóstico, estadiamento clínico da lesão, realização da exérese do osso necrótico, debridamento, controle da lesão e confecção de prótese obturadora. Resultados: Apesar da dificuldade oferecida pela condição patológica e uso contínuo da medicação, o tratamento empregado apresentou sucesso, restabelecendo a saúde e proporcionando maior qualidade de vida a paciente, o que permitiu a confecção da prótese obturadora no pós-operatório de 8 meses. Conclusão: O resultado obtido enfatiza a importância do conhecimento do estadiamento da lesão para aplicação do protocolo de tratamento, além de reconhecer os pacientes de risco para a prática da prevenção nos tratamentos odontológicos.
