This article gives an overview of service learning as it relates to the integration of faith and learning for a pre-service teacher candidate. A survey was given to pre-service, education candidates who were completing a service-learning project in a local Christian school. Results from the survey indicate that all major elements of service learning in a faith based institution were achieved to include agreeing on a mission, having administration buy-in, candidate learning, and an awareness of faith and learning. In addition teacher education candidates were more likely to work in Christian school after having been exposed to this experience. The research supports the importance of providing a variety of experiences that integrate faith and learning.

Background. Pulmonary metastasectomy is well accepted in patients with isolated metastases from an extrathoracic malignancy. The standard approach involves careful intraoperative palpation of the lungs because more metastases are frequently found than were seen by preoperative conventional computed tomography (CT). Helical CT detects more nodules than conventional CT, raising the question of whether palpation of the lungs is still necessary if helical CT is used. Methods. Retrospective review was done of medical records of patients undergoing metastasectomy with curative intent at the University of North Carolina (UNC) from 1999 to 2003. During this time at UNC, helical CT was routinely performed using a standardized technique, and all metastasectomy patients underwent manual lung palpation. The primary outcome measure of this study was whether malignant nodules (palpated, resected, and proven histologically) were reliably detected preoperatively by helical CT. Results. Thirty-four patients were identified who underwent 41 cases of pulmonary metastasectomy with lung palpation. Our analysis revealed that in 22% (9/41), more malignant nodules were found intraoperatively than were detected by helical CT. Of 88 malignant intraparenchymal nodules, 69 were detected by helical CT (sensitivity 78%). Subset analyses of tumor histology, disease-free interval, the presence of a single lesion versus multiple lesions, the interval between the CT and metastasectomy, and the size of the largest lesion were unable to identify a cohort in which lung palpation was no longer needed after preoperative helical CT. Conclusions. Despite the advent of helical CT, palpation of the lung is necessary if the goal is to resect all detectable disease.

BACKGROUND Induction chemoradiotherapy followed by surgery may improve survival of patients with esophageal carcinoma. Computed tomography (CT) has been used to evaluate the tumor response after completing induction chemoradiotherapy. The authors examined the ability of CT to evaluate the pathologic tumor response to induction therapy and to stage the tumor correctly. METHODS Preinduction and postinduction chemoradiotherapy CT scans were reviewed retrospectively for 50 patients enrolled in a protocol of induction chemoradiotherapy followed by surgery. All studies were performed on third-generation or fourth-generation scanners. Radiographic response was determined using Eastern Cooperative Oncology Group solid tumor response criteria for bidimensional measurable disease. This was compared with the pathologic tumor response. CT-tumor (T) classification using the modified Tio scale was compared with the pathologic T classification. RESULTS CT-T classification did not correlate with the pathologic stage (P = 0.09) or the pathologic tumor response (P = 0.22). The postinduction chemoradiotherapy CT accurately staged the T classification in 42% of patients but overstaged 36% of patients and understaged 20% of patients. CT had a sensitivity of 65%, a specificity of 33%, a positive predictive value of 58%, and a negative predictive value of 41% in evaluating the pathologic tumor response. CONCLUSIONS CT is a poor diagnostic study tool for determining the pathologic tumor response or the pathologic disease stage after induction chemoradiotherapy in patients with esophageal carcinoma. PURPOSE: To estimate the prevalence of abdominal computed tomographic (CT) findings in sarcoidosis and to correlate these findings with those at chest radiography, clinical status, and level of angiotensin converting enzyme (ACE). MATERIALS AND METHODS: Abdominal CT examinations in 59 patients with sarcoidosis were evaluated for adenopathy, liver and spleen size, and discrete lesions within the liver or spleen. RESULTS: Extensive adenopathy was seen in 10% of patients. Marked hepatic and splenic enlargement was seen in 8% and 6%, respectively. Nodules were seen in the spleen in eight (15%) patients and in the liver in three (5%). Although liver size, spleen size, and adenopathy were directly related (P < .0001), the presence of nodules was not strongly related to organ size. Abdominal CT findings were related to clinical status and elevated ACE levels but not to chest radiographic stage. Medical records and radiologic studies of 238 patients with non-small cell lung cancer who had preoperative evaluation by chest radiography and CT were reviewed. Thirty-six patients were staged as T1N0M0 by chest radiograph. Of this group, 18 (50%) had abnormalities on CT requiring additional evaluation. Confirmation of abnormalities was by tissue sampling or clinical follow-up. Evidence for unresectable spread of disease was obtained in 12 (33%). We conclude that routine preoperative staging of T1N0M0 lung cancer with CT has a positive impact on patient management.

Background In thoracic surgery, manual lung palpation for detection of pulmonary metastases during resection is the standard of care, despite improvements in computed tomography (CT) imaging. In our previous study based on chart review alone, the accuracy of helical CT in the detection of pulmonary metastases was surprisingly low, with a sensitivity of 78%. We hypothesized that this may be improved by scan interpretation with adequate clinical history and focused documentation of all pulmonary lesions, and may be influenced by the training of the reader. Methods Preoperative CT scans of 53 patients undergoing 60 cases of pulmonary metastasectomy at our center from 1996 to 2004 were retrospectively reviewed by a dedicated chest radiologist and a non–chest radiologist. Nodules detected on preoperative helical CT were compared with pathologically confirmed metastases. Results In 27 of 59 (46%) cases read by radiologist 1, and 27 of 58 (47%) cases read by radiologist 2, metastases found by lung palpation were not seen on preoperative CT. Preoperative CT was entirely correct (no missed metastases or false-positive lesions) in only 11 of 59 (19%) of cases read by radiologist 1, and 11 of 58 (19%) of cases read by radiologist 2. Conclusions Helical CT misses metastases in 46% to 47% of cases. Accuracy of preoperative CT scanning for detection of pulmonary metastases was not improved with the provision of clinical history to the reader, nor was it influenced by the interpreter's training. A combined approach to pulmonary metastasectomy including preoperative and postoperative CT as well as manual lung palpation is necessary to render the patient disease-free.

Pulmonary effusions associated with ovarian cancer indicate advanced disease. Although many patients tolerate these effusions, some are symptomatic and manifest respiratory distress. Therapeutic sclerosis of the pleural cavity is successful in some patients; however, not all patients are relieved of respiratory symptoms. We present two symptomatic patients in whom sclerosis was unsuccessful or not an option; their respiratory symptoms were relieved with placement of pigtail catheter chest tubes that allowed discharge from the hospital and management at home. Catheters were easy to manage at home and there were no infectious complications.

The radiographic demonstration of lymph node involvement in amyloidosis is rare. We report a case of calcified, enlarged perioartic and mediastinal lymph nodes detected by CT.

Background. Induction chemoradiotherapy followed by esophagectomy may provide results superior to those of single-modality treatment in patients with esophageal cancer. The purpose of this study was to review our experience with this approach for esophageal cancer. Methods. From 1988 to 1996, 166 consecutive patients with esophageal cancer were evaluated; 66 entered a protocol of chemotherapy (5-fluorouracil, cisplatin) concurrent with radiation (45 Gy) followed by esophagectomy. Fifty-four patients completed the protocol. Results. Toxicity associated with induction chemoradiotherapy was minimal. The actuarial survival at 12, 24, and 36 months was 59%, 42%, and 32%, respectively. The pathologic complete response (pCR) rate was 41%, with 12-, 24-, and 36-month survivals of 77%, 50%, and 45%, whereas non-pCR patients had survivals of 46%, 35%, and 23%. The difference in survival between pCR and non-pCR patients was not significant ( p = 0.13), but the difference in recurrence-free survival was significant ( p = 0.007). Conclusions. This well-tolerated protocol resulted in a high pCR. Trimodality treatment for esophageal cancer may provide long-term survival in some patients regardless of their pCR status.

Background Chest computed tomographic angiograms (CTAs) are frequently ordered for evaluation of suspected pulmonary embolism (PE) in the emergency department, but non-PE findings are often noted. Our objective was to determine the prevalence and management implications of incidental findings on chest CTAs ordered to assess for PE. Methods In a cross-sectional study, we reviewed 589 pulmonary CTAs that were ordered in the emergency department of a tertiary care hospital. We measured the prevalence of PE and placed other findings into the following 3 categories: (1) findings that provided potential alternative explanations for acute symptoms, (2) incidental findings that required clinical or radiologic follow-up, and (3) other findings that required less urgent or no follow-up. We reviewed all newly diagnosed pulmonary nodules and significant thoracic adenopathy and determined standard recommended clinical follow-up. Results Pulmonary embolism was found in 55 of 589 CTAs (9%). A total of 195 CTAs (33%) had findings that supported alternative diagnoses. A total of 141 patients (24%) had a new incidental finding that required diagnostic follow-up, including 73 patients (13%) with a new pulmonary nodule and 51 patients (9%) with new adenopathy. Using current clinical guidelines, follow-up computed tomography or another procedure would be recommended for 96% of patients with new incidental pulmonary nodules. Conclusions The CTAs that are ordered in the emergency department are more than twice as likely to find an incidental pulmonary nodule or adenopathy than a PE. Systematic approaches should be developed to help primary care physicians contend with a growing number of clinically relevant incidental radiologic findings.

PreviewFew patients survive transection of the aorta caused by blunt trauma. However, among those who do are a small number who go on to live with an unrecognized pseudoaneurysm that may rupture at any time. Because these aneurysms may be mistaken for more common disease processes, such as hilar adenopathy, atherosclerotic aneurysm, or neoplasia, the authors describe radiographic findings that suggest the correct diagnosis.

New medical image display devices or processes are commonly evaluated by anecdotal reports or subjective evaluations which are informative and relatively easy to acquire but do not provide quantitative nieasures. On the other hand, experinients eniploying ROC analysis, yield quantitative measurements but are very laborious and demand pathological proof of outcome. We have designed and are employing a new approach, which we have termed "agreement experiments," to quantitatively test the equivalence of observer performance on two systems. This was specifically developed to test whether a radiologist using a new display technique, which has some clear advantages over the standard technique, will detect and interpret diagnostic signs as he would with the standard display technique. Agreement experiments use checklists and confidence ratings to measure how well two radiologists agree on the presence of diagnostic signs when both view images on the standard display. This yields a baseline measure of agreement. Agreement measurements are then obtained when the two radiologists view cases using the new display, or display method, compared to the standard technique. If the levels of agreement when one reads from the new and one reads from the standard display are not statistically different from the baseline measures of agreement, we conclude the two systems are equivalent in conveying diagnostic signs. We will report on an experiment using this test. The experiment compares the agreement of radiological findings for chest CT studies viewed on the conventional multiformat film/lightbox to agreement of radiological findings from chest CT images presented on a multiple screen video system. The study consists of 80 chest CT studies. A retrospective analysis of 47 cases of clinical stage IB cervical carcinoma radiologically staged with computed tomography (CT) was undertaken. There were no cases where CT provided information which altered staging. In 5 cases, CT suggested extension of disease beyond the cervix which was not confirmed surgically or with other staging procedures. In 3 cases, one or more normal-sized obturator or internal iliac nodes with metastatic disease were discovered at surgery, but the high common iliac and periiaortic nodes were disease free. We conclude that routine use of CT in patients with clinical stage IB cervical carcinoma is not warranted unless the patient's body habitus precludes accurate physical examination.

Increased hepatic density on CT has been described in several clinical entities. We present a case of arsenic poisoning demonstrating both increased hepatic and splenic attenuation.

For patients with pyogenic lung abscesses who do not respond to medical therapy, thoracotomy with pulmonary resection is the widely-accepted treatment of choice. Six patients with lung abscess who failed to respond to conservative medical management were treated by percutaneous catheter drainage using small catheters (10 Fr or smaller). Five patients showed prompt clinical improvement and the sixth improved after a modification in antibiotic therapy. All patients recovered with radiographic resolution of the abscess and were well at followup periods from two months to two years. In such patients, percutaneous drainage with small catheters provides an excellent clinical result with minimal risk and trauma.

A fatal case of massive hemoptysis in a patient with isolated peripheral pulmonary artery stenosis is described. Such patients are predisposed to developing pulmonary artery aneurysms. When hemoptysis develops, erosion of an aneurysm into an adjacent bronchus should be considered and aggressive diagnostic evaluation undertaken.

A case of wandering spleen with torsion of the splenic vascular pedicle and pancreatic tail is presented. The entity was diagnosed and evaluated preoperatively with various imaging modalities (ultrasound, nuclear medicine, and CT) as well as visceral angiography.

Fulminant necrotizing arteritis can be a rapidly fatal disease with protean manifestations often suggesting other diagnoses. We present two cases with angiographic and CT correlation and discuss CT findings that are suggestive of the diagnosis.

Computed tomography was used to evaluate three patients with complicated pulmonary vascular patterns. The physiologic and anatomic changes seen on chest radiography were well defined by CT and in each case CT suggested the correct diagnosis.
