8. Computer-assisted diagnosis of prostate cancer using DCE-MRI data: design, implementation and preliminary results

Abstract

Abstract: Objects: We present computer-assisted diagnosis (CAD) software designed to improve prostate cancer detection using perfusion MRI data. Methods: In addition to standard visualization features, this software allows for the 2D and multislice 2D contouring of suspicious areas based on a seeded region growing algorithm, and area labeling based on zonal anatomy. Tumor volume assessment and the semiquantitative analysis of DCE-MRI sequences can both be performed. We retrospectively analyzed DCE-MRI examinations of 100 patients and found 121 lesions showing a suspiciously high intensity with early enhancement in 84 of them. Seventy-one patients turned out to be malignant, whereas 50 were benign. Based on an analysis of the median wash-in and wash-out values of these foci, we designed a standardized 5-level cancer suspicion score (ranging from “probably benign” to “highly suspicious”). This comprehensive score provides a scaled likelihood of malignancy in the region of interest taking account of its location in relation to prostate zonal anatomy. We compared its accuracy with that of visual assessments of time–intensity curves performed by specialist and non-specialist radiologists. Results: Parameters of the scoring algorithm were designed to provide the greatest possible sensitivity in our sample population. A re-substitution evaluation provided an Se/Sp of 100/45% for peripheral zone cancer, and 100/40% for transition zone cancer characterization. When identifying malignant areas using time–intensity curves data, this simple algorithm performed significantly better (AUC = 0.77) than a non-specialist (AUC = 0.57, P < 0.0001) radiologist, and better than a trained (AUC = 0.70) radiologist, although this difference was not significant. Conclusion: Our new prostate MRI CAD software provides a standardized cancer suspicion score for suspicious foci detected in DCE-MRI T1-w images. Our results suggest that it may improve radiologists’ performances in prostate cancer identification, especially when they are not specialized in prostate imaging.

Abstract

Videofluoroscopic Swallow studies (VFSS) are useful radiological examinations to explore swallowing disorders but which require ionizing radiation. The aim of our study was to evaluate the comparability of pediatric VFSS at 15 frames per second (fps) with 30 fps. Fifty-five loops including 190 swallowings of VFSS at 30 fps performed on 32 consecutive pediatric patients in a University Hospital Center were retrospectively modified by a software to delete one image out of two to obtain secondary loops with a frame rate of 15 fps. An otorhinolaryngologist-phonatrician and a radiologist reviewed all swallowings blindly and randomly using the penetration and aspiration scale (PAS). In case of discordance, they concluded a consensual interpretation. Fifteen girls and seventeen boys were included. The median age was 4 years and 8 months (range = 4 months-16 yr.). 144 swallowings were normal. Swallowing disorder was confirmed in 46 swallowings, (23 supraglottic penetrations and 23 aspirations). Considering each swallowing at 15 fps, sensitivity and specificity were, respectively, 93% (CI 0.82-0.98) and 98% (CI 0.94-0.99). The Cohen'Kappa coefficient between each interpretation at 15 and 30 fps was "almost perfect" (κ = 0.95; CI 0.88-0.99). Considering each loop, conclusion was identical. Reducing frame rate at 15 fps during pediatric VFSS seemed to be acceptable with comparable diagnostic performances without clinical impact compared to 30 fps, while being an efficient way to reduce the ionizing radiation exposition in children. We would suggest reconsidering the possibility of using VFSS with a 15 fps in a pediatric population.

Abstract

Recent technologies such as those using coal, natural gas or biomass as fuel are often facing the challenge of removing H2S impurities. Among the various existing routes for sulfur removal, the conversion of transition metal oxides into sulfides is often considered for deep gas purification. The ideal regenerative system, preventing waste generation, should combine a high affinity material towards H2S and an easy way for its regeneration into the initial oxide form. The present paper describes the reactivity of the ZnMoO4 mixed oxide material and ZnO–MoO3 oxides mixture as potential candidates for the regenerative H2S sorption process. The use of the QXAS technique allowed us to get time resolved information about both sulfidation and oxidative regeneration processes at Mo and Zn K-edges. Faced with the complexity of gas–solid reactions involving several phases, QXAS in combination with multivariate data analysis enabled us to follow the sulfidation and oxidative regeneration kinetics of both materials, with a description of the evolution of several intermediate phases. Both Mo and Zn K-edge spectroscopic data were analyzed and comparison of the evolution of ternary oxides containing the two elements proved to be an effective way for validating the results. Abstract

* Oncocytosis is a term recently used to describe diffuse renal involvement by numerous oncocytic nodules. We report herein a case of a 53-year-old man with end-stage renal disease requiring hemodialysis. His kidneys were involved by numerous tumors. Histologic examination revealed more than 100 oncocytomas and an associated papillary renal cell carcinoma in the right kidney. Abstract

Prebiopsy multiparametric prostate MRI (mp-MRI), followed by transrectal ultrasound-guided (TRUS-G) target biopsies (TB) of the prostate is a key combination for the diagnosis of clinically significant prostate cancers (CSPCa), to avoid prostate cancer (PCa) overtreatment. Several techniques are available for guiding TB to the suspicious mp-MRI targets, but the simplest, cheapest, and easiest to learn is “cognitive,” with visual registration of MRI and TRUS data. This review details the successive steps of the method (target detection, mp-MRI reporting, intermodality fusion, TRUS guidance to target, sampling simulation, sampling, TRUS session reporting, and quality insurance), how to optimize each, and the global indications of mp-MRI-targeted biopsies. We discuss the diagnostic yield of visually-registered TB in comparison with conventional biopsy, and TB performed using other registration methods.
