Abstract: Future cosmic microwave background (CMB) experiments are primarily targeting a detection of the primordial $B$-mode polarisation. The faintness of this signal requires exquisite control of systematic effects which may bias the measurements. In this work, we derive requirements on the relative calibration accuracy of the overall polarisation gain ($\Delta g_\nu$) for LiteBIRD experiment, through the application of the blind Needlet Internal Linear Combination (NILC) foreground-cleaning method. We find that minimum variance techniques, as NILC, are less affected by gain calibration uncertainties than a parametric approach, which requires a proper modelling of these instrumental effects. The tightest constraints are obtained for frequency channels where the CMB signal is relatively brighter (166 GHz channel, $\Delta {g}_\nu \approx 0.16 \%$), while, with a parametric approach, the strictest requirements were on foreground-dominated channels. We then propagate gain calibration uncertainties, corresponding to the derived requirements, into all frequency channels simultaneously. We find that the overall impact on the estimated $r$ is lower than the required budget for LiteBIRD by almost a factor $5$. The adopted procedure to derive requirements assumes a simple Galactic model. We therefore assess the robustness of obtained results against more realistic scenarios by injecting the gain calibration uncertainties, according to the requirements, into LiteBIRD simulated maps and assuming intermediate- and high-complexity sky models. In this case, we employ the so-called Multi-Clustering NILC (MC-NILC) foreground-cleaning pipeline and obtain that the impact of gain calibration uncertainties on $r$ is lower than the LiteBIRD gain systematics budget for the intermediate-complexity sky model. 2. Unveiling V Modes: Enhancing CMB Sensitivity to BSM Physics with a Non-Ideal Half-Wave Plate

Abstract: V-mode polarization of the cosmic microwave background is expected to be vanishingly small in the $\Lambda$CDM model and, hence, usually ignored. Nonetheless, several astrophysical effects, as well as beyond standard model physics could produce it at a detectable level. A realistic half-wave plate - an optical element commonly used in CMB experiments to modulate the polarized signal - can provide sensitivity to V modes without significantly spoiling that to linear polarization. We assess this sensitivity for some new-generation CMB experiments, such as the LiteBIRD satellite, the ground-based Simons Observatory and a CMB-S4-like experiment. We forecast the efficiency of these experiments to constrain the phenomenology of certain classes of BSM models inducing mixing of linear polarization states and generation of V modes in the CMB. We find that new-generation experiments can improve current limits by 1-to-3 orders of magnitude, depending on the data combination. Abstract: Large angular scale surveys in the absence of atmosphere are essential for measuring the primordial $B$-mode power spectrum of the Cosmic Microwave Background (CMB). Since this proposed measurement is about three to four orders of magnitude fainter than the temperature anisotropies of the CMB, in-flight calibration of the instruments and active suppression of systematic effects are crucial. We investigate the effect of changing the parameters of the scanning strategy on the in-flight calibration effectiveness, the suppression of the systematic effects themselves, and the ability to distinguish systematic effects by null-tests. Next-generation missions such as LiteBIRD, modulated by a Half-Wave Plate (HWP), will be able to observe polarisation using a single detector, eliminating the need to combine several detectors to measure polarisation, as done in many previous experiments and hence avoiding the consequent systematic effects. While the HWP is expected to suppress many systematic effects, some of them will remain. We use an analytical approach to comprehensively address the mitigation of these systematic effects and identify the characteristics of scanning strategies that are the most effective for implementing a variety of calibration strategies in the multi-dimensional space of common spacecraft scan parameters. We also present Falcons, a fast spacecraft scanning simulator that we developed to investigate this scanning parameter space.

4. Relational event models with global covariates

Abstract: Traditional inference in relational event models from dynamic network data involves only dyadic and node-specific variables, as anything that is global, i.e. constant across dyads, drops out of the partial likelihood. We address this with the use of nested case-control sampling on a time-shifted version of the event process. This leads to a partial likelihood of a degenerate logistic additive model, enabling efficient estimation of global and non-global covariate effects. The method's effectiveness is demonstrated through a simulation study. An application to bike sharing data reveals significant influences of global covariates like weather and time of day on bike-sharing dynamics.

Abstract: The current standard model of cosmology successfully describes a variety of measurements, but the nature of its main ingredients, dark matter and dark energy, remains unknown. Euclid is a medium-class mission in the Cosmic Vision 2015-2025 programme of the European Space Agency (ESA) that will provide high-resolution optical imaging, as well as near-infrared imaging and spectroscopy, over about 14,000 deg^2 of extragalactic sky. In addition to accurate weak lensing and clustering measurements that probe structure formation over half of the age of the Universe, its primary probes for cosmology, these exquisite data will enable a wide range of science. This paper provides a high-level overview of the mission, summarising the survey characteristics, the various data-processing steps, and data products. 6. Controlled Query Evaluation through Epistemic Dependencies

Abstract: In this paper, we propose the use of epistemic dependencies to express data protection policies in Controlled Query Evaluation (CQE), which is a form of confidentiality-preserving query answering over ontologies and databases. The resulting policy language goes significantly beyond those proposed in the literature on CQE so far, allowing for very rich and practically interesting forms of data protection rules. We show the expressive abilities of our framework and study the data complexity of CQE for (unions of) conjunctive queries when ontologies are specified in the Description Logic DL-Lite_R. Interestingly, while we show that the problem is in general intractable, we prove tractability for the case of acyclic epistemic dependencies by providing a suitable query rewriting algorithm. The latter result paves the way towards the implementation and practical application of this new approach to CQE.

In this paper, we delve into the fascinating realm of slow-roll inflation, contextualizing the primordial acoustic perturbations as the resonant echoes akin to the iconic sound of Chewbacca from the Star Wars universe. By extrapolating polynomial potentials for these primordial sounds, we illuminate their role in shaping the inflationary landscape. Leveraging this framework, we calculate the scalar spectral index ($n_s$) and tensor-to-scalar ratio ($r$), providing insights into the underlying physics governing the inflationary epoch. Employing a rigorous chi-square ($\chi^2$) analysis, we meticulously scrutinize the Planck data combined with that offered by the BICEP/Keck collaboration to identify the Chewbacca sound profile that best aligns with observational constraints. Abstract: We present detailed forecasts for the constraints on primordial magnetic fields (PMFs) that will be obtained with the LiteBIRD satellite. The constraints are driven by the effects of PMFs on the CMB anisotropies: the gravitational effects of magnetically-induced perturbations; the effects on the thermal and ionization history of the Universe; the Faraday rotation imprint on the CMB polarization; and the non-Gaussianities induced in polarization anisotropies. LiteBIRD represents a sensitive probe for PMFs and by exploiting all the physical effects, it will be able to improve the current limit coming from Planck. In particular, thanks to its accurate $B$-mode polarization measurement, LiteBIRD will improve the constraints on infrared configurations for the gravitational effect, giving $B_{\rm 1\,Mpc}^{n_{\rm B} =-2.9} < 0.8$ nG at 95% C.L., potentially opening the possibility to detect nanogauss fields with high significance. We also observe a significant improvement in the limits when marginalized over the spectral index, $B_{1\,{\rm Mpc}}^{\rm marg}< 2.2$ nG at 95% C.L. From the thermal history effect, which relies mainly on $E$-mode polarization data, we obtain a significant improvement for all PMF configurations, with the marginalized case, $\sqrt{\langle B^2\rangle}^{\rm marg}<0.50$ nG at 95% C.L. Faraday rotation constraints will take advantage of the wide frequency coverage of LiteBIRD and the high sensitivity in $B$ modes, improving the limits by orders of magnitude with respect to current results, $B_{1\,{\rm Mpc}}^{n_{\rm B} =-2.9} < 3.2$ nG at 95% C.L. Finally, non-Gaussianities of the $B$-mode polarization can probe PMFs at the level of 1 nG, again significantly improving the current bounds from Planck. Abstract: We present a study of the impact of an uncertainty in the beam far side-lobe knowledge on the measurement of the Cosmic Microwave Background $B$-mode signal at large scale. It is expected to be one of the main source of systematic effects in future CMB observations. Because it is crucial for all-sky survey missions to take into account the interplays between beam systematic effects and all the data analysis steps, the primary goal of this paper is to provide the methodology to carry out the end-to-end study of their effect for a space-borne CMB polarization experiment, up to the cosmological results in the form of a bias $\delta r$ on the tensor-to-scalar ratio $r$. LiteBIRD is dedicated to target the measurement of CMB primordial $B$ modes by reaching a sensitivity of $\sigma \left( r \right) \leq 10^{-3}$ assuming $r=0$. As a demonstration of our framework, we derive the relationship between the knowledge of the beam far side-lobes and the tentatively allocated error budget under given assumptions on design, simulation and component separation method. We assume no mitigation of the far side-lobes effect at any stage of the analysis pipeline. We show that $\delta r$ is mostly due to the integrated fractional power difference between the estimated beams and the true beams in the far side-lobes region, with little dependence on the actual shape of the beams, for low enough $\delta r$. Under our set of assumptions, in particular considering the specific foreground cleaning method we used, we find that the integrated fractional power in the far side-lobes should be known at a level as tight as $\sim 10^{-4}$, to achieve the required limit on the bias $\delta r < 1.9 \times 10^{-5}$. The framework and tools developed for this study can be easily adapted to provide requirements under different design, data analysis frameworks and for other future space-borne experiments beyond LiteBIRD.

Abstract: We estimate the efficiency of mitigating the lensing $B$-mode polarization, the so-called delensing, for the $LiteBIRD$ experiment with multiple external data sets of lensing-mass tracers. The current best bound on the tensor-to-scalar ratio, $r$, is limited by lensing rather than Galactic foregrounds. Delensing will be a critical step to improve sensitivity to $r$ as measurements of $r$ become more and more limited by lensing. In this paper, we extend the analysis of the recent $LiteBIRD$ forecast paper to include multiple mass tracers, i.e., the CMB lensing maps from $LiteBIRD$ and CMB-S4-like experiment, cosmic infrared background, and galaxy number density from $Euclid$- and LSST-like survey. We find that multi-tracer delensing will further improve the constraint on $r$ by about $20\%$. In $LiteBIRD$, the residual Galactic foregrounds also significantly contribute to uncertainties of the $B$-modes, and delensing becomes more important if the residual foregrounds are further reduced by an improved component separation method., Comment: 21 pages, 7 figures

Abstract: We explore the capability of measuring lensing signals in $LiteBIRD$ full-sky polarization maps. With a $30$ arcmin beam width and an impressively low polarization noise of $2.16\,\mu$K-arcmin, $LiteBIRD$ will be able to measure the full-sky polarization of the cosmic microwave background (CMB) very precisely. This unique sensitivity also enables the reconstruction of a nearly full-sky lensing map using only polarization data, even considering its limited capability to capture small-scale CMB anisotropies. In this paper, we investigate the ability to construct a full-sky lensing measurement in the presence of Galactic foregrounds, finding that several possible biases from Galactic foregrounds should be negligible after component separation by harmonic-space internal linear combination. We find that the signal-to-noise ratio of the lensing is approximately $40$ using only polarization data measured over $90\%$ of the sky. This achievement is comparable to $Planck$'s recent lensing measurement with both temperature and polarization and represents a four-fold improvement over $Planck$'s polarization-only lensing measurement. The $LiteBIRD$ lensing map will complement the $Planck$ lensing map and provide several opportunities for cross-correlation science, especially in the northern hemisphere.

Abstract: We study the possibility of using the $LiteBIRD$ satellite $B$-mode survey to constrain models of inflation producing specific features in CMB angular power spectra. We explore a particular model example, i.e. spectator axion-SU(2) gauge field inflation. This model can source parity-violating gravitational waves from the amplification of gauge field fluctuations driven by a pseudoscalar "axionlike" field, rolling for a few e-folds during inflation. The sourced gravitational waves can exceed the vacuum contribution at reionization bump scales by about an order of magnitude and can be comparable to the vacuum contribution at recombination bump scales. We argue that a satellite mission with full sky coverage and access to the reionization bump scales is necessary to understand the origin of the primordial gravitational wave signal and distinguish among two production mechanisms: quantum vacuum fluctuations of spacetime and matter sources during inflation. We present the expected constraints on model parameters from $LiteBIRD$ satellite simulations, which complement and expand previous studies in the literature. We find that $LiteBIRD$ will be able to exclude with high significance standard single-field slow-roll models, such as the Starobinsky model, if the true model is the axion-SU(2) model with a feature at CMB scales. We further investigate the possibility of using the parity-violating signature of the model, such as the $TB$ and $EB$ angular power spectra, to disentangle it from the standard single-field slow-roll scenario. We find that most of the discriminating power of $LiteBIRD$ will reside in $BB$ angular power spectra rather than in $TB$ and $EB$ correlations., Comment: 22 pages, 13 figures. Submitted to JCAP

Abstract: Statistical physics and dynamical systems theory are key tools to study high-impact geophysical events such as temperature extremes, cyclones, thunderstorms, geomagnetic storms and many more. Despite the intrinsic differences between these events, they all originate as temporary deviations from the typical trajectories of a geophysical system, resulting in well-organised, coherent structures at characteristic spatial and temporal scales. While statistical extreme value analysis techniques are capable to provide return times and probabilities of occurrence of certain geophysical events, they are not apt to account for their underlying physics. Their focus is to compute the probability of occurrence of events that are large or small with respect to some specific observable (e.g. temperature, precipitation, solar wind), rather than to relate rare or extreme phenomena to the underlying anomalous geophysical regimes. Abstract: Abstract Psoriasis is a chronic, immune-mediated, inflammatory skin disease, associated with multiple comorbidities and psychological and psychiatric disorders. The quality of life of patients with this disease is severely compromised, especially in moderate-to-severe plaque psoriasis. Secukinumab, a fully humanized monoclonal antibody, was the first anti-interleukin (IL)-17 biologic approved for treating psoriasis. Secukinumab demonstrated long-lasting efficacy and a good safety profile in individuals with plaque psoriasis, and it is associated with an improvement in health-related quality of life. While there is evidence that early treatment with systemic therapy can affect disease progression and improve long-term outcomes in other autoimmune diseases, evidence is limited in psoriasis, especially in real-world settings. This review provides an overview of studies describing the effectiveness of secukinumab in the treatment of psoriasis summarizing the literature and focusing on real-world evidence and early intervention.

Abstract: Background Weight loss trajectories after bariatric surgery vary widely between individuals, and predicting weight loss before the operation remains challenging. We aimed to develop a model using machine learning to provide individual preoperative prediction of 5-year weight loss trajectories after surgery. Methods In this multinational retrospective observational study we enrolled adult participants (aged $\ge$18 years) from ten prospective cohorts (including ABOS [NCT01129297], BAREVAL [NCT02310178], the Swedish Obese Subjects study, and a large cohort from the Dutch Obesity Clinic [Nederlandse Obesitas Kliniek]) and two randomised trials (SleevePass [NCT00793143] and SM-BOSS [NCT00356213]) in Europe, the Americas, and Asia, with a 5 year followup after Roux-en-Y gastric bypass, sleeve gastrectomy, or gastric band. Patients with a previous history of bariatric surgery or large delays between scheduled and actual visits were excluded. The training cohort comprised patients from two centres in France (ABOS and BAREVAL). The primary outcome was BMI at 5 years. A model was developed using least absolute shrinkage and selection operator to select variables and the classification and regression trees algorithm to build interpretable regression trees. The performances of the model were assessed through the median absolute deviation (MAD) and root mean squared error (RMSE) of BMI. Findings10 231 patients from 12 centres in ten countries were included in the analysis, corresponding to 30 602 patient-years. Among participants in all 12 cohorts, 7701 (75$\bullet$3%) were female, 2530 (24$\bullet$7%) were male. Among 434 baseline attributes available in the training cohort, seven variables were selected: height, weight, intervention type, age, diabetes status, diabetes duration, and smoking status. At 5 years, across external testing cohorts the overall mean MAD BMI was 2$\bullet$8 kg/m${}^2$ (95% CI 2$\bullet$6-3$\bullet$0) and mean RMSE BMI was 4$\bullet$7 kg/m${}^2$ (4$\bullet$4-5$\bullet$0), and the mean difference between predicted and observed BMI was-0$\bullet$3 kg/m${}^2$ (SD 4$\bullet$7). Abstract: We propose a new framework for combining entity resolution and query answering in knowledge bases (KBs) with tuple-generating dependencies (tgds) and equality-generating dependencies (egds) as rules. We define the semantics of the KB in terms of special instances that involve equivalence classes of entities and sets of values. Intuitively, the former collect all entities denoting the same real-world object, while the latter collect all alternative values for an attribute. This approach allows us to both resolve entities and bypass possible inconsistencies in the data. We then design a chase procedure that is tailored to this new framework and has the feature that it never fails; moreover, when the chase procedure terminates, it produces a universal solution, which in turn can be used to obtain the certain answers to conjunctive queries. We finally discuss challenges arising when the chase does not terminate.

Abstract: Abstract Background Antimicrobial resistance (AMR) poses a significant global health threat, particularly in low- and middle-income countries (LMICs). Self-treatment with antibiotics, the practice of using antibiotics without professional guidance, is often considered an important contributor to the emergence and spread of AMR. Method This study investigated the drivers of self-treatment in three common types of agricultural communities in northern Tanzania. The research employed a comprehensive array of methods, including cross-sectional surveys (n = 790), interviews (n = 30) and observations (n = 178) targeting both antibiotic (human and animal) providers and users (patients and farmers). Qualitative interview data were analysed using a coding and association matrix, while descriptive analyses were performed on survey and observation data. Results Self-treatment with antibiotics was highly prevalent in all communities. Between 41.0% (self-reported) and 60.3% (observed) of human antibiotics were obtained without a prescription and we observed that veterinary antibiotics were regularly purchased in retail shops without referral by a professional. Structural deficiencies in the healthcare system drove this practice: limited access to healthcare facilities, medication stockouts and prolonged waiting times were identified as key factors. The absence of safety nets like insurance schemes further contributed to self-medication. Retail shops offered a convenient and cost-effective alternative when antibiotics were inaccessible or unaffordable. Notably, informal networks comprising treatment vendors, friends or neighbours, as well as personal experiences played a crucial role in guiding individuals in their self-treatment decisions by providing advice on treatment choice and modalities. Conclusions Addressing self-treatment requires a multi-faceted approach. Improving the availability and accessibility of antibiotics, enhancing healthcare services and involving retail vendors in antibiotic stewardship are essential. Structural issues like access to diagnostics and medicines must be tackled, alongside reducing barriers and incentivising individuals to use professional healthcare services. Training retail vendors to sell specific first-line antibiotics over the counter with guidance on appropriate usage should be considered. Such bottom-up interventions will enable sustainable promotion of responsible antibiotic use, mitigating AMR emergence and securing a healthier future for all.

Abstract: Abstract Obesity condition causes morphological and functional alterations involving the cardiovascular system. These can represent the substrates for different cardiovascular diseases, such as atrial fibrillation, coronary artery disease, sudden cardiac death, and heart failure (HF) with both preserved ejection fraction (EF) and reduced EF. Different pathogenetic mechanisms may help to explain the association between obesity and HF including left ventricular remodelling and epicardial fat accumulation, endothelial dysfunction, and coronary microvascular dysfunction. Multi‐imaging modalities are required for appropriate recognition of subclinical systolic dysfunction typically associated with obesity, with echocardiography being the most cost‐effective technique. Therapeutic approach in patients with obesity and HF is challenging, particularly regarding patients with preserved EF in which few strategies with high level of evidence are available. Weight loss is of extreme importance in patients with obesity and HF, being a primary therapeutic intervention. Sodium–glucose co‐transporter‐2 inhibitors have been recently introduced as a novel tool in the management of HF patients. The present review aims at analysing the most recent studies supporting pathogenesis, diagnosis, and management in patients with obesity and HF.

Abstract: Abstract Total protein levels in fish are widely used in health and welfare studies, providing a simple and accessible measure. However, the multifaceted role of blood proteins makes it sometimes challenging to link total protein content to specific health issues, while specific protein fractions may offer more precise insights into fish biology and health, particularly in farmed fish species where such data is often lacking. Data were gathered from two experiments involving Dicentrarchus labrax and Sparus aurata, key species in European marine aquaculture. The aim was (1) to assess how different globulin fractions contribute to total protein content in blood and (2) how these contributions vary across different sampling times in healthy animals. In D. labrax, the beta1 globulin fraction emerged as the major contributor (34.16%), followed by albumin and alpha2 globulins (18.24% and 16.41%, respectively). In contrast, pre-albumins and alpha1 fractions had the least contribution (5.49% and 7.71%). S. aurata exhibited albumin as the primary contributor (23.39%), followed by beta1 and alpha2 globulins (19.71% and 19.15%, respectively), with gamma and alpha1 fractions contributing the least (5.34% and 8.63%). Notably, the study revealed relatively stable contributions of globulin fractions to total proteins within both species, albeit with minor variations over time, potentially linked to environmental and individual factors. Furthermore, larger fish displayed higher total protein levels. This research underscores the need for further investigation into the diverse factors influencing globulin contributions to total proteins, ultimately enhancing health and welfare monitoring for farmed fish species.

Abstract: Abstract The African buffalo (Syncerus caffer) is a wild bovid with a historical distribution across much of sub-Saharan Africa. Genomic analysis can provide insights into the evolutionary history of the species, and the key selective pressures shaping populations, including assessment of population level differentiation, population fragmentation, and population genetic structure. In this study we generated the highest quality de novo genome assembly (2.65 Gb, scaffold N50 69.17 Mb) of African buffalo to date, and sequenced a further 195 genomes from across the species distribution. Principal component and admixture analyses provided little support for the currently described four subspecies. Estimating Effective Migration Surfaces analysis suggested that geographical barriers have played a significant role in shaping gene flow and the population structure. Estimated effective population sizes indicated a substantial drop occurring in all populations 5-10,000 years ago, coinciding with the increase in human populations. Finally, signatures of selection were enriched for key genes associated with the immune response, suggesting infectious disease exert a substantial selective pressure upon the African buffalo. These findings have important implications for understanding bovid evolution, buffalo conservation and population management.

Abstract: BACKGROUND & AIMS: Practice guidelines promote a routine noninvasive, non-endoscopic initial approach to investigating dyspepsia without alarm features in young patients, yet many patients undergo prompt upper endoscopy. We aimed to assess tradeoffs among costs, patient satisfaction, and clinical outcomes to inform discrepancy between guidelines and practice. METHODS: We constructed a decision-analytic model and performed cost-effectiveness/cost-satisfaction analysis over a 1-year time horizon on patients with uninvestigated dyspepsia without alarm features referred to gastroenterology. A RAND/UCLA expert panel informed model design. Four competing diagnostic/management strategies were evaluated: prompt endoscopy, testing for Helicobacter pylori and eradicating if present (test-and-treat), testing for H pylori and performing endoscopy if present (test-and-scope), and empiric acid suppression. Outcomes were derived from systematic reviews of clinical trials. Costs were informed by prospective observational cohort studies and national commercial/federal cost databases. Abstract: INTRODUCTION: Chronic idiopathic constipation (CIC) is a common disorder associated with significant impairment in quality of life. This clinical practice guideline, jointly developed by the American Gastroenterological Association and the American College of Gastroenterology, aims to inform clinicians and patients by providing evidence-based practice recommendations for the pharmacological treatment of CIC in adults. METHODS: The American Gastroenterological Association and the American College of Gastroenterology formed a multidisciplinary guideline panel that conducted systematic reviews of the following agents: fiber, osmotic laxatives (polyethylene glycol, magnesium oxide, lactulose), stimulant laxatives (bisacodyl, sodium picosulfate, senna), secretagogues (lubiprostone, linaclotide, plecanatide), and serotonin type 4 agonist (prucalopride). The panel prioritized clinical questions and outcomes and used the Grading of Recommendations Assessment, Development, and Evaluation framework to assess the certainty of evidence for each intervention. The Evidence to Decision framework was used to develop clinical recommendations based on the balance between the desirable and undesirable effects, patient values, costs, and health equity considerations. RESULTS: The panel agreed on 10 recommendations for the pharmacological management of CIC in adults. Based on available evidence, the panel made strong recommendations for the use of polyethylene glycol, sodium picosulfate, linaclotide, plecanatide, and prucalopride for CIC in adults. Conditional recommendations were made for the use of fiber, lactulose, senna, magnesium oxide, and lubiprostone. DISCUSSION: This document provides a comprehensive outline of the various over-the-counter and prescription pharmacological agents available for the treatment of CIC. The guidelines are meant to provide a framework for approaching the management of CIC; clinical providers should engage in shared decision making based on patient preferences as well as medication cost and availability. Limitations and gaps in the evidence are highlighted to help guide future research opportunities and enhance the care of patients with chronic constipation.

44. Probing Lorentz-violating electrodynamics with CMB polarization

Abstract: We perform a comprehensive study of the signatures of Lorentz violation in electrodynamics on the Cosmic Microwave Background (CMB) anisotropies. In the framework of the minimal Standard Model Extension (SME), we consider effects generated by renormalizable operators, both CPT-odd and CPT-even. These operators are responsible for sourcing, respectively, cosmic birefringence and circular polarization. We propagate jointly the effects of all the relevant Lorentz-violating parameters to CMB observables and provide constraints with the most recent CMB datasets. We bound the CPT-even coefficient to $k_{F,E+B} < 2.31 \times 10^{-31}$ at 95\% CL. This improves previous CMB bounds by one order of magnitude. The limits we obtain on the CPT-odd coefficients, i.e. $|k_{(V)00}^{(3)}| < 1.54 \times 10^{-44} \; {\rm GeV}$ and $|\mathbf{k_{AF}}| < 0.74 \times 10^{-44} \; {\rm GeV}$ at 95\% CL, are respectively one and two orders of magnitude stronger than previous CMB-based limits, superseding also bounds from non-CMB searches. This analysis provides the strongest constraints to date on CPT-violating coefficients in the minimal SME from CMB searches.

Abstract: Introdução: A coccidioidomicose é uma doença que ocorre em locais de clima seco, causada pelos fungos C. immitis e C. posadasii, este sendo predominante nos estados do nordeste brasileiro, como Maranhão, Ceará e Piauí, e aquele predominante nos Estados Unidos, nos estados da Califórnia e do Arizona. A exposição ao solo contaminado é o principal fator de risco para a doença. O pulmão é o órgão mais comumente afetado, mas pode haver disseminação para ossos, pele e sistema nervoso central. Objetivo: Relatar um caso de tratamento de coccidioidomicose com uso de voriconazol. Método: Coleta de dados e revisão de prontuário. Resultados: Jovem de 17 anos, natural de São Paulo e procedente da Califórnia, previamente hígido, inicia quadro subagudo de cefaleia holocraniana com piora ao decúbito. Relata quadro respiratório há um mês com resolução espontânea. Na admissão apresentava rigidez de nuca e papiledema à fundoscopia, com correspondente hidrocefalia no exame de tomografia computadorizada (TC). Exame de líquor apresentou pressão de abertura de 55 cmH2O, pleocitose linfomonocitária, hiperproteinorraquia e hipoglicorraquia. A TC de tórax evidenciou lesão pulmonar cavitária. Houve crescimento de Coccidioides spp. no líquor. O teste de sensibilidade revelou concentração inibitória mínima (CIM) elevada para fluconazol (8 µg/ml) e mais baixa (< 1 µg/ml) para os demais azólicos. Na internação, necessitou de derivação ventrículo-peritoneal (DVP) e recebeu alta com fluconazol em doses altas. Retornou em nosso serviço após 20 dias por cefaleia intensa, com piora ao decúbito. Ajustes pressóricos na DVP foram necessários devido à hidrocefalia descompensada. O líquor evidenciava pleocitose linfomonocitária, hiperproteinorraquia e hipoglicorraquia persistentes. Diante da piora associada à CIM elevada para fluconazol, optou-se pela substituição da medicação por voriconazol, com necessidade de altas doses (∼20mg/kg/d) para atingir nível sérico adequado. Conclusão: : O fluconazol é a terapia de escolha para a coccidioidomicose, a despeito das maiores CIMs entre os azólicos. O voriconazol é uma alternativa possível para casos refratários, mas há poucos estudos que suportem seu uso. Talvez nosso paciente fosse um metabolizador rápido de azólicos, explicando a melhor resposta à terapia guiada por nível sérico. Recomenda-se, entretanto, terapia vitalícia para meningoencefalite e a toxicidade do uso prolongado do voriconazol poderá ser um fator limitante.

Abstract: Introdução: Em 2024, no Estado de São Paulo até a semana epidemiológica 19 (Maio), totalizavam-se 296.763 casos de Dengue, número maior do que a somatória de casos dos últimos 10 anos. Desses, cerca de 0,5% necessitou de internação hospitalar. Diante do grande número de pacientes hospitalizados, observamos o aparecimento de casos de infecções por Staphylococcus aureus com diagnóstico recente de Dengue, porém nem todos classicamente relacionados ao cuidado hospitalar. Objetivo: Apresentar série de casos recentes de infecções estafilocócicas em pacientes hospitalizados com Dengue. Método: Análise de 5 casos de um hospital privado da cidade de São Paulo. Resultados: Os 5 casos ocorreram entre abril e maio de 2024, todos tinham um diagnóstico de Dengue anterior à infecção estafilocócica. A idade dos pacientes variou de 15 a 70 anos, todos apresetavam comorbidades, sendo hipertensão e diabetes as mais comuns (3/5. A infecção estafilocócica foi diagnosticada entre 5 e 27 dias a partir do início dos sintomas da Dengue. Dos 5 pacientes, todos apresentaram bacteremia, 3 apresentaram flebite, 2 endocardite, 1 pneumonia hematogênica, 1 espondilodiscite e 1 pioartrite de ombro. Em relação à sensibilidade, 4/5 dos isolados de S. aureus eram sensíveis a meticilina. Dois pacientes haviam recebido alta após melhora da Dengue e retornaram com infecções estafilocócicas (espondilodiscite e pioartrite de ombro). Apenas 1 paciente foi a óbito durante o acompanhamento após 16 dias, com endocardite complicada com insuficiência cardíaca grave, necessidade de ECMO e sangramento de sistema nervoso central. Conclusão: A maioria dos pacientes hospitalizados com dengue grave necessitam de acesso venoso periférico para hidratação, contribuindo com a quebra de barreira e sendo um facilitador para a bacteremia. Entretanto, observamos alguns casos com disseminação hematogênica e apresentações menos habituais, mesmo sem flebite ou outra porta de entrada óbvia. Desse modo, é possível que a infecção por S. aureus em concomitância com a dengue tenha uma fisiopatogenia multifatorial adicional. A fase virêmica da dengue produz resposta inflamatória exacerbada via antígeno NS1 com vasodilatação intensa e imunossupressão, facilitando a ação de mecanismos de virulência do S. aureus. A alteração endotelial pode ainda facilitar a translocação bacteriana. Mais estudos são necessários durante epidemias de Dengue para entender completamente sua interação com as infecções estafilocócicas.

Abstract: Introdução: A paracoccidioidomicose (PCM) é uma micose sistêmica causada por fungos do gênero Paracoccidioides. Pode se apresentar sob a forma aguda (juvenil) e crônica (adulta). Objetivo: Relatar o caso de uma gestante com PCM disseminada para medula óssea, fígado e gânglios. Método: Coleta de dados de prontuário. Resultados: Paciente do sexo feminino, de 31 anos, procedente do estado do Mato Grosso do Sul/SP, que foi transferida para hospital terciário em São Paulo/SP por quadro de febre, anemia, hepatoesplenomegalia e icterícia há um mês. Era previamente hígida e estava gestante de 34 semanas quando, foi submetida a uma cesárea de urgência por amniorrexe prematura. Permaneceu internada após o parto com quadro de anemia importante e icterícia persistente. Diante de um Coombs direto fracamente positivo e sendo descartada colestase gestacional, foi aventada a hipótese de anemia hemolítica autoimune, sendo realizada pulsoterapia com metilprednisolona ainda no serviço de origem. Na admissão hospitalar em nosso serviço estava sonolenta, ictérica e febril. Em exames de imagem foi observado fígado com aumento difuso e sinais de edema periportal, esplenomegalia e linfonodomegalias generalizadas. Evoluiu com piora progressiva da colestase, anasarca com ascite de grande monta e piora hemodinâmica. Realizou biópsias de linfonodo cervical, fígado e medula óssea. Todas as três evidenciaram processo inflamatório crônico granulomatoso com estruturas fúngicas arredondadas (leveduras capsuladas) fagocitadas por celulas gigantes multinucleadas com multibrotamentos. A imuno-histoquímica confirmou a presença de Paracoccidioides no linfonodo cervical. Em exame de contraimunoeletroforese para PCM, teve resultado reagente com título de 1/64. Foi iniciada anfotericina lipossomal (L-AmB). Após quatro semanas de L-AmB, a paciente já se encontrava afebril e com melhora progressiva clínica e laboratorial. Recebeu alta hospitalar com sulfametoxazol-trimetoprim e programação de retorno ambulatorial. Conclusão: Em contrapartida à população alvo da doença que predomina em homens e trabalhadores rurais, neste caso descrevemos uma apresentação em mulher jovem da área urbana contribuído pela crescente urbanização. Nas mulheres, a PCM incide antes da menarca ou após a menopausa, sendo infrequente na idade fértil. Durante a gestação, hormônios maternos e placentários podem resultar em uma imunodepressão componente celular, podendo aumentar o risco de incidência de infecções oportunistas.

Abstract: Introdução: A aspergilose pulmonar invasiva (API) ocorre predominantemente em pacientes onco-hematológicos com neutropenia prolongada. Entretanto, sua ocorrência após quadros infecciosos virais graves (como Influenza e COVID-19) têm sido cada vez mais descrita e estudada, principalmente no contexto de terapia intensiva. Objetivo: Relatar o caso de um paciente com dengue grave que desenvolveu aspergilose invasiva pulmonar. Método: Os dados para descrição do caso foram obtidos através da revisão de prontuário. Resultados: Trata-se de um paciente do sexo masculino, 27 anos, com antecedente de lúpus eritematoso sistêmico (LES), em uso de azatioprina. Deu entrada no serviço de emergência com hipotensão e taquicardia e história de febre alta, dor abdominal intensa e astenia há quatro dias. Em exames apresentava disfunção renal, linfopenia e plaquetopenia. A sorologia IgM para Dengue foi reagente. O paciente evoluiu com instabilidade hemodinâmica e necessidade de ventilação mecânica (VM) associada a febre persistente sem agentes isolados. Foi aventada hipótese de síndrome hemofagocítica (SHF) que foi confirmada após avaliação medular. O paciente então recebeu corticoterapia em doses altas e imunoglobulina endovenosa. Duas semanas depois, evoluiu com febre e piora de parâmetros ventilatórios. A tomografia do tórax evidenciou lesões nodulares com sinal do halo, sugestivas de aspergilose invasiva. A análise do lavado broncoalveolar revelou galactomanana com índice superior a 6 e PCR para Aspergillus fumigatus positivo. Após uma semana de tratamento com anfotericina lipossomal devido a disfunção hepática, houve melhora clínica e respiratória, com desmame da VM. Evoluiu na 4ª semana de internação com pancreatite aguda necrotizante e por refratariedade de tratamento clínico das coleções foi indicado abordagem cirúrgica. Entretanto, o paciente evoluiu a óbito no pós-operatório. Conclusão: É reconhecido que pacientes críticos acometidos por quadros virais graves são mais propensos ao desenvolvimento de DFI como a Influenza associada a aspergilose pulmonar (IAPA). A relação entre Dengue grave e o desenvolvimento de API ainda foi pouco explorada, porém há alguns relatos em literatura da coinfecção em pacientes críticos cujo elo foi justamente a ocorrência de SHF. Dada a gravidade dessa associação, mais estudos são necessários a fim de evitar desfechos desfavoráveis.

Abstract: Background: Sex-specific differences in left ventricular (LV) geometry might help in developing tailored strategies for hypertension management. Objectives: The purpose of the study was to evaluate sex-related differences in LV geometry at baseline and over time in hypertension. Methods: From a prospective registry, we included hypertensives without prevalent cardiovascular disease, incident myocardial infarction, chronic kidney disease > stage III, and with normal LV ejection fraction. LV mass index >115 g/m2 in males and >95 g/m2 in females, identified LV hypertrophy (LVH). Relative wall thickness ≥0.43 defined LV concentric geometry.
