Abstract

Background: At the first interim analysis of the phase 3 ENGOT-cx11/GOG-3047/KEYNOTE-A18 study, the addition of pembrolizumab to chemoradiotherapy provided a statistically significant and clinically meaningful improvement in progression-free survival in patients with locally advanced cervical cancer. We report the overall survival results from the second interim analysis of this study. Methods: Eligible patients with newly diagnosed, high-risk (FIGO 2014 stage IB2–IIB with node-positive disease or stage III–IVA regardless of nodal status), locally advanced, histologically confirmed, squamous cell carcinoma, adenocarcinoma, or adenosquamous cervical cancer were randomly assigned 1:1 to receive five cycles of pembrolizumab (200 mg) or placebo every 3 weeks with concurrent chemoradiotherapy, followed by 15 cycles of pembrolizumab (400 mg) or placebo every 6 weeks. Pembrolizumab or placebo and cisplatin were administered intravenously. Patients were stratified at randomisation by planned external beam radiotherapy type (intensity-modulated radiotherapy [IMRT] or volumetric-modulated arc therapy [VMAT] vs non-IMRT or non-VMAT), cervical cancer stage at screening (FIGO 2014 stage IB2–IIB node positive vs III–IVA), and planned total radiotherapy (external beam radiotherapy plus brachytherapy) dose (<70 Gy vs ≥70 Gy [equivalent dose of 2 Gy]). Primary endpoints were progression-free survival per RECIST 1.1 by investigator or by histopathological confirmation of suspected disease progression and overall survival defined as the time from randomisation to death due to any cause. Safety was a secondary endpoint. Abstract

En este artículo se presenta el desarrollo de un robot móvil controlado vía web, diseñado para ejecutar tareas de monitoreo. El sistema es controlado mediante la tarjeta Orange Pi+ 2E y una tarjeta Arduino. El Robot está construido en acrílico, posee Una pantalla LCD touch para la comunicación con el usuario, 4 motores (para mover el vehículo), un controlador de carga, Sensores ultrasónicos para medir la distancia entre otros dispositivos de control. Se programó el sistema para que efectuará las tareas de manera óptima transmitiendo las imágenes de la cámara, la respuesta de los sensores y las órdenes de control del usuario cada 100 ms, la herramienta empleada es el framework ROS (Robot Operating System). En este caso se empleó el sistema operativo Linux server en la Orange Pi; que permite usar múltiples lenguajes de programación como lo son Java, PHP, Xajax y SH, en conjunto se ejecutan en un solo programa o interfaz de usuario que, a través de HTML5 y un navegador WEB permiten: captar video de la cámara, leer los comandos del usuario y hacer la petición a la Arduino para que dé la orden a los motores de efectuar el movimiento, leer el sensor y publicar en la pantalla LCD.

Abstract

El propósito del presente artículo expone como el diagnóstico de VIH ha afectado a mujeres chilenas en su rol de trabajadoras, conociéndolo desde su propia perspectiva. Es una investigación cualitativa, donde se entrevistan a 3 mujeres seropositivo de edad entre los 20 y 65 años, que cuentan con experiencia laboral previo al diagnóstico de VIH. Los resultados identifican un antes y después del diagnóstico de VIH, donde para mantener su rol de trabajadoras ocultan su diagnóstico al empleador y a sus compañeros de trabajo por miedo a ser discriminada; dan a conocer abiertamente su diagnóstico al contexto social que se desenvuelven para evitar prejuicios; o cambian su rubro laboral. De esta manera, se reconoce que la ley del sida de Chile no logra cubrir una protección real que permita el respeto, fiscalización y responsabilidad de estas normas, siendo vulnerados los derechos de las mujeres que viven con VIH. Las mujeres diagnosticadas sufren apartheid ocupacional e injusticia ocupacional, ya que no logran realizar su rol como trabajadoras debido a la desinformación que existe en la sociedad frente al VIH y a los estigmas que se encuentran entorno a ella, desencadenando prejuicios sociales históricos que han favorecido que mujeres vivan ocultando su realidad.

35. Percepción del dolor según el modo de punción de la fístula arteriovenosa en pacientes sometidos a hemodiálisis. Revisión sistemática

Abstract

Objetivo: Valorar la posible asociación entre la forma de punción de la fístula arteriovenosa y el dolor que siente el paciente por la inserción de la aguja. Metodología: Se ha seguido la declaración PRISMA para revisiones sistemáticas. Se han consultado 6 bases de datos; Medline, Scopus, Cuiden, CINAHL, SciELO, y Cochrane PLUS. Además, se realizó una búsqueda secundaría manual para detectar literatura gris no en-contrada en las bases de datos o mediantes las estra-tegias definidas. Criterios de inclusión; estudios de ni-vel de evidencia 1 en la escala Scottish Intercollegiate Guidelines Network, publicados en los últimos 10 años, idiomas español/inglés, y temática relacionada con las técnicas utilizadas en hemodiálisis para el abordaje de la fistula. Los artículos se evaluaron de forma crítica para detectar cualquier riesgo de sesgo mediante el ins-trumento CASPe y el manual Cochrane para Revisiones Sistemáticas de Intervenciones.Resultados: Se identificaron un total de 150 resulta-dos, seleccionándose finalmente 9 estudios. Los resulta-dos han sido muy heterogéneas, encontrándose resulta-dos para el dolor, tasas de infección, número de intentos de canalización, flujo sanguíneo, presión venosa, tiempo de hemostasia, seguridad, dilataciones aneurismáticas, estética del brazo portador de la fístula y calidad de vida del paciente., Aims: To assess the possible association between the manner of arteriovenous fistula cannulation and the patient pain related to the insertion of the needle for the hemodialysis session.Methodology: The PRISMA statement for systematic reviews has been followed. Six databases have been consulted: Medline, Scopus, Cuiden, CINAHL, SciELO, and Cochrane PLUS. In addition, a secondary manual search was performed to detect grey literature not found in databases or through defined strategies. Inclusion criteria: studies of level of evidence 1 on the Scottish Intercollegiate Guidelines Network scale, published in the last 10 years, Spanish/English languages, and topics related to the techniques used to canalize the hemodialysis fistula. The articles were critically evaluated to detect any risk of bias using the CASPe instrument and the Cochrane Handbook for Systematic Reviews of Interventions.Results: A total of 150 results were identified, 9 of which were finally selected. The results have been very heterogeneous, finding results for pain, infection rates number of attempts of channalization, blood flow, venous pressure, time of hemostasis, safety, aneurysmal dilations, esthetics of the fistula-carrying arm and quality of life of the patient.Conclusions: The evidence does not support the preferential use of the buttonhole technique over traditional channeling. Therefore, it is necessary to improve the methodological quality and a larger sample size to elucidate which technique gives better benefits with respect to pain, thus allowing to improve the quality of life of hemodialysis patients.

Abstract

Immune protection relies on the capacity of neutrophils to infiltrate challenged tissues. Naive tissues, in contrast, are believed to remain free of these cells and protected from their toxic cargo. Here, we show that neutrophils are endowed with the capacity to infiltrate multiple tissues in the steady-state, a process that follows tissue-specific dynamics. By focusing in two particular tissues, the intestine and the lungs, we find that neutrophils infiltrating the intestine are engulfed by resident macrophages, resulting in repression of Il23 transcription, reduced G-CSF in plasma, and reinforced activity of distant bone marrow niches. In contrast, diurnal accumulation of neutrophils within the pulmonary vasculature influenced circadian transcription in the lungs. Neutrophil-influenced transcripts in this organ were associated with carcinogenesis and migration. Consistently, we found that neutrophils dictated the diurnal patterns of lung invasion by melanoma cells. Homeostatic infiltration of tissues unveils a facet of neutrophil biology that supports organ function, but can also instigate pathological states.

La diabetes mellitus (DM) va en aumento en nuestro país, con alta prevalencia de complicaciones especialmente por mal control metabólico. Objetivo: evaluar asociación de condición de salud, variables sociodemográficas y años de diagnóstico con satisfacción del paciente con características del tratamiento de diabetes. Material y método: estudio de prevalencia en 340 pacientes de 1.100 usuarios DM2 controlados en Programa Cardiovascular de un Centro de Salud Familiar (CESFAM) de Macul, Región Metropolitana, Chile, previo consentimiento informado. Muestreo aleatorio simple (confianza 95%, merma 10%). Recolección de información en CESFAM y/o visita domiciliaria por investigadoras e internas de enfermería entrenadas. Satisfacción del paciente, medida con subescala satisfacción del cuestionario Diabetes Quality of Life (EsDQOL). Análisis con estadística descriptiva, Chi2, Odd Ratios, IC (95%). Resultados: Población mayoritariamente adulta mayor, femenina, promedio años estudio 8,7. El 42,6% tiene diagnóstico de DM hace más de 10 años. Refieren mayor satisfacción con: tratamiento medicamentoso, controles recibidos, vida en general. Existe mayor insatisfacción con: tiempo ocupado en ejercicios, cómo duerme, vida sexual. Satisfacción con tratamiento medicamentoso se asocia a compensación de diabetes (p= 0,026). Pacientes insatisfechos con tratamiento tienen 2 veces más riesgo de descompensación que los satisfechos, IC [1,08-3,7]. Insatisfacción con tiempo dedicado a enfermedad, controles, tiempo ocupado en controles y ejercicio indicado se asocian a complicaciones (p < 0,05). Conclusión: Insatisfacción con características del tratamiento de DM2 se asocia mayormente a complicaciones que a compensación de DM, pacientes insatisfechos tienen más riesgo de complicaciones que los satisfechos.

41. Purification and Some Properties of Hemagglutinating Protein Mutina from Bushmaster Lachesis muta Snake Venom

Abstract

Lachesis muta snake venom induced aggregation of bromelain sensitized human erythrocytes al a concentration of 1 mg/ml. The hemagglutinating protein was purified by DEAE-Sephadex A-50 column chromatography. Polyacrylamide gel electrophoresis revealed at least three bands, whereas SDS electrophoresis in the presence of 2-mercaptoethanol showed a single one. Isoelectric focusing revealed hemagglutinating activity in the range of pH 3-8. The maximun peak (mutina) al pH 5.5. This fraction was active in agglutinating human RBC of types A, B. O Rh (+) and B. O Rh (-).One mM EDTA and 1 mM Ca++ did not alter the agglutinating time significantly. Lactase and inositol inhibited the agglutination of A, S, O Rh (+) and B, O Rh (-) human RBC. The present study showed the non specificity of the hemagglutinating activity of mutina. It was also shown that mutina is a non-mitogenic protein.

Informed consent was requested. Simple randomized sample (CI 95%, loss 10%). Data collected in the outpatient clinic or through home visits done by researchers or trained senior nursing students. Patient satisfaction was measured by the satisfaction subscale of the Diabetes Quality of Life Questionnaire (EsDQOL). Analysis: descriptive statistics, Chi2, Odds Ratios, CI (95%). Results: Population of mainly older adults, female, average years of school attendance 8.7. 42.6% has been diagnosed since more than 10 years. They refer greater satisfaction with: pharmacologic treatment, care received in outpatient visits, life in general. There exists more dissatisfaction with: time spent in exercise, sleep, sexual life. Satisfaction with pharmacologic treatment has association with diabetes compensation (p= 0.026). Patients dissatisfed with treatment have 2 times more risk of poor metabolic control than satisfed patients, IC[1.08-3.7]. Dissatisfaction with time spent in the disease, outpatient visits, and time spent in exercises, are associated with complications (p < 0.05). Conclusion: Dissatisfaction is more associated to complications than to poor metabolic control. Patient dissatisfed have more risk of complications than those satisfed.

La condición física, adiposidad y distribución grasa observadas en la infancia, han mostrado tener relación con la salud cardiovascular en la edad adulta. Objetivo: evaluar el nivel de condición física en escolares de 7 a 12 años y su relación con niveles saludables de condición aeróbica y distribución grasa. Se valoraron 1068 niños y niñas aragoneses de 7-12 años de edad. Se evaluó la condición física con la batería Eurofit y el grado de adiposidad y distribución grasa mediante antropometría. Se obtienen valores normativos de condición física. Un 9,1% de los chicos y 4,8% de las chicas presenta riesgo fututo de salud sobre la base de su condición aeróbica. Mejor condición aeróbica se asocia con cantidades significativamente menores de grasa subcutánea total y en el tronco. Es importante incorporar la evaluación del nivel de condición física y distribución grasa en la valoración del riesgo de salud desde edades tempranas., The association between physical fitness, adiposity and trunk fat mass during childhood and cardiovascular health later in adult life has been well established. Aim: to determine the physical fitness levels of children (7-12 y) and their relationship with a healthy aerobic fitness level and fat distribution. A cross sectional study including 1068 boys and girls aged 7-12 y was performed. Anthropometric measurements and Eurofit battery test were used. A full set of physical fitness reference values for school age children (7-12 y) is presented. A percentage of 9.1 of boys and 4.8% of girls, do not accomplish the minimum levels recommended for a healthy cardiovascular fitness. A relationship between high physical fitness levels and low subcutaneous fat mass (whole body and the trunk area) was observed. Objetivo: evaluar el nivel de condición física en escolares de 7 a 12 años y su relación con niveles saludables de condición aeróbica y distribución grasa.
