Study Design: Retrospective review. Objective: To assess long-term pulmonary function after surgical correction of adolescent idiopathic scoliosis. Summary Of Background Data: Pulmonary function is known to decline with increasing age. Although there are published reports on 2- and 5-year pulmonary function test outcomesafter various surgical approaches for adolescent idiopathic scoliosis, 10-year pulmonary function tests of these patients is unknown. Methods: Forty-nine patients were divided into two groups who underwent surgery for adolescent idiopathic scoliosis at a single institution. There were 46 female patients and three male patients. All group one patients (n = 38) had some form of chest wall disruption during their spinal fusion: group 1A (n = 17) underwent open anterior spinal fusion/instrumentation, group 1B (n = 9) had a combined open anteroposterior spinal fusion, and group 1C (n = 12) had a posterior spinal fusion/instrumentation with thoracoplasty. Group 2 patients (n = 11) underwent posterior spinal fusion/instrumentation with iliac crest bone graft and no chest cage disruption. We evaluated preoperative and 10-year pulmonary function test values. Results: Within-group comparison revealed that group 1 experienced no change in forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV(1)), but demonstrated a significant decrease in percent-predicted FVC (85% vs. 79%, P = 0.00086) and FEV(1) values (80% vs. 76%, P = 0.038). Conversely, group 2 experienced a significant increase in both FVC (3.25L vs. 3.66L, P = 0.011) and FEV(1) (2.77L vs. 3.10L, P = 0.0019), but no changes in percent-predicted values from baseline to 10 years. No changes were noted in percent-predicted values between 5 and 10 years in either group. Conclusion: Intraoperative chest wall violation produced detrimental pulmonary effects, as reflected by reduced percent-predicted values at 10-year follow-up. Background: There have been no standardized surgical options for severe scoliotic curvatures ≥100°. Halo-gravity traction is a viable option for surgical treatment of severe scoliosis. The aim of this study was to evaluate the efficacy and safety of perioperative halo-gravity traction for scoliosis curves ≥100° with respect to radiographic outcomes and clinical complications. Methods: A total of 21 scoliosis patients with ≥100° curves (average 118.7°; range 100°-158°) with a minimum 2-year follow-up (average 41.8 months; range 24.0-97.0 months) who underwent spinal instrumented fusion using perioperative halo-gravity traction were analyzed. Diagnoses were neuromuscular scoliosis (n = 10), idiopathic (n = 9), and congenital (n = 2). In all, 15 patients were treated by the anterior release procedure followed by final posterior fusion and 6 patients by posterior fusion alone. Six patients had only preoperative traction preceding posterior fusion alone, 6 patients only staged traction between anterior release and final posterior fusion, and 9 patients had both preoperative traction preceding anterior release and staged traction preceding final posterior fusion. The average overall traction period in all patients was 67 days (range 10-78 days). Results: Radiographic outcomes demonstrated 51.3% correction of the major Cobb angle, 40 mm correction of apical vertebral translation, 76 mm increase of T1-S1 length, and 20.7% increase of space available for lungs at the ultimate follow-up (all comparisons P < 0.05). Preoperative traction demonstrated 27.5% correction of the major curve Cobb angle, 51.5 mm increase of T1-S1 length, 14.9% increase of space available for the lungs (all comparisons P < 0.05). Staged traction after anterior release demonstrated 37.2% correction of the major curve Cobb angle, 26.1 mm correction of apical vertebral translation, 56.5 mm increase of T1-S1 length, 14.2% increase of space available for the lungs (all comparisons P < 0.05). There were only two patients with a pin-site problem, and one required débridement. There were no neurological deficits or clinical complications. Conclusions: Scoliosis patients with ≥100° curves can be managed successfully by corrective fusion surgery concomitant with perioperative halo-gravity traction without significant complications. Study Design: Prospective clinical series. Objective: To determine how many thoracic scoliotic pedicles have cancellous versus cortical versus absent channels. Summary Of Background Data: Although morphologic evaluations of thoracic pedicles have been well reported, the results do not practically reflect clinical findings during actual pedicle screw placement. We propose a novel pedicle channel classification describing the osseous anatomy encountered during pedicle probe insertion. Methods: We noted 4 pedicle types in 53 consecutive scoliosis patients. Type A: pedicle probe smoothly inserted without difficulty; the morphology is described as a "Large Cancellous Channel." Type B: pedicle probe inserted snugly with increased force; described as a "Small Cancellous Channel." Type C: pedicle probe cannot be manually pushed but must be tapped with a mallet down the pedicle into the body; described as a "Cortical Channel." Type D: pedicle probe cannot locate a channel thus necessitating a "juxtapedicular" screw position; described as a "Slit/Absent Channel." The average age at time of surgery was 23.4 ± 16.7 years. Diagnoses included idiopathic scoliosis (n = 38) and syndromic scoliosis (n = 15). The average main thoracic Cobb angle was 73° ± 26°. Evaluation of pedicle morphology of the 4 types was also performed in 21 consecutive cases of adolescent idiopathic scoliosis using preoperative computed tomography images. Results: A total of 1021 pedicles with screws placed were evaluated. The average percent per type was as follows: 61.0% type A; 29.2% type B, 6.8% type C, and 3.0% type D. On the convexity, 98.2% of pedicles were type A or B versus 81.5% on the concavity (P < 0.05). There were significant differences between adolescent versus adult idiopathic scoliosis (P = 0.007), and syndromic scoliosis versus adult idiopathic scoliosis (P = 0.017) regarding pedicle morphologic proportions. There was a significant tendency toward a decrease in the proportion of type A pedicles, an increase in the proportion of type B pedicles as the Cobb angle increased (P < 0.0001). Evaluation based on 312 thoracic pedicles in 21 consecutive adolescent idiopathic scoliosis patients using preoperative computed tomography axial images confirmed assumptions of the 4 pedicle types. Conclusion: We propose a classification for pedicle channels describing the osseous anatomy encountered during pedicle probe insertion. Based on the classification, surprisingly, we found during surgery that 90% of thoracic pedicles had a cancellous channel, whereas 7% had a cortical channel and only 3% had an absent channel. Unlabelled: The ability to treat severe pediatric and adult spinal deformities through an all-posterior vertebral column resection (VCR) has obviated the need for a circumferential approach in primary and revision surgery, but there is limited literature evaluating this new approach. Our purpose was therefore to provide further support of this technique. We reviewed 43 patients who underwent a posterior-only VCR using pedicle screws, anteriorly positioned cages, and intraoperative spinal cord monitoring between 2002 and 2006. Diagnoses included severe scoliosis, global kyphosis, angular kyphosis, or kyphoscoliosis. Forty (93%) procedures were performed at L1 or cephalad in the spinal cord (SC) territory. Seven patients (18%) lost intraoperative neurogenic monitoring evoked potentials (NMEPs) data during correction with data returning to baseline after prompt surgical intervention. All patients after surgery were at their baseline or showed improved SC function, whereas no one worsened. Two patients had nerve root palsies postoperatively, which resolved spontaneously at 6 months and 2 weeks. Spinal cord monitoring (specifically NMEP) is mandatory to prevent neurologic complications. Although technically challenging, a single-stage approach offers dramatic correction in both primary and revision surgery of severe spinal deformities.Level Of Evidence: Level IV, therapeutic study. Study Design: Retrospective matched cohort analysis. Objective: To determine if posterior-only (post-only) surgical techniques consisting of pedicle screws, osteotomies, transforaminal lumbar interbody fusion, and bone morphogenetic protein-2 may provide similar results as compared anterior (thoracotomy/thoracoabdominal)/posterior surgical approaches for the treatment of adult spinal deformity with respect to correction, fusion rates, or outcomes. Summary Of Background Data: Combined anterior/posterior (A/P) fusion has traditionally been used to treat many adult scoliosis deformities. Anterior approaches negatively impact pulmonary function and require additional operative time and anesthesia. Methods: Twenty-four patients who had A/P fusion for primary adult scoliosis (16 staged, 8 same-day) were matched with a cohort of 24 patients who had post-only treatment. Anterior fusion was performed via a thoracotomy (n = 1)/thoracoabdominal (n = 23) approach. All post-only surgeries were under one anesthesia. Minimum 2-year follow-up included radiographic, clinical, and outcomes data. Results: There were no significant differences between groups for age, gender, diagnosis, comorbidities, preoperative curve magnitudes, or global balance. Postoperative radiographic correction and alignment were similar for both groups except for thoracolumbar curve percent improvement which was statistically better in the post-only group (P = 0.03). The average surgical time was higher in A/P versus post-only group (11.6 vs. 6.9 hours, P < 0.0001) as was total estimated blood loss (1330 vs. 980 mL, P = 0.04). Hospital length of stay (LOS) was longer in A/P versus post-only group (11.9 vs. 8.3 days, P = 0.03). There were no significant differences between postoperative complications. Revision surgery was performed in 5 A/P and 2 post-only patients. Higher pseudarthrosis rates found in the A/P versus post-only (17 vs. 0%) were not significant (P = 0.11). SRS-30 and Oswestry scores reflected a similar patient assessment before surgery, and improvement between groups at follow-up. Conclusion: Post-only adult scoliosis surgery achieved similar correction to A/P surgery while decreasing blood loss, operative time, length of stay, and avoiding additional anesthesia. STUDY DESIGN: Retrospective study. OBJECTIVE: To analyze the prevalence of and reasons for unanticipated revision surgery in an adult spinal deformity population treated at one institution. SUMMARY OF BACKGROUND DATA: No recent studies exist that analyze the rate or reason for unanticipated revision surgery for adult spinal deformity patients over a long period. METHODS: All patients presenting for primary instrumented spinal fusion with a diagnosis of adult deformity at a single institution from 1985 to 2008 were reviewed using a prospectively acquired database. All surgical patients with instrumented fusion of > or =5 levels using hooks, hybrid, or screw-only constructs were identified. Patient charts and radiographs were reviewed to provide information as to the indication for initial and any subsequent reoperation. A total of 643 patients underwent primary instrumented fusion for a diagnosis of adult idiopathic scoliosis (n = 432), de novo degenerative scoliosis (n = 104), adult kyphotic disease (n = 63), or neuromuscular scoliosis (n = 45). The mean age was 37.9 years (range, 18-84). Mean follow-up for the entire cohort was 4.7 years, and 8.2 years for the subset of the cohort requiring reoperation (range, 1 month-22.3 years). RESULTS: A total of 58 of 643 patients (9.0%) underwent at least one revision surgery and 15 of 643 (2.3%) had more than one revision (mean 1.3; range, 1-3). The mean time to the first revision was 4.0 years (range, 1 week-19.7 years). The most common reasons for revision were pseudarthrosis (24/643 = 3.7%; 24/58 = 41.4%), curve progression (13/643 = 2.0%; 13/58 = 20.7%), infection (9/643 = 1.4%; 9/58 = 15.5%), and painful/prominent implants (4/643 = 0.6%; 4/58 = 6.9%). Uncommon reasons consisted of adjacent segment degeneration (3), implant failure (3), neurologic deficit (1), and coronal imbalance (1). Revision rates over the follow-up period were: 0 to 2 years (26/58 = 44.8%), 2 to 5 years (17/58 = 29.3%), 5 to 10 years (7/58 = 12.1%), >10 years (8/58 = 13.8%). CONCLUSION: Repeat surgical intervention following definitive spinal instrumented fusion for primary adult deformity performed at a single institution demonstrated a relatively low rate of 9.0%. Study Design: A retrospective comparative study. Objective: To investigate the morphologic features of proximal vertebral fractures in adults following spinal deformity surgery using segmental pedicle screw instrumentation. Summary Of Background Data: Fractures above pedicle screw constructs are a clinical problem that warrants further investigation for prevention and treatment. Methods: Ten adult patients (6 lumbar scoliosis, 4 degenerative sagittal imbalance) who underwent segmental spinal instrumented fusion were analyzed. Patients were divided into 2 groups according to the features of vertebral fracture: upper instrumented vertebral collapse + adjacent vertebral subluxation (SUB group: n = 5), and adjacent vertebral fracture (Fracture group: n = 5). Results: Both groups demonstrated a high frequency of osteopenia and all patients in the SUB group had comorbidities before surgery. The SUB group demonstrated a shorter interval between initial surgery and the fracture (subluxation: 3 +/- 1.9 months; fracture: 33 +/- 25.3 months, P < 0.05), and hypokyphosis (T5-T12) in the thoracic region before surgery (SUB: 13 degrees +/- 6.4 degrees; fracture: 33 degrees +/- 15.6 degrees). Both groups demonstrated severe global sagittal imbalance (SUB: 151 +/- 62.8 mm; fracture: 94 +/- 102.2 mm), and hypolordosis (T12-S1) in the lumbar spine (SUB: -19 degrees +/- 24.4 degrees ; fracture: -33 degrees +/- 22.7 degrees) before surgery. Global sagittal imbalance in the SUB group was corrected to 8 +/- 17.4 mm immediately postoperative (P < 0.05), but increased to 64 +/- 19.9 mm after the junctional fractures (P < 0.05). The SUB group demonstrated a significantly higher wedging rate (SUB: 65% +/- 12.4%; fracture: 36% +/- 16.0%, P < 0.05) and greater local kyphosis (SUB: 42 degrees +/- 11.1 degrees; fracture: 17 degrees +/- 4.1 degrees, P < 0.05) after the fracture. Two of 5 patients in the SUB group demonstrated severe neurologic deficit from E to B after the fractures by a modified Frankel classification. Conclusion: Old age, osteopenia, preoperative comorbidities, and severe global sagittal imbalance were found to be frequent in patients with proximal junctional fracture. In addition, marked correction of sagittal malalignment might be considered as a risk factor of upper instrumented vertebra collapse followed by adjacent vertebral subluxation, which occurred in the first 6 months after corrective surgery with the potential for causing severe neurologic deficit because of the severe local kyphotic deformity. STUDY DESIGN: Retrospective clinical outcome study. OBJECTIVE: To evaluate the clinical outcomes and satisfaction associated with the surgical treatment of neuromuscular spinal deformity secondary to cerebral palsy. SUMMARY OF BACKGROUND DATA: Controversy still exists regarding whether spinal deformity surgery is truly a beneficial surgery for patients with cerebral palsy (CP) since there is limited functional benefit and higher perioperative complications rates in this patient population. METHODS: Neuromuscular patient evaluation questionnaires were answered retrospectively by 84 patients/families of spastic CP patients undergoing spinal fusion. The average follow-up was 6.2 years (range: 2-16). The questionnaires were designed to assess expectation, cosmesis, function, patient care, quality of life, pulmonary function, pain, health status, self-image, and satisfaction. Questionnaire results, complications, and radiographic data were divided into 'satisfied group' and 'less satisfied group' and we analyzed reasons of satisfaction and dissatisfaction. RESULTS: The overall satisfaction rate was 92%. Ninety-three percent reported improvement with sitting balance, 94% with cosmesis, and 71% in patient's quality of life. Functional improvements seemed limited, but 8% to 40% of the patients still perceived the surgical results as improvement. The postoperative complication rate was 27%. The mean preoperative Cobb angle of the major curve was 88 degrees (range: 53 degrees-141 degrees), which corrected to 39 degrees (range: 5 degrees-88 degrees) after surgery. The less satisfied group had a significantly higher late complication rate, less correction of the major curve, greater residual major curve, and hyperlordosis of the lumbar spine after surgery. CONCLUSION: Despite the perioperative difficulties seen with CP patients, the majority of the patient/parents were satisfied with the results of the spinal deformity surgery. Functional improvements were limited but 8% to 40% of the patients still perceived the results as improved. The reason for less than optimal satisfaction appears to be due to less correction of the major curve, greater residual major Cobb angle, hyperlordosis of the lumbar spine after surgery, and late postoperative complications. Study Design: Retrospective review of a prospectively accrued patient cohort. Objective: The ability to treat severe pediatric spinal deformity through an all-posterior vertebral column resection (VCR) has obviated the need for a circumferential approach in both primary and revision settings. We examined indications, correction rates, and complications of this challenging procedure in the pediatric population. Summary Of Background Data: Traditionally, severe pediatric spinal deformities were treated through a combined anterior/posterior spinal fusion. Methods: Between 2000 and 2005, 35 consecutive patients underwent a posterior-only VCR by 1 of 2 surgeons at a single institution. Patients were divided into 5 diagnostic categories: (1) severe scoliosis (S) (n = 2; mean, 115 degrees; range, 79-150 degrees; average flexibility, 12%); (2) global kyphosis (GK) (n = 3; mean, 101 degrees; range, 91-113 degrees; average flexibility, 16%); (3) angular kyphosis (AK) (n = 10; mean, 86 degrees; range, 45-135 degrees, average flexibility, 23%); (4) kyphoscoliosis (KS) (n = 8; mean kyphosis, 103 degrees/scoliosis 87 degrees; mean combined, 190 degrees; range, 144-237 degrees); (5) congenital scoliosis (CS) (n = 12; mean, 43 degrees; range, 23-69 degrees; average flexibility, 20%). There were 20 primary/15 revision surgeries. There were 20 one-level, 11 two-level, and 4 three-level resections. Results: The major curve correction averaged: Group S = 61 degrees/51%, Group GK = 56 degrees/55%, Group AK = 51 degrees/58%, Group KS = 98 degrees/54%, and Group CS = 24 degrees/60%. The average OR time was 460 minutes (range, 210-822), with an average EBL of 691 mL (range, 125-2200). There were no spinal cord-related complications; however, 2 patients (8.5%) lost intraoperative neuromonitoring data during correction with data returning to baseline following prompt surgical intervention. Two patients had implant revisions, 1 for a delayed deep infection at 2 years and the other for implant prominence at 3-year follow-up. Conclusion: A posterior-based VCR is a safe but challenging technique to treat severe primary or revision pediatric spinal deformities. Intraoperative SCM (especially motor-evoked potentials) is mandatory to prevent spinal cord-related neurologic complications. STUDY DESIGN: A matched cohort comparison of adult idiopathic scoliosis (AdIS) patients treated with all pedicle screw constructs compared to hook/hybrid constructs. OBJECTIVE: To compare clinical and radiographic results of AdIS treatment using all pedicle screw constructs versus hook/hybrid constructs. SUMMARY OF BACKGROUND DATA: Pedicle screw instrumentation has demonstrated excellent clinical efficacy in the treatment of pediatric spinal deformity. No prior reports have compared the outcomes of pedicle screw only constructs to hook/hybrid constructs in the treatment of AdIS. METHODS: We analyzed 34 consecutive patients undergoing posterior-only correction for AdIS, using pedicle screw instrumentation at minimum 2-year follow-up. Thirty-four matching patients (11 with anterior releases) were selected from a cohort of 58 patients treated with hook/hybrid constructs based on similar age, curve type, magnitude, and fusion levels. RESULTS: Significantly greater curve correction was seen in the pedicle screw compared to the hook/hybrid group (56 vs. 40%, P < 0.01). Coronal and sagittal imbalance were equivalent between the groups (P = 0.91 and 0.23, respectively). Thoracic kyphosis (T5-T12) was maintained in the pedicle screw patients but significantly increased in the hybrid/hook patients over time (P < 0.05). Scoliosis Research Society outcome scores significantly improved in both groups. Blood loss was equivalent but operative time was longer in the hook/hybrid patients.No pedicle screw patients were revised for instrumentation complications with 1 lumbosacral nonunion revised at 5 years postoperative (3% revision rate). Eight of 58 patients among the hook/hybrid cohort underwent 9 revisions for instrumentation failure (n = 3) or nonunion (n = 6) (14% revision rate; P = 0.04). CONCLUSION: Pedicle screw correction of AdIS is safe and effective. Compared to hook/hybrid constructs, these patients displayed significantly improved correction of the major curve (even in the absence of anterior releases), maintenance of thoracic kyphosis, and a lower revision rate. Similar SRS scores and blood loss were noted. The hook/hybrid patients had significantly longer operative times. The use of BMP-2 in 7 patients and TLIF/ALIF also in 7 of the pedicle screw patients may have decreased the need for revision surgery up to the latest follow-up. [ABSTRACT FROM AUTHOR]

STUDY DESIGN: Retrospective study. OBJECTIVE: To analyze the complications and patient satisfaction related to an anterior thoracolumbar approach in the treatment of adult spinal deformity. SUMMARY OF BACKGROUND DATA: There is no long-term follow-up data on the effects of an anterior thoracolumbar approach on adult spinal deformity patients. METHODS: A specific questionnaire was used to evaluate long-term follow-up (average, 10.3 years; range, 5-20.6) of 62 adult patients who underwent spinal deformity surgery performed through an anterior thoracolumbar approach. Twenty-six patients had over 10 years follow-up and 36 were between 5 and 10 years follow-up. The questionnaire was composed of detailed scar-related subquestions for pain, appearance, bulging, daily life, and patient's personal opinion of surgery. Postoperative Oswestry Disability Index (scores) were also obtained. RESULTS: The average age and number of anterior fusion levels were 47.9 (range, 20-74) and 5.6 (range, 2-12), respectively. Although 82.2% patients were satisfied with the results of their surgery, in general, many of the patients were dissatisfied with aspects related to their anterior incision. For the pain domain, 20 patients (32.3%, 6 with >10 years follow-up, 14 patients with 5 years follow-up) had pain over their thoracolumbar scar, which they rated as moderate to severe. Twenty-seven patients (43.5%) had bulging of their scar region, 4 were surgically indicated for repair, and 1 had multiple surgical repairs. Twelve patients (19.4%) felt they had a poor outcome related to the postoperative appearance of their anterior wound. Fifteen patients (24.2%) showed limitations in activities of daily living due to their anterior incision. Three patients with >10 years of follow-up and 4 with >5 years of follow-up felt they were getting worse. The average Oswestry Disability Index score was 25.0 +/- 16.3 (range, 0-52) postoperative. CONCLUSION: This is the first long-term (minimum 5 years) follow-up study focusing on patient outcomes after an anterior thoracolumbar approach for adult spinal deformity treatment. This approach appears to be associated with an appreciable high rate of postoperative pain (32.3%), bulging (43.5%), and functional disturbance (24.2%). Study Design: Retrospective study. Objective: To determine the appropriate distal fusion level in posterior instrumentation and fusion for thoracic hyperkyphosis by investigating the relationship between the sagittal stable vertebra ([SSV]-the most proximal lumbar vertebral body touched by the vertical line from the posterior-superior corner of the sacrum), first lordotic vertebra (just caudal to the first lordotic disc), and selected lowest instrumented vertebra (LIV). Summary Of Background Data: It has been recommended that the distal end vertebra and the first lordotic disc beyond the transitional zone distally be included in distal fusion for thoracic hyperkyphosis; however, we have seen distal junctional breakdown even when these rules have been followed. Methods: Thirty-one patients (mean age: 18 years, range: 13-38) who underwent long posterior instrumentation and fusion for thoracic hyperkyphosis with a minimum 2-year follow-up were reviewed. Preoperative diagnoses included Scheuermann kyphosis (n = 29), post-traumatic kyphosis (n = 1), and postlaminectomy kyphosis (n = 1). According to the distal fusion level, patients were divided into 2 groups. Group I (n = 24): LIV included the SSV; group II (n = 7): the LIV was proximal to the SSV. Patients were evaluated using standing radiographs and chart review. Results: Preoperative mean thoracic kyphosis was 86.6 +/- 8.5 degrees and 53.0 +/- 10.4 degrees at final follow-up with a correction rate of 39%. Preoperative average sagittal balance was slightly negative (-0.24 +/- 3.8 cm), and became slightly more negative (-1.33 +/- 2.8 cm) by final follow-up. There were no statistical differences in thoracic kyphosis between the 2 groups. However, there was a statistically significant difference with group II having a more posterior translation of the center of the LIV from the posterior sacral vertical line before surgery and at final follow-up (P = 0.003). In group I, distal junctional problems developed in 2 of 24 (8%) patients and in group II, problems occurred in 5 of 7 (71%) patients (P < 0.05). Despite extending the fusion to the first lordotic vertebra, distal junctional problems developed in 3 of 8 (38%) patients. Conclusion: The distal end of a fusion for thoracic hyperkyphosis should include the SSV. Study Design: Preoperative review of a prospective study, single institution, consecutive series. OBJECTIVE.: To analyze the intermediate-term follow-up of consecutive adolescent idiopathic scoliosis (AIS) patients treated with pedicle screw constructs. Summary Of Background Data: There have been no reports of the intermediate-term findings in North America following posterior spinal fusion with the use of pedicle screw-only constructs. Methods: One hundred and fourteen consecutive patients having a minimum 3-year follow-up (mean 4.8 +/- 1.1; range, 3.0-7.3 years) with AIS were evaluated. The average age at surgery was 14.9 +/- 2.2 years. Radiographic measurements included preoperative (Preop), postoperative (PO), 2-year (2 years), and final follow-up (FFU). A chart review evaluated PFTs, Scoliosis Research Society scores, presence of thoracoplasty, Risser sign, Lenke classification, and complications. Results: The most frequent curve pattern was Lenke type 1 (45.6%), followed by type 3 (21.9%). The average main thoracic curve measured 59.2 degrees +/- 12.2 SD Preop, and corrected to 16.8 degrees +/- 9.9 PO (P < 0.0001). Sagittal thoracic alignment (T5-T12) decreased from 25.8 degrees to 15.5 degrees at FFU (P = 0.05). Nash-Moe grading for apical vertebral rotation (AVR) in the proximal thoracic curve decreased from 2.0 Preop to 1.1 at FFU (P < 0.0001), and AVR in the thoracolumbar/lumbar spine decreased from 1.6 Preop to 1.1 at FFU (P < 0.0001). Importantly, the horizontalization of the subjacent disc measured -8.3 degrees Preop which decreased to -0.9 degrees PO (P < 0.001). PFT follow-up averaged 2.4 years with a 7.1% improvement in FVC (P = 0.004) and 8.8% in FEV1 (P < 0.0001). SRS scores averaged 83.0% at latest follow-up. Age, gender, Risser sign, or complications did not have a significant effect on outcomes. There were 2 cases of adding-on, 3 late onset infections, 1 with a single pseudarthrosis, but no neurologic complications. Conclusion: This is the largest (N = 114), consecutive series of North American patients with AIS treated with pedicle screws having a minimum of 3-year follow-up. Objectives: To analyze radiographic and functional outcomes after posterior segmental spinal instrumentation and fusion (PSSIF) with and without an anterior apical release of the lumbar curve in adult scoliosis patients. Summary Of Background Data: No comparison study on PSSIF of adult lumbar scoliosis with apical release versus without has been published. Methods: Forty-eight adult patients with lumbar scoliosis (average age at surgery 49.6 years, average follow-up 3.7 years) who underwent PSSIF were analyzed with respect to radiographic change, perioperative and postoperative complications, and Scoliosis Research Society (SRS) outcome scores. Twenty-three patients underwent an anterior apical release of the lumbar curve via a thoracoabdominal approach followed by PSSIF (Group I). The remaining 25 patients underwent a PSSIF of the lumbar curve followed by anterior column support at the lumbosacral region through an anterior paramedian retroperitoneal or posterior transforaminal approach (Group II). Results: Before surgery, Group I showed a somewhat larger lumbar major Cobb angle (63.2 degrees vs. 55.9 degrees , P = 0.07), and both groups demonstrated significant differences in lumbar curve flexibility (26.9% vs. 37.2%, P = 0.02) and thoracolumbar kyphosis (27.0 degrees vs. 15.0 degrees , P = 0.03). After surgery, at the ultimate follow-up, there were no significant differences in major Cobb angle, C7 plumbline to the center sacral vertical line (P = 0.17), C7 plumbline to the posterior superior endplate of S1 (P = 0.44), and sagittal Cobb angles at the proximal junction (P = 0.57), T10-L2 (P = 0.24) and T12-S1 (P = 0.51). There were 4 pseudarthroses in Group I and one in Group II (P = 0.02). Postoperative total normalized SRS outcome scores at ultimate follow-up were significantly higher in Group II (69% vs. 79%, P = 0.01). Conclusion: Posterior segmental spinal instrumentation and fusion without anterior apical release of lumbar curves in adult scoliosis demonstrated better total SRS outcome scores and no differences in radiographic parameters without differences in clinical complications. Study Design: A retrospective consecutive case series. Objective: To illustrate a new surgical technique for closing osteotomies in fixed sagittal imbalance patients, and to evaluate the radiographic outcomes and complications. Summary Of Background Data: During an osteotomy site closure, various techniques were used, most of which involve patient positioning, rod cantilevering, extending the fixation points, allowing less correction, and compressing through pedicle fixation points. Most of these techniques place added stress on the spinal pedicle screw fixation points, which may lead to screw loosening and eventual fixation failure. We used a central compression hook-rod construct for osteotomy closure to avoid adding compression force to the pedicle screws. Methods: Twenty-three consecutive patients in whom fixed sagittal imbalance was treated with a lumbar pedicle subtraction osteotomy or multilevel Smith-Petersen Osteotomies (SPO) using a central compression hook-rod construct for osteotomy closure were analyzed. Compression hooks were inserted into the fusion mass above and below the osteotomy site and centrally attached to a short rod. Compression forces were applied to securely close the osteotomy site. The mean age was 49.0 years (range, 33-75 years). Diagnoses included 11 scoliosis, 10 degenerative sagittal imbalance, and 2 ankylosing spondylitis patients. All but 2 cases were revisions. Sixteen patients had a pedicle subtraction osteotomy, and 7 had multilevel SPOs. A radiographic and clinical analysis was performed to evaluate the efficacy and possible complications of this technique. Results: Overall, lumbar lordosis increased an average of 31 degrees (range, 11 degrees -68 degrees ) and local lordosis through the osteotomy site increased by an average of 28 degrees (range, 3 degrees -47 degrees ). The C7 sagittal plumbline improved an average of 109 mm (range, 11-240 mm). In all cases, the osteotomy closures were performed without screw loosening or loss of correction intraoperatively. There were no hook-rod construct failures, but one transient root paresis, which might be related to this method. There was no pseudarthroses or significant correction loss after surgery. Conclusion: A central hook-rod construct is a safe, quick, controlled, and effective method for spinal osteotomy site closure. It may add strength to the overall construct and avoids the placement of direct compression force on the main pedicle screw fixation points that may lead to screw loosening during the osteotomy site closure, and ultimately to fixation failure. Study Design: : A retrospective study. Objective: : To evaluate the safety of free-hand pedicle screw placement during revision surgery at levels with a solid posterior fusion mass and/or identified pseudarthroses of the thoracic and lumbosacral spine. Summary Of Background Data: : Placement of pedicle screws into previous fusion masses or pseudarthrosis levels of the spine is challenging because of the loss of anatomic landmarks. We are aware of no study focusing on screw placement without any type of radiographic or navigational assistance. Methods: : Thirty-seven patients underwent revision spinal surgery with posterior spinal instrumentation and fusion using 552 transpedicular screws by a single surgeon from 1994 to 2003. Among 552 screws, 184 were inserted into virgin levels, 60 were inserted into the vertebral body through previous screw holes, 208 were inserted into fusion masses, and 100 were inserted into pseudarthrosis levels. We used the quadrangulation method to gain pedicle access in a prior fusion mass after diligent exposure of any and all visible anatomic landmarks. All screws were analyzed using radiographs, intraoperative monitoring data, and clinical outcomes. Results: : There were 29 women and 8 men. The mean age of patients at the time of surgery was 38 + 6 years (range, 8-75 years). Six screws were removed intraoperatively according to the triggered electromyography criteria and 4 of them were repositioned after checking the integrity of the pedicle by a careful probing technique (1.09%). Six screws were removed after checking the intraoperative radiographs and 5 screws were misplaced, as noted, in postoperative radiographs (1.99%). Two patients underwent revision surgery at 3 days and 6 weeks after initial surgery, respectively, for root decompression at the osteotomy site although screw positions were correct. Conclusion: : The free-hand technique of thoracic and lumbosacral pedicle screw placement in revision spinal surgery is reliable and safe when using the quadrangulation method of gaining pedicle access in a prior fusion mass or at pseudarthrosis levels. Study Design: A retrospective review of pediatric kyphosis patients undergoing a spinal cord-level osteotomy for correction. Objective: To evaluate the prevalence, etiology, timing, and intervention related to loss of spinal cord monitoring data during surgical correction of pediatric kyphosis in the spinal cord region. Summary Of Background Data: Although much has been written regarding the risks inherent to scoliosis surgery, there is less literature available regarding the neurologic outcomes of pediatric kyphosis surgery. As more surgeons contemplate posterior-only kyphosis correction with spinal cord-level osteotomies, the importance of maintaining spinal cord neurologic function is paramount. Methods: Forty-two patients with pediatric kyphosis undergoing a posterior-only spinal reconstruction with a spinal cord level osteotomy or posterior-based vertebral column resection performed were reviewed. Patients were categorized by diagnosis, type and incidence of osteotomies, and loss of neurogenic mixed-evoked potential (NMEP) data. Interventions required to regain data and postoperative neurologic outcomes were also reviewed. Results: Of the 42 patients, 9 (21.4%) demonstrated a complete loss of NMEP data sometime during surgery while concomitant somatosensory sensory-evoked potentials (SSEP) remained within acceptable limits of baseline values. All 9 patients had intraoperative intervention including: blood pressure elevation (n = 1), release of corrective forces (n = 2), blood pressure elevation and correction release (n = 3), malalignment/subluxation adjustment (n = 1), further bony decompression (n = 1), or restoration of anterior column height via a titanium cage along with further posterior decompression (n = 1). In all cases, SSEPs were unchanged and NMEPs returned varying from 8 to 20 minutes after loss, with all patients having a normal wake-up test intraoperatively and a normal neurologic examination after surgery. Conclusion: Intraoperative multimodality monitoring with some form of motor tract assessment is a fundamental component of kyphosis correction surgery in the spinal cord region in order to create a safer, optimal environment and to minimize neurologic deficit. Study Design: A retrospective comparative study. Objective: To compare the efficacy and safety of several different anchors in the apical levels of scoliotic curves > or = 100 degrees using radiographic outcomes and clinical complications. Summary Of Background Data: To the best of our knowledge, no reports have compared various anchors at the apical level for correction of scoliosis curves > or = 100 degrees. Methods: Sixty-eight scoliosis patients (44 neuromuscular, 21 idiopathic, and 3 congenital) with major curves > or = 100 degrees (mean, 112.7 degrees; range, 100 degrees -159 degrees ) who underwent segmental spinal instrumentation and fusion with different anchors in the apical level were analyzed. All patients had a minimum 2-year follow-up (mean, 4.0 years; range, 2.0-10.5) and were divided into Group W (sublaminar wires n = 26), Group H (hooks n = 18), Group A (anterior vertebral screws n = 7), and Group PS (pedicle screws n = 17) based on the type of apical anchor used. Radiographic parameters and complications were investigated. Results: The 4 groups did not demonstrate any significant differences in gender, age at surgery, preoperative major Cobb angle, or curve flexibility (all P > 0.05). However, the PS group demonstrated a shorter follow-up period compared with the other 3 groups (P < 0.05). The PS group demonstrated the greatest correction rate, smallest loss of correction (P < 0.05), and greatest amount of correction of the apical vertebral translation (P < 0.0005) at ultimate follow-up. There were 4 cases (5.9%) of pseudarthrosis (3 in Group W, 1 in Group H; P > 0.05), 6 cases (8.8%) of implant failure (4 in Group W, 2 in Group H; P > 0.05). Despite one (1.5%) intraoperative neurologic complication (differences among groups, P > 0.05), there was no permanent neurologic deficit. Conclusion: All 4 constructs were able to achieve and maintain acceptable correction safely without permanent neurologic deficit and all demonstrated acceptable implant failure rate. Pedicle screw constructs in the apical levels demonstrated the best coronal correction, smallest loss of correction, and greatest amount of apical vertebral translation correction of the major Cobb angle compared with the other constructs without neurologic complications. Study Design: Prospective clinical study of a retrospective database. Objective: To compare the prospective pulmonary function changes following anterior spinal instrumentation and fusion through an open thoracotomy with the same procedure through a thoracoabdominal approach at 2 years follow-up. Summary Of Background Data: Open anterior spinal instrumentation and fusion for adolescent idiopathic scoliosis (AIS) has been known to decrease pulmonary function after surgery. However, the differential effect of an open thoracotomy versus thoracoabdominal approach on pulmonary function in AIS is unknown. Methods: Sixty-four AIS patients who underwent an anterior spinal instrumentation and fusion through an open thoracotomy (TC group) for a major main thoracic scoliosis were compared with 55 patients who underwent the same procedure through a thoracoabdominal approach (TA group) for a major thoracolumbar/lumbar (TL/L) scoliosis using pulmonary function tests assessing forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) before surgery and 2 years after surgery. The TC group was divided into 2 groups, TC group with thoracoplasty (1-5 ribs, n = 35) and TC group without thoracoplasty (n = 29). Results: Preoperative and 2-year postoperative absolute FVC in the TC group averaged 3.05L and 2.74L, respectively (0.31L decrease, P < 0.0001) versus 3.27L and 3.21L, respectively (0.06L decrease, P = 0.15) in the TA group. The TC group demonstrated a significantly larger decrease in absolute FVC at 2 years postoperative (P < 0.0001). Preoperative and 2-year postoperative absolute FEV1 in the TC group averaged 2.56L and 2.35L, respectively (0.21L decrease, P < 0.0001) versus 2.82L and 2.81L, respectively (0.02L decrease, P = 0.67) in the TA group. The TC group demonstrated a significantly larger decrease in absolute FEV1 at 2 years postoperative (P = 0.001). TC patients with thoracoplasty (n = 35) demonstrated a similar average decrease (0.34L) of absolute FVC at 2 years postoperative compared with those without thoracoplasty (0.26L, P = 0.49) and a similar average decrease of absolute FEV1 at 2 years postoperative (0.23L vs. 0.19L without thoracoplasty, P = 0.76). Conclusion: An open thoracotomy approach for treating a main thoracic curve demonstrated a significant decrease in the absolute pulmonary function tests values at 2 years postoperative while the thoracoabdominal approach for treating a TL/L curve did not. Study Design: Retrospective study. Objective: Determine proximal junctional kyphosis (PJK) prevalence and analyze risk factors associated with PJK in adolescent idiopathic scoliosis (AIS) patients following 3 different posterior segmental spinal instrumentation and fusion surgeries. Summary Of Background Data: No comparison study exists on proximal junctional AIS changes following 3 different segmental posterior spinal instrumentation and fusion surgeries at 2 years postoperative. Methods: A clinical/radiographic assessment was conducted in 410 consecutive AIS patients (average age = 14.7, range = 10.6-20) (men/women = 73/337) treated with instrumented segmental posterior spinal fusion with 2-year follow-up. Revision and anterior cases were not included. Standing long-cassette radiographic measurements were analyzed including various sagittal/coronal parameters for preoperative, early postoperative, and 2-year follow-up. Abnormal PJK was defined by proximal junction sagittal Cobb angles between the lower endplate of the uppermost instrumented vertebra and the upper endplate of 2 supradjacent vertebrae >or=+10 degrees and at least 10 degrees greater than the preoperative measurement at 2 years postoperative. Results: PJK prevalence defined at 2 years postoperative was 27% (111 of 410 patients). Statistically significant factors: larger preoperative thoracic kyphosis angle (T5-T12 >40 degrees vs. T5-T12 10 degrees -40 degrees vs. T5-T12 <10 degrees ; P < 0.0001), greater immediate postoperative thoracic kyphosis angle decrease (decrease >5 degrees vs. 5 degrees decrease-5 degrees increase vs. increase >5 degrees ; P < 0.0001), thoracoplasty versus no thoracoplasty (P = 0.001), and men versus women (P = 0.007). Instrumentation types (hook-only vs. proximal hook, distal pedicle screw vs. pedicle screw P = 0.058), number of fused vertebrae >12 versus 12>or= (P = 0.12), the uppermost instrumented vertebra among T2, T3, T4, T5 (P = 0.75). There were no significant differences in Scoliosis Research Society Patient Questionnaire-24 outcome-scores (PJK total score = 97.0, self-image subscales = 21.3 vs. non-PJK group = 95.3, 21.0) (P = 0.34 total score, P = 0.54 self-image subscale). Conclusion: Two-year postoperative PJK prevalence in AIS following 3 different posterior segmental spinal instrumentation and fusion surgeries was 27%. A larger preoperative thoracic kyphosis angle, greater immediate postoperative thoracic kyphosis angle decrease, thoracoplasty, and male sex correlated significantly with PJK. Study Design: A retrospective review. Objective: To evaluate the incremental accuracy of pedicle screws used in spinal deformity via a free-hand technique at a single institution over an 8-year period. Summary Of Background Data: The in vivo accuracy of free-hand pedicle screws placed throughout the deformed spine as evaluated by computed tomography (CT) scanning is unknown over a long time period. Methods: A total of 1023 pedicle screws inserted from T1 to L4 in 60 patients (928 screws in 54 scoliosis patients and 95 screws in 6 kyphosis patients) over an 8-year period were investigated via postoperative CT scans. Patients were divided into 3 groups (group I = 1998-1999, group II = 2001-2002, and group III = 2005). All pedicle screws were inserted via the free-hand technique using anatomic landmarks, specific entry sites, neurophysiologic, and radiographic confirmation. Pedicle screw position on CT scan was graded as acceptable versus violated, defined as the screw axis being outside the pedicle wall. Results: One hundred seven of 1023 pedicle screws (10.5%) demonstrated significant mediolateral pedicle wall violations (19 medial vs. 88 lateral, P = 0.001). groups I and III had significantly higher lateral wall violations than group II (P < 0.05) as did the kyphotic spines (vs. scoliotic spine, P < 0.05). There were significantly more screws placed in the periapical region over time (P < 0.0001), with left-sided lateral violations (T5-T8) increasing from group II to group III, while the number of medial violations significantly decreased with time (P < 0.0001). Pedicle screws placed on the right side showed a significant decrease in accuracy from group II to group III (P = 0.03). The average transverse angle of the acceptable screws was 15.3 degrees which was significantly different from the medial (23.0 degrees , P < 0.001) and lateral (10.6 degrees , P < 0.001) violations between group I and group II. No screws demonstrated neurologic, vascular, or visceral complications. Conclusion: Overall accuracy of acceptable screws using the free-hand pedicle screw placement technique in the deformed spine was 89.5%, without any neurologic, vascular, or visceral complications over an 8-year period. Study Design: Retrospective review. Objective: To determine the reliability of supine long-cassette radiographs as compared with side-bending films in predicting curve flexibility in operative cases of adolescent idiopathic scoliosis (AIS). Summary Of Background Data: The value of side-bending films is important in the classification of AIS, as well as predicting curve flexibility. Methods: A total of 675 patients with a diagnosis of operative AIS were evaluated. All curves were classified by the Lenke classification. Coronal parameters included: proximal thoracic (PT), main thoracic (MT), and thoracolumbar/lumbar (TL/L) Cobb measurements; sagittal data including: T2-T5, T5-T12, and TL/L measurements. Curves were divided into Lenke Types 1 (N = 263), 2 (N = 118), 3 (N = 52), 4 (N = 31), 5 (N = 57), and 6 (N = 54). Lenke Types 1 to 4 (Group I-MT Major) were compared with Types 5 and 6 (Group II-TL/L Major). Results: For Group I, MT supine films were highly predictive of MT side-bending while TL/L supine films were highly predictive of TL/L side-bending and standing films. An equation was derived to predict the value of the side-bending radiographs for each part of the curve. For Group II, MT supine films were highly predictive of MT side-bending and standing films. TL/L supine films were highly predictive of TL/L side-bending and standing films. Contingency table analysis for Group I resulted in the supine film providing a strong statistical ability to predict a nonstructural PT curve (sensitivity = 0.952, PPV = 0.864, NPV = 0.865) and also a nonstructural TL/L curve (sensitivity = 0.958, PPV = 0.916). Similarly, in Group II, we found a strong statistical ability to predict a nonstructural PT (sensitivity 1.00, PPV = 0.982, NPV = 1.00) and a nonstructural MT curve (sensitivity 0.789, specificity = 0.842, PPV = 0.833, NPV = 0.80). Conclusion: A single preoperative supine radiograph is highly predictive of side-bending radiographs and can be used as an adjunct to predicting curve type, flexibility, and structurality. STUDY DESIGN: A retrospective analysis of 1078 spinal surgical procedures with lumbar pedicle screw placement at a single institution. OBJECTIVE: Based on previously established normative values, triggered electromyographic stimulation (TrgEMG) was re-examined to evaluate its efficacy in determining screw malposition. SUMMARY OF BACKGROUND DATA: Threshold values for confirmation of intraosseous placement of pedicle screws with EMG stimulation is controversial. METHODS: TrgEMG threshold values for 4857 pedicle screws placed from L2 to S1 from 1996 to 2005 were analyzed. An ascending method of constant current stimulation was applied to each pedicle screw to obtain a compound muscle action potential (CMAP) from lower extremity myotomes. Previously determined threshold value normative data from a published clinical series of 233 screws were as follows: 0 to 4 mA, high likelihood of pedicle wall breach; 4 to 8 mA, possible pedicle wall breach; >8 mA, no pedicle wall defect. RESULTS: A total of 7.74% (376 of 4857) of all screws tested had threshold values <8.0 mA. A total of 19.1% (72 of 376) of these were <4.0 mA: 54% (39 of 72) were repositioned (26) or removed (13) while the remaining 33 screws were left in place following repalpation. A total of 80.9% (304 of 376) had thresholds between 4 and 8 mA: 17.4% (53) were repositioned (38) or removed (15). Nine screws had thresholds of 8.0 mA, 17.4% for 4.0 to 8.0 mA, 54.2% for <4.0 mA, and 100% for <2.8 mA. At 2.8 mA, triggered EMG has a specificity of 100%, with sensitivity of 8.4%; at 4.0 mA, specificity of 99% and sensitivity of 36%; and at 8.0 mA, 94% specificity and 86% sensitivity. STUDY DESIGN: Prospective clinical study. OBJECTIVE: To investigate if a correlation exists between various parameters including major thoracic curve correction and postoperative pulmonary function test (PFT) improvement at 2 years postoperative following posterior segmental spinal fusion (PSSF) and instrumentation with iliac crest bone graft (ICBG). SUMMARY OF BACKGROUND DATA: There are no studies available on the correlation between major thoracic curve correction and postoperative PFT improvement following PSSF and instrumentation with ICBG with a homogenous diagnosis, similar operation method, and similar age population. METHODS: One hundred thirty-nine patients with adolescent idiopathic scoliosis (Lenke type 1-4), undergoing PSSF and instrumentation with ICBG at a single institution, were before surgery and 2 years after surgery prospectively evaluated in regard to PFTs, assessing forced vital capacity, and forced expiratory volume in 1 second. PFTs change at 2 years postoperative was compared by the various parameters including major thoracic Cobb curve correction and the types of instrumentation. We defined a significant clinical improvement as a 10% or more increase of percent predictive FEV1 value at 2 years postoperative. RESULTS: PSSF and instrumentation with ICBG demonstrated statistically significant improvement of absolute and percent-predicted PFTs at 2 years postoperative. There was a significant clinical improvement in 31 patients (22%) at 2 years postoperative. Significant clinical improvement was related to thoracic pedicle screw instrumentation (vs. thoracic hook instrumentation, P = 0.030). Background: The reported prevalence of non-neurologic complications following corrective surgery for adolescent idiopathic scoliosis ranges from 0% to 10%. However, most studies were retrospective evaluations of treatment techniques and did not focus solely on complications. The purpose of this study was to determine the prevalence of non-neurologic complications following surgery for adolescent idiopathic scoliosis and to identify preoperative and operative factors that can increase this risk.Methods: The demographic data, medical and surgical histories, and prevalence of non-neurologic complications were reviewed in a prospective cohort of 702 patients who had undergone corrective surgery for adolescent idiopathic scoliosis and were consecutively enrolled in a multicenter database.Results: There were 556 female and 146 male patients. The mean age at the time of surgery was 14.25 years (range, eight to eighteen years). Five hundred and twenty-three patients had only posterior spinal surgery, 105 had only anterior spinal surgery, and seventy-four had a combined anterior and posterior procedure. There was a total of 108 complications in eighty-one patients, for an overall prevalence of 15.4%. There were ten respiratory complications (1.42%), six cases of excessive bleeding (0.85%), five wound infections (0.71%), and five cases of wound hematoma, seroma, or dehiscence (0.71%). Five patients, two with an early infection and three with late failure of the implant, required a reoperation. Factors that did not correlate with an increased prevalence of complications were age, body mass index, presence of cardiac or respiratory disease, previous surgery, pulmonary function, surgical approach, number of levels fused, graft material, use of a diaphragmatic incision, Lenke curve type, or region of the major curve. Although the number of patients with renal disease was small, these patients were 7.90 times more likely to have a non-neurologic complication. Increased blood loss as well as prolonged operative and anesthesia times were associated with a higher prevalence of non-neurologic complications.Conclusions: The prevalence of non-neurologic postoperative complications following surgery for correction of adolescent idiopathic scoliosis in this study was 15.4%. The few factors noted to significantly increase the rate of complications include a history of renal disease, increased operative blood loss, prolonged posterior surgery time, and prolonged anesthesia time. Study Design: A retrospective analysis, including prospectively collected patient outcomes data. Objective: To determine the rate of complications and outcomes in patients >or=60 years of age who underwent major spinal deformity surgery requiring a minimum 5-level arthrodesis procedure. Summary Of Background Data: As the population ages, an increasing number of older patients are presenting with spinal deformity disorders that may require major reconstructive procedures. Previous studies have reported complication rates as high as 80% in this age group for 1- and 2-level fusion procedures. The prevalence of complications was found to increase with the greater number of levels fused. Methods: Forty-six patients who were 60 years of age or older underwent a thoracic or lumbar arthrodesis procedure consisting of 5 levels or greater. Diagnosis, comorbidities, operative data, hospital data, major and minor complications, and deaths were recorded. Oswestry Disability Index (ODI) Scores were used to evaluate clinical outcomes. Results: Thirty-eight females and 8 males with a mean age of 67 years (range, 60-85 years) and a mean follow-up of 4.2 years (range, 2-11 years) had complete records. Thirty-six (78%) patients had at least 1 comorbidity. Twenty-nine (63%) patients had at least 1 prior spinal surgery. A mean of 9 levels (range, 5-16 levels) were fused in each patient. The overall complication rate was 37%. The major complication rate was 20%. ODI improved from 49 to 25 for a mean improvement of 24 (49%) (P < 0.0001). Conclusion: The overall complication rate was 37% and the major complication rate was 20%. Increasing age was a significant factor (P < 0.05) in predicting the presence of a complication. Patients older than 69 years had more complications. The presence of a comorbidity had no association with complication rates and neither had an effect on final patient reported outcomes, which showed significant improvement (ODI preoperative, 49; postoperative, 25) (P < 0.0001).
