5. Introducing wearable haptics for rendering velocity feedback in VR serious games for neuro-rehabilitation of children

Abstract

Rehabilitation in virtual reality offers advantages in terms of flexibility and parametrization of exercises, repeatability, and continuous data recording and analysis of the progress of the patient, also promoting high engagement and cognitive challenges. Still, most of the proposed virtual settings provide a high quality, immersive visual and audio feedback, without involving the sense of touch. In this paper, we show the design, implementation, and first evaluation of a gaming scenario for upper limb rehabilitation of children with cerebral palsy. In particular, we took care to introduce haptic feedback as a useful source of sensory information for the proposed task, considering—at the same time—the strict constraints for haptic wearable devices to comply with patient’s comfort, residual motor abilities, and with the embedded tracking features of the latest VR technologies. To show the potential of haptics in a rehabilitation setup, the proposed device and rendering method have been used to improve the velocity control of upper limb movements during the VR exercise, given its importance as a motor recovery metric. Eight healthy participants were enrolled, and results showed that haptic feedback can lead to lower speed tracking errors and higher movement smoothness, making the proposed setup suitable to be used in a rehabilitation context as a way to promote movement fluidity during exercises.

6. Discrete Cutaneous Feedback for Reducing Dimensions of Wearable Haptic Devices

Abstract

In this article, we explore alternative cutaneous haptic feedback for rendering modulation of the grasping force. The aim of the study was to reduce power requirements and in turn dimensions of the actuators, in wearable devices applied to virtual or teleoperated manipulation. This is critical in certain rehabilitation or training scenarios where haptics should not interfere with dexterity of the user. In the study, we experimented discrete, pulsed cutaneous force feedback and compared it with conventional continuous proportional feedback, in a virtual pick and place task. We made use of wearable thimbles based on voice coil actuators in order to provide high-quality, low-noise haptic feedback to the participants. The evaluation was performed on the basis of both objective measurements of task performance (measured virtual forces and correct ratio) and a questionnaire evaluating participants’ preferences for the different feedback conditions. On the basis of the obtained results, in the article, we discuss the possibility of providing high-frequency, discretized cutaneous feedback only, driven by modulation of the grasping force. The opportunity is to reduce volume and mass of the actuators and also to consider alternative design solutions, due to the different requirements in terms of static and high-frequency components of the output force.

Abstract

Background/objectives: The aim of the present study was to validate a short questionnaire on habitual dietary salt intake, to quickly and easily identify individuals whose salt consumption exceeds recommended levels. Subjects/methods: A total of 1131 hypertensive subjects participating in the MINISAL-SIIA study were included in the analysis. Anthropometric indexes, blood pressure, and 24-h urinary sodium excretion (NaU) were measured. A fixed-sequence questionnaire on dietary salt intake was administered. Results: NaU was significantly associated with scores, with a linear association across categories (p for trend <0.0001). In addition, participants who achieved a total score above the median value (eight points) had significantly higher NaU than those whose score was below median (p < 0.0001). In the total sample, the prevalence of “high NaU” (NaU > 85 mmol/day) and “very high NaU” (NaU > 170 mmol/day) was 86 and 35%, respectively. The score of the questionnaire had a significant ability to detect both “high NaU”—with a specificity of 95% at the score of 10 points—and “very high NaU”—with a specificity of 99.6% at score of 13 points. Conclusions: The main results of the study indicates that a higher score of this short questionnaire is distinctive of habitual high salt consumption in hypertensive patients.

Abstract

Alterations in innate immunity play a role in renal damage in experimental models but the role of these alterations in the progression of CKD in humans is still poorly defined. Procalcitonin (PCT), is a biomarker of innate immunity produced by C-cells of the thyroid and by the adipose tissue. Objectives: We measured serum plasma PCT levels in a cohort of 670 patients with stage 3–5 CKD and tested the relationship between this biomarker metrics of adiposity, proteinuria, GFR and progression to kidney failure over a 3 year follow-up. None of the patients had intercurrent infectious or acute inflammatory processes. Methods: PCT was measured by an ultrasensitive immunoluminometric assay. The GFR was estimated by a Cystatin-C based equation. The relationship between PCT and renal events was tested by multivariate Cox’s regression and interaction analysis. Results: Procalcitonin exceeded the upper limit of the normal range (>0.064 ng/mL) in 492 patients (67 %) while the corresponding figure for high sensitivity CRP (>1 mg/L) was 170 (%). Abstract

In renal transplant patients, the prevalence of nocturnal hypertension by far exceeds that of hypertension as assessed by clinic, daytime, and average 24-hour ABPM and night-time systolic BP (SBP) and the night-day ratio but no other BP metrics are independently associated with IMT in this population (Transplantation 2015 Dec 17). BP during nighttime may provide information for the assessment of CV risk attributable to BP burden in renal transplant patients but the determinants of this phenomenon are still undefined in these patients. Design and method: We recorded 24h-ABPM and performed polysomnography in 215 clinically stable transplant patients. Major classical CV risk factors and CKD-specific risk factors (including phosphate, eGFR, proteinuria and C Reactive Protein) were measured alongside. Our primary goal was to identify the functional correlates of average BP during night-time and of the night/day ratio, a parameter quantifying the BP dipping phenomenon. Results: The Apnea-Hypopnea Index (AHI) exceeded 5 episodes/h in 56 patients and 15 episodes/h in 19. Thus, 17% of patients had mild-to-moderate SDB and 9% severe SDB. In unadjusted analyses, AHI was directly related with night-time SBP (r = 0.19, P = 0.005) and the night/day SBP ratio (r = 0.18, P = 0.01). AHI also associated with average 24h-SBP and DBP (both r = 0.14, P = 0.04). Furthermore, age, gender, BMI, diabetes, phosphate, C-Reactive Protein, and background CV comorbidities were all associated with AHI. In multiple regression analyses adjusting for these parameters as well as for average day-time SBP, Hb, and proteinuria, AHI remained related with average night-time SBP (beta = 0.10, P = 0.02) and with the night/day SBP ratio (beta = 0.17, P = 0.02). In these models, the AHI ranked as the strongest correlate of nigh-time SBP burden. These results did not change also forcing into the model immunosuppressive treatment. Conclusions: In stable transplant patients, sleep disordered breathing is the most powerful functional correlate of nocturnal BP. These relationships are independent of age and comorbidities, eGFR and proteinuria and likely represents irreversible effects of long-term past exposure to kidney failure on the central nervous system

Abstract

Background and methods: Since heart rate (HR) is a cardiovascular risk factor and a marker of sympa- thetic activity, we tested the predictive value of HR for progression to kidney failure in a well characterized cohort of 759 patients with stage 2-5 CKD followed up for 29 ± 11 months. Results: Overall, a total of 244 patients had renal events. In an unadjusted analysis by age tertiles the predictive value of HR for renal events was appar- ent only in patients in the third age tertile (older than 68 years) but not in those in the first two tertiles indi- cating effect modification by age of the HR - progres- sion to kidney failure relationship. In a multiple Cox regression model adjusting for potential confounders, a 5 beats/min increase in HR entailed a 16% risk ex- cess (Hazard Ratio = 1.16, P = .004) for renal events in patients in the third age tertile but no excess risk for the same events in patients in the first two tertiles. 29. Augmentation of the maxillary sinus with calcium sulfate: one-year clinical report from a prospective longitudinal study.

Abstract

The aim of the present investigation was to evaluate the clinical and histologic results of a sinus augmentation procedure performed using calcium sulfate as the grafting material. A group of 12 patients (15 sinuses) formed the pilot group. Based on the experience of the pilot group, the technique of calcium sulfate application was modified, and a second group of 45 patients (50 sinuses) was subsequently treated (test group). In the pilot group, a total of 30 implants (Biolock) was placed. In the test group, a total of 100 implants (Biolock and Biohorizons) was placed. The clinical data reported in the present study are related to the 1-year follow-up for both groups. Clinical evaluations, including assessment of implant mobility and probing pocket depth, were recorded on a monthly basis following implant uncovering until final prosthesis placement, and every 6 months thereafter. Radiographs were taken prior to sinus augmentation, monthly until 6 months postoperatively, 9 and 12 months after implantation, and at yearly intervals thereafter. One implant in the pilot group was not integrated at second-stage surgery, and 1 in the test group failed to maintain osseointegration after the abutment connection (at the 1-year evaluation). Based on defined criteria, the overall success rate for the 130 placed implants 1 year postimplantation was 98.5%. Clinical and radiographic evaluation revealed that the augmentation procedure resulted in new tissue formation within the sinuses. The technique used in the test group suggested a slowdown in material resorption and a reduction in graft shrinkage during healing. Bone biopsies were harvested for histologic evaluation. The application of a resorbable barrier membrane to the access window reduced the invagination of soft tissue at that level. Abstract

This paper presents the preliminary results of the project BRAVO (Brain computer interfaces for Robotic enhanced Action in Visuo-motOr tasks).The objective of this project is to define a new approach to the development of assistive and rehabilitative robots for motor impaired users to perform complex visuomotor tasks that require a sequence of reaches, grasps and manipulations of objects. BRAVO aims at developing new robotic interfaces and HW/SW architectures for rehabilitation and regain/restoration of motor function in patients with upper limb sensorimotor impairment through extensive rehabilitation therapy and active assistance in the execution of Activities of Daily Living. The final system developed within this project will include a robotic arm exoskeleton and a hand orthosis that will be integrated together for providing force assistance. The main novelty that BRAVO introduces is the control of the robotic assistive device through the active prediction of intention/action. The system will actually integrate the information about the movement carried out by the user with a prediction of the performed action through an interpretation of current gaze of the user (measured through eye-tracking), brain activation (measured through BCI) and force sensor measurements.

Abstract

An excessive blood pressure (BP) reduction might be dangerous in high-risk patients with cardiovascular disease. In the Studio Italiano Sugli Effetti CARDIOvascolari del Controllo della Pressione Arteriosa SIStolica (Cardio-Sis), 1111 nondiabetic patients with systolic BP ≥150 mm Hg were randomly assigned to a systolic BP target P for interaction=0.82). The main secondary end point, a composite of cardiovascular events and all-cause death, occurred less frequently in the tight than in the standard control group both in patients without (1.47 versus 3.68 patient-years; P =0.016) and with (7.87 versus 11.22 patient-years; P =0.049) previous cardiovascular disease. The hypothesis that a therapeutic strategy aimed at lowering systolic blood pressure (SBP) below 130 mm Hg is superior to a conventional strategy targeted at below 140 mm Hg in hypertensive subjects has never been tested in randomized intervention studies. The Studio Italiano Sugli Effetti Cardiovascolari del Controllo della Pressione Arteriosa Sistolica (Cardio-Sis) is a multi-centre study in non-diabetic, treated hypertensive subjects aged >55 years with uncontrolled SBP (>or=150 mm Hg) and at least one additional cardiovascular risk factor (ClinicalTrials.gov identifier: NCT00421863). Abstract

Background and objectives High serumIL-6 is amajor risk factor for cardiovascular disease (CVD) in the general population. This cytokine is substantially increased in patientswith CKD, but it is still unknownwhether the link between IL-6 and CVD in CKD is causal in nature. Design, setting, participants, & measurements In a cohort of 755 patientswith stages 2–5 CKD, consecutively recruited from 22 nephrology units in southern Italy, this study assessed the relationship of serum IL-6 with history of CVD, as well as with incident cardiovascular (CV) events (mean follow up6SD, 31610 months) and used the functional polymorphism (2174 G/C) in the promoter of the IL-6 gene to investigate whether the link between IL-6 and CV events is causal. Results In adjusted analyses, serum IL-6 above the median value was associated with history of CVD (P,0.001) and predicted the incidence rate of CV events (hazard ratio, 1.66; 95% confidence interval [95% CI], 1.11 to 2.49; P=0.01). Patients homozygous for the risk allele (C) of the2174G/C polymorphismhad higher levels of IL-6 than did those with other genotypes (P=0.04). Abstract

INTRODUCTION:The aim of the study was to assess the age-specific, sex-specific, and region-specific average sodium and potassium intake and its association with anthropometric characteristics in a sample of the Italian adult hypertensive population. METHODS:A total of 1232 hypertensive patients were recruited consecutively by 47 centers recognized by the Italian Society of Hypertension. The enrolled participants were on stable antihypertensive treatment. Anthropometric indices, blood pressure, 24-h urinary sodium, and potassium excretion were measured and used as proxy for the average daily sodium and potassium intake. RESULTS:The average sodium intake was 172 mmol (or 10.1 g of salt/day) among men and 138 (or 8.1) among women, with no difference among geographical areas. Over 90% of men and 81% of women had a consumption higher than the recommended standard dietary intake of 5 g/day. Abstract

Background: The Multiple Intervention and Audit in Renal Diseases to Optimize Care (MAURO) study was a cluster randomized controlled trial in 22 renal clin- ics which aimed to assess the efficacy of a multimodal quality improvement intervention to increase compli- ance with guideline recommendations for prevention of chronic kidney disease (CKD) progression and car- diovascular (CV) complications. The trial aimed to test whether this multimodal intervention improved adher- ence to recommended targets for a series of surro- gate indicators relevant to blood pressure (BP) control, sodium intake, proteinuria, dyslipidemia, anemia and calcium-phosphate alterations. The trial also tested whether the same intervention slowed CKD progres- sion and prevented CV complications in CKD patients. Method: Twenty-two renal clinics were randomized to 2 arms: an intervention arm applying a multimethod quality improvement intervention and a control arm providing standard care. Surrogate indicators were measured to evaluate blood pressure (BP) control, sodium intake, proteinuria, dyslipidemia, anemia and calcium-phosphate alterations. Results: Notwithstanding the fact that the vast major- ity of patients enrolled in this study (95%) were being treated with antihypertensive drugs, BP goals at base- line (specific for proteinuria level and diabetes) were met only in 45% of nonproteinuric patients and in just 14% and 18% of proteinuric and diabetic patients, respectively. The use of diuretics in hypertensive pa- tients was less than what was needed, and about 80% of patients showed a salt excretion >100 mmol/ 24 hours. Total and low-density lipoprotein cholesterol levels were out of target in over 40% of patients, but only about 60% of these were on statins. A large ma- jority of CKD patients were obese or overweight. The proportion of smokers was relatively small (13%), and 37% of patients had quit smoking, indicating patients' awareness of the health risks of smoking. Conclusions: In our cohort, management of modifiable risk factors for CKD progression and CV disease could be substantially improved.
