Background & Objective: Therapeutic communication with patients is one of the most critical competencies for nursing students in clinical settings. In this study, we investigated the impact of an online education program based on the COMSKIL model on the communication skills of undergraduate nursing students. Material & Methods: This quasi-experimental study involved presenting a specific education program based on the COMSKIL model to a group of undergraduate nursing students in the 4th to 8th academic semesters. The program was delivered through the online platform of the university during the COVID-19 pandemic. The data were collected at three time points: baseline (T1), immediately after education (T2), and two months after education (T3). Two instruments were used to collect data: a sociodemographic information questionnaire and a Persian version of the Quindam Communication Skills Questionnaire. The data were analyzed using independent t tests, one-way Analysis of Variance (ANOVA), and repeated measures analysis of variance. Results: The study's hypothesis was accepted, showing that the education program had a positive impact on the communication skills of the students over time. The total mean score of students' communication skills significantly improved (p < 0.0001) from T1 (107.91 ± 7.53) to T3 (121.76 ± 4.91) two months after the education program. The effect size indicated that this increase was moderate. Conclusion: The findings of this study have practical implications for nursing students in real-world patient care scenarios in clinical settings. The innovative and evidence-based educational model used in this study can replace outdated methods of instruction. Given that effective communication between nurses and patients is vital for providing high-quality patient care in a supportive environment, we recommend integrating this person-centered program into the curriculum planning of undergraduate nursing students.

Introduction: Obesity and overweight are major global health challenges. One of the bad effects of noise that has been recently expressed is the effect of noise on obesity. This study aimed to investigate the effect of high-frequency noise exposure on obesity, food intake, and abdominal visceral fat in adult male guinea pigs. Material and Methods: The animals in this study were 24 adult male guinea pigs randomly divided into 3 groups (control and two case groups). Each case group was separately exposed to high- frequency white noise with sound pressure levels in 65 dB and 85 dB for 5 days per week in 30 days. The food intake was measured daily. The weight of animals was measured at the start and on days 6, 12, 18, 24, and at the end of exposure period. The abdominal visceral fat was extracted and weighted at the end of the study period. The data were assessed using SPSS V.22 software. Results: ANOVA analysis showed that exposure to high-frequency noise at 65dB and 85dB had a significant effect on weight gain, food intake, and abdominal visceral fat weight (P-value< 0.05) which in the group exposed to the noise with 65 dB was more than other groups. Conclusion: Based on this study, exposure to high-frequency noise may be an effective factor in obesity and increasing abdominal visceral fat. Further studies are needed to investigate the mechanism affecting weight status following noise exposure.

Background: In pursuing improving healthcare quality and enhancing efficiency, public hospitals in Iran have undergone numerous reforms over the past two decades. This study aimed to assess the efficiency of all public hospitals in Iran from 2012 to 2016.Methods: This study was conducted as a quantitative and descriptive-analytical research project. The authors employed an innovative approach called Extended Data Envelopment Analysis (Extended-DEA), a modification of conventional DEA, to assess the technical efficiency and productivity of 568 public hospitals. They obtained nationally representative data from official annual health reports. The data were analyzed using GAMS software version 24.3.Results: The study found that the average efficiency score for all hospitals was 0.733. Among all the hospitals, 10.1% were deemed efficient, while 2.68% had low-efficiency scores below 0.2. The Malmquist Productivity Index (MPI) showed improvement in 49.3% of hospitals and remained unchanged at 2.3%. In comparison, 48.2% of hospitals experienced a regression in productivity from 2015 to 2016. On average, the MPI was 1.07 throughout the analysis.Conclusion: The findings of this study suggest that there is a need for increased efforts to improve the efficient utilization of resources in public hospitals. It highlights the importance of developing appropriate policy solutions and tools to address these efficiency challenges. In particular, one proposed strategy is the merger of small-sized district hospitals to establish larger and more efficient hospitals in different geographical regions across the country.

This study investigates the influence of the morphology of concentrate on the oxidation and sintering rates of magnetite pellets during induration and its impact on the indurated pellets' quality. Various morphologies of concentrates were obtained by ball milling (BM) and high-pressure grinding rolls (HPGR). Pelletizing and induration processes were carried out at different temperatures and times. After a comparative study of the physical characteristics of concentrate powders, the effect of morphology was evaluated through thermal behavior during induration. The physical, microstructural, and strength properties of pellets were also investigated. The HPGR concentrate had angular and elongated-shaped particles with a broad particle size distribution, while the BM particles had a more rounded morphology, and their size distribution was narrower. Results showed that BM and HPGR magnetite pellets are oxidized between 270 and 950 °C. HPGR pellets oxidize slower than BM pellets. Cold compressive strength (CCS) was increased by induration time, although porosity was reduced. HPGR pellets had less porosity than BM pellets; however, their CCS was significantly higher. For pellets indurated at 1200 °C, the difference in CCS between HPGR and BM pellets increased with an increment in the induration time and reached 80 kg/pellet in 20 min. The microstructural analysis took place on the concentrate powder and pellets fracture surface. It was found that HPGR powder sinters faster than BM powder due to its angular shape and broad particle size distribution.

Background: Transthoracic echocardiography (TTE) is the recommended imaging technique for the evaluation of patients with aortic stenosis (AS). However, in cases with inconclusive findings, cardiac magnetic resonance (CMR) planimetry is used to grade AS severity. This study aimed to compare the results derived from TTE and CMR in patients with severe AS with normal left ventricular (LV) function.Methods: In a prospective study, 20 patients with severe AS were recruited and data derived from TTE and CMR modalities were compared with the archived records of 28 age- and sex-matched healthy controls. The data included aortic valve area (AVA), MRI-derived biventricular global strains, and TTE-derived global longitudinal strain (GLS). We aimed to explore the cardiovascular magnetic resonance (CMR) of Takayasu arteritis (TA) and its cardiovascular complications. CMR was conducted on 37 TA patients and 28 healthy individuals. We evaluated the CMR findings and adverse cardiovascular complications at the time of the CMR (ACCCMR). After 8 to 26 months, the major adverse cardiac and cerebrovascular events (MACCEs) were evaluated. The TA included 25 women (67.6%), aged 36 ± 16 years old, and 28 age- and sex-matched healthy controls. Left ventricular (LV) ejection fraction was significantly lower in the TA group than in the control group (51 ± 9% vs. 58 ± 1.7%; p < 0.001). Aortic mural edema was present in 34 patients (92%) and aortic mural hyperenhancement in 36 (97%). Left ventricular global longitudinal strain (LVGLS) was significantly lower in the TA group (median [interquartile range] = 13.70 [3.27] vs. 18.08 [1.35]; p < 0.001). ACCCMR was seen in 13 TA patients (35.1%), with the most common cardiac complication being myocarditis (16.2%). During a median follow-up of 18 months (8–26 months), nine patients developed MACCEs, of which the most common was cerebrovascular accident in five (13.5%). The LVGLS of the CMR had the strongest association with complications. Myocardial strain values, especially LVGLS, can reveal concurrent and future cardiovascular complications in TA patients.

Ischemia in the medial prefrontal cortex (mPFC) causes cognitive impairment in stroke cases. This study aimed to examine the effects of varenicline as α7 and α4β2 nicotine acetylcholine receptors (nAChRs) agonist, on cognitive impairment, inflammation, apoptosis, and synaptic dysfunction in mPFC ischemia. Mice were divided to three groups of control, sham, or photothrombotic mPFC ischemia model. The control and sham groups received 2 ml/kg of normal saline for a 14-day period. As well, the animals in the ischemia groups received normal saline (2 ml/kg) or varenicline at 0.1, 1, and 3 mg/kg doses for a 14-day period. Anxiety-like behaviors were then assessed by open field (OFT) and elevated plus-maze (EPM) tests. Memory was also evaluated using Morris water maze (MWM) and novel object recognition (NOR) tests. The levels of inflammatory (IL-1β, TNF-α), apoptotic (Bax, caspase3, BCL-2), and synaptic (SYP, PSD-95, and GAP-43) proteins were examined using the western blot method. In addition, the histological evaluation was performed to assess tissue damage. The administration of Varenicline at the dose of 3 mg/kg reduced the IL-1β, TNF-α, Bax, and caspase3 levels. Moreover, it increased BCL-2, SYP, PSD-95, and GAP-43 levels at the same dose and ameliorated memory impairment and anxiety-like behaviors in mPFC ischemic mice. Varenicline improved cognitive impairment by blocking inflammation and apoptosis, improving synaptic factors, and diminishing tissue damage in the mPFC ischemic mice.

Objective:Misconduct is one of the important issues in research integrity. Cochrane systematic reviews are known for their best level of evidence. Since kidney failure is a major public health problem worldwide, the Cochrane Library provides a robust and reliable database to upgrade medical knowledge and make the best medical decisions. Therefore, this study aimed to assess the quality of randomized controlled trials (RCTs) that are included in the Cochrane systematic reviews of kidney and transplant groups.Materials and Methods:This analytic cross-sectional study was conducted on systematic reviews of kidney and transplant group of Cochrane reviews. All types of biases in the understudied RCTs or quasi-RCTs of these systematic reviews were evaluated using the Cochrane appraisal checklist. The types of biases in included studies were also separated and stratified. Descriptive statistics were used for data analysis using the Statistical Package for the Social Sciences 16.Results:A total of 267 systematic reviews and their understudied 3213 RCTs were evaluated. In the kidney and transplant group, the highest risk of bias was seen in allocation concealment bias, whereas the most common bias was unclear allocation concealment (selection bias). From 2008 to 2009, high random sequence generation bias has dramatically increased, and after decreasing, the gradual growth has been continuing over time. Furthermore, the low detection bias has reduced surprisingly in 2011 then decreased in 2012-2013.Conclusion:Regarding high risks of performance and random sequence generation biases in understudied RCTs, critical structure deficiencies were obvious. Therefore, observing integrity principles to prevent research misconduct is recommended.

Abstract A subgroup of COVID‐19 patients with cardiac magnetic resonance imaging evidence of myocardial inflammation may exhibit subendocardial fibrosis, compatible with myocardial infarction, while epicardial coronary arteries are normal.

Abstract Unilateral absence of pulmonary artery is a rare congenital disorder that can remain asymptomatic until adulthood. Absence of left pulmonary artery (ALPA) has been reported in one‐third of these patients. We are the first to introduce an adult case of ALPA associated with partial anomalous pulmonary venous connection.

Abstract COVID‐19 affects the heart through various pathways. It can increase thrombotic complications that ultimately lead to myocardial infarction even in patients without a history of underlying heart disease.

Introduction: Overexposure to heat conditions can affect the functioning of the cardiovascular system and may promote cardiovascular disorders. Heat shock induced myocardial injury via increasing endoplasmic reticulum response-mediated apoptosis. This study investigated the impact of pretreatment with Rosa canina (RC), a natural antioxidant, on myocardial damage induced by heat stress exposure and underlying mechanisms in cardiomyocytes in rats. Methods: Sixty adult male Wistar rats were allocated into five groups, including Control: received normal saline (NS), Heat Stress (HS), and HS+RC groups. Animals in the HS groups were subjected to heat stress (43 ºC) for 15 minutes once a day for two weeks. Animals in the HS+RC groups received three doses of RC (250, 500, and 1000 mg/mL) one hour before being subjected to heat shock. The endoplasmic reticulum (ER) transmembrane kinases, including PKR-like endoplasmic reticulum kinase (PERK), immunoreactivity of CCAAT/enhancer-binding protein homologous protein (CHOP), and eukaryotic translation initiation factor 2-alpha (eIF2α) as well as caspase 8 were detected by Western blot. The levels of reactive oxygen species (ROS) were assessed. Moreover, histopathological changes and apoptosis were also assayed in the heart tissue by using histopathological and TUNEL assays. Results: Heat exposure increased the level of ROS and induced oxidative damage in the heart tissue. The results demonstrated that RC administration decreased the overproduction of ROS induced by heat stress in cardiomyocytes. Moreover, heat stress up regulated the expression of p-PERK, p-eIF2α,and CHOP protein while pretreatment with RC decreased expression of ER stress-related markers in cardiomyocytes. Besides, RC diminished heat stress-induced cellular damage and apoptosis associated with inhibition of caspase 8 activation, a pro-apoptotic protein in cardiomyocytes. Conclusion: These findings indicate that RC exerts a protective effect on heart tissue, at least in part,through inactivation of PERK/eIF2α/CHOP pathway or inhibition of ER stress and oxidative stress triggeredapoptosis in cardiomyocytes induced by heat stress.

Background and Aim: Clinical governance is one of the important frameworks for continuous quality improvement and safety in health care systems. Identifying the axes of this approach according to local conditions is one of the important priorities of the health system. The aim of this study was to identify the views of stakeholders on the axes of clinical governance in primary health care based on family physicians in Mazandaran province. Methods: The present qualitative study was conducted using the conceptual framework analysis method in 2018-2019. The study population were key policy makers of Ministry of Health, Health deputy of the University, the county health network, family physicians association, family physicians, and the parliament research center. Participants were selected using purposeful and snowball sampling methods. Data were collected through semi-structured interviews and were analyzed and coded using MAXQDA 11 software. Findings: According to the interviewees' views, the research findings were classified into 4 main dimensions: dimensions of clinical governance, requirements and structures, decision-makers and dimensions of quality and safety assessment. 17 sub-themes including community participation, clinical audit, clinical effectiveness, personnel management, training, information use, risk management, guidelines and procedures, promotion of health indicators, equipment and facilities, referral system, financing, policy makers, effectiveness, efficiency, human aspects of services and justice were identified and extracted. Conclusion: According to the research findings, to facilitate the implementation of clinical governance, solutions such as the existence of appropriate infrastructure, commitment of managers, supportive culture, sufficient knowledge, monitoring and evaluation, appropriate culture building, facilities and equipment and sufficient financial resources are suggested.

Objective. In hypertrophic cardiomyopathy (HCM), myocardial fibrosis is routinely shown by late gadolinium enhancement (LGE) in cardiac magnetic resonance (CMR) imaging. We evaluated the efficacy of 2 novel contrast-free CMR methods, namely, diffusion-weighted imaging (DWI) and feature-tracking (FT) method, in detecting myocardial fibrosis. Methods. This cross-sectional study was conducted on 26 patients with HCM. Visual and quantitative comparisons were made between DWI and LGE images. Regional longitudinal, circumferential, and radial strains were compared between LGE-positive and LGE-negative segments. Moreover, global strains were compared between LGE-positive and LGE-negative patients as well as between patients with mild and marked LGE. Results. Objectives. Cardiac amyloidosis (CA) and constrictive pericarditis (CP) are described as the differential diagnoses of restrictive hemodynamic alterations of the heart. We aimed to explain cardiac magnetic resonance (CMR) imaging findings (especially feature tracking (FT)) of CA and CP cases and compare them with healthy controls. Moreover, we evaluated the role of biventricular FT parameters in differentiating CA from CP. Methods. Thirty-eight patients who underwent CMR between February 2016 and January 2018 with the ultimate diagnosis of CA (19 patients) or CP (19 patients) were enrolled. We included biopsy-proven light-chain amyloidosis patients. The data of 28 healthy controls were utilized for comparison. The patients were followed up for 8–23 months to register mortality and their surveillance. All CMR morphological and functional data, including FT parameters, were recorded and analyzed. Results. Of only 13/19 (68.4%) CA patients who had the follow-up data, 11/13 (84.6%) died. One of The CP patients (5.3%) expired during the follow-up. Purpose: The best anthropometric index has not been established to investigate the relationship between hypertension and obesity. Since a similar study was not conducted in Bojnurd, we began to investigate the comparison of anthropometric indices (body mass index, waist circumference, and waist-to-hip and waist-to-height ratios) in predicting the risk of hypertension. Materials and Methods: The present cross-sectional study was conducted on women referring to Bojnurd health centers. For analyzing the data, we used t-test, Chi-square, logistic regression, and the receiver operating characteristic curve in SPSS 19 software. Results: The prevalence of blood pressure was 53.4% (confidence interval = 46.8–60). All four anthropometric indicators were used as a screening tool for hypertension diagnosis; however, body mass index (BMI) had a higher sub-curved surface than other anthropometric indices (area under curve = 0.717). The cutoff point of BMI for predicting the risk of hypertension was 25.6. Conclusions: This study suggested that BMI as an anthropometric indicator to evaluate the risk of hypertension. Since a significant percentage of people are unaware of the existence of hypertension, continuing education is needed to encourage people to pay more attention to this problem.

Clinical governance is a systematic approach to enhancing the quality of primary health care and ensuring high clinical standards, responsiveness to performance, and continuous improvement in service quality. The objective of the current study was to investigate the global experiences of clinical governance in primary health care. In the present systematic review, relevant articles from different countries were searched in various databases such as MD PubMed from Medline portal, Emerald Springer link, ProQuest, Cochrane, Scopus, Web of Science, and Consult until April 2019. The searched articles were checked through CASP and PRISMA checklists, and their results were extracted. Of the 17 selected studies, 16 belonged to developed countries, including England (13), Australia, Italy, and New Zealand, and one was from Turkey. The findings were divided into three general categories: (1) principles of effectiveness and risk management, (2) deployment requirements such as structural and organizational needs, resource and communication, and information management, and (3) barriers of clinical governance toward providing primary health care. it is recommended that a suitable framework or model be developed and designed adapted to the local culture and taking into account all effective dimensions for a proper establishment and implementation of clinical governance in primary health care.

Background: Population growth and use of the car in daily life entails new incidents and accidents everyday. Adolescents′ entering the new world of adults, their insufficient knowledge of rules, and high-risk behaviors expose them to more risks. Accordingly, a study was conducted with the aim to evaluate the status of preventive behaviors in traffic accidents in boy and girl junior high school students in Isfahan regarding vehicle use. Methods: A descriptive-analytical cross-sectional study was conducted on 7000 junior high school boy and girl students from 20 towns in Isfahan Province using multi-stage cluster sampling method in 2009-2010. A researcher-made questionnaire was used as data collection tool, which evaluated students′ practice and preventive behaviors with 21 questions, each examining students′ practice in accidents and incidents that may occur in school and on the way to school. Data were analyzed with Epi 6 and SPSS software using t-test and Chi-square test. Results: Girls comprised 49.9% of students and 50.1% were boys, 84% lived in urban areas and 15.5% in rural areas. The frequency of an accident location was school in 53.9% with 3739 cases and on the way to school in 10.6% with 732 cases. Mean practice score of preventive behaviors in traffic accidents involving cars, taxi, and school bus (72.6 ± 17.52 girls, 72.7 ± 18.31 boys, P = 0.88), motorbike (79.1 ± 14.048 girls, 74.1 ± 19.73 boys, P < 0.001), bicycle (71.4 ± 16.56 girls, 68.5 ± 14.69 boys, P = 0.152), bus and minibus (91.8 ± 13.16 girls, 87 ± 18.65 boys, P < 0.001), crossing the street (30.5 ± 26.67 girls, 32.7 ± 28.03 boys, P = 0.003), and skating (60.6 ± 29.103 girls, 61.2 ± 26.84 boys, P = 0.927). Results indicate that girls have better preventive practices than boys in use of motorbikes, buses, and minibuses. Conclusions: According to the results obtained, the majority of students walk to school and have the lowest practice score in this respect. It is recommended that as the first step, students be given necessary road traffic rules training, particularly how to cross the street.

Background: Adolescents are being threatened by several accidents because of entering the new world of adulthood, separation from families, a greater presence in community individually, the lack of knowledge of rules and risk-taking spirit of adolescents, and events. The contribution of these damages and their related costs are higher in developing and low-income countries. The current study was conducted for proper planning focused on reducing accidents that have the highest incidence, evaluating school events, determining the place and time that most accidents happened to students and determining the body part injured. Methods: A cross-sectional study using a multistage cluster sampling was conducted on 6933 students 11-14 years old (middle school) in 2009-2010 in Isfahan Province. The data were collected through observation and a researcher-made questionnaire and then entered into the EPi6 software, and analyzed by the SPSS 11.5 software and the statistical test of Chi-square. Results: The findings showed that 49.9% of students were girls (n = 3462) and 50.1% (n = 3471) were boys, 84.5% of students (n = 5860) were living in urban areas and 15.5% of students (n = 1073) were living in rural areas. Frequency distribution of incidents was 53.9% (3739 cases) at school and 10.6% (732 cases) on the commuting route. The most injured body part was hand with 1018 cases (18.5%) and foot with 1267 cases (23.1%), and mostly they were injured in boys. Conclusions: The findings showed that a high percentage of students were injured at school, where the first step to prevent the incidence of such preventable incidents at school is to secure workshop environments and school yards.

Introduction: Aortic valve sclerosisis considered as a manifestation of coronary atherosclerosis. Recent studies demonstrated an association between aortic valve sclerosis and obstructive coronary artery disease. The purpose of this study was to evaluatethe correlation betweenaortic valve sclerosis andobstructive coronary artery disease and the extent of coronary artery disease in patients hospitalized for chest pain. Materials and Methods: A total of 230 consecutive patients were referred to the coronary angiography of GhaemMedical Center and were subjected to transthorasic echocardiography for screening of aortic valve sclerosis and coronary risk assessment. The diagnostic value of obstructive coronary artery disease for aortic valve sclerosis was calculated. Results: The patients with obstructive coronary artery disease had a higher prevalence of aortic valve sclerosis compared to those with no coronary artery disease (P< 0.05). Aortic valve sclerosis was an independent predictor for obstructive coronary artery disease by multivariate analysis (P< 0.05). Aortic valve sclerosis had sensitivity of 47% and specificity of 79% and positive predictor value of 92%. Conclusions: Aortic valve sclerosis was an independent predictor for obstructive coronary artery disease in patients with chest pain and was strongly interrelated with the extent of coronary artery disease in these patients.

The present study was aimed to investigate the effects of crocin and chlorpheniramine on local edema induced by histamine. Materials and Methods: Local edema was induced by subcutaneous injection of histamine (100 μl, 0.1%) in ventral surface of right hind paw. The thickness of paw was measured at 1 h before and 1, 2, 3 h after injection of histamine, using a fine caliper. In this paper, the problem of grouping machines and parts into cells (machine-part cell formation problem) is considered with the objective of minimizing grouping efficacy. An ant colony optimization algorithm is developed to solve such problem. In the proposed algorithm, solutions are constructed in a new manner, two heuristic algorithms are used to determine part families and machine cells and the similarity between parts is used as heuristic information. The proposed algorithm is experimented on 35 test problems from the literature which shows its advantage over existing algorithms. The algorithm improves the best known values of the grouping efficacy for 5 problems.

This paper deals with the NP-hard single-machine total weighted tardiness problem with sequence dependent setup times. Incorporating fuzzy sets and genetic operators, a novel ant colony optimization algorithm is developed for the problem. In the proposed algorithm, artificial ants construct solutions as orders of jobs based on the heuristic information as well as pheromone trails. To calculate the heuristic information, three well-known priority rules are adopted as fuzzy sets and then aggregated. When all artificial ants have terminated their constructions, genetic operators such as crossover and mutation are applied to generate new regions of the solution space. A local search is then performed to improve the performance quality of some of the solutions found. Moreover, at run-time the pheromone trails are locally as well as globally updated, and limited between lower and upper bounds. The proposed algorithm is experimented on a set of benchmark problems from the literature and compared with other metaheuristics.

The total mean score of students' communication skills significantly improved (p < 0.0001) from T1 (107.91 ± 7.53) to T3 (121.76 ± 4.91) two months after the education program. The effect size indicated that this increase was moderate. Conclusion: The findings of this study have practical implications for nursing students in realworld patient care scenarios in clinical settings. The innovative and evidence-based educational model used in this study can replace outdated methods of instruction. Given that effective communication between nurses and patients is vital for providing high-quality patient care in a supportive environment, we recommend integrating this person-centered program into the curriculum planning of undergraduate nursing students.
